Introducción: la hipertensión arterial se encuentra dentro de las enfermedades crónicas más prevalentes en nuestro país, el consumo excesivo de sal podría ser un factor preponderante de esta situación, el conocer lo que piensa nuestra población sobre ello y que actitud y predisposición al cambio demuestra, aportaría información relevante para desarrollar estrategias de prevención. Objetivos: El objetivo fue comprender el significado, el valor y el alcance que los sujetos atribuyen al consumo de sal/sodio alimentario y sus actitudes y prácticas para el cambio en relación al consumo de sal/sodio en la alimentación. Materiales y métodos: se enmarcó en un proyecto cualitativo, que permitió indagar sobre los conocimientos y actitudes de las personas sobre el consumo de sal/ sodio alimentario, la adquisición de los productos a consumir y las condiciones para intentar un cambio de actitud y comportamiento. Resultados: fueron agrupados de acuerdo a las categorías y descriptores definidos según el problema de investigación. Conclusión: Esta investigación nos ha llevado a comprender mejor el pensamiento y la actitud hacia el consumo de sal de un grupo población muy importante, que son las mujeres, en quienes recae la tarea de comprar y cocinar para la familia. A través de la información obtenida se crea un abanico de posibilidades para intentar un cambio positivo y reforzar aquello que ya está encaminado.


Introduction: arterial hypertension is one of the most prevalent chronic diseases in our country, excessive salt consumption could be a major factor in this situation, knowing what our population thinks about it and the attitude and predisposition to change it, would provide relevant information to develop prevention strategies. Objectives: The objective was to understand the meaning, value and scope that the subjects attribute to the consumption of salt/sodium in the diet. Materials and methods: it was framed in a qualitative Project, which made it possible to inquire about people's knowledge and attitudes about the consumption of dietary salt/sodium, the acquisition of the products to consume and conditions to attempt a change in attitude and behavior. Results: they were grouped according to the categories and descriptors defined according to the research problem. Conclusion: this research has led us to better understand the thinking and attitude towards salt consumption of a very important population group, that are the woman, in whom falls the task of buying and cooking for the family. Through the information obtained, a range of possibilities is created to try a positive change and reinforce what is already underway.

Resumen La sal es necesaria para el buen funcionamiento del organismo, pero el aumento excesivo de su ingesta está produciendo incremento en la incidencia de hipertensión arterial (HTA), la que constituye el principal factor de riesgo de muerte en el mundo, y el segundo de discapacidad por enfermedad cardiaca y accidente cerebrovascular. El objetivo de este trabajo fue revisar, desde una perspectiva actual, las relaciones existentes entre el consumo de sal (CNS) y la salud pública, haciendo énfasis en la HTA, así como en las políticas existentes encaminadas a reducir su sobreuso. Múltiples estudios señalan que la reducción del CNS en la población es una de las medidas más eficaces. Sin embargo, la adición de sal durante el cocinado no es el único problema, ya que la mayor cantidad proviene de los alimentos preparados o precocinados. Por tanto, la lucha contra el consumo excesivo de sal debe concebirse como una actividad permanente de los servicios médicos, con el apoyo constante de las autoridades públicas y la colaboración de las industrias alimentarias, a través de reducir o eliminar la adición de sal de los preparados, siguiendo las recomendaciones científicas emitidas en los últimos años.


Abstract Salt is necessary for the proper functioning of the organism, but the excessive use of it in feeding has increased the incidence of arterial hypertension (AHT), which constitutes the main risk factor of death in the world, and the second of disability due to heart disease and brain strokes. The aim of this work was to review, from a current perspective, the relationships between salt consumption (SCN) and public health, with emphasis on AHT, as well as in the existing policies aimed at reducing its overuse. Multiple studies indicate that the reduction of SCN is one of the most effective measures. However, the addition of salt during cooking is not the only problem, since the greatest amount of salt comes from precooked foods. Therefore, the fight against excessive SCN should be conceived as a permanent activity of medical services, with the constant support of public authorities, and collaboration of food industries by reducing or eliminating salt addition from prepared meals, following the scientific recommendations issued in recent years.

