ABSTRACT

Objetivo: analisar o nível de estresse percebido e sofrimento psíquico em gestores de saúde na pandemia da Covid-19. Método: estudo descritivo, transversal com abordagem quantitativa. A coleta ocorreu de abril a setembro de 2021, com 40 gestores de serviço de saúde. Utilizou-se para a coleta de dados um instrumento para caracterização sociodemográfica e ocupacional e as escalas "Perceived Stress Scale-14" e "Self Reporting Questionnaire" para avaliação do estresse percebido e sofrimento psíquico. Os dados coletados foram analisados no Statistical Package for the Social Sciences versão 22.0. O presente estudo faz parte de um projeto intitulado "Trabalhadores dos Serviços de Saúde Frente à Pandemia de Covid-19", aprovado pelo Comitê de Ética em Pesquisa sob CAAE número 35260620.9.0000.5231. Resultados: a maioria dos profissionais eram do sexo feminino (90%, N=36), casados (70%, N=28), com filhos (80%, N=32), com média de idade de 45 anos e com pós--graduação (47,5%, N=19). A média dos escores relacionados ao estresse percebido foi 31,13 pontos (DP=3,77) sendo o mínimo 24 e máximo de 42 pontos. Com relação ao sofrimento psíquico, (40%, N=16) os gestores apresentaram prováveis casos de transtornos. A prática de atividades físicas e de lazer (p<0,05) tem papel importante na diminuição do estresse percebido e do sofrimento psíquico. Conclusão: os gestores em saúde apresentaram, durante a pandemia, estresse e sofrimento psíquico, resultados esses que devem ser considerados para promoção de autocuidado aos gestores de saúde, enfatizando a necessidade da realização de atividades físicas e de lazer.


Objective: to analyze the level of perceived stress and psychological suffering in health managers during the Covid-19 pandemic. Method: descriptive, cross-sectional study with a quantitative approach. The collection took place from April to September 2021, with 40 health service managers. An instrument for socio-demographic and occupational characterization and the "Perceived Stress Scale-14" and "Self Reporting Questionnaire" scales were used for the assessment of perceived stress and psychic suffering. The collected data were analyzed using the Statistical Package for the Social Sciences version 22.0. The present study is part of a project entitled "Health Service Workers in the Face of the Covid-19 Pandemic", approved by the Research Ethics Committee under CAAE number 35260620.9.0000.5231. Results: most professionals were female (90%, N=36), married (70%, N=28), with children (80%, N=32), with a mean age of 45 years and with a postgraduate degree. -graduation (47.5%, N=19). The average score related to perceived stress was 31.13 points (SD=3.77), with a minimum of 24 and a maximum of 42 points. With regard to psychic suffering, (40%, N=16) the managers presented probable cases of disorders. The practice of physical and leisure activities (p<0.05) plays an important role in reducing perceived stress and psychological distress. Conclusion: health managers presented, during the pandemic, stress and psychic suffering, results that should be considered for promoting self-care to health managers, emphasizing the need to carry out physical and leisure activities.Keywords: Health manager; Occupational stress; Covid-19; Coronavirus infections.

ABSTRACT

The purpose of this study was to investigate which protective strategies against occupational stress were developed for health professionals during the COVID-19 pandemic. This was an integrative literature review conducted in 2021. The Medical Literature Analysis and Retrieval System Online and the Latin American and Caribbean Literature in Health Sciences databases were searched using the Boolean operator AND and the controlled vocabularies "health professionals" AND "occupational stress" AND "COVID-19", both in English and Portuguese. After applying the inclusion criteria and reading the selected articles, a final sample of 24 articles was obtained. Protective strategies against occupational stress developed by institutions included psychological support, support from managers and team leaders, provision of personal protective equipment, appropriate schedules and workload, and training. As for the strategies developed by health professionals themselves, these included resilience, peer support, and self-care. Several protective strategies during the pandemic were observed, but institutions still need to develop and/or improve practices to offer better psychological conditions to health professionals in general.


O objetivo deste estudo é investigar as estratégias de proteção ao estresse ocupacional em profissionais de saúde na pandemia de covid-19. Trata-se de uma revisão integrativa da literatura realizada nas bases de dados Medical Literature Analysis and Retrieval System Online e Literatura Latino-Americana e do Caribe em Ciências da Saúde em 2021. Utilizou-se o operador booleano AND e vocabulários controlados, a citar: profissionais de saúde AND estresse ocupacional AND COVID-19 e suas respectivas equivalências com termos em inglês. Após a aplicação dos critérios de inclusão e leitura do material, obteve-se uma amostra final de 24 artigos. Suporte psicológico, apoio de gestores e líderes das equipes, disponibilidade de equipamentos de proteção individuais, cronogramas e carga horária apropriados e treinamentos foram estratégias de proteção ao estresse ocupacional organizadas pelas instituições. Quanto à resiliência, o apoio entre pares e os cuidados pessoais foram estratégias criadas pelos profissionais de saúde. Evidenciou-se o uso de diversas estratégias no cenário pandêmico, porém ainda se torna necessário que as instituições elaborem e/ou aprimorem suas práticas de modo a oferecer melhores condições psicológicas aos profissionais de saúde.

Introduction: Presenteeism, defined as the physical presence of the worker in the work environment despite being sick, has been little investigated among public servants in leadership positions. Managers can suffer stress and even burnout, since the organizational environment is full of complex situations, which can generate strong tension. Objectives: To evaluate the relationship between work stress and burnout syndrome with presenteeism among civil servants in management positions at a Brazilian federal university. Methods: A total of 106 managers responded to an online questionnaire, including questions about the psychosocial aspects of work. Associations between presenteeism, sociodemographic aspects, dimensions of job stress (demand, control and social support) and Burnout syndrome (emotional exhaustion, dehumanization, emotional distancing and professional achievement) were performed using logistic regression methods. Results: In relation to presenteeism, 51.9% reported having worked in the presence of health problems, and 45.2% of presenteeist workers showed a drop in the performance of work activities due to presenteeism. Burnout syndrome was observed in 10.5% of the group. Regarding the stress at work, 42.9% showed high demand, 55.7% low control and 59.6% low social support. For presenteeists, associations were observed between decreased control, increased emotional exhaustion and dehumanization with greater chances of falling in work performance due to presenteeism. Conclusions: The characteristics of the management positions observed here are factors that can contribute to the greater occurrence of presenteeism and/or greater drop in work performance due to presenteeism.


