RESUMO

【Objective】 To investigate the predictive value of regional cerebral oxygen saturation (rScO2) monitoring during total aortic arch replacement and stent trunk surgery for perioperative neurocognitive disorders (PND) and changes in plasma S100β protein and neuron-specific enolase (NSE) concentrations and their relationship with PND. 【Methods】 Sixty-five Stanford type A aortic dissection patients who planned to undergo total aortic arch replacement and trunk stenting were selected. Their rScO2 values were monitored throughout the operation and recorded after induction (T1), the beginning of CPB (T2), during deep hypothermic circulatory arrest (T3), rewarming to 36℃(T4), CPB stop for 1 hour (T5), and post-operation (T6). After induction (Ta), rewarming to 36℃ (Tb),1 h (Tc), 6 h (Td) and 24 h (Te) after cessation of cardiopulmonary bypass, central venous blood was collected from patients, and the concentrations of S100β protein and NSE in plasma were detected by ELISA. The patients were divided into PND group and non-PND group by the evaluation of MMSE scale at time of before operation, on the day of extubation, and 7 days after operation. 【Results】 The incidence of PND was 44.6%. The rScO2 value at T2 was significantly lower than that at T1 (P<0.05). The rScO2 value of PND group at T3 and T6 was significantly lower than that at T1 and non-PND group (P<0.05). The mean value of rScO2 and the minimum value of rScO2 in PND group were significantly lower than those in non-PND group, while rScO2 %max in PND group was significantly higher than that in non-PND group (P<0.05). The intraoperative critical value of rScO2 %max was >9.89%, the area under curve (AUC) was 0.658 (95% CI: 0.525-0.791, P<0.05), and sensitivity and specificity were 48.3% and 75.0%, respectively. The concentrations of S100β protein and NSE protein in PND group were significantly higher than those in non-PND group at Tc and Td (P<0.01). Compared with Ta, the concentration of S100β protein in PND group was significantly increased at Tc and Td (P<0.001), and the concentration of NSE protein was significantly increased at Tb-Te (P<0.01). CPB time was an independent risk factor for PND. 【Conclusion】 The occurrence of PND after total arch replacement and stenting may be related to the decrease of rScO2 and the increase of S100β protein and NSE protein. Intraoperative rScO2 %max >9.89% can be a potential predictor of PND.

Abstract Learning to read transforms lives. Reading contributes to knowledge acquisition, cultural engagement, and success in the school. The unequal distribution of literacy skills in a society is associated with economic and social inequalities as a result, children with a poor foundation in literacy before entering formal schooling are more likely to struggle academically and to drop out of school. For these reasons, there has been an intense scientific interest for decades in understanding how children learn to read. It is well established that in the early stages of reading development, phonological awareness, letter name-sounds knowledge, and the naming speed are three independent longitudinal predictors of children's later word-reading skills in alphabetic-writing systems. Phonological awareness constitutes the ability to recognize and manipulate the sounds of their own language, meanwhile letter knowledge promotes the discovery of systematic relationships between writing and oral language. As early readers develop some level of phonological awareness and some level of letter knowledge, they can recognize written words through phonological recoding processes, in which graphemes are recoded as phonemes and assembled to pronounce words. In addition, rapid naming expresses the speed at which phonological information is accessed from a graphic label. Phonological processing and letter knowledge are powerfully affected by the experience, stimulation, and support that children receive before beginning formal education. Most children acquire these abilities relatively effortlessly during early childhood. However, there is a significant number of children in Latin America who experience difficulties in their pre-literacy skills development. This study examined the cognitive profiles of a total of 50 Uruguayan preschoolers from different socioeconomic backgrounds from two public schools in Montevideo, Uruguay. Twenty-six children from low-income households were compared to peers from middle-income. At the end of the pre-schooling period (time 1) receptive vocabulary, phonological awareness, letter name-sounds knowledge, and object naming speed tests were administered to children. One year later (time 2), word -reading of a subgroup of children was measured. Significant correlations were observed between all predictors at time 1; between predictors at time 1 and word reading at time 2; and between all measured abilities and socioeconomic status. Comparative analysis between children of different socioeconomic status showed that children growing up in poverty contexts performed more poorly than their peers from middle-income families in all the tests. Nonetheless, both groups performed poorly in word reading. Descriptive statistics indicated that, out of a total of 26 words, low SES children correctly read a total of 7 words per minute, and medium SES children a total of 14 words. Finally, regression analyses indicated that phonological awareness contributed 30 % variance in predicting the total score achieved in a reading-word test in children of low-income families, meanwhile letter name knowledge contributed 74 % variance in predicting the total score achieved in a reading-word test in their peers from middle-income families. In general terms, results of pre-reading skills and reading performance seem to indicate that children of different socioeconomic status use different word recognition strategies according to their level of letter-knowledge of and phonological processing. Discussion considers international literature pointing out that children who enter elementary school with limited reading-related skills are unlikely to be able to keep pace with their peers. These findings warn about the importance to elaborate systematic and high-quality educational proposals to try to reduce the gap in reading development for children from low-income families. Developing literacy and language skills before formal schooling sets a child up for success in school and life. Results also suggest the importance of analyzing the variables that affect reading development in populations that are not the majority described.

