The COVID-19 pandemic has driven the search for alternative therapies, including convalescent plasma, historically used in infectious diseases. Despite results in other diseases, its effectiveness against COVID-19 remains uncertain with conflicting results in clinical trials. A pragmatic, single-center, prospective, and open randomized controlled trial was carried out in a hospital in Brazil, with the aim of evaluating the impact of convalescent plasma on the clinical improvement of patients hospitalized with COVID-19. The World Health Organization (WHO) ordinal scale was used to measure clinical improvement, focusing on the reduction in disease severity by up to 2 points, while antibody and C-reactive protein levels were monitored over time. After hospital admission, participants were randomized 1:1 to receive convalescent plasma and standard treatment or to be part of the control group with standard treatment. Follow-up was carried out on days 1, 3, 7, 14 and/or at discharge. From January 14 to April 4, 2022, 38 patients were included, but 3 were excluded due to protocol deviations, resulting in a total of 35 patients: 19 in the control group and 16 in the plasma group. There was no significant difference in clinical improvement between the convalescent plasma group and the control group, nor in secondary outcomes. The study had limitations due to the small number of patients and limited representation of COVID-19 cases. Broader investigations are needed to integrate therapies into medical protocols, both for COVID-19 and other diseases. Conducting randomized studies is challenging due to the complexity of medical conditions and the variety of treatments available.

OBJECTIVE: This analysis assessed the association between regional income, screening coverage for cervical and breast cancer, and temporal trends in mortality from these cancers in different Brazilian health regions. STUDY DESIGN: Spatiotemporal ecological study across 450 health regions of Brazil. METHODS: Data from 2010 Demographic Census were used to assess income. Variations in income distribution within health regions were measured using the Gini index. Data on screening coverage were obtained from the Ambulatory Information System (SIA/SUS). Mortality was assessed from the Global Burden of Disease Study 2019 data. The average annual percentage change (AAPC) in cervical and breast cancer mortality rates, 2010-2018, was calculated by health regions. Results were presented in regional maps. The associations between income, screening coverage and mortality changes were estimated by bivariate spatial correlation. RESULTS: Health regions located in the South and Southeast regions of Brazil had the greatest percentages of screening coverage and highest per capita incomes with the lowest Gini index values. From 2010 to 2018, mortality rates for cervical cancer were highest in the North and Northeast health regions. Breast cancer mortality rates were highest in the South and Southeast health regions. The AAPC in breast and cervical cancer mortality had a negative association with per capita income and screening coverage, and a positive association with the Gini index. CONCLUSIONS: There are large regional variations in income, screening coverage, and mortality rates for women with breast and cervical cancer. These inequities could be mitigated by policies to address income disparities and improved access to screening.

Nanoemulsions are increasingly gaining importance in the development of topically applied medicine and cosmetic products because their small droplets favor the penetration rates of active compounds into the body. In this scenario, the measurements of their diffusion rates as well as eventual physicochemical changes in the target tissues are of utmost importance. It is also recognized that the use of natural surfactants can avoid allergic reactions as frequently observed for synthetic products. The natural saponins extracted from Sapindus Saponaria have the property of forming foam and are exploited as biocompatible and biodegradable, while cellulose nanocrystals are known to increase the stability of a formulation avoiding the coalescence of drops at the interface. Therefore, nanoemulsions combining natural saponins and cellulose nanocrystals are promising systems that may facilitate greater diffusion rates of molecules into the skin, being candidates to substitute synthetic formulations. This study applied the Photoacoustic Spectroscopy technique to measure the diffusion rates and the physicochemical properties of nanoemulsified formulations containing saponins and cellulose nanocrystals topically applied to the skin. The ex vivo study combined the first-time photoacoustic measurements performed in both ultraviolet-visible and mid-infrared spectral regions. The toxicity of these formulations in L929 cells was also evaluated. The results showed that the formulations were able to propagate throughout the skin to a depth of approximately 756 µm, reaching the dermal side. The non-observation of absorbing band shifting or new bands in the FTIR spectra suggests that there were no structural changes in the skin as well as in the formulations after the nanoemulsions administration. The cytotoxicity results showed that the increase of cellulose nanocrystals concentration decreased cellular toxicity. In conclusion, the results demonstrated the advantage of combining photoacoustic methods in the ultraviolet-visible and mid-infrared spectral regions to analyze drug diffusion and interaction with the skin tissues. Both methods complement each other, allowing the confirmation of the nanoemulsion diffusion through the skin and also suggesting there were no detectable physicochemical changes in the tissues. Formulations stabilized with saponins and cellulose nanocrystals showed great potential for the development of topically administered cosmetics and drugs.

