BACKGROUND: There is limited data available regarding the prevalence of intracranial arterial stenosis (ICAS) among acute ischemic stroke (AIS) patients in Brazil and Latin America. OBJECTIVE: The present study sought to investigate the frequency and predictors of ICAS among patients with AIS or transient ischemic attack (TIA) in a Brazilian center, with transcranial color-coded duplex sonography (TCCS) technique. METHODS: Consecutive AIS and TIA patients, admitted to an academic public comprehensive stroke center in Brazil from February to December 2014, evaluated by TCCS were prospectively selected. Vascular narrowings > 50% were considered as ICAS, based on ultrasound criteria previously defined in the literature. RESULTS: We assessed 170 consecutive patients with AIS or TIA, of whom 27 (15.9%) were excluded due to an inadequate transtemporal acoustic bone window. We confirmed ICAS in 55 patients (38.5%). The most common location was the proximal segment of the middle cerebral artery (28.2%), followed by the vertebral (15.4%), posterior cerebral (13.6%), terminal internal carotid (9.1%) and basilar (8.2%) arteries. On multivariate models adjusting for potential confounders, systolic blood pressure (OR: 1.03, 95%CI: 1.01-1.04; p = 0.008) was independently associated with ICAS. CONCLUSION: We found significant ICAS in approximately ⅓ of patients admitted with symptoms of AIS or TIA in a public tertiary academic stroke center in Brazil. The TCCS is an accessible and noninvasive technique that can be used to investigate the presence of moderate and severe ICAS, especially in patients who cannot be exposed to more invasive exams, such as the use of intravenous contrast agents.


ANTECEDENTES: Dados acerca da prevalência da estenose arterial intracraniana (EAIC) entre os pacientes com acidente vascular isquêmico (AVCi) agudo no Brasil e América Latina são limitados. OBJETIVO: O presente estudo pretendeu investigar a frequência e os preditores da EAIC nos pacientes AVCi ou ataque isquêmico transitório (AIT) em um centro brasileiro utilizando o Doppler transcraniano colorido (duplex transcraniano). MéTODOS: Pacientes consecutivos com AVCi ou AIT, admitidos entre fevereiro e dezembro de 2014 em um centro acadêmico brasileiro especializado em doenças cerebrovasculares, foram avaliados prospectivamente com duplex transcraniano. Os estreitamentos vasculares > 50% foram considerados como EAIC, baseado em critérios ultrassonográficos definidos previamente na literatura. RESULTADOS: Foram avaliados 170 pacientes com AVCi ou AIT, dos quais 27 (15,9%) foram excluídos em decorrência da janela óssea transtemporal acústica inadequada. Confirmamos EAIC em 55 pacientes (38,5%). A localização mais comum foi o segmento proximal da artéria cerebral média (28,2%), seguida pelas artérias vertebral (15,4%), cerebral posterior (13,6%), carótida interna terminal (9,1%) e basilar (8,2%). No modelo multivariado, ajustado para os potenciais confundidores, a pressão arterial sistólica aumentada (OR: 1,03; IC 95%: 1,01­1,04; p = 0,008) foi independentemente associada a EAIC. CONCLUSãO: Foi identificada EAIC significativa em quase ⅓ dos pacientes admitidos com sintomas de AVCi ou AIT em um serviço acadêmico público de atendimento especializado em doenças cerebrovasculares. O Doppler transcraniano colorido é uma ferramenta acessível e não invasiva que pode ser utilizada com segurança para a investigação da presença de EAIC moderada ou grave, especialmente nos pacientes que não podem ser expostos a exames complementares mais invasivos com uso de contraste intravenoso.

