Abstract Bazex syndrome is a paraneoplastic disorder most commonly linked to squamous cell carcinomas of the upper aerodigestive tract, followed by lung cancer and other malignancies. It manifests through three stages of skin involvement that mirror the tumor's progression. Remarkably, skin lesions precede tumor symptoms or diagnosis in two-thirds of cases, underscoring the cru cial role of suspecting this condition as it can promptly reveal an underlying neoplasm. Treatment primarily focuses on addressing the root neoplasm, with recurrent skin lesions potentially indicating tumor relapse. In this context, we present a clinical case involving a male patient whose manifestation of this syndrome facilitated the timely diagnosis of lung adenocarcinoma. This case underscores the significance of understand ing this uncommon syndrome and its link to cancer, enabling early and accurate oncological diagnosis.

Abstract Anticoagulant treatment, together with antiplatelet therapy, plays an important role in the treatment of acute coronary syndromes. Its use is associated with a reduction in new ischemic events, stent thrombosis, and lower mortality. However, in clinical practice there is great heterogene ity in its use, leading to suboptimal results in treatment. This paper conducts a narrative review on the use of parenteral anticoagulants in patients with acute coronary syndromes, depending on the clinical scenario, as well as the revascularization strategy used and the bleeding risk. The different anticoagulant schemes available in acute coronary syndromes with and without segment ST elevation are addressed, based on the updated evidence. Finally, evidence-based strategies for risk stratifi cation for bleeding and therapeutic management are developed.

Abstract Hodgkin lymphoma (HL) comprises a heterogeneous group of lymphoid neoplasms whose origin lies in B lym phocytes. The neurological manifestations of this pathol ogy are infrequent, and may arise from direct invasion of neoplastic cells to the nervous system, or indirectly, through paraneoplastic syndromes or as a complication of treatment. Among the neurological paraneoplastic syndromes that affect patients with HL, paraneoplastic cerebellar degeneration is the most common. Other few cases include limbic encephalitis, sensory, motor, and autonomic neuronopathy. These syndromes can be the initial manifestation of neoplastic disease, and the lack of information regarding this association can lead to a delay in diagnosis and consequently in the initiation of therapy worsening the prognosis. We report the case of a woman with HL who presented sensory and autonomic neuronopathy at the onset of her disease as paraneo plastic neurological manifestations. After the initiation of the specific treatment for the lymphoma, the autonomic neuronopathy had almost complete resolution, unlike the sensory neuronopathy, which showed limited recovery.

RESUMO

This study aimed to test the hypotheses: (i) the deferred pasture with lower height in late winter has greater herbage accumulation rate (HAR) and better structure, which facilitates the animal selective grazing during the subsequent spring and summer; (ii) the mowed of high pasture in late winter improves the sward structure but decreases its HAR from the spring. Four pasture condition in late winter of Urochloa brizantha cv. Marandu were evaluated: low (24.1 cm and 2,420 kg.ha-1 of DM); low/mowed (25.2 cm and 2,198 kg.ha-1 of DM, mowed at 8.0 cm); high (49.0 cm and 3,837 kg.ha-1 of DM); and high/mowed (50.0 cm and 4,211 kg.ha-1 DM, mowed at 8.0 cm). The highest live leaf blade percentage (LLBP) and the lowest dead stem percentage (DSP) occurred in the mowed pastures. The live stem (LSP) and dead leaf blade percentages of the grazing simulation sample were higher in the high pasture. This same pattern of response occurred for apparent selectivity indices (ASI) of the live leaf blade and live stem. The ASI of the dead leaf blade was greater in the high pasture, intermediate in the mowed pastures, and lower in the low pasture. The ASI of the dead stem was lower in the low pasture. The HAR was higher in low pasture. Our results support the first hypothesis. The mowing of high pasture in late winter improves the sward structure and the animal selective grazing but does not decrease the HAR during spring and summer.

