ABSTRACT

AIM: To identify Pseudomonas aeruginosa and Staphylococcus aureus in oral biofilms of intubated and non-intubated patients admitted to an Intensive Care Unit (ICU). METHODS: This was a cross-sectional study, with 30 biofilm sites sampled. S. aureus and P. aeruginosa were identified by conventional biochemical assays. Antimicrobial susceptibility was tested by disk-diffusion. RESULTS: Of 30 sites, 50% contained P. aeruginosa and 3.33% S. aureus. P. aeruginosa was detected in similar amounts in all 3 sample sites, with 5 colonized sites (50%). S. aureus colonized a single supragingival site (3.33%). There was resistance to multiple antimicrobial agents of P. aeruginosa in 7 sites (100%) and S. aureus in 1 (100%). CONCLUSIONS: This study revealed an important relationship between P. aeruginosa and S. aureus colonization at supragingival, subgingival and lingual sites and intubation, thus revealing antimicrobial resistant bacteria colonization of medical interest, which may contribute to the therapy choice directed to these microorganisms.

Abstract Introduction: Cocaine is one of the most often used psychoactive drugs worldwide, being extracted from the leaves of Erytroxylus coca plant. Its abusive use can trigger several consequences for the human body, including the oral cavity. Objective: To identify the oral disorders that are most commonly found in individuals who abuse cocaine, in addition to the main diagnostic and treatment methods. Methods: An integrative review was carried out on the databases: LILACS, BBO, LIS, MEDLINE, SciELO, Science Direct and PubMed. The following keywords were used: ‟Cocaína", ‟Boca", ‟Palato" and ‟Odontologia", together with their synonyms and variations in English, obtained from DeCS andMeSH. The inclusion criteria were original articles, articles in Portuguese, English and Spanish, studies involving individuals, without restriction related to the year of publication. Animal studies, literature reviews, book chapters, theses and dissertations were excluded. Results: In total, 1373 records were identified. Of these, 22 articles were selected to comprise the review. Several oral alterations caused by cocaine abuse were found, primarily perforation of the palate, predisposition to periodontal diseases, temporomandibular disorders, bruxism, damage to oral tissues, dental caries, destructive lesions of the facial midline, xerostomia and ageusia. Among the diagnostic methods used by the professionals, anamnesis, intraoral examinations and head and neck computed tomography were the most frequently mentioned. As for treatment, in patients with palatal perforation, the reconstruction of the affected area or the use of prosthetic obturators is carried out. Conclusion: The management of these patients is not an easy task, as many of the users do not even seek professional help. The health professionals must be able to recognize these manifestations and alterations to establish timely and accurate diagnosis and treatment planning. © 2021 Associação Brasileira de Otorrinolaringologia e Cirurgia Cérvico-Facial. Resumo Introdução: A cocaína é uma das drogas psicoativas mais usadas no mundo, é extraída das folhas da Erytroxylus coca. Seu uso abusivo pode desencadear inúmeras consequências para o corpo humano, inclusive na cavidade oral. Objetivo: Identificar quais as alterações orais mais comumente encontradas nos indivíduos que fazem uso abusivo de cocaína, além das principais formas de diagnóstico e tratamento. Método: Fez-se uma revisão integrativa nas bases de dados Lilacs, BBO, LIS, Medline, Sci-ELO, Science Direct e PubMed. Foram usados os descritores ‟cocaína", ‟boca", ‟palato" e ‟odontologia" junto de seus sinônimos e variações em inglês, retirados do DeCS e MeSH. Os critérios de inclusão foram artigos originais, artigos nas línguas portuguesa, inglesa e espanhola, estudos que envolveram pessoas e sem restrição quanto ao ano de publicação. Foram excluídos estudos feitos em animais, revisões da literatura, capítulos de livros, teses e dissertações. Resultados: Foram identificados 1.373 registros. Desses, 22 artigos foram selecionados para compor a revisão. Diversas alterações orais causadas por uso abusivo de cocaína foram encontradas. As principais foram perfuração do palato, predisposição para doenças periodontais, desordens temporomandibulares, bruxismo, danos aos tecidos orais, cárie dentária, lesões destrutivas da linha média facial, xerostomia e ageusia. Entre as formas de diagnóstico usadas pelos profissionais, a anamnese, exames físicos intraorais e a tomografia computadorizada da região da cabeça e pescoço foram as mais mencionadas. Como tratamento, nos pacientes com perfuração palatina, é feita a reconstrução da área afetada ou o uso de obturadores protéticos. Conclusão: O manejo desses pacientes não é tarefa fácil, pois muitos usuários sequer procuram ajuda profissional. Os profissionais da saúde devem ser capazes de reconhecer tais alterações e manifestações para que sejam feitos diagnósticos e planejamentos de tratamentos oportunos e precisos. DESTAQUES Revisão integrativa que visa detectar alterações orais em usuários de cocaína. Identificaram-se as principais manifestações orais, formas de diagnóstico e tratamento. Necessidade da feitura de boa anamnese e profissionais da saúde capacitados.

