ABSTRACT

Aim: To evaluate the effect of a new Fe-cyclam complex on pathogenic bacterial species, including multidrug-resistant clinical specimens. Materials & methods: The complex [Fe(cyclam)ox]PF6 (D2) was tested in cytotoxicity and MIC tests. Clinical and reference strains of Gram-negative and Gram-positive bacteria were used. Considering Staphylococcus aureus strains, the profile of antimicrobial susceptibility and time-kill kinetics for D2 was performed. An in silico analysis for D2 was also performed. Results: D2 showed broad bacterial activity, mainly against specimens of Cutibacterium acnes, S. aureus, Pseudomonas aeruginosa and Acinetobacter baumannii. Low cytotoxicity in human cells was demonstrated. Conclusion: The tested compound proved to be a promising agent against resistant bacterial infections.

ABSTRACT

A pandemia de COVID-19 trouxe grandes desafios aos gestores do Sistema Único de Saúde (SUS), tais como a manutenção de abastecimento de medicamentos utilizados para intubação orotraqueal para pacientes graves, denominado como Kit Intubação (KI). A gestão da assistência farmacêutica (AF) precisou de estratégias inovadoras para a manutenção do abastecimento do KI. Este artigo tem como objetivo descrever a experiência da AF no estado mais populoso do Brasil nas ações relacionadas ao abastecimento do KI durante a pandemia. Trata-se de um relato de experiência sobre a gestão realizada para abastecimento de 22 medicamentos nos estoques de hospitais públicos de administração direta e indireta, sob gestão estadual e municipal. Dentre as principais ações realizadas pela Coordenadoria de Assistência Farmacêutica da Secretaria de Estado da Saúde de São Paulo, temos: gestão do abastecimento e elaboração de relatórios gerenciais; centralização da logística de distribuição de medicamentos; gestão de compras nacionais e internacionais, gestão documental, financeira e de estratégias de comunicação. Conclui-se que as ações gerenciais da AF foram fundamentais para o sucesso do monitoramento e a manutenção de estoques hospitalares no momento de escassez do KI assim como trabalho em equipe e articulação junto aos gestores da AF. The COVID-19 pandemic has posed a significant challenge for managers of the Bra-zilian Unified Health System (SUS), particularly in ensuring an adequate supply of tracheal intubation medicines, commonly referred to as the intubation kit (IK). The availability and distribution of these drugs are crucial for the treatment of critically ill patients. To address this issue, the Pharmaceutical Assistance's Services and Policies (PASP) management has implemented innovative strategies for the supply of IK. This article aims to describe the experience of PASP in the most populous state of Brazil in the actions taken to manage the supply of IK during the peak of the COVID-19 pandemic. This management report focuses on 22 medicines sto-cked in publichospitals under the direct management of the State or City Council, with some of these hospitals being managed through contractual arrangements with social healthcare organizations.The coordination of PASP, supervised by the Sao Paulo State Health Department, employed various methods, including supply and distribution management, daily management reports from hospitals as part of the State Contingency Plan, centralized logistics for IK distribution, a document mana-gement system to track IK orders, the importation of medicines from abroad, and the management of documents, finances, and strategic communications. In conclu-sion, the actions taken to improve PASP management were crucial in monitoring the availability of IK during the shortage of medicines.The success of these efforts was greatly dependent on teamwork and effective coordination among all PASP managers involved. Cystic fibrosis (CF) is a genetic disease that results in dysfunction of the CF transmembrane conductance regulator (CFTR) protein, which is a chloride and bicarbonate channel expressed in the apical portion of epithelial cells of various organs. Dysfunction of that protein results in diverse clinical manifestations, primarily involving the respiratory and gastrointestinal systems, impairing quality of life and reducing life expectancy. Although CF is still an incurable pathology, the therapeutic and prognostic perspectives are now totally different and much more favorable. The purpose of these guidelines is to define evidence-based recommendations regarding the use of pharmacological agents in the treatment of the pulmonary symptoms of CF in Brazil. Questions in the Patients of interest, Intervention to be studied, Comparison of interventions, and Outcome of interest (PICO) format were employed to address aspects related to the use of modulators of this protein (ivacaftor, lumacaftor+ivacaftor, and tezacaftor+ivacaftor), use of dornase alfa, eradication therapy and chronic suppression of Pseudomonas aeruginosa, and eradication of methicillin-resistant Staphylococcus aureus and Burkholderia cepacia complex. To formulate the PICO questions, a group of Brazilian specialists was assembled and a systematic review was carried out on the themes, with meta-analysis when applicable. The results obtained were analyzed in terms of the strength of the evidence compiled, the recommendations being devised by employing the GRADE approach. We believe that these guidelines represent a major advance to be incorporated into the approach to patients with CF, mainly aiming to favor the management of the disease, and could become an auxiliary tool in the definition of public policies related to CF.