OBJECTIVES: The most recent survey conducted by the World Health Organization described Tuberculosis (TB) as one of the top 10 causes of death and the leading cause of death from a single infectious agent. The increasing number of TB-resistant cases has contributed to this scenario. In light of this, new strategies to control and treat the disease are necessary. Our research group has previously described furoxan derivatives as promising scaffolds to be explored as new antitubercular drugs. RESULTS: Two of these furoxan derivatives, (14b) and (14c), demonstrated a high selectivity against Mycobacterium tuberculosis. The compounds (14b) and (14c) were also active against a latent M. tuberculosis strain, with MIC90 values of 6.67 µM and 9.84 µM, respectively; they were also active against monoresistant strains (MIC90 values ranging from 0.61 to 20.42 µM) and clinical MDR strains (MIC90 values ranging from 3.09 to 42.95 µM). Time-kill experiments with compound (14c) showed early bactericidal effects that were superior to those of the first- and second-line anti-tuberculosis drugs currently used in therapy. The safety of compounds (14b) and (14c) was demonstrated by the Ames test because these molecules were not mutagenic under the tested conditions. Finally, we confirmed the safety, and high efficacy of compounds (14b) and (14c), which reduced M. tuberculosis to undetectable levels in a mouse aerosol model of infection. CONCLUSION: Altogether, we have identified two advanced lead compounds, (14b) and (14c), as novel promising candidates for the treatment of TB infection.

The primary job of the epididymis is to mature and protect the luminally transiting spermatozoa. Mounting evidence is showing that innate immune components [including Toll-like receptors (TLRs) and antimicrobial proteins, among which are ß-defensins] and inflammatory mediators, under the primary influence of androgens, participate in the cellular and molecular processes that define this tissue. Here, we present an overview of the contributions of these signaling pathway components during epididymal homeostasis and discuss the hypotheses as to their involvement in epididymitis, the most common urological inflammatory condition in men, frequently impairing their fertility. Drawing primarily from rodent models, we also focus on how the distribution and functional expression of innate immune components are differentially regulated in the prenatal developing epididymis, providing new insights into the disruption of these signaling pathways throughout the lifespan. Male infertility is caused by a variety of conditions, such as congenital malformations, genetic and endocrine disorders, exposure to environmental toxicants, and inflammatory/infectious conditions. More than one-third of infertile men with an idiopathic condition cannot currently be adequately diagnosed. Thinking about the innate immunity and inflammation context of the epididymis may provide new insights and directions as to how these systems contribute to male fertility, as well as also uncover urological and andrological outcomes that may aid clinicians in diagnosing and preventing epididymal pathologies.

Zoonotic visceral leishmaniasis (ZVL), caused by protozoans of the genus Leishmania, it is a worldwide of great importance disease. In the northeast region of Brazil, the state of Alagoas has an endemic status for ZVL. Thus, this work aimed to analyze the epidemiological situation of human and canine visceral leishmaniasis in Alagoas, Northeast, Brazil, from 2007 to 2013. We conducted a descriptive, observational, retrospective study using secondary data from the Notifiable Diseases Information System, the Center of Zoonosis Control of Maceió, and the Central Laboratory of Public Health of Alagoas. During the studied period, it was observed that the highest incidence of human visceral leishmaniasis was in 2011 and the lowest in 2013. On the other hand, canine visceral leishmaniasis had its highest incidence in 2007 and its lowest in 2012. Of the 55 municipalities in the State of Alagoas that showed human visceral leishmaniasis (HVL), São José da Tapera presented an average of 4.4 cases over the past five years, being classified as of intense transmission. Regarding canine visceral leishmaniasis, in the same studied period, 45,112 dogs were examined in the State, of which 4,466 were positive. It resulted, thus, in a 9.9% positivity rate. Conclusions: Our data are important because canine infection is an important risk factor for the human disease.(AU)

