Abstract Acute cholangitis is a bile duct infection associated with bile duct obstruction. Bile culture is positive in most cases, and the most frequent etiological agent is Escherichia coli. Candida sp acute cholangitis is a rare finding, which is more common in patients with im munosuppression, use of corticosteroids, prolonged antibiotic treatment or surgical procedures of the bile duct. We present the case of a 67-year-old woman with none of the above-mentioned history who consulted for fever, abdominal pain and jaundice. MRI of the ab domen revealed a lithiasic image in the common bile duct with dilation. It required endoscopic drainage of the biliary tract. Direct microscopic examination of the bile fluid revealed gram-negative bacilli and yeast, and in the culture of bile fluid Klebsiella pneumoniae produc ing extended spectrum beta-lactamase (ESBL) and Can dida glabrata were isolated. The patient completed the antibiotic treatment with piperacillin tazobactam and anidulafungin with good evolution. Bile duct infection by association of Gram-negative bacilli and Candida sp is a rare entity, more in patients without underlying diseases.

Abstract Renal abscesses are a rare complication of urinary tract infections and may be associated with increased morbidity and mortality. Most cases occur in patients with predisposing factors such as immunosuppression. Diagnosis requires high clinical suspicion and its treat ment consists in the use of parenteral antibiotics and antifungals associated or not with surgical interventions such as nephrostomy and nephrectomy. Few cases have been published in the medical literature of multifocal bilateral renal abscesses and even fewer due to Candida albicans. We present the case of a 20-year-old woman with type 1 diabetes mellitus, diagnosed at age 8, multiple hospitalizations for diabetic ketoacidosis, and recent hospitalization for candidemia (Candida albicans) treated with fluconazole for 23 days. Eighteen days after her discharge, she consulted for dull flank pain and gen eral symptoms. Contrast enhanced abdominal tomography showed bilateral multifocal abscesses and Candida albicans was isolated in one of the samples obtained from lesions. She received fluconazole 400 mg, 6 weeks i.v. and 2 weeks via enteral route, evolving favorably with clinical and imag ing improvement, continuing outpatient clinical monitoring. This report highlights the importance of diagnosis and treatment of this rare complication in complex diseases such as diabetes mellitus.

Aim: Periodontitis in primary dentition is exceptional in children without systemic diseases. The objective of this article is to describe the clinical and radiographic charac- teristics of two cases of systemically healthy 3-year-old chil- dren with periodontitis, and their treatment, with a 5-year follow-up. Clinical cases: In both cases, at 3 years of age, the chil- dren were referred to a periodontic specialist by their pediat- ric dentist, due to the very early loss of lower incisors. Clin- ical and radiographic examination showed loss of clinical attachment, bone loss and dental mobility in other upper and lower incisors. A medical consultation was carried out and diseases related to the diagnosis of periodontitis as a mani- festation of a systemic disease were ruled out. The treatment consisted of instruction on oral hygiene measures that had to be carried out by the parents, subgingival instrumentation, local antiseptics, systemic antibiotic medication, and perio- dontal maintenance. No extractions were performed as part of the treatment. In both cases, one of the incisors present at the time of consultation was lost prematurely, before the age of 4 years. The rest of the primary incisors completed their normal cycle. After 5 years of follow-up, at the age of 8 years, both children showed periodontally healthy incisors and first permanent molars, and the rest of the primary teeth without signs of periodontitis (AU)

The objective of this study was to carry out an adaptation of the Positive Driver Behaviors Scale (PDBS) for Brazil and verify if it would be possible to propose a brief version of the scale. The instrument originally composed of 38 items was applied to 204 motorists in Brazil, the majority being men (65.7%), with a mean age of 35.6 years. The result showed the possibility of a one-factor structure, consisting of a total of 19 items and Cronbach's alpha = .90, explaining 37.9% of the total variance. The relevance of this measure is highlighted because it is a tool capable of evaluating non-traffic behavior. Besides, it provides support to traffic professionals and psychologists, in order to instrumentalize their practices. It also allows further understanding of the phenomenon of transport, which can facilitate the development of effective measures to combat accidents.

