ABSTRACT

La planificación preoperatoria resulta fundamental en la cirugía de escoliosis idiopática del adolescente (EIA) para determinar los niveles a fusionar y realizar el procedimiento con mayor precisión y diligencia. Sin embargo, su realización protocolizada no está tan extendida entre los especialistas en formación. El objetivo de este artículo es describir en detalle el método de planificación preoperatoria en la EIA empleado en una unidad especializada de raquis infantil y del adolescente de un centro de referencia, así como, la logística y la táctica, apoyado en un software gratuito de medición digital semiautomática y planificación. Se muestran 3 casos representativos de diferentes deformidades vertebrales, intervenidos mediante una artrodesis vertebral posterior tras una planificación preoperatoria según el método descrito. Este método resulta muy adecuado para el cirujano en formación al aunar las ventajas de los métodos tradicionales y los más modernos, y ser sencillo, de bajo coste, accesible, reproducible y con carácter formativo.(AU)

Introdução:O aprimoramento das resinas compostas nos últimosanos em associação com a difusão de informações nas redes sociais tornou as facetas diretas tratamentos populares na dentística restauradora. No entanto, são procedimentos que exigem ampla destreza manual e conhecimento técnico. O fluxo digital através doescaneamento, enceramento digital e prototipagem 3D para construção de guias tem se tornado uma excelente alternativa para aumentar a previsibilidade e aumentar a longevidade destes trabalhos. Objetivo:Descrever o protocolo de confecção de facetas diretas em resina composta, através de um relato de caso, utilizando como auxílio o planejamento digital para confecção de modelo 3D, guia de silicone e paredes palatinas. Descrição do Caso:Paciente do gênero masculino, 43 anos, queixava-se do formato dos seus dentes. Ao exame clínico percebeu-se desgaste dental nos incisivos centrais e linha do sorriso levemente invertida. Após duas sessões de clareamento de consultório com Peróxido de hidrogênio (35%) e mockup direto com resina composta, foi realizada a moldagem e escaneamento do modelo de gesso no laboratório. O enceramento digital foi aprovado, o modelo 3D foi impresso para confecção da guia de silicone. Com auxílio da guia foram executadas facetas diretas nos elementos 13, 12, 11, 21, 22 e 23. Conclusão:O fluxo digital pode ser uma alternativa viável para minimizar as falhas na confecção de facetas diretas em resina composta (AU).


Introduction:The improvement of composite resins in recent years, together with information disseminated on social media, has made direct veneers popular treatments in restorative dentistry. However, these procedures require significant manual dexterity and technical knowledge. Digital work flow using scanning, digital wax-up and 3D prototyping for the construction of guides has become an excellent alternative to increase predictability and the longevity of these procedures. Objective:Describe the manufacturing protocol for direct composite resin veneers, using a case report and digital to construct the 3D model, silicone guide and palatine walls. Case description:Male patient, 43 years old, complained of the shape of his teeth. Clinical examination revealed tooth wear on the central incisors and a slightly inverted smile line. After two whitening sessions with hydroigen peroxide (35%) and direct mockup with composite resin, the plaster model was molded and scanned in the laboratory. Digital wax-up was approved, and the 3D model was printed to manufacture the silicone guide. With the help of the guide, the direct veneers were applied to elements 13, 12, 11, 21, 22 and 23.Conclusions:Digital flow may be a feasible alternative to minimize manufacturing flaws in direct composite resin veneers (AU).


Introducción: La mejora de las resinas compuestas en los últimos años, y la difusión de información en las redes sociales, ha popularizado las facetas directas en los tratamientos en odontología restauradora. Sin embargo, son procedimientos que requieren demasiado destreza manual y conocimientos técnicos. El flujo digital usando escaneo, encerado digital y prototipado 3D para la construcción de guías se ha convertido en una excelente alternativa para aumentar la previsibilidad y la longevidad de estos procedimientos. Objetivo: Describir el protocolo para la realización de carillas directas en resina compuesta, a través de un reporte de caso, utilizando el planeo digital como ayuda para la realización de un modelo 3D, guía de silicona y paredes palatinas. Descripción del caso: Paciente masculino, 43 años, se quejó de la forma de sus dientes. El examen clínico reveló desgaste dental en los incisivos centrales y una línea de sonrisa levemente invertida. Después de dos sesiones de blanqueamiento en consultorio con peróxido de hidrógeno (35%) y maqueta directa con resina compuesta, el modelo de yeso fue moldeado y escaneado en el laboratorio. El encerado digital fue aprovado, el modelo 3D fue impreso para hacer la guía de silicona. Con la ayuda de la guía se realizaron carillas directas en los elementos 13, 12, 11, 21, 22 y 23. Conclusiones: El fluxo digital puede ser una alternativa viable para minimizar fallas en la fabricación de carillas directas en resina compuesta (AU).