RESUMO

The pasture mowing in late winter removes the old forage, improving the pasture structure in spring and summer. However, the residue after mowing can affect tillering, thus limiting forage production and the structural characteristics of pasture. We hypothesized that the high amount of cut vegetal residues on the plants in late winter causes a decrease in forage production and modifies the structural characteristics of the forage canopy during the spring and summer. The treatments were four cut vegetal residues (0; 2,000; 4,000 and 8,000 t ha-1 of natural material) deposited on Urochloa brizantha cv. Marandu canopy in late winter. After that, the forage production and structure characteristics were evaluated during the spring and summer for two years. The complete randomized block design, with four replications, was used. The defoliation management was characterized by the adoption of pre- and post-cut heights of 25 and 15 cm, respectively. For both years, there was no effect of cut vegetal residue on all the characteristics evaluated. No variable was affected by the interaction cut vegetal residue amount × year of evaluation. The dead leaf blade percentage was greater in year 2 (6.9%) than in year 1 (3.5%). The average values of variables were live leaf blade percentage = 84.2%; live stem percentage = 8.7%; dead stem percentage = 1.9%; number of vegetative tiller = 653 tiller/m2; number of reproductive tiller = 5 tiller m-2. The forage production rate presented an average value of 46 kg/ha/day of dry matter. The cut vegetal residue amount of up to 8,000 t/ha of natural material on the plants in late winter does not affect the forage production or modifies the structural characteristics of marandu palisadegrass during the subsequent spring and summer.

RESUMO

The control of sward height at the early stockpiling period influences the structural characteristics of stockpiled sward in winter. So, we conducted this work on the Cerrado region of Brazil based on the hypothesis that the reduction of pasture height at the beginning of stockpiling period results in: (i) lower stock of forage mass; and (ii) higher live leaf mass, but lower stem mass in stockpiled pasture during the winter. The treatments were four sward heights (15, 25, 35, and 45 cm) at early stockpiling period of marandu palisadegrass (Brachiaria brizantha syn. Urochloa brizantha cv. Marandu). The experiment was conducted in a completely randomized design, with three replications (paddocks with 800 m²). The stockpiled swards were evaluated during the grazing period in winter on days 7 (beginning), 45 (middle) and 90 (end). The vegetative tiller number (VEGT), live leaf mass (LLM) and leaf area index (LAI) were greater in the 15 cm stockpiled pasture than the other treatments. Greater heights (35 and 45 cm) at stockpiling resulted in greater forage mass than lower heights (15 and 25 cm). The VEGT, LLM, number of reproductive tillers, live stem mass, forage mass and LAI values were higher at the beginning than at the end of the grazing period. Our hypothesis has been proven, so that the reduction of sward height of the marandu palisadegrass at the beginning of the stockpiling period results in lesser forage mass, but with better structural characteristics.

RESUMO

The objective of this study was to evaluate the spatial variability of structural characteristics of deferred pastures of Brachiaria decumbens cv. Basilisk (signal grass) subjected to associations of sward heights (10, 20, 30, and 40 cm) at the beginning of deferment and grazing periods (1, 28, 56, 85, and 113 days). The experiment was arranged in a split-plot and completely randomized-block design with two replicates. Pastures remained deferred from March to June 2010. From June to the beginning of October 2010, deferred pastures were utilized by steers under continuous grazing and at a fixed initial stocking rate of 3.5 AU ha-1. At the beginning of the deferment, the coefficient of variation (CV) for pasture height was reduced linearly with the pasture height. During the grazing period, in the winter, the CV for pasture height was not influenced by initial pasture height; however, it responded quadratically to the grazing period, with a maximum value of 36.4% at 71 days of the period of utilization. The CV for tiller height was reduced linearly with pasture height at the beginning of deferment but increased linearly along with the grazing period. The initial pasture height did not change the CV of the falling index. However, the latter was quadratically influenced by the grazing period, with a maximum value of 59.5% at 67 days of the utilization period. Signal grass pastures deferred at a lower height have a higher spatial variability of the vegetation. In the grazing period, there are changes in the spatial variability of the vegetation of the deferred signal grass.

