ABSTRACT

Bronchoalveolar lavage, bronchial brushing and transbronchial biopsy are of fundamental importance in the diagnosis of pathologies affecting the lungs of immunosuppressed patients, especially those infected with HIV. This was a descriptive and retrospective study, in which the results of bronchoalveolar lavages, bronchial brushings and transbronchial biopsies of HIV-positive patients attended at the Clinical Hospital of the Federal University of Triangulo Mineiro from 1999 to 2015 were reviewed to determine the most frequent findings in these patients, to evaluate the diagnostic accuracy of these procedures and to correlate bronchoscopy results with clinical and radiological findings. Serological tests for HIV were confirmed and cases with negative or unverified serology were excluded. Medical records were reviewed for correlation with clinical and radiological findings. A total of 1,423 patients with a mean age of 50 years were initially selected; 727 cases had no serology for HIV; 696 had serology for HIV and 64 were positive. Of these, 47 were men, aged 24 to 84 years, and 17 women, aged 31 to 69 years. Biopsies and cytological tests were positive in 20 (31.25%) of the 64 patients and the most frequent diagnosis was pneumocystosis, found in 8 cases (12.5%). Of the 20 bronchofibroscopy-positive patients, only 2 did not show agreement between histopathological and clinical-radiological diagnoses. The analysis of the cytological tests and biopsy specimens obtained by bronchofibroscopy seems to be valuable for the etiological diagnosis of pulmonary infections in HIV- positive patients; however, negative results do not always exclude the diagnosis. In these cases, clinical symptoms and imaging findings may help to guide the best therapy.

ABSTRACT

The hantavirus cardiopulmonary syndrome is considered an emerging disease in the Americas. Since 1993, thousands of cases have been reported from different countries, but mainly from Brazil. This study aims to describe some epidemiological, clinical and anatomopathological aspects of patients with hantavirus who presented poor outcome and were autopsied in a teaching hospital in Brazil, from 2000 to 2014. Of the 10 patients included, nine were male (mean age 43.5 years) and seven reported previous contact with rodents. Fever was present in eight of ten patients, dyspnea in nine of ten and myalgia in seven of ten patients; hemoconcentration, leukocytosis, thrombocytopenia and renal involvement were evidenced in all the 10 cases. At autopsy, the main alterations were seen in the lungs: pleural effusion (8/10 cases), increased weight 2.5 to 3 times, congestion/edema (10/10), interstitial mononuclear inflammation (10/10), alveolar hemorrhage (7/10), pulmonary collapse (7/10), hyaline membranes (7/10) and alveolar neutrophilic infiltrate (2/10). Pericardial effusion (2/10), mild myocardium inflammation (4/10), right ventricle dilation (1/10), polyploidy nuclei (3/10) and pericardial diffuse petechial (1/10) were also observed. The other organs exhibited discrete and non-specific alterations. Currently, this syndrome continues to be associated with high mortality directly linked to a late diagnosis and/or a misdiagnosis in the medical centers where these patients were seen for the first time. The anatomopathological findings at autopsy revealed the final phase of the process with pulmonary alterations, allowing a direct correlation with the severity of respiratory distress observed in these patients at admission.

Paracoccidioidomycosis is a systemic fungal disease associated with agricultural activities. Its incidence and prevalence are underestimated because of the lack of reporting in several Brazilian states. If paracoccidiodomycosis is not diagnosed and treated early and adequately, endemic fungal infection may result in serious sequelae. In addition to the Paracoccidioides brasiliensis (P. brasiliensis) complex, the appearance of a new species, Paracoccidioides lutzii (P. lutzii), in Rondônia state, where the disease has reached epidemic levels, and in the country's Midwest region and Pará state, are challenges to diagnosis and to the urgent availability of antigens that are reactive with patients' sera. These guidelines aim to update the first Brazilian consensus on paracoccidioidomycosis by providing evidence-based recommendations for bedside patient management. The guidelines provide data on etiology, epidemiology, immunopathogenesis, diagnosis, treatment and sequelae, with emphasis on diagnosis and treatment, as well as current recommendations and challenges in this field of knowledge.


A paracoccidioidomicose (PCM) é uma micose sistêmica, relacionada às atividades agrícolas, com incidência e prevalência subestimadas, pela ausência de notificação em várias Unidades da Federação (UFs). A evolução insidiosa do quadro clínico pode ter como consequência sequelas graves se o diagnóstico e o tratamento não forem instituídos precoce e adequadamente. Ao lado do complexo Paracoccidioides brasiliensis (P. brasiliensis), a descrição de nova espécie, Paracoccidioides lutzii (P. lutzii), em Rondônia, onde a doença alcançou níveis epidêmicos, bem como na região Centro-Oeste e no Pará, constituem-se em desafios para a instituição do diagnóstico e a urgente disponibilização de antígenos que tenham reatividade com os soros dos pacientes. Este consenso visa atualizar o primeiro consenso brasileiro em PCM, estabelecendo recomendações para o manejo clínico do paciente, com base nas evidências conhecidas. São apresentados dados de etiologia, epidemiologia, imunopatogenia, diagnóstico, terapêutica e sequelas, enfatizando-se o diagnóstico e a terapêutica, bem como recomendações e desafios atuais nessa área do conhecimento.

