RESUMO

Since the 18th National Congress of the Communist Party of China (CPC), a continuous stream of scientific and technological innovations has unfolded in the realm of traditional Chinese medicine (TCM). With the aim of implementing the spirit of the 20th National Congress of the CPC, and the Opinions on Promoting the Inheritance, Innovation and Development of TCM, and to underscore the exemplary role of significant scientific and technological achievements, the China Association of Chinese Medicine, in alignment with relevant requirements and under the guidance of authoritative experts, has organized a comprehensive review of the important scientific and technological achievements in the field of TCM since the 18th National Congress of the CPC. Through rigorous procedures, including collecting and reviewing achievements, writing achievement reports, organizing expert reviews, and seeking public opinions, remarkable research achievements in TCM during 2012—2022 were compiled.

Abstract Objective: After Endoscopic Sinus Surgery (ESS), packing plays an important role in wound healing and hemostasis. However, the effect of the packing removal procedure on physician stress has not been evaluated. The purpose of this study was to evaluate physician stress during packing removal for patients treated with AQUACEL® Ag Advantage versus KALTOSTAT®. Methods: This retrospective study included 15 patients who underwent packing with ESS for chronic rhinosinusitis performed at two centers; 9 were treated with AQUACEL® Ag Advantage and 6 were treated with KALTOSTAT®. Physician stress during packing removal was evaluated with the National Aeronautics and Space Administration-Task Load Index (NASA-TLX). The time required to remove the packing and the number of instruments used in the procedure were recorded. Postoperative bleeding (Boezaart bleeding score) and wound healing were graded. Patient symptoms on the day after surgery and pain during packing removal were assessed using a visual analog scale. Results: Computed tomography scores, asthma complications, and blood eosinophil counts were significantly higher in the AQUACEL® Ag Advantage group. Patient symptoms on the day after surgery were not significantly different between the two groups. Physician stress during the task of packing removal was significantly lower in the AQUACEL® Ag Advantage group than in the KALTOSTAT® group (35.5 vs. 81.0, p = 0.016) according to the NASA-TLX scores. The number of instruments used in the procedure was significantly lower in the AQUACEL® Ag Advantage group than in the KALTOSTAT® group (3.0 vs. 6.0, p = 0.015). There were no significant differences in procedure time for packing removal, postoperative bleeding, wound healing, or patient pain at the time of packing removal between the groups. Conclusion: Physicians feel stressed about packing removal. In addition, AQUACEL® Ag Advantage is useful for packing after ESS, requiring fewer instruments for the procedure than KALTOSTAT® and reducing physician stress about the procedure. Model-induced drug development (MIDD) is a mathematical and statistical method for constructing, validating and utilizing disease model, drug exposure-response model and pharmaceutical model to promote drug development. With the development of pharmaceutical technology, MIDD is widely used in the field of traditional Chinese medicine and has high practical value. This article summarizes the relevant literature at home and abroad, and finds that MIDD has the advantages of improving the research and development efficiency of traditional Chinese medicine, quickly identifying the applicable population of traditional Chinese medicine, predicting the interaction of drugs, and optimizing the dosage. MIDD has been applied in the studies of effective components of traditional Chinese medicine, quantitative design of prescription, dosage form and preparation process, pilot scale- up, quality and safety, regulatory decision-making and evaluation, etc.

RESUMO

The most common subtype of lung cancer is non-small cell lung cancer (NSCLC), which has a poor prognosis and seriously threatens the health of human beings. The multidisciplinary comprehensive treatment model has gradually become the mainstream of NSCLC treatment. Traditional Chinese medicine (TCM) can be used effectively either as an adjunctive therapy or alone throughout the NSCLC therapy, which has a significant impact on survival, quality of life, and reduction of toxicity. Therefore, this paper reviewed the theoretical basis, the latest clinical application, and combined treatment mechanisms in order to explore the advantage stage of TCM treatment and the synergistic therapeutic mechanisms.

