A significant modification in photoinduced energy transfer in cancer cells is reported by the assistance of a dynamic modulation of the beam size of laser irradiation. Human lung epithelial cancer cells in monolayer form were studied. In contrast to the quantum and thermal ablation effect promoted by a standard focused Gaussian beam, a spatially modulated beam can caused around 15% of decrease in the ablation threshold and formation of a ring-shaped distribution of the photothermal transfer effect. Optical irradiation was conducted in A549 cells by a 532 nm single-beam emerging from a Nd:YVO4 system. Ablation effects derived from spatially modulated convergent waves were controlled by an electrically focus-tunable lens. The proposed chaotic behavior of the spatial modulation followed an Arneodo chaotic oscillator. Fractional dynamic thermal transport was analyzed in order to describe photoenergy in propagation through the samples. Immediate applications of chaos theory for developing phototechnology devices driving biological functions or phototherapy treatments can be considered.

Resumen Objetivo: Identificar la percepción de violencia en las relaciones de noviazgo en estudiantes de enfermería de una Universidad Pública de Minatitlán, Veracruz México. Materiales y Métodos: Estudio descriptivo, transversal y correlacional, la muestra fueron 182 estudiantes de enfermería inscritos en una universidad pública, de Minatitlán, Veracruz, México. El muestreo fue aleatorio simple. Se utilizó el Cuestionario de Violencia en las Relaciones de Parejas jóvenes (VIREPA), integrado por 20 ítems con una escala tipo Likert con 5 opciones de respuesta y preguntas sociodemográficas. Resultados: El 26.9% de los estudiantes indicó maltrato en sus relaciones pasadas y 3.8% maltrato en su relación actual. Se encontraron diferencias entre hombres y mujeres, en el maltrato emocional (z=-2.248, p=0.004) y el control social y económico (z=-2.681, p=0.002) con datos más elevados en los hombres. La edad del participante y la edad de la pareja se relacionó con el maltrato emocional, físico y psicológico y la desvalorización personal (p<0.05). Conclusiones: Los estudiantes universitarios de enfermería, han percibido experiencias de violencia durante sus relaciones de noviazgo, conducta que puede pasar desapercibida tanto para el perpetrador como para la víctima, por lo que se considera relevante el diseño de programas de prevención de la violencia en los contextos universitarios, dirigidos a identificar la violencia y su impacto en el estudiante, que aporte conocimiento y dé oportunidad para identificar cuando están viviendo una situación violenta dentro de sus relaciones de noviazgo.
Abstract Objective: To identify the perception of violence in dating relationships in nursing students of a public university in Minatitlán, Veracruz, Mexico. Methodology: Descriptive, cross-sectional and correlational study, the sample consisted of 182 nursing students enrolled in a public university in Minatitlán, Veracruz, Mexico. The sampling was simple random. The Violence in Young Partner Relationships Questionnaire (VIREPA) was used, composed of 20 items with a Likert-type scale with 5 response options and sociodemographic questions. Results: 26.9% of the students indicated abuse in their past relationships and 3.8% indicated abuse in their current relationship. Differences between males and females were found in emotional abuse (z=-2.248, p=.004) and social and economic control (z=-2.681, p=.002) with higher data in males. Participant's age and partner's age was related to emotional, physical and psychological mistreatment and personal devaluation (p<.05). Conclusions: University nursing students, have perceived experiences of violence during their dating relationships, behavior that can go unnoticed by both the perpetrator and the victim, so it is considered relevant the design of violence prevention programs in university contexts, aimed at identifying violence and its impact on the student, which provides knowledge and opportunity to identify when they are living a violent situation within their dating relationships.

