ABSTRACT

BACKGROUND: Cancer is currently a major public health problem worldwide, with a marked increase of about 70% in the number of expected diagnosed cases over the next two decades. The amount of tobacco and alcohol consumed is calculated based on the subjective information provided by the user. Tobacco exposure can be assessed using the Fagerström Test for Cigarette Dependence (FTCD) and alcohol consumption by the Alcohol Use Disorder Identification Test (AUDIT). MATERIALS AND METHODS: Forty-eight subjects answered the Fagerström, and AUDIT tests and we studied them as likely screening tools for oral cancer and their correlation with the expression of CYP1A1, GSTM1, GSTP1, and GSTT1 genes by the RT-qPCR method. RESULTS: There were significant differences in the AUDIT score and CYP1A1 expression between cancer and control groups. Participants in advanced stages, whether due to tumor size or regional metastasis, showed significant differences in the duration of tobacco use, FTCD, AUDIT score, and CYP1A1 expression when compared to patients in early stages. Among subjects without cancer, we found a significant correlation between participant age and GSTP1 expression. Furthermore, the expression of GSTP1 was significantly correlated with the number of cigarettes smoked per day, duration of tobacco use, and FTCD. CONCLUSIONS: Questionnaires designed to evaluate the degree of tobacco and alcohol exposure and dependence combined with gene expression tests can be useful to assess the risk of developing oral cancer. Furthermore, raising the awareness of individuals regarding their degree of dependence and encouraging them to participate in cessation programs are important educational measures for the prevention of tobacco-related malignancies.

ABSTRACT

Oral squamous cell carcinoma (OSCC) represents 90% of oral malignant neoplasms. The search for specific biomarkers for OSCC is a very active field of research contributing to establishing early diagnostic methods and unraveling underlying pathogenic mechanisms. In this work we investigated the salivary metabolites and the metabolic pathways of OSCC aiming find possible biomarkers. Salivary metabolites samples from 27 OSCC patients and 41 control individuals were compared through a gas chromatography coupled to a mass spectrometer (GC-MS) technique. Our results allowed identification of pathways of the malate-aspartate shuttle, the beta-alanine metabolism, and the Warburg effect. The possible salivary biomarkers were identified using the area under receiver-operating curve (AUC) criterion. Twenty-four metabolites were identified with AUC > 0.8. Using the threshold of AUC = 0.9 we find malic acid, maltose, protocatechuic acid, lactose, 2-ketoadipic, and catechol metabolites expressed. We notice that this is the first report of salivary metabolome in South American oral cancer patients, to the best of our knowledge. Our findings regarding these metabolic changes are important in discovering salivary biomarkers of OSCC patients. However, additional work needs to be performed considering larger populations to validate our results.