ABSTRACT

Municipal wastewater is a source of pathogenic protozoan (oo)cysts and may play a significant role in spreading waterborne diseases. This scenario becomes more critical as treated sewage from municipal wastewater treatment plants (WWTP) is discharged into springs, which are often used for water supply, irrigation, recreation and, further downstream, indirect potable reuse, quite common in Brazil. This study aimed to elucidate, regarding microbiological quality, the performance of a full-scale WWTP, consisting of preliminary treatment, upflow anaerobic sludge blanket (UASB) reactor, activated sludge system and ultraviolet (UV) radiation disinfection. Pathogenic protozoa (Giardia spp. cysts and Cryptosporidium spp. oocysts), as well as microbiological indicators (Escherichia coli and Clostridium perfringens), were evaluated in terms of their removal. In addition, (oo)cyst viability and fluorescence reduction were assessed. By using the data obtained from this research, the prevalence of infection estimated for the population served by the WWTP was between 7.4% and 14.8% for giardiasis, and between 0.055% and 0.11% for cryptosporidiosis.

Introdução: Oacompanhamento odontológico das gestantes pode propiciar inúmeros benefícios para a mãe e o bebê. Entretanto, ainda não são todas as gestantes que têm acesso ao pré-natal odontológico no Brasil, embora a atenção à saúde de forma integral seja uma prioridade no período gestacional. Objetivo: Identificar os aspectos gerais da atenção à saúde bucal das gestantes brasileiras atendidas na Atenção Básica de saúde disponíveis nas publicações científicas. Metodologia: Trata-se de um estudo de revisão integrativa da literatura. A busca dos artigos foi realizada via Medline, Lilacs, BBO e SciELO, sem restrição de idiomas, entre 2010 e 2020. Resultados: Foram selecionados 17 artigos científicos, cujo conteúdo foi categorizado em quatro conjuntos: Conhecimento das gestantes acerca da saúde bucal, Hábitos de higiene bucal das gestantes, Condição de saúde bucal das gestantes e Atenção à saúde bucal das gestantes. Existe desconhecimento de muitas gestantes sobre o que é o pré-natal odontológico e sua importância. Estudos apontam que algumas não são orientadas durante o pré-natal a realizar consultas com o cirurgião-dentista. As gestantes usam a escova e creme dental habitualmente, mas o fio dental não faz parte da rotina de higienização. Cárie e doença periodontal sãoos problemas bucais encontrados com maior frequência, principalmente entre gestantes que possuem menor renda e menor nível de escolaridade. Foi mais comum as gestantes estarem inseridas em ações de educação em saúde. Conclusão: Conclui-se que as gestantes atendidas na Atenção Básica do Sistema Único de Saúde possuem poucas informações sobre saúde bucal, convivem com os mitos odontológicos e apresentam medo de se submeter atratamentos odontológicos durante o período gestacional. Ademais, apresentam saúde bucal precária e encontram barreiras no acesso aos serviços de saúde bucal (AU).