Hyaluronan (HA) is a natural biodegradable biopolymer; its biological functions include cell adhesion, cell proliferation, and differentiation as well as decreasing inflammation, angiogenesis, and regeneration of damaged tissue. This makes it a suitable candidate for fabricating nanomaterials with potential use in tissue engineering. However, HA nanofiber production is restricted due to the high viscosity, low evaporation rate, and high surface tension of HA solutions. Here, hybrids in the form of continuous and randomly aligned polyvinyl alcohol (PVA)-(HA)-siloxane nanofibers were obtained using an electrospinning process. PVA-HA fibers were crosslinked by a 3D siloxane organic-inorganic matrix via sol-gel that restricts natural hydrophilicity and stiffens the structure. The hybrid nanofiber mats were characterized by FT-IR, micro-Raman spectroscopy, SEM, and biological properties. The PVA/HA ratio influenced the morphology of the hybrid nanofibers. Nanofibers with high PVA content (10PVA-8 and 10PVA-10) form mats with few beaded nanofibers, while those with high HA content (5PVA-8 and 5PVA-10) exhibit mats with mound patterns formed by "ribbon-like" nanofibers. The hybrid nanofibers were used as mats to support osteoblast growth, and they showed outstanding biological properties supporting cell adhesion, cell proliferation, and cell differentiation. Importantly, the 5PVA-8 mats show 3D spherical osteoblast morphology; this suggests the formation of tissue growth. These novel HA-based nanomaterials represent a relevant advance in designing nanofibers with unique properties for potential tissue regeneration.

Sentence-final completion tasks serve as valuable tools in studying language processing and the associated predictive mechanisms. There are several established sentence-completion norms for languages like English, Portuguese, French, and Spanish, each tailored to the language it was designed for and evaluated in. Yet, cultural variations among native speakers of the same language complicate the claim of a universal application of these norms. In this study, we developed a corpus of 2925 sentence-completion norms specifically for Mexican Spanish. This corpus is distinctive for several reasons: Firstly, it is the most comprehensive set of sentence-completion norms for Mexican Spanish to date. Secondly, it offers a substantial range of experimental stimuli with considerable variability in terms of the predictability of word sentence completion (cloze probability/surprisal) and the level of uncertainty inherent in the sentence context (entropy). Thirdly, the syntactic complexity of the sentences in the corpus is varied, as are the characteristics of the final word nouns (including aspects of concreteness/abstractness, length, and frequency). This paper details the generation of the sentence contexts, explains the methodology employed for data collection from a total of 1470 participants, and outlines the approach to data analysis for the establishment of sentence-completion norms. These norms provide a significant contribution to fields such as linguistics, cognitive science, and machine learning, among others, by enhancing our understanding of language, predictive mechanisms, knowledge representation, and context representation. Neutrophil extracellular trap formation (NETosis) is a mechanism used by neutrophils to capture pathogens with their own DNA. However, the exacerbation of this immune response is related to serious inflammatory diseases. Aging is known to lead to an excessive increase in NETosis associated with various diseases. Under this scenario, the search for strategies that regulate the release of NETosis in older people becomes relevant. High-intensity interval training (HIIT) involves repeated bouts of relatively intense exercise with alternating short recovery periods. This training has shown beneficial effects on health parameters during aging and disease. However, little is known about the potential role of HIIT in the regulation of NETosis in healthy older people. The aim of this study was to evaluate the induction of NETosis by serum from healthy young and older men, before and after 12 weeks of HIIT using healthy neutrophils as a biosensor. HIIT was performed 3 times per week for 12 weeks in young (YOUNG; 21 ± 1 years, BMI 26.01 ± 2.64 kgâ m-2, n = 10) and older men (OLDER; 66 ± 5 years, BMI 27.43 ± 3.11 kgâ m-2, n = 10). Serum samples were taken before and after the HIIT program and NETosis was measured with live cell imaging in donated neutrophils cultured with serum from the participants for 30 h. Our results showed that serum from older men at baseline induced greater baseline NETosis than younger men (p < 0.05; effect size, ≥0.8), and 12 weeks of HIIT significantly reduced (Interaction Effect, p < 0.05; effect size, 0.134) the induction of NETosis in older men. In conclusion, HIIT is a feasible non-invasive training strategy modulating NETosis induction. Additionally, the use of neutrophils as a biosensor is an effective method for the quantification of NETosis induction in real time.