Introducción: El consumo de sal es un problema de salud debido a que supera la necesidad fisiológica. La adición de sal a los alimentos es cada vez mayor en los diferentes grupos etarios, hábito que se adquiere en hogares y entorno universitario. Objetivo: Determinar los hábitos alimenticios asociados al consumo añadido de sal en estudiantes de la Facultad de Medicina, Universidad Nacional Mayor de San Marcos. Diseño: Estudio descriptivo, observacional y transversal. Lugar: Facultad de Medicina Universidad Nacional Mayor de San Marcos, Lima, Perú. Participantes: estudiantes universitarios del primero a cuarto año de cinco escuelas académico profesionales. Intervención: Muestra de 150 estudiantes de ambos sexos, muestreo por cuotas. Se aplicó encuesta de hábitos alimenticios relacionados al consumo añadido de sal. Los datos fueron analizados en SPSS vs. 21. Se aplicó estadística descriptiva de frecuencias y porcentaje. Resultados: La adición de sal fue igual y mayor a una pizca. Al agua de cocción de verduras, papas, pastas en 63 por ciento, a mezclas preparadas de alimentos: ensaladas, ají, salsas en 54 por ciento, y alimentos sobre la mesa: palta, huevo, carnes en 60 por ciento. Mientras que el hábito de adicionar sal a los alimentos después de probarlos y la comida servida antes de probarla fue de 29 por ciento y 8 por ciento respectivamente. Conclusiones: Los universitarios en su mayoría, adicionan de 1 a más pizca s de sal al agua de cocción de algunos alimentos, a mezclas preparadas y a los alimentos sobre la mesa, lo cual contribuye a una alta ingesta de sal.


Introduction: Salt intake is a health problem because it overcomes the physiological need. Adding salt to food is growing in different age groups, habit acquired in homes and university environment. Objective: To determine the eating habits related to the added salt intake students of the Faculty of Medicine. National University of San Marcos. Design: descriptive, observational and cross-sectional study. Place: Faculty of Medicine National University of San Marcos, Lima, Peru. Participants: University students of the first to fourth academic year of five professional schools. Intervention: Sample of 150 students of both sexes, quota sampling. Survey of eating habits related to the added salt intake was applied. Data were analyzed using SPSS statistical vs. 21. Frequency and descriptive percentage was applied. Results: The addition of salt was equal to and greater than a pinch. The cooking water of vegetables, potatoes, pasta by 63 per cent, to mixtures prepared food: salads, chili, sauces at 54 per cent and food on the table: avocado, egg, meat by 60 per cent. While the habit of adding salt to food after testing and before tasting food served was 29 per cent and 8 per cent respectively. Conclusions: The University mostly added 1 more pinches of salt to the cooking water of some foods, prepared mixes and food on the table, which contributes to high salt intake.

Las etapas del modelo conductual de las personas describen el cumplimiento o no de las recomendaciones para una vida sana. Por ello, se planteó una investigación observacional descriptiva cuyo objetivo fue evaluar las etapas del modelo conductual de los estilos de vida propuestos por las Guías Alimentarias del Paraguay en adultos que residen en el barrio Salvador del Mundo de Asunción. Las variables de interés fueron las etapas de cambio, medidas a partir del modelo de Prochaska, para el consumo de sal, ingesta de bebidas alcohólicas, actividad física y manipulación de alimentos, tres de ellas están relacionadas al no cumplimiento y dos al cumplimiento de las recomendaciones. El 92,9% (91/98) de los adultos no cumple con las recomendaciones de actividad física y el 71,4% (70/98) con las recomendaciones de consumo de sal. El 46,9% (46/98) cumple con la recomendación para el consumo de bebidas alcohólicas, aunque el 74,5% (73/98) refirió su consumo. Ningún encuestado cumple con las recomendaciones de lavado y manipulación de alimentos. Es preocupante el alto porcentaje de adultos que no cumple las recomendaciones, agravado por la no intención de cambio. Por lo tanto, se puede concluir que el objetivo inicial de las guías alimentarias del Paraguay de orientar a la población sobre los estilos de vida adecuados no se cumple. Debido a esto, se debe promover la revisión del medio de implementación de las mismas para alcanzar a un sector mayor de la población, y así lograr la adopción de mejores estilos de vida.


