BACKGROUND: Food allergy affects around 6% of the European population and its prevalence worldwide has been increasing in the last decades, but studies focused on investigating food allergy epidemiology in Europe are lacking. OBJECTIVE: The Cibus project was created to register the main culprit foods and their clinical manifestations in food allergic patients in Catalonia. METHODS: A specific online database was designed. Allergists from eight different Catalan hospitals registered the new diagnoses of food allergy. RESULTS: 618 food allergic patients were included. Egg and milk were the main elicitors in the early ages, while fruits and nuts were the most frequent in patients >14 years old. Fish was more frequent in children, while seafood and Anisakis allergy were more frequent in the >14-year-old group. Overall, peach was the most prevalent food eliciting an allergic reaction (10%). Food allergy diagnosis was reached using compatible clinical history and positive skin prick test to the involved food in 98% of cases. Globally, urticaria was the most frequently reported manifestation in our population (48.2%), followed by oral allergy syndrome (25.6%) and anaphylaxis (24.8%). CONCLUSIONS: The Cibus project gives a full overview of the profile of food allergic patients in Catalonia and reinforces the predominance of plant food allergies in the Mediterranean area

OBJECTIVE: Explore convergences and divergences between perception of nurses and of critically ill patients, in relation to the satisfactory care given and received. METHODS: It is part of a larger qualitative study, according to the Grounded Theory. Carried out in 3 intensive care units with 34 boxes. Sampling theoretical profiles with n=19 patients and n=7 nurses after data saturation. Recruitment of patients included in the profiles of elderly and long-stay got stretched over some time due to the low incidence of cases. Data collection consisted of: in-depth interview to critically ill patients, group discussion of expert nurses in the critical care patient and field diary. Analysis themed on Grounded Theory according Strauss and Corbin: open coding, axial and selective. Analysis followed criteria of Guba and Lincoln rigor, Calderón quality and Gastaldo and McKeever ethical reflexivity. There was a favorable report from the ethical committee of the Hospital and informed consent of the participants. RESULTS: Four matching categories were found: professional skills, human, technical and continued care. Combination of these elements creates feelings of security, calmness and feeling like a person, allowing the patient a close and trusting relationship with the nurse who takes individualized care. Not divergent categories were found. CONCLUSIONS: Perceptions of nurses in relation to care match perceptions of critically ill patients in both the definition and dimensions upon satisfactory care.

AIM: To investigate and understand patient's satisfaction with nursing care in the intensive care unit to identify the dimensions of the concept of 'satisfaction' from the patient's point of view. To design and validate a questionnaire that measures satisfaction levels in critical patients. BACKGROUND: There are many instruments capable of measuring satisfaction with nursing care; however, they do not address the reality for critical patients nor are they applicable in our context. DESIGN: A dual approach study comprising: a qualitative phase employing Grounded Theory and a quantitative and descriptive phase to prepare and validate the questionnaire. METHODS: Data collection in the qualitative phase will consist of: in-depth interview after theoretical sampling, on-site diary and expert discussion group. The sample size will depend on the expected theoretical saturation n = 27-36. Analysis will be based on Grounded Theory. For the quantitative phase, the sampling will be based on convenience (n = 200). A questionnaire will be designed on the basis of qualitative data. Descriptive and inferential statistics will be used. The validation will be developed on the basis of the validity of the content, the criteria of the construct and reliability of the instrument by the Cronbach's alpha and test-retest approach. Approval date for this protocol was November 2010. DISCUSSION: Self-perceptions, beliefs, experiences, demographic, socio-cultural epistemological and political factors are determinants for satisfaction, and these should be taken into account when compiling a questionnaire on satisfaction with nursing care among critical patients.

Introducción El grado de satisfacción es un indicador fundamental de la calidad asistencial. Existen numerosos instrumentos que miden la satisfacción con los cuidados enfermeros, aunque no responden a la realidad del paciente crítico ni a nuestro contexto. Objetivos Explorar y comprender la satisfacción de los pacientes ingresados en Cuidados Intensivos de un hospital nivel iii con los cuidados enfermeros, para definir e identificar las dimensiones del concepto de satisfacción desde su punto de vista. Material y métodos Estudio cualitativo según la teoría fundamentada en 3 unidades de Cuidados Intensivos de 34 boxes individuales. Muestreo teórico. La muestra, tras la saturación de datos, fue de 19 participantes. La recogida de datos fue a través de la entrevista en profundidad y diario de campo. Análisis de contenido según teoría fundamentada. Se siguieron los criterios de rigor de Guba y Lincoln. Informe favorable del comité de ética del centro y consentimiento informado. Resultados Emergen 4 categorías: definición y dimensiones del concepto de satisfacción, expectativas y vivencias. Los participantes incluyen en su definición de satisfacción las dimensiones: competencias profesionales, cuidados humanos, técnicos y continuados. La combinación de estos elementos produce sentimientos de seguridad, tranquilidad, estar vigilado, sentirse persona, percibiendo una relación cercana y de confianza con la enfermera que realiza cuidados individualizados. Conclusiones La definición y las dimensiones del concepto de satisfacción desde el punto de vista del paciente muestran los aspectos importantes para la persona además de clarificar sus dimensiones, permitiendo la construcción de instrumentos más acordes con el contexto y su percepción real (AU)

Conclusions The definition and dimensions of satisfaction concept from the patient's point of view show the important aspects of the person and also clarify their dimensions, allowing the construction of tools more in line with the context and real perception (AU)