RESUMO

Objective. The present study aims to evaluate the conditions associated with the progression of Multiple Sclerosis (MS) and patients' dysfunctions. Methods. We perform aretrospective longitudinal analytical observational study in 46 patients with MS from a polyclinic in Rio de Janeiro, Brazil. We used the Expanded Disability Status Scale (EDSS) to rank the pa-tients according to their disability and establish a correlation with risk factors, treatment, and time of disease. Results. Of 46 patients, 69.6% were female, and 67.4% were white. Patients with fewer functional systems affected at the beginning of the disease had a longer time for disease progression, according to EDSS. Low-efficacy drugs led to a high rate of discon-tinuation of the treatment. Patients who used a continuous treatment took longer to reach higher EDSS values than those who discontinued treatment. Conclusion. Despite the control of MS with high-effective drugs, there is still some disability for the patient. The factors that influenced the progression of the disability were: multiple symptoms at the beginning of the disease, more than 30 years old at the beginning of the MS, delay in diagnosis and initiation of treatment, among others. (AU)

Background: Oral candidiasis occur as an opportunistic infection. The transition of candida from commensal to pathogen is often associated with immune- compromised chronic renal patients receiving hemodialysis. Candida species identification and differentiation is important for treatment in these patients. Aim: To differentiate candida species present in the oral cavity of chronic renal failure patients undergoing hemodialysis. Material and Methods: A total of 120 individuals with study group (n=60) of chronic renal failure patients undergoing hemo- dialysis (CRF with HD) and control group (n=60) healthy individuals, were studied. Salivary samples were cultured for candida species using Sabouraud’s Dextrose Agar (SDA) and CHROM agar culture media, for the growth of candida species in 24, 48 to 72 hours at 37ºC. Colonies were counted and quantitatively expressed as colony forming units/milliliter (CFU). Results: Positive candidal growth was seen on SDA and CHROM agar medium among CRF with HD and Control Groups, can- dida species were present in 55 (91.6%) and absent in 5(8.3%) and 57(95%) and absent 3(5%) in individuals respectively. Can- dida species differentiation in CRF with HD and Control groups were C. albicans (green colonies), C. Kruzei (pink colonies) and C. Tropicalis (blue colonies) were 46(81.6%), 6(10.0%), 2(3.3%) in CRF with HD cases while 45(75%) 11(18.3%) and 0(0%) in control cases respectively. Conclusion: Isolation and differentiation of candida was highly significant (p<0.05) in chronic renal failure patients undergoing hemodialysis than healthy individuals.

Abstract Introduction: Child malnutrition is a public health problem that has affected the survival of some indige nous communities. Several studies have approached it from the biomedical perspective, although only a few have analyzed it from these communities' perspective. The objective of the present article is to under stand the community perspectives related to malnutrition in children aged 0-7 years in the Wayuu commu nities of Taiguaicat, Pañarrer, and Limunaka del Resguardo Manaure, La Guajira, Colombia. Development: A qualitative study was performed. Additionally, as a part of a participatory community-based research, 21 semi-structured interviews, 3 community forums, and a photovoice exercise were performed with 12 participants. The information was analyzed with analytical tools of the grounded theory. The findings around the community perspectives of child malnutrition were grouped into two analytical categories: (a) the Arijuna practices (or those carried out by non-indigenous people), from the loss of the ancestral to the disease of hunger, and (b) the disease of hunger and its relationship with the feeding of the moth er-child binomial during and after pregnancy. Conclusions: Child malnutrition in the Wayuu is a complex historical phenomenon, with a background of social inequalities and unjust power structures. According to the participants' responses, the survival of Wayuu children is threatened by malnutrition, which mainly affects the mother-child relationship caused by poor access to food and its limited availability, inadequate western practices in the territory, and the loss of traditional practices, such as planting and grazing.

