RESUMO

We report the case of a 54-year-old woman with concurrent Sweet's syndrome, pathergy phenomenon and eythema nodosum-like lesions associated with suppurative tonsillitis. Tender, violaceous and highly edematous papules and plaques were detected on the forearms and legs, some of which were pseudovesicular. Similar edematous papules were detected on the dorsum of the hands at the sites of intravenous injection. Biopsies of a plaque on the forearm and a papule at the site of intravenous injection confirmed the presence of Sweet's syndrome. Multiple, tender, violet-red, subcutaneous erythematous nodules were revealed on the legs. A biopsy taken from a nodule on the leg revealed septal panniculitis. To our knowledge, no patient with concurrent Sweet's syndrome, pathergy phenomenon, and erythema nodosum-lesions had been reported yet in previous literature.

RESUMO

Introducción. La atrofodermia idiopática de Pasini y Pierini es una atrofia dérmica de etiología desconocida que en la mayoría de los casos se localiza en el tronco y el abdomen. El diagnóstico clínico es difícil por la infrecuencia de cuadro y su semejanza con otras patologías, pero su histología es característica. Caso Clínico. Se describe el cuadro clínico e histológico de esta patología en una paciente de tres años de edad quien presentó una lesión atrófica, grisácea, unilateral, localizada en muslo izquierdo, con 5 meses de evolución. El estudio histológico reveló hialinización y esclerosis del colágeno compatible con Atrofodermia de Pasini y Pierini. Conclusión. Este caso se reporta por su presentación morfológica e histopatológica típica de esta patología, pero atípica por la edad de la paciente y por la localización anatómica.

RESUMO

Sclerosing panniculitis is a fibrotic process that usually occurs on the legs, commonly in women older than 40. The principal features are indurated woody plaques with erythema, edema, telangiectasia, and hyperpigmentation. Although the exact pathogenesis is uncertain, it is thought to occur as a result of ischemic changes. We present a 28-year-old married female who had a 10-year history of painful sclerotic plaques, repeated ulceration and healing with fibrosis of the both lower legs and abdomen. Venogram and Doppler investigations were normal. Skin biopsy from the edge of the ulcer demonstrated the feature of chronic sclerosing panniculitis. Satisfactory improvement was found with methotrexate 7.5mg weekly for 4 months. No recurrence was noted within 1 year follow up

RESUMO

Pancreatic panniculitis is a rare disease in which necrosis of fat in the panniculus and other distant foci occurs in the setting of pancreatic diseases; these diseases include acute and chronic pancreatitis, pancreatic carcinoma, pseudocyst, and other pancreatic diseases. This malady is manifested as tender erythematous nodules on the legs, buttock, or trunk. Histopathologically, it shows the pathognomonic findings of focal subcutaneous fat necrosis and ghost-like anucleated cells with a thick shadowy wall. We herein report a case of fatal pancreatic panniculitis that was associated with acute pancreatitis in a 50-yr-old man. He presented with a 3-week history of multiple tender skin nodules, abdominal pain and distension. Laboratory and radiologic findings revealed acute pancreatitis, and skin biopsy showed pancreatic panniculitis. Despite intensive medical care, he died of multi-organ failure 3 weeks after presentation.

Calcinosis cutis es el depósito de calcio en la piel. Considerando el mecanismo fisiopatológico causal, existen cuatro formas de calcinosis cutis: Metastásica, Distrófica, Iatrogénica e Idiopática. Se revisan brevemente las formas señaladas y se muestran tres casos de calcinosis cutis distrófica