Introdução: A presença física do trabalhador no ambiente de trabalho, mesmo quando doente, definida como presenteísmo, tem sido pouco investigada entre servidores públicos em cargos de chefia. Os gestores podem sofrer estresse e burnout, visto que o ambiente organizacional é repleto de situações complexas que podem gerar forte tensão. Objetivos: Avaliar as relações do estresse no trabalho e da síndrome de burnout com o presenteísmo entre servidores nos cargos de gestão de uma universidade federal brasileira. Métodos: Estudo transversal com 106 gestores que responderam a um questionário on-line, incluindo perguntas sobre os aspectos psicossociais do trabalho. As associações bivariadas entre o presenteísmo, os aspectos sociodemográficos, as dimensões do estresse no trabalho (demanda, controle e apoio social) e da síndrome de burnout (exaustão emocional, desumanização, distanciamento emocional e realização profissional) foram realizadas por meio de análises de regressão logística binomial. Resultados: Do total, 51,9% dos participantes referiram ser presenteístas, sendo que 45,2% dos presenteístas apresentaram queda no desempenho das atividades laborais devido ao presenteísmo. A síndrome de burnout foi observada em 10,5% do grupo. Em relação ao estresse, 42,9% apresentaram alta demanda, 55,7% baixo controle e 59,6% apoio social baixo. Entre os presenteístas, foram observadas associações entre a diminuição do controle, o aumento da exaustão emocional e da desumanização com as maiores chances de queda no desempenho laboral devido ao presenteísmo. Conclusões: As características dos cargos de gestão aqui observadas são fatores que podem contribuir para a maior ocorrência de presenteísmo e/ou maior queda do desempenho laboral devido ao presenteísmo.

Introduction: Low back pain can be defined as pain and/or discomfort between the coastal margins of the lowest rib and the gluteal fold, and it can cause motor dysfunction, loss of productivity, and changes in job function. There is a greater number of nursing professionals in hospital environments, and, regardless of their numerical contingent, they have the highest percentage of absenteeism due to this condition. Objectives: To evaluate the association between perception of low back pain and occupational stress in hospital nursing professionals. Methods: Eleven nurses and 95 practical nurses (n = 106) were evaluated on their perception of low back pain (Visual Analogue Scale) and occupational stress (job stress scale) using the Demand-Control Model. The chi-square test was used to assess associations between categorial variables (p < 0.05). Results: Low back pain was reported by 74% of the study sample (n = 81). Assessment of occupational stress using the Demand-Control Model showed that 54.7% (n = 58) had low psychological demand and 63.2% (n = 67) had high control at work. Active work was observed in 33% (n = 35) and low demand at work in 30.2% (n = 32). There were no significantly associations between the perception of low back pain and the occupational stress domains described by the Demand-Control Model (p = 0.721). Conclusions: Although there was a high prevalence of low back pain in this sample of nursing professionals, it was not associated with occupational stress.


Introdução: A dor lombar pode ser definida como dor e/ou desconforto entre as margens costais da última costela e a prega glútea, podendo causar disfunções motoras, perda de produtividade e mudança de emprego. Os profissionais de enfermagem atuam em maior número no âmbito hospitalar e, independentemente de seu contingente numérico, apresentam o maior percentual de absenteísmo devido a essa afecção. Objetivos: Avaliar a associação entre a percepção de dor lombar e o estresse ocupacional em profissionais de enfermagem de âmbito hospitalar. Métodos: Foram avaliados 11 enfermeiros e 95 técnicos de enfermagem (n = 106) quanto à percepção da dor lombar (Escala Visual Analógica) e o estresse ocupacional ( Job Stress Scale) por meio do Modelo Demanda-Controle. A associação entre variáveis categóricas foi avaliada pelo teste de qui-quadrado (p < 0,05). Resultados: Houve prevalência de dor lombar em 74% da amostra (n = 81). A avaliação do estresse ocupacional por meio do Modelo Demanda-Controle evidenciou que 54,7% (n = 58) apresentou baixa demanda psicológica e 63,2% (n = 67) apresentou alto controle no trabalho. O trabalho ativo foi evidenciado em 33% (n = 35) e a baixa exigência no trabalho em 30,2% (n = 32). Não houve associação entre a percepção de dor lombar e os domínios do estresse ocupacional (p = 0,721). Conclusões: Houve alta prevalência de dor lombar, sem que tal sintoma tenha se associado ao estresse ocupacional nos profissionais de enfermagem avaliados.

Introduction: Health care workers in intensive care units have a high propensity to develop burnout syndrome. Objectives: To evaluate the relationship between occupational stress and markers of oxidative stress and inflammation in health care workers in intensive care units. Methods: The sample consisted of 133 intensivists from the city of Colatina, state of Espírito Santo, Brazil. The Maslach Inventory Burnout Survey was used to assess burnout syndrome. Oxidative stress was measured in proteins and lipids, and cytokine levels were assessed via enzyme-linked immunosorbent assay. Results: The highest levels of burnout syndrome (emotional exhaustion dimension) were found in nurses and physical therapists and showed greater changes in markers of protein damage and inflammation. On the emotional exhaustion dimension, it was higher among professionals who consumed some type of alcoholic beverage and some type of stimulant, whether caffeine, tea, or soft drinks, at least twice a week. There was a positive relationship between the development of burnout syndrome, specifically in the dimension of low personal involvement at work, and oxidative damage in lipids (thiobarbituric acid reactive substances). Conclusions: There is evidence of relationship between occupational stress and oxidative stress in professionals with low personal involvement in their work.


