RESUMO

Objective To explore the imaging features of solitary fibrous tumor (SFT)in the head and neck.Methods The CT and MR findings of 10 patients with SFT in the head and neck confirmed by hsitopathology were analyzed retrospectively.All patients underwent CT (including contrast-enhanced dual-phase CT),and MR imaging was obtained in 2 patients.Results All 10 lesions were found as solitary masses ranging in maximum diameter from 20 to 1 1 5 mm (average 35.5 mm),with the location in the orbit (2 cases),nasal cavity and nasal sinuses (4 cases),parotid gland (1 case),supraclavicular region (1 case),subcutaneous tissue of the maxillofacial (1 case)and occipital region (1 case).The lesions showed elliptic shape in 7 cases and lobulated configuration in 3 cases.The majority (8/10 )of lesions demonstrated well-defined margin.The lesions on CT imaging were isodensity or low density, homogeneous density in 4 cases and inhomogeneous density in 6 cases,cystic changes in 2 cases,calcification in 1 case.9 lesions showed strongly homogeneous (n=3)or inhomogeneous (n=6)enhancement,1 lesion showed mildly homogeneous enhancement on contrast-enhanced CT imaging.On dual-phase enhanced CT,8 patients exhibited a rapidly enhancing and slow washout pattern,1 patient exhibited a rapidly enhancing and rapid washout pattern,and 1 patient exhibited delayed enhancement.The lesions showed homogeneous isointense relative to gray matter on T1 WI in 1 patient,mixed isointense and hypointense in 1 patient.On T2 WI,the lesion showed isointense signal in 1 patient,and heterogeneous mixed isointense and hyperintense signal in another.2 lesions demonstrated markedly homogeneous and inhomogeneous enhancement respectively.Conclusion Solitary fibrous tumors shall be considered in cases of inhomogeneous hypervascular tumors in the head and neck.Compared with the gray matter,isointense or mildly hyperintense signal on T2 WI, marked enhancement on contrast-enhanced CT,and a rapidly enhancing and slow washout pattern on dual-phase enhanced CT may be valuable imaging features of SFT.

RESUMO

Metabolic syndrome is the fundamental factor in the pathogenesis of a variety of diseases, and it has not yet been fully understood due to its complicated mechanism. Multiple re-searches have implicated that Wnt/β-catenin signaling pathway may have a significant effect on the formation and development of metabolic syndrome. LRP6 is an important co-receptor of Wnt/β-catenin signaling pathway ,and there are some researches im-plicating the correlation between LRP6 and metabolic syndrome. The in-depth research on the gene polymorphism and its modula-tion mechanism can provide new ideas and directions for meta-bolic syndrome therapy.

<p><b>OBJECTIVE</b>To investigate the endovascular treatments for the ruptured aneurysms located at anterior communicating artery complex (ACoAC).</p><p><b>METHODS</b>The data of patients with ruptured ACoAC aneurysms treated in Department of Neurosurgery, First Affiliated Hospital to Fourth Military Medical University from May 2013 to December 2014 was retrospectively analyzed. Sixty-six cases were recruited including 50 male and 16 female patients. The patients aged from 31 to 69 years old, averaging (51±8) years. The Hunt-Hess grade at admission were 13 cases with grade Ⅰ, 36 cases with grade Ⅱ, 11 cases with grade Ⅲ, and 6 cases with grade Ⅳ. The most diameter of aneurysms sac: 14 cases less than or equal to 3 mm, 36 cases more than 3 mm but less than or equal to 7 mm, and 16 cases more than 7 mm. The height diameter/neck width ratio: 8 cases with absolute wide neck, 50 cases with relatively wide neck, and 8 cases with narrow neck. There were 28 cases underwent single micro-catheter embolization, 18 cases underwent double micro-catheters embolization, 14 cases underwent stent-assisted embolization and 6 cases underwent balloon-assisted embolization. The patients were followed up for 6 to 12 months and evaluated by modified Rankin score (mRS) and digital subtraction angiography (DSA). The ratio of total embolization, recurrence rate, and time from operation to reexamination of four groups managed by different endovascular treatment were compared by χ(2) test or F test.</p><p><b>RESULTS</b>Sixty cases were totally embolized, 3 cases subtotally embolized, 3 cases incompletely embolized. Mild hemiparalysis and aphasia occurred in 2 cases, and 1 case died of infarction induced by subarachnoid haemorrhage. The mRS at six months after operation were 0 in 31 cases, 1 in 22 cases, 2 in 8 cases, 3 in 2 cases, 4 in 2 cases, 6 in 1 case. All the included cases reexamined the DSA at averaging (7.5±1.0) month post-operatively and 4 cases recurred. There were not significant differences of the ratio of total embolization, recurrence rate, time from operation to reexamination among four groups (all P>0.05).</p><p><b>CONCLUSION</b>The endovascular treatment maybe an ideal management for ruptured ACoAC aneurysms.</p>

<p><b>OBJECTIVE</b>To study the pathologic characteristics and prognosis in different subtypes of adult medulloblastoma (MB).</p><p><b>METHODS</b>The clinical information, imaging findings and pathologic characteristics of 151 cases of adult medulloblastomas were retrospectively reviewed and analyzed by chi-square test. The survival data were assessed using the Kaplan-Meier method.</p><p><b>RESULTS</b>Amongst the 151 MB cases studied, there were 73 cases of classic MB, 36 cases of desmoplastic/nodular MB, 39 cases of anaplastic MB and 3 cases of large cell MB. The primary tumors were more frequently located in cerebral hemisphere in desmoplastic/nodular MB than in other subtypes (P=0.000).On the other hand, large cell/anaplastic MB were associated with more frequently local recurrence and distant metastasis (P=0.003). The post-operative overall survival time ranged from 6 to 150 months, with median survival being (103.3±5.7) months (95%CI, 92.52 to 115.09). The median survival of classic MB, desmoplastic/nodular MB and large cell/anaplastic MB was (110.7±7.8) months, (125.5±7.6) months and (57.6±7.6) months, respectively. The differences were statistically significant (P=0.000).</p><p><b>CONCLUSIONS</b>The three variants of MB show different biologic behavior. Large cell/anaplastic MB represents an independent poor prognostic indicator in adults.</p>

RESUMO

Objective To analyze the clinical manifestations,treatment and prognosis of adult medulloblastoma (MB).Methods A total of 163 cases of adult MB,confirmed by surgical and pathological diagnosis,were retrospectively analyzed about the clinical manifestations,imaging,treatment and prognosis.There are 108 males and 55 females in the group,whose average age was 28.6-year-old.Results The main clinical manifestations include headache,nausea,vomiting,dizziness,gait disturbance,hypopsia,diplopia,hearing loss and cerebellum crisis.Gross total resection was achieved in 90 cases,subtotal resection in 67 cases and biopsy in 6 cases.Survival time from surgery to progression or death or the last date of follow-up were measured and estimated by the Kaplan-Meier method.Among a total of 160 follow-up patients with MB,postoperative overall survival time was from 7 to 170 months,median survival time was (127±6) months (95% CI 115.3-138.68).The 5-year overall survival time of adult MB patients was 73.1% (120/163).The log-rank test was used to compare the significance of the following prognostic variables.Among all clinical factors,patients undergoing the craniospinal irradiation had a significantly better survival rate than those without this treatment (44 months vs 34 months,x2 =8.712,P =0.003).Conclusion Good therapeutic effect of MB be achieved by adopting surgical resection with early enough craniospinal irradiation.Recurrence and metastasis are the two main factors for bad prognosis in adult MB.
