Introducción: La deficiencia de hierro en la insuficiencia cardiaca crónica (ICC), con o sin anemia concomitante, se halla asociada a la calidad de vida relacionada con la salud, clase funcional NYHA, y a la capacidad de realización de ejercicio. Estudios prospectivos aleatorizados han demostrado que la corrección de la deficiencia de hierro mejora la calidad de vida y el estadio funcional de estos pacientes con ICC, incluidos aquellos que no presentaban anemia. Objetivo: El objetivo de este estudio es analizar la frecuencia de determinaciones de estos parámetros de hierro y, por consiguiente, conocer la implementación de esta herramienta de mejoría de la calidad en pacientes que ingresan por ICC. Métodos: Estudio observacional retrospectivo sobre pacientes de un hospital universitario, que fueron diagnosticados al ingreso de ICC, entre el 1/1/2012 y el 11/6/2013. Resultados: El número de pacientes analizados fue de 824, de los que a un 39% (324) les fueron evaluados los parámetros de hierro. Entre los pacientes no evaluados y sí evaluados de hierro, no se observó diferencia significativa en la edad, aunque sí en el género, (p=0,007). Los valores del filtrado glomerular y de hemoglobina fueron significativamente inferiores en el grupo de pacientes analizados de hierro (p<0,001). La proporción de pacientes con anemia, insuficiencia renal y de aquellos que presentaban conjuntamente ambas comorbilidades fue significativamente superior en el grupo de pacientes analizados de hierro (p<0,001). Entre los 324 pacientes evaluados de parámetros férricos, 164 pacientes (51%) mostraban deficiencia de hierro. Entre los no deficientes y sí deficientes en hierro, no se observaron diferencias significativas en edad, ni en género. Los parámetros férricos de ambos grupos, ferritina e índice de saturación de la transferrina fueron significativamente inferiores entre los deficientes de hierro, (p<0,001). Los valores de filtrado glomerular fueron significativamente inferiores en aquellos que no mostraban deficiencia de hierro, (p<0,001). Se observaron igualmente diferencias significativas en la proporción de pacientes con insuficiencia renal, entre no deficientes y sí deficientes de hierro, (79 vs. 66%; p=0,013), aunque no en los valores de hemoglobina. Conclusión: La ICC se asocia con alta frecuencia a anemia, deficiencia de hierro e insuficiencia renal. El estudio de los parámetros férricos en los pacientes que ingresan con ICC, pese a que la corrección de la deficiencia de hierro se asocia a mejoría de la sintomatología, no se realiza con la frecuencia necesaria (AU)

OBJECTIVE: Arterial hypertension is a prevalent complication that occurs in 75-90% of kidney-transplant recipients. Data about resistant arterial hypertension are scarce. The aim of this multicenter, cross-sectional, and observational study was to assess the prevalence and the clinical features of true resistant hypertension among renal-transplant patients. METHODS: Eligible patients included hypertensive cadaveric kidney-transplant recipients aged below 70 years, with functioning kidney for at least 1 year, and with an estimated glomerular filtration rate at least 30 ml/min per 1.73 m and serum creatinine below 2.5 mg/dl. Recorded data included demographic characteristics, office blood pressure, and ambulatory blood pressure monitoring and laboratory investigations. A total of 868 patients (mean age 53.2 ±â€Š11.6 years) were included. RESULTS: Mean systolic and diastolic office blood pressure was 140.2 ±â€Š18 and 80.4 ±â€Š10 mmHg, respectively. Mean 24-h ambulatory SBP and DBP was 131.5 ±â€Š14 and 77.4 ±â€Š8.7 mmHg and the prevalence of true resistant hypertension was 18.9%. Those with resistant hypertension were older and men, with a worse cardiovascular risk profile and history of cardiovascular disease. Apart from this, these patients had worse graft function and treatment with steroids. CONCLUSIONS: The present study provides evidence about the prevalence of true resistant hypertension in renal-transplant patients. It also shows the very high cardiovascular risk of true resistant hypertension and the elevated association of this condition with renal failure, organ damage, and history of cardiovascular events.