A fibrose cística (FC) é uma doença genética que resulta em disfunção da proteína reguladora de condutância transmembrana da FC (CFTR), que é um canal de cloro e bicarbonato expresso na porção apical de células epiteliais de diversos órgãos. A disfunção dessa proteína resulta em manifestações clínicas diversas, envolvendo primariamente os sistemas respiratório e gastrointestinal com redução da qualidade e expectativa de vida. A FC ainda é uma patologia incurável, porém o horizonte terapêutico e prognóstico é hoje totalmente distinto e muito mais favorável. O objetivo destas diretrizes foi definir recomendações brasileiras baseadas em evidências em relação ao emprego de agentes farmacológicos no tratamento pulmonar da FC. As perguntas PICO (acrônimo baseado em perguntas referentes aos Pacientes de interesse, Intervenção a ser estudada, Comparação da intervenção e Outcome [desfecho] de interesse) abordaram aspectos relativos ao uso de moduladores de CFTR (ivacaftor, lumacaftor + ivacaftor e tezacaftor + ivacaftor), uso de dornase alfa, terapia de erradicação e supressão crônica de Pseudomonas aeruginosa, e erradicação de Staphylococcus aureus resistente a meticilina e do complexo Burkholderia cepacia. Para a formulação das perguntas, um grupo de especialistas brasileiros foi reunido e realizou-se uma revisão sistemática sobre os temas, com meta-análise quando aplicável. Os resultados encontrados foram analisados quanto à força das evidências compiladas, sendo concebidas recomendações seguindo a metodologia GRADE. Os autores acreditam que o presente documento represente um importante avanço a ser incorporado na abordagem de pacientes com FC, objetivando principalmente favorecer seu manejo, podendo se tornar uma ferramenta auxiliar na definição de políticas públicas relacionadas à FC.

ABSTRACT

Microparticulate systems such as microparticles, microspheres, microcapsules or any particle in a micrometer scale (usually of 1-1000 µm) are widely used as drug delivery systems, because they offer higher therapeutic and diagnostic performance compared to conventional drug delivery forms. These systems can be manufactured with many raw materials, especially polymers, most of which have been effective in improving the physicochemical properties and biological activities of active compounds. This review will focus on the in vivo and in vitro application in the last decade (2012 to 2022) of different active pharmaceutical ingredients microencapsulated in polymeric or lipid matrices, the main formulation factors (excipients and techniques) and mostly their biological activities, with the aim of introducing and discussing the potential applicability of microparticulate systems in the pharmaceutical field.

The compound 3a,10b-dihydro-1H-cyclopenta[b]naphtho[2,3-d]furan-5,10-dione (IVS320) is a naphthoquinone with antifungal and antichagasic potential, which however has low aqueous solubility. To increase bioavailability, inclusion complexes with ß-cyclodextrin (ßCD) and methyl-ß-cyclodextrin (MßCD) were prepared by physical mixture (PM), kneading (KN) and rotary evaporation (RE), and their in vitro anti-SARS-CoV-2 and antichagasic potential was assessed. The formation of inclusion complexes led to a change in the physicochemical characteristics compared to IVS320 alone as well as a decrease in crystallinity degree that reached 74.44% for the IVS320-MßCD one prepared by RE. The IVS320 and IVS320-MßCD/RE system exhibited anti-SARS-CoV-2 activity, showing half maximal effective concentrations (EC50) of 0.47 and 1.22 µg/mL, respectively. Molecular docking simulation suggested IVS320 ability to interact with the SARS-CoV-2 viral protein. Finally, the highest antichagasic activity, expressed as percentage of Tripanosoma cruzi growth inhibition, was observed with IVS320-ßCD/KN (70%) and IVS320-MßCD/PM (72%), while IVS320 alone exhibited only approximately 48% inhibition at the highest concentration (100 µg/mL).