Introdução: Os profissionais de saúde que atuam em Unidades de Terapia Intensiva apresentam forte propensão a desenvolver a síndrome de burnout. Objetivos: Este estudo teve como objetivo avaliar a relação entre o estresse ocupacional e os marcadores de estresse oxidativo e inflamação em profissionais de saúde em Unidade de Terapia Intensiva. Métodos: A amostra foi composta por 133 profissionais intensivistas de Colatina, Espírito Santo, Brasil. O Maslach Inventory Burnout Survey foi aplicado para avaliar a síndrome de burnout. O estresse oxidativo em proteínas e lipídios foi medido e os níveis de citocinas foram avaliados pelo ensaio enzyme-linked immunosorbent assay. Resultados: Os resultados do estudo mostraram que os níveis mais elevados para síndrome de burnout (dimensão exaustão emocional) foram mais significativos entre enfermeiros e fisioterapeutas e apresentaram maiores alterações nos marcadores de dano proteico e inflamação. Na dimensão exaustão emocional, foi maior nos profissionais que consomem algum tipo de bebida alcoólica pelo menos duas vezes por semana e algum tipo de estimulante, seja cafeína, chá ou refrigerante. Houve relação positiva no desenvolvimento da síndrome de burnout, na dimensão de baixo envolvimento pessoal no trabalho, com dados oxidativos em lipídios (thiobarbituric acid reactive substances). Conclusões: Há evidências de relação entre estresse ocupacional e estresse oxidativo em profissionais com baixo envolvimento pessoal em seu trabalho.

Introducción. El acoso laboral y sexual son problemas latentes durante la formación de los cirujanos, que conllevan repercusiones negativas a nivel individual, organizacional y del sistema sanitario. El objetivo del presente estudio fue explorar la prevalencia de estos tipos de acoso en residentes de cirugía colombianos. Métodos. Estudio nacional, de corte transversal, realizado en los veinte programas de residencia del país en septiembre de 2023. Los residentes autoevaluaron su exposición a ambas formas de acoso mediante cuestionarios validados. Se realizaron comparaciones entre víctimas y no víctimas de acoso, en función de las características demográficas de la población. Se evaluó la frecuencia de conductas de acoso laboral cometidas por diferentes perpetradores. Resultados. Se incluyeron 238 residentes (64,7 % del total de la población). Las tasas de acoso laboral y sexual fueron de 35,3 % y 18,1 %, respectivamente. Ambos tipos de acoso fueron significativamente mayores en los programas ubicados en el centro del país. El acoso sexual fue significativamente mayor entre las mujeres. Los profesores de cirugía y los residentes de niveles superiores fueron los principales perpetradores de conductas de acoso laboral. Conclusiones. El acoso laboral y sexual es altamente prevalente en la formación de los cirujanos colombianos. Estos hallazgos deben alertar a profesores, médicos residentes y otros grupos de interés, para fomentar ambientes y cultura educativa saludables, que permitan disminuir la prevalencia de estos comportamientos.


Introduction. Workplace and sexual harassment are latent problems during surgical training, which have negative repercussions at the individual, organizational and health system levels. The objective of the present study was to explore the prevalence of these types of harassment in all Colombian surgical residents. Methods. National cross-sectional study conducted in twenty residency programs in September 2023. Residents self-assessed their exposure to both forms of harassment using validated questionnaires. Comparisons were made between victims and non-victims of bullying, based on the demographic characteristics of the population. The frequency of workplace harassment behaviors committed by different perpetrators was evaluated. Results. A total of 238 residents were included (64.7% of population). Rates of workplace and sexual harassment were 35.3% and 18.1%, respectively. Both types of harassment were significantly higher in programs located in the central region of the country. Sexual harassment was significantly higher among women. Surgery professors and senior residents were the main perpetrators of workplace bullying behaviors. Conclusions. Workplace and sexual harassment is highly prevalent during surgical training of Colombian surgeons. These findings should alert professors, residents, and other interest groups to promote healthy educational environments and culture, which will reduce the prevalence of these behaviors.

Introduction: There is a need to understand which factors are associated with sickness absence in the context of public service in order to guide efforts to prevent illness in workers. Objectives: We investigated whether lifestyle, locus of health control, work-related stress, and self-perception of physical and mental health are associated with sickness absence from a biopsychosocial perspective. Methods: This is a cross-sectional study using an online questionnaire and the participants comprised 898 employees at a federal university. The assessment included instruments on sociodemographic and occupational characteristics, sickness absence, lifestyle (FANTASTIC Lifestyle scale), locus of control (Multidimensional Health Locus of Control Scale), workrelated stress (Health Safety Executive), and physical and mental health (Short-Form Health Survey 12 - version 2). A Poisson regression model was constructed using Generalized Estimating Equations to identify the variables associated with sickness absence, with a p ≤ 0.05 significance level. Results: We found that work-related stress, locus of control, physical and mental health, administrative or technical job role, female gender, and longer service time at the institution were associated with a higher number of days absent from work due to illness (for all associations, p < 0.001). Conclusions: The present study contributes to the literature by offering additional data on sickness absence in the context of Brazilian public university employees.


Introdução: É necessário compreender quais fatores estão associados ao absenteísmo-doença no contexto do serviço público para direcionar a prevenção do adoecimento em trabalhadores. Objetivos: Foi investigado se estilo de vida, lócus de controle da saúde, estresse relacionado ao trabalho e autopercepção de saúde física e mental estão associados com absenteísmodoença, a partir de uma perspectiva biopsicossocial. Métodos: Trata-se de um estudo transversal por meio de questionário online. Participaram 898 servidores públicos de uma universidade federal. Utilizaram-se cinco instrumentos: um questionário de características sociodemográficas e ocupacionais; a escala FANTASTIC Lifestyle, para avaliação do estilo de vida; a escala Multidimensional Health Locus of Control Scale, que avalia lócus de controle da saúde; as diretrizes do Health and Safety Executive, para avaliação do estresse relacionado ao trabalho; e o Short-Form Health Survey 12 - version 2, para avaliação da saúde física e mental. Para identificar as variáveis associadas ao absenteísmo-doença, foi empregado o modelo de regressão de Poisson através de equações de estimativas generalizadas, sendo considerado o nível de significância p ≤ 0,05. Resultados: Apontou-se que maior estresse relacionado ao trabalho, lócus de controle da saúde, pior autopercepção de saúde física e mental, técnicos administrativos, gênero feminino e maior tempo de serviço na instituição se associam com mais dias de ausência ao trabalho por motivo de doença (para todas as associações: p < 0,001). Conclusões: O presente estudo contribui com a literatura ao oferecer dados adicionais sobre absenteísmo-doença no contexto de serviço público universitário brasileiro.