RESUMO

OBJECTIVE@#To investigate the mechanism of valproic acid (VPA) -induced impairment of the dendritic spines and synapses in the prefrontal cortex (PFC) for causing core symptoms of autism spectrum disorder (ASD) in mice.@*METHODS@#Female C57 mice were subjected to injections of saline or VPA on gestational days 10 and 12, and the male offspring mice in the two groups were used as the normal control group and ASD model group (n=10), respectively. Another 20 male mice with fetal exposure to VPA were randomized into two groups for stereotactic injection of DMSO or Wortmannin into the PFC (n=10). Open field test, juvenile play test and 3-chamber test were used to evaluate autistic behaviors of the mice. The density of dendrite spines in the PFC was observed with Golgi staining. Western blotting and immunofluorescence staining were used to detect the expressions of p-PI3K, PI3K, p-AKT, AKT, p-mTOR, mTOR and the synaptic proteins PSD95, p-Syn, and Syn in the PFC of the mice.@*RESULTS@#Compared with the normal control mice, the mice with fetal exposure to VPA exhibited obvious autism-like behaviors with significantly decreased density of total, mushroom and stubby dendritic spines (P < 0.05) and increased filopodia dendritic spines (P < 0.05) in the PFC. The VPA-exposed mice also showed significantly increased expressions of p-PI3K/PI3K, p-AKT/AKT, and p-mTOR/mTOR (P < 0.01) and lowered expressions of PSD95 and p-Syn/Syn in the PFC (P < 0.05 or 0.001). Wortmannin injection into the PFC obviously improved the ASD-like phenotype and dendritic spine development, down-regulated PI3K/Akt/mTOR signaling pathway and up-regulated the synaptic proteins in VPA-exposed mice.@*CONCLUSION@#In male mice with fetal exposure to VPA, excessive activation of PI3K/Akt/mTOR signaling pathway and decreased expressions of the synaptic proteins PSD95 and p-Syn cause dendritic spine damage and synaptic development disturbance in the PFC, which eventually leads to ASD-like phenotype.

RESUMO

This work was conducted to evaluate the effect of deferred pasture condition of Brachiaria brizantha cv. Marandu in the late winter on tillering during the growing season. The treatments were three pasture conditions at late winter: short pasture, tall pasture and tall/mown pasture. In September and October, tiller appearance rate (TApR) and tiller mortality rate (TMoR) were greater in the tall/mown pasture. In November and December, tall pasture presented greater TApR. From November to January the TMoR was greater in the tall pasture. The tiller stability index of short and tall/mown pastures were greater in October. The short pasture presented a greater tiller number than the tall and tall/mown pastures during the entire experimental period. Deferred and short pasture of marandu palisade grass at late winter presents in general lower tiller mortality and higher population density of tillers from the early spring onwards, in comparison to tall pasture. The mowing of marandu palisade grass with high forage mass at the late winter, although it only temporarily compromises the population stability of tillers, also stimulates its fast tillering from spring on.(AU)

ABSTRACT Fracture and migration of totally implantable venous access devices is a rare but potentially serious complication. Pinch-off syndrome was described in subclavian venous accesses when the intermittent compression of the catheter between the first rib and the clavicle produces catheter fracture. The report the case of a patient with cancer who underwent implantation of a totally implantable venous access device through the subclavian vein under ultrasound and radioscopy guidance. After several cycles of chemotherapy, the patient started with malfunctioning of the device. The chest X-ray showed a complete fracture of the catheter at the level of the costoclavicular space with migration of a fragment of the catheter to the right cardiac chambers. The fractured catheter was percutaneously removed via the right femoral vein using nitinol gooseneck snare without complications. The incidence of the pinch-off syndrome, risk factors and prevention measures in totally implantable venous access devices are analyzed.

RESUMO

Parkinson's disease (PD) is the second major neurodegenerative disease. The pathogenesis of PI) is still unclear. It is generally believed that neural damage, mitochondrial dysfunction, inflammation, oxidative stress and autophagy dysfunction caused by the transmission and aggregation of a- synuclein play an important role in the occurrence and development of PD.More and more research show- that metabolic disorder is one of the pathogenesis of PD.We examined whether overexpression of a- synuclein could induce metabolic disorder in mice and the possible mechanisms.Mice were divided into two groups: Thyl-aSYN transgenic mice (TG) and the control wild-type (WT) group. The rotarod test was used to analyze motor function in mice.We detected the body weight, plasma insulin content, glucose tolerance and insulin tolerance in the two group mice. The morphology of islets in the two groups were observed by hematoxylin eosin (HE) staining, and the islets were isolated to detect the glucose- stimulated insulin secretion (GSIS).The results showed that compared with the WT group, exercise tolerance of 12-month-old TG group decreased by 23.1% (P < 0.05) , body weight increased by 7% (P < 0.01), glucose tolerance decreased (P < 0.05), insulin tolerance decreased (P < 0.05), and insulin contents in the peripheral blood decreased by 20% (P < 0.05).Compared with the WT group, the levels of ce -syn proteins in the pancreas of the TG group increased by 1.32 times (P < 0.05) , the area of islets in the TG group decreased (P < 0.05 ) , the number of islets decreased (P < 0.01) , and the insulin secretion function decreased (P< 0.01).This study showed that the role of a-synuclein in PD is not limited to the damage of dopaminergic neurons, it also can affect metabolism and the morphology and function of peripheral organs, which provides a new theoretical basis for the pathogenesis of PD.