AIM: To evaluate the participation of both Th1 and Th2 responses in periapical cysts by assessing the presence of M2 macrophages, as well as acute IL-1 ß, TNF-α and IL-6 cytokines. METHODOLOGY: Twenty-four cases of periapical cysts were selected. Immuno-expressions of IL-1 ß, IL-6, TNF-α and CD163 were analysed in the cystic capsules in both superficial and deeper regions. Data were analysed with paired Wilcoxon test and Spearman correlation coefficient (P ≤ 0.05). RESULTS: There was a higher expression of IL-1ß, IL-6, TNF-α and M2 macrophages in the superficial region (P < 0.001) of cystic capsules. All acute cytokines had significant positive correlations amongst them regardless of the cystic capsule region. Regarding CD163, positive correlations occurred only with TNF-α (P = 0.007; r = 0.537) and IL-6 (P = 0.018; r = 0.478) in the superficial regions of the cystic capsule. CONCLUSIONS: M2 macrophages participated actively in the inflammatory response of periapical cysts and correlated with the expression of certain acute Th1-related cytokines. This illustrates the coexistence of an acute and chronic Th2-driven immune response in these lesions. Although M2 macrophages favour the healing process, their presence is not sufficient for periapical cyst regression, once an acute active response has occurred due to an infectious stimuli.

O planejamento e execução de ações de manejo das populações de cães e gatos em áreas urbanas é um grande desafio para os gestores da saúde, pela falta de diagnóstico situacional relacionado à realidade da população destes animais no território urbano do município. São indicados três métodos para o controle populacional de cães e gatos: restrição da movimentação, controle do habitat e controle reprodutivo. As ações de educação permanente em saúde para a posse responsável de animais de estimação e a esterilização cirúrgica de cães e gatos são medidas indicadas como estratégias de controle populacional para essas espécies. Com base nestas afirmações, o objetivo do presente trabalho foi realizar um diagnóstico situacional do aspecto de castração dos animais levados aos postos de vacinação contra a Raiva, em 2013, na 18ª Campanha de vacinação anual contra a Raiva de cães e gatos de Lençóis Paulista/SP. Os dados foram obtidos a partir da ficha de cadastro de animais que foi utilizada na campanha de 2013, que continham campos para preenchimento de informações relacionadas à castração e à localização do proprietário. Com base nesses dados realizou-se a análise descritiva bem como a distribuição espacial dos resultados. O software ArcGIS 10.1® foi utilizado para elaboração dos mapas. HIV/AIDS is an extremely important public health challenge. Bipolar disorder spectrum has a significant prevalence, reported to be around 2.6%. This study analyses the relationship between sexual behaviors among MDQ positive and MDQ negative patients and its influence in HIV infection, and the impact of HIV diagnosis in both groups. Two hundred outpatients from a specialized clinic for HIV-care located in Walter Cantídio's University Hospital in Fortaleza, Brazil answered to a demographic questionnaire, the Mood Disorder Questionnaire (MDQ) and a sexual behavior questionnaire based on WHO's Behavioral Surveillance Surveys (BSS). Fifteen percent (N=30) of all HIV positive patients were MDQ+. The MDQ+ group was more likely to: be or have been married, have offspring, have sex with commercial and non-regular partners, have infrequent condom use with non-regular partners and of not have used condom in their first sex. Despite more sexual practices among MDQ+ patients before HIV diagnosis, these patients had a more significant reduction of all behaviors after HIV diagnosis than the MDQ- group.

The epididymis is known to be exposed to a constant risk of inflammatory conditions resulting from bacterial infections, or from idiopathic and noninfectious causes. A common result of these conditions is epididymitis, a pathology that may lead to the destruction of the epididymal duct and transient or permanent sterility. The negative impact of epididymitis on semen quality and fertility justifies the need for a better understanding of cellular and molecular mechanisms by which innate immunity is modulated in the epididymis and the pathophysiology of this clinical condition. Our laboratory has been interested in the last years in the cellular and biochemical events involved in the epididymal defense response against a bacterial challenge, as well as in how these responses are integrated by glucocorticoids and the glucocorticoid receptor. We have been also interested in naturally occurring antimicrobial proteins secreted by the epididymis. Antimicrobial proteins are known to be part of the innate immune system, with potential biological role during a defense response against invasion by pathogens in the epididymis. Here we review the expression and regulation of antimicrobial proteins, particularly beta-defensins in the epididymis, highlighting isoforms of the beta-defensin-like SPAG11B gene. Aspects of their broad range of biological roles and potential use as targets to help to prevent or treat diseases, such as epididym itis, are also discussed.