ABSTRACT

BACKGROUND: Evidence indicates that physical activity reduces stress and promote a myriad of health-enhancing effects through anti-inflammatory mechanisms. However, it is unknown whether these mechanisms interfere in the association between psychosocial job stress and headache disorders. OBJECTIVE: To test whether physical activity and its interplay with the systemic inflammation biomarkers high-sensitivity C-reactive protein (hs-CRP) and acute phase glycoproteins (GlycA) would mediate the associations between job stress and headache disorders. METHODS: We cross-sectionally evaluated the baseline data from the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil) regarding job stress (higher demand and lower control and support subscales), migraine and tension-type headache (ICHD-2 criteria), self-reported leisure-time physical activity, and plasma hs-CRP and GlycA levels. Conditional process analyses with a sequential mediation approach were employed to compute path coefficients and 95 % confidence intervals (CI) around the indirect effects of physical activity and biomarkers on the job stress-headache relationship. Separate models were adjusted for sex, age, and depression and anxiety. Further adjustments added BMI smoking status, and socioeconomic factors. RESULTS: In total, 7,644 people were included in the study. The 1-year prevalence of migraine and tension-type headache were 13.1 % and 49.4 %, respectively. In models adjusted for sex, age, anxiety, and depression, the association between job stress (lower job control) and migraine was mediated by physical activity [effect = -0.039 (95 %CI: -0.074, -0.010)] but not hs-CRP or GlycA. TTH was associated with higher job control and lower job demand, which was mediated by the inverse associations between physical activity and GlycA [Job Control: effect = 0.0005 (95 %CI: 0.0001, 0.0010); Job Demand: effect = 0.0003 (95 %CI: 0.0001, 0.0007]. Only the mediating effect of physical activity in the job stress-migraine link remained after further adjustments including socioeconomic factors, BMI, smoking, and the exclusion of major chronic diseases. CONCLUSION: In the ELSA-Brasil study, physical activity reversed the link between job stress and migraine independently of systemic inflammation, while the LTPA-mediated downregulation of GlycA was associated with lower job stress-related TTH.

INTRODUCTION: The increase in the workload of health professionals and the degree of complexity of patients, attribute greater risk to psychosocial stress. OBJECTIVE: To evaluate the associations between occupational stress, quality of life at work, and coping strategies by the hospital nursing team during the COVID-19 pandemic. METHODS: Cross sectional, quantitative study with convenience sampling, data collection from August to December 2020; in two units of the private hospital network, with sociodemographic, occupational and health questionnaires; visual analogue scale for assessing quality of life at work; Demand-Control-Support (DCS); Occupational Coping Scale. RESULTS: The total sample consisted of 196 nursing professionals. There was significant certainty (negative, however, the dimension "Demand" of the DCS and QWL (<0.001, r=-0.367). Control over work-related work has a significant quality (but the "Control" dimension of the DCS and QWL (=0.025, r=0.160); and significantly negative, however, between the "Social Support" dimension of DCS and "Negative Equivalence" of Coping (p=0.003, r=-0.2013). CONCLUSION: The findings of this study allowed the correlation between occupational stress, coping and quality of life at work, showing that the lower the social support, the greater the use of avoidance strategies and consequently decline in quality of life at work. They also allowed us to identify the coping strategies used by the nursing staff and quality of life at work in the face of occupational stress during the COVID-19 pandemic.


INTRODUÇÃO: O aumento da carga de trabalho dos profissionais de saúde e o grau de complexidade dos pacientes, atribuem maior risco para o estresse psicossocial. OBJETIVO: Avaliar as associações entre o estresse ocupacional, a qualidade de vida no trabalho e as estratégias de enfrentamento pela equipe de enfermagem hospitalar na pandemia de COVID-19. MÉTODOS: Estudo transversal, quantitativo com amostragem por conveniência, coleta de dados de agosto a dezembro de 2020; em duas unidades da rede hospitalar privada, com questionário sociodemográfico, ocupacional e de saúde; escala visual analógica de avaliação da qualidade de vida no trabalho; Escala de Demanda-Controle-Apoio Social (DCS); Escala de Coping ocupacional. RESULTADOS: A amostra total foi de 196 profissionais de Enfermagem. Houve correlação significativa negativa, porém fraca, entre a dimensão "Demanda" da DCS e QVT (p<0,001, r=-0,367). O controle sobre o trabalho relacionado à qualidade de vida no trabalho possui correlação significativa positiva, porém fraca, entre a dimensão "Controle" da DCS e QVT (p=0,025, r=0,160); e correlação significativa negativa, porém fraca, entre a dimensão "Apoio social" da DCS e "Esquiva" de Coping (p=0,003, r=-0,213). CONCLUSÃO: Os achados deste estudo permitiram a correlação entre o estresse ocupacional, coping e qualidade de vida no trabalho, mostrando que quanto menor o apoio social, maior uso das estratégias de esquiva e consequentemente declínio na qualidade de vida no trabalho. Permitiram também identificar as estratégias de enfrentamento (coping) utilizadas pela equipe de enfermagem e qualidade de vida no trabalho frente ao estresse ocupacional durante a pandemia da COVID-19.

