ABSTRACT

Sporotrichosis is a subcutaneous mycosis caused by traumatic inoculation into the skin by fungi species of the genus Sporothrix. The disease has different clinical manifestations (cutaneous, lymphocutaneous, and disseminated), and can also progress to a systemic infection. Despite having a worldwide distribution, sporotrichosis is most prevalent in tropical and subtropical countries. In Brazil, reports of the disease are higher frequent, where cases of the disease were found in Rio de Janeiro, Sao Paulo, Curitiba, Pernambuco, and Paraiba, among others. Certain groups of people may be more exposed to the causative agent of disease, such as residents of rural areas. Thus, this work aimed to carry out a seroepidemiological survey of the prevalence of sporotrichosis in four rural locations in the south of Minas Gerais State, Brazil. In this study, we used an indirect ELISA test in the survey on the prevalence of sporotrichosis. Data obtained in this study evaluated a population of 631 individuals and showed a prevalence of 44.69%. The distribution of seroprevalence of sporotrichosis with respect to age groups and gender showed no significant statistical difference. Thus, we found a high seroprevalence of sporotrichosis-infection in rural regions of southern Minas Gerais State, Brazil, with no difference in prevalence in relation to gender and age.

ABSTRACT

INTRODUCTION: A primary cutaneous CD-30 positive T -cell lymphoproliferative disorders are rare and heterogeneous group of primary skin tumors, which include primary cutaneous anaplastic large cell lymphoma (PCALCL) and lymphomatoid papulois. CASE REPORT: We report a rare an atypical case of PCALCL with an aggressive and refractory behavior that occurred in the lip vermilion, and that had been initially diagnosed as aggressive herpes. The lesion was recurrent and refractory to the CHOEP chemotherapy protocol followed by radiotherapy, but after hyper CVAD (acronym) + brentuximab with a considerable improvement. After 4 cycles of hyper CVAD plus brentuximab, the patient underwent 100% compatible sibling bone marrow transplantation, with success in the procedure. CONCLUSION: Primary cutaneous anaplastic large cell lymphoma is a rare disease of difficult diagnosis and may be confused with chronic infectious diseases, postponing treatment.

ABSTRACT

AIMS: to describe a case in which dental changes were observed and investigation proceeded to consider Enamel-Renal-Syndrome (ERS), a rare disorder that associates amelogenesis imperfecta with nephrocalcinosis. CASE REPORT: an 11-year-old male patient upon intraoral examination revealed generalized gingival hyperplasia, a few teeth were absent clinically and the remaining ones were yellowish-brown in color. The enamel alterations were suggesting of amelogenesis imperfecta. Unerupted teeth with increased pericoronal spaces, suggesting hyperplasic follicles or dentigerous cysts and an enamel with lower thickness and density were observed in the panoramic radiography. The patient was referred for an assay to investigate mucopolysaccharidosis; however, it was negative. A renal ultrasound showed bilateral nephrocalcinosis and laboratory exams, including calcium, phosphate, and creatinine levels were below the average. An incisional gingival biopsy showed numerous round to ovoid basophilic calcifications in the connective tissue. The final diagnosis was ERS. CONCLUSION: Dentists should refer patients with similar clinical presentation for renal ultrasound evaluation in order to rule out the possible diagnosis of ERS.

This study aimed to estimate the prevalence of paracoccidioidal infection by intradermal reaction (Delayed-Type Hypersensitivity, DTH) to Paracoccidioides brasiliensis in rural areas in Alfenas, Southern Minas Gerais (MG) State, Brazil, and to assess risk factors (gender, occupation, age, alcohol intake and smoking) associated with infection. We conducted a population-based cross-sectional study using intradermal tests with gp 43 paracoccidioidin in 542 participants, who were previously contacted by local health agents and so spontaneously attended the test. Participants underwent an interview by filling out a registration form with epidemiological data and were tested with an intradermal administration of 0.1 mL of paracoccidioidin in the left forearm. The test was read 48 hours after injection and was considered positive if induration was greater than or equal to 5 mm. Out of 542 participants, 46.67% were positive to the skin test. Prevalence increased in accordance with an increase of age. There was statistical significance only for males. Occupation, alcohol intake and smoking habits were not significantly associated with the risk of paracoccidioidomycosis infection. There is relevance of paracoccidioidomycosis infection in such rural areas, which suggests that further epidemiological and clinical studies on this mycosis should be done in the southern part of Minas Gerais State.

