RESUMO

Objective:To compare the application effect of two different kinds of joint mobilization combined with muscle strength training in patients with shoulder impingement syndrome.Methods:From January 2017 to March 2019, 69 patients with shoulder impingement syndrome were divided into control group(34 cases) and observation group(35 cases) by random number table method.The control group received Maitland joint loosening and muscle strength training, and the observation group received Mulligan dynamic joint loosening and muscle strength training.The differences of visual analogue scale(VAS), shoulder pain and disability index(SPADI) and active shoulder joint activity were compared between the two groups before and after treatment.Results:Before treatment, there were no statistically significant differences in VAS score, SPADI and shoulder active joint mobility between the two groups(all P>0.05). After treatment, the VAS score and SPADI score of the control group were (2.75±0.29)points, (29.88±4.85)points, respectively, which of the observation group were (1.20±0.15)points, (12.04±2.29)points, respectively, which of the two groups were significantly improved compared with before treatment(control group: t=9.60, 9.15; observation group: t=17.47, 19.51, all P<0.01). The VAS score and SPADI score of the observation group were better than those of the control group( t=28.00, 19.63, all P<0.01). After treatment, the ranges of motion of the active joint in the control group were (69.06±8.77)°, (67.83±13.25)°, (164.06±17.94)°, (160.15±20.24)°, respectively, which in the observation group were (84.74±10.13)°, (82.24± 15.06)°, (179.84±19.16)°, (176.45±23.26)°, respectively, the differences were statistically significant between the two groups( t=6.87, 4.22, 3.53, 3.10, all P<0.01). Conclusion:Compared with Maitland joint loosening and muscle strength training, Mulligan dynamic joint loosening and muscle strength training can effectively relieve shoulder pain in patients with shoulder impingement syndrome, increase shoulder mobility, improve shoulder mobility limitation and promote shoulder function recovery.

Objective:To compare the effect of double plate internal fixation with medial and lateral knee incision and anterior straight knee incision on the knee function of patients with complex tibial plateau fracture.Methods:From January 2016 to March 2018, 94 patients with complex tibial plateau fractures treated with double plate internal fixation were divided into the control group(straight incision in the middle of the knee) and the observation group(double incisions on the inside and outside of the knee) according to the random digital table method, with 47 cases in each group.The differences of perioperative indicators, posterior tibial plateau inclination angle, varus angle and knee function scores between the two groups were compared.Results:There was no statistically significant difference in intraoperative blood loss between the two groups( P>0.05). The operation time of the observation group[(84.35±11.85)min]was longer than that of the control group[(62.14±10.05)min], the treatment cost of the observation group[(3.28±0.61)ten thousand CNY]was more than that of the control group[(2.08±0.58)ten thousand CNY]( t=9.80, 9.12, all P<0.01). The days of hospitalization, the time of loading and the time of fracture healing in the observation group were (6.78±2.17)d, (44.76±9.33)d, (3.57±1.04)months, which were shorter than those in the control group[(10.24±2.25)d, (55.09±10.25)d, (5.57±1.37)months]( t=7.59, 5.11, 7.97, all P<0.01). There were no statistically significant differences in the posterior inclination angle and varus angle of the tibial plateau between the two groups at 6 months after surgery(all P>0.05). In the control group, the posterior inclination angle and varus angle of the tibial plateau were significantly increased at 12 months after operation compared with 6 months after operation(all P<0.01). There were no statistically significant differences in the posterior inclination angle and varus angle of the tibial plateau in the study group between 12 months after surgery and 6 months after surgery(all P>0.05). At 12 months after operation, the tibial plateau caster angle[(6.49±1.14)°]and the varus angle[(85.17±2.70)°] in the observation group were significantly lower than those in the control group[(8.05±1.55)° and (91.35±2.88)°]( t=5.56, 10.73, all P<0.01). There were no statistically significant differences in the Rasmussen and Harris scales scores of knee joints between 12 months after surgery and 6 months after surgery in the two groups(all P>0.05). The results of Rasmussen scale and Harris scale in the study group were significantly higher than those in the control group(all P<0.01). Conclusion:Compared with the knees straight incision, although knee inside and outside double incision double plate fixation treatment for patients with complex tibial plateau fractures can lead to increased operation time, high cost of treatment, but can shorten in-hospital time, promote the fracture healing after surgery, promote the recovery of the original anatomical structures, which can effectively improve the function of knee joint, thus is worthy of clinical application.