ABSTRACT

Objetivo: Describir la relación entre el estrés laboral y el desempeño profesional en el servicio de Ginecología y Obstetricia en el Hospital Sergio Bernales durante el año 2023. Métodos: Estudio de tipo cuantitativo, observacional, prospectivo y transversal. Los participantes del estudio formaban parte del personal de salud que labora en el servicio de Ginecología y Obstetricia del Hospital Nacional Sergio E. Bernales. Resultados: El personal de salud presenta un nivel medio de estrés laboral 45,4 %, seguido por un 29,3 % con alto nivel y un 25,3 % con bajo nivel de estrés laboral. Los médicos residentes (18; 100 %) y los obstetras (26; 46,2 %) mostraron, predominantemente, un nivel medio de estrés laboral. Los niveles medio y alto de estrés se asociaron a un nivel de rendimiento bueno (26 personas; 45,6 %) y 15 entrevistados (26,3 %), respectivamente, y muy bueno: 18 personas (56,3 %) y 13 (40,6 %). Conclusión: Se destaca que el 57,6 % del personal tiene un buen desempeño profesional, siendo las obstetras mayoritarias en este grupo. Se observó que predominaron los niveles medio y alto de estrés laboral en el personal de salud, sin embargo, se encontró un nivel medio de desempeño profesional. Los hallazgos revelan que la mayoría de encuestados logró mantener un buen desempeño profesional a pesar de experimentar niveles medios o altos de estrés laboral, lo que sugiere la complejidad de la relación entre el estrés y el rendimiento laboral en este ámbito de la salud(AU)


Objective: To describe the relationship between work stress and professional performance in the Gynecology and Obstetrics service at the Sergio Bernales Hospital during the year 2023. Methods: Quantitative, observational, prospective and cross-sectional study. The study participants were health personnel working in the Gynecology and Obstetrics service of the Sergio E. Bernales National Hospital. Results: Health personnel have a medium level of work-related stress (45.4%), followed by 29.3% with a high level and 25.3% with a low level of work-related stress. Resident physicians (18; 100%) and obstetricians (26; 46.2%) predominantly showed a medium level of work-related stress. Medium and high levels of stress were associated with a good level of performance (26 people; 45.6%) and 15 interviewees (26.3%), respectively, and very good performance: 18 people (56.3%) and 13 (40.6%). Conclusions: It should be noted that 57,6% of the personnel have a good professional performance, with obstetricians being the majority in this group. It was observed that medium and high levels of work stress predominated among health personnel; however, a medium level of professional performance was found. The findings reveal that the majority of respondents managed to maintain good professional performance despite experiencing medium or high levels of job stress, suggesting the complexity of the relationship between stress and job performance in this health care setting(AU)

Introduction: The conditions of teachers' work during the COVID-19 pandemic affected teachers' lives regarding voice disorder and stress, even in emergency remote classroom situation. Objective: To analyze the relationship between the presence of voice disorder, job stress, and COVID-19 in teachers when in emergency remote classroom teaching situation at the time of the pandemic. Method: This is a primary, exploratory, observational cross-sectional study with the use of survey forwarded online during the period of emergency classes after the arrival of COVID-19 pandemic in Brazil. The teachers answered the sociodemographic questions about the presence of COVID-19 and the following instruments: Condition of Vocal Production-Teacher [Condição de Produção Vocal - Professor (CPV-P)], Screening Index for Voice Disorder (SIVD), and Job Stress Scale (JSS). Results: Of the 118 teachers analyzed, 94.1% were female; the average age was 44 years. The SIVD recorded the presence of voice disorder in 66.9% of the participants. Regarding the JSS, which are the findings related to stress at work in the demand domain, the teachers showed high levels, a fact which presupposes the existence of pressure of psychological nature to perform their work. Conclusion: The teachers self-reported the presence of voice disorder even in remote class situation, on the occasion of COVID-19, which were more common in older teachers. When comparing the presence of voice disorders, coronavirus symptoms, and stress domains in relation to demand, control, and social support, there was no significance. It is hoped that this study will help to reflect on the need to improve teachers' working conditions, strengthening work-related voice disorder actions and guiding actions for vocal care and well-being.


Introducción: Las condiciones de trabajo de los profesores durante la pandemia de COVID-19 afectaron sus vidas en lo que respecta al trastorno de la voz y el estrés, incluso en situaciones de emergencia en aulas remotas. Objetivo: Analizar la relación entre la presencia de trastorno de la voz, estrés laboral y COVID-19 en profesores cuando se encontraban en situación de emergencia de enseñanza en aulas remotas en la época de la pandemia. Método: Se trata de un estudio primario, exploratorio, observacional de tipo transversal, con el uso de encuesta remitida online durante el periodo de clases de emergencia tras la llegada de la pandemia de COVID-19 en Brasil. Los profesores respondieron a las preguntas sociodemográficas sobre la presencia de COVID-19 y a los siguientes instrumentos: Condición de Producción Vocal-Profesor (CPV-P), Índice de Detección de los Trastornos de la Voz (SIVD) y Escala de Estrés Laboral (JSS). Resultados: De los 118 profesores analizados, el 94,1% eran mujeres; la mediana de edad era de 44 años. El (SIVD) registró la presencia de trastorno de la voz en el 66,9% de los participantes. En cuanto a la JSS, que son los hallazgos relacionados con el estrés laboral en el dominio de la demanda, los profesores mostraron niveles elevados, hecho que presupone la existencia de presiones de naturaleza psicológica para realizar su trabajo. Conclusión: Los profesores autoinformaron de la presencia de trastornos de la voz incluso en situación de clase a distancia, con ocasión del COVID-19, que fueron más frecuentes en los profesores de más edad. Al comparar la presencia de trastornos de la voz, los síntomas del coronavirus y los dominios de estrés en relación con la demanda, el control y el apoyo social, no hubo resultados significativos. Se espera que este estudio ayude a reflexionar sobre la necesidad de mejorar las condiciones de trabajo de los docentes, fortaleciendo las acciones de Trastorno de la voz relacionado con el trabajo (WRVD) y orientando acciones para el cuidado y bienestar vocal.