OBJECTIVE: The visit-to-visit variability (VVV) of blood pressure (BP) has been recognized as a risk factor for cardiovascular events and chronic kidney disease (CKD). The objective of this study is to valuate the association between the VVV of BP and changes in estimated glomerular filtration rate (eGFR) in elderly CKD patients at different stages of renal function. MATERIALS AND METHODS: For 60 months, we analyzed the medical records of 105 patients with and without diabetes and hypertension. Systolic BP (SBP), diastolic BP (DBP), and pulse pressure (PP) were examined. A multivariable linear regression model was used to analyze the correlation between eGFR and the VVV of BP. RESULTS: No differences were demonstrated between the groups in the clinical characteristics. Mean SBP and DBP were not significant between the groups, and we observed no decrease in renal function. A significant negative correlation between PP and eGFR was observed in the total CKD population with a P of .010 (95% CI: -0.20, -0.03) and a correlation coefficient of -0.11. CONCLUSION: Our study shows no statistical significances in terms of the VVVs of BP in any of the geriatric groups, with no significant decreases in renal function. However, we observed a significant negative correlation between PP and eGFR. We demonstrated that if a VVV of BP does not occur, there is no decrease in eGFR.

BACKGROUND: Transcatheter aortic-valve implantation (TAVI) was introduced in 2002 and the first implants in our country were performed in 2010. AIM: To review the TAVI experience in our hospital, considering the technology improvements and gained experience throughout this period. MATERIAL AND METHODS: All patients undergoing TAVI in our center were included. Results and complications were adjudicated according to the Valve Academic Research Consortium-2 (VARC-2) criteria. Patients were divided in 3 groups, according to procedural year: Period 1: 2010-2015 (n = 35); Period 2: 2016-2018 (n = 35); Period 3: 2019-2021 (n = 41). Mortality up to one year after the procedure was recorded. RESULTS: Between 2010 and 2021, 111 TAVI procedures were performed. The mean age of patients was 82 years and 47% were women. Risk scores for in-hospital mortality were STS 6.7%, EUROSCORE II 8.0% and ACC/STS TAVR Score 4.9%. The trans-femoral route was used in 88% and a balloon-expandable valve was chosen in 82% of patients. A successful implant was achieved in 96%, with an in-hospital mortality of 1.8%. Mortality at 30 days and 1-year were 2.7 and 9.0%, respectively. During period 3, 100% of implants were successful, with no in-hospital mortality, less vascular complications (p < 0.01), less stroke (p = 0.04), less severe paravalvular leak (p = 0.01) and significantly lower rate of acute complications (p < 0.01). CONCLUSIONS: TAVI achieves excellent results. With greater experience and better available technologies, these results are even more favorable.

People with Down syndrome (DS) have several difficulties in language learning, and one of the areas most affected is language production. Theoretical frameworks argue that prediction depends on the production system. Yet, people with DS can predict upcoming nouns using semantically related verbs. Possibly, prediction skills in people with DS are driven by their associative mechanism rather than by the prediction mechanism based on the production system. This study explores prediction mechanisms in people with DS and their relationship with production skills. Three groups were evaluated in a preferential-looking task: young adults, children with DS, and a typically developing control group paired by sex and mental age. Participants saw two images, a target and a distractor. They also heard a sentence in one of the three conditions: with a verb that was closely related to the object (e.g., "The woman read the book"), with a verb that was moderately related to the object (e.g., "My uncle waited for the bus"), or with a verb that was unrelated to the object (e.g., "My sister threw a broom"). Their productive vocabulary was then measured. In the young adult and typically developing groups, the results showed prediction in sentences with highly and moderately related verbs. Participants with DS, however, showed prediction skills only in the highly related context. There was no influence of chronological age, mental age, or production on prediction skills. These results indicate that people with DS base prediction mainly on associative mechanisms and they have difficulty in generating top-down predictions.