ABSTRACT

OBJECTIVE: to assess the efficacy of the single transverse extended cervical incision in radical neck dissection. METHOD: we conducted a prospective study, from January 2008 to January 2009, with 18 patients undergoing surgical treatment of malignant tumors of the upper aero-digestive tract. The primary lesion was located in the oral cavity in eight cases, in the oropharynx in three, in the hypopharynx in three, in the larynx in two, in the maxillary sinus, and in one case, the primary injury was hidden. There were 29 neck dissections, eight bilateral and 10 unilateral (26 radical and three selective). Staging revealed nine patients with T4 tumor, one T3, six T2, one T1 and one Tx. Five patients were N0, nine N2b, one N2c and three N3. The average number of dissected lymph nodes was 34.25. We performed the neck dissection through a single incision located in the middle neck, coincident with the skinfold, with a length of about 2 to 3 cm behind the anterior edge of the trapezius muscle and 3 to 4 cm from the midline for the unilateral neck dissections. RESULTS: as complications, there were myocutaneous flap necrosis in one patient with prior radiation therapy, one lymphatic fistula, one dehiscence of the tracheostomy, one cervical abscess, one salivary fistula and one suture dehiscence. CONCLUSIONS: the single extended incision provides adequate exposure of the neck structures, without compromising surgical time, even in bilateral dissections. It does not compromise the resection of all cervical lymph nodes; it has excellent aesthetic and functional results and is easily associated with other approaches to resection of the primary tumor. A lesão primária se situava na cavidade oral em oito casos, na orofaringe em três, no seio piriforme em três, na laringe em dois, no seio maxilar em um e em um caso a lesão primária era oculta. Houve 29 esvaziamentos, sendo oito bilaterais e 10 unilaterais (26 radicais e três seletivos). O estadiamento revelou nove pacientes com tumor T4, um T3, seis T2, um T1 e um Tx. Cinco pacientes eram N0, nove N2b, um N2c e três N3. A média de linfonodos dissecados foi de 34,25. O esvaziamento cervical foi realizado por meio de uma única incisão localizada no terço médio do pescoço, coincidente com dobra cutânea, com extensão de cerca de 2 a 3 cm para trás da borda anterior do músculo trapézio e 3 a 4 cm da linha média para os esvaziamentos cervicais unilaterais. RESULTADOS: como complicações houve necrose de retalho miocutâneo em um paciente com radioterapia prévia, uma fistula linfática, uma deiscência do traqueostoma, um abscesso cervical, uma fístula salivar e uma deiscência de sutura. CONCLUSÕES: a incisão única e estendida proporciona exposição adequada das estruturas do pescoço, sem comprometer o tempo cirúrgico, mesmo em esvaziamentos bilaterais. Não compromete a ressecção de todos os linfonodos cervicais, apresenta excelentes resultados estéticos e funcionais e é facilmente associada com outras abordagens para ressecção do tumor primário.

ABSTRACT

RACIONAL: Prognóstico nutricional ruim no período pré-operatório -, principalmente nos pacientes que apresentam perda de peso -, pode levar à complicações pós-operatórias em maior frequência. OBJETIVO: Investigar indicadores de risco nutricional em pacientes internados em enfermaria de cirurgia, no período pré-operatório, estratificados por grupos de doenças mais frequentes no serviço. MÉTODO: Foram avaliados 512 pacientes portadores de abdome agudo, doenças do trato digestório, doenças de vias biliares, doenças ginecológicas, doenças vasculares, hérnias e traumas, sendo analisados o índice de massa corporal, a perda de peso recente, o consumo energético habitual, a contagem de linfócitos e o tempo de internação. Os dados foram comparados entre os grupos de doenças, sendo utilizado o teste qui-quadrado para a comparação de proporções ou teste exato de Fisher, quando necessário, com nível de significância de 5 por cento. RESULTADOS: Os pacientes com trauma, seguidos daqueles portadores de doenças do trato digestório e abdome agudo, apresentaram índice de massa corporal mais baixos, com diferença significativa entre os grupos (P=0,0222), e entre os que mais apresentaram perda de peso recente na internação, estavam os pacientes com abdome agudo, (P=0,0048). Verificou-se maior percentual de depleção imunológica grave e moderada nos portadores de abdome agudo (P<0,0001) e, maior percentual de pacientes com consumo energético inferior a 1000 kcalorias, entre aqueles com doenças vasculares, (P=0,0311). Os pacientes com trauma apresentaram maior tempo de internação (P<0,0001). CONCLUSÃO: Os pacientes portadores de abdome agudo e trauma apresentaram maior risco nutricional no período pré-operatório, o que deveria demandar estratégias de atenção nutricional logo no início da internação.


BACKGROUND: Bad nutricional status in preoperative period - mainly associated to weight loss -, may lead to grater number of postoperative complications. AIM: To investigate the nutritional risk indicators in preoperative period, stratified by disease groups treated at the site. METHODS: A total of 512 patients with the following conditions were assessed: acute abdomen, digestive tract diseases, biliary diseases, gynecological diseases, vascular diseases, hernias and traumas. The following data were analyzed: body mass index, recent weight loss, habitual energy intake, lymphocyte count and length of hospital stay. The data were compared among the disease groups. The chi-square test or Fisher's exact test when necessary was used to compare proportions. The significance level was set at 5 percent. RESULTS: Trauma patients followed by those with digestive tract diseases and acute abdomen had significantly lower body mass index (P=0.0222), and the ones with acute abdomen presented recent weight loss at hospital admittance (P=0.0048). A greater percentage of severe and moderate immune depletion was seen in patients with acute abdomen (P<0.0001) and those with vascular diseases had a greater percentage of patients with energy intake below 1000 kcalories (P=0.0311). Trauma patients presented longer hospital stays (P<0.0001). CONCLUSION: Trauma and acute abdomen patients had greater nutritional risk during the preoperative period. Thus, nutritional care strategies should be oriented towards them since the hospital admittance.