BACKGROUND: Birch pollen-specific immunotherapy (SIT) decreases allergy to foods containing birch pollen-homologous allergens. Cross-reactivity was also observed between plane tree pollen and some vegetable foods. OBJECTIVE: The aim of this study was to evaluate the outgrowing of food allergy by patients suffering from vegetable food allergy associated with plane tree pollinosis (rhinoconjunctivitis and/or asthma) during plane tree pollen SIT. METHODS: An observational and prospective study was conducted in 16 adult patients suffering from vegetable food allergy (hazelnut, walnut, lettuce, peach and cherry) and from plane tree pollinosis receiving plane tree pollen SIT for 1 year. Open oral challenges with the implicated food were performed before and after SIT. Blood samples were drawn for measurement of pollen- and food-specific IgE and IgG4 before and after treatment. RESULTS: Plane tree SIT resulted in a significant decrease in food allergy, since the mean food quantity provoking objective symptoms increased from 2.19 to 13.74 g (p < 0.05), and 6 of the 11 patients tolerated the highest level (25 g) of the challenged food after plane tree SIT. Laboratory data also showed a decrease in IgE levels and an increase in IgG4 levels after immunotherapy. CONCLUSION: SIT with plane tree pollen has a positive impact on food allergy in plane tree pollen-allergic subjects.

Human seminal plasma allergy in women is an uncommon phenomenon. A great variety of reactions ranging from local swelling to generalized systemic reactions have been described, and local symptoms have often been misdiagnosed as chronic vulvovaginitis. Sperm barriers, such as condoms, are the most widely advocated method for avoiding these reactions; however this is not acceptable to couples who wish to have children. We present a case of a woman with human seminal plasma allergy who became pregnant after a fourth cycle of artificial insemination. Sodium dodecyl sulfate polyacrylamide gel electrophoresis immunoblotting showing an IgE binding band at 28kDa in the husband's seminal fluid identified the culprit allergen. Artificial insemination is an effective way to achieve a pregnancy in patients with seminal plasma allergy.

Introducción: Tacrolimus es un inmunosupresor tipo macrólido. Recientemente se ha introducido como un nuevo tratamiento para la dermatitis atópica (DA). Su acción se basa en la inhibición de la transcripción de citocinas en las células inflamatorias y del sistema inmunitario. Objetivos: Determinar la eficacia y seguridad a largo plazo ( 12 meses) del tratamiento con tacrolimus por vía tópica en pacientes con DA grave. Métodos: Estudio abierto de seguimiento prospectivo de eficacia y seguridad en niños de 2 a 12 años de edad con DA grave. Se prescribió tratamiento durante tres meses con dos aplicaciones diarias de tacrolimus por vía tópica obre las regiones afectas. Durante el .seguirniento se reiniciaron nuevo ciclos cuando presentaron exacerbaciones. La gravedad de la DA e determinó con el índice SCORAD al inicio y mensualmente durante el seguimiento. Se realizaron pruebas de laboratorio al inicio y final de cada tanda de tratamiento. Se pasó un cuestionario de efecto adversos una vez al mes. Resultados: Se incluyeron nueve pacientes en el estudio, 5 varones y 4 mujeres, con una edad media de 6,3 años y una duración media de la DA de 51 meses. Tras el tratamiento se observó una reducción del índice SCORAD estadísticamente significativa (p = 0,011 al mes: p = 0,012 a los dos meses: p = 0,012 a los tres meses de tratamiento y p = O.O 18 al mes de suspenderlo). Un 75% de los pacientes presentaron un aumento leve del prurito y enrojecimiento en las lesiones en las primeras aplicaciones de minutos a horas de duración. o detectamos alteraciones analíticas ni efectos adversos relevantes. Conclusiones: Tacrolimus en su forma de aplicación tópica e un tratamiento eficaz y seguro para los niños afectados de DA grave. El mantenimiento de dicha eficacia a largo plazo parece relacionarse con la gravedad inicial de la DA (AU)


Background: Tacrolimus is a macrolide-immunosupressor. It has recently been introduced as a new treatment of atopic dermatitis (AD). lts action is based on the inhibition of cytokine transcription in inflammatory and immune cell . Objectives: To determine long-term efficacy and safety of tacrolimus ointment treatment in patient with evere AD. Methods: Prospective open long-term study of safety and efficacy of tacrolimus ointment in 2 to 12 year old children with severe AD. Tacrolimus ointment was prescribed for three months twice a day over affected areas. Ewcye les of treatrnent were prescribed during the follow up period when severe AD reappeared. Severity of AD was determined based on SCORAD index, are the beginning and monthly during the follow-up period (12 months). A blood sample was taken at the beginning and after every cycle of treatment. Any adverse effect was registered monthly. Results: Nine patients were included. Five males and four females, with a medium age of 6.3 year and medium duration of AD of 51 months. The SCORAD index was significantly reduced after treatment (p=O.O 11 at first month: p=O.O 12 two month: p=O.O 12 three and p=O.O 18 one month after having finished the treatment). Seventy and five per cent of the patient presented a low increase of pruritus and buming in the first applications that disappeared in minutes-hours. Analytical alterations and clinical adverse events were not detected. Conclusions: Tacrolimus ointment is a safe and effective treatment for children with severe AD. The long-term efficacy seems to be related to the initial severity of AD (AU)