ABSTRACT

Objectives. This study aimed to map the evidence on burnout syndrome in higher education teachers and its relationship with ergonomic and biopsychosocial factors. Methods. We performed a scoping review of articles published up to 2021, extracting the location, study design, sample characteristics and assessment methods, and investigated ergonomic and biopsychosocial factors. Results. Eighteen studies were found in 12 countries, and most used a cross-sectional design (n = 17/18, 94%). The Maslach burnout inventory was the most used assessment method (n = 9, 50%). There is a consensus that burnout syndrome in higher education teachers requires more attention, as it can be related to physical, mental and social factors. Conclusions. In the professional environment, physical ergonomic characteristics or those related to work organization deserve greater attention, as well as psychosocial factors, as they are strongly associated with burnout syndrome.

OBJECTIVE: To investigate the effects of sociodemographic and working condition variables, as well as the coping strategies used by nurses, on their occupational stress during the COVID-19 pandemic. METHODS: A cross-sectional study was conducted with 104 nurses who worked in intensive and emergency care at a public hospital in the state of São Paulo, Brazil. Data collection was performed in person and online using a questionnaire assessing sociodemographic and occupational characteristics, the Nursing Stress Inventory, and the Occupational Coping Scale. RESULTS: The participants had a high level of stress (median = 132), especially in the 'interpersonal relations' domain (median = 63), and made little use of occupational coping strategies (median = 87). Income (p = 0.027), work shift (p = 0.028), being on leave from work (p = 0.020), number of hospitals with employment ties (p = 0.001), and relationship with management were independently associated with the levels of stress among the nurses. CONCLUSION: In the present study, the high levels of stress among nurses were influenced by financial and work-related factors as well as interpersonal relationships. No significant association was found between stress among the nurses and the use of occupational coping strategies.

ABSTRACT

Introduction: Occupational stress affects health professionals; however, no literature was found on the middle-range theory for this outcome in health professionals. Methods: Methodological study for the theoretical validation of a nursing diagnosis using the theoretical framework of Walker and Avant and the Betty Neuman systems models. The research was conducted in five stages: comprehension of the system model; selection and review of studies; development of the conceptual-theoretical-empirical structure; elaboration of a diagram and proposition of a nursing diagnosis; and evaluation of the empirical adequacy of the theory and validity of the system model. These steps were conducted using a scoping review and a sample of 138 articles selected in the Scopus, Web of Science, MEDLINE/PubMed, CINAHL, PsycINFO, LILACS, Science Direct, Cochrane Library, and Google Scholar databases. A data extraction instrument was developed, and study variables (attributes, antecedents, and clinical consequences) were analyzed using descriptive statistics (absolute and relative frequencies) and presented in tables. Results: The middle-range theory comprised 16 concepts, 20 propositions, and one diagram. A total of 15 related factors, 29 defining characteristics, six at-risk populations, and one associated condition were indicated to propose the nursing diagnosis for occupational stress. Conclusion: The middle-range theory supported elaborating elements to propose a nursing diagnosis for occupational stress.

BACKGROUND: Previous studies have supported the relevance of using broad and complex approaches, including multiple explanatory categories, to analyze mental disorders in the working population. This study aimed to assess the direct and indirect effects of gender, race, social class, and occupational stressors on mental health. METHODS: A cross-sectional study used a random sample of 3343 health workers. The effort-reward imbalance (ERI) scale measured occupational stressors. The World Health Organization Self-Reporting Questionnaire (SRQ-20) evaluated common mental disorders (CMDs) as outcomes. The role of gender, race/color, and class determinants (level of schooling and income) in the relationship between occupational stressors and CMD was assessed. Structural equation modeling was used to determine associations and effects. RESULTS: Occupational stressors were directly associated with CMD and mediated the relationship between income and CMD. Gender was directly associated with occupational stressors, income, and domestic overload. Race was associated with education and with CMD through indirect paths mediated by class indicators. Class indicators contributed to increasing exposure to occupational stressors and the occurrence of CMD. CONCLUSION: The results highlight the relevance of gender, race/color, and class in understanding the unequal distribution of work stressors and mental illness in health workers.

ABSTRACT

Objective: to evaluate the effectiveness of a program in increasing coping strategies focused on military firefighters' problems and emotions. Method: randomized, parallel, single-masked clinical trial. The sample consisted of 51 participants in the intervention group and 49 in the control group. The intervention group received the intervention program including coping strategies based on the Nursing Interventions Classification, lasting six consecutive weeks, one day a week. The control group followed the Service Unit routine. Descriptive statistics, Student's T test with Welch's correction and the Mann-Whitney test were used for the analyses. The magnitude of the intervention effect was calculated using Cohen's d index. A p-value of ≤0.05% was considered. Results: in the analysis of the mean difference between the scores in the groups, the means of the intervention group increased significantly for the coping strategies: social support (p = 0.009), acceptance of responsibility (p = 0.03), problem solving (p = 0.05) and positive reappraisal (p = 0.05). The impact of the intervention was moderate in magnitude for social support (d = 0.54). Conclusion: the intervention program enabled the increase of coping strategies focused on military firefighters' problems and emotions. ReBEC: RBR-8dmbzc.

Método: ensayo clínico aleatorizado, paralelo, con enmascaramiento simple. La muestra se constituyó de 51 participantes en el grupo intervención y 49 en el control. El grupo intervención recibió el programa interventivo incluyendo las estrategias de coping basadas en la Clasificación de las Intervenciones de Enfermería, con una duración de seis semanas consecutivas, un día por semana. El grupo control siguió la rutina de la Unidad de Servicio. Para los análisis se utilizó estadística descriptiva, test T de Student con corrección de Welch y el test de Mann-Whitney. La magnitud del efecto de la intervención se calculó con el índice d de Cohen. Se consideró valor p≤0,05%. ABSTRACT Objective: to evaluate the quality of life at work of health professionals in direct and indirect care of COVID-19 cases. Methods: this was a cross-sectional study with 156 health professionals from a referral hospital. The relationship between sociodemographic and work-related variables and perceived stress and domains of the Quality of Life at Work Scale was investigated using inferential statistics and regression. Results: Satisfaction with Compassion was moderate (mean: 38.2), with low perception of stress, Burnout and Secondary Traumatic Stress (means: 18.8, 21.6 and 19.1). There were associations between: education, salary, multiple jobs and direct care with Compassion Satisfaction; low income, being a nurse and working overtime with Burnout; and working more than 12 hours, underlying disease and hospitalization for COVID-19 with Secondary Traumatic Stress. Conclusion: quality of life at work was satisfactory, despite the presence of Burnout and Secondary Traumatic Stress.


