RESUMO

AIM: To evaluate the efficacy of high-intensity focused ultrasound cyclo plasty(UCP)in the treatment of glaucoma and to investigate related influencing factors.METHODS: The study involved a total of 110 patients(134 eyes)who received UCP treatment between January 2019 and January 2022 at three medical centers: Tianjin Eye Aier Eye Hospital, Chongqing Aier Eye Hospital, and Chongqing Nanping Aier Eye Hospital. Patients were classified into three categories: primary angle-closure glaucoma, primary open-angle glaucoma, and secondary glaucoma. Best corrected visual acuity, intraocular pressure, and the usage of anti-glaucoma medications, etc., were recorded at 6 and 12 mo postoperatively.RESULTS: After 6 months of the UCP procedure, statistically significant differences in intraocular pressure were observed across all groups(all P<0.05). At 12 mo postoperatively, intraocular pressure of the primary angle-closure and primary open-angle glaucoma groups showed differences(all P<0.05). Notably, there were no significant changes in visual acuity at either the 6 or 12 mo compared to preoperative values across all patient groups(all P>0.05). Furthermore, the study identified a statistically significant correlation between the changes in intraocular pressure at 6 mo and factors such as age, history of previous glaucoma surgery, baseline white-to-white(corneal diameter), and the extent of UCP treatment(all P<0.05).CONCLUSION: The UCP procedure has been demonstrated to effectively lower intraocular pressure in patients with glaucoma. The efficacy appears to be influenced by variables including patient age, previous glaucoma surgery history, baseline white-to-white(corneal diameter), and the scope of UCP treatment. Importantly, UCP treatment did not adversely affect visual acuity, underscoring its favorable safety profile.

RESUMO

ObjectiveTo retrospectively analyze the effect of modified Shugan Dingji Decoction (疏肝定悸汤) on the occurrence of endpoint events in patients with paroxysmal atrial fibrillation of liver constraint and qi stagnation. MethodsA retrospective cohort study was conducted using the electronic medical record database of Longhua Hospital affiliated to Shanghai University of Traditional Chinese Medicine to screen and include patients with paroxysmal atrial fibrillation of liver constraint and qi stagnation from January 1st, 2018, to December 31th, 2021. The included patients were divided into an exposure group and a non-exposure group, each consisting of 100 cases, based on whether they received modified Shugan Dingji Decoction. General information of the patients including age, gender, body mass index, duration of illness and comorbidities, medication history, cardiac structure and function indicators such as left atrial diameter, left ventricular end-diastolic diameter, stroke volume and ejection fraction, and the occurrence of endpoint events assessed through 24-hour dynamic electrocardiography or electrocardiogram to determine the recurrence of paroxysmal atrial fibrillation were collected. Kaplan-Meier (K-M) curves and Log-Rank tests were used to conduct survival analysis on the occurrence of endpoint events in the two groups of patients. Univariate and multivariate Cox regression analyses were used to analyze the impact of various factors on entry into endpoint events. Additionally, a safety assessment was performed by comparing liver and kidney function indicators before and after treatment. ResultsIn the non-exposure group, a total of 49 cases (49.0%) experienced endpoint events, while in the exposure group, there were 26 cases (26.0%). The Log-rank test indicated significant difference between the two groups (χ2=11.211, P=0.001). Univariate Cox regression analysis showed that age, duration of illness, hypertension, diabetes, chronic heart failure, left atrial diameter, stroke volume, and the use of modified Shugan Dingji Decoction may be the influencing factors for the occurrence of endpoint events in patients with paroxysmal atrial fibrillation of liver constraint and qi stagnation (P<0.05 or P<0.01). Multivariate Cox regression analysis showed that the risk of endpoint events in the exposure group was significantly lower than that in the non-exposure group (P<0.01). Patients with a duration of illness >12 months had a significantly higher risk of endpoint events compared to those with a duration of illness ≤12 months (P<0.01). Patients without concomitant hypertension had a lower risk of endpoint events compared to those with hypertension (P<0.05). Patients with left atrial diameter >40 mm had significantly higher risk of endpoint events than those with left atrial diameter ≤40 mm (P<0.01). There was no statistically significant difference in liver and kidney function indicators between the two groups before and after treatment (P>0.05). ConclusionThe use of modified Shugan Dingji Decoction is a protective factor for patients with paroxysmal atrial fibrillation of liver constraint and qi stagnation, which can help to reduce the recurrence and progression of atrial fibrillation. Long duration of illness, concomitant hypertension, and enlarged left atrial diameter are risk factors for patients to experience endpoint events.