BACKGROUND: The delay in recognizing stroke symptoms is a significant obstacle to receiving acute treatment. Therefore, it is essential to understand the gaps in the knowledge about stroke among the general population and promote campaigns based on these gaps. OBJECTIVE: To investigate the general knowledge about stroke in a capital in Northeastern Brazil in a sample of individuals who attended a public hospital and the impacts of the coronavirus disease 2019 (COVID-19) pandemic on the use of emergency services. METHODS: We included patients older than 18 years of age and their family members and/or companions. After obtaining consent, the researcher presented a typical case of stroke, and the participants filled out a questionnaire divided into 2 sections: sociodemographic data and 15 questions about stroke detection and seeking health services and treatment. RESULTS: We included 154 individuals with a mean age of 44.45 ± 16.21 years. After presenting the case, 60.4% mentioned the acronym AVC (acidente vascular cerebral, or cerebrovascular accident [stroke], in Portuguese) as a possible explanation, and 54.5% reported that they would call the Mobile Emergency Care Service. However, 62.9% provided the incorrect telephone number for the Mobile Emergency Care Service or lacked knowledge of the accurate number. Regarding the risk factors for stroke, 27.9% did not know any of them, 65.5% were unaware of any treatment, and no reference was made to thrombolytic therapy. About their chosen conduct in the same case in the context of the COVID-19 pandemic, 98.1% of the participants would not change their behavior. CONCLUSION: These results can assist in the planning of public policies and campaigns emphasizing the issue of risk factors and how to access emergency medical services in the state of Alagoas, Brazil.


ANTECEDENTES: O atraso no reconhecimento dos sinais do acidente vascular cerebral (AVC) é um dos principais obstáculos para receber o tratamento de fase aguda. Portanto, é importante compreender as lacunas no conhecimento da população sobre o AVC e promover campanhas de acordo com essas lacunas. OBJETIVO: Investigar o conhecimento geral sobre o AVC em uma amostra de indivíduos acompanhados em um hospital público de uma capital do Nordeste brasileiro e o impacto da pandemia de doença do coronavírus 2019 (coronavirus disease 2019 - COVID-19, em inglês) na utilização dos serviços de emergência. MéTODOS: Foram incluídos pacientes acima de 18 anos e seus acompanhantes e/ou familiares. Após a assinatura do termo de consentimento, o pesquisador apresentou um caso típico de AVC, e os participantes responderam um questionário dividido em 2 partes: dados sociodemográficos e 15 perguntas sobre o reconhecimento do AVC, procura por serviços de saúde e tratamento. RESULTADOS: Foram incluídos 154 participantes com idade média de 44,45 ± 16,21 anos. Após a apresentação do caso, 60,4% mencionaram AVC como uma possível explicação, e 54,5% chamariam o Serviço de Atendimento Móvel de Urgência (SAMU). Entretanto, 62,9% erraram ou não sabiam o número do SAMU. Quanto aos fatores de risco de desenvolver AVC, 27,9% não sabiam identificar nenhum, 65,5% não conheciam qualquer tratamento, e não houve menção ao tratamento trombolítico. Com relação à conduta no mesmo caso, mas no contexto da pandemia de COVID-19, 98,1% dos participantes não mudariam sua conduta. CONCLUSãO: Os resultados deste estudo podem auxiliar no planejamento de políticas públicas com ênfase em campanhas sobre os fatores de risco e o acionamento do SAMU em Alagoas, Brasil.

BACKGROUND: Autoimmune encephalitis (AIE) is a group of inflammatory diseases characterized by the presence of antibodies against neuronal and glial antigens, leading to subacute psychiatric symptoms, memory complaints, and movement disorders. The patients are predominantly young, and delays in treatment are associated with worse prognosis. OBJECTIVE: With the support of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, ABN) and the Brazilian Society of Child Neurology (Sociedade Brasileira de Neurologia Infantil, SBNI), a consensus on the diagnosis and treatment of AIE in Brazil was developed using the Delphi method. METHODS: A total of 25 panelists, including adult and child neurologists, participated in the study. RESULTS: The panelists agreed that patients fulfilling criteria for possible AIE should be screened for antineuronal antibodies in the serum and cerebrospinal fluid (CSF) using the tissue-based assay (TBA) and cell-based assay (CBA) techniques. Children should also be screened for anti-myelin oligodendrocyte glucoprotein antibodies (anti-MOG). Treatment should be started within the first 4 weeks of symptoms. The first-line option is methylprednisolone plus intravenous immunoglobulin (IVIG) or plasmapheresis, the second-line includes rituximab and/or cyclophosphamide, while third-line treatment options are bortezomib and tocilizumab. Most seizures in AIE are symptomatic, and antiseizure medications may be weaned after the acute stage. In anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis, the panelists have agreed that oral immunosuppressant agents should not be used. Patients should be evaluated at the acute and postacute stages using functional and cognitive scales, such as the Mini-Mental State Examination (MMSE), the Montreal Cognitive Assessment (MoCA), the Modified Rankin Scale (mRS), and the Clinical Assessment Scale in Autoimmune Encephalitis (CASE). CONCLUSION: The present study provides tangible evidence for the effective management of AIE patients within the Brazilian healthcare system.