To observe the efficacy of cinnamaldehyde on dextran sulfate sodium(DSS)-induced ulcerative colitis(UC) with Can-dida albicans(Ca) colonization and its effect on dectin-1/TLRs/NF-κB signaling pathway in mice. C57 BL/6 mice were randomly divided into normal group, DSS group, DSS+Ca group, cinnamaldehyde group and mesalazine group. Mice in DSS+Ca group were given Ca(1×10~8 CFU per mouse) through intragastrical administration for 4 consecutive days and then distilled water with 3.0% DSS for 7 consecutive days. In cinnamaldehyde group and mesalazine group, in addition to the induction method of the DSS+Ca group, mice were given 75 mg·kg~(-1) cinnamaldehyde and 200 mg·kg~(-1) mesalazine accompanied with 3.0% DSS for 7 consecutive days, respectively. Mice in normal group and DSS group were correspondingly administered with distilled water. The general conditions of the mice were observed daily, the diseased activity index(DAI) score was calculated, and fungal loads of feces were detected by plate method. The mice were sacrificed on day 12, colon length was measured, colon mucosa damage index(CMDI) score was calculated, and histopathological analysis was carried out by HE staining. Anti-saccharomces cerevisiae antibody(ASCA) and β-1,3-glucan in serum, and TNF-α, IL-1β, IL-6, IL-8, IL-10 in serum and colon tissue were detected by ELISA. The contents of β-1,3-glucan and macrophage infiltration in colon tissues were examined by immunofluorescence staining. The protein expressions of dectin-1, TLR2, TLR4 and NF-κB were detected by Western blot and immunohistochemistry staining. The results showed that cinnamaldehyde could significantly improve the general conditions of UC mice with Ca colonization, decrease DAI and histopathological scores, reduce intestinal mucosal congestion, erosion and colon shortening, decrease Ca load in mouse feces and tissues, down-regulate the contents of ASCA and β-1,3-glucan in serum, reduce the contents of TNF-α, IL-1β, IL-6, IL-8 and increase IL-10 in serum and colon tissues, inhibit macrophages infiltration and down-regulate the protein expression of dectin-1, TLR2, TLR4 and NF-κB in colon tissue. These results suggested that cinnamaldehyde had a therapeutic effect on UC mice with Ca colonization, which might be related to the inhibition of Ca proliferation, the regulation of dectin-1/TLRs/NF-κB signaling pathways and the coordination of the balance between pro-inflammatory and anti-inflammatory factors.

RESUMO

OBJECTIVE: To study the phylogenetic relationships of 3 basic plants of Tibetan medicine “Dida”, such as Swertia puricea, Wertia mileensis, Halenia elliptica. METHODS: ISSR technology was used for PCR amplification of 9 samples of S. puricea (ZT-1 to ZT-5 from Gantongsi in Dali Cangshan, ZC-1 to ZC-4 from Binchuan county of Dali), 2 samples of W. mileensis (QYD-1 to QYD-2) and 2 samples of H. elliptica (HM-1 to HM-2). Using DNA genome of S. puricea as template, 8 primers were screened and used for PCR reaction. The PCR amplification products were read by hand, the original data matrix was established, and the polymorphic band ratio was calculated. At the same time, genetic similarity coefficient was calculated by using NTSYS 2.1 software, and UPGMA method was used to draw cluster diagram. RESULTS: A total of 113 clear and identifiable amplification product bands were obtained by 8 ISSR primers. The rate of polymorphic site was 100%. The genetic similarity coefficients for totally 13 samples of S. puricea, W. mileensis and H. elliptica ranged 0.301-0.500. Intraspecific genetic similarity coefficients for 9 samples of S. puricea ranged from 0.752 to 0.929. The cluster analysis showed, when the range line was 0.410, 13 samples could be divided into three groups, i.e. S. puricea, W. mileensis, H. elliptica; when the range line was 0.780, 9 samples of S. purpurea could be divided into 2 subgroups, one of which was only sample ZT-1 collected from Gantongsi in Cangshan, and the other contained the remaining 8 samples. CONCLUSIONS: ISSR technology can be used to identify S. punicea, S. glabra and H. elliptica at the molecular level. S. punicea has some genetic relationship with S. glabra and H. elliptica, but the genetic relationship is relatively distant and the genetic difference is large. S. punicea from two different locations in Dali area has little genetic difference and close relationship, but it shows abundant genetic diversity.

Objective Rapid elevation of the IgG antibody against Candida Enolase ( Eno ) has been observed in patients with invasive candidosis in an early stage .The present study was to confirm the association of Candida colonization with humoral im-mune anamnestic response of the dominant antigen . Methods Twenty-four mice were randomized into group 1 treated by oral Candi-da colonization plus intraperitoneal infection ( immunocompetent , n=8) , 2 treated with immunosuppressant in addition to the treatment of group 1 ( immunocompromised , n=8) , 3 treated by oral Candida colonization only ( immunocompetent , n=4) and 4 treated by in-traperitoneal injection only( immunocompetent, n=4).The number of Eno-specific memory B-cells in the spleen and the levels of IgG , IgM and IgA antibodies were determined in the peripheral blood of the immunocompetent and immunocompromised invasive candidiasis mice . Results At 7 days after invasive infection , there were significantly more Eno-specific memory B-cells in the mice of groups1 ( 47.25 ± 13.81) and 2 (43.14±15.95) than in groups 3 (8.00±3.74) and 4(8.50±2.38) (P0.05).Eno-IgG antibodies were detected in the serum of the mice of the first two groups in the early stage of invasive infection and positively corre -lated with antigen-specific memory B-cells (r=0.737,P <00.1 ). Conclusion Rapid elevation of the Eno-IgG antibody level in the early stage of invasive infection after Candida colonization may be attributed to the rapid proliferation of humoral immune memory cells.

