Table grapes are considered non-climacteric fruit, not showing a rapid increase in respiration rate and ethylene production during ripening. Previous research has suggested that abscisic acid (ABA) may have a more crucial role in grape postharvest behaviour. This study aimed to identify biomarkers of postharvest resilience and flavour life of imported table grapes. An experiment was designed to determine i) the role of ABA and catabolites on grape berry senescence; ii) the spatial distribution of these hormones within the grape berry, and iii) the effect of 1-MCP and storage temperature on its postharvest quality. Hence, the use of an ethylene inhibitor, 1-methylcyclopropane (1-MCP), during table grape storage was investigated. Table grapes (Vitis vinifera L.) cv. 'Krissy' were subjected to i) control (untreated); and ii) 1-MCP (1 µL L-1; 12 hours; 15°C) and stored under two scenarios: i) 15 days at 0.5°C, followed by five days at 5.5°C to simulate shelf-life; and ii) 20 days at 5.5°C to simulate a higher storage temperature followed by shelf-life. Physiological (i.e. mould incidence, skin colour, firmness, respiration rate) and biochemical analysis (i.e. individual sugars, organic acids, abscisic acid and catabolites) were performed. Grapes subjected to 5.5°C showed significantly higher mould incidence at the end of the shelf-life compared to 0.5°C storage temperature (12.6% vs. 3.1%). Also, and for the first time, the spatial distribution of ABA during the senescence of table grapes was profiled; the distal section had three times more ABA and metabolites than the proximal. We demonstrated that senescence processes were initiated after a significant increase in respiration rate (from 1 to 2.8 mL CO2 kg-1 h-1), and that ABA could be considered a biomarker for table grapes senescence, since an ABA peak preceded the increase in respiration rate, mould incidence, organic acids, and sucrose hydrolysis during postharvest storage; and coincided with a decrease in berry firmness. These findings are of significant importance for the industry as understanding how ABA regulates both senescence processes and quality changes during postharvest cold storage of tables grapes can improve the consistency and reduce waste and consumer complaints.

Non-accidental head trauma (NAHT) is a common cause of traumatic brain injury in childhood, origin of profound and disabling neurological sequalae, and in the most disgraceful cases, ultimately death.Subdural hematoma (SDH) is the most common intracranial finding in NAHT. On the other hand, congenital bleeding disorders are a minor but a significant cause of ICH in the neonate and toddler. Not uncommonly, intracranial bleeding is the first sign of a severe inherited coagulation disorder. In the presence of an unexpected intracranial bleeding after a minor trauma or without a clear history of the related events, physicians and caregivers may be confronted to the dilemma of a possible child abuse. It must be bear in mind that physical abuse and bleeding disorders can co-exist in the same child.We report here the case of two siblings in whom a diagnosis of hemophilia coexisted with the presumption of a non-accidental head trauma. Child abuses were inflicted in both children with a spare time of 2 years. A diagnosis of mild hemophilia was prompted in the first sibling after initial NAHT, while inflicted trauma was evident in the second sibling after neuroimaging findings and concomitant lesions. Lessons from this case in co-existing bleeding disorders and inflicted trauma and legal implications derived will be discussed thereafter.The possibility of a bleeding disorder should be considered in all children presenting with unexplained bleeding at a critical site in the setting of suspected physical maltreatment, particularly intracranial hemorrhage (ICH).

Using a combination of biochemical, transcriptomic, and physiological analyses, we elucidated the mechanisms of physical and chemical withering of tea shoots subjected to UV-C and ethylene treatments. UV-C irradiation (15 kJ m-2) initiated oxidation of catechins into theaflavins, increasing theaflavin-3-monogallate and theaflavin digallate by 5- and 13.2-4.4-fold, respectively, at the end of withering. Concomitantly, a rapid change to brown/red, an increase in electrolyte leakage, and the upregulation of peroxidases (viz. Px2, Px4, and Px6) and polyphenol oxidases (PPO-1) occurred. Exogenous ethylene significantly increased the metabolic rate (40%) and moisture loss (30%) compared to control during simulated withering (12 h at 25 °C) and upregulated transcripts associated with responses to dehydration and abiotic stress, such as those in the ethylene signaling pathway (viz. EIN4-like, EIN3-FBox1, and ERFs). Incorporating ethylene during withering could shorten the tea manufacturing process, while UV-C could enhance the accumulation of flavor-related compounds.