Background: Mental health is an integral part of health. Having some psychiatric condition without treatment predisposes to a poor quality of life. These alterations have been found with an incidence of up to 50% in the general population. There are very few studies in Mexico on these conditions in health workers during the COVID-19 pandemic. Objective: To determine the prevalence of stress, anxiety, and depression during the COVID-19 pandemic in health workers. Material and methods: A descriptive, cross-sectional observational study was conducted on health workers in a tertiary hospital, through the application of randomized and self-completed surveys, in search of levels of stress, anxiety, and depression during the COVID-19 pandemic. Results: 44.7% of those surveyed showed depression, 83.1% anxiety, and 66.3% stress. The main groups affected were female staff, singles, and medical residents. Those with type 2 diabetes mellitus had higher anxiety and depression. Conclusion: The prevalence of stress, anxiety, and depression in health workers is similar to that of the general population in our study. Depression was the alteration most found. The scrutiny of alterations in mental health in health workers is of the utmost importance, with greater emphasis in the context of the aforementioned pandemic.
Introducción: la salud mental es parte integral de la salud. Tener algún padecimiento psiquiátrico sin tratamiento predispone a una mala calidad de vida. Estas alteraciones se han encontrado con una prevalencia de hasta 50% en la población general. En México hay muy pocos estudios acerca de la incidencia de dichos padecimientos en los trabajadores de la salud durante la pandemia por COVID-19. Objetivo: determinar la presencia de estrés, ansiedad y depresión durante la pandemia de COVID-19 en los trabajadores de la salud de un hospital de tercer nivel. Material y métodos: se realizó un estudio observacional transversal, descriptivo en los trabajadores de la salud de un hospital de tercer nivel, mediante la aplicación de encuestas aleatorizadas y autocomplementadas, en búsqueda de grado de estrés, ansiedad y depresión, durante la pandemia por COVID-19. Resultados: el 44.7% de los encuestados presentó depresión, 83.1% ansiedad y 66.3% estrés. Los principales grupos afectados fueron el personal femenino, solteros y médicos residentes. Aquellos con diabetes mellitus tipo 2 tuvieron mayor incidencia de ansiedad y depresión. Conclusión: la presencia de estrés, ansiedad y depresión en los trabajadores de la salud es similar a la de la población general en nuestro estudio. La depresión fue la alteración más encontrada. Es de suma importancia el escrutinio de alteraciones en la salud mental en los trabajadores de la salud, con mayor énfasis en el contexto de la pandemia mencionada.

Recently, several studies have introduced nanotechnology into the area of dental materials with the aim of improving their properties. The objective of this study is to determine the antibacterial and mechanical properties of type I glass ionomers reinforced with halloysite nanotubes modified with 2% chlorhexidine at concentrations of 5% and 10% relative to the total weight of the powder used to construct each sample. Regarding antibacterial effect, 200 samples were established and distributed into four experimental groups and six control groups (4 +ve and 2 -ve), with 20 samples each. The mechanical properties were evaluated in 270 samples, assessing microhardness (30 samples), compressive strength (120 samples), and setting time (120 samples). The groups were characterized by scanning electron microscopy and Fourier transform infrared spectroscopy, and the antibacterial activity of the ionomers was evaluated on Streptococcus mutans for 24 h. The control and positive control groups showed no antibacterial effect, while the experimental group with 5% concentration showed a zone of growth inhibition between 11.35 mm and 11.45 mm, and the group with 10% concentration showed a zone of growth inhibition between 12.50 mm and 13.20 mm. Statistical differences were observed between the experimental groups with 5% and 10% nanotubes. Regarding the mechanical properties, microhardness, and setting time, no statistical difference was found when compared with control groups, while compressive strength showed higher significant values, with ionomers modified with 10% concentration of nanotubes resulting in better compressive strength values. The incorporation of nanotubes at concentrations of 5% and 10% effectively inhibited the presence of S. mutans, particularly when the dose-response relationship was taken into account, with the advantage of maintaining and improving their mechanical properties.