Este estudo teve como objetivo estimar a prevalência de sensibilização da pele pelo Paracoccidioides brasiliensis em áreas rurais em Alfenas, MG, Brasil, e avaliar os fatores de risco associados à infecção. Foi realizado um estudo transversal de base populacional utilizando testes intradérmicos com paracoccidioidina em 542 indivíduos selecionados por demanda espontânea. Os participantes foram submetidos a uma entrevista através do preenchimento de um formulário de inscrição com os dados epidemiológicos e os testes com a administração intradérmica de 0,1 mL de paracoccidioidina no antebraço esquerdo. O teste foi lido 48 h após a injeção e foi considerado positivo se enduramento era maior ou igual a 5 mm. De 542, 46,67% participantes foram positivos ao teste de pele. Prevalência aumentou de acordo com o aumento da idade. Houve significância estatística apenas para o sexo masculino. Profissão, alcoolismo e tabagismo não foram significativamente associados com o risco de infecção paracoccidioidomicose. Há relevância da infecção paracoccidioidomicose em áreas rurais, o que sugere mais estudos epidemiológicos e clínicos sobre esta micose no sul do estado de Minas Gerais.

ABSTRACT

A pericoronarite e o cisto paradentário são freqüentes e a compreensão das etiopatogenias, aspectos clínicos, radiográficos e microscópicos é necessária para um diagnóstico preciso e adoção de protocolos terapêuticos adequados. Para caracterizar a pericoronarite crônica comparando-a com o cisto paradentário e folículo pericoronário de dentes parcialmente irrompidos e estabelecer critérios de diferenciação anatomopatológica utilizou-se dos arquivos do Serviço de Anatomia Patológica da FOB-USP. Três grupos foram estabelecidos: 27 folículos pericoronários de dentes parcialmente irrompidos, 69 pericoronarites e 50 cistos paradentários. Aplicaram-se os testes de qui-quadrado, Kruskal- Wallis e o índice Kappa de concordância. As conclusões foram: 1. O folículo pericoronário de dente parcialmente irrompido pode ser diferenciado microscopicamente da pericoronarite e do cisto paradentário, pois nos folículos não se observa infiltrado inflamatório; 2. É possível estabelecer critérios para o diagnóstico diferencial de pericoronarite e cisto paradentário em lesões císticas bem constituídas. No cisto paradentário o infiltrado inflamatório quase sempre é intenso e difuso, com intensa exocitose; O revestimento será do tipo estratificado pavimentoso hiperplásico e desorganizado pela intensa migração celular inflamatória. A cavidade cística é contornada por parede espessa e ricamente infiltrada por leucócitos, com acúmulos de neutrófilos. No lume, tem-se presença de aglomerados bacterianos. Nas lesões incipientes é necessário associar os achados microscópicos com os dados clínicos, radiográficos e principalmente trans-cirúrgicos enviados, destacando-se a presença de cavidade e de líquido entre o folículo e a coroa; 3. A diferenciação microscópica entre folículo pericoronário de dente parcialmente irrompido, pericoronarite e cisto paradentário incipiente, do ponto de vista clínico, não traz benefícios clínicos imediatos, pois a forma de tratamento e a proservação obedecem ao mesmo protocolo de conduta; 4. A ausência de alterações neoplásicas nos espécimes examinados sugere que em lesões foliculares inflamatórias dificilmente ocorre esta transformação dos componentes epiteliais e conjuntivos, provavelmente, pela ação deletéria do processo inflamatório sobre os remanescentes embrionários do folículo pericoronário