The effect of oxalic acid (OA) in determining poorly coloured table grape quality remains relatively unknown. Some red cultivars, such as seedless table grape 'Magenta' are characterised by a poor berry colour, an attribute highly demanded by the consumer. The aim of this research was to elucidate the effect of a preharvest OA treatment (5 mM) on berry colour and quality of table grape by investigating its role in berry development, on-vine ripening, and postharvest senescence. We found that OA significantly increased abscisic acid (ABA) and ABA glucose ester (ABA-GE) content in treated berries. This increase was mediated by changes in the ABA biosynthetic pathway, specifically by the upregulation of the 9-cis-epoxycarotenoid dioxygenase (VvNCED1) gene. The accumulation of ABA in treated berries resulted in colour improvement and a higher individual and total anthocyanins content at harvest compared with control; whereas at harvest, OA-treated table grapes showed a significantly lower glucose and fructose content and a higher content of tartaric, ascorbic, and succinic acids. Furthermore, antioxidant enzyme activity was increased during berry development in OA-treated berries. On the other hand, those berries treated with OA showed a delay in loss of firmness and colour during cold storage, as well as less susceptibility to postharvest decay incidence. This effect of OA delaying the senescence process was also related to enzymatic antioxidant system stimulation. For the first time, the role of OA on increasing quality, mainly colour, in table grapes was elucidated, highlighting that this treatment upregulated ABA metabolism, relative VvNCED1 gene expression and antioxidant system, delaying postharvest berry senescence.

PURPOSE: We describe our series of 4 patients with megalencephaly-capillary malformation syndrome (MCAP) and review the literature in order to assess the optimal treatment for the associated hydrocephalus. METHODS: We review our institutional series of hydrocephalus associated with MCAP and review the literature, analyzing the causes that could originate the hydrocephalus and the different types of treatments proposed for them. RESULTS: Of our patients treated with ventriculoperitoneal (VP) shunt, one suffered a surgical revision of the shunt and died due to a cranial trauma unrelated to her syndrome or the previous shunt surgery, and the other did not undergo surgical revisions until the end of her follow-up. Our patients treated with endoscopic third ventriculostomy (ETV) have improved their symptomatology and have not suffered of any complications related to the hydrocephalus after the ETV surgery. CONCLUSIONS: We update the treatment of MCAP-associated hydrocephalus and propose ETV as a valid treatment, as it seems a safe procedure with a low rate of complications.

Continuous ethylene supplementation suppresses postharvest sprouting, but it can increase reducing sugars, limiting its use as an alternative to chlorpropham for processing potatoes. To elucidate the mechanisms involved, tubers were treated after curing with or without the ethylene binding inhibitor 1-methylcyclopropene (1-MCP at 1 µL L-1 for 24 h), and then stored in air or air supplemented with continuous ethylene (10 µL L-1). Across three consecutive seasons, changes in tuber physiology were assessed alongside transcriptomic and metabolomic analysis. Exogenous ethylene alone consistently induced a respiratory rise and the accumulation of undesirable reducing sugars. The transient respiratory peak was preceded by the strong upregulation of two genes encoding 1-aminocyclopropane-1-carboxylate oxidase (ACO), typical of wound and stress induced ethylene production. Profiles of parenchymatic tissue highlighted that ethylene triggered abscisic acid (ABA) catabolism, evidenced by a steep fall in ABA levels and a transient rise in the catabolite phaseic acid, accompanied by upregulation of transcripts encoding an ABA 8'-hydroxylase. Moreover, analysis of non-structural carbohydrate-related genes revealed that ethylene strongly downregulated the expression of the Kunitz-type invertase inhibitor, already known to be involved in cold-induced sweetening. All these ethylene-induced effects were negated by 1-MCP with one notable exception: 1-MCP enhanced the sprout suppressing effect of ethylene whilst preventing ethylene-induced sweetening. This study supports the conclusions that: i) tubers adapt to ethylene by regulating conserved pathways (e.g. ABA catabolism); ii) ethylene-induced sweetening acts independently from sprout suppression, and is similar to cold-induced sugar accumulation.