RESUMO

The structure of the sward is important because it influences the responses of plants and animals in grazing conditions. The objective with this work was to evaluate the structural characteristics of the Marandu palisade grass pasture (Brachiaria brizantha syn. Urochloa brizantha) during spring and summer, depending on the condition of the sward at the end of winter. Four grazing conditions at the end of winter were evaluated: short (24.1 cm), short (25.2 cm)/mown (8 cm), tall (49.0 cm) and tall (50.0 cm)/mown (8 cm). The foliar area index was lower in the tall pasture than in the tall/mown pasture. The mass and the volumetric density of dead stem were higher in the tall pasture, intermediate in the short pasture, and lower in mowed pastures. At the beginning of the grazing period, the live stem mass was higher in the tall pasture. At the beginning and middle of the grazing period, the volumetric density of dead leaf lamina was lower in mowed pastures. At the beginning of the pasture period, the short pasture had a higher tiller number (TN). In the middle of the grazing period, the short and short/mown pastures had higher TN. At the end of the pasture period, the lowest TN value occurred in the tall pasture. The tall pasture at the end of winter presents an unfavorable structure to the animal in grazing, while the mowing and reduction in the height of the marandu palisade grass improves the structure of the sward in spring and summer.

The relative contribution of young, mature and old tillers in the canopy influences the production and structure of the pasture. The objective with this work was to evaluate the balance between tiller appearance and tiller death (BAL) during spring and early summer, the morphology and percentages of young, mature and old tillers in Brachiaria brizantha cv. Marandu (marandu palisadegrass) with three conditions in late winter: short (24.1 cm), tall (49.0 cm) and tall (50.0 cm)/mown (8 cm). Tall and tall/mown pastures presented higher BAL in September and October, respectively. In January, BAL was higher in short and tall/mown pastures than in tall pasture. The tiller number was higher in short pasture, intermediate in tall/mown pasture and lower in tall pasture. The percentage of old tillers was higher in short and tall pastures compared to tall/mown pasture. The percentage of live leaf lamina reduced, while the percentage of dead leaf lamina increased with tiller age. Mowing of the tall marandu palisadegrass pasture in late winter increases the renewal of tillers in the spring and decreases the percentage of old tillers in the summer. The old tillers present worse morphology than young tillers.

<b>Objective::To investigate the mechanism of Buyang Huanwu Tang (BYHWT) in improving synaptic structural plasticity after cerebral ischemia-reperfusion in rats. <b>Method::Middle cerebral artery occlusion and reperfusion model was established. SD rats were randomly divided into sham-operated group, model group, BYHWT group, BYHWT+ Gap26(connexin43 inhibitor)groups. BYHWT was given twice a day(16 g·kg-1), Gap26 was intraperitoneally injected once a day since the third day after surgery (25 g·kg-1). Brain was taken out at the 7th day. The changes of neuronal synaptic and gap junction ultrastructure were observed by transmission electron microscopy. Synaptophysin (SYN) and growth-associated protein-43 (GAP-43) protein expression were detected by Western blot and immunofluorescence. <b>Result::The structure of synapses was integrated, and the gap junctions were clear in sham-operated group. In the hippocampus of model group, the structure was destroyed, and the gap junctions disappeared. Compared with the sham-operated group, model group up-regulated the expressions of SYN and GAP-43 (P<0.05, P<0.01). In the hippocampus of BYHWT group, the structure was close to the normal. Furthermore, BYHWT up-regulated the expressions of SYN and GAP-43 (P<0.05, P<0.01). However, after the combined administration with Cx43 inhibitor (Gap26), the damage of synaptic structural decreased, only a small number of gap junctions with the structural integrity can be seen, and the effect of BYHWT on SYN and GAP-43 was inhibited (P<0.05, P<0.01). <b>Conclusion::BYHWT could improve the hippocampal synaptic structural plasticity obviously after the CIRI. The mechanism may be related to the increase of the expression of Cx43 and the promotion of the intervention of SYN and GAP-43.