In this study, we compared changes in inflammatory markers-C-reactive protein (CRP), pentraxin 3 (PTX3), serum component of amyloid A (SAA), and procalcitonin (PCT)-in 182 subjects: 69 from the general population (GP), 47 with CKD, 19 with an implanted intra-abdominal catheter for peritoneal dialysis ("prePD"), and 47 on peritoneal dialysis (PD). These were the results [median (95% confidence interval)] for the GP CKD, prePD, and PD groups respectively: CRP: 1.40 mg/L (1.15-2.10 mg/L), 5.30 mg/L (3.04-8.06 mg/L), 3.33 mg/L (2.15-12.58 mg/L), 7.25 mg/L (4.43-15.16 mg/L). SAA: 3.10 mg/L (2.90-3.53 mg/L), 7.77 mg/L (4.17-15.83 mg/L), 7.30 mg/L (4.81-10.96 mg/L), 9.14 mg/L (5.31-23.54 mg/L). PCT: 0.028 ng/mL (0.022-0.032 ng/mL), 0.121 ng/mL (0.094-0.166 ng/mL), 0.160 ng/mL (0.090-0.277 ng/mL), 0.363 ng/mL (0.222-0.481 ng/mL). PTX3: 0.54 ng/mL (0.33-0.62 ng/mL), 0.71 ng/ mL (0.32-1.50 ng/mL), 0.56 ng/mL (0.44-1.00 ng/ mL), 1.04 ng/mL (0.65-1.56 ng/mL). After catheter insertion, CRP showed a nonsignificant declining trend that disappeared throughout PD. The behavior of SAA was similar to that of CRP and was not modified by the changes induced by the start of PD. An increase in PTX3 was observed only with PD, which may be related to a local proinflammatory state caused by PD solution. We can conclude that catheter insertion for PD does not account for most of the local inflammatory changes observed in PD patients.

Chronic kidney disease (CKD) is associated with a proinflammatory state and an excess of cardiovascular risk. In this work, we describe changes in inflammatory markers-C-reactive protein (CRP), pentraxin 3 (PTX3), serum component of amyloid A (SAA), and procalcitonin (PCT)--in CKD patients compared with a control group of subjects with a normal estimated glomerular filtration rate (eGFR). Blood samples were obtained from 69 healthy individuals (GP) and 70 end-stage CKD patients--25 not yet on dialysis, 22 on peritoneal dialysis (PD), and 23 on hemodialysis (HD). These were the results [median (95% confidence interval)] for the GP CKD, PD, and HD groups respectively: CRP: 1.40 mg/L (1.19-2.11 mg/L), 6.50 mg/L (3.57-8.32mg/L), 7.60 mg/L (2.19-22.10mg/L), 9.60 mg/L (6.62-16.38 mg/L). SAA: 3.10 mg/L (2.90-3.53 mg/L), 7.11 mg/L (3.81-15.40mg/L), 9.69 mg/L (5.07-29.47mg/L), 15.90 mg/L (6.80-37.48 mg/L). PCT: 0.03 ng/mL (0.02-0.03 ng/mL), 0.12 ng/mL (0.09-0.16 ng/mL), 0.32 ng/mL (0.20-0.46 ng/ mL), 0.79 ng/mL (0.45-0.99 ng/mL). PTX3: 0.54 ng/mL (0.33-0.62 ng/mL), 0.71 ng/ mL (0.32-1.50 ng/mL), 1.52 ng/mL (0.65-2.13 ng/mL), 1.67 ng/mL (1.05-2.27 ng/mL). Compared with levels in the GP group, levels of SAA and CRP (systemic response) were significantly higher in CKD patients on and not on dialysis. Levels of PTX3 were higher only in dialyzed patients, significantly so in those on HD (greatly different from the CRP levels). These differing levels might be related to a local reaction caused by an invasive intervention (PD or HD). As eGFR declines and with the start of renal replacement therapy, PCT increases. Levels of PCT could potentially cause confusion when these patients are being evaluated for the presence of infection, and may also demonstrate some microvascular implications of dialysis therapy.

