ABSTRACT

Tonsillectomy is one of the more frequent surgical techniques in the otolaryngological practice. In the first century A. D., Cornelius Celsus in Rome described the removal of the tonsils. Since them, devices used for tonsillectomy included a wide variety of instruments as snares, guillotines, scalpels ("cold" and ultrasonic), forceps and lasers. Likewise, the improvement of the anesthetic procedures and the perioperative management have contributed greatly to success with this surgical procedure. To sum up, the instruments and procedures used for tonsillectomy have evolved to render it a precise operation.

ABSTRACT

La amigdalectomía es una de las técnicas quirúrgicas más habituales en la práctica otorrinolaringológica. Esta técnica operación fue ya descrita en el siglo I de nuestra era por el romano Cornelius Celsus. Desde entonces han sido empleados para realizarla los más variados instrumentales como asas de alambre, guillotinas, bisturís (“fríos” y ultrasonidos), pinzas (mono y bipolares) y laceres. Igualmente, la mejoría en las técnicas anestésicas y los cuidados perioperatorios han contribuido al incremento del éxito de esta cirugía. Como conclusión, los instrumentos y las técnicas para la realización de amigdalectomía han evolucionado hasta convertirla en una operación de gran precisión

In this paper we carried out an immunohistochemical study of cyclin D1 (DCS6 ) expression in a series of 195 patients with laryngeal carcinoma that were diagnosticated, treated and followed at the Department of Otolaryngology at "Virgen de la Salud" Hospital (Toledo, Spain) for a time of 5 years. In the cases with lymph node metastasis we also studied cyclin D1 expression at this level. Furthermore we have analysed the value of cyclin D1 expression as a prognostic factor (tumor recurrence, deads due to cancer and survival) and we evaluate the relationship between cyclin D1 expression and other clinic and pathologic parameters.

ABSTRACT

En este trabajo hemos estudiado la expresión inmunohistoquímica de la proteína ciclina D1 (DCS6) en una serie de 195 pacientes con cáncer de laringe que fueron diagnosticados, tratados y seguidos durante 5 años en el servicio ORL del Hospital «Virgen de la Salud» (Toledo). En los casos que presentaron metástasis ganglionares analizamos también la expresión de ciclina DI a dicho nivel. Además se estudia el valor pronóstico de la expresión de ciclina D1 en este tipo de tumores (desarrollo de recidivas, mortalidad por cáncer de laringe y supervivencia) y se analiza la posible relación de dicha expresión con otros parámetros clínico-patológicos

ABSTRACT

OBJECTIVE: To determine the utility of prophylactic antibiotics in non-risk pediatric patients undergoing adenoidectomy. METHODS: We performed a prospective, controlled, randomized, and double-blind study on patients under 14 years of age, scheduled for adenoidectomy who accomplished the following criteria: absence of immunosuppressive and/or cardiovascular risk factors, no antimicrobial therapy for at least 15 days prior to operation, and no fever 1 week before surgery. Venous blood samples for culture were obtained at 30s and 20 min after the curettage of adenoidal tissue. Likewise, immediate and delayed complications were registered in all cases. The usefulness of prophylaxis was analyzed according to three major standpoints: bacteremia, immediate complications, and delayed complications. RESULTS: One-hundred one patients fulfilled the inclusion criteria and were included in the study. Fifty-one children received prophylaxis and the remainder did not. In the non-prophylactic group incidence of bacteremia at 30s was significantly higher than in the prophylactic group (32.7% versus 4.0%) (p<0.001). Neither bacteremia at 20 min, nor immediate or delayed complications showed statistical differences between both treatment groups. CONCLUSIONS: Preoperative antimicrobial prophylaxis in pediatric adenoidectomy did not offer advantages preventing complications in non-risk patients. Only bacteremia that occurs 30s after the curettage of adenoid tissue is reduced with the employment of prophylactic antibiotics.

ABSTRACT

OBJECTIVE: Adenoidectomy is frequently performed in children. The goals of this work were to determine the incidence of bacteremia during the adenoidectomy, to identify the microorganisms implicated, and to analyze the possible association of bacteremia with postoperative complications. METHODS: One hundred pediatric patients operated of adenoidectomy without preoperative antibiotic prophylaxis were prospectively included in this study. They had no immunosuppressive and/or cardiovascular risk factors, no antimicrobial therapy for at least 15 days prior to operation, and no fever 1 week before surgery. Venous blood samples were obtained 30 s and 20 min after the curettage of adenoidal tissue. RESULTS: The 33 and 14% of the samples were positive at 30 s and 20 min. The organism more commonly isolated was the streptococcus viridans. Bacteremia was only related to postoperative acute otitis media (p=0.012). CONCLUSIONS: Bacteremia exists after pediatric adenoidectomy, although it seldom correlates with clinical signs or symptoms. Postoperative acute otitis media is the only complication related to postsurgical bacteremia.

ABSTRACT

One of the Spain's pioneer out patient surgery units is the Toledo's one. We have realized a retrospective study of the 10th first years of its functioning. During this period, 4733 patients were operated, on tonsills and adenoids, laryngeal microsurgeries, septoplasties, miringotomies, and miringoplasties. We obtained a 91.8% of discharge from hospital in the same day and few complications (5.5% without hospitalization and 0.7% with). Thanks to a questionary we know that the degree of satisfaction of the patients is high, and so, added to our good statistic results, stimulate us to follow in this working line.

ABSTRACT

Una de las unidades de cirugía mayor ambulatoria pioneras de Espala, es la de Toledo. Hemos realizado un estudio retrospectivo de los 10 primeros años de su funcionamiento. En este lapso, se han intervenido 4733 pacientes, realizándose cirugías de amígdalas y adenoides, microcirugías laringeas, septoplastias, miringotomías y miringoplastias. Hemos obtenido un 91,8 por ciento de altas en el día y escasas complicaciones (5,5 por ciento sin hospitalización, 0,7 por ciento con ingreso). Gracias a una encuesta, sabemos que el grado de satisfacción del usuario es alto, lo que, sumado a nuestros buenos resultados estadísticos, nos estimulan a seguir en esta línea de trabajo (AU)

ABSTRACT

Five cases of malignant melanomata of the nasal cavities have been diagnosed and studied at ENT--and Anatomopathological Departments, between 1990 and 1997, the group ranging from 65 to 90 years, middle age 74.2. Paramount symptoms were epistaxis and lack of pain in all patients. Radical surgery was performed only in one case, followed by radiotherapy; the remainder were submitted to palliative treatment. We have reviewed the literature and also an immunohistochemical analysis about p53, p16INK4 and Rb oncogenes with the aim to establish its importance in these tumor's type. The whole group have showed high levels of p53 and MIB-1 and 60 percent a loss of oncogen p16 expression.

A descriptive, retrospective study was made of 110 cases of laryngeal neoplasm diagnosed at the Hospital Virgen de la Salud of Toledo, Spain, between 1988 and 1991. Cases were studied by sex, age, alcohol use, smoking, first symptom, and time to treatment.

ABSTRACT

A statistical study is made of the results of 78 stapedectomies carried out during the last six years in the Virgen de la Salud Hospital of Toledo, Spain. An auditive gain of 90%, averaging 27 dBs, was found. Other parameters assessed were age, sex, prosthesis type, and failures.