Objective To study the biomechanical properties of porous titanium cages used for different lumbar interbody fusion surgeries. Methods The three-dimensional (3D) finite element model of the lumbar spine was constructed, and mechanical parameters of porous materials were obtained by mechanical test. The biomechanical properties of porous titanium cages in anterior lumbar interbody fusion (ALIF), posterior lumbar interbody fusion (PLIF), transforaminal lumbar interbody fusion (TLIF), direct lateral interbody fusion (DLIF) were compared. Results After lumbar interbody surgery, the predicted range of motion (ROM) and the maximum stress in cage of DLIF model and ALIF model were substantially lower than those of PLIF model and TLIF model. The maximum stress in endplate of DLIF model, ALIF model and TLIF model were obviously lower than that of PLIF model. Conclusions DLIF with the porous cage showed advantages in biomechanical properties, which was simple to operate and suitable for minimally invasive surgery in clinical practice. DLIF performed the superior comprehensive properties.

OBJECTIVE@#To investigate the effect of bilateral superior cervical sympathetic ganglion occlusion (SCG) on aortic dissection and its possible mechanism.@*METHODS@#Forty-five SD rats were randomly divided into three groups with 15 in each:blank control group, sham operation group and SCG group. β-aminopropione (666 mg·kg·d) was given by subcutaneous injection for 4 weeks to establish the aortic dissection model. Rats in SCG group were given SCG before the injection of β-aminopropione. Blood pressure and heart rate of the rats were monitored using noninvasive tail artery blood pressure measuring instrument; sympathetic activity was monitored using drug block method; the structure of aortic wall was observed using HE staining; collagen fibers in aortic wall was observed using Sirius red staining; protein expression of Apelin was detected by immunohistochemistry; and the protein expression of matrix metalloproteinase (MMP)-2, 9 was detected by Western blotting.@*RESULTS@#During the experiment, the body mass of the sham operation group and SCG group was smaller than that of the blank control group (all <0.05), and the body mass of the SCG group was larger than that of the sham operation group (all <0.05). The heart rate and sympathetic activity of the sham operation group were higher than those of the blank control group (all <0.05), while the SCG group were lower (all <0.05). Compared with the blank control group, the aortic wall in the sham operation group was thickening, while that in the SCG group was improved. A large number of collagen-1 in the aortic wall of the blank control group was stained brown by Sirius red, which was lighter in SCG group, and the staining in the sham operation group was the lightest. Compared with the blank control group, the expression of Apelin, MMP-2 and MMP-9 protein in the sham operation group increased (all <0.05), while those in the SCG group decreased (all <0.05).@*CONCLUSIONS@#SCG can effectively reduce the incidence and mortality of aortic dissection in rats, which may be related to the inhibition of sympathetic activity and the decrease of collagen-1, Apelin, MMP-2 and MMP-9 expression.

RESUMO

The research progress of finite element method (FEM) applied in biomechanics of lumbar fusion and artificial lumbar disc replacement was reviewed and its prospect was forecasted. The main research directions of FEM are optimal selection of operation plans before the surgery, performance evaluation of implanted devices and prediction of postoperative outcomes. Based on the recent research progress, the application prospects of FEM in simulation of personalized surgery, evaluation of elastic implants and postoperative prediction of novel operation method were discussed. By reviewing and prospecting the application of FEM in biomechanical research of lumbar fusion and artificial lumbar disc replacement, the purpose of this paper is to provide theoretical references and practical guidance for the treatment of lumbar diseases in clinic.