Kaposi's sarcoma (KS) is an angioproliferative neoplasia associated with human herpesvirus 8 (HHV-8) infection. HHV-8 generates KS by means of the secretion of vascular endothelial growth factor (VEGF) andup-regulation of VEGF receptor, KDR, in endothelial cells. We report a case of KS in a 72-year-old male with a renal transplant who had received immunosuppressant drugs including sirolimus, mycophenolate mofetil, tacrolimus and steroids. KS developed 11 months after transplantation, in relation to deep venous thrombosis and withdrawal of sirolimus due to toxicity. Multiple purple papules and nodules were observed exclusively in the limb affected by thrombosis. Diagnosis of KS was confirmed by biopsy. Progressive withdrawal of prednisone was accompanied by full remission of the tumour. The thrombosis and withdrawal of sirolimus may have acted as cofactors in the development of KS, favouring the activation of the VEGF/KDR autocrine loop. Our experience contributes to further evidence that sirolimus may protect against KS.

OBJECTIVES: To analyze the surgical aspects and complications from retransplantation into the iliac fossa for third and fourth kidney transplants. METHODS: Retrospective study of the 34 third and 5 fourth transplants performed in our department. We analyze patient's characteristics, surgical aspects and complications, and graft outcomes. RESULTS: Mean patient age was 41.6 years. 67% of the first and second transplants had been lost to vascular problems (19%) or chronic rejection (48%). Average time from last transplant in the retransplanted iliac fossa was 9 years (3 days-17 years). There were not significant differences between the groups of first and second transplant and third and fourth in cold ischemia time, number of mismatches, and number of days on hemodialysis after transplantation; there were significant differences in receptor age, number of transfusions, maximum and current antibodies and donor age, all of which were higher in third and fourth transplants. The graft was basically implanted in the right iliac fossa (71%) through a lumbar-iliac iterative incision; vascular anastomosis were equally made to the common and external iliac vessels; ureteral reimplant was performed following an extravesical technique; simultaneous transplant nephrectomy of the previous graft was performed in 33% of the cases. 59% of the cases had immediate diuresis and 49% did not require dialysis within the first 7 postransplant days. Surgical complications were mainly vascular: 4 cases of hemorrhage, 3 venous thrombosis and 2 arterial thrombosis. There were also 4 cases of lymphocele, 1 perirenal hematoma, and 1 enterocutaneous fistula with an abscess of the surgical bed. There were no urologic complications in the series. Globally, there was 1 death (2.5%) secondary to hemorrhage and another 6 grafts (15%) were lost to complications, 5 vascular thrombosis and 1 after surgical bed abscess. 1, 3, 5, and ten-year actuarial graft survival were 65%, 52%, 40% and 28% respectively. CONCLUSIONS: Retransplantation into the iliac fossa for third and fourth transplants is associated with a small increase in the number of surgical complications, mainly vascular complications.

OBJETIVO: Analizar los aspectos quirúrgicosy las complicaciones derivadas de la reutilizaciónde la fosa ilíaca para realizar terceros y cuartos retrasplantesrenales.MÉTODO: Estudio retrospectivo de los 34 terceros y 5cuartos retrasplante renales de nuestra serie de 1364trasplantes. Se analizan las características de lospacientes, aspectos y complicaciones quirúrgicas y laevolución de los injertos.RESULTADOS: La edad media fue de 41.6 años. El67% de los primeros y segundos trasplantes se habíanperdido por problemas vasculares (19%) o por rechazocrónico (48%). El tiempo medio desde el último trasplanteen la fosa ilíaca reutilizada fue de 9 años (3 días - 17 años). No hubo diferencias significativasentre el grupo de primer y segundo trasplante respectodel tercero y cuarto en cuanto al tiempo de isquemiafría, incompatibilidades y días de hemodiálisis post-trasplante;fueron significativas la edad del receptor, númerode trasfusiones, anticuerpos máximos y actuales yedad del donante, todos ellos mayores en los tercerosy cuartos. El injerto se colocó fundamentalmente en lafosa ilíaca derecha (71%) a través de un incisión lumboilíacaiterativa; la anastomosis vascular se hizo porigual a los ilíacos externos y comunes; el reimplante ureteralse hizo mediante técnicas extravesicales; en un33% de los casos se realizó trasplantectomía simultáneadel injerto anterior. En el 59% de los casos hubodiuresis inmediata y el 49% no precisó diálisis en los 7primeros días post-trasplante. Las complicaciones quirúrgicasfueron fundamentalmente vasculares; hubo 4casos de hemorragia, 3 trombosis venosas y 2 arteriales.También tuvimos 4 casos de linfocele, 1 hematomaperirenal y 1 fístula enterocutánea con absceso dellecho quirúrgico; no hubo en esta serie ninguna complicaciónurológica. En total hubo 1 fallecimiento(2.5%) debido a hemorragia y se perdieron otros 6injertos (15%) por las complicaciones, 5 por trombosisvasculares y otro por absceso del lecho quirúrgico.La supervivencia actuarial de los injertos fue del 65% alaño, 52% a los 3, 40% a los 5 y 28% a los 10 años.CONCLUSIONES: La reutilización de la fosa ilíacapara realizar terceros y cuartos trasplantes conlleva unligero aumento en las complicaciones quirúrgicas,especialmente vaculares