RESUMEN Objetivo: evaluar la calidad de vida laboral de los profesionales sanitarios que prestan atención directa e indirecta a los casos de COVID-19. Métodos: se trata de un estudio transversal llevado a cabo con 156 profesionales sanitarios de un hospital de referencia. Se investigó la relación entre las variables sociodemográficas, laborales y el estrés percibido y los dominios de la Escala de Evaluación de la Calidad de Vida Laboral mediante estadística inferencial y de regresión. Resultados: la satisfacción por Compasión fue moderada (media: 38,2), con nivel de estrés percibido, Burnout y Estrés Traumático Secundario bajos (medias: 18,8, 21,6 y 19,1). Hubo asociaciones entre: escolaridad, sueldo, pluriempleo y atención directa con la Satisfacción por Compasión; bajos ingresos, ser enfermero y hacer horas extras con el Burnout; y trabajar más de 12 horas, enfermedad subyacente y hospitalización por COVID-19 con Estrés Traumático Secundario. Conclusión: la calidad de vida en el trabajo fue satisfactoria, a pesar de la presencia de Burnout y Estrés Traumático Secundario.

Resultados: a Satisfação por Compaixão foi moderada (média: 38,2), com percepção de estresse, Burnout e Estresse Traumático Secundário baixos (médias: 18,8, 21,6 e 19,1). ABSTRACT Objective: To evaluate the association between occupational stressors and common mental disorders (CMD) among "invisible" health workers in the context of the COVID-19 pandemic. Method: Cross-sectional study including a probabilistic sample of 1,014 health workers from three municipalities in Bahia. CMDs were assessed using the SRQ-20. The Effort-Reward Imbalance (ERI) scale and the Demand-Control Model assessed occupational stressors. Descriptive, bivariate, and multiple analysis to evaluate the association between the variables of interest. Results: The global prevalence of CMD was 39.9%; it was higher among CHA/EDCA (47.2%), followed by management and surveillance staff (38.6%), technicians (35.4%), and support/maintenance/cleaning staff (29.9%). The association between occupational stressors and CMD varied among occupations: 1. Excessive work commitment (EWC), effort-reward imbalance (ERI), and psychological demand were associated with CMD among support/maintenance/cleaning workers; 2. EWC and ERI were associated with CMD among CHA/EDCA; 3. EWC, ERI, and low control over work were associated with CMD among technicians; 4. Among management and surveillance workers, only ERI remained associated with CMD. Conclusions: Occupational stressors played a relevant role in mental illness, with variation between occupational strata, demanding attention, monitoring, and control.


RESUMO Objetivo: Avaliar associação entre estressores ocupacionais e transtornos mentais comuns (TMC) entre trabalhadores e trabalhadoras "invisíbilizados" da saúde, no contexto da pandemia de COVID-19. Métodos: Estudo transversal com amostra probabilística de 1.014 trabalhadores(as) da saúde de três municípios baianos. Os TMC foram avaliados pelo SRQ-20. A escala Desequilíbrio Esforço-Recompensa (DER) e o Modelo Demanda-Controle avaliaram os estressores ocupacionais. Análise descritiva, bivariada e múltipla avaliaram associação entre as variáveis de interesse. Resultados: A prevalência global de TMC foi de 39,9%, sendo mais elevada entre os Agentes Comunitários de Saúde/Agente de Combate às Endemias — ACS/ACE (47,2%), seguidos pelo pessoal da gestão e vigilância (38,6%), técnicos (35,4%) e pessoal de apoio/conservação/limpeza (29,9%). A associação entre estressores ocupacionais e TMC variou entre as ocupações: 1. Comprometimento excessivo com o trabalho (CET), DER e demandas psicológicas associaram-se aos TMC entre trabalhadores de apoio/conservação/limpeza; 2. CET e DER estavam associadas aos TMC entre ACS/ACE; 3. CET, DER e baixo controle sobre o trabalho associaram-se aos TMC entre os técnicos; 4. Entre trabalhadores de gestão e vigilância, apenas o DER estava associado aos TMC. Conclusões: Estressores ocupacionais tiveram papel relevante no adoecimento mental, com variação entre os estratos de ocupação, demandando atenção, acompanhamento e controle.

Este estudo analisa o estresse ocupacional entre psicólogos que atuavam na Atenção Primária à Saúde durante a pandemia ocasionada pela covid-19, assim como as características sociodemográficas e laborais dos participantes e sua relação com o estresse ocupacional. Participaram da pesquisa 70 psicólogos atuantes em 51 unidades básicas de saúde das regiões Oeste e Extremo Oeste catarinense. Para coleta de dados, um questionário sociodemográfico e a versão reduzida da Job Stress Scale (JSS) foram aplicados. A análise dos dados foi realizada por meio da estatística descritiva e inferencial. Identificou-se que 35,7% dos psicólogos apresentaram alto desgaste no trabalho; 28,6% baixo desgaste; 27,1% se mostraram em trabalho passivo; e 8,6% em trabalho ativo. No modelo de regressão linear, os fatores associados à dimensão Demanda da JSS foram: possuir filho (a) (coeficiente -1,49; IC 95% -2,75 a -0,23) e afastamento do trabalho nos últimos 12 meses (coeficiente 1,88; IC 95% 0,60 a 3,15). Os psicólogos com hipertensão arterial sistêmica autorreferida apresentaram, em média, 3,96 pontos a menos no escore de Apoio social (IC 95% -7,06 a -0,85), quando comparados aos não hipertensos, e entre os psicólogos que trabalhavam no turno da manhã identificou-se aumento de 4,46 pontos, em média, no escore de Apoio social (IC 95% 0,90 a 8,02) em relação aos profissionais do turno manhã e tarde. Evidenciou-se que um número significativo de psicólogos apresentava-se em alto desgaste no trabalho, com potenciais implicações para sua saúde e atuação profissional.(AU)