The stages of the behavioral model describe the compliance or not of therecommendations for a healthy life. Due to this, a descriptive observational study wasproposed to evaluate the stages of the behavioral model of the life styles proposed by theFood Guides of Paraguay in adults living in the Salvador del Mundo neighborhood ofAsunción. The variables were the stage of changes, measured from Prochaska’s model,for salt consumption, alcohol intake, physical activity and food handling, three of themrelated to noncompliance and two to compliance of the recommendations. The 92.9%(91/98) of the adults did not comply with the recommendations of physical activity and71.4% (70/98) with the recommendations of salt intake. The 46.9% (46/98) compliedwith the recommendation of alcohol intake although it was observed that 74.5% (73/98)consumed some. None of the survey respondents complied with the recommendations offood cleaning and handling. It is worrying the high percentage of adults that did notcomply with the recommendations of physical activity, worsened by the non-intention ofchanging. Therefore, it can be concluded that the initial objective of the food guides ofParaguay to orientate population about the adequate life styles is not complied. Due to this, the revision of the implementation means or system should be encouraged to reacha larger sector of the population and thus, comply with the adoption of better life styles.

The effects of selective (e.g. mineral supplement formulated on the basis of nutritional and clinical examination of the herd) or commercial mineral supplementation of crossbred dairy heifers (Holstein-Mantiqueira), on daily weight gain (DWG), body condition score (BCS), age at first mating (AFM) and the intake of mineral mixture (IMM) managed on Xaraés palissadgrass pasture (Brachiaria brizantha cv. Xaraés) were evaluated from February 2006 to March 2008. Structural characteristics, forage allowance and nutritional value of Xaraés palissadgrass were also evaluated. The structural variables of pasture, allowances and nutritive value of forage, besides the DWG were only affected (p<0.05) by season, with highest results for spring and summer. Throughout the experimental period (730 days), no sign of mineral deficiency was detected in heifers that ingested the selective supplement. The DWG and AFM were not influenced by the mineral mixture offered (0.52 and 0.33 kg/day during the spring/summer and in fall/winter for DWG, respectively, and 813 days of AFM). The IMM was higher for commercial mineral mixture than for selective (61.6 and 51.0g/day respectively). The BCS was slightly higher for selective (3.17) than for the commercial (3.02) mineral mixture. Thus, the selective mineral supplement was a strategy to prevent mineral deficiency in this herd and resulted in similar performance at lower costs (e.g. due to less IMM) of crossbred heifers when compared with the commercial mineral supplementation.


Foram avaliados os efeitos da suplementação mineral seletiva (i.e. aquela feita com base no exame clínico-nutricional do rebanho) ou comercial sobre o desempenho de novilhas leiteiras Holandês-Mantiqueira, com base em seus ganhos de peso diários (GMD), escores da condição corporal (ECC), idades à primeira cobertura (IPC) e consumos de mistura mineral (CMM), criadas em pastagem de capim-Xaraés (Brachiaria brizantha cv. Xaraés), no período de fevereiro de 2006 a março de 2008. Foram também avaliadas as características estruturais do pasto, oferta e valor nutritivo da forragem. Durante todo período experimental (730 dias), nenhum sinal de deficiência mineral foi detectado nas novilhas que ingeriram o suplemento seletivo. As variáveis estruturais do pasto, as ofertas e o valor nutritivo da forragem, além do GMD, somente foram influenciadas (p<0,05) pela estação do ano, com maiores e melhores resultados obtidos para a primavera e verão. O GMD e a IPC não foram influenciados pela mistura mineral ofertada (0,52 e 0,33 kg/dia durante a primavera/verão e outono/inverno para GMD, respectivamente, e 813 dias de IPC). O CMM foi maior para mistura mineral comercial que para seletiva (61,6 e 51,0g/UA/dia, respectivamente). O ECC foi superior para mistura mineral seletiva (3,17) que para comercial (3,02). Esta suplementação mineral seletiva foi uma estratégia que permitiu evitar o aparecimento de estados carenciais no rebanho e obter desempenho similar e com menor custo quando comparada com a suplementação mineral comercial.

En la actualidad, las enfermedades cardiovasculares son la primera causa de muerte en el mundo. La presión arterial elevada es uno de los factores más importantes para su desarrollo, la cual está fuertemente relacionada con el consumo de sal. A nivel mundial el consumo de sal actual sobrepasa en más del doble la ingesta recomendada, lo que se ha asociado con el desarrollo de enfermedades cardiovasculares y de algunos cánceres. Los efectos beneficiosos demostrados al reducir su consumo (menor morbi-mortalidad y menores gastos en salud) han servido de aliciente para desarrollar diversas estrategias nacionales para lograr este efecto. Dentro de las estrategias más utilizadas se encuentran campañas educativas y la disminución paulatina de la sal agregada en los alimentos industrializados. Chile, se ha sumado a estas iniciativas con un acuerdo entre los productores de pan y el Ministerio de Salud con el fin de disminuir progresivamente la concentración de sal en el pan a nivel nacional. El objetivo de esta revisión es aportar información actualizada sobre las recomendaciones de ingesta de sal, ingesta real, efectos nocivos del exceso de consumo, beneficios atribuidos a su disminución y analizar las estrategias globales para reducir el consumo de sal en la población.


