ABSTRACT

BACKGROUND: Websites serve as recruitment and educational tools for many fellowship programs, including neuroanesthesiology. Since the COVID-19 pandemic, when interviews, conferences, and institutional visits were moved online, websites have become more important for applicants when deciding on their preferred fellowship program. This study evaluated the content of the websites of neuroanesthesiology fellowship programs. METHODS: Neuroanesthesiology fellowship program websites were identified from the websites of the International Council on Perioperative Neuroscience Training and the Society for Neuroscience in Anesthesiology and Critical Care. The content was assessed against 24 predefined criteria. RESULTS: Fifty-three fellowship programs were identified, of which 42 websites were accessible through a Google search and available for evaluation. The mean number of criteria met by the 42 fellowship websites was 12/24 (50%), with a range of 6 to 18 criteria. None of the evaluated fellowship websites met all 24 predefined criteria; 20 included more than 50% of the criteria, whereas 7 included fewer than 30% of the criteria. Having a functional website, accessibility through a single click from Google, and a detailed description of the fellowship program were the features of most websites. Information about salary and life in the area, concise program summaries, and biographical information of past and current fellows were missing from a majority of websites. CONCLUSION: Important information was missing from most of the 42 evaluated neuroanesthesiology fellowship program websites, potentially hindering applicants from making informed choices about their career plans.

ABSTRACT

BACKGROUND AND OBJECTIVES: Due to current limitations in diagnosing chronic traumatic encephalopathy (CTE) clinically, traumatic encephalopathy syndrome (TES) has been proposed as the clinical presentation of suspected CTE. This study aimed to determine whether there was an association between a clinical diagnosis of TES and subsequent temporal decline in cognitive or MRI volumetric measures. METHODS: This was a secondary analysis of the Professional Athletes Brain Health Study (PABHS), inclusive of active and retired professional fighters older than 34 years. All athletes were adjudicated as TES positive (TES+) or TES negative (TES-) based on the 2021 clinical criteria. General linear mixed models were used to compare MRI regional brain volumes and cognitive performance between groups. RESULTS: A total of 130 fighters met inclusion criteria for consensus conference. Of them, 52 fighters (40%) were adjudicated as TES+. Athletes with a TES+ diagnosis were older and had significantly lower education. Statistically significant interactions and between-group total mean differences were found in all MRI volumetric measurements among the TES+ group compared with those among the TES- group. The rate of volumetric change indicated a significantly greater increase for lateral (estimate = 5,196.65; 95% CI = 2642.65, 7750.66) and inferior lateral ventricles (estimate = 354.28; 95% CI = 159.90, 548.66) and a decrease for the hippocampus (estimate = -385.04, 95% CI = -580.47, -189.62), subcortical gray matter (estimate = -4,641.08; 95% CI = -6783.98, -2498.18), total gray matter (estimate = -26492.00; 95% CI = -50402.00, -2582.32), and posterior corpus callosum (estimate = -147.98; 95% CI = -222.33, -73.62). Likewise, the rate of cognitive decline was significantly greater for reaction time (estimate = 56.31; 95% CI = 26.17, 86.45) and other standardized cognitive scores in the TES+ group. DISCUSSION: The 2021 TES criteria clearly distinguishes group differences in the longitudinal presentation of volumetric loss in select brain regions and cognitive decline among professional fighters 35 years and older. This study suggests that a TES diagnosis may be useful in professional sports beyond football, such as boxing and mixed martial arts. These findings further suggest that the application of TES criteria may be valuable clinically in predicting cognitive decline.

