ABSTRACT

Este artigo descreve o processo de construção de um contexto de diálogo e colaboração em uma equipe multidisciplinar, especializada em pacientes com doenças neuromusculares. A equipe experimenta diariamente sentimentos de impotência diante da precocidade da morte e da incerteza do futuro, por vezes sem espaço para serem compartilhados na rotina de trabalho. Relata especificamente, a produção de um documento coletivo e posteriormente a realização de uma cerimônia de definição, ambos inspirados nas chamadas Práticas Narrativas Coletivas descritas por David Denborough. Narrar e renarrar histórias vividas com os pacientes, ressaltando habilidades e conhecimentos da equipe ao enfrentar as dificuldades para estar da melhor maneira possível com tais pacientes e seus familiares, colaborou para o fortalecimento da equipe e proporcionou o início de uma grande rede conversacional sobre o tema.(AU)


This article describes the process of reconstruction within a dialogue context and collaboration of a team of multidisciplinary professionals who are specialised in patients with neuromuscular diseases. The team experiences feelings of impotence created by the deep impact of early deaths and uncertainty of the future. These feelings often had no room for sharing in the daily routine of work. This report also a production of a document of collective production and subsequently the ceremony of definition, both inspired by the called Collective Narratives written by David Denborough. This team tells and retells stories lived with their patients enhancing the team’s skills and knowledge while facing difficulties and doing their best when spending time with the patients and families. These facts strengthened the team and offered the beginning of a large conversational network about the theme.(AU)

ABSTRACT

La trombosis de los senos venosos es una patología poco común en pediatría y que puede ser causada por una larga lista de factores etiológicos. En general, sus manifestaciones clínicas son inespecíficas: letargia, convulsiones, cefalea, trastornos visuales, signos de focalización, etc. La sospecha clínica se complementa con neuroimágenes y especialmente con tomografía y resonancia nuclear magnética cerebrales. La evolución de la enfermedad es variable y depende de la extensión y la localización del daño parenquimatoso. El tratamiento es un tema aún controversial. En algunos centros se ha usado la terapia anticoagulante, con aparentemente buenos resultados, pero aún faltan estudios serios, multicéntricos y randomizados, que definan criterios acerca del uso de anticoagulantes en pediatría. Se presenta el caso clínico de un paciente de 4 meses de edad, que presentó una sintomatología poco especifica posterior a un accidente automovilístico. Se describen su diagnóstico, evolución y manejo posterior.

ABSTRACT

The opercular syndrome is a rare neurological disease, affecting a small area of the telencephalic cortex overlying the insula known as operculum. Clinical signs are loss of voluntary control of orofacial muscles, with preservation of reflexes and automatic actions such as crying and laughing. Three cases of opercular syndrome are reported. The first is a boy with congenital bilateral parasylvian(?) syndrome; the second is a boy with opercular syndrome secondary to an acute viral encephalitis; and the third is a boy with psychomotor retardation whose diagnosis was established while being studied for convulsions. Aetiology, clinical aspects and imaginology of yhe syndrome - occasionally also affecting adults - are reviewed and discussed.
