RESUMO

@#Prosthetic rehabilitation offers an alternative to surgical procedures for repositioning implant-retained auricular prosthesis. This report illustrates a prosthetic rehabilitation of a 15-year- old male with a unilateral microtia who presented with an unfavorable implant-retained left auricular prosthesis. The implants placed seven years ago were drifted posterosuperior away from the ideal ear canal position. The fabrication of a new prosthesis on the existing bar-clip attachment using a custom acrylic base was planned to correct the location. A skin-colour perforated custom acrylic base was fabricated and designed to extend anteriorly, therefore, shifting the prosthesis forwards into a more natural location. The custom base was able to relocate the prosthesis’ position without compromising its retention whilst engaging the existing implant attachment. This in turn enhanced prosthesis acceptability and improved the patient’s confidence. The custom acrylic base serves as a viable option to reposition the prosthesis influenced by age related growth and development.

PURPOSE: Zirconia materials have been used for implant-retained restorations, but the stress distribution of zirconia is not entirely clear. The aim of this study is to evaluate the stress distribution and risky areas caused by the different design of zirconia restorations on the atrophic bone of the posterior maxilla. MATERIALS AND METHODS: An edentulous D4-type bone model was prepared from radiography of an atrophic posterior maxilla. Monolithic zirconia and zirconia-fused porcelain implant-retained restorations were designed as splinted or non-splinted. 300-N occlusal forces were applied obliquely. Stress analyses were performed using a 3D FEA program. RESULTS: According to stress analysis, the bone between the 1) molar implant and the 2) premolar in the non-splinted monolithic zirconia restoration model was stated as the riskiest area. Similarly, the maximum von Mises stress value was detected on the bone of the non-splinted monolithic zirconia models. CONCLUSION: Splinting of implant-retained restorations can be more critical for monolithic zirconia than zirconia fused to porcelain for the longevity of the bone.

En este artículo espero poder hacer llegar al lector a la comprensión de la evolución protética implanto-asistida, desde el mejoramiento de la retención otorgada a la prótesis completa inferior (PCI) con solo 2 implantes, hasta la rehabilitación totalmente implanto-asistida en paciente edéntulos. Para lograr este objetivo tan ambicioso tendremos que valernos de varios artículos en diferentes números de revista del AAO. En este artículo continuaremos describiendo las resoluciones protéticas para los siguientes casos: A- Sobredentaduras retenidas sobre dos implantes con Ball Attach. B- Sobredentaduras retenidas por Barras utilizando dos implantes. C- Soluciones protéticas para casos con implantes en mal posición. D- Casos en que se agrega un implante por haber dos implantes en mal posición (AU)


The goal of this article is to show the evolution of implant retained prosthesis driven by enhancements on retention of complete lower jaw prosthesis in edentulous patients. This could be reached by a different array of options, ranging from a simple one such as two implants retaining the lower jaw complete prosthesis, to a complex one completely supported and retained by implants prosthesis. Therefore, to reach such an ambitious goal, it will be necessary to devote several AAO series of journal´s articles to describe all these possibilities. In this paper we will continue describing the four following prosthetic cases: A- Ball Attachment retained Over Denture using two implants. B- Bar retained Over Dentures using two implants. C- Prosthetic solution for cases of misplaced implants. D - Adding a third implant for cases of two misplaced implants (AU)

Clinicians are faced with many difficulties when planning prosthodontic restorations with implants in a complete edentulous patient. When planning fixed implant prosthetics, it is often necessary to have additional surgery due to highly reduced alveolar bone, as well as high treatment costs and long-term treatment durations can be required. In addition, lack of interocclusal space can be a problem when planning implant supported overdentures. In this study, we planned to place a small number of implants on the anterior maxilla and used them as the abutments for distal-extension removable partial dentures on the posterior side in a maxillary fully edentulous patient. This would reduce the possibility of additional invasive operations such as alveolar bone graft, shorten the treatment time, and be relatively easy for elderly patients to burden. In this case, the patient was provided with a distal-extension removable partial denture and anterior implant fixed prostheses, which was similar to the previous one, and showed good adaptation, and chewing efficiency and esthetics was recovered.

RESUMO

Overdenture using dental implants could improve the problems of conventional complete denture function which are pain during mastication, insufficient retention and stability. Locator attachment used widely for implant-retained overdenture has advantages that it needs the smallest vertical space and also its nylon male cap allows personalized retention for each case. However its retention force decreases rapidly with function rather than the bar and ball attachment. So, implant fixture should be positioned as parallel as possible.