ABSTRACT

OBJECTIVE: To examine the characteristics of those who fulfil the recent National Institute of Neurological Disease and Stroke (NINDS) Consensus Diagnostic Criteria for Traumatic Encephalopathy Syndrome (TES) and test whether they show differences in MRI-based regional brain volumes, cognitive domains, and certain plasma biomarkers. METHODS: Professional fighters 35 years of age or older and/or retired were included. Participants were categorised as either having TES (TES+) or not (non-TES). TES+ participants were further subtyped by their cognitive profile. Multiple linear regression models were used to compare MRI-based regional brain volumes, cognitive performance, plasma tau and neurofilament light levels between TES- and TES+ groups. RESULTS: 176 participants (110 boxers and 66 MMA) were included in the analysis. 72 (41%)/176 were categorised as having TES, the likelihood of TES increasing with age. TES+ participants tended to be boxers, started fighting at a younger age, had more professional fights and knocked out more frequently. The TES+ group had lower regional brain volumes including both grey and white matter structures. TES+ also had lower scores on simple and choice reaction time, psychomotor speed and Trails A . CONCLUSION: The new TES criteria does distinguish a group of fighters with differences in regional brain volumes and reduced cognitive function. Our findings support the use of the NINDS criteria for TES in further research of the long-term effects of repetitive head impacts.

ABSTRACT

BACKGROUND: Drug development for neurodegenerative disorders (NDDs) is a long, complex, and expensive enterprise. Methods to optimize drug development for NDDs are needed. Basket trials have been widely used in oncology and have been promoted by the Food and Drug Administration as a means of enhancing the efficiency of drug development. DISCUSSION: We reviewed clinical trials for NDDs registered on clinicaltrials.gov in the past 10 years. We identified 59 basket trials assessing the impact of treatment on more than one NDD in the trial. Forty-one of the trials were for 25 agents addressing symptoms of NDD such as motor impairment, hypotension, or psychosis. Eighteen of the trials assessed 14 disease-modifying therapies; the principal targets were mitochondrial function, tau biology, or alpha-synuclein aggregation. Basket trials are most common in phase 2 but have been conducted in phase 1, phase 3, and phase 4. The duration and size of the basket trials are highly variable depending on their developmental phase and the intent of the trial. Parkinson's disease was the most common disorder included in basket trials of symptomatic agents, and Alzheimer's disease was the most common disorder included in basket trials of disease-modifying therapies. Most of the basket trials of symptomatic agents were sponsored by pharmaceutical companies (29 of 41 trials); similarly, most of the basket trials investigating DMTs in basket trials were sponsored by the biopharmaceutical industry (11/17 trials). CONCLUSIONS: Basket trials may increase drug development efficiency by reducing redundancy in trial implementation, enhancing recruitment, sharing placebo groups, and using biomarkers relevant to the mechanism of action of the treatment across NDDs. There have been relatively few basket trials including multiple NDDs in the same trial conducted over the past 10 years. The use of the basket trial strategy may represent an opportunity to increase the efficiency of development programs for agents to treat NDDs.

ABSTRACT

The oenological industry has benefited from the use of Nuclear Magnetic Resonance (1H-NMR) spectroscopy in combination with Multivariate Statistical Analysis (MSA) as a foodomics tool for retrieving discriminant features related to geographical origins, grape varieties, and further quality controls. Said omics methods have gained such attention that Intergovernmental Organizations and Control Agencies are currently recommending their massive use amongst countries as quality compliances for tracking standard and degradation parameters, fermentation products, polyphenols, amino acids, geographical origins, appellations d'origine contrôlée and type of monovarietal strains in wines. This study presents, for the first time, a 1H-NMR/MSA profiling of industrial Mexican wines, finding excellent statistical features to discriminate between oenological regions and grape varieties with supervised Orthogonal Projections to Latent Structures Discriminant Analysis (OPLS-DA). In a comparative way, it is applied with the 1H-NMR/OPLS-DA workflow for the first time in ancestral and artisanal Mexican mezcals with promising results to discriminate between regions, agave species and manufacturing processes. The central aim of this comparative study is to extrapolate the know-how of wine-omics into the non-professionalized mezcal industry for establishing the NMR acquisition, preprocessing and statistical analysis basis to implement novel, non-invasive and highly reproducible regional, agave species and manufacturing-quality controls.