Global strategies to reduce salt intake.


Currently, cardiovascular diseases (CVD) are the leading cause of death worldwide. High blood pressure is one of the main risk factors for the development of CVD and blood pressure levels are strongly associated with salt intake. Worldwide, salt consumptions accounts more than two fold the recommended daily intake, which has been described to be associated with CVD and some cancers. Benefits of decrease salt intake (reduction of morbidity, mortality and health related costs) have promoted several public health strategies to reduce salt consumption globally. Among the most commonly used strategies include educational campaigns and the gradual decrease of added salt in processed foods. Chile has joined these initiatives with an agreement between the producers of bread and the Ministry of Health to gradually decrease the concentration of salt in bread nationwide. The purpose of this review is to provide updated information regarding recommended intakes of salt, real intake, adverse effects of excess consumption, profits attributable to a decline and analyze the global strategies to reduce salt intake in the population.

Pediatric arterial hypertension (AH) is an underdiagnosed disease, with a known prevalence of 2-3 percent. Its preventive management should begin early and includes life-style changes and diet salt reduction to a maximum of 5.8 g (2.3 g of sodium), since there is a direct relationship between total salt intake and arterial blood pressure. It has been previously shown that in populations with low salt diet (less than 3g), AH is rare and it does not increase with age. It has been estimated that 77 percent of salt found in regular diets comes from processed food. Mechanisms involved in salt intake and high blood pressure are analyzed in this paper. Arterial hypertension secondary to renal diseases and other pathologies are excluded. Considering renal physiology, the role of the kidney is crucial in arterial blood pressure regulation, through the capacity to affect the salt and water excretion; therefore, controlling total blood volume. The relationship between salt and AH in the newborn and older children, as well as genetic aspects of this disease, are discussed. In conclusion, there are biological and behavioural risk factors that can be modified in young population. It is necessary to promote these changes through active, as well as passive, prevention strategies. A government public health policy including educational publicity campaigns, permanent media information and accessible health food labelling is essential. Habits, old customs and trends need to be changed through a multifactorial approach to groups, families and community. The pediatrician should lead this effort.


La hipertensión arterial (HA) en pediatría es una patología sub diagnosticada, con una prevalencia de alrededor de un 2 a 3 por ciento. Su prevención debe comenzar precozmente con indicaciones de estilos de vida saludables, en especial reducción de sodio a 2,3 g equivalentes a 5,8 g de cloruro de sodio diarios, dada la relación directa entre cantidad de sal de la dieta y la presión arterial. En poblaciones humanas con dietas conteniendo menos de 3 g de sal por día la HA es infrecuente y no aumenta con la edad. La mayor parte del sodio ingerido se aporta por los alimentos elaborados industrialmente: el 77 por ciento se obtiene de los alimentos procesados. En esta comunicación se revisan los mecanismos que relacionan las alzas tensiónales con la ingesta de sal. Se excluyen los mecanismos que producen HA en las enfermedades renales y otras formas de HA secundarias. Desde el punto de vista fisiológico el riñón tiene un papel primordial en la regulación de la presión arterial, entre otros por su capacidad de modificar la excreción de agua y sal y con ello regular el volumen de sangre circulante. Se analiza la relación sodio/HA en el recién nacido y en el niño mayor y algunos aspectos genéticos de esta enfermedad. Se concluye que hay factores de riesgos biológicos y conductuales en los jóvenes que son modificables. Es necesario promover cambios en estos grupos a través de estrategias de prevención poblacional tanto activas como pasivas. Esto requiere un compromiso de políticas públicas que incluyan campañas educativas, manejo de la publicidad y fácil identificación de los alimentos saludables. Deben intervenirse los hábitos, costumbres y tendencias a través de acciones con un enfoque multifactorial, familiar, grupal y comunitario, siendo el pediatra quien debe liderar este desafío.

