ABSTRACT

Objetivo: Describir la calidad de vida relacionada con la salud (CVRS) en consumidores de benzodiacepinas y comprobar si existe asociación con las características del tratamiento, su efectividad y las variables sociodemográficas. Diseño: Estudio descriptivo de carácter transversal. Emplazamiento: Consultas de medicina de familia. Participantes: Cuatrocientos cincuenta y dos pacientes mayores de 18 años consumidores de benzodiacepinas o fármacos análogos. Mediciones principales: La CVRS se evaluó mediante el cuestionario EuroQol-5D. Otras variables: síntomas de ansiedad o insomnio, variables sociodemográficas y características del tratamiento. Resultados: La puntuación media ± desviación estándar en el estado de salud fue de 62,80 (IC del 95%: 60,69-64,86), inferior en personas sin estudios (59,27 ±21,97; p = 0,004) y menor categoría social (60,02 ± 21,27; p < 0,001). En cuanto a la tarifa social (índice EQ) se obtuvo una puntuación media de 0,6025 (IC del 95%: 0,5659-0,6391), superior en personas con mayor escolarización (0,6577 ± 0,3574; p = 0,001), más categoría social (0,7286 ± 0,3381; p < 0,001) y edad inferior a 65 años (0,6603 ± 0,3426; p < 0,001). Las variables que mediante regresión múltiple se asociaron con el valor del índice EQ fueron ausencia de ansiedad/insomnio, pertenencia a clases sociales superiores, edad menor de 65 años y menor consumo de ansiolíticos/hipnóticos. Conclusiones: Los pacientes consumidores de benzodiacepinas manifiestan, a pesar del tratamiento, una moderada CVRS, inferior a la obtenida en población general o en pacientes de atención primaria. La situación es más favorable en los de menor edad, en quienes no presentan ansiedad/insomnio, en pertenecientes a clases sociales superiores y cuando el consumo de fármacos es menor.(AU)

The situation is more favorable in the youngest, in those who do not present anxiety/insomnia, in those belonging to higher social classes and when the consumption of drugs is lower.(AU)

ABSTRACT

Introducción. La lepra es una enfermedad infecciosa causada por la bacteria Mycobacterium leprae. Presenta especial avidez por la piel y los troncos nerviosos, y, de hecho, ambos se afectan en la mayor parte de los infectados. Se trasmite por exposición con enfermos y en ocasiones por reactivación. Una posibilidad inhabitual es la lepra neural pura, caracterizada por neuropatía, pero sin lesiones en la piel. Se describe un paciente con lepra neural pura y se revisan los aspectos diagnósticos. Caso clínico. Varón de 40 años, inmigrante, diagnosticado y tratado de lepra 20 años antes. Acudió por parestesias y disestesias dolorosas en las manos y las piernas sin lesiones en la piel. Se demostró mononeuritis múltiple aguda con principal afectación de cubitales. La enfermedad, tipificada como tuberculoide paucibacilar, se trató y en pocas semanas la mejoría fue evidente. Conclusiones. En este caso de lepra neural pura por reactivación, el diagnóstico temprano permitió un rápido tratamiento. Es recomendable la evaluación de la neuropatía integrada con criterios clínicos, electrofisiológicos y ecográficos. De este modo se consigue una alta sensibilidad y especialmente una precocidad en el diagnóstico y la instauración del tratamiento, y por consecuencia una mejor recuperación funcional (AU)


Introduction. Leprosy is an infectious disease caused by the bacteria Mycobacterium leprae. It is particularly prone to affect the skin and the nerve trunks and, in fact, both are compromised in most infected patients. It is transmitted by exposure to those with the disease and sometimes by reactivation. One uncommon possibility is pure neural leprosy, which is characterised by neuropathy, but without skin lesions. We report the case of a patient with pure neural leprosy and review the diagnostic aspects. Case report. A 40-year-old male, an immigrant who was diagnosed and treated for leprosy 20 earlier. The patient visited due to painful paraesthesias and dysesthesias in the hands and legs without the presence of any skin lesions. Acute multiple mononeuritis with mainly ulnar involvement was observed. The disease, typified as paucibacillary/tuberculoid, was treated and in a few weeks there was a clear improvement. Conclusions. In this case of pure neural leprosy due to reactivation, early diagnosis allowed timely treatment to be established. Evaluation of neuropathy together with clinical, electrophysiological and ultrasound criteria is recommended. By so doing, a high degree of sensitivity is achieved as well as allowing early diagnosis and treatment, and therefore a better functional recovery (AU)

ABSTRACT

Objetivo: Conocer las actitudes de los residentes de Medicina Familiar y Comunitaria (MF y C) hacia las actividades preventivas y de promoción de la salud. Diseño: Estudio observacional de carácter transversal. Emplazamiento: Unidades docentes de MF y C de la comunidad autónoma de Castilla-La Mancha. Participantes: Los 266 residentes adscritos a dichas unidades docentes. Mediciones Principales: Actitudes de los médicos residentes de MF y C hacia las actividades preventivas y de promoción de la salud evaluadas mediante la escala CAPPAP (cuestionario sobre las actitudes ante la prevención y promoción de la salud en atención primaria). Otras variables: características demográficas y profesionales. Resultados: Más del 90 % de los residentes se muestran dispuestos a colaborar en la provisión de actividades preventivas. Las actividades preventivas más valoradas son las relacionadas con la hipertensión arterial y con el consejo sobre el cese del hábito tabáquico. Los residentes de MF y C extracomunitarios (45,2 %) conceden una mayor importancia a las actividades preventivas y de promoción de la salud. Conclusiones: Los médicos residentes de MF y C presentan una actitud favorable hacia la realización de las actividades preventivas y de promoción de la salud, considerando como las más importantes la detección y seguimiento de la hipertensión arterial y el consejo sobre el cese del hábito tabáquico (AU)


Objective: To provide insight into the attitudes of Family and Community Medicine (FCM) residents towards preventive activities and health promotion. Design: Observational cross-sectional study. Location: FCM teaching units in the autonomous community of Castilla-La Mancha. Participants: The 266 residents assigned to those teaching units. Main measures: Attitudes of FCM medical residents towards preventive and health promotion activities evaluated by CAPPAP (questionnaire on attitudes towards prevention and health promotion in primary care). Other variables: demographic and professional features. Results: Over 90% of the residents are willing to collaborate on the provision of preventive activities. The most highly valued preventive activities are those related to arterial hypertension and to advice on smoking cessation. FCM residents from other autonomous communities (45,2%) give greater importance to preventive and health promotion activities. Conclusions: Family and Community Medicine residents have a favourable attitude towards conducting preventive and health promotion activities. The ones they consider most important are those related to detecting and monitoring arterial hypertension and to advice on smoking cessation (AU)