Introduction:Dental care for pregnant women can provide many benefits for the mother and baby. However, not all pregnant women have access to dental prenatal care in Brazil, although comprehensive health care is a priority during pregnancy. Objective:Toidentify the general aspects of oral health care for Brazilian pregnant women attended in Primary Health Care available in scientific publications. Methodology:This is an integrative literature review study. The studiessearch wascarried out onMedline, Lilacs, BBO and SciELO,with no language restriction, from 2010 and 2020. Results:17 scientific articles were selected, whose content was categorized into four sets: Pregnant women 's knowledge of oral health, Pregnant women's oral hygiene habits, Pregnant women 's oral health condition and Pregnant women's oral health care. There is still a lack of knowledge among many pregnant women about what dental prenatal care is and its importance. Studies indicate that some are not advised during prenatal care to consult with the dentist. Pregnant women use the brush and toothpaste routinely, butdental floss is not part of routine oral hygiene. Caries and periodontal disease are the most frequent oral problems, especially among pregnant women who have a lower income and a lower level of education. It was more common for pregnant women to be inserted in health education actions. Conclusions:It is concluded that pregnant women attended in Primary Health Care of the Unified Health System have little information about oral health, live with dental myths and are afraid to dental treatment during pregnancy. In addition, they have poor oral health and find barriers to accessing oral health services (AU).


Introducción: El cuidado dental para mujeres embarazadas puede brindar muchos beneficios para la madre y el bebé. Sin embargo, no todas las mujeres embarazadas tienen acceso a la atención odontológica prenatal en Brasil, aunque la atención integral es una prioridad durante el embarazo. Objetivo: Identificar aspectos generales de la atención de salud bucal de las gestantes brasileñas atendidas en Atención Primaria de Salud disponibles en publicaciones científicas. Metodología: Es una revisión integradora de la literatura.La búsqueda de artículos se realizóvíaMedline, Lilacs, BBO y SciELO, sin restricción de idiomas, entre 2010 y 2020.Resultados: Se seleccionaron 17 artículos científicos, cuyo contenido se categorizó en cuatro conjuntos: Conocimientos de la mujer embarazada sobre salud bucal, Hábitos de higiene bucal de la mujer embarazada, Estado de salud bucal de la mujer embarazada y Atención a la salud bucal de la mujer embarazada. Aún existe desconocimiento entre muchas mujeres embarazadas sobre qué es el cuidado dental prenatal y su importancia. Los estudios indican que algunas mujeres no reciben orientación para consultar con el dentista durante el período prenatal. Las mujeres embarazadas usan el cepillo y la pasta de dientes con regularidad, pero el hilo dental no es parte de la higiene de rutina. La caries y la enfermedad periodontal son los problemas bucales más frecuentes, especialmente entre mujeres embarazadas que tienen menores ingresos y menor nivel de educación. Era más común que las embarazadas se insertaran en acciones de educación en salud. Conclusiones: Se concluye que las gestantes atendidas en Atención Primaria del Sistema Único de Salud tienen poca información sobre salud bucal, viven con mitos dentales y tienen miedo de recibir tratamientos dentales durante el embarazo. Además, tienen mala salud bucal y encuentran barreras para acceder a los servicios de salud bucal (AU).

ABSTRACT

Azithromycin is one of the drugs used in the combined therapy for syndromic treatment of gonorrhoea in many countries, including Brazil. Our research group, which receives isolates from clinical laboratories since 2006, has detected, after 2016, a tendency of rising rates of azithromycin resistance, with isolates showing higher minimal inhibitory concentrations (MICs) than those previously reported in this country. In this study, we report the susceptibility to azithromycin of 93 N. gonorrhoeae isolates obtained between 2014 and 2017. Strains with MIC ≥2 µg/mL were characterized according to azithromycin resistance mechanisms and strain typing. Results indicate that azithromycin resistance has emerged in all these years in unrelated MLST-STs, but after 2016 a clonal complex connected with ST1901 has been more frequently detected, grouping isolates with MIC varying from 2 to 64 µg/mL, with DelA mutations at the mtrR promoter region associated or not with mutations at rrl alleles. High rates of azithromycin resistance may compromise the use of this drug in the combined therapy with ceftriaxone. Inclusion of Rio de Janeiro in the Brazilian gonococcal surveillance program is important to evaluate if this data indicates an epidemiological phenomenon in the country.