INTRODUCTION: Germ-cell tumors (GCTs) are the most common malignancy in young men. There is a paucity of data on GCTs in developing countries. LACOG 0515 study aimed to evaluate clinical characteristics and treatment outcomes in patients with GCTs from Brazilian cancer centers. MATERIALS AND METHODS: This is a retrospective cohort study evaluating male patients diagnosed with GCTs from 2000 to 2018 in 13 Brazilian hospitals. We described baseline characteristics, progression-free survival (PFS), and overall survival (OS). RESULTS: A total of 1232 patients were included, with a median age of 30 years. Histology was seminoma in 47.1% and non-seminoma GCT (NSGCT) in 52.9%. The primary tumor site was testis in 96.5%. At diagnosis, clinical stage I was present in 68.1% and 34.7% and clinical stages IS/II/III in 31.9% and 65.2% of patients with seminoma and NSCGT, respectively. Following orchiectomy, 55.2% of patients with clinical stage I were managed with surveillance. The 5-year disease-free survival rates among patients with stage I were 98.0% in seminoma and 92.3% in NSGCT, with 5-year OS of 99.6% and 97.6%, respectively. Among patients with advanced disease (IS, II, and III), the 5-year PFS were 88.7% in seminoma and 68.7% in NSGCT, with 5y-OS of 97.6% and 82.8%, respectively. CONCLUSION: This is the largest Brazilian cohort of GCTs. Our results show a high rate of adjuvant chemotherapy in patients with clinical stage I. Although our data demonstrate slightly inferior PFS compared with the International Germ Cell Cancer Collaborative Group and other contemporary series, the OS rates were similar.

ABSTRACT

Over the last decades, accumulating evidence has shown tumor-dependent profiles of miR-708, being either up- or downregulated, and thus, acting as a "Janus" regulator of oncogenic pathways. Herein, its functional duality was assessed through a thorough review of the literature and further validation in silico using The Cancer Genome Atlas (TCGA) and Gene Expression Omnibus (GEO) databases. In the literature, miR-708 was found with an oncogenic role in eight tumor types, while a suppressor tumor role was described in seven cancers. This double profile was also found in TCGA and GEO databases, with some tumor types having a high expression of miR-708 and others with low expression compared with non-tumor counterparts. The investigation of validated targets using miRBase, miRTarBase, and miRecords platforms, identified a total of 572 genes that appeared enriched for PI3K-Akt signaling, followed by cell cycle control, p53, Apellin and Hippo signaling, endocrine resistance, focal adhesion, and cell senescence regulations, which are all recognized contributors of tumoral phenotypes. Among these targets, a set of 15 genes shared by at least two platforms was identified, most of which have important roles in cancer cells that influence either tumor suppression or progression. In a clinical scenario, miR-708 has shown to be a good diagnostic and prognosis marker. However, its multitarget nature and opposing roles in diverse human tumors, aligned with insufficient experimental data and the lack of proper delivery strategies, hamper its potential as a sequence-directed therapeutic.

ABSTRACT

The relationship between food and human health drives the search for knowledge of food components that are related to these benefits. The scientific community shows a growing interest in the knowledge of the interactions between components of citrus fruits and probiotics to develop ways to improve the quality of the food produced. In this bibliometric review, a study of scientific publications is carried out on the potential of probiotics in citrus fermentation, addressing the importance and future trends of plant-based products in the functional food group as an alternative to the dairy market. The review process of the articles initially took place with a bibliometric analysis and was followed by a literature review. The Scopus database was used in the search for articles, carried out in May 2021. The use of foods as carriers of probiotics is an alternative that has been growing and the surveys evaluated show the desire to diversify the probiotics available on the market. In addition, it was observed that citrus fruits have great potential for the development of functional foods due to their high acceptability and possibilities of development and application in various products.