The present study explored the effective approaches to realize the leading role of traditional Chinese medicine (TCM) in preventing diseases, the synergistic role in treating serious diseases, the core role in the rehabilitation of diseases and summarized the experience to provide feasible plans for the evaluation of other dominant diseases of TCM. To evaluate the effectiveness, safety, and economy of TCM in the treatment of ischemic stroke, encephalopathy project team of the China Center for Evidence-based Traditional Chinese Medicine(CCEBTCM) established an evaluation group to determine the work plan and complete the evaluation work. The concepts of the evaluation involved high-quality evidence, expert opinion survey, expert interview, and drug catalog. Under the guidance of clinical experts and methodologists, the evaluation work was completed in accordance with four steps, i.e., plan making, data collection and data extraction, evidence synthesis and evaluation, and report writing with the rapid review method. Through the review of TCM and western medicine experts, the advantage of TCM in the treatment of ischemic stroke was positioned in the convalescence period with the predominant effects of improving the neurological function defect and improving the daily living ability. In the convalescence period of stroke, TCM treatment could improve post-stroke motor dysfunction, post-stroke cognitive impairment, consciousness disorder, swallowing disorder, aphasia, constipation, urinary function, diplopia, etc., and the advantages of acupuncture, Chinese medicine, and traditional exercise were more prominent. In terms of safety, TCM treatment of ischemic stroke showed lower incidence of adverse reactions, fewer adverse events, and a milder degree of related symptoms. In terms of economic performance, the combined treatment of TCM and western medicine played a synergistic role and made the treatment cost more reasonable. Compared with conventional intervention, the integrated TCM and western medicine rehabilitation program showed more economic and social benefits.

Objective:To analyze the application for and funding for individual key clinical specialties under the Youth Program at the approved hospital groups, for suggestions on future development of hospitals.Methods:Based on the Youth Program data of the applications and approval as summarized by Beijing Hospitals Authority from year 2015 through 2018, 22 municipal hospitals were classified into three groups with more, less and none key clinical specialties respectively(the more group, the less group and the non group for short). The categories were made according to the number of state key clinical specialties approved by National Health Commission and key specialties approved by the National Administration of Traditional Chinese Medicine. χ2 test was used to compare the baseline situation, the approval rate of project applications, and the categories of approved project among the three hospital groups. Results:During 2015—2018 period, the number of applicants for Youth Program was 1 128 persons/cases. The groups with more, less, and none key clinical specialties were 7, 10, 5 respectively. In the more specialties group, the applicants of seniority less than five years accounted for 40.93%, who had higher than those of the other two groups( χ2=13.794, P=0.032), and higher doctoral degree applicants(80.31%) as well. The difference was statistically significant( χ2=116.611, P<0.001). The approval rate of the Youth Program in 2015—2018 in the group with key clinical specialties vs. the group with none key clinical specialties was 36.09%(166/460) rersus. 29.04%(194/668) with significant difference( χ2=6.222, P=0.013). The approval rate of the departments with more key clinical specialties was 37.82%, higher than those of less and none key clinical specialties(approval rate at 31.82% and 21.50%, respectively), with significant difference( χ2=16.897, P<0.001). However, there was no significant difference between the approval rates of key specialties group and non-key specialties in each hospital group( P>0.05). Of the three groups, the proportion of clinical medical projects in each group was all higher than such other types as scientific research, medical technology, hospital administration and nursing. Conclusions:The applications and approval rate of key specialties under the Youth Program were higher than the rest, reflecting the discipline advantage. At the same time, it can play an important leading role in the construction of research-oriented hospitals. It is necessary to further strengthen the showcase role of specialties, for encouraging innovative talents cultivation of other specialties, in the hope for promoting the construction of research-oriented hospitals in their clinical, scientific research, medical technology, management and nursing comprehensively.