Control of dormancy and sprouting in onion bulbs is commercially important for postharvest management. Although ethylene application is sometimes used to extend dormancy, the underlying mechanisms regulating dormancy transition remain unclear. Since the sprout leaves emerge from the bulb baseplate, we used this tissue to assess the impact of ethylene treatment and storage time on the hormone profile and the transcriptome. Reads from 30 libraries were assembled and annotated, with 94,840 unigenes retained after filtering. The de novo transcriptome assembly was of high quality and continuity (N50: 1809 bp, GC content: 36.21 %), and was used to analyse differential expression and Gene Onotologies. Across two years, applied ethylene resulted in delayed dormancy break and reduced post-dormancy sprout vigour. Ethylene supplementation enhanced endogenous ethylene production and caused a transient climacteric-like increase in respiration. Significant changes in hormone and associated transcript profiles occurred through storage and in response to ethylene. In particular, abscisic acid (ABA) and its metabolite phaseic acid (PA) increased under ethylene during the longer dormancy period; however, cytokinin increases observed during storage appeared largely independent of ethylene treatment. Several hormone-related transcripts showed differential expression over time and/or in response to ethylene. Expression of ethylene biosynthesis (ACO), receptor (EIN4) and transcription factor (EIL3) genes were modified by ethylene, as were ABA biosynthesis genes such NCED, and cytokinin biosynthesis genes such as LOG and CKX. We conclude that ethylene substantially modifies expression of genes in several phytohormone pathways, and some of these changes may underlie the dormancy-extending effects of exogenous ethylene.

Continuous supply of high quality onion bulbs to meet year-round demand is dependent on maintaining dormancy and bulb quality during storage. Sprouting impacts negatively on the storage quality of onion bulbs. Ethylene supplementation has previously been revealed to inhibit sprout growth in stored onion bulbs. Fructans content, especially those at higher degree of polymerisation (DP), are reported to positively correlate with delayed sprouting. However, little is known about the impact of pre-harvest irrigation regimes on fructans accumulation and redistribution in relation to onion bulb dormancy and quality in store. Across two seasons, onion plants of cultivars 'Red Baron' and 'Sherpa' were subjected to full irrigation (FI) (100% replenishment of crop evapotranspiration) or deficit irrigation (DI) (50% of FI treatment) from bulb initiation to harvest. Bulbs were harvested at full maturity and stored at 1 °C for five months. Bulbs were treated with or without 1-MCP (1 µL L-1) for 24 h before storage under continuous ethylene supplementation (10 µL L-1) or air. DI had no effect on dormancy-break, sprout emergence, total fructans content and total sugar content. In contrast, ethylene delayed sprout emergence and suppressed sprout growth; added 1-MCP enhanced this effect. The concentration of DP3-8 fructans were higher in top and bottom sections compared to the baseplate. Before sprout emergence, fructans of DPs 7-8 were no longer present in the top and bottom wedges, while they accumulated in the baseplate; irrespective of pre- or postharvest treatments. This redistribution of fructans within the bulb suggested a transition in dormancy state and could be used as a predictive marker for sprouting in stored onion bulbs.

BACKGROUND: 'Nules Clementine' mandarin was used to investigate the potential involvement of endogenous plant hormones in mediating citrus fruit susceptibility to rind breakdown disorder (RBD). The effect of light exposure (namely canopy position and bagging treatments) on the endogenous concentration of ABA, 7'-hydroxy-abscisic acid (7-OH-ABA), ABA-glucose ester (ABA-GE) and dihydrophaseic acid (DPA), and t-zeatin was tested using four preharvest treatments: outside, outside bagged, inside and inside bagged. Phytohormones concentration was evaluated during nine weeks of postharvest storage at 8 °C. RESULTS: The shaded fruit inside the canopy had the highest RBD score (0.88) at the end of postharvest storage, while sun-exposed fruit had the lowest score (0.12). Before storage, ABA concentration was lowest (462.8 µg kg-1 ) for inside fruit, and highest in outside bagged fruit (680.5 µg kg-1 ). Although ABA concentration suddenly increased from the third week, reaching a maximum concentration of 580 µg kg-1 at week 6 in fruit from inside position, it generally reduced 1.6-fold ranging from 240.52 to 480.65 µg kg-1 throughout storage. The increase of 7-OH-ABA was more prominent in fruit from inside canopy. Overall, the concentration of ABA-GE increased three-fold with storage time. DPA concentration of bagged fruit from inside canopy position was significantly higher compared to outside fruit. The lower ABA-GE and higher DPA concentration in inside bagged fruit throughout storage also coincided with higher RBD. CONCLUSION: The strong positive correlations between 7-OH-ABA, DPA and RBD incidence demonstrated that these ABA catabolites could be used as biomarkers for fruit susceptibility to the disorder. © 2019 Society of Chemical Industry.