La morfología y dimensiones de ciertas estructuras anatómicas varían de población a población, así como de individuo a individuo; el canal nasopalatino (CNP) es una de estas estructuras, este se encuentra ubicado en la línea media del paladar y aloja el nervio nasopalatino y la rama terminal de la arteria nasopalatina. El propósito de este estudio es determinar la morfología y dimensiones promedio del CNP en la población mexicana mediante tomografía computarizada de haz cónico (CBCT). Se analizaron 120 CBCT de manera coronal, transversal y sagital; y se clasificaron siguiendo los parámetros de Bornstein. Para el análisis estadístico se determinó la normalidad de las variables empleando la prueba de Shapiro Wilk y la significancia estadística mediante la prueba de UMann Whitney. Los resultados mostraron diferencias estadísticas significativas en las variables analizadas del canal nasopalatino entre hombres y mujeres. De acuerdo con los datos obtenidos se puede establecer que la morfología del CNP es muy variable y se recomienda realizar un estudio morfológico y dimensional antes de cualquier intervención quirúrgica relacionada con esta zona.
Certain human structures present different dimensions and morphologies in each population and individual, the nasopalatine canal being one of these. It is located in the midline of the palate, and it contains the nasopalatine nerve and the terminal branch of the nasopalatine artery. The purpose of this study was to analyze and record measurements of the nasopalatine duct in Mexican population by Cone Beam Computed Tomography (CBCT). A total of 120 CBCT coronal, transversal and sagittal views were analyzed. The data were classified according to Bornstein´s parameters. The normality of the variables was determined with the Shapiro Wilk test and the statistical significance was determinate by U-Mann Whitney test. A statistically significant difference was found in the evaluated variables of the nasopalatal canal between men and women. The data obtained determined that the morphology of the nasopalatine canal is variable and a morphological and dimensional analysis before any surgical intervention related with the area is recommended.

Introducción: las intervenciones educativas (IE) dirigidas al cuidador primario (CP) se tornan efectivas para la adopción de estilos de vida saludables al fomentarse el autocuidado, el cambio de conductas nocivas para la salud, la prevención y el tratamiento de síndromes agudos, así como en el aprendizaje de habilidades asociadas al cuidado. Su implementación es frecuente, sin embargo en ocasiones se omiten aspectos fundamentales en su desarrollo. Objetivo: analizar la producción científica de las IE dirigidas al CP a partir de los componentes utilizados para describir la calidad de las intervenciones educativas: estructura, proceso y resultado. Métodos: de acuerdo con los lineamientos de la declaración PRISMA se realizó una búsqueda a través bases de datos especializadas como: Pubmed, Scielo, Biblioteca Virtual en Salud, Biblioteca Digital UNAM, Cochrane, Medigraphic e Imbiomed. Se utilizaron los términos DeCS: Estilo de Vida, Autocuidado, Cuidadores, Intervención Educativa y Estudio de Intervención; así como los términos MeSH: Life-style, Selfcare, Caregivers y Health Cate Educational Interventions. Se seleccionaron publicaciones sobre IE para mejorar el autocuidado y/o estilos de vida del CP, de acceso libre a texto completo, publicados desde enero 2014 a noviembre 2018 en español, inglés y portugués. Se analizaron 21 investigaciones. Resultados: el 66% de las IE no mencionan la teoría desde la que dan sustento a su acción educativa, la práctica educativa fue de tipo unidireccional controladora tecnológicamente. Se observó que el 100% de las IE son efectivas. Conclusiones: la mayor parte de las IE son efectivas, sin embargo no fue posible identificar en gran parte de estas los aspectos fundamentales para su desarrollo, replica o evaluación.
Introduction: Educational interventions (El) aimed at the primary caregiver (CP) become effective for the adoption of healthy lifestyles by promoting self-care, changing harmful behaviors for health, prevention and treatment of acute syndromes, as well as in learning skills associated with care. Its implementation is frequent, however sometimes fundamental aspects are omitted in its development. Objective: To analyze the sdentific production of Els directed to the CP based on the components used to describe the quality of educational interventions: structure, process and result. Methods: According to the guidelines of the PRISMA statement, a search was conducted through specialized databases such as: Pubmed, Scielo, Virtual Health Library, UNAM Digital Library, Cochrane, Medigraphic and Imbiomed. The DeCS terms used were: Lifestyle, Self-care, Caregivers, Educational Intervention and Intervention Study; as well as the MeSH terms: Life-style, Selfcare, Caregivers and Health Care Educational Interventions. Publications on El were selected to improve the self-care and or lifestyles of the CP, with free access to full text, published from January 2014 to November 2018 in Spanish, English and Portuguese. 21 investigations were analyzed. Results: The 66% of the El do not mention the theory from which they sustain their educational action, the educational practice was technologically controlling unidirectional type. It was observed that 100% of El are effective. Conclusions: Most of the El are effective, however it was not possible to identify in a large part of these fundamental aspects for their development, replication or evaluation.