Licorice extract has the efficacy of anti-inflammatory, anti-viral, anti-oxidation and anti-aging. It is widely used in the pharmaceutical, food and cosmetic industries. This paper analyzes the status of licorice extract’s global trade and its international competitiveness. The main purpose is to clarify the status of our country’s licorice extract in the international market and provide reference for improving the international competitive advantage of licorice extract of China. Germany, the United States, Netherlands, France, Japan, Israel and China were the main import and export countries for licorice extract. The international market share (IMS), trade competitiveness index (TC Index), revealed comparative advantage index (RCA Index) and competitive advantage index (CA Index) were used to compare the licorice extract’s international competitiveness of these countries. It showed that Israel has the strongest international competitiveness and followed by France. The United States has the maximum IMS, but its competitiveness was on sagging tendency. Germany and the Netherlands are the major importers of licorice extract. Although they are lack of international competitiveness, its international competitiveness is on the rise, especially in Germany. China’s licorice extract has a high IMS, but its international trade competitiveness is weak, far lower than the United States and France. Japan was the basic export country of China’s licorice extract. The TC index and CA index of licorice extract in Japan are less than zero, which means Japan’s licorice extract is not competitive.

This paper mainly summarizes the development history, technological advantages, influencing factors, design principles, preparation methods of co-processed excipients, and summarizes the current situation of industrial development of co-processed excipients, which provides a reference for the in-depth study of composite excipients.

With the development of medicine, the pros and cons of both medical systems become increasingly evident. How to integrate them and learn from each other will be the direction of future development of medicine. The formation and development of integrated medicine will, inevitably, usher in a new era for medicine.

RESUMEN Introducción: Pacientes portadores de hipoacusia simétrica en la audiometría pueden percibir su déficit como asimétrico. La explicación de este fenómeno podría tener su origen en variaciones en la conducción del estimulo hablado hacia la corteza auditiva, o procesamiento auditivo. Objetivo: Evaluar el procesamiento auditivo en pacientes que refieren escuchar mejor por un oído en presencia de umbrales audiométricos simétricos. Material y método: Pacientes mayores de 65 años. Criterios de exclusión: patología otológica, deterioro cognitivo (Mini Mental test <21 puntos), usuario de audífono, hipoacusia asimétrica. La batería de procesamiento auditivo incluyó evaluación de comprensión del habla en competencia (habla en ruido), pruebas dicóticas (dígitos dicóticos y la versión en español de Staggered spondaic words, SSW), y evaluación temporal (patrón de frecuencia y discriminación de silencio). Todas estas pruebas estaban previamente validadas en población chilena. Análisis estadístico mediante programa SPSS. Se utilizaron estadísticos descriptivos y pruebas no paramétricas (Kruskal Wallis y Mann Whitney). p <.05. Protocolo aprobado por comité de ética de Investigación, pacientes consienten. Trabajo financiado por Concurso de Proyectos de Investigación de la SOCHIORL y Proyecto Anillo ACT1403. Resultados: 49 pacientes cumplen criterios de ingreso, 37 son de sexo femenino. Se identifican tres grupos: escuchan mejor por oído derecho (n: 19), por oído izquierdo (n: 14), o sin diferencia entre ambos oídos (n: 16). El grupo de pacientes con audición simétrica tenia PTP significativamente menor que los otros dos grupos. Al analizar los distintos tests de procesamiento auditivo no hay diferencia entre los grupos. Al evaluar habla dicótica se observó que el oído derecho tuvo un significativo mejor rendimiento que el oído izquierdo para dígitos dicóticos y SSW (p .000 y .007 respectivamente, test de Mann Whitney) en todo el grupo de pacientes evaluados, sin diferencia al agrupar por percepción auditiva (p .835, test de Kruskal Wallis). Conclusión: Para el grupo de pacientes evaluados no se pudo demostrar una relación entre procesamiento auditivo y percepción asimétrica de la palabra.