RESUMO

【Objective】 To analyze the risk factors of postpartum stress urinary incontinence (SUI) and to establish a nomogram model. 【Methods】 A total of 278 puerpera who gave birth at our hospital during Dec.2018 and Aug.2020 were selected as the modeling group, and 132 puerpera who gave birth during Sep.2020 and Sep.2021 were involved in the verification group. Factors affecting postpartum SUI were identified with univariate and multivariate logistic regression, and a nomogram prediction model was constructed with R software. The predictive effectiveness and discrimination of the model were assessed, and the decision curve analysis (DCA) was drawn to evaluate the clinical application value of the model. 【Results】 A total of 84 cases (30.22%) in the modeling group developed SUI 2 months after delivery. Fetal weight, delivery method, maternal age, mobility (Δhy) and rotation Angle (Δβ) were factors affecting postpartum SUI (P<0.05). Multivariate logistic regression analysis showed that increased fetal weight, normal delivery, increased Δhy, and increased Δβ were independent risk factors of postpartum SUI (P<0.05). The constructed nomogram fitted well. The H-L fit curve of the modeling group and verification group were (χ2=7.514, P=0.312) and (χ2=6.157, P=0.267), respectively. The area under the receiver operating characteristic curve of the modeling group and verification group were 0.815 and 0.760, respectively, indicating high specificity and consistency. DCA indicated that when the high-risk threshold probability of the model was between 0.06-0.80, the nomogram model had a high clinical value. 【Conclusion】 Increased fetal weight, normal delivery, increased Δhy and elevated Δβ are independent risk factors that affect postpartum SUI. The nomogram model constructed has good predictive effectiveness and discrimination, and high clinical application value.

RESUMO

BackgroundPatients with schizophrenia are at high risk of suffering from metabolic syndrome. Most previous studies on the influencing factors of metabolic syndrome focused on the inpatients and limited ones on patients dwelling in community. ObjectiveTo explore the influencing factors at different risk levels of metabolic syndrome in community-dwelling patients with schizophrenia in Guangzhou, so as to provide references for future interventions on metabolic syndrome in this patient population. MethodsIn November 2021, 3 339 patients with schizophrenia who were registered in and administered by Guangzhou Mental Health Information System were included. All these patients had finished the physical examination in 2020, and whether they had metabolic syndrome was assessed basing on Guideline for the prevention and treatment of type 2 diabetes mellitus in China (2020 edition). Patients were divided into high-risk group (n=423), critical group (n=1 524) and metabolic syndrome group (n=1 392) according to the Chinese expert consensus on the management of metabolic syndrome in patients with schizophrenia. Multiple logistic regression analysis were performed on the risk factors of metabolic syndrome in community-dwelling patients with schizophrenia. ResultsThe prevalence rate of metabolic syndrome in community-dwelling patients with schizophrenia was 41.69%. Univariate analysis showed that the results in gender (χ2=44.610), age (χ2=55.992), marriage status (χ2=30.755), illness course (χ2=25.913) and body mass index (χ2=829.265) were significantly different among the three groups (P<0.01). Kruskal-Wallis H test showed that the levels of waist circumference (H=920.331), systolic blood pressure (H=436.673), diastolic blood pressure (H=393.337), fasting blood glucose (H=807.304), triglyceride (H=1 134.125) and high-density lipoprotein cholesterol (H=593.615) among the three groups were significantly different (P<0.01). Logistic regression analysis showed that age ≥50 (OR=1.761, 95% CI: 1.087~2.853), overweight (OR=2.418, 95% CI: 1.862~3.140) and obesity (OR=57.903, 95% CI: 14.340~233.802) were risk factors contributing to high-risk patients becoming critical population (P<0.05 or 0.01). Female gender (OR=1.295, 95% CI: 1.034~1.622), aged 40~49 (OR=2.597, 95% CI: 1.582~4.263), age ≥50 (OR=4.392, 95% CI: 2.609~7.395), overweight (OR=7.844, 95% CI: 6.018~10.223) and obesity (OR=426.785, 95% CI: 105.724~1 722.839) were risk factors for high-risk patients developing into metabolic syndrome population (P<0.05 or 0.01). ConclusionThe prevalence rate of metabolic syndrome is higher in community-dwelling patients with schizophrenia. Female gender, older age, overweight and obesity would increase the risk of metabolic syndrome in schizophrenic patients. RESUMO