RESUMO

Aiming at the background and significance of biomechanical researches on lumbar interbody fusion, the research progress of interbody cage and interbody fusion was reviewed and its prospect was forecasted. The related work was summarized, including research method of lumbar biomechanics, biomechanics of interbody cage, and biomechanics of lumbar interbody fusion. The main research directions on biomechanical study of lumbar interbody fusion were: modeling refinement of finite element method, geometrical optimization of traditional fusion device, clinical application of new porous fusion device, and diversification of the supplemented fixation method. Finally, the prospect of biomechanics of lumbar interbody fusion was discussed. The review and prospect on biomechanics of lumbar interbody fusion will provide references for clinical treatment of lumbar spine diseases.

RESUMO

Objective To assess the effects of thalidomide on VEGF mRNA level in human breast cancer cell lines. Methods Breast cancer cells were treated with thalidomide for 24~72h, RT-PCR was uced to detect the level of VEGF mRNA expression. Results When the concentration of drug is 50μg/L,thalidomide strongly inhibited the level of VEGF mRNA expression. Conclusion Within certain concentration range of thalidomide can inhibit the level of VEGF mRNA expression in breast cancer cell.

RESUMO

Objective: To investigate the function of healthy education in early enteral nutrition after operation.Methods: 72 patients of upper digestive tract operation were divided into test group(n=36)and control group(n=36).The education about enteral nutrition knowledge was intensified in test group. Results: The degree of grasping enteral nutrition knowledge and the time of receiving enteral nutrition were increased in test group.The complications of EN were decreased in test group.Conclusion: It is helpful to intensify the healthy education in early enteral nutrition after operation.

@#ObjectiveTo study the relationship between type of vertebral fracture and degree of spinal cord injury.MethodsMRI signs of 200 cases of thoracolumbar SCI were analyzed retrospectively.ResultsAmong all cases, 49 cases were with compressed fracture, 60 cases with burst fracture, 79 cases with fracture and dislocation, 158 cases with angle formed posteriorly. The number of cases with complete SCI were respectively 14 cases, 39 cases, 54 cases and 94 cases.Conclusions Common sites of thoracolumbar SCI. were at T12 and L1 level (123 cases, 74%). The degree of SCI was related to the type of vertebral fracture. The complete SCI occurred most frequently in patients with burst fracture and patients with fracture and dislocation, the incidence of the latter was higher.

@#The authors retrospectively analysed 67(63 in cervical cord)cases of traumatic SCIWOAR with positive MR abnormalities. It was deduced that the cases of SCIWOAR might be related with (1)those traumatic forces resulting in SCI being not so heavy and severe, (2)the cases orignally with cervical spondylopathy,(3)the limitation of the conventional X-ray examination for SCI,and (4)the cases of children relativly rare in our country and hospital.The ALL hematomas are the pathognomonic signs in the case of the acute cervical SCI within one week.The analysis demonstrated that MR investigation of the traumatic spine could give the clinical activities the correct diagnosis of different degree of SCI and dependable reference.

@#The analysis of 87 cases of MR imaging given impression of dysraphic myelodysplasias and their clinical materials found tethered cord deformity in 70 and non - "tethered" cord or "free" cord abnormalities in 17.The purpose of this study was to show that tethered cord was the most important clinical entity resulting in nervous disfunction of the spinal cord of the children among various dysraphic abnormalities and that MRI investigation was the best choice in order to obtain early and correct diagnosis and management of dysraphic myelodysplasias and tethered cord particularly.

Objective To study the effect of TBTCl on the immunity of mice. Methods The mice were randomly divided into 5 groups, namely groups of negative control, low dose (0.8 mg/kg), moderate dose (4 mg/kg), high dose (20 mg/kg) and positive control. Gavage was employed to administrate TBTCl to mice for two consecutive weeks, the number of T-lymphocyte in blood and invert ability of T-lymphocytes in spleen, hemolysin in serum, clearance exponent and viscera quotiety were determined to study the effect of TBTCl on immunity of rats. Results Compared with the control, no significantly increased T-lymphocytes were seen in low dose group, in moderate dose group the number of T-lmphocyte and the production of hemolysin increased, no obvious changes in the clearance exponent and viscera quotiety were seen. In high dose group, a significantly decreased T-lymphocytes, the clearance exponent and viscera quotiety were observed. Conclusion Low level TBTCl exposure may stimulate the immune system of mice, high exposed to TBTCl may intensively inhibit the immune system.