ANTECEDENTES: Encefalites autoimunes (EAIs) são um grupo de doenças inflamatórias caracterizadas pela presença de anticorpos contra antígenos neuronais e gliais, que ocasionam sintomas psiquiátricos subagudos, queixas de memória e distúrbios anormais do movimento. A maioria dos pacientes é jovem, e o atraso no tratamento está associado a pior prognóstico. OBJETIVO: Com o apoio da Academia Brasileira de Neurologia (ABN) e da Sociedade Brasileira de Neurologia Infantil (SBNI), desenvolvemos um consenso sobre o diagnóstico e o tratamento da EAIs no Brasil utilizando a metodologia Delphi. MéTODOS: Um total de 25 especialistas, incluindo neurologistas e neurologistas infantis, foram convidados a participar. RESULTADOS: Os especialistas concordaram que os pacientes com critérios de possíveis EAIs devem ser submetidos ao rastreio de anticorpos antineuronais no soro e no líquido cefalorraquidiano (LCR) por meio das técnicas de ensaio baseado em tecidos (tissue-based assay, TBA, em inglês) e ensaio baseado em células (cell-based assay, CBA, em inglês). As crianças também devem ser submetidas ao rastreio de de anticorpo contra a glicoproteína da mielina de oligodendrócitos (anti-myelin oligodendrocyte glycoprotein, anti-MOG, em inglês). O tratamento deve ser iniciado dentro das primeiras 4 semanas dos sintomas, sendo as opções de primeira linha metilprednisolona combinada com imunoglobulina intravenosa (IGIV) ou plasmaférese. O tratamento de segunda linha inclui rituximabe e ciclofosfamida. Bortezomib e tocilizumab são opções de tratamento de terceira linha. A maioria das crises epilépticas nas EAIs são sintomáticas, e os fármacos anticrise podem ser desmamadas após a fase aguda. Em relação à encefalite antirreceptor de N-metil-D-aspartato (anti-N-methyl-D-aspartate receptor, anti-NMDAR, em inglês), os especialistas concordaram que agentes imunossupressores orais não devem ser usados. Os pacientes devem ser avaliados na fase aguda e pós-aguda mediante escalas funcionais e cognitivas, como Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Modified Rankin Scale (mRS), e Clinical Assessment Scale in Autoimmune Encephalitis (CASE). CONCLUSãO: Esta pesquisa oferece evidências tangíveis do manejo efetivo de pacientes com EAIs no sistema de saúde Brasileiro.

Edaphic factors can modulate the effects of microbial inoculants on crop yield promotion. Given the potential complexity of microbial inoculant responses to diverse soil management practices, we hypothesize that sustainable management of soil and water irrigation may improve soil quality and enhance the effects of plant growth-promoting bacteria (PGPB). Consequently, the primary objective was to assess the effectiveness of microbial inoculants formulated with Herbaspirillum seropedicae (Hs) and Azospirillum brasilense (Ab) on maize growth in soils impacted by different historical conservation management systems. We evaluated two soil management systems, two irrigation conditions, and four treatments: T0 - without bioinoculant and 100% doses of NPK fertilization; T1 - Hs + humic substances and 40% of NPK fertilization; T2 - Ab and 40% of NPK fertilization; T3 - co-inoculation (Hs + Ab) and 40% of NPK fertilization. Using a reduced fertilization dose (40% NPK) associated with microbial inoculants proved efficient in increasing maize shoot dry mass : on average, there was a 16% reduction compared to the treatment with 100% fertilization. In co-inoculation (Hs + Ab), the microbial inoculants showed a mutualistic effect on plant response, higher than isolate ones, especially increasing the nitrogen content in no-tillage systems irrigated by swine wastewater. Under lower nutrient availability and higher biological soil quality, the microbial bioinputs positively influenced root development, instantaneous water use efficiency, stomatal conductance, and nitrogen contents.