ABSTRACT

OBJETIVO: Apresentar as várias formas de displasia fibrosa craniofacial em um estudo revisional com 14 exames de tomografia computadorizada. MATERIAIS E MÉTODOS: Foram selecionados 14 casos de displasia fibrosa craniofacial atendidos no Serviço de Cirurgia de Cabeça e Pescoço e Otorrinolaringologia e no Serviço de Diagnóstico por Imagem do Hospital Heliópolis, São Paulo, SP, entre 1991 e 2002. Foram avaliados, pelos autores, três dos achados tomográficos mais relevantes: o número de ossos acometidos, a apresentação radiológica predominante e a uni/bilateralidade da doença no crânio. RESULTADOS: Observou-se predomínio do padrão misto em relação à forma de apresentação da doença. Foram vistos, ainda, acometimento contíguo de dois ou mais ossos e equivalência entre casos de acometimento craniofacial unilateral e bilateral, possivelmente por conta dos pacientes serem, em sua maioria, portadores de displasia fibrosa do tipo monostótica. CONCLUSÃO: O conhecimento das características tomográficas da displasia fibrosa craniofacial é de grande importância para que seja feito planejamento cirúrgico adequado e seguimento pós-tratamento.


OBJECTIVE: To describe the different presentations of craniofacial fibrous dysplasia in a revisional study of 14 computed tomography exams. MATERIALS AND METHODS: The authors have selected 14 cases of craniofacial fibrous dysplasia attended at the Head & Neck Surgery/Otorhinolaryngology Service and Diagnostic Imaging Service of "Hospital Heliópolis", São Paulo, SP, Brazil, between 1991 and 2002. Three of the following most relevant tomographic findings were analyzed: number of bones affected, predominant radiological presentation and the disease uni- or bilaterality. RESULTS: A predominance of a mixed pattern was observed in relation to the disease presentation. Also, contiguous involvement of two or more bones was observed as well as equivalence between cases of unilateral and bilateral craniofacial involvement, possibly due the fact that most patients presented the monostotic type of fibrous dysplasia. CONCLUSION: The knowledge of tomographic features of craniofacial fibrous dysplasia is of paramount importance for surgical planning and post-treatment follow-up.

ABSTRACT

Relatamos um caso de actinomicose laríngea mimetizando neoplasia, com revisão da literatura a respeito dos aspectos clínicos, patológicos e de imagens. Utilizamos tomografia computadorizada, exame loco-regional, nasofibroscopia e estudo anatomopatológico. A importância do estudo se deve à raridade da enfermidade, localização atípica e particularmente ao diagnóstico diferencial com neoplasia de laringe. Não identificamos, na literatura, casos com avaliação por meio de tomografia computadorizada.


We report a case of a patient with actinomycosis of the larynx mimicking a neoplasm. A literature review, clinical features, pathology and imaging findings is also presented. This paper reports a rare disease occurring in an atypical location, simulating larynx neoplasm. To our knowledge, there is no report of this disease and locations evaluated with computed tomography.

ABSTRACT

O objetivo deste estudo é relatar um caso raro de aneurisma da artéria hepática simulando lesão em cabeça de pâncreas. É importante a análise precisa por tomografia computadorizada e ressonância magnética, pois desempenham papel importante no planejamento terapêutico e influenciam também o prognóstico.


The aim of this study is to report a rare case of hepatic artery aneurysm simulating a lesion in the head of the pancreas. A precise evaluation using computed tomography and magnetic resonance imaging is important in the treatment planning, which will influence the prognosis