Rapid analysis of oil-contaminated soils is important to facilitate risk assessment and remediation decision-making process. This study reports on the potential of a handheld mid-infrared (MIR) spectrometer for the prediction of total petroleum hydrocarbons (TPH), including aliphatic (alkanes) and polycyclic aromatic hydrocarbons (PAH) in limited number of fresh soil samples. Partial least squares regression (PLSR) and random forest (RF) modelling techniques were compared for the prediction of alkanes, PAH, and TPH concentrations in soil samples (n = 85) collected from three contaminated sites located in the Niger Delta, Southern Nigeria. Results revealed that prediction of RF models outperformed the PLSR with coefficient of determination (R2) values of 0.80, 0.79 and 0.72, residual prediction deviation (RPD) values of 2.35, 1.96, and 2.72, and root mean square error of prediction (RMSEP) values of 63.80, 83.0 and 65.88 mg kg-1 for TPH, alkanes, and PAH, respectively. Considering the limited dataset used in the independent validation (18 samples), accurate predictions were achieved with RF for PAH and TPH, while the prediction for alkanes was less accurate. Therefore, results suggest that RF calibration models can be used successfully to predict TPH and PAH using handheld MIR spectrophotometer under field measurement conditions.

Potato tuber bud dormancy break followed by premature sprouting is a major commercial problem which results in quality losses and decreased tuber marketability. An approach to controlling premature tuber sprouting is to develop potato cultivars with a longer dormancy period and/or reduced rate of sprout growth. Our recent studies using a potato diploid population have identified several quantitative trait loci (QTLs) that are associated with tuber sprout growth. In the current study, we aim to characterize a candidate gene associated with one of the largest effect QTLs for rapid tuber sprout growth on potato chromosome 3. Underlying this QTL is a gene encoding a TERMINAL FLOWER 1/CENTRORADIALIS homologue (PGSC0003DMG400014322). Here, we use a transgenic approach to manipulate the expression level of the CEN family member in a potato tetraploid genotype (cv. Désirée). We demonstrate a clear effect of manipulation of StCEN expression, with decreased expression levels associated with an increased rate of sprout growth, and overexpressing lines showing a lower rate of sprout growth than controls. Associated with different levels of StCEN expression were different levels of abscisic acid and cytokinins, implying a role in controlling the levels of plant growth regulators in the apical meristem.

Withering is considered a crucial stage of black tea processing. In this study, tea shoots from two cultivars (cvs. Yabukita and Clone 2) were stored at 5 °C, in either a low or high vapour pressure deficit (VPD) environment, to determine the impact of different withering rates on physiology (viz. respiration rate [RR], colour and moisture loss) and biochemical profile (viz. individual catechins, methylxanthines) of tea shoots (Camellia sinensis). Low VPD and high VPD conditions during withering increased caffeine levels in Clone 2 and Yabukita, respectively (p < 0.05). Caffeine levels steadily increased over time in both cultivars (p < 0.05), coinciding with a rapid decline in theobromine (TB). Furthermore, stems contained lower epigallocatechin gallate (EGCG) and caffeine (ca. 75 and 56%, respectively) compared to bud and larger leaf (LL) (p < 0.05). Overall, the results of this study highlight factors such as mechanical harvesting, and hard or soft withering, which could affect final tea beverage quality.