Introducción: Los implantes subperiósticos personalizados de titanio, diseñadosa partir de un software de planificación yfabricación, constituyen una alternativaen la rehabilitación de atrofias severasen los maxilares, evitando cirugías de reconstrucción óseas y el de partes blandasmás complejas y disminuyendo los tiempos de cicatrización. El objetivo de este trabajo es presentar la rehabilitación enun caso clínico con una atrofia severa enmaxilar superior, utilizando una estructuracon implantes subperiósticos, medianteun protocolo digital.Descripción del caso: Se diseñó unaestructura subperióstica de titanio sinterizada con seis conexiones transepitelialesque fueron rehabilitadas con una prótesisfija inmediata implantosoportada fabricada en PMMA. Dos meses después, serealizó una estructura sinterizada en cromo-cobalto con bases mecanizadas recubierta con dientes de resina acrílica comorestauración final. En el seguimiento a unaño, el caso permanece estable.Conclusiones: Hoy en día, la rehabilitación con implantes subperiósticos constituye una herramienta alternativa en casosde cirugías complejas con grandes atrofias con la posibilidad, además, de realizar una carga inmediata. Introduction: Customized subperiosteal titanium implants, designed from planning and fabrication software, constitute analternative in the rehabilitation of severe maxillary atrophy, avoiding more complex bone and soft reconstruction surgeries and reducing healing times. The aim of this article is to present the rehabilitation in a clinical case with severe atrophy in the upper jaw, using a structure with subperiosteal implants, by means of a digital protocol.Clinical Case: A subperiosteal sintered titanium structure was fabricated with six transepithelial connections that were rehabilitated with an immediate implant-supported fixed prosthesis made of PMMA. Two months later, a sintered Chromium-Cobalt framework with machined bases covered with acrylic resin teeth was fabricated as the final restoration. At one year follow-up, the case remains stable. Conclusions: Nowadays, rehabilitatin with subperiosteal implants is an alternative tool in cases of complex surgeries with large atrophies with the possibility of immediate loading. (AU)

Describimos un método de planificación digital, entre lo tradicional y lo tecnológico, que conserva su esencia educativa, permite comprender la fractura e individualizar la osteosíntesis. Material y métodos: Tras el análisis inicial de la fractura y características del paciente, se realizan diferentes mediciones en las imágenes de Rx y TC con un programa de imagen médica digital. Estas imágenes se copian en un programa de presentación (Microsoft® PowerPoint o Keynote ©Apple Inc.), en el que se reproducen con el puntero del ordenador los principales fragmentos y líneas de fractura. A continuación, estos se mueven a una posición reducida y se representan gráficamente los implantes para la fijación interna junto con un guion de la estrategia quirúrgica. Resultados: Mostramos 4 casos de diferentes tipos de fracturas intervenidas mediante reducción y osteosíntesis tras una planificación preoperatoria según el método descrito. Se detallan los puntos básicos para la planificación quirúrgica, logística, táctica y los resultados radiológicos postoperatorios de cada caso. Conclusiones: A pesar del auge de programas informáticos avanzados de planificación, los métodos tradicionales con lápiz y papel siguen siendo fundamentales, más aún para el traumatólogo en formación. El método de planificación digital descrito resulta muy adecuado para este objetivo, al aunar las ventajas de ambos métodos: sencillez, accesibilidad, rapidez, bajo coste, reproducibilidad, carácter formativo y eficacia y por posibilitar la simulación, correcciones y la reutilización de casos.(AU)