Abstract Introduction: Presbycusis corresponds to a symmetrical loss in the audiogram. Nevertheless, some patients complaint that they hear better in one ear. Aim: The purpose of this work is to analyze if this asymmetrical perception relates with auditory processing. Material and method: Subjects older than 65 years of age. Exclusion criteria: ear pathology, cognitive decline (Mini Mental test with score <21 points), previous use of hearing aid, asymmetrical thresholds in audiometry. Auditory processing was evaluated with speech in noise test, dichotic tests (dichotic digits and the Spanish version of Staggered spondaic words, SSW), and temporal resolution (frequency pattern and gap in noise). Statistical analysis with SPSS. Descriptive tests, Kruskal Wallis and Mann Whitney, p <.05. Protocol approved by the local ethics committee. Subjects consented. Financial support by the Chilean Society of Otorhinolaryngology and Proyecto Anillo ACT1403. Results: 49 subjects were recruited, 37 female. 19 of them reported better hearing with the right ear, 14 with the left ear, and 16 no difference among ears. Those subjects with symmetrical perception had better PTP than the other two groups. The three groups had no difference in auditory processing evaluation. In the dichotic hearing assessment most patients had a significant better performance for the right ear both for dichotic digits and SSW, independent of the better ear. Conclusions: For the cases included there was no relation between speech perception and auditory processing. In dichotic tests the right ear had a significant better performance.

In modern society, the most popular surgery for correction of myopia and myopic astigmatism are small incision lenticule extraction ( SMILE ) and femtosecond assisted laser in situ keratomileusis ( FS-LASIK ) . FS-LASIK is widely accepted by myopic patients and corneal refractive surgeons for its excellent safety, efficacy, stability and predictability. With the use of femtosecond laser, SMILE makes the corneal refractive surgery enter a full femtosecond era and become one of the most popular refractive surgery in the world, which is a novel minimally invasive corneal refractive surgery and characterized by flap - free, minimally invasive, small incision, femtosecond. But there is still some controversy about visual acuity, stability of diopter, corneal high order aberration, contrast sensitivity, dry eye, corneal biomechanical property and complication after SMILE and FS-LASIK in correction of myopia. This article reviews the advantages and disadvantages after SMILE and FS-LASIK in correction of myopia and myopic astigmatism among all above mentioned aspects.

The number of medical studies being published is increasing exponentially, and clinicians must routinely process large amounts of new information. Moreover, the results of individual studies are often insufficient to provide confident answers, as their results are not consistently reproducible. A meta-analysis is a statistical method for combining the results of different studies on the same topic and it may resolve conflicts among studies. Meta-analysis is being used increasingly and plays an important role in medical research. This review introduces the basic concepts, steps, advantages, and caveats of meta-analysis, to help clinicians understand it in clinical practice and research. A major advantage of a meta-analysis is that it produces a precise estimate of the effect size, with considerably increased statistical power, which is important when the power of the primary study is limited because of a small sample size. A meta-analysis may yield conclusive results when individual studies are inconclusive. Furthermore, meta-analyses investigate the source of variation and different effects among subgroups. In summary, a meta-analysis is an objective, quantitative method that provides less biased estimates on a specific topic. Understanding how to conduct a meta-analysis aids clinicians in the process of making clinical decisions.

Abstract This study investigated the acquisition of a serial motor skill in individuals with Down syndrome with two levels of handicap, mild group (mean age = 14.5 years, SD = 2.3, 7 individuals) and moderate group (mean age = 15.2 years, SD = 3.2, 7 individuals). The task involved single-arm sequential movements to five. The measures to access performance were overall sequence error, reaction time, and total movement time. To evaluate action program, formation variability of sequencing and relative timing variability were considered. Although there was no clear practice effect, the results showed that the level of handicap led to different strategies to plan and control the actions. The moderate group presented a less stable action program expressed in the variability in sequencing and timing. Their longer reaction times also suggest a heavy demand on central processing in accord with the one-target advantage hypothesis and also due to memory deficits to select and plan movements.