O aumento da pressão arterial não é a única consequência da sobrecarga de sal na dieta. Independente dos efeitos hemodinâmicos, o excesso de sal pode induzir alterações estruturais no miocárdio. A avaliação dos mecanismos destas alterações foi o objetivo do presente estudo. Para tanto, ratos Wistar machos foram alimentados com dieta: normossódica (NR: 1,3% de NaCl), hipersódica 1 (HR1: 4%) e hipersódica 2 (HR2: 8%) desde o desmame até a 18a semana de idade. O grupo HR2 foi dividido em HR2, HR2+Hidralazina (HZ: 15mg/ kg/ dia) e HR2+Losartan (LOS: 20mg/ kg/ dia). As drogas foram administradas a partir da 7a semana de idade. Foram avaliados pressão arterial caudal (PAc), atividade de renina plasmática (ARP), aldosterona sérica, ecocardiograma, massa ventricular esquerda (MVE) e direita (MVD), medida do diâmetro transverso do miócito (DTM), fibrose intersticial (FI), expressão protéica do receptor de angiotensina II do tipo I (AT1) e tipo 2 (AT2), dosagem de angiotensina II (AII) e ligação do anticorpo que reconhece a conformação ativada dos receptores AT1 e AT2 no ventrículo esquerdo (VE) e direito (VD). A PAc foi maior no grupo HR1 e HR2 comparado com o grupo NR. A PAc do grupo HR2+HZ e HR2+LOS não diferiu do grupo NR. ARP e ALDO foram menores nos grupos HR1, HR2, HR2+HZ e HR2+LOS. A espessura do septo interventricular na diástole e da parede posterior do ventrículo esquerdo na diástole foram maiores nos grupos HR1, HR2, HR2+HZ e HR2+LOS comparado com o grupo NR. A MVE e MVD foram maiores nos grupos HR2, HR2+HZ e HR2+LOS comparado com o grupo NR. O DTM do VE foi maior nos grupos HR1, HR2, HR2+HZ comparado com o grupo NR e HR2+LOS. DTM do VD foi maior no grupo HR2 e HR2+HZ comparado com o grupo NR, HR1 e HR2+LOS. A FI no VE e VD foi maior nos grupos HR1, HR2 e HR2+LOS comparado com o grupo NR e HR2+HZ. A expressão da proteína do receptor AT1 no VE e VD foi maior nos animais do grupo HR2 e HR2+HZ quando comparado com o grupo NR, HR1 e HR2+LOS...


Increased blood pressure is not the only consequence of salt overload. Independently from the hemodynamic effect, salt excess may induce structural alterations in the myocardium. The aim of the present study was to evaluate the mechanisms of the myocardium structural alteration in response to high salt intake. Male Wistar rats were fed normal (NR: 1.3% NaCl), high 1 (HR1 4%) or high 2 (HR2 8%) salt diet since weaning until 18th week of age. HR2 group was divided in HR2, HR2+hydralazine (HZ: 15mg/ kg/ dia) and HR2+losartan (LOS: 20mg/ kg/ dia). Drugs were administered since the 7th week of age. Tail-cuff blood pressure (Tc-BP), plasma renin activity (PRA), serum aldosterone (ALDO), echocardiography, left (LV) and right (RV) ventricular mass, cardiomyocyte transverse diameter (CTD), interstitial fibrosis (IF), protein expression of AT1 and AT2 receptors, angiotensin II content (AII), binding of the conformational specific anti-AT1 and anti-AT2 antibody in both ventricles were determined in the LV and RV. Tc-BP was higher in the HR1 and HR2 groups when compared to NR. Tc-BP on HR2+HZ and HR2+LOS did not differ from NR. PRA and ALDO were lower in the HR1, HR2, HR2+HZ and HR2+LOS when compared to NR. Interventricular septal and left ventricular posterior wall thicknesses were higher on HR1, HR2, HR2+HZ and HR2+LOS compared to NR. LV and RV mass was higher in the HR2, HR2+HZ and HR2+LOS when compared to NR. CTD in the LV was higher on HR1, HR2 and HR2+HZ groups than on NR and HR2+LOS groups. CTD in the RV was higher in the HR2 and HR2+HZ when compared to NR, HR1 and HR2+LOS groups. IF was higher in the LV and RV in HR1, HR2 and HR2+LOS groups than in NR and HR2+HZ groups. AT1 protein expression was higher in the HR2 and HR2+HZ compared to NR, HR1 and HR2+LOS groups. High salt intake did not increase AT2 protein expression in the HR1, HR2 and HR2+HZ groups.