This study investigated the sensitivity of visible near-infrared spectroscopy (vis-NIR) to discriminate between fresh and weathered oil contaminated soils. The performance of random forest (RF) and partial least squares regression (PLSR) for the estimation of total petroleum hydrocarbon (TPH) throughout the time was also explored. Soil samples (n = 13) with 5 different textures of sandy loam, sandy clay loam, clay loam, sandy clay and clay were collected from 10 different locations across the Cranfield University's Research Farm (UK). A series of soil mesocosms was then set up where each soil sample was spiked with 10 ml of Alaskan crude oil (equivalent to 8450 mg/kg), allowed to equilibrate for 48 h (T2 d) and further kept at room temperature (21 °C). Soils scanning was carried out before spiking (control TC) and then after 2 days (T2 d) and months 4 (T4 m), 8 (T8 m), 12 (T12 m), 16 (T16 m), 20 (T20 m), 24 (T24 m), whereas gas chromatography mass spectroscopy (GC-MS) analysis was performed on T2 d, T4 m, T12 m, T16 m, T20 m, and T24 m. Soil scanning was done simultaneously using an AgroSpec spectrometer (305 to 2200 nm) (tec5 Technology for Spectroscopy, Germany) and Analytical Spectral Device (ASD) spectrometer (350 to 2500 nm) (ASDI, USA) to assess and compare their sensitivity and response against GC-MS data. Principle component analysis (PCA) showed that ASD performed better than tec5 for discriminating weathered versus fresh oil contaminated soil samples. The prediction results proved that RF models outperformed PLSR and resulted in coefficient of determination (R2) of 0.92, ratio of prediction deviation (RPD) of 3.79, and root mean square error of prediction (RMSEP) of 108.56 mg/kg. Overall, the results demonstrate that vis-NIR is a promising tool for rapid site investigation of weathered oil contamination in soils and for TPH monitoring without the need of collecting soil samples and lengthy hydrocarbon extraction for further quantification analysis.

Visible and near infrared spectrometry (vis-NIRS) coupled with data mining techniques can offer fast and cost-effective quantitative measurement of total petroleum hydrocarbons (TPH) in contaminated soils. Literature showed however significant differences in the performance on the vis-NIRS between linear and non-linear calibration methods. This study compared the performance of linear partial least squares regression (PLSR) with a nonlinear random forest (RF) regression for the calibration of vis-NIRS when analysing TPH in soils. 88 soil samples (3 uncontaminated and 85 contaminated) collected from three sites located in the Niger Delta were scanned using an analytical spectral device (ASD) spectrophotometer (350-2500nm) in diffuse reflectance mode. Sequential ultrasonic solvent extraction-gas chromatography (SUSE-GC) was used as reference quantification method for TPH which equal to the sum of aliphatic and aromatic fractions ranging between C10 and C35. Prior to model development, spectra were subjected to pre-processing including noise cut, maximum normalization, first derivative and smoothing. Then 65 samples were selected as calibration set and the remaining 20 samples as validation set. Both vis-NIR spectrometry and gas chromatography profiles of the 85 soil samples were subjected to RF and PLSR with leave-one-out cross-validation (LOOCV) for the calibration models. Results showed that RF calibration model with a coefficient of determination (R2) of 0.85, a root means square error of prediction (RMSEP) 68.43mgkg-1, and a residual prediction deviation (RPD) of 2.61 outperformed PLSR (R2=0.63, RMSEP=107.54mgkg-1 and RDP=2.55) in cross-validation. These results indicate that RF modelling approach is accounting for the nonlinearity of the soil spectral responses hence, providing significantly higher prediction accuracy compared to the linear PLSR. It is recommended to adopt the vis-NIRS coupled with RF modelling approach as a portable and cost effective method for the rapid quantification of TPH in soils.

Potatoes represent an important staple food crop across the planet. Yet, to maintain tuber quality and extend availability, there is a necessity to store tubers for long periods often using industrial-scale facilities. In this context, preserving potato quality is pivotal for the seed, fresh and processing sectors. The industry has always innovated and invested in improved post-harvest storage. However, the pace of technological change has and will continue to increase. For instance, more stringent legislation and changing consumer attitudes have driven renewed interest in creating alternative or complementary post-harvest treatments to traditional chemically reliant sprout suppression and disease control. Herein, the current knowledge on biochemical factors governing dormancy, the use of chlorpropham (CIPC) as well as existing and chemical alternatives, and the effects of pre- and post-harvest factors to assure potato tuber quality is reviewed. Additionally, the role of genomics as a future approach to potato quality improvement is discussed. Critically, and through a more industry targeted research, a better mechanistic understanding of how the pre-harvest environment influences tuber quality and the factors which govern dormancy transition should lead to a paradigm shift in how sustainable storage can be achieved.