This study analyzed occupational stress among psychologists who worked in Primary Health Care during the COVID-19 pandemic and participants' sociodemographic and work characteristics and their relationship with occupational stress. In total, 70 psychologists working in 51 basic health units in the West and Far West regions of Santa Catarina participated in this research. A sociodemographic questionnaire and the short version of the Job Stress Scale (JSS) were applied to collect data. Data were analyzed by descriptive and inferential statistics. In total, 35.7% of psychologists showed high stress at work; 28.6%, low burn out; 27.1%, passive work; and 8.6%, active work. The factors in the linear regression model that were associated with the JSS demand dimension referred to having children (coefficient −1.49; 95% CI −2.75 to −0.23) and absence from work in the last 12 months (coefficient 1.88; 95% CI 0.60 to 3.15). Psychologists with self-reported systemic arterial hypertension showed, on average, 3.96 points lower in the Social Support score (95% CI −7.06 to −0.85) than non-hypertensive ones and psychologists who worked in the morning shift, an average increase of 4.46 points in the Social Support score (95% CI 0.90 to 8.02) in relation to professionals working in the morning and afternoon shifts. A significant number of psychologists had high stress at work, with potential implications to their health and professional performance.(AU)


Este estudio evalúa el estrés laboral entre los psicólogos que trabajaron en la atención primaria de salud durante la pandemia provocada por la COVID-19, así como las características sociodemográficas y laborales de ellos y su relación con el estrés laboral. En la investigación participaron setenta psicólogos que trabajan en 51 unidades básicas de salud en las regiones oeste y lejano oeste de Santa Catarina (Brasil). Para la recolección de datos se aplicó un cuestionario sociodemográfico y la versión corta de la Job Stress Scale (JSS). El análisis de los datos se realizó mediante estadística descriptiva e inferencial. Se identificó que el 35,7% de los psicólogos presentaban alto estrés en el trabajo; el 28,6% tenían poco desgaste; el 27,1% se encontraban en trabajo pasivo; y el 8,6% en trabajo activo. En el modelo de regresión lineal, los factores asociados a la dimensión demanda de la JSS fueron: tener hijo (coeficiente -1,49; IC 95% -2,75 a -0,23) y baja laboral en los últimos 12 meses (coeficiente 1,88; IC 95% 0,60 a 3,15). Los psicólogos con hipertensión arterial sistémica autoinformada presentaron un promedio de 3,96 puntos más bajo en la puntuación de apoyo social (IC 95% -7,06 a -0,85) en comparación con los no hipertensos, y entre los psicólogos que trabajaban en el turno de la mañana, se identificó un aumento promedio de 4,46 puntos en la puntuación de apoyo social (IC 95% 0,90 a 8,02) con relación a los profesionales que laboran en el turno de mañana y tarde. Quedó evidente que un número significativo de psicólogos se encontraba en situación de alto estrés en el trabajo, con posibles implicaciones para su salud y desempeño profesional.(AU)

ABSTRACT Objective: to analyze prevalence and association between stress and anxiety among perioperative nursing professionals. Method: this is mixed methods explanatory sequential research. Data were collected between October 2022 and March 2023. Cross-sectional quantitative stage was developed with 56 perioperative nursing professionals, who answered a sociodemographic questionnaire, the List of Signs and Symptoms of Stress and General Anxiety Disorder 7-item (GAD-7). Qualitative data obtained from eight semi-structured interviews. Data mixing occurred per connection. Results: professionals with moderate anxiety (n=21; 36.8%) and high stress (n=24; 42.1%) prevailed. An association was found between anxiety and stress (r=0.827; p=0.01). Qualitative data supported aspects of the perioperative work context that predispose to stress and anxiety, such as work overload, little interprofessional collaboration, limited governance over routines and dichotomy between macroprocess management and care practice. Conclusion: an association was evidenced between high stress and anxiety present in the professionals investigated, injuries related to aspects of the work carried out in perioperative care contexts.


RESUMEN Objetivo: analizar la prevalencia y asociación entre estrés y ansiedad entre profesionales de enfermería perioperatoria. Método: estudio secuencial explicativo de método mixto. Los datos se recopilaron entre octubre de 2022 y marzo de 2023. Se desarrolló una etapa cuantitativa transversal con 56 profesionales de enfermería perioperatorios, quienes respondieron un cuestionario sociodemográfico, el Listado de Signos y Síntomas de Estrés y GAD 7 (Trastorno de Ansiedad General). Datos cualitativos obtenidos de ocho entrevistas semiestructuradas. Se produjo una mezcla de datos por conexión. Resultados: predominaron los profesionales con ansiedad moderada (n=21; 36,8%) y alto estrés (n=24; 42,1%). Se encontró asociación entre ansiedad y estrés (r=0,827; p=0,01). Los datos cualitativos corroboraron aspectos del contexto laboral perioperatorio que predisponen al estrés y la ansiedad, como la sobrecarga de trabajo, la poca colaboración interprofesional, la gobernanza limitada de las rutinas y la dicotomía entre la gestión de macroprocesos y la práctica asistencial. Conclusión: hubo asociación entre el alto estrés y la ansiedad presentes en los profesionales investigados, problemas relacionados con aspectos del trabajo realizado en contextos de atención perioperatoria.

Dados qualitativos obtidos com oito entrevistas semiestruturadas. Mixagem de dados por conexão. Resultados: prevaleceram profissionais com ansiedade moderada (n=21; 36,8%) e alto estresse (n=24; 42,1%). Constatou-se associação entre ansiedade e estresse (r=0,827; p=0,01). Dados qualitativos corroboraram com aspectos do contexto de trabalho perioperatório que predispõem ao estresse e à ansiedade, como sobrecarga de trabalho, pouca colaboração interprofissional, limitada governabilidade sobre as rotinas e a dicotomia entre gestão de macroprocessos e prática assistencial. Conclusão: evidenciou-se associação entre alto estresse e ansiedade presente nos profissionais investigados, agravos relacionados a aspectos do trabalho desenvolvido em contextos de cuidado perioperatório.
