ABSTRACT

Introduction: In February 2022, the Emerging Thoracic Surgery Group of the Spanish Society of Pneumology and Thoracic Surgery initiated a multicenter study on the surgical management of primary spontaneous pneumothorax (PSP). As a preliminary step, this survey was developed with the aim of finding out the current situation in our country to specify and direct this project. Method: A descriptive study was carried out based on the results of this survey launched through the Google Docs® platform. The survey was sent to all active national thoracic surgeons, a total of 319. It consisted of 20 questions including demographic, surgical and follow-up data. Results: We obtained 124 responses (39% of all specialists and doctors in training in the national territory). The most consistent indications were: homolateral recurrence for 124 (100%), lack of resolution of the episode for 120 (96.7%), risk professions for 104 (84%) and bilateral pneumothorax for 93 (75%). The approach of choice for 100% of respondents was videothoracoscopy. Of these, 96 contemplated pulmonary resection of obvious lesions (77%). Regarding the pleurodesis technique, pleural abrasion was the technique most used by 70 respondents (56.7%) while 49 (40%) performed chemical pleurodesis with talc either alone or in combination with mechanical pleurodesis. Conclusions: While there is some consistency in some aspects of surgical management of PSP, this survey makes evident the variability in pleurodesis techniques applied among surgeons in our country.

ABSTRACT

Glioblastoma (GBM) is the most common and malignant primary brain cancer in adults. Without treatment the mean patient survival is approximately 6 months, which can be extended to 15 months with the use of multimodal therapies. The low effectiveness of GBM therapies is mainly due to the tumor infiltration into the healthy brain tissue, which depends on GBM cells' interaction with the tumor microenvironment (TME). The interaction of GBM cells with the TME involves cellular components such as stem-like cells, glia, endothelial cells, and non-cellular components such as the extracellular matrix, enhanced hypoxia, and soluble factors such as adenosine, which promote GBM's invasiveness. However, here we highlight the role of 3D patient-derived glioblastoma organoids cultures as a new platform for study of the modeling of TME and invasiveness. In this review, the mechanisms involved in GBM-microenvironment interaction are described and discussed, proposing potential prognosis biomarkers and new therapeutic targets.

RESUMEN: Describir el perfil epidemiológico de los pacientes con FLMP no sindrómica de acuerdo a los factores que involucran a la madre. Estudio de tipo observacional, retrospectivo y analítico a partir de los datos de los casos de FLMP no sindrómicos. Se clasifico el tipo de FLMP y las variables que involucran a la madre. Fueron evaluadas 236 fichas completas de pacientes. Un 56,60 % pertenecen al sexo masculino y 43,40 % al sexo femenino. El mayor porcentaje nació el año 2011 (15,57 %), la FLP es la más prevalente (44,92 %), el 26,67 % de las madres tenía entre 21-25 años al momento del parto, un 80,50 % y 87,50 % consumió alcohol y tabaco respectivamente durante primer trimestre del embarazo, y un 41,33 % se desempeñó en rubro agrícola. Pueden ser considerados como factores de riesgo durante el embarazo: estrés post catástrofe natural, enfermedades crónicas, consumo de fármaco, hábito tabáquico y alcohólico y exposición a pesticidas.


ABSTRACT: The objective of the study was to describe the epidemiologic profile of patients with non-syndromic cleft palate according to factors that involve the mother. Observational, retrospective and analytical study from data obtained of non-syndromic cleft lip palate cases. The type of cleft lip and palate (CLP) and the variables that involve the mother were classified. In this analysis 236 fulfilled clinical records from patients were evaluated; 56,60 % correspond to male and 43,40 % to female. Most births occurred in 2011 (15,57%), cleft palate is the most prevalent (44,92 %), at the moment of birth mothers were 21-25 years old (26,67 %), most of the mothers consumed alcohol or tobacco during the first trimester of pregnancy (80,50 % and 87,50 %, respectively), and 41,33% worked in agriculture related jobs. In conclusion post natural catastrophe stress, chronic diseases, consumption of drugs, smoking and alcohol habits and exposure to pesticides can be considered as a risk factor for having a child with CLP.

ABSTRACT

The diarrheagenic pathogen enteropathogenic Escherichia coli (EPEC) limits the death of infected enterocytes early in infection. A number of bacterial molecules and host signaling pathways contribute to the enhanced survival of EPEC-infected host cells. EspZ, a type III secreted effector protein that is unique to EPEC and related "attaching and effacing" (A/E) pathogens, plays a role in limiting host cell death, but the precise host signaling pathways responsible for this phenotype are not known. We hypothesized that EspZ contributes to the survival of infected intestinal epithelial cells by interfering with apoptosis. Consistent with previous studies, scanning electron microscopy analysis of intestinal epithelial cells infected with an EPEC espZ mutant (ΔespZ) showed increased levels of apoptotic and necrotic cells compared to cells infected with the isogenic parent strain. Correspondingly, higher levels of cytosolic cytochrome c and increased activation of caspases 9, 7, and 3 were observed for ΔespZ strain-infected cells compared to wild-type (WT) EPEC-infected cells. Finally, espZ-transfected epithelial cells were significantly protected from staurosporine-induced, but not tumor necrosis factor alpha (TNF-α)/cycloheximide-induced, apoptosis. Thus, EspZ contributes to epithelial cell survival by mechanisms that include the inhibition of the intrinsic apoptotic pathway. The enhanced survival of infected enterocytes by molecules such as EspZ likely plays a key role in optimal colonization by A/E pathogens.
