RESUMO

Objective To evaluate the clinical value of serum carcinoembryonic antigen (CEA ) ,carbohy-drate antigens(CA)125 and CA19-9 in patients with lung cancer .Methods Eighty patients with lung cancer who were admitted to our hospital from 2010 to 2016 were selected ,57 cases of pulmonary benign lesion pa-tients were collected at the same period and 80 cases of healthy subjects with physical examination were select-ed as control .Serum CEA ,CA125 and CA19-9 levels were detected ,comparing the diagnostic value of 3 inde-xes separately and combined for lung cancer ,and analyzing the value of the combined detection of serum CEA , CA125 and CA19-9 in the evaluation of lung cancer .Results T he levels of serum CEA ,CA125 and CA19-9 in patients with lung cancer were significantly higher than those in benign lesion group and control group ,and data of the lung benign lesion group was higher than that of the control group ,and the difference was statisti-cally significant (P<0 .05) .The contents of CEA ,CA125 and CA19-9 increased gradually with the increase of TNM stage of lung cancer ,there were significant differences among stage Ⅰ ,Ⅱ and Ⅲ (P<0 .05) .The sensi-tivity of combined detection of the 3 indexes was significantly higher than those of separate detections (P<0 .05) ,but the specificity decreased ,the area under the receiver operating characteristic curve was significantly higher than that of the independent test .The effective rate of treatment in lung cancer group was 73 .75%(59/80) ,and the ineffective rate was 26 .25% (21/80) .The average of CEA ,CA125 and CA19-9 in the effec-tive group was significantly lower than that in the ineffective group ,the difference was statistically significant (P<0 .05) .The serological results of the patients with different curative effects (complete remission ,partial remission ,stability and progression) showed that the levels of CEA ,CA125 and CA19-9 were statistically sig-nificant (P<0 .05) ,and the better the therapeutic effect ,the levels of CEA ,CA125 and CA19-9 in the patients were higher ,and there was a statistically significant difference between the groups (P<0 .05) .The follow-up results showed that the levels of CEA ,CA125 and CA19-9 in the recurrent group were significantly higher than those in the non recurrent group (P<0 .05) ,and the level of CEA ,CA125 and CA19-9 in the tumor me-tastasis group was significantly higher than that in the non metastatic group (P<0 .05) .In the end ,55 cases were included in the 5 year survival rate ,and the survival rate was 38 .18% (21/55) .The serum CEA ,CA125 and CA19-9 levels in the survival group were significantly lower than those in the death group ,the difference was statistically significant (P<0 .05) .Conclusion Combined detection of serum CEA ,CA125 and CA19-9 can effectively diagnose lung cancer ,and has important application value for clinical efficacy ,recurrence and metastasis assessment .

Objective To explore the best cut-off value of serum human epididymis protein 4(HE4)in the diagnosis of ovarian cancer,and analyze the clinical significance of combined detection of HE4,carbohydrate antigen 125 (CA125)and D-dimer for early screening and diagnosis of ovarian cancer.Methods A total of 49 patients with ovarian cancer,63 patients with benign tumor and 47 healthy women were collected.The content of CA125,HE4 and D-dimer were detected by chemical luminescence immunoassay and turbidimetric inhibition immunoassay,respectively.Results The new cut-off value of HE4 was 54.31 pmol/L.The serum levels of HE4,CA125 and D-dimer in the ovarian cancer group were all significant higher than those in the ovarian benign tumor group and the healthy group(P<0.05).No significant difference between the healthy group and benign disease group was found in CA125 and D-dimer content(P=0.293,0.359),while a significant difference in HE4 level(P<0.01).The sensitivity and specificity of combined detection of the HE4+CA125 increased to 91.8%,85.4%,which was higher than that of the diagnosis detection of the three indicators respectively.The sensitivity and specificity of combined detection of the HE4 +CA125 +D-dimer were 95.0%,76.5%,which was higher than the combined detection of the HE4+CA125.Conclusion The combined detection of CA125+HE4+D-dimer could improve the sensitivity for early diagnosis and screening of ovarian cancer.