Objective:To evaluate the clinical outcomes of implant-retained overdentures.Methods:57 patients treated by implant-retained overdentures were included.Parameters for peri-implant tissue conditions (e.g.peri-implant probing depth,plaque index,bleeding on probing,mucosal hyperplasia,peri-implant marginal bone loss) and prosthetic complications were examined and recorded.The precentage of satisfaction of the patients was assessed using the visual analog scale (VAS).Results:After an average follow-up of (48±11.3) months,the survival rate of the implants was 98.1%,the marginal bone loss was (1.38±0.74) mm.There was no statistically difference among the different attachment groups(bar,magnet and ball) regarding the peri-implant marginal bone loss or bleeding on probing(P>0.05).The peri-implant probing depth and plaque index in patients with magnet and ball attachments were lower than those in patients with bar attachments(P0.05),except that magnet and ball attachments were much easier to clean compared with bar attachments(P<0.05).Conclusion:Implant-retained overdenture is a successful and satisfactory treatment option for patients with edentulous jaw.The patients should been given regular clinical examinations to keep peri-implant tissue health and reduce the complications,especially those with bar attachments.

RESUMO

Treatment options for edentulous patients are complete denture and implant prosthesis. A two implant-retained overdenture can be considered the first treatment in the edentulous mandible, but there is no clear consensus of treatment for edentulous maxilla. Implant-retention/support overdenture shows better retention and stability than complete denture and is less expensive and more esthetic than implant-supported fixed prosthesis. CM LOC® Pekkton® attachment is a solitary type attachment and evaluated to have excellent abrasion resistance and retention with a female part made of poly-ether-ketone-ketone. Meanwhile, SR Ivocap system is injection molding method and discussed to show few changes in the vertical dimension of denture and have excellent fracture resistance. In this case, we restored maxillary arch with a four implant-retained overdenture using CM LOC® Pekkton® and SR Ivocap system, and mandibular arch with a removable partial denture. Through this procedure, satisfactory outcomes were achieved both in functional and esthetic aspects.

RESUMO

Orbital defects can result from cancer, birth anomalies, or trauma leading to an onslaught of problems in the function and psyche of the patient. These defects are restored by surgical reconstruction and followed by placement of orbital prosthesis for cosmetic makeup. The use of dental implants in retaining orbital prosthesis improves patient acceptance of the prosthesis owing to better retention and stability than conventional adhesive retained prosthesis. This case report describes a custom‑made magnetic retentive assembly anchored by a dental implant which offers the orbital prosthesis the simplicity of self‑alignment and ease of use.

RESUMO

Conventional removable partial dentures (RPDs) with distal extensions are associated with some problems, including lack of stability that calls for frequent relining, and cantilever actions of claps that can produce excessive loading to abutment teeth, and the need for unesthetic retentive arm clasps. Therefore, IARPDs (Implant-assisted RPD) that use implants to support or retain RPDs has been reported to improve stability, esthetics and masticatory performance of RPDs. Also, an IARPD that has zirconia occlusal table can prevent the incongruity of occlusal plane and the extrusion of antagonistic tooth. In this case of partially edentulous patient with crossed occlusion, each edentulous area was restored with implant fixed prosthesis and implant retained partial denture to suit each situation. Through the procedure, satisfactory outcomes were achieved both in functional and esthetic aspects.

Objetivo: avaliar e comparar o nível de satisfação de pacientes que receberam próteses totais convencionais (PTC) ou overdentures implantorretidas (OIR) estabilizadas com attachment bola na região mandibular. Material e Métodos: aplicação de questionário composto por fatores de satisfação (adaptação, retenção, mastigação, fonética, conforto, condições de higienização, estética e autoestima) e por níveis de satisfação (insatisfeito, satisfeito e muito satisfeito), em 124 pacientes com idade média de 55 anos, divididos em dois grupos: grupo PTC (n = 60) e grupo OIR (n = 64). Resultados: os dados obtidos foram analisados através de análise estatística descritiva e inferencial (teste de Mann-Whitney) e mostraram que os pacientes que receberam OIR demonstraram maior satisfação em todos os fatores avaliados (p < 0,05). Conclusão: a OIR proporciona maior satisfação aos pacientes quando comparada com a PTC; no entanto, há necessidade de mais estudos comparativos em relação a seleção das amostras e aplicação de outros instrumentos de avaliação


Objective: the aim of this study was to evaluate and compare satisfaction levels of patients receiving conventional dentures (CDS) or implant-retained mandibular overdentures (IROVD) with ball attachments. Material and Methods: one-hundred and twenty four patients (mean age 55 years) were divided into two groups: CD (n = 60) and IROVD (n = 64). A questionnaire consisting of several factors (adaptation, retention, chewing, phonetics, comfort, oral hygiene, esthetics, and self-esteem) and levels of satisfaction (unsatisfied, satisfied, and very satisfied) was applied. Descriptive and inferential statistics (Mann-Whitney) were performed. Results: IROVD patients demonstrated greater satisfaction for all factors evaluated (p < 0.05). Conclusion: the IROVD provides better results than CDS. However, there is a need for more comparative studies regarding sample selection and the use of other evaluation instruments.

