ABSTRACT

Introduction: although nutritional differences between different types of texture-modified diet (TMD) have been evaluated, the resources and costs associated with their preparation have been less studied. Objective: to describe the nutritional, microbiological properties and costs of: 1) in-home produced pureed food (hTMD); 2) concentrated nutrient-dense commercial food products, hand-blended (cTMD); 3) food prepared using the MixxPro® automatic food mixer (cTMD-Mix). Methods: an observational, prospective study carried out in three geriatric nursing-homes. Patients ≥ 65 years, receiving TMD, with a stable clinical condition, estimated survival/expected internment > 1 month, and sufficient cognitive capacity were included. The following data were recorded: 1) patient socio-demographic and clinical variables; 2) TMD compliance and symptoms related to dysphagia during the meal; 3) patient appetite; and 4) kitchen information and resources used to prepare a TMD. Results: sixty-two residents were included (65.0 % women, 88.3 years (SD: 9.3); 43.5 % malnourished, 79.0 % with good appetite). The proportion of food eaten/median kcal served/portion/mean kcal consumed were: hTMD: 95.5 % (SD: 10.7)/92.4 kcal (IQR: 75.6-128.1)/88.2 kcal (IQR: 72.2-122.3); cTMD: 89.2 % (SD: 15.9)/323.4 kcal (IQR: 284.2-454.1)/288.5 kcal (IQR: 253.5-325.1); and cTMD-Mix: 80.3 % (SD: 21.4)/358.0 kcal (IQR: 344.0-372.1)/287.5 kcal (IQR: 276.5-298.8). No microorganisms were detected. The average time spent in preparing each portion and its costs were: hTMD: 11.2 min (SD: 3.89)/€2.33 (SD: 0.63); cTMD: 1.7 min (SD: 0.28)/€2.01 (SD: 0.39); and cTMD-Mix: 1.6 min (SD: 0.00)/€2.00 (SD: 0.33). Introducción: aunque existe evidencia acerca de las diferencias nutricionales entre los distintos tipos de dieta de textura modificada (DTM), los recursos y los costos asociados a su preparación se han estudiado menos.Objetivo: describir las propiedades nutricionales, las microbiológicas y los costes de: 1) una dieta triturada de manera artesanal (hDTM); 2) una dieta preparada con alimentación básica adaptada (ABA) (cDTM); y 3) una ABA preparada con el mezclador automático de alimentos MixxPro® (cDTM-Mix). ABSTRACT

Objetivos. El método One step nucleic acid amplification (OSNA) se ha incorporado para el estudio del ganglio centinela (GC) en cáncer de mama como alternativa al estudio convencional histológico (MC). El propósito de nuestro estudio fue comparar la estadificación por ganglio centinela (EGC) obtenida por el método OSNA con la obtenida mediante MC. Material y métodos. Se seleccionaron pacientes con cáncer de mama y EGC recogidas durante los años 2009-2010 y 2012-2013, estudiadas con MC y método OSNA. Se analizaron diferentes parámetros clínico-patológicos. Resultados. Se incluyó a 1.124 pacientes, 590 estudiadas por MC y 534 por método OSNA. La EGC inicial fue: pN0: MC 349 (59,2%) y OSNA 335 (62,7%); pN0(i+): MC 74 (12,5%) y OSNA 14 (2,6%); pN1mi: MC 59 (10%) y OSNA 77 (14,4%); pN1: MC 108 (18,3%) y OSNA 108 (20,3%). Se encontraron diferencias estadísticamente significativas entre la EGC por método OSNA y MC (p<0,001), a expensas de las tasas de pN1mi y pN0(i+). Se seleccionó a 224 pacientes con EGC pN1mi y pN0(i+) para determinar si las diferencias encontradas podrían atribuirse a distintas características clínico-patológicas. El método OSNA detecta el doble de micrometástasis (84,6%). Conclusiones. En nuestra casuística, por el método OSNA se observa un incremento significativo de pN1mi (84,6% vs. 44,4%) y una disminución de pN0(i+) respecto al estudio convencional, diferencias que no están condicionadas por los parámetros clínico-patológicos. El 75% de casos con pN1mi por OSNA muestra un número de copias inferior a 1.000 (AU)


Objetives. The One Step Nucleic Acid Amplification (OSNA) method has been incorporated in the study of the sentinel lymph node (SLN) in breast cancer as an alternative to conventional histological study. The aim of our study was to compare sentinel lymph node staging (SLNS) obtained by the OSNA method with that obtained by the conventional method (CM). Material and methods. We identified patients with breast cancer and SLN study during the periods 2009-2010 and 2012-2013, who underwent the CM and by OSNA. We analysed different clinicopathological parameters. Results. A total of 1124 patients were studied, 590 by CM and 534 by OSNA. SLNS was: pN0: CM 349 (59.2%) and OSNA 335 (62.7%); pN0(i+): CM 74 (12.5%) and OSNA 14 (2.6%); pN1mi: CM 59 (10%) and OSNA 77 (14.4%); pN1: CM 108 (18.3%) and OSNA 108 (20.3%). Statistically significant differences were found between the SLNS by OSNA and CM (p <0.001), due to the rates of pN1mi and pN0(i+). To determine whether this statistical significance could be attributed to different clinicopathological features, 224 patients were selected from the initial series with SLN pN1mi and pN0(i+). In this subgroup, the OSNA method detected twice as many micrometastases (pN1mi) (84.6%). Conclusions. In our series, the OSNA method resulted in a significant increase in pN1mi (84.6% vs 44.4%) and a decrease in pN0(i+) compared with the conventional method. Those differences were not affected by clinicopathological parameters. Most cases (75%) with pN1mi by OSNA showed less than 1000 copies (AU)

ABSTRACT

Our objective was to evaluate the usefulness of proton magnetic resonance spectroscopy ((1)H MRS) in categorizing brain tumours. In vivo single-voxel (1)H MRS at an echo time of 136 ms was performed in 108 patients with brain neoplasms that included 29 meningiomas (MEN), 15 low-grade astrocytomas (LGA), 12 anaplastic astrocytomas (AA), 25 glioblastomas (GBM) and 27 metastases (MET). Time-domain fitted areas of nine resonances were evaluated in all spectra. Twenty-five additional tumours were prospectively included as independent test set. Differences in at least two resonances were found in all pairwise comparisons of tumour groups except in GBM vs MET. Large lipid resonance at 1.30 ppm was found to be characteristic of GBM and MET, and alanine was characteristic of MEN. Significant differences were found between LGA and AA in choline-containing compounds and total creatine resonances. When implemented in a stepwise algorithm, these findings correctly classified 84% (21 of 25) tumours in the independent test set. Some additional utility was found in glycine/myo-inositol at 3.55 ppm for bilateral differentiation between GBM and MET (9 of 11, 82% correct classification in the test set). (1)H MRS provides useful information to categorize the most common brain tumours that can be implemented in clinical practice with satisfactory results.
