ABSTRACT

Objetivo. Analizar la importancia que la dependencia conjunta de alcohol y cocaína tiene en la presentación de diferentes trastornos psicopatológicos. Material y métodos. Se describe el caso de un paciente que solicitó una consulta en la unidad de alcoholismo por presentar dependencia de alcohol y cocaína. Asimismo, se mencionan los datos más relevantes que se observaron durante los 6 años en que el paciente estuvo en seguimiento. Resultados. A pesar de los diferentes tipos de abordaje terapéutico utilizados (desintoxicación hospitalaria y tratamiento ambulatorio), el paciente estuvo abstinente de alguna de las sustancias durante cortos períodos de tiempo. Durante el seguimiento presentó dos episodios que fueron diagnosticados como trastorno bipolar tipo dos. Además, presentaba un trastorno del comportamiento sexual, etiquetado como parafílico, cuyas primeras manifestaciones precedieron al consumo de drogas y se manifestaba clínicamente bajo su efecto. Conclusiones. El consumo de alcohol y cocaína incrementa la morbilidad psiquiátrica y empeora el pronóstico de la dependencia de ambas sustancias, el abordaje terapéutico es más complejo y el cumplimiento terapéutico es menor


Objective. To analyze the importance that both alcohol and cocaine dependence have in the appearance of different psychopathological disorders. Material and methods. The authors describe a patient’s case who demanded a consult in the alcoholism unit to treat alcohol and cocaine dependence. They also mention the most important data they observed during the six-year treatment of the patient. Results. The patient was treated with various kinds of therapeutic approaches (hospital detoxification, outpatient treatment) and he was off alcohol and/or cocaine for short periods of time. In the follow-up the patient had two events which were diagnosed as bipolar disorder, type two. He also had a sexual disorder labelled as paraphilias, which first symptoms appeared before drug use and at the present moment this disorder shows with drug taking. Conclusions. The alcohol and cocaine taking increases the psychiatric morbidity and the dependence prognosis of both substances is worse, the therapeutic approach is more complex and the compliance with the treatment is lesser

ABSTRACT

This survey analyzes the evolution of one sample of alcoholic patients two years after finishing treatment. Its target is to determine the percentage of patients that remain abstinent, their rate of retention, and what factors can have an influence on abstinence. In 1990, 72 alcoholic patients were treated in the Alcoholism Unit, who make up our study sample. The average age was 37.9 +/- 11.47 years old; 77.8% were diagnosed as being Alcohol-dependents and 22.2% as Alcohol-abusers; 19.4% dropped out the treatment early. In 1992 our sample of study was 53 patients (2 died and 17 refused to participate), of whom 77.7% were abstinent and 28.3% continued ingesting alcohol; the average abstinence was 22.92 +/- 8.73 months. An important finding of this study was that the diagnosis, sex, and treatment with aversives had not an influence on abstinence; however the percentage of abstinent patients in those who had attended Therapeutic Discussion Groups was significantly higher than in those who had not.

ABSTRACT

This work is a study of the personality of the wives of alcoholic patients composed by means of a structures interview, the MMPI personality questionnaire and the Instrument I used to evaluate aggressivity. Among the discoveries made, we must emphasize that 20% of the wives knew of their partner's excessive alcohol consumption before marriage, while married, 75% were victims of some kind of violent incident, 43% had personal psychopathological backgrounds, 15% were "repeaters wives" "those who had alcoholic parents). In the MMPI test, these women represent a significantly high profile, in which the scales Hs, D and Hy are the highest, amongst which their most outstanding personality traits include passivity, dependency and insecurity. Furthermore, those wives whose husbands have had relapses, represent a higher Pd scale than the other group. The Instrument I used to evaluate aggressivity also emphasizes this passive-aggressive tendency in the wives.