The protozoa Giardia and Cryptosporidium are associated with numerous outbreaks of waterborne diseases worldwide. This study aimed to evaluate the concentration of Giardia spp. cysts, Cryptosporidium spp. oocysts, total coliforms, Escherichia coli and Clostridium perfringens in raw wastewater and their removals at UASB reactor, activated sludge system (operated conventionally and extended aeration) and slow sand filtration. Giardia spp. cysts were present in 100% and Cryptosporidium spp. oocysts in 31.4% of the analysed wastewater samples. The UASB reactor followed by activated sludge system obtained approximately 2.0 log of removal for total coliforms and E. coli, whereas for C. perfringens and Giardia spp. cysts, it obtained 1 log. There was a high percentage of (oo)cysts still viable after secondary treatment, therefore, the risk of contamination of water courses and, consequently, for public health is considerable. However, after tertiary filtration, no (oo)cysts were found in any of the filtered effluent samples, being a good option for future reuse. Seasonal variations did not affect the concentrations and removals of microorganisms observed. Lack of correlations of concentrations of indicator microorganisms and (oo)cysts raise caveats and doubts regarding the true microbiological quality when using only indicator microorganisms.

BACKGROUND: Complete tumor removal by transsphenoidal surgery is usually difficult for large nonfunctioning pituitary adenomas (NFPAs). A validated medical treatment may be useful for their management. This study evaluates the clinical efficacy of the dopaminergic agonist cabergoline for residual NFPA. DESIGN, SETTING, AND PARTICIPANTS: We conducted a randomized, parallel, open-label clinical trial that compared cabergoline with nonintervention in patients with residual NFPA after transsphenoidal surgery over 2 years. The primary outcome was clinical efficacy (tumor reduction). The secondary outcome was the relationship between tumor dopamine D2 receptor (D2R) expression and clinical responsiveness. Tumor measurements and clinical evaluations were performed every 6 months. RESULTS: In total, 59 and 57 individuals were randomly assigned to the study and control groups, respectively. At the end of the study, residual tumor shrinkage, stabilization, and enlargement were observed in 28.8%, 66.1%, and 5.1% of patients, respectively, in the medical-therapy group and in 10.5%, 73.7%, and 15.8% of patients, respectively, in the control group (P=0.01). The progression-free survival rate was 23.2 and 20.8 months for the study and control groups, respectively (P=0.01). D2R was not associated with cabergoline responsiveness. No major side effects were related to cabergoline use. CONCLUSIONS: Cabergoline was an effective drug for treating residual NFPA, and its use was associated with a high rate of tumor shrinkage (ClinicalTrials.gov NCT03271918).

Oxysterols are cholesterol oxygenated derivatives which possess several biological actions. Among oxysterols, 7-ketocholesterol (7KC) is known to induce cell death. Here, we hypothesized that 7KC cytotoxicity could be applied in cancer therapeutics. 7KC was incorporated into a lipid core nanoemulsion. As a cellular model the murine melanoma cell line B16F10 was used. The nanoparticle (7KCLDE) uptake into tumor cells was displaced by increasing amounts of low-density-lipoproteins (LDL) suggesting a LDL-receptor-mediated cell internalization. 7KCLDE was mainly cytostatic, which led to an accumulation of polyploid cells. Nevertheless, a single dose of 7KCLDE killed roughly 10% of melanoma cells. In addition, it was observed dissipation of the transmembrane potential, evidenced with flow cytometry; presence of autophagic vacuoles, visualized and quantified with flow cytometry and acridine orange; and presence of myelin figures, observed with ultrastructural microscopy. 7KCLDE impaired cytokenesis was accompanied by changes in cellular morphology into a fibroblastoid shape which is supported by cytoskeletal rearrangements, as shown by the increased actin polymerization. 7KCLDE was injected into B16 melanoma tumor-bearing mice. 7KCLDE accumulated in the liver and tumor. In melanoma tumor 7KCLDE promoted a >50% size reduction, enlarged the necrotic area, and reduced intratumoral vasculature. 7KCLDE increased the survival rates of animals, without hematologic or liver toxicity. Although more pre-clinical studies should be performed, our preliminary results suggested that 7KCLDE is a promising novel preparation for cancer chemotherapy.