In this study, films of chitosan and 2-amino-4,5,6,7-tetrahydrobenzo[b]thiophene-3-carbonitrile (6CN), a 2-aminothiophene derivative with great pharmacological potential, were prepared as a system for a topical formulation. 6CN-chitosan films were characterized by physicochemical analyses, such as Fourier-transform infrared spectroscopy (FTIR), differential scanning calorimetry (DSC), thermogravimetric analysis (TGA), X-ray diffraction (XRD), and scanning electronic microscopy (SEM). Additionally, the antifungal potential of the films was evaluated in vitro against three species of Candida (C. albicans, C. tropicalis, and C. parapsilosis). The results of the FTIR and thermal analysis showed the incorporation of 6CN in the polymer matrix. In the diffractogram, the 6CN-chitosan films exhibited diffraction halos that were characteristic of amorphous structures, while the micrographs showed that 6CN particles were dispersed in the chitosan matrix, exhibiting pores and cracks on the film surface. In addition, the results of antifungal investigation demonstrated that 6CN-chitosan films were effective against Candida species showing potential for application as a new antifungal drug.

ABSTRACT

Astaxanthin (AST) is a biomolecule known for its powerful antioxidant effect, which is considered of great importance in biochemical research and has great potential for application in cosmetics, as well as food products that are beneficial to human health and medicines. Unfortunately, its poor solubility in water, chemical instability, and low oral bioavailability make its applications in the cosmetic and pharmaceutical field a major challenge for the development of new products. To favor the search for alternatives to enhance and make possible the use of AST in formulations, this article aimed to review the scientific data on its application in delivery systems. The search was made in databases without time restriction, using keywords such as astaxanthin, delivery systems, skin, cosmetic, topical, and dermal. All delivery systems found, such as liposomes, particulate systems, inclusion complexes, emulsions, and films, presented peculiar advantages able to enhance AST properties, among which are stability, antioxidant potential, biological activities, and drug release. This survey showed that further studies are needed for the industrial development of new AST-containing cosmetics and topical formulations.

ABSTRACT

Herein we provide a taxonomic revision of Phyllodoce species from Brazil, describing 10 new species in addition to two new records, Phyllodoce longipes Kinberg, 1866 and Phyllodoce cf. madeirensis Langerhans, 1880. Phyllodoce sp. A. and Phyllodoce sp. B. are probably new but the number and condition of available specimens do not provide adequate and reliable diagnostic features for a formal description. These species have been collected in diverse marine and estuarine environments from shallow estuarine bottoms to continental shelf and slope sediments and submarine canyons from off southern and southeastern Brazil. A key to the fourteen species of Phyllodoce occurring in Brazil is also given.

ABSTRACT

The recovery of valuable materials from waste fits the principle of circular economy and sustainable use of resources, but contaminants in the waste are still a major obstacle. This works proposes a novel approach to recover high-purity phosphorus (P) and nitrogen (N) from digestate of municipal solid waste based on the combination of two independent membrane processes: electrodialytic (ED) process to extract P, and gas permeable membranes (GPM) for N extraction. A laboratory ED cell was adapted to accommodate a GPM. The length of waste compartment (10 cm; 15 cm), current intensity (50 mA; 75 mA) and operation time (9 days; 12 days) were the variables tested. 81% of P in the waste was successfully extracted to the anolyte when an electric current of 75 mA was applied for 9 days, and 74% of NH4+ was extracted into an acid-trapping solution. The two purified nutrient solutions were subsequently used in the synthesis of a biofertilizer (secondary struvite) through precipitation, achieving an efficiency of 99.5%. The properties of the secondary struvite synthesized using N and P recovered from the waste were similar to secondary struvite formed using synthetic chemicals but the costs were higher due to the need to neutralize the acid-trapping solution, highlighting the need to further tune the process and make it economically more competitive. The high recycling rates of P and N achieved are encouraging and widen the possibility of replacing synthetic fertilizers, manufactured from finite sources, by secondary biofertilizers produced using nutrients extracted from wastes.

