ABSTRACT

An atypical Advancing Wavelike Epitheliopathy case, consecutive to topical treatment for a 360º Conjunctival Intraepithelial Neoplasia, is presented. Mitomycin (0.2 mg/mL) and interferon (1 MUI/mL) drops were used. An atypical presentation, with migrating limbal focus, non clearly delimited in its hourly site through its evolution. Treated with flurometholone drops plus artificial tears, working to complete resolution.

ABSTRACT

Se presenta un caso atípico de epiteliopatía progresiva en ondas consecutiva a tratamiento tópico de una neoplasia intraepitelial conjuntival en 360°. Se usaron colirios de mitomicina (0,2mg/ml) e interferón (1MUI/ml). Presentación atípica con foco limbar principal migratorio, y no claramente delimitado en su sitio horario a través de su evolución. Tratado con flurometolona y lágrimas artificiales, con resultado de resolución completa (AU)

ABSTRACT

OBJECTIVE: To assess if anterior segment optical coherence tomography (AS-OCT) is a noninvasive diagnostic method suitable to differentiate benign corneo-conjunctival lesions (pterygium) from premalignant lesions (corneo-conjunctival intraepithelial neoplasia, CIN). MATERIAL AND METHODS: An observational, analytical and cross-sectional study was conducted in 22 eyes with conjunctival lesions clinically suspicious for pterygium and CIN during two years. Morphological differences between both lesions were studied with AS-OCT; epithelial thicknesses (EE) and extension length on corneal surface (GIC) were compared between both groups. A surgical excision of the lesion was performed for histopathological diagnosis. RESULTS: Mean age of patients with pterygium (n=18) was 52.67±15 y.o and 74±12 y.o in subjects with CIN (n=4) (p<0.021). In pterygia, AS-OCT showed typical features (normal, thinning or slightly thickened EE; 77.4±26µm), in addition to an increase in wedge-shaped subepithelial tissue. Patients with CIN had a mean thickened EE (262.5±124µm) and strongly hyperreflective, with abrupt transition between normal and pathological epithelium. Analysis of EE between subjects with pterygium and CIN revealed statistically significant differences (p<0.002). ROC curve revealed a 100% sensitivity and specificity of OCT-SA in differentiation between CIN and pterygium, using 141µm as cutoff point of EE. CONCLUSION: AS-OCT is a useful tool for the differentiation between pterygium and CIN able to provide typical morphological characteristics. An EE greater than 141µm in AS-OCT suggests a sensitivity and specificity of 100% for the diagnosis of CIN.

OBJETIVO: identificar el patrón epidemiológico de la neoplasia intraepitelial de la conjuntiva en el Instituto Cubano de Oftalmología «Ramón Pando Ferrer¼ (2005-2010). MÉTODOS: se realizó un estudio descriptivo en 106 pacientes con neoplasia intraepitelial unilateral según edad, sexo, enfermedad ocular asociada y años de diagnóstico. Se calcularon porcentajes, tasa de incidencia y prevalencia proporcional y se estimó la tendencia por método gráfico. RESULTADOS: el 59,4 % perteneció al sexo femenino (n = 63); el 53,8 % (n = 57) se encontró en edades entre 30 y 39 años, seguido por ³ 60 años con 33 % (n = 35); el 18,9 % (n = 20) presentó enfermedad ocular asociada, donde el 60 % (n = 12) correspondió al pterigium. Hubo entre 34 % (n = 36), 31, 1 % (n = 33) y 30,2 % (n = 32) con NIC I, NIC II y NIC III respectivamente, y el 4,7 % (n = 5) con carcinoma in situ. La incidencia en el 2009 alcanzó 3,1 % (n = 34) y 85,8 % de prevalencia (n = 91), con tendencia lineal al incremento. CONCLUSIONES: se manifiesta tendencia al incremento de la enfermedad; por eso merece atención el estudio de los factores de riesgo como enfoque preventivo para reducir este comportamiento, si se parte del aumento de la expectativa de vida de la población cubana con tendencia al envejecimiento, pues la neoplasia intraepitelial de la conjuntiva es característica de personas en edad avanzada.