Resumen Objetivo: Describir un caso de histocitosis de células de Langerhans en un paciente pediátrico. Método: Paciente de 4 años con proptosis, diplopía, fiebre y pérdida de agudeza visual, con imagen en tomografía computarizada evidente de masa orbitaria con erosión ósea. Se realizó biopsia excisional por craneotomía coronal, el resultado histopatológico fue histiocitosis de células de Langerhans. Resultados: El estudio anatomopatológico transoperatorio mostró una neoplasia de células pequeñas redondas y azules, compatible con rabdomiosarcoma. Las pruebas de inmunohistoquímica concluyeron el diagnóstico de histiocitosis de células de Langerhans. Conclusión: La histiocitosis de células de Langerhans es una enfermedad poco frecuente que requiere de una sospecha clínica y diagnóstico oportuno, adecuar el tratamiento y mejorar la sobrevida de los pacientes.
Abstract Objective: To describe a case of Langerhans cell histiocytosis in a pediatric patient. Method: A 4-year-old patient with proptosis, diplopia, fever and loss of visual acuity, with evident image in a computed tomography of orbital mass with bone erosion. An excisional biopsy was performed by coronal craniotomy. The histopathological result was Langerhans cell histiocytosis. Results: The anatomopathological study showed a neoplasm of small round and blue cells, compatible with Rhabdomyosarcoma. Immunohistochemical tests concluded the diagnosis of Langerhans cell histiocytosis. Conclusion: Langerhans cell histiocytosis is a rare disease that requires clinical suspicion and a timely diagnosis to adjust the treatment and improve the survival of patients.

Rheumatoid arthritis (RA) has been associated with a higher risk of developing cardiovascular (CV) diseases. It has been proposed that systemic inflammation plays a key role in premature atherosclerosis development, and is therefore crucial to determine whether systemic components from RA patients promotes endothelial cell-oxidative stress by affecting reactive oxygen species (ROS) and nitric-oxide (NO) production. The aim of this study was to evaluate whether plasma from RA patients impair NO synthesis and ROS production by using the cell-line ECV-304 as a biosensor. NO synthesis and ROS production were measured in cells incubated with plasma from 73 RA patients and 52 healthy volunteers by fluorimetry. In addition, traditional CV risk factors, inflammatory molecules and disease activity parameters were measured. Cells incubated with plasma from RA patients exhibited reduced NO synthesis and increased ROS production compared to healthy volunteers. Furthermore, the imbalance between NO synthesis and ROS generation in RA patients was not associated with traditional CV risk factors. Our data suggest that ECV-304 cells can be used as a biosensor of systemic inflammation-induced endothelial cell-oxidative stress. We propose that both NO and ROS production are potential biomarkers aimed at improving the current assessment of CV risk in RA.