In addition, it was observed dissipation of the transmembrane potential, evidenced with flow cytometry; presence of autophagic vacuoles, visualized and quantified with flow cytometry and acridine orange; and presence of myelin figures, observed with ultrastructural microscopy. 7KCLDE impaired cytokenesis was accompanied by changes in cellular morphology into a fibroblastoid shape which is supported by cytoskeletal rearrangements, as shown by the increased actin polymerization. 7KCLDE was injected into B16 melanoma tumor-bearing mice. 7KCLDE accumulated in the liver and tumor. In melanoma tumor 7KCLDE promoted a > 50% size reduction, enlarged the necrotic area, and reduced intratumoral vasculature. 7KCLDE increased the survival rates of animals, without hematologic or liver toxicity. Although more pre-clinical studies should be performed, our preliminary results suggested that 7KCLDE is a promising novel preparation for cancer chemotherapy.

Abstract Deforestation and road construction have inflicted negative effects on wildlife populations. To contribute to the comprehension and preservation of wildlife species, this study aimed to evaluate collared anteater presented to a reference center located in São Paulo state, Brazil. Furthermore, the locations where the animals were most frequently found were determined. Data on collared anteaters (Tamandua tetradactyla) examined at the Center for Medicine and Research in Wild Animals (CEMPAS) were evaluated retrospectively for a period of 13 years. A total of 46 collared anteaters was examined, comprised of 14 males, 10 females, and 22 without identifying information. On average, adult males weigh 4.41 kg while adult females weigh 4.95 kg. The most common admission circumstance was vehicular strikes (36.96%), followed by dog bites (21.74%); 23.91% were considered clinically healthy. Animal outcomes were 36.96% (n=17) reintroduced, 34.78% (n=16) fatalities, 15.22% (n=7) transferred to other institutions, and 6.52% (n=3) escaped from the enclosure; and 6.52% (n=3) presented no medical record information. The collared anteaters were found mainly in the municipal areas of Botucatu (21.73%), São Manuel (17.39%), Lençóis Paulista (6.52%), and Bauru (4.34%), with SP-300 being the main highway associated with the occurrences. In conclusion, the main cause of lesion in the collared anteaters evaluated in this study was vehicular strike that that led to death. However, including all collared anteaters at least half of them had a positive outcome.


Resumo O desmatamento e a construção de estradas têm promovido uma série de problemas à fauna. Visando contribuir com informações que podem ser usadas na compreensão e preservação das espécies, o trabalho teve por objetivo avaliar tamanduás-mirins atendidos em um centro de referência localizado no Estado de São Paulo, Brazil. Foram avaliados retrospectivamente dados referentes a tamanduás-mirins (Tamandua tetradactyla), que foram trazidos ao Centro de Medicina e Pesquisa em Animais Selvagens (CEMPAS), durante um período de 13 anos. De um total de 46 tamanduás-mirins atendidos, 14 foram machos e 10 foram fêmeas, e 22 não possuíam identificação quanto ao sexo. Em média, os machos adultos pesaram 4,41 kg, enquanto as fêmeas adultas pesaram 4,95 kg. Com relação à circunstância que levou ao atendimento, as mais importantes foram o atropelamento (36,96%) e a mordedura de cães (21,74%), sendo que 23,91% foram considerados clinicamente saudáveis. No tocante à evolução dos casos, 36,96% (n=17) foram reintroduzidos, 34.78% (n=16) foram a óbito, 15,22% (n=7) foram transferidos para outras instituições, 6,52% (n=3) evadiram do recinto e 6,52% (n=3) não há informação. Os animais foram provenientes principalmente dos municípios de Botucatu (21,73%), São Manuel (17,39%), Lençóis Paulista (6,52%) e Bauru (4,34%), sendo a SP-300 a principal rodovia relacionada as ocorrências. Foi possível concluir que dos tamanduás-mirins avaliados a principal causa de lesão foi o atropelamento, que teve por evolução o óbito; porém ao se incluir os tamanduás-mirins saudáveis e demais, ao menos metade evoluíram positivamente.

