Conversation with Bruce Ritson.

In this occasional series we record the views and personal experience of people who have especially contributed to the evolution of ideas in the journal's field of interest. Dr Bruce Ritson was formerly Consultant Psychiatrist at the Royal Edinburgh Hospital and Senior Lecturer at Edinburgh University. He currently chairs the medical campaign group, Scottish Health Action on Alcohol Problems (SHAAP), which is hosted by the Royal College of Physicians of Edinburgh. He has had a long and fruitful research career.

Alcohol licensing laws: proposals for changes in Scottish law.

Scotland, England and Wales, and Ireland have each recently reviewed how licensing laws might influence the increasing levels of alcohol-related problems in each of these countries. Each legislature has arrived at somewhat different recommendations. Scotland may move towards liberalization, albeit within firm guidelines. It is unknown whether emphasis on local review, server training and some restrictions on bar venues offering discount pricing, will be sufficient to alter current trends in alcohol-related problems.

Durante los últimos cincuenta años los servicios especializados en problemas relacionados con el alcohol han experimentado una profunda transformación. En este texto se reseña brevemente cuál ha sido la evolución de los centros de tratamiento en el Reino Unido. Actualmente, la eficacia de los tratamientos a pacientes externos y la atención domiciliaria goza de excelente reputación. Este modelo de servicios implica una serie de derivaciones que incluyen la necesidad de formar y apoyar a los trabajadores de los servicios de atención primaria asegurándose, al mismo tiempo, de que no se pierda la red de tratamientos especializados, que son los que se dirigen a individuos con necesidades complejas. Finalmente, los cambios tienen que llevarse a cabo asumiendo que la respuesta social a los problemas relacionados con el alcohol debe ser mucho más amplia (AU)

Gemeindebezogene Alkoholinitiativen und kommunale Maßnahmen

Europa ist ein bedeutender Alkoholhersteller und -verbraucher, und alkoholbedingte Probleme wirken sich erheblich auf die Bereiche Gesundheit, Soziales und Wirtschaft aus. Trotz dieser Kosten betrachten viele Gemeinden die Alkoholproblematik weiterhin als rein medizinisches bzw. überwiegend psychiatrisches Thema. Gemeindenahe Programme und lokale Maßnahmen aller Art können gesunde Lebensweisen maßgeblich fördern und öffentliche und politische Unterstützung für Veränderungen in bezug auf den Alkoholhandel und -konsum sicherstellen. Lokale Maßnahmen dienen nicht nur der Entwicklung der Kommunalpolitik, sondern können auch gewaltigen Einfluß auf die nationale und sogar auf die internationale Politik ausüben. In jeder Gemeinde bestehen Möglichkeiten für Präventivmaßnahmen. Die Familie und die Schule, die Arbeitswelt und die Einrichtungen der Gesundheitsversorgung bieten Gelegenheit, zu gesundheitsbewußtem Verhalten anzuregen, die soziale Unterstützung zu verbessern und Einstellungen zu fördern, die eine Verringerung des Alkoholkonsums begünstigen. Die hier vermittelten Orientierungshilfen sind im wesentlichen praxisbezogen und enthalten Handlungsansätze, die unterschiedlichen Umfeldern angepaßt werden können. Sie eignen sich als Grundlage für die spezifischen Methoden und Erfahrungen der jeweiligen Gemeinde. Sie sollen das lokale Interesse und die Erarbeitung eines detaillierten lokalen Aktionsplans anregen. In den einzelnen Kapiteln der vorliegenden Veröffentlichung werden spezifische Bereiche des kommunalen Lebens angesprochen, in denen gute Chancen zur Entwicklung einer effektiven Alkoholpolitik bestehen.

La lutte contre l’alcoolisme au niveau associatif et municipal

L’Europe est l’un des principaux producteurs et consommateurs d’alcool. Or, les problèmes liés à l’alcool ont un impact important sur la santé, le bien-être social et l’économie. Malgré ce coût, de nombreuses collectivités continuent de considérer l’alcool comme un problème strictement médical ou psychiatrique. Les programmes axés sur la collectivité et les mesures de toutes sortes prises à l’échelon local sont autant de moyens qui permettent de favoriser un mode de vie sain et d’obtenir le soutien du public et des autorités pour modifier les conditions de vente et de consommation de l’alcool. L’action locale peut exercer une influence considérable sur les politiques menées non seulement à l’échelon municipal, mais aussi à l’échelon national et international. Chaque collectivité peut prendre des mesures préventives. Les ménages, les écoles, les entreprises et les établissements de santé sont autant de lieux où il est possible d’encourager l’adoption de comportements sains, de renforcer l’aide sociale et de conforter les attitudes favorables à une modération de la consommation. Les orientations présentées ici se veulent essentiellement pratiques et les approches évolutives proposées pourront aider chaque collectivité à concevoir des méthodes adaptées à sa situation. Le but du présent ouvrage est de stimuler l’intérêt des autorités locales et d’encourager, à ce niveau, l’élaboration d’un plan d’action détaillé. On y aborde notamment certains domaines de la vie quotidienne où il est très possible d’élaborer des stratégies efficaces de lutte contre l’alcoolisme.

Community and municipal action on alcohol

Europe is a major producer and consumer of alcohol, and alcohol-related problems make a major impact on health, social welfare and the economy. Despite these costs, many communities continue to regard alcohol problems solely as a medical or even a narrowly psychiatric issue. Community programmes and local action of all kinds are one important way of supporting healthier lifestyles, as well as of securing public and political support for modifying the sale and use of alcohol. In addition to the development of local policy, local action can exert a powerful influence on national and even international policy. Every community has the potential for preventive action. Homes, schools, workplaces and health care establishments offer opportunities to encourage healthy behaviour, improve social support and strengthen attitudes that favour lighter drinking. These guidelines are essentially practical and contain approaches that can be adapted to different settings. They can serve as a template around which to build the methods and experiences that are unique to particular communities. They are designed to stimulate local interest and the creation of a detailed local action plan. The book comprises chapters that address specific areas of community life where there are good prospects for developing effective alcohol policies.