ABSTRACT

Anionic peptides of scorpions are molecules rich in aspartic and/or glutamic acid residues and correspond to a class of peptides without disulfide bonds that are still little explored. TanP is a linear anionic peptide (50 amino acid residues and net charge -20) present in the venom gland of the scorpion, Tityus stigmurus, with chelating properties for Cu2+ ion and immunomodulatory properties. The therapeutic application of chelating molecules is related to cases of acute or chronic intoxication by metals, neurodegenerative diseases, hematological diseases, healing of skin wounds, cardiovascular diseases, and cancer. In this approach, the chelating activity of TanP was evaluated in relation to new metal ions (Fe2+ and Zn2+) of biological importance, as well as its antioxidant, hemostatic, immunomodulatory, and healing potential, aiming to expand the biological and biotechnological potential of this peptide. TanP (25 µM) was able to form stable complexes with Fe2+ in a ratio of 1:5 (TanP: Fe2+). Theoretical results suggest that TanP can work as a sensor to identify and quantify Fe2+ ions. The fluorescence intensity of TanP (1.12 µM) decreased significantly after the addition of Fe2+, obtaining the highest ratio 1: 7.4 (TanP: Fe2+) that led to the lowest fluorescence intensity. For Zn2+, no relevant spectral change was noted. TanP (50 µM) showed a maximum of 3% of hemolytic activity, demonstrating biocompatibility, as well as exhibiting a 1,1-diphenyl-2-picrylhydrazyl radical-scavenging activity of above 70% at all the concentrations tested (1-25 µM), and 89.7% iron-chelating activity at 25 µM and 96% hydroxyl radical-scavenging activity at 73.6 µM. In addition, TanP (12.5 and 25 µM) revealed an anticoagulant effect, prolonging the clotting time in prothrombin time and activated partial thromboplastin time assays, with no fibrinogenolytic activity. TanP (12.5 and 25 µM) induced the release of TNF-α by murine macrophages, in the absence of lipopolysaccharides, with a concentration-dependent increase and also stimulated the migration of 3T3 cells in the in vitro healing assay. Thus, TanP revealed a multifunctional potential, being useful as a prototype for the development of new therapeutic and biotechnological agents.

Objetivo: Analisar a produção do cuidado da equipe de enfermagem aos usuários de álcool e outras drogas na Rede de Atenção Psicossocial. Método: Estudo exploratório e descritivo de abordagem qualitativa, desenvolvido com 11 profissionais da equipe de enfermagem atuantes em três serviços de saúde da Rede, localizados num município do interior da Bahia, Brasil. Os dados foram coletados entre dezembro de 2017 e fevereiro de 2018 utilizando-se da entrevista semiestruturada. Resultados: Os resultados apontam para um acesso dos usuários a Rede a partir de encaminhamentos e referências; as atividades de acolhimento têm sido desenvolvidas apenas pelos Centros de Atenção Psicossocial e o vínculo tem se demonstrado como frágil. Conclusão: Conclui-se que o cuidado de enfermagem na Rede tem se desenvolvido de modo fragmentado e pontual, logo, dispositivos como acesso, acolhimento e vínculo devem ser trabalhados com toda equipe de saúde em prol de uma assistência integral

Objective: To analyse the production of care provided by the nursing team to users of alcohol and other drugs in the Psychosocial Care Network. ABSTRACT