Papillary tumor of the pineal region (PTPR) is a neuroectodermal tumor thought to originate from cells of the subcommissural organ. Its oncologic properties are still under investigation, as well as the most suitable therapeutic measures for this type of neoplasm.We report the case of a 36-year-old woman with a 1-year history of headache and intermittent diplopia. The magnetic resonance imaging (MRI) scan showed a heterogeneously enhancing mass in the pineal region that caused an acute hydrocephalus, and an emergency shunt derivation was necessary. One week later, the patient was submitted to subtotal tumor resection, and remained asymptomatic in the post-operative period. In the follow-up, the patient remained asymptomatic; in the imaging control 3.5 years after the surgical resection, local recurrence was identified, and the patient was submitted to a local radiation protocol. Our literature review showed an early clinical onset due to intracranial hypertension signs. Definitive clinical onset might be reached only through a histopathological examination. Gross total resection followed by radiotherapy is the current standard of care. Local recurrence is often observed, with rare dissemination to the cerebral spinal fluid. The natural history of the PTPR remains unknown, as well as the best treatment strategy. Large case series with longer follow-ups are necessary for further conclusions.

ABSTRACT

BACKGROUND: Small cell lung cancer (SCLC) and high-grade extrapulmonary neuroendocrine carcinomas (EPNEC) share similar histopathological features and treatment, but outcomes may differ. We evaluated in our study the expression of biomarkers associated with response rate (RR) to chemotherapy and overall survival (OS) for these entities. MATERIALS AND METHODS: This is a multicentre retrospective analysis of advanced EPNEC and SCLC patients treated with platinum-based chemotherapy. Paraffin-embedded tumour samples were reviewed by a single pathologist and tested for immunohistochemistry (IHC) expression of Ki-67, ERCC1, Bcl-2, and Lin28a. All images were evaluated by the same radiologist and RR was determined by RECIST 1.1. RESULTS: From July, 2006 to July, 2014, 142 patients were identified, being 82 (57.7%) SCLC and 60 (42.3%) EPNEC. Clinical characteristics and median Ki-67 (SCLC: 60%; EPNEC: 50%; p = 0.86) were similar between the groups. RR was higher for SCLC patients (86.8% versus 44.6%; p<0.001), but median OS was similar (10.3 months in SCLC and 11.1 months in EPNEC; HR 0.69, p = 0.07). Bcl-2 expression was higher in SCLC patients (46.3% versus 28.3%, p = 0.03) and was associated with worse prognosis in EPNEC (median OS 8.0 months versus 14.7 months; HR 0.47, p = 0.02). CONCLUSION: EPNEC patients presented inferior RR to platinum-based chemotherapy than SCLC but tended to live longer. Neither ERCC1, Lin28, or Ki-67 were prognostic or predictive for RR in EPNEC or SCLC. High Bcl-2 expression was associated with poor prognosis in EPNEC patients.

Neuroendocrine tumours are a heterogeneous group of diseases with a significant variety of diagnostic tests and treatment modalities. Guidelines were developed by North American and European groups to recommend their best management. However, local particularities and relativisms found worldwide led us to create Brazilian guidelines. Our consensus considered the best feasible strategies in an environment involving more limited resources. We believe that our recommendations may be extended to other countries with similar economic standards.

ABSTRACT Introduction: Epstein-Barr virus (EBV) may serve as a target in therapeutic treatments, thus reliable diagnostic results are necessary. Objective: The aim of this study was to evaluate the accuracy of EBV detection by in situ hybridization (ISH) using five commercial probes in formalin-fixed and paraffin-embedded samples of nodular sclerosis Hodgkin's lymphoma (HL), and to compare the results with immunohistochemistry (IHC) and polymerase chain reaction (PCR). Material and method: Thirty samples were selected, 28 were lymph nodes, one bone marrow and one mediastinum. The following parameters were analyzed: signal intensity; proportionality of positive cells; quality of the reaction according to comfort for evaluation, sign quality and homogeneity of labeled cells; background reaction; morphology; presence of artifacts; and positivity in other non-neoplastic cells. All samples were analyzed for EBV detection using the five probes, IHC for latent membrane protein type 1 (LMP1) and PCR for Epstein Barr virus nuclear antigen 1 (EBNA1). Statistical analyses were performed with the R1 software; Fleiss' test and Cohen Kappa index of 5% were considered significant. Results: The detection by IHC-LMP1 was 26.7% (8/30) and 66.7% (20/30) by PCR-EBNA1. All probes detected EBV. Positivity was observed in 42/90 (46.7%), 38/90 (42.2%), 45/90 (50%), 27/90 (30%) and 61/90 (67.8%) for probes A, B, C, D and E, respectively. Discussion: All five probes demonstrated positivity. Conclusion: Probe E showed better rate (67.8%), sensitivity, specificity and accuracy (100%), a very good correlation among the different observers and with PCR, besides great cost-benefits relation.