@#Abstract: Objective To investigate the occurrence of multidrug-resistance among tuberculosis patients in Hainan Province from 2014 to 2020 and to analyze the influencing factors, aiming to provide reference for formulating drug-resistant tuberculosis control strategies in this region. Methods This study collected sputum samples from the patients with pulmonary tuberculosis admitted to the Second Affiliated Hospital of Hainan Medical University from 2014 to 2020, and performed isolation and identification of Mycobacterium tuberculosis and drug susceptibility testing. After the strains were identified as positive, drug sensitivity tests were conducted, and multi-drug resistant patients were found. Clinical data was retrospectively collected, and chi-square test and unconditioned logistic regression were used to analyze the influencing factors of multidrug resistance. Results A total of 2 672 patients underwent sputum culture, strain identification, and drug susceptibility testing in TB designated hospitals in Hainan Province from January 1, 2014 to December 31, 2020. Among them, 1 942 patients with available drug susceptibility test results and complete clinical data were enrolled, among which 398 cases with drug-resistant TB were included in the case group, and 1 544 cases without drug resistance were included in the control group. Multivariate logistic regression analysis showed that farmers, rural residence, treatment history of retreatment, irregular medication history, number of pulmonary cavities ≥3, and BMI<18.5 were independent risk factors for MDR-TB. The risk of MDR-TB in farmers was higher than that in non-farmers (OR=1.542, 95%CI: 1.150-2.020); patients living in rural areas had a higher risk of multidrug resistance than those living in urban areas (OR=1.445, 95%CI: 1.095-1.907); the risk of MDR in the retreatment patients was higher than that in the initial treatment patients (OR=5.616, 95%CI: 4.250-7.421); the risk of multi-drug resistance in patients with irregular medication was higher than that in patients with regular medication (OR=2.665, 95%CI: 2.012-3.531); the risk of multidrug resistance in patients with pulmonary cavity number ≥3 was higher than that in patients with pulmonary cavity number <3 (OR=5.040, 95%CI: 3.768-6.740); compared with patients with BMI<18.5, patients with BMI=18.5-24.0 and BMI≥24.0 had a lower risk of multidrug resistance (OR=0.735, 95%CI: 0.555-0.975 and OR=0.447,95%CI:0.225-0.888, respectively). Conclusions Retreatment, farmer occupation, rural residence, irregular medication and low BMI may be the risk factors for multidrug resistance in Hainan Province.