RESUMO

Full mouth rehabilitation always claims careful attention and meticulous treatment planning. It becomes more challenging with the partial edentulism where bilateral segment is found missing. Successful restoration can be done with plenty of contemporary and conventional treatment planning. Attachment retained partial dentures are one such kind of treatment modality in prosthodontics. Studies have also demonstrated that precision-attachment partials last longer, wear less, need less adjustments, look better, work better, less destructive, protect abutment teeth, and are easier to clean. In this case report patient’s functional and aesthetic expectations were met by doing full mouth rehabilitation using combination of Porcelain fused to metal (PFM) restorations and semiprecision attachments.

RESUMO

Over the past decades, conventional complete dentures were used for various patients although they have incomplete function. Overdenture using dental implants could help the improvement of denture function. In this case, a fully edentulous patient was treated with a conventional complete denture on the maxilla and implant retained overdenture retained by 2 freestanding implants placed in the anterior region of the mandible, following recommendations from the 2002 McGill. Locator attachments were used for retention of the mandibular overdenture. Patient was satisfied with the final maxillary full denture and mandibular implant retained overdenture in esthetic and functional aspect.

PURPOSE: This study compared the marginal bone loss around dental implant that were placed in the canine areas of the mandibles and finded the survival rate of implants, marginal bone loss around implants and prosthetic complications in 10 patients treated with overdentures retained with Locator attachments. MATERIALS AND METHODS: Ten patients who had received implant retained overdentures in the mandibules using two implants and Locator attachments at Daegu Catholic University Medical Center from 2004 to 2010 were included in this study. Evaluations of the survival rate of implants, marginal bone loss and prosthetic complications were performed. RESULTS: Implants placed in this study showed a 100% survival rate and the average annual bone loss was 1.03 mm +/- 0.20 mm in the first year. The patients have verbally indicated that they are comfortable and that their overdentures function well. But, implant retained overdentures had various prosthetic complications such as male change, relining, rebasing and denture fracture. CONCLUSION: Implant retained overdentures using two implant and Locator attachments in the fully edentulous mandibles is considered as a stable way for long terms because of minimal invasive operation, simple prosthetic process and easy repair.

RESUMO

This article describes the immediate placement and loading of implants in the aesthetic zone using an implant-retained, fixed prosthesis with a modified design. One section of the implant prosthesis has cemented crowns and the other section is the conventional screw-retained. This combined approach significantly offsets the unsuitable implant position, alignment or angulation, while ensuring the easy retrievability, repair and maintenance of the prosthesis at the same time.

STATEMENT OF PROBLEM: Conventional mandibular complete denture lacks in support and stability for edentulous patients to use. PURPOSE: To enhance support and stability of mandibular complete denture, 2 implants were inserted on the inter-mental foramina space of edentulous patients. With 2 magnetic attachments on the inner side of the overdenture, we gave immediate loads and evaluated the stability of the implants, the responses of the supporting tissues, and the satisfaction of the patients. MATERIAL & METHODS: 6 edentulous patients (5 male, 1 female) each recieved 2 SLA implants in the inter-mental foramina region and implants were immediately loaded with overdenture with magnet attachments. To evaluate the implants stability and the peri-implant tissues, clinical exams, RFA tests and radiographic exams were preformed at the 1 week, 2wks, 6wks, 12wks, and 24wks. post-surgery. RESULTS: The mean surgery time was 45.7 +/- 7.7 mins., while the denture delivery time was 45.5 +/- 12.6 mins. Only 2 of 14 implants were failed. Survived implants that remained were clincally and radiographically stable. Mean ISQ values were relatively stable, showing 69.71+/-5.55, 69.00+/-9.48, 67.92+/-7.86, 67.92+/-9.58, 70.08+/-7.61, 71.92+/-6.43 at the 1 wk, 2 wks, 6 wks, 12 wks, 24 wks. follow up check. Crestal bone changes were -1.18mm+/-0.68mm, -1.35+/-0.69mm, -1.47+/-0.68 mm at the 6wks, 12wks, 24wks. follow up check. Bleeding on probing(BOP index) was not significant. CONCLUSION: Mandibular ovedentures with 2 magnetic attachments over two interforaminal implants on edentulous patients for immediate function is a recommendable novel treatment for edentulous patients which shows stability on the implants and supporting tissue.

STATEMENT OF PROBLEM: The proper materials of attachments for implant retained overdenture are unknown, such as the correlation between retention and abrasion, as well as the types of materials that are suitable for patrix and for matrix individually. PURPOSE OF THIS STUDY: The aim of this study was to select a proper clinical attachment system for a successful treatment as well as patient satisfaction. METHOD: Retention and abrasion of 14 commercial attachments were measured during 15,000 removes. RESULT: A retentive part (matrix) which requires elasticity has to be made of gold while the patrix part which does not require elasticity has to be made of titanium. This gold matrix / titanium patrix combination showed the most retentive force and the least retention loss.
