ABSTRACT

Objective To observe the changes of liver and kidney function in hepatitis B virus(HBV)carriers after renal transplantation. Methods A total of 116 patients with HBV infection undergoing renal transplantation and 348 counterparts without HBV infection were recruited in this clinical trial. The liver function parameters including alanine aminotransferase(ALT)and aspartate aminotransferase(AST)levels and renal function parameter including serum creatinine(Scr)level were measured before and at 1, 3, 6, 12, 18, 24, 36 and 60 months after renal transplantation. Preoperative and postoperative changes of liver and kidney function were statistical y compared between the hepatitis B surface antigen(HBsAg)(+)and HBsAg(–)groups. According to the results of preoperative HBV serology, preoperative quantitative detection of HBV DNA and preoperative liver function test, 116 HBsAg(+)patients undergoing renal transplantation were divided into(HBsAg, HBeAg and anti-HBc all positive)and(HBsAg, anti-HBe and anti-HBc all positive)groups, HBV DNA(+)and HBV DNA(–)groups, and normal and abnormal liver function groups.Preoperativeandpostoperativechangesofliverandkidneyfunctionwerestatisticalycomparedbetweendifferentsubgroups. Results(1)Preoperative ALT and AST levels in HBsAg(+)patients were significantly higher compared with those in their HBsAg(–)counterparts. In 36 months after renal transplantation, liver function parameters significantly differed between two groups(al P<0.05), whereas no statistical significance was noted at postoperative 60 months(al P>0.05). Before and in 60 months after renal transplantation, no statistical significance was observed in the Scr levels between the HBsAg(+)and HBsAg(–)groups(all P>0.05).(2)Before and in 60 months after renal transplantation, no statistical significance was observed in the liver and kidney function parameters between the(HBsAg, HBeAg and anti-HBc all positive)and(HBsAg, anti-HBe and anti-HBc all positive)groups, and HBV DNA(+)and HBV DNA(–)groups(all P>0.05).(3)The ALT levels before and at 1, 3, 6 and 12 months after renal transplantation significantly differed between the normal and abnormal liver function groups(al P<0.05), whereas no statistical significance was observed at other time points(all P>0.05). The AST levels before and at 1 month after renal transplantation significantly differed between two groups(both P<0.05), whereas did not significantly differ at alternative postoperative time points(all P>0.05). No statisticalsignificancewasobservedinthekidneyfunctionparametersbeforeandat60monthsfolowingrenaltransplantation between two groups(al P>0.05). Conclusions HBV infection cannot exert significant effect upon kidney function within 5 years after renal transplantation, whereas it can affect short-term postoperative liver function.

ABSTRACT

Objective To explore the associations between different HBeAg staus ,serum HBV DNA level and hepatic injury in patients with chronic hepatitis B (CHB) .Methods HBV DNA level and serum ALT were detected by quantitative Real‐time PCR and velocity methods separately .The degree of hepatic injury was determined by the level of ALT ,and the associations between HBV DNA level and hepatic injury both in HBeAg negative CHB and HBeAg positive CHB patients were analyzed accordingly .Re‐sults Elder age and lower HBV DNA level were found in HBeAg negative CHB patients than in HBeAg positive patients (P0 .05) .Serum HBV DNA level was posi‐tively correlated with the degree of hepatic injury in HBeAg negative CHB patients (P<0 .05) but no similar correlation was found in HBeAg positive patients .Conclusion Positive serum HBeAg is an effective indicator of HBV replication and no correlation is presented between HBV DNA level and intrahepatic inflammation in HBeAg positive CHB patients .However ,HBV DNA level is closely correlated with liver damages in HBeAg negative CHB patients ,more attention should be paid to clinical treatment and fol‐low‐up monitoring of HBeAg negative CHB patients .

ABSTRACT

Objective To investigate the value and clinical significance of CD4+CD25+ regulatory T cells (Treg) ,inteleukin‐18 , interferon‐γand transforming growth factor‐βin patients with hepatitis B virus (HBV) infection .Methods A total of 175 patients with HBV infection were divided into chronic type B hepatitis (CHB) group and chronic asympotomatic HBV carrier (ASC) group , which were further divided into hepatitis B e antigen (HBeAg) positive and negative groups .CD4+ CD25+ Treg ,cytokines levels and liver function were measured .Healthy subjects were enrolled into control group .Results Proportion of CD4+CD25+ Treg were without significant difference among healthy controls ,HBeAg(+ ) and HBeAg(-) ASC groups ,and HBeAg(+ ) and HBeAg(-) CHB groups (P>0 .05) .Compared with control group ,cytokines levels were significant higer in CHB and ASC group (P<0 .05) . CD4+CD25+ Treg level was significant positive correlation to alanine aminotransferase and ratio of aspartate aminotransferase to plateles in HBeAg(+ )ASC group (P<0 .05) .Conclusion CD4+CD25 + Treg and related cytokines could play important roles in the course of CHB ,while CD4+CD25+ Treg expressing might be correlated with inflammatory degree of hepatitis .

ABSTRACT

Objective To explore the curative effect and safety of of Entecavir ( ETV) combined Liuweiwul-ing Pian in the treatment of chronic hepatitis B patients with HBeAg +.Methods 124 patients according to the ran-dom number table were divided into the two groups ,which respectively used ETV combined Liuweiwuling Pian treat-ment and ETV.The liver function and index of viral replication of the two groups were compared .Results The effec-tiveness of the observation group was 91.94%,which of the control group was 82.26%,the difference between the two groups had statistical significance(Ridit=32.251,P=0.000);Since 24 week treatment,HBV DNA of the observa-tion group was lower than the control group,the difference was statistically significant (t =2.365,4.595,all P<0.05);After treatment,ALT,AST of the observation group decreasing degree was higher than the control group ,the difference was statistically significant (t=78.512,62.145,all P<0.05);after treatment,the ALT rate,effective rate and bounce rate in the two groups was statistically significant differences (χ2 =25.621,46.241,11.512, all P<0.05);After treatment,the HBV DNA and HBeAg positive in two groups was below cut-off test cases,differences was statistically significant(χ2 =17.265,21.264,all P<0.05).Conclusion Entecavir combined Liuweiwuling Pian in the treatment of chronic hepatitis B patients with HBeAg +can reduce liver fibrosis , and has obvious detoxification and anti-viral.