RESUMO

【Objective】 To investigate the influencing factors of red blood cell transfusion volume for premature neonatal pneumonia and its predictive value for feeding intolerance. 【Methods】 272 infants of premature neonatal pneumonia treated with red blood cell transfusion were collected as the research objects. Red blood cell transfusion volume was investigated and its influencing factors were analyzed by multiple linear regression. Receiver operating curve (ROC) was used to analyze the predictive value of red blood cell transfusion volume on feeding intolerance in infants with premature neonatal pneumonia. 【Results】 The average red blood cell transfusion volume in infants with premature neonatal pneumonia was (76.19±26.03) mL. Multiple linear regression analysis showed that gestational age, birth weight, volume of blood collection and hemoglobin level before blood transfusion were influencing factors of red blood cell transfusion volume in infants with premature neonatal pneumonia (B=-1.930, -6.215, 1.041, -0.249, P<0.05). The incidence of feeding intolerance in infants with premature neonatal pneumonia was 19.5%. The transfusion volume of feeding intolerance group was significantly higher than that of the non-feeding intolerance group(P<0.05). ROC analysis showed that the area under curve (AUC) of red blood cell transfusion volume for predicting feeding intolerance was 0.755. 【Conclusion】 Gestational age, birth weight, volume of blood collection and hemoglobin level before blood transfusion are influencing factors of red blood cell transfusion volume in infants with premature neonatal pneumonia. The incidence of feeding intolerance in premature neonatal pneumonia is high. The red blood cell infusion volume is of good predictive value for the occurrence of feeding intolerance. Latrogenic blood loss and red blood cell transfusion volume should be minimized clinically.

RESUMO

【Objective】 To investigate the re-donation willingness and its influencing factors of street blood donrs, so as to stable the team of blood donors and enhance the efficiency of blood collection. 【Methods】 From July 2021 to December 2022, 1 210 blood donors were randomly selected from 5 street blood donation sites affiliated to Nantong Central Blood Station, and their willingness to re-donate blood and influencing factors were surveyed via telephone interviews within 24 hours and 2 to 3 weeks after blood donation. The frequency and type of adverse donation reactions (ADRs) was gathered. 【Results】 Among 1 210 street blood donors, 590 were first-time blood donors, accounting for 48.76% (590/1 210), and 620 were repeated blood donors, accounting for 51.24% (620/1 210). The response rate of this survey was 96.03% (1 162/1 210), and the incidence of delayed ADRs was 1.29% (15/1 162) according to telephone interviews after blood donation. Among the 620 repeated blood donors, "health promotion" (61.77%, 383/620) was the most important reason for willingness of redonation(P<0.05), and the top of the list for on-site experience was "blood donation locations" (56.61%, 351/620) (P< 0.05). 【Conclusion】 The intention of blood donors, location of blood donation sites, service and publicity of blood donation are the main factors that affect the re-donation of blood donors. It's vital to increase the proportion of blood re-donation, and promote the development of voluntary blood donation by improving blood donation service, policy popularization and incentive measures.

RESUMO

【Objective】 To investigate the factors affecting the length of hospitalization after the Sun's procedure in patients with type A aortic coarctation. 【Methods】 From January 2018 to June 2023, the clinical data, related laboratory indicators and perioperative blood transfusion data of patients with type A aortic dissection who underwent Sun's procedure in the First Hospital of Lanzhou University were collected. LASSO regression was used to screen the characteristics related to the length of hospital stay, and linear regression analysis was used to determine the risk factors for prolonged length of hospital stay. 【Results】 The statistical analysis of 242 patients showed that the amount of red blood cell transfusion, plasma transfusion, platelet transfusion and autologous blood transfusion were the influencing factors of the length of hospital stay in patients with type A aortic dissection after operation. The total sum of squared deviations of the linear regression equation fitting the total length of hospital stay was statistically significant (F= 10.504, P<0.001). 【Conclusion】 Perioperative red blood cell transfusion, plasma transfusion, platelet transfusion and autologous blood transfusion are risk factors for prolonged postoperative hospitalization in patients undergoing the Sun's procedure for type A aortic coarctation. Control of operation time and reduction of intraoperative blood loss may help to prevent prolonged postoperative hospital stay and other adverse conditions.