Ocular involvement is present in 50-60% of granulomatosis with polyangiitis (GPA) patients and can affect any part of the ocular globe. The present study describes ophthalmologic manifestations, association with systemic symptoms, disease activity and damage in GPA. A cross-sectional study was conducted including patients with GPA who underwent rheumatologic and ophthalmologic evaluation. Demographics, comorbidities, ophthalmologic symptoms, serologic markers, radiographic studies, disease activity and damage were assessed. Descriptive statistics, correlation, univariable logistic regression analyses, Student's t, Mann-Whitney U, Chi-square and Fisher's exact tests were performed. Fifty patients were included, 60% female, the median age was 56 years, disease duration 72.5 months. Nineteen (38%) patients had ocular manifestations at GPA diagnosis, scleritis being the most frequent; 27 (54%) patients presented ocular involvement during follow-up, repeated scleritis and dacryocystitis being the most common manifestations. Concomitant ophthalmic and sinonasal involvement was present in 12 (24%). Ocular and ENT damage occurred in 58% and 70%, respectively. Epiphora and blurred vision were the most frequent symptoms; scleromalacia and conjunctival hyperemia (27%) the most frequent clinical abnormalities. Ocular involvement at diagnosis was associated with concomitant ocular and sinonasal involvement at follow-up (OR 4.72, 95% CI 1.17-19.01, p = 0.01). Ocular involvement at follow-up was associated with age at GPA diagnosis (OR 0.94, 95% CI 0.90-0.99, p = 0.03), VDI (OR 1.29, 95% CI 1.03-1.61, p = 0.02), and ENT damage (OR 5.27, 95% CI 1.37-20.13, p = 0.01). In GPA, ocular involvement is frequent, therefore, non-ophthalmologist clinicians should be aware of this manifestation to reduce the risk of visual morbidity and organ damage.

Las presentes recomendaciones han sido elaboradas en base a nuestra experiencia, con el propósito de entregar algunas pautas sobre analgesia, sedación y anestesia para cateterismos diagnósticos y/o terapéuticos en pacientes pediátricos con cardiopatías congénitas, de acuerdo a la clasificación de riesgo de los mismos. El propósito de ésta es contribuir a disminuir la incidencia de eventos adversos asociados a morbilidad y mortalidad.
Recommendations based con clinical experience with analgesia, sedation and anesthesia in diagnostic or therapeutic cardiac catheterization in pediatric patients with congenital heart disease are outlined. These recommendations, based on clinical experience at a single institution (Catholic University Hospital in Santiago, Chile) are intended to help reduce de incidence of adverse effects, morbidity and mortality in this patient population.

Background: Mitral balloon valvuloplasty (MBV) is the therapy of choice for the treatment of symptomatic mitral stenosis with suitable anatomy. Although its short and mid-term results are favorable, there is a paucity of information about long-term follow-up. Aim: To assess the late results of MBV. Material and Methods: A cohort of 225 patients aged 8 to 20 years who were subjected to a MBV from 1989 to 2001, was studied. All variables at the time of the procedure, short and long-term results and major events during follow-up (new mitral intervention and mortality) were recorded. Uni and multivariate analysis were used to assess prognosis. Results: The mean follow-up lapse was 13.5 years (range 8 to 20 years). During this period, 88 patients (39.1%) remained event-free and in acceptable functional capacity. Eight percent died, 8% required a second MBV and 43.5% required a surgical mitral valve replacement. A post-procedural area equal or greater to 1.9 cm² was associated with a greater likelihood of free-event survival (log rank test: p = 0.02/Cox proportional regression model: coefficient 0.54, p = 0.04). Conclusions: MBV is effective, although there is a high chance of new interventions in the long-term follow-up. A larger post-procedure mitral area is associated with a better prognosis.

BACKGROUND: The Cobra-P drug-eluting stent (DES) system consists of cobalt chromium alloy with bio-absorbable siloxane sol-gel matrix coating that elutes low dose paclitaxel within 6 months. The aim of this first-in-man trial was to evaluate the safety and performance of 2 doses of the Cobra-P DES. METHODS: A total of 60 lesions (54 patients) were sequentially assigned to 2 different paclitaxel doses: group A (3.7 µg/18mm, n=30) or group B (8 µg/18mm, n=30). The primary endpoint was MACE at 4 months defined as cardiac death, myocardial infarction, and target lesion revascularization. RESULTS: Patient and lesion characteristics were matched between the 2 groups except for male sex. MACE at 4 months was 3.3% and 0% respectively (P=1.000) and at 1-year follow-up remained unchanged. In-stent late loss at 4 months was similar in both groups (0.36 ± 0.30mm and 0.34 ± 0.20mm P=.773). CONCLUSIONS: In this FIM study, implantation of the Cobra-P low dose paclitaxel-eluting stent with a bioabsorbable sol-gel coating was proven to be feasible and safe. Moderate neointimal proliferation was observed as well as an acceptable MACE rate up to 1 year.