To study the data of the trade of the ginseng products in the UN COMTRADE database and analyze the overall situation of the international trade of the ginseng products. China, South Korea, Canada, and the United States are not only the market for the production of ginseng products, but also the main consumer markets for Ginseng products. Hong Kong, China is dominated by entrepot trade and is an important consumer market for ginseng products as well. The four countries mentioned above and Hong Kong, China, play a decisive role in the international trade of ginseng products. Using trade specialization coefficient (TSC), international market share (IMS), revealed comparative advantage (RCA), and competitive advantage (CA), the trade status of the Ginseng products in the four countries from 2007 to 2015 was studied, and the situation of its trade competitiveness was analyzed. The results showed that the average values of three indicators of international trade competitiveness of Canada were ranked the first with the absolute superiority except the mean value of TSC, which was slightly lower than that of South Korea. The average of TSC indicators of South Korea was ranked first, the means of RCA and CA were ranked the second in the world. As for China, the three indexes were all ranked the third except for IMS, ranked the second. However, the four indicators in the United States were all ranked in the last place.

Background: Intra-abdominal abscess remains a significant cause of morbidity and mortality in the patient population. Despite therapeutic advances, it is a perplexing diagnostic and management challenges and remains a dreaded complication of surgery. Materials and methods: Forty eight patients underwent fifty one percutaneous drainage procedures in the Barnard Institute of Radiology at the General Hospital, Chennai. There were 44 men and 7 women. The youngest of the patients was 13 years old and the oldest 56 years old. Results: Of the 51 drainage, 35 were done under CT guidance and 16 under US guidance. Successful drainage of the collection with avoidance of operation was achieved in 43 of the 48 abscesses (89.5%). If the total number of drainages were considered, the success rate was 84.3% (43 out of 51). Conclusion: Percutaneous catheter drainage is an important treatment option for management of intra-abdominal abscesses. It is especially valuable in patients at high risk for general anesthesia. It is also an effective alternative to operative drainage in other healthier patients. Computed tomography is the imaging modality of choice for percutaneous abscess drainage.

La Alianza del Pacífico se define como una iniciativa de integración profunda que procura abordar temas complejos (no de índole exclusivamente comercial) y que se fundamenta en las motivaciones y aspiraciones de los países miembros y fundadores, que tienen como objetivo lograr: una eficiente apertura comercial, la incursión a nuevos mercados y la inserción en las Cadenas Globales de Valor (GCV). A pesar de que entre Colombia y cada uno de los países que hoy conforman la Alianza del Pacífico existen acuerdos y tratados comerciales de décadas atrás, una revisión de la dinámica comercial entre el país cafetero y este grupo evidencia: por una parte, el deterioro en el volumen de las exportaciones, y por otra, un aumento de las importaciones, principalmente con México, lo que ha generado una balanza comercial en déficit, y un rezago considerable de apertura comercial, medida en términos per cápita. No obstante, al profundizar en el análisis por medio de la aplicación de indicadores como el de Balassa, se determina una cierta estabilidad y eficiencia de Colombia al exportar hacia estos países, como también la existencia de un grupo de productos con una considerable ventaja comparativa, pero poco exportados por el país cafetero.


The Pacific Alliance is an initiative characterized by deeper integration that seeks to address complex issues (not those related exclusively to trade). The PA is founded on the motivations and aspirations of its founder countries. The PA aims to achieve efficient trade liberalization, entry into new markets and integration into Global Value Chains (GVC). Although the existing trade agreements between Colombia and each of the member countries date from several decades ago, a current review of trade dynamics between Colombia and all the other member countries highlights, on the one hand, the deterioration in the volume of exports, and on the other hand, an increase in imports, mainly from Mexico. This has generated a negative trade balance and a considerable lag in terms of openness to foreign trade, measured in per capita terms. However an in-depth analysis through the application of indicators such as Balassa, shows Colombia possesses a measure of stability and efficiency to export to these countries, as well as highlighting the existence of a group of products with considerable comparative advantage, but barely exported by Colombia.