BACKGROUND: It is has been suggested that dialysis patients have lower mortality rates for pulmonary embolism than the general population, because of platelet dysfunction and bleeding tendency. However, there is limited information whether dialysis is indeed associated with a decreased mortality risk from pulmonary embolism. OBJECTIVE: The aim of our study was to evaluate whether mortality rate ratios for pulmonary embolism were lower than for myocardial infarction and stroke in dialysis patients compared with the general population. METHODS: Cardiovascular causes of death for 130,439 incident dialysis patients registered in the ERA-EDTA Registry were compared with the cardiovascular causes of death for the European general population. RESULTS: The age- and sex-standardized mortality rate (SMR) from pulmonary embolism was 12.2 (95% CI 10.2-14.6) times higher in dialysis patients than in the general population. The SMRs in dialysis patients compared with the general population were 11.0 (95% CI 10.6-11.4) for myocardial infarction, 8.4 (95% CI 8.0-8.8) for stroke, and 8.3 (95% CI 8.0-8.5) for other cardiovascular diseases. In dialysis patients, primary kidney disease due to diabetes was associated with an increased mortality risk due to pulmonary embolism (HR 1.9; 95% CI 1.0-3.8), myocardial infarction (HR 4.1; 95% CI 3.4-4.9), stroke (HR 3.5; 95% CI 2.8-4.4), and other cardiovascular causes of death (HR 3.4; 95% CI 2.9-3.9) compared with patients with polycystic kidney disease. CONCLUSIONS: Dialysis patients were found to have an unexpected highly increased mortality rate for pulmonary embolism and increased mortality rates for myocardial infarction and stroke.

The volatile composition of fruit from four Citrus varieties (Powell Navel orange, Clemenules mandarine, and Fortune mandarine and Chandler pummelo) covering four different species has been studied. Over one hundred compounds were profiled after HS-SPME-GC-MS analysis, including 27 esters, 23 aldehydes, 21 alcohols, 13 monoterpene hydrocarbons, 10 ketones, 5 sesquiterpene hydrocarbons, 4 monoterpene cyclic ethers, 4 furans, and 2 aromatic hydrocarbons, which were all confirmed with standards. The differences in the volatile profile among juices of these varieties were essentially quantitative and only a few compounds were found exclusively in a single variety, mainly in Chandler. The volatile profile however was able to differentiate all four varieties and revealed complex interactions between them including the participation in the same biosynthetic pathway. Some compounds (6 esters, 2 ketones, 1 furan and 2 aromatic hydrocarbons) had never been reported earlier in Citrus juices. This volatile profiling platform for Citrus juice by HS-SPME-GC-MS and the interrelationship detected among the volatiles can be used as a roadmap for future breeding or biotechnological applications.

By looking through our cases and literature, an analysis of the surgical complications derived from direct cerebral stimulation under general anesthesia and local anesthesia and sedation was made. A retrospective descriptive study was performed including patients who were intervened in our centre from 2004 to 2008 and had the cortico-subcortical mapping technique. Common pre-operation variables were as follow: Age, sex, tumor localization and tumor's pathology; On patients intervened while awake, we collected the ASA, BMI and duration of the intervention. Afterwards, variable like epileptic attacks and cerebral edema were included in two groups. In addition, on those awake, respiratory and circulatory complications, local anesthesia toxicity, poor level of sedation, nausea and vomiting, pain and feeling uncomfortable with body posture were collected as well. A total of 20 patients had surgery. 10 of them were operated under local anesthesia and sedation with a mean age of 41 years, mean BMI of 26.8 and a pre-operatory ASA score of I or II ( except one patient with ASA III). The mean time duration of the surgical procedure was 5 hours and 20 minutes. On the other side, 10 patients were intervened with general anesthesia with a mean age of 55 years. There were no cases of cerebral edema in either group, although in 4 patients had epileptic attacks which resolved with cold saline irrigation. Five patients with local anesthesia and sedation did not have any complication, 2 patients showed desaturation episodes without further complications, three manifested hypertension episodes and two signs of feeling uncomfortable with body posture. None of the intra-operatory complications registered interfered wit the overall surgical procedure. This is a safe technique if performed following recommended indications and parameters. Regarding complications, respiratory complications are the most common and the most dangerous, as they can influence in the surgical procedure for difficult cases.