The objective of this study was to evaluate the apparent selectivity of sheep in marandu palisadegrass (Urochloa brizantha cv. Marandu) pastures with four heights at the beginning of the deferment period (15, 25, 35 and 45cm). The deferment period was 92 days and started on 03/21/2014. Evaluations occurred in the beginning (first week), middle (45th day) and end (92nd day) of the grazing period, in winter (06/21/2014 to 09/21/2014). Deferred pastures with 15 and 25cm presented lower forage mass (FM), but higher live leaf (LL) percentage in FM than deferred pastures with 35 and 45cm. The live stem percentage in the FM and the apparent selectivity index (ASI) of the LL were superior in the deferred pasture with 45cm. The dead stem (DS) percentage in the grazing simulation (GS) and the ASI of this morphological component were lower in the pasture with 15cm, compared to the deferred pasture with 45cm. The FM and the LL percentages in FM and in the GS sample decreased, while the DS percentages in FM and in GS sample increased with the grazing period. Marandu palisadegrass with 15cm at beginning of the deferment period improves the morphology of the deferred pasture. Selective grazing is difficult during the grazing period.(AU)

Dept of Physiology, School of Basic Medical Sciences, Bitehou Medical University, Yantai Shandong 264003, China Aim To explore the behavior and CBS/H2S levels in infralimbic (IL) medial prefrontal cortex (mPFC) in conditioned fear rats and study the effect of exogenous H2S on conditioned fear extinction of rats and the related mechanism. Methods The rat conditioned fear model was established with sound pairing with an aversive foot shock. Later the repeated single sound was used for extinction training. CBS/H2S con-tent in IL tissues was measured using Western blot and methylene blue method. In vivo extracellular single u-nit recordings were used to examine the frequency of spontaneous discharges of IL neurons. Results 1. CBS/H2S level in IL tissues of conditioned fear model (control group) decreased (P <0. 01) and the freezing level was aggravated (P < 0. 01) , while after extinction training CBS/H2 S level increased (P < 0.01, P < 0. 05) and the freezing level was alleviated (P < 0.01). 2. Freezing level decreased in NaHS group compared with extinction group (P <0. 01) . 3. L-cys-teine increased the frequency of spontaneous discharges of IL neurons (P < 0. 01). Conclusions Learning and memory ability decreases in PTSD model rats owing to the inhibition of CBS/H2S content in IL tissues. The mechanism of behavior improvement of H2S on PTSD model rats may be related to the excitation of H2S on frequency of spontaneous discharges of IL neurons.

RESUMO

Adult specimens of Echinochasmus caninus n. comb. (Verma, 1935) (Trematoda: Echinostomatidae) (syn. Episthmium caninum Yamaguti, 1958) were recovered from 11 riparian people who resided along the Mekong River in Khammouane Province, Lao PDR. In fecal examinations done by the Kato-Katz technique, the cases revealed eggs of Opisthorchis viverrini/minute intestinal flukes, hookworms, and in 2 cases echinostome eggs. To recover the adult helminths, praziquantel 30–40 mg/kg and pyrantel pamoate 10–15 mg/kg in a single dose were given and purged with magnesium salts. Various species of trematodes (including O. viverrini and Haplorchis spp.), cestodes, and nematodes were recovered from their diarrheic stools. Among the trematodes, small echinostome flukes (n=42; av. 3.8 specimens per case) of 0.7–1.2 mm in length are subjected in this study. They are morphologically characterized by having 24 collar spines interrupted dorsally and anterior extension of vitellaria from the cirrus sac or genital pore level to the posterior end of the body. Particularly based on this extensive distribution of vitellaria, the specific diagnosis was made as Echinochasmus caninus. The cases were co-infected with various other helminth parasites; thus, clinical manifestations specific for this echinostome infection were difficult to determine. The present paper describes for the first time human E. caninus infections in Lao PDR. Our cases marked the 4–14th human infections with this echinostome around the world following the 3 previous cases reported from Thailand.

RESUMO

Mycoplasma pneumoniae (MP)is one of the most common causes of community-acquired pneu-monia in children. The incidence of respiratory failure due to Mycoplasma pneumoniae pneumonia (MPP)is relatively rare despite the high prevalence of MP infection. Prevalence of respiratory failure was unsure in children, most of them presented with hypoxia or acute respiratory distress syndrome. Key clinical findings include cough, fever, and dyspnea along with diffuse abnormal findings in radiological examinations. Levels of inflammatory markers such as white blood cells and C-reactive protein are elevated. The exact pathogenesis of fulminant MPP remains unclear, radiological ima-ges usually showed bilateral infiltrates and pleural effusion commonly present in MPP cases with respiratory failure. Treatment options involve pairing the appropriate anti-mycoplasma agent with a corticosteroid which aimed at decrea-sing the hypersensitivity response, further research is necessary to determine the exact pathogenesis of MPP with respira-tory failure. Now,the most recent data in epidemiology, research situation, pathogenesis, clinical and X-ray presenta-tion,and treatment of respiratory failure with MPP are described.