RESUMO

Objective To study the effects and side effects of different dose of rHu-EPO on the treatment of brain injury in premature infants. Methods The infants who had suffered fetal distress and first one minute of Apgar was under 7score after birth and were sent to NICU within 24h were studied. We totally collected 90 infants and divided into three groups randomly, including large dose group, small dose group and control group. The large and small group were injected hypodermicly with rHu-EPO 1000U/kg, 500U/kg, three times per week for three weeks, and the control group were given general treatment without rHu-EPO at same time. Before treatment, one week and three weeks after treatment, we collected concentration of NSE, S100B and skull ultrasound to assess the effects. Neonatal behavioral neurological assessment(NBNA) were performed twice before and at weeks of correct gestational age. To survey the side effects, we collected general information such as the incidence rate of ROP and hemangioma, AST/ALT/PLT/Urea /Cr and so on. Results After one-week treatment, the concentration of NSE and S100B were no significant difference(P > 0. 05) in the small dose group, but were statistically significant in the large dose group(P < 0. 05). After three-week treatment, the comparison of NSE、S100B in both groups was statistically significant(P < 0. 05). The head ultrasound comparison was of statistically significant in both group(P < 0. 05), and so as NBNA and head MRI. The frequency of blood transfusion was statistically significant in both group(P < 0. 05) compared with control group. Routine blood test including liver and kidney function showed that there was no significant difference before and after treatment(P > 0. 05). Conclusion The neuroprotective effect of rHu-EPO on brain injury in preterm infants is connected with its dose and period of treatment, it need high dose or long time to express neuroprotective function.

RESUMO

Objective To observe the efficacy difference between different needling manipulations in treating acute stroke with point-toward-point needling at scalp acupoints, for seeking the optimal treatment protocol. Method Ninety patients with acute stroke were randomized into a manual needling group, an electroacupuncture group, and a needle-retaining group, 30 cases each. In addition to the neurological treatment, the three groups were all given point-toward-point needling from Baihui (GV20) to Taiyang (EX-HN5) on the affected side, respectively with manual twisting, electroacupuncture and needle retaining methods. Before and after 14-day treatment, the Neurological Deficit Score (NDS), level of neuron specific enolase (NSE) and total efficacy were compared among the three groups. Result The NDS dropped significantly in the three groups after the intervention (P<0.05); the manual needling group showed the most significant improvement, better than the electroacupuncture group and needle retaining group. The level of NSE declined significantly in the three groups after the intervention (P<0.05); the decrease in the manual needling group was the most significant compared to that in the electroacupuncture group and needle retaining group. The total effective rate in the manual needling group (90.0%) was significantly higher than that in the electroacupuncture group and needle retaining group (P<0.05). Conclusion Point-toward-point needling at scalp acupoints with manual operation can effectively improve the neurological deficit in stroke patients, and the application of needling manipulations can further enhance the therapeutic efficacy, which is specific.

RESUMO

BACKGROUND: Neuron-specific enolase (NSE) is an enzyme specifically found in neurons and neuroendocrine tissue. It is a common marker for small cell lung cancer diagnosis and is also useful as a predictor of brain damage. This study evaluates the performance of Elecsys NSE (Roche Diagnostics, Switzerland), an electrochemiluminescent immunoassay. METHODS: The precision, linearity, limit of detection, and reference interval of the Elecsys NSE, as well as the correlation between Elecsys NSE and ELSA-NSE (Cis-Bio International, France) were evaluated in accordance with the Clinical Laboratory Standards Institute (CLSI) guidelines. PreciControl Tumor Marker (Roche Diagnostics), patient sera, and sera from healthy individuals were used for the analysis. RESULTS: The measured coefficient of variation for the assay was below 3%, and it demonstrated linearity from 0.20 to 234.5 ng/mL. The detection limit was 0.032 ng/mL and the reference interval ranged from 0.05 to 16.3 ng/mL. Compared with the ELSA-NSE assay, the correlation coefficient was 0.9128. CONCLUSIONS: The Elecsys assay showed suitable precision, linearity, limit of detection and reference range for clinical laboratory use; however, the correlation coefficient of Elecsys NSE as compared to ELSA-NSE was below 0.975. This result may be associated with the use of different monoclonal antibodies in the two different NSE assays. Elecsys NSE demonstrated a high sensitivity without the use of radioactive reagents; therefore, Elecsys NSE will be quite useful for NSE analysis in the clinical laboratory setting.