Mediante una revisión de nuestra casuística y dela literatura analizamos las complicaciones intraoperatoriasderivadas del uso de las estimulaciones cerebralesdirectas bajo anestesia general y anestesialocal y sedación. Se realizó un estudio retrospectivodescriptivo de los pacientes intervenidos (2004-2008)en que se utilizó la técnica del mapeo corticosubcortical.Se determinaron como variables preoperatoriascomunes: edad, sexo, localización tumoral, anatomíapatológica, y en los pacientes operados despiertos, seregistró el ASA, IMC y duración de la intervención.En los dos grupos se constataron la aparición de edemacerebral o crisis comiciales. En los pacientes despiertosse registró además las complicaciones respiratorias,hemodinámicas, toxicidad por anestésico local, nivelinadecuado de sedación, náuseas/vómitos, dolor eincomodidad postural. Bajo anestesia local y sedaciónse operaron 10, de 41 años de edad media, IMC mediode 26,8 y un ASA preoperatorio de I o II . La duraciónmedia de la intervención fue de 5 horas y 20 minutos.Mediante anestesia general se intervinieron 10 pacientes,de 55 años de media. En ningún caso de los dosgrupos se constató edema cerebral, 4 pacientes presentaroncrisis comiciales (autolimitadas con irrigación desuero frío). En 5 pacientes con anestesia local y sedaciónno registramos ninguna complicación, 2 pacientes episodiosde desaturación sin complicaciones, 3 episodiosde hipertensión y 2 incomodidad postural. Ninguna delas complicaciones intraoperatorias que registramosinterfirió en el desarrollo de la cirugía. Es una técnicasegura si se realiza siguiendo las indicaciones y losparámetros recomendados. Las crisis y complicacionesrespiratorias son las complicaciones más frecuentes ya su vez las más temidas, puesto que pueden influir encasos problemáticos en el proceso de la cirugía (AU)

UNLABELLED: There are various surgical approaches to treat Chiari I malformation. In spite of the good clinical results that are reported with most of them, there is still controversy about the optimal treatment of this pathology. OBJECTIVE: To compare the clinical and radiological results of surgical treatment of the Chiari I malformation with suboccipital craniectomy, posterior arch of C1 resection with or without dural graft, analyzing clinical and radiological findings and describing the complications. MATERIAL AND METHODS: Retrospectively clinical cases series of patients who underwent Chiari I malformation surgery between 1998 and 2006 in the Hospital Germans Trias i Pujol in Badalona. The inclusion criteria consisted in: patients older than 18 years, who have had surgery in our hospital, detailed neurological examination before and after surgery (calculating the EDSS scale punctuation), craniospinal magnetic resonance imaging before and after surgery and minimal follow up period of 6 months. The election of the surgical approach was left to the discretion of the main surgeon. Patients were divided in two groups depending of the surgical technique: Group A (with dural graft) and Group B (without dural graft). To evaluate the morphological results in both groups, measurements of the position of the fastigium above a basal line in the midsagittal T1 weighted magnetic resonance images were obtained. In patients with syringomyelia, siringo-to-cord ratio was measured before and after surgery. To evaluate the clinical results, neurological examination was recorded in both groups before and after surgery. RESULTS: The mean age of Group A patients was 47 (-/+12.89) years, and of Group B was 38.3 (-/+7.77) years. Mean follow up period was 2.48 (-/+2.44) years in Group A and 4.2 (-/+4.46) in Group B. Creation of an artificial cisterna magna was observed en 35.7% of Group A patients and only in 3.5% of Group B patients (p=0.022). In 8 patients front Group A, 8 patients (28.6%) an upward migration of the cerebellum was seen, whereas any of the Group B patients presented it (p=0.022) Siringo-to-cord ratios were decreased in both groups without significant differences. All of the Group A patients improved their clinical exploration. In Group B, 60% of the patients improved and the 40% left maintained clinical stability. Any patient worsened. All patients that maintained clinical stability belonged to Group B, the differences between the two groups were statistically significant (p=0.04). Five patients presented immediate surgical complications (2 pseudomeningoceles, 2 meningitis and 1 hydrocephalus). All this patients where operated with dural graft (p=0.049). CONCLUSIONS: According to our study, suboccipital craniectomy with resection of the posterior arch of C1 and dural graft shows better clinical and radiological results than without dural graft. Nevertheless this technique can increase the incidence of surgical complications.