RESUMO Introdução: O vírus Epstein-Barr (EBV) pode servir como alvo nos tratamentos terapêuticos, sendo necessário resultado diagnóstico confiável. Objetivo: Avaliar a acurácia da detecção do EBV pela hibridização in situ (ISH), utilizando cinco sondas comerciais em amostras fixadas em formalina e incluídas em parafina de linfoma de Hodgkin (LH) esclerose nodular, comparando os resultados com a imuno-histoquímica (IHQ) e a reação em cadeia pela polimerase (PCR). Material e método: Trinta amostras foram selecionadas, sendo 28 linfonodos, uma medula óssea e um mediastino. Os seguintes parâmetros foram analisados: intensidade do sinal; proporcionalidade das células positivas; qualidade da reação de acordo com o conforto na avaliação, qualidade do sinal e homogeneidade das células marcadas; reação de fundo; morfologia; presença de artefatos; e positividade em outras células não neoplásicas. Todas as amostras foram analisadas para a detecção do EBV usando as cinco sondas, IHQ para proteína da membrana latente tipo 1 (LMP1) e PCR para antígeno nuclear do EBV (EBNA1). As análises estatísticas foram realizadas com o software R1; os índices de 5% para Kappa de Fleiss e Cohen foram considerados significantes. Resultados: A detecção pela IHQ-LMP1 foi de 26,7% (8/30) e 66,7% (20/30) pela PCR-EBNA1. Todas as sondas detectaram EBV. A positividade foi observada em 42/90 (46,7%), 38/90 (42,2%), 45/90 (50%), 27/90 (30%) e 61/90 (67,8%) para as sondas A, B, C, D e E, respectivamente. Discussão: Todas as sondas demonstraram positividade. Conclusão: A sonda E mostrou melhor taxa (67,8%), sensibilidade, especificidade e precisão (100%), boa correlação entre os diferentes observadores e com a PCR, além de ótimo custo/benefício.

ABSTRACT

Gliomas are the most common type of primary brain tumors. The most aggressive type, Glioblastoma multiforme (GBM), is one of the deadliest human diseases, with an average survival at diagnosis of about 1 year. Previous evidence suggests a link between human cytomegalovirus (HCMV) and gliomas. HCMV has been shown to be present in these tumors and several viral proteins can have oncogenic properties in glioma cells. Here we have investigated the presence of HCMV DNA, RNA and proteins in fifty-two gliomas of different grades of malignancy. The UL83 viral region, the early beta 2.7 RNA and viral protein were detected in 73%, 36% and 57% by qPCR, ISH and IHC, respectively. Positivity of the viral targets and viral load was independent of tumor type or grade suggesting no correlation between viral presence and tumor progression. Our results demonstrate high prevalence of the virus in gliomas from Brazilian patients, contributing to a better understanding of the association between HCMV infection and gliomas worldwide and supporting further investigations of the virus oncomodulatory properties.

ABSTRACT

More precise methods are needed to recover Giardia and Cryptosporidium (oo)cysts from wastewater in order to advance research related to their inactivation, removal, quantification, and species differentiation. This study applied different methods to recover the maximum number of (oo)cysts from wastewater samples using ColorSeed®. Immunomagnetic separation assisted in capturing oocysts mainly in samples with medium and low turbidity. A triple centrifugation method reached recovery rates of 85% and 20%, for Giardia cysts and Cryptosporidium oocysts, respectively, in raw wastewater, and 62.5 and 17.5% in secondary-treated effluent. For low turbidity-treated effluent, membrane filtration reached 67.5% recovery for Giardia cysts and 22.5% for Cryptosporidium oocysts. Simple, quick and low-cost methods do not involve much handling of the samples and could be useful, particularly in developing countries.