IVS320 (3a,10b-dihydro-1H-cyclopenta[b]naphtho[2,3-d]furan-5,10-dione) is a naphthoquinone that has low solubility in aqueous medium, a physical behavior that limits its biological activities, considering that compounds from this class have several activities. In this work, solid dispersions (SDs) prepared between IVS320 and polymers hydroxypropyl methylcellulose (HPMC), polyethylene glycol (PEG), and polyvinylpyrrolidone (PVP) were developed using physical mixture (PM), kneading (KN), and rotary evaporation (RE) methods. Dispersions were investigated using Fourier transform infrared spectroscopy (FTIR), differential scanning calorimetry (DSC), thermogravimetry (TG), powder X-ray diffraction (PXRD), and scanning electron microscopy (SEM). In addition, in vitro antiparasitic activity in Trypanosoma cruzi Y strains was evaluated. Physical-chemical characterization demonstrated the formation of SDs through the interaction of IVS320 with polymeric matrices. SDs of IVS320-polymer presented a significant potentiation of antichagasic activity, with inhibitory growth around 62% (IVS320-HPMC/RE), 55% (IVS320-PEG/RE), and 85% (IVS320-PVP/RE), while pure IVS320 showed a value of 48% for the highest concentrations evaluated (50 µg/mL).

ABSTRACT

This review aims to provide an overview of studies that address the use, in therapeutic applications, of solid dispersions (SDs) with biological activities in vitro and/or in vivo mainly made up of polymeric matrices, as well as to evaluate the bioactive activity of their constituents. This bibliographic survey shows that the development of solid dispersions provides benefits in the physicochemical properties of bioactive compounds, which lead to an increase in their biological potential. However, despite the reports found on solid dispersions, there is still a need for biological assay-based studies, mainly in vivo, to assist in the investigation and to devise new applications. Therefore, studies based on such an approach are of great importance to enhance and extend the use of solid dispersions in the most diverse therapeutic applications.

Objetivo: Conhecer as dificuldades que mães cuidadoras de crianças com microcefalia vivenciaram desde a descoberta até experimentarem as práticas de cuidado. Método: Estudo qualitativo, descritivo e exploratório, realizado com mães de crianças com microcefalia nas cidades de Campo Formoso e Senhor do Bonfim-Ba, cujas análises se deram por método de análise de conteúdo. Resultados: O conteúdo analisado foi agrupado nas seguintes categorias: Vivenciando o medo; Assumindo responsabilidades: é hora de cuidar; As especificidades do cuidado; O desafio do acesso ao serviço de saúde e Deslocamento: problema que gera problema. Considerações Finais: O estudo permitiu perceber que os familiares enfrentam diversos tipos de dificuldades, estas surgem no momento da revelação da microcefalia e se estendem no cotidiano familiar, entretanto, mesmo diante das adversidades existentes, as mães cuidadoras se empenham para ofertar o melhor cuidado possível à sua criança.


Objective: To know the difficulties that mothers who care for children with microcephaly experienced since the discovery until they tried the care practices. Method: Qualitative, descriptive and exploratory study, conducted with mothers of children with microcephaly in the cities of Campo Formoso and Senhor do Bonfim-Ba, whose analyzes were carried out by content analysis method. Results: The analyzed content was grouped into the following categories: Experiencing fear; Taking responsibility: it's time to take care; The specifics of care; The challenge of access to health services and displacement: a problem that creates a problem. Final Considerations: The study made it possible to perceive that the family members face several types of difficulties, these arise at the moment of the disclosure of microcephaly and extend into the family routine, however, even in the face of existing adversities, the caregiving mothers strive to offer the best possible care to your child.


Objetivo: Conocer las dificultades que experimentaron las madres que cuidan a niños con microcefalia desde el descubrimiento hasta que probaron las prácticas de cuidado. Método: Estudio cualitativo, descriptivo y exploratorio, realizado con madres de niños con microcefalia en las ciudades de Campo Formoso y Senhor do Bonfim-Ba, cuyos análisis se realizaron mediante el método de análisis de contenido. Resultados y discusión: El contenido analizado se agrupó en las siguientes categorías: Experimentar miedo; Asumir la responsabilidad: es hora de cuidarnos; Los detalles de la atención; El desafío del acceso a los servicios de salud y el desplazamiento: un problema que genera un problema Consideraciones finales: el estudio nos permitió darnos cuenta de que los miembros de la familia enfrentan diferentes tipos de dificultades, que surgen en el momento de la divulgación de la microcefalia y se extienden a la rutina familiar, sin embargo, incluso Ante las adversidades existentes, las madres que se preocupan se esfuerzan por brindar la mejor atención posible a sus hijos.
