RESUMO
A partir da mobilização da classe médica, foi idealizado, no Rio de Janeiro, o Programa de Atendimento ao Paciente Fora de Possibilidades Terapêuticas Atuais (Pro- FPTA) pela médica Magda Rezende, a qual pertencia, então, a extinta Campanha Nacional de Combate ao Câncer (CNCC), onde coordenava o Programa de Oncologia (Pro-Onco). O Pro-FPTA implantou uma modalidade de serviço no Hospital de Oncologia (HO), que é a atual unidade II do Instituto Nacional de Câncer (INCA), sendo este serviço o embrião do antigo Centro de Suporte Terapêutico Oncológico CSTO). Foi objeto de estudo, na presente issertação, a participação da enfermeira no processo de implantação deste Centro, que atualmente constitui a unidade IV do INCA. Tornou-se problema de pesquisa compreender a atuação da enfermeira inserida nocontexto dos movimentos que deram origem ao CSTO, uma vez que a assistência ao FPTA ou paciente terminal, é parte fundamental do trabalho de enfermagem. Nortearam este estudo, as seguintes questões: Que jogos de forças permearam a plantação do CSTO no INCA? Como foi configurado o espaço das enfermeiras no INCA no processo de implantação do CSTO? Foram fixados os seguintes objetivos: 1- Descrever as circunstâncias do processo de implantação do CSTO no INCA; 2 Analisar as lutas simbólicas das enfermeiras no processo de implantação do CSTO; 3 Discutir a configuração do espaço real e relacional das enfermeiras durante o processo de implantação do CSTO no INCA.Tratou-se de um estudo qualitativo, de cunho históricosocial e está inserido no núcleo de Pesquisa da História da Enfermagem Brasileira (NUPHEBRAS) da Escola de nfermagem Anna Nery, da Universidade Federal do Rio de Janeiro (UFRJ). Foi utilizado como coleta de dados a técnica da História Oral em entrevistas junto aos sujeitos participantes do processo de implantação do CSTO e a um outro que integrou a Organização Mundial de Saúde como membro representante do Brasil no programa de controle da dor do câncer.O referencial teórico foi Pierre Bourdieu, sociólogo francês que utilizou o simbolismo para decifrar os significados das relações sociais. As circunstâncias de mplantação do CSTO reuniram fatos políticos,sociais e institucionais. Mas teve como fio condutor a busca de soluções para a melhoria local da prática-assistencial aos clientes com câncer avançado através da impulsão da assistência domiciliar como estratégia de desvio dessa clientela das antigas casas de apoio que prestavam serviços adequados ferindo princípios de cidadania. O estudo concluiu que as enfermeiras configuraram espaço real e relacional nas oportunidades que emergiram dos movimentos da classe médica, desempenhando atividades que diziam respeito às práticas operacionais, porém sem a participação na gênese de decisões.
From the mobilization of the medical class, was idealized, at Rio de Janeiro, the Programa de Atendimento ao Paciente Fora de Possibilidades Terapêuticas Atuais (ProFPTA) ("Program of Attention to the Patient Out of Present Therapeutic Possibilities") by the physician Magda Rezende, who pertained, then, to the extinct Campanha Nacional de Combate ao Cancer (CNCC) ("National Campaign of Cancer Combat, where coordinated the Programa de Oncologia (Pro-Onco) ("Program of Oncology"). The Pro-FPTA implanted a modality of service in the Hospital de Oncologia (HO) ("Hospital of Oncology"), that is the present II unit of the Instituto Nacional de Cancer (INCA) ("National Cancer Institute"), being this service the o embryo of the old Centro de Suporte Terapêutico Oncológico (CSTO) ("Oncological Therapeutic Support Center"). It was object of the study, in the present dissertation, thc nurse's participation in the implantation process of this Center, that at present constitutes the lV.Unit of the INCA. It became problem of the research comprehend the nurse's actuation inserted in the context of the movements which originate the CSTO, since the assistance to the FPT A or terminal patient, is part fundamental of the work of nursing. There guided this study, the following questions: Which power gamcs permeated the implantation process of the CSTO in the INCA? How was represented the nursc's space in the INCA in the implantation process of the CSTO? There were established the following objectives:: 1- Describe the implantation process circumstances of the CSTO in the INCA; 2 - Analize the symbolic fights of the nurses in the implantation process of the CSTO; J - Discuss the configuration of thc nurse's actual and relational space during the implantation process of the CSTO at the INCA. It was a question of a qualitative study, of historical - social nature and is inserted in the Pesquisa tia História da Enfermagem Brasileira (NUPHEBRAS) nucleus ("Brazilian Nursing History Research") of the School of Nursing Anna Nery, of thc Universidade Federal do Rio de Janeiro (UFRJ) ("Federal University of the Rio de Janeiro"). It was used as data collection the Oral History in interviews near to the subjects participants of the CSTO implantation process and another one who integrated the Organização Mundial de Saúde ("World Health Organization ")member represcntative of the Brazil in the cancer pain control program. The theoretical reference was Pierre Bourdieu, French sociologist who used the symbolism to understand the social relations meanings. The circumstances of the CSTO implantation joined political facts, social and institutional. But it had as connecting thread the search of solutions for the local improvement of the assistance practice to the clients with advanced cancer through the domicile assistance impulsion as these clientele's deviation strategy of the old hospices that did inadequate services reaching citizenship principles. The study concluded that the nurses represented actual and relational space in the opportunities that emerged thm1 the medical class movements, carrying out activities that concerned the operational practices, but without the participation in the decisions origin.