The basic points for the surgical planning, logistics, tactics and postoperative radiological results of each case are detailed. Conclusions: Despite rise of advanced planning software, traditional paper and pencil methods are still fundamental, even more so for the trauma surgeon in training. The digital planning method described is very appropriate for this purpose, as it combines the advantages of both methods: simplicity, accessibility, quickness, low-cost, reproducibility, educational character, efficiency and possibility of simulation, corrections and reuse of cases.(AU)

ABSTRACT

Se expone el caso de una paciente de 76 años con antecedente de carcinoma epidermoide intraoral (2003) que tras la cirugía y radioterapia posterior presenta e dentulismo y atrofia maxilar severa.Tras la exploración clínica y radiológica con una tomografía axial computarizada (TAC) se decide rehabilitar el maxilar superior con implantes cigomáticos y una prótesis completa atornillada de carga inmediata. Mediante exportación DICOM del TAC maxilar al software Mimics (Materialise, Belgium) se realiza una planificación quirúrgica virtual tomando como referencia una propuesta digital protésica (DentalCAD, Exocad, USA). En el entorno 3-Matic (Materialise, Belgium) se diseñandos guías quirúrgicas óseos portadas para la colocación óptima de cuatro implantes cigomáticos combinando la técnica ZAGA (siglas en inglés de Zigoma Anatomy-Guided Approach o lo que es lo mismo, Abordaje Cigomático Guiado Anatómicamente) y Quad approach. Protésicamente, se diseña un prototipado que sirve como la tradicional prueba de dientes en cera y como férula radiológica prequirúrgica. El objetivo es simplificar la sistemática de trabajo con un nuevo método digital combinado gracias a la digitalización de los pilares de cicatrización de tipo Multi-Unit® (denominados healing caps). Con el presente caso clínico se desarrolla un protocolo de trabajo que simplifica toma de registros, abarata los costes y acorta el tiempo quirúrgico y de gabinete para la confección de una prótesis de carga inmediata de polimetil metacrilato acrílico (PMMA). Además, gracias al TAC postquirúrgico se compara y analiza la precisión aportada por las guías empleadas en el proceso de colocación de los implantes en relación con el software de planificación quirúrgica virtual (VSP –Virtual Surgical Planning-), confirmándose así la sencillez, exactitud y seguridad en todo el procedimiento quirúrgico. Se tomó como referencia para la comparación el extremo coronal y apical de cada implante cigomático junto con el ángulo resultante de la discrepancia entre el implante real y el digital. Los resultados obtenidos comparados con la bibliografía, demuestran una desviación sin repercusión en el proceso rehabilitador (AU)

ABSTRACT

El trasplante dentario autólogo se define como el po-sicionamiento de una pieza dentaria autóloga en un alvéolo post extracción o en un lecho formado qui-rúrgicamente. Actualmente, siguiendo los criterios de selección adecuados y una técnica quirúrgica mi-nuciosa, resulta una alternativa terapéutica ideal en ciertos pacientes. La gran variabilidad de implantes dentales y alternativas de injertos óseos han resuelto el problema de sustituir una pieza dentaria perdida. En los pacientes en crecimiento o con potencial de crecimiento residual del proceso alveolar, se pone en evidencia el inconveniente que puede representar la colocación de implantes, lo que hace necesario ree-valuar la conservación de dientes y la utilización de técnicas quirúrgicas clásicas. Las tasas de éxito de los autotrasplantes autólogos han aumentado con el tiempo, alcanzando cifras mayores al 90%. En el pre-sente artículo se reporta un caso clínico de autotras-plante dentario con planificación digital y una réplica tridimensional de la pieza a trasplantar (AU)


Autologous tooth transplantation is defined as the positioning of an autologous tooth in a post-extraction socket or in a surgically formed bed. Currently, following the appropriate selection criteria and a meticulous surgical technique, it is an ideal therapeutic alternative in certain patients. The great variability of dental implants and bone graft alternatives have solved the problem of replacing a lost tooth. In growing patients or with potential for residual growth of the alveolar process, the inconvenience that implant placement can represent is evident, which makes it necessary to reevaluate tooth conservation and the use of classic surgical techniques. The success rates of autologous autologous transplants have increased over time, reaching figures greater than 90%. This article reports a clinical case of dental autotransplantation with digital planning and a three-dimensional replica of the piece to be transplanted (AU)