RESUMO

The ITS2 barcode was used toidentify Tibetan medicine "Dida", and tosecure its quality and safety in medication. A total of 13 species, 151 experimental samples for the study from the Tibetan Plateau, including Gentianaceae Swertia, Halenia, Gentianopsis, Comastoma, Lomatogonium ITS2 sequences were amplified, and purified PCR products were sequenced. Sequence assembly and consensus sequence generation were performed using the CodonCode Aligner V3.7.1. The Kimura 2-Parameter (K2P) distances were calculated using MEGA 6.0. The neighbor-joining (NJ) phylogenetic trees were constructed. There are 31 haplotypes among 231 bp after alignment of all ITS2 sequence haplotypes, and the average G±C content of 61.40%. The NJ tree strongly supported that every species clustered into their own clade and high identification success rate, except that Swertia bifolia and Swertia wolfangiana could not be distinguished from each other based on the sequence divergences. DNA barcoding could be used as a fast and accurate identification method to distinguish Tibetan medicine "Dida" to ensure its safe use.

RESUMO

Tibetan medicine "Dida" isoccasionally misused due to its complex origins, which ultimately affects its clinical efficacy. The accurate name, origin, property, and efficacy of "Dida"are highly important for its further research and development. In the present study, by viewing the classic Tibetan medicine and modern literature, and combining the clinical practice of Tibetan medicine, the origins, properties and the clinic effects of "Dida" were defined. "Dida" originated from multiple plant species of Swertia, Gentianopsis, Halenia, Lomatogonium, Comastoma(Gentianaceae), Hedyotis (Saxifragaceae) and Erysimum (Cruciferae). The medicinal properties of "Dida" is mainly bitter and cold. It has been commonly used to treat febrile diseases and hepatic and gall diseases. This study suggested that the relevant herbalogical study, species identification and pharmacological effects of "Dida" should be taken based on the Tibetan medicine theories and clinical practice. Thus the medicine can be better used and ensure its safety and quality simultaneously.

RESUMO

"Dida" is one of the most representative commonly used Tibetan medicines in treatment of hepatobilaiary diseases. To analyze and compare the content of 7 effective constituents in 11 species which are used as Dida. Integrated evaluation of them, which supply a basis for the resources exploitation for them. In this paper, most appropriate absorption wavelengths of different natural iridoid compounds, xanthones and oleanolic acid have been performed by using 3D spectrum technique. According to the results, the wavelength 210, 240 and 265 nm were used for detection of 7 effective constituents. By means of calculation, determination of 7 effective constituents indicated good linearity over the linear range with coefficients (r) of 0.999 9 respectively. The recoveries were found in the average range of 99% to 101% with RSD being less than 3%. Normalize and integrate by three-dimension the data. Get their integrated chemical quality index(ICQI). Results suggested that these plants used as Dida in Tibet medicine mostly contain these 7 effective constituents, but the different is notable. The integrated evaluation by the integrated chemical quality index (ICQI) has reference value for quality control.

RESUMO

Aim To investigate the inhibitory effect of ellagic acid( EA) on fungal strains both in vitro and in vivo. Methods The minimal inhibitory concentration ( MIC ) was determined by the NCCLS ( M27-A2 ) standard method in vitro. The therapeutic action was studied using a Candida albicans infect mice model, the survival rate and the content of SOD, MDA in ser-um were measured, and the pathological changes of liver were observed under light microscope after HE staining. Results EA was active against three Candida strains, with MICs between 25. 0 and 75. 0 mg·L-1 . The most sensitive Candida species was C. albicans ( MIC=25. 0 mg·L-1 ) . It was inactive against Can-dida glabrata. An in vivo test demonstrated that intrap-eritoneal injection of EA(40 and 80 mg·kg-1 ) signif-icantly enhanced the pathological changes of liver, im-proved the symptom, increased the weight and the SOD activity, and decreased the MDA activity in a mice in-fection model of C. albicans. Conclusion Ellagic acid has the potential to be developed as a natural antifungal agent.

RESUMO

The incidence of invasive candida infection in critically ill patients is rising year by year. It's clinical manifestation lacks specificity which is easy to result in missed diagnosis or misdiagnosis.Tradi-tional diagnosis methods need very long time and have low positive rate, which are easy to delay treatment,so early diagnosis is particularly important.1 ,3-β-D-glucan,mannan and antibodies,anti-enolase antibodies,can-dida score and colonization index may be helpful to make early diagnosis.