RESUMO

Objective To observe the effects of Najia method of midday-midnight point selection for acute ischemic stroke (AIS) model rats onthe contents of NSE and S100B protein in serum. Methods SPF SD male rats were chosen to establish the models by middle cerebral artery bolt method. Rats were divided into blank group, sham-operation group, model group, channel-point group, and Najia method group by random number table method. Blank group, sham-operation group, and model group were in the absence of treatment, while the channel-point group received acupuncture treatment according to differentiation syndrome. Najia method group used Najia method of midday-midnight point selection to conduct acupuncture treatment once a day. Improvement of neural function and cerebral infarction volume were observed. The contents of NSE and S100B protein in serum were detected. Results Compared with model group, neurological function score, infarct volume and infarct volume percentage, and the contents of NSE and S100B protein in serum decreased in Najia method group and channel-point group (P<0.05, P<0.01). The effects of Najia group were generally better than the channel-point group. Conclusion Najia method of midday-midnight point selection can decrease the content of NSE and S100B protein in serum of AIS model rats, so as to achieve the effects of neuroprotection and treatment.

RESUMO

Objective To investigate the relationship between the serum levels of leptin, neuron-specific enolase (NSE) and S-100B in patients of stroke and the incidence of post-stroke depression (PSD). Methods The clinical data of 121 cases of acute ischemic stroke, admitted to Zhengzhou Central Hospital affiliated to Zhengzhou University from Jun. 2010 to Dec. 2012, were retrospectively analyzed. After six months of follow-up 42 patients were diagnosed as suffering from PSD (Hamilton Depression Scale score ≥8). Another 42 participants with available matching data on onset time, age, gender and lesions of brain were selected. The serum samples were collected from all patients at time of discharge, and the concentrations of serum leptin, NSE and S-100B were analyzed by enzyme-linked immunosorbent assay (ELISA) kit. Correlation and efficiency of diagnosing PSD among them was validated by receptor operator curve (ROC). Results The concentration of serum leptin, NSE, and S-100B in PSD group (25.84±13.80, 2.59±1.48 and 25.03±8.24μg/L, respectively) was higher than that in the control group (8.67±6.17, 2.27±1.84 and 22.40±6.84μg/L, respectively). No obvious correlation was found between serum leptin and the NSE and S-100B in PSD patients. Based on the ROC curve, the area under the curve of serum leptin in PSD patients was 0.935 (95%CI 0.885-0.984), and the optimal cutoff value of serum leptin level was 16.17μg/L, which was an indicator for predicting of PSD with 81.0% sensitivity and 90.1% specificity. Conclusion Elevation of serum leptin level at admission was found to be associated with PSD, and it may act as a new marker for predicting the occurrence of PSD.

RESUMO

Objective: To verify possible relations between 5'-nucleotidase, xanthine oxidase to E3 small ubiquitin-like modifier-protein ligase non structural maintenance of chromosomes elements 2 in sera patients with kidney stones and to evaluate the possibility of a new biomarker for the evaluation of kidney damage. Methods: A sixty patients with known kidney stones who appeared the government health clinics in Kuantan-Pahang and fifty apparently healthy were taken as control group. The 5'- nucleotidase, xanthine oxidase and other biochemical parameters were measured by colorimetric tests. The serum NSMCE2 were measured by enzyme linked immunosorbent assay. Results: The mean serum xanthine oxidase [(39.98 ± 19.70) IU/L] and ecto-5'-nucleotidase activity (40.03 ± 9.53 IU/L) were significantly higher than the controls' levels of (18.04 ± 6.26) and (16.06 ± 4.61) IU/L respectively. There were 85.00% and 83.33%, of patients with kidney stones who had abnormal ecto-5'-nucleotidase activity and uric acid respectively while xanthine oxidase activity was less sensitive 58.33%. Conclusions: The present study suggests that the increase in serumof xanthine oxidase,ecto- 5'-nucleotidase activities E3 small ubiquitin-like modifier-protein ligase NSE2 concentration can be used as biomarkers for diagnosis of kidney damage in patients with kidney stone, also in developments of change DNA damage and inflammation disorders in these patients.