We have been also interested in naturally occurring antimicrobial proteins secreted by the epididymis. Antimicrobial proteins are known to be part of the innate immune system, with potential biological role during a defense response against invasion by pathogens in the epididymis. Here we review the expression and regulation of antimicrobial proteins, particularly beta-defensins in the epididymis, highlighting isoforms of the beta-defensin-like SPAG11B gene. Aspects of their broad range of biological roles and potential use as targets to help to prevent or treat diseases, such as epididym itis, are also discussed.(AU)

Squamous cell carcinomas (SCCs) are amongst the commonest malignancies in adults but in paediatric patients are exceptionally rare, particularly those involving the oral mucosa. The aim of the present report is to describe the features of a gingival well-differentiated SCC in a 7-year-old Brazilian boy. Immunostaining for p53, Ki-67 and Mcm2 showed increased cellular proliferation compared with normal epithelium. In situ hybridization failed to identify human papilloma virus infection. Correct diagnosis of well-differentiated squamous carcinoma can be difficult in children and differentiation from pseudoepitheliomatous hyperplasia is essential to establish proper treatment.

Tritrichomonas fetus causes infertility and abortion in cattle; however, there is scarce information regarding the susceptibility of bovine sperm to this parasite. The objective of this study was to analyze in vitro the interaction between T. fetus and bovine sperm and to evaluate the effect of extracellular products secreted by the parasite on these reproductive cells. Sperm from five fertile bulls (Bos taurus taurus, Holstein-Friesian), selected through a Percoll gradient, adhered to T. fetus after 30min of interaction, resulting in agglutination between the two kinds of cells. Based on reverse transcription-polymerase chain reaction (RT-PCR), T. fetus continuously expressed its gene for cysteine peptidase in the presence or absence of sperm. Computer-assisted semen analysis (CASA) revealed that, after 1h incubation of sperm in T. fetus culture extract, the extracellular products secreted by the parasite decreased sperm progressive motility (P<0.05). Although T. fetus extracellular products did not lead to loss of sperm viability (P<0.05) based on the Annexin-V/propidium iodide assay, the percentage of Annexin-V fluorescein isothiocyanate-positive and propidium iodide-positive cells increased (P<0.05) during incubation of sperm in T. fetus culture extract, consistent with cellular damage. In conclusion, extracellular products secreted by T. fetus were cytotoxic to bovine sperm, as they decreased sperm progressive motility; perhaps this contributes to the pathogenesis of T. fetus-induced infertility.

A dibenzothiophene (DBT)-degrading bacterial strain able to utilize carbazole as the only source of nitrogen was identified as Gordonia sp. F.5.25.8 due to its 16S rRNA gene sequence and phenotypic characteristics. Gas chromatography (GC) and GC-mass spectroscopy analyses showed that strain F.5.25.8 transformed DBT into 2-hydroxybiphenyl (2-HBP). This strain was also able to grow using various organic sulfur or nitrogen compounds as the sole sulfur or nitrogen sources. Resting-cell studies indicated that desulfurization occurs either in cell-associated or in cell-free extracts of F.5.25.8. The biological responses of F.5.25.8 to a series of mutagens and environmental agents were also characterized. The results revealed that this strain is highly tolerant to DNA damage and also refractory to induced mutagenesis. Strain F.5.25.8 was also characterized genetically. Results showed that genes involved in desulfurization (dsz) are located in the chromosome, and PCR amplification was observed with primers dszA and dszB designed based on Rhodococcus genes. However, no amplification product was observed with the primer based on dszC.

Os germinomas intracranianos são histologicamente indistinguíveis do neurinoma testicular e disgerminoma ovariano; eles ocorrem mais frequentemente na região pineal, supra-selar e gânglios da base. O envolvimento primário do germinoma na haste hipofisária é raro. Neste artigo, relatamos um germinoma da haste hipofisária e revisamos a literatura sobre o assunto.