Embora entre a Colômbia e cada um dos países que hoje compõem a Aliança do Pacífico, existem acordos comerciais e tratados de décadas atrás, uma revisão da dinâmica das trocas comerciais entre o país de café e este grupo evidência: por um lado, a deterioração o volume de exportações, e, por outro, o aumento das importações, principalmente do México, o que gerou um déficit da balança comercial e um atraso considerável de liberalização do comércio, medido em termos per capita. No entanto, uma análise mais aprofundada através da aplicação de indicadores, tais como Balassa, determina-se uma certa estabilidade e eficiência da Colômbia para exportar para esses países, bem como a existência de um grupo de produtos com considerável vantagem comparativa, mas pouco café exportado pelo país.

Several nanoformulations are successfully used for brain delivery which includes nanoparticles system (polymeric/solid lipid), liposomes, dendrimer‟s, nanoemulsions, nanosuspension and ligand mediated nanosystems.Nanoparticles are defined as particulate dispersions or solid particles drug carrier that may or may not be biodegradable. Several techniques are used for preparation of nanoparticles like Solvent Evaporation, Double Emulsification method, Emulsions - Diffusion Method, Nanoprecipitation, Coacervation method, Salting Out Method, Dialysis and Supercritical fluid technology. Nanoparticles are subjected to several evaluation parameters such as yield of nanoparticles, Drug Content / Surface entrapment / Drug entrapment, Particle Size and Zeta Potential , Surface Morphology, Polydispersity index, In-vitro release Study, Kinetic Study, Stability of nanoparticles.

Objective:To study the comparison of advantages of 3D and 2D laparoscope applied in clinical surgery.Methods: 42 cases were received retrospective analysis using 3D and 2D laparoscopic technology to complete abdominal surgery and were divided into 3D group and 2D group according to the methods of surgery. We observed the indexes for operation time, intraoperative blood loss, postoperative exhaust, catheter indwelling time, placement time of abdominal cavity drainage tube and postoperative hospitalization days, etc.Results: The two groups of patients completed smoothly laparoscopic surgery. There was no significant difference between 3D and 2D laparoscopic monitoring on indexes in the postoperative exhaust, catheter indwelling time, placement time of abdominal cavity drainage tube and postoperative hospitalization days. But the difference was statistically more significant in the operation time and intraoperative blood loss in 3D than that in 2D group (t=0.358,t=2.455;P<0.05).Conclusion:3D high definition amplification of three-dimensional surgical field similar with the traditional laparotomy is the future trend in the development of minimally invasive surgery. Compared with 2D, 3D technology can improve the overall level of the operation, with the features of easier and faster operation, smaller wound, quicker recovery, less postoperative complications, etc.

RESUMO

Most of the transfusion adult outpatients are old people with chronic disease, and theirsmall blood vessels become less elastic with age. They need long course and repeated localised puncture site. On the base of that fact, the author describes the 5.5gauge scalp acupuncture and 7 gauge scalp acupuncture about their use on the patients. The 5.5 gauge scalp acupuncture is smaller and shorter, so it has the advantage of high success ratio and it is easier to be fixed. Additionally, the transfusion speed is easy to be controlled, which reduces the ratios of transfusion reaction and adverse drug reactions. The pinhole is small and can be healed faster. The congestion around the puncture location is less than that of the 7 scalp acupuncture. It can alleviate the pain for patients and increase satisfaction. To sum up, the 5.5 gauge scalp acupuncture should be used widely as we allocate“people oriented”and“All for the patients”.

RESUMO

Andrology is an ancient branch of science which has gained a new development in the recent years and therefore has both traditional and modern characteristics. On the one hand, andrology keeps benefiting from the achievements of modern medicine and, on the other hand, it relies on the support of the theories of Traditional Chinese Medicine (TCM). An integration of Chinese and Western medical principles may further facilitate the development of andrology. TCM may contribute to the development of andrology by giving full play to its advantage as a psychosomatic medicine, enriching treatment strategies for male diseases with comprehensive TCM therapies, integrating the advantages of Western medicine to improve clinical efficacy, and normalizing the use of patent TCM drugs.