ABSTRACT

Present work comprises the use of different solid-state Nuclear Magnetic Resonance strategies for characterizing structural and motional aspects of the peptide matrix that compose a set of four lyophilized Mexican cheese aqueous soluble extracts, each with a controlled ripening. Heteronuclear dipolar coupling modulation schemes allowed to characterize local mobility and structural homogeneity of cheeses' peptide segments in the solid-state as a function of ripening. Results suggest that ripened samples with certain local flexibility but important structural homogeneity present efficient microbial inhibition against tested bacterial strains, whilst high local rigidity of peptides within ripened cheese soluble fractions could partially explain the observed lack of antimicrobial activity. The present study attempts to propose novel observables for lyophilized cheese water soluble extracts that could be partially associated to their ripening-dependent antimicrobial activities, whereas said observables shall contribute to the better targeting, design and optimization of solid-state natural food bio-preservatives.

ABSTRACT

Poro cheese is a regional product originally from the area of Los Rios, Tabasco in Mexico. In the context of preserving the heritage of Poro cheese and protecting the specific characteristics that define its typicity through an origin designation, the present study was conducted to establish a general profile of Poro cheese by characterizing their physicochemical, textural, rheological, sensorial and microbiological characteristics. Differences in moisture, proteins, fats, NaCl, titrable acidity, pH, color texture and rheology amongst cheese factories were observed and ranges were established. Fifteen descriptors were generated to provide a descriptive analysis, eight of which were significantly different amongst the factories with no differences in the global acceptability of cheese. The favorite cheese had the highest scores for aroma attributes. Conventional and molecular methods were used to identify the main microorganisms, for which Lactobacillus plantarum, L. fermentum, L. farciminis and L. rhamnosus were the main microorganisms found in Porocheese. The obtained data constituted the parameters for characterizing Poro cheese, which will strongly help to support its origin appellation request process.

ABSTRACT

The multiple elements which characterize the teaching-learning process demand appropriate analysis techniques that enable a simultaneous examination of the many variables pertaining to that process. The statistics techniques in the Multivariate Analysis provide the adequate procedures for that purpose. This work presents a descriptive, longitudinal and prospective study as a follow up of another study which was published in 2007 in the Cuban Journal of Medical Informatics, and it was done taking into account the results obtained in evaluating the students from Nursing (Plan “D”) from “10 de Octubre” Faculty, Havana Medical College, after receiving the 5 subjects which constitute Informatics discipline applied to medical sciences, from the academic year 2006-2007 to 2008-2009. The main objective was to describe the behaviour of the academic results. Three main components were detected: Biostatistics, Computing and Qualitative Investigation. ABSTRACT

For years, the aspects related with the checking and evaluation of the students' academic results and the goals for their education have been the subject of many strong debates among scientists, professors and academics all over the world. This research shows an experience which was carried out in "10 de Octubre" Faculty, Havana School of Medical Sciences from the academic year 2004-2005 to 2007-2008, concerning the evaluation in the subjects that make up the curriculum in Informatics for Medical Sciences, specifically in the Nursingcareer, aiming to identify concepts and skills presenting higher degrees of difficulties in the teaching-learning process and also to evaluate the students final results on each subject. To achieve these objectives, some procedures were suggested, one of them being the Cluster Analysis(AU)

The multipurpose elements that typify the learning process require of proper analysis techniques, able to allow the simultaneous exam of the multiple variables included in the process. The statistic techniques of the Multivariate Analysis allow adequate procedures to these purposes. This paper is a descriptive, prospective, longitudinal study, carried out of the evaluations on the Infirmary Licentiate career in the 10 de Octubre faculty of the Superior Institute of Medical Sciences of Havana on the students that went through the five subjects that match the discipline Informatics and Investigation in Health during the courses 2004-2007, with the general purpose of making a characterization of the Docency evaluations in this discipline.A Factorial Analysis was performed to detect the existent relationships in the compound of evaluative variables and to abridge them to a lesser number without losing the original's explicative capacities. It was also performed a Cluster's Analysis to classify the students in accordance to their rendering. Three main components were detected: Biostatistics, Computation and Investigation Methodology and there was a validation in the relationship between the obtained result in the Final subject of the Health's Informatics discipline and its precedents