BACKGROUND AND OBJECTIVE: The correct management of patients with chronic renal disease depends on an early diagnosis. The aim of this study was to evaluate the usefulness, in the daily clinical practice, of the weight/creatinine formula as an indirect measurement of glomerular filtration. PATIENTS AND METHOD: 1,025 ambulatory patients were referred to the Nephrology Laboratory for basic blood and urine analysis. Creatinine clearance was calculated with the standard formula. RESULTS: A good correlation between the creatinine clearance adjusted for the corporal surface and that estimated by the weight/creatinine formula was observed, especially when creatinine levels were between 1.5-3 mg/dl and patients were older than 60 years. The mean difference between both methods was 6.3 (14.5) ml/min for males and 2.4 (10.5) ml/min for females. The weight/creatinine formula had a sensitivity of 91% and a specificity of 80% to detect a clearance below 50 ml/min. CONCLUSIONS: The weight/creatinine formula underestimates the clearance for normal creatinine values but fits quite well for creatinine levels between 1.5-3 mg/dl, mainly in patients older than 60 years. Although the estimation of clearance through this formula could be inaccurate, in most cases this is clinically irrelevant. Moreover, such a simple formula could avoid potential mistakes appearing at the time of evaluating renal function only by the serum creatinine.

FUNDAMENTO Y OBJETIVO: El tratamiento adecuado de los pacientes con insuficiencia renal crónica depende de su diagnóstico temprano. El objetivo de este estudio fue evaluar la utilidad, en la práctica clínica diaria, de la fórmula peso/creatinina como medida indirecta del filtrado glomerular analizando el posible error cometido con la valoración exclusiva de la creatinina sérica. PACIENTES Y MÉTODO: Se incluyó a 1.025 pacientes ambulatorios remitidos al laboratorio de nefrología para la realización de una analítica básica en sangre y orina. El aclaramiento de creatinina se calculó mediante la fórmula estándar con recogida de orina de 24 h ajustado a la superficie corporal y por el estimado mediante la fórmula peso/creatinina. RESULTADOS: Se observó una buena correlación entre el aclaramiento de creatinina corregido por superficie corporal y el estimado por la fórmula de peso/creatinina especialmente para valores de creatinina entre 1,5 y 3 mg/dl y para pacientes mayores de 60 años. La diferencia promedio (DE) de ambos métodos fue de 6,3 (14,5) ml/min para varones y 2,4 (10,5) para mujeres. La fórmula peso/creatinina tuvo una sensibilidad del 91 por ciento y una especificidad del 80 por ciento para detectar un aclaramiento inferior a 50 ml/min. CONCLUSIONES: La fórmula peso/creatinina estima muy a la baja el aclaramiento para valores de creatinina considerados normales, pero se aproxima mucho para cifras de creatinina entre 1,5 y 3 mg/dl en sujetos mayores de 60 años. Aunque la estimación del aclaramiento por la fórmula puede ser inexacta, en la mayoría de los casos esto es clínicamente irrelevante y una fórmula tan sencilla permite evitar el error de valorar la función renal únicamente por la creatinina (AU)