Al lado del complejo Paracoccidioides brasiliensis (P. brasiliensis), la descripción de nueva especie, Paracoccidioides lutzii (P. lutzii) en Rondonia, donde la enfermedad alcanzó niveles epidémicos, y en la región Centro Oeste y en Pará, se constituyen en desafíos para la institución del diagnóstico y la urgente puesta a disposición de antígenos que tengan reactividad con los sueros de los pacientes. El presente consenso tiene por objeto actualizar el primer consenso brasileño en paracoccidioidomicosis, estableciendo recomendaciones para el manejo del paciente al borde del lecho, con base en las evidencias conocidas. Abstract Paracoccidioidomycosis is a systemic fungal disease occurring in Latin America that is associated with rural environments and agricultural activities. However, the incidence and prevalence of paracoccidiodomycosis is underestimated because of the lack of compulsory notification. If paracoccidiodomycosis is not diagnosed and treated early and adequately, the endemic fungal infection could result in serious sequelae. While the Paracoccidioides brasiliensis ( P. brasiliensis ) complex has been known to be the causal agent of paracoccidiodomycosis, a new species, Paracoccidioides lutzii ( P. lutzii ), has been reported in Rondônia, where the disease has reached epidemic levels, and in the Central West and Pará. Accurate diagnoses and availability of antigens that are reactive with the patients' sera remain significant challenges. Therefore, the present guidelines aims to update the first Brazilian consensus on paracoccidioidomycosis by providing evidence-based recommendations for bedside patient management. This consensus summarizes etiological, ecoepidemiological, molecular epidemiological, and immunopathological data, with emphasis on clinical, microbiological, and serological diagnosis and management of clinical forms and sequelae, as well as in patients with comorbidities and immunosuppression. The consensus also includes discussion of outpatient treatments, severe disease forms, disease prevalence among special populations and resource-poor settings, a brief review of prevention and control measures, current challenges and recommendations.

ABSTRACT

Objetivo: Estudar as lesões anatomopatológicas no SNC, analisando se contribuíram para o óbito des s es pacientes e avaliar a concordância entr e os diagnósticos neuropatológ icos premortem e postmortem. Método: Estudo retrospectivo com análise de 90 necrópsias sequenciais de SIDA, realizadas entre 1989 e 1996, além de busca de dados epidemiológicos/clínicos e comparação com a suspeita clínica para avaliar concordância entre diagnóstico pre e postmortem. Resultados: Idade média 34 ± 11 anos; 81,1% do sexo masculino; todos apresentavam alguma alteração no SNC, sendo relevantes para o óbito em 54 (60%) casos. Apenas em 15 (27,8%) desses 54 casos houve s uspeita clínica da le s ão. As lesões encontr adas no SNC for am: toxoplasmos e (34,4%), c r i ptococo s e (12,2%), c itomega lovi r o s e (4,4%), encefa lite nodul a r m ic r og li a l (3,3%), mening ite bacter iana (2,2%), infecção pelo vír us JC (2,2%), tuberculos e/micobacter ios e (2,2%), histoplasmose (1,1%) e paracoccidioidomicose (1,1%). As alterações não relacionadas ao óbito (40%) for am: g lios e, fibros e meníngea focal, infar to antigo, calcificações e edema . Este estudo confirma que existem diversas alterações no SNC de pacientes com SIDA, sobretudo infecções oportunistas, frequentemente não suspeitadas clinicamente, evidenciando a importância da necrópsia. Esses achados sugerem que investigação minuciosa deveria ser feita no SNC de pacientes com SIDA, particularmente naqueles com diagnóstico tardio ou que não estão respondendo ao tratamento.


Agreement between premortem e postmortem neuropathological diagnoses in 90 patients with acquired immune deficiency syndrome, submitted to autopsy at a teaching hospital in Brazil. Objective: The aim was to study the anatomopathological lesions in CNS, analyzing if they have contributed to the death of those patients, and to evaluate the agreement between premortem and postmortem neuropathological diagnoses. Method: Retrospective study on 90 necropsies on patients with AIDS, performed between 1989 and 1996, besides having been performed the search for clinical and epidemiologycals datas, was done a comparison with the clinical suspicion, to analyze the concordance between premortem and postmortem diagnoses. Results: The patients? mean age was 34 ± 11 years, 81.1% were male. CNS lesions were found in all of the cases. In 54 (60%) cases, CNS lesions of relevance to the death were found, but there had only been clinical suspicion of such lesions in 15 (27.8%) of them. The lesions consisted of toxoplasmosis (34.4%), cryptococcosis (12.2%), cytomegalovirosis (4.4%), microglial nodular encephalitis (3.3%), bacterial meningitis (2.2%), JC virus infection (2.2%), tuberculosis/mycobacteriosis (2.2%), histoplasmosis (1.1%) and paracoccidioidomycosis (1.1%). The unrelated lesions with the death (40%) were: gliosis, meningeal fibrosis, old infarct, calcifications and edema. Conclusion: This study confirms that several changes occurred in the CNS of patients with AIDS, especially opportunistic infections, that frequently are not suspected clinically. These findings suggest that detailed investigation should be conducted on CNS of patients with AIDS, particularly those who are not responding to treatment or with late diagnoses.

É relatado caso de paracoccidioidomicose pulmonar em paciente masculino que evoluiu com pneumotórax espontâneo e morreu por disseminaçäo sistêmica um ano e dez meses após o diagnóstico. O paciente apresentava sorologia positiva para HIV. Os autores fazem breve revisäo da literatura desta associaçäo incomum.