Objective To study the application of carcinoembryonic antigen (CEA),carbohydrate antigens19-9 (CA19-9) and carbohydrate antigens72-4 (CA72-4) in the diagnosis of gastric cancer.Methods A total of 78 patients with gastric cancer underwent treatment in our hospital from August,2007 to June,2011 were enrolled as the case group.A total of 78 patients with benign gastric diseases and 78 healthy residents of the same period and district were randomly selected as the benign group and healthy control group,respectively.Measurements of serum CA72-4,CA19-9,CEA of all the cases and control group were performed.And the measurements of the three groups were statistically compared.Results The Serum concentration of CA72-4,CA19-9,CEA were significantly higher than the other two groups [CEA:(27.56 ± 18.36) μg/L vs (2.44 ±0.97) μg/L vs (2.37 ± 1.25) μg/L,F =145.346,P <0.0001 ; CA19-9:(99.87 ±80.53) kU/L vs (18.21 ±10.36) kU/Lvs (17.48 ±9.66) kU/L,F=78.503,P<0.0001; CA72-4:(56.13 ±39.26)kU/L vs (5.77 ±2.95) kU/L vs (3.62 ±2.18) kU/L,F =133.892,P <0.0001].And the positive rate of the gastric cancer group were significantly higher than those of the other two groups [CEA:69.23%,10.26%and 7.69%; CA19-9:57.69%,23.07% and 20.51%; CA72-4:83.33%,10.26% and 8.97%].The combination of the 3 measurements obtained the highest accuracy of diagnosis,while for single measurement,CA72-4 had the highest accuracy.Conclusion CEA,CA19-9 and CA72-4 have a significant meaning in the early diagnosis of gastric cancer,which has high sensitivity and specificity.The detection rate of the gastric cancer will be improved if the test of CEA,CA19-9 and CA72-4 are combined.

Objective To investigate the expression of tumor-associated carbohydrate antigen sTn in breast neoplasm,and analyzed the association between the expression of sTn and size of tumor,age,menopause,axillary lymph node metastasis,clinical stage,pathologic grade and the level of estrogen receptor in breast carcinoma.Methods The monoclonal antibody TKH-2 was used to detect the immunohistochemical expression of sTn in 46 cases of breast carcinoma,4 breast adenoma and 2 gynecomastia.The level of estrogen receptor was detected by S-P method in 46 breast carcinoma.Results Immunohistochemical expression of sTn was not detected in breast adenoma and gynecomastia.The expression of sTn was found in 28 of 46 breast carcinoma(60.9%).The expression of sTn was significantly associated with axillary lymph node metastasis(P0.05).In 46 breast carcinoma,the positive quantity of estrogen receptor was 25(54.3%).Conclusion STn immunostaining appears to be a poor prognostic factor in patients with breast carcinoma.It is more useful in detecting the expression of sTn together with the level of estrogen receptor in breast carcinoma to investigate the prognosis of the patients.

OBJECTIVES:Long standing observation, which may relate either to the causes or the effects of UC, reveals that there is a pronounced alteration of mucin such as quantitative and qualitative abnormalities of mucin glycoprotein. But recently in situ hybridization technique showed no specific difference in the expression of apomucin mRNA in UC. Therefore we investigated whether abnormality of mucin was originated from defect in glycosylation. And we also tried to find differences in the expression of Tn and sTn antigens between Korean and Jewish patients with UC. METHODS: We performed the immunohistochemical staining using the monoclonal antibody of mucin carbohydrate antigens Tn and sTn in 19 patients with UC. RESULTS: Tn and sTn antigens were not expressed throughout the crypt and surface epithelium in normal colon but both of mucin carbohydrates antigens were well expressed in mild UC, Tn antigen was seen in the surface epithelium with perinuclear pattern and sTn antigen was shown not only in surface but also in crypt epithelium. In severe UC, Tn antigen was well expressed, but sTn antigen was not expressed. Tn antigen seemed to be ex-pressed more frequently than sTn antigen with severity of inflammation. These results were similar in Korean and Jewish patients with UC. CONCLUSION: These results suggest that inflammatory bowel disease has some deterioration in the step of glycosylation in the cytoplasm and there was no racial difference in the expression of Tn and sTn antigen in Korean and Jewish patients with UC.