This article presents the rare case of a patient with gemination and fusion involving conical supernumerary teeth and the second mandibular molar, along with its treatment. The 13-year-old girl, without any associated syndromes and currently undergoing orthodontic treatment, was referred for evaluation due to the presence of a supernumerary tooth. Following a clinical examination and analysis of a panoramic radiograph, a cone beam computed tomography was requested for a more detailed assessment of the supernumerary tooth and its relationship with adjacent structures. The sagittal sections revealed the presence of two conical paramolar supernumerary teeth, which were geminated and fused to the mesial root of tooth 47. The suggested treatment was the surgical removal of the supernumerary teeth, which was successfully performed without complications and with good healing. After one-year post-procedure, the formation of periodontal ligament was verified at the site where the supernumerary teeth were extracted. The simultaneous occurrence of gemination of two supernumerary teeth and their fusion to a mandibular molar is extremely rare, and their surgical removal provided the patient with better plaque control in the region and contributed to the progress of orthodontic treatment. Strategies seeking to increase the use efficiency of nitrogen (N) fertilizers and that benefit plant growth through multiple mechanisms can reduce production costs and contribute to more sustainable agriculture free of polluting residues. Under controlled conditions, we investigated the compatibility between foliar inoculation with an endophytic diazotrophic bacterium (Herbaspirillum seropedicae HRC54) at control and low, medium and high N fertilization levels (0, 25, 50 and 100 mg of N kg-1 as urea, respectively) in Marandu palisadegrass. Common procedures in our research field (biometric and nutritional assessments) were combined with isotopic techniques (natural abundance - δ15N‰ and 15N isotope dilution) and root scanning to determine the contribution of fixed N and recovery of N fertilizer by the grass. Overall, the combined use of 15N isotopic techniques revealed that inoculation not only improved the recovery of applied N-urea from the soil but also provided fixed nitrogen to Marandu palisade grass, resulting in an increase in the total accumulated N. When inoculated plants grew at control and low levels of N, a positive cascade effect encompassing root growth stimulation (nodes of smaller diameter roots), better soil and fertilizer resource exploitation and increased forage production was observed. In contrast, increasing N reduced the contributions of N fixed by H. seropedicae from 21.5% at the control level to 8.6% at the high N level. Given the minimal to no observed growth promotion, this condition was deemed inhibitory to the positive effects of H. seropedicae. We discuss how to make better use of H. seropedicae inoculation in Marandu palisadegrass, albeit on a small scale, thus contributing to a more rational and efficient use of N fertilizers. Finally, we pose questions for future investigations based on 15N isotopic techniques under field conditions, which have great applicability potential.

Over the last three decades, stroke care has undergone significant transformations mainly driven by the introduction of reperfusion therapy and the organization of systems of care. Patients receiving treatment through a well-structured stroke service have a much higher chance of favorable outcomes, thereby decreasing both disability and mortality. In this article, we reviewed the scientific evidence for stroke reperfusion therapy, including thrombolysis and thrombectomy, and its implementation in the public health system in Brazil.


Nas últimas três décadas, o tratamento do AVC sofreu transformações significativas, impulsionadas principalmente pela introdução das terapias de reperfusão e pela organização dos serviços de AVC. Os pacientes que recebem tratamento em um serviço de AVC bem estruturado têm uma probabilidade muito maior de resultados favoráveis, diminuindo assim a incapacidade funcional e a mortalidade. Neste artigo, revisamos as evidências científicas para as terapias de reperfusão do AVC, incluindo trombólise e trombectomia e sua implementação no sistema público de saúde no Brasil.

Background: The frequency of antibodies in autoimmune encephalitis (AIE) may vary in different populations, however, data from developing countries are lacking. To describe the clinical profile of AIE in Brazil, and to evaluate seasonality and predictors of AIE in adult and pediatric patients. Methods: We evaluated patients with possible AIE from 17 centers of the Brazilian Autoimmune Encephalitis Network (BrAIN) between 2018 and 2022. CSF and serum were tested with TBAs and CBAs. Data on clinical presentation, complementary investigation, and treatment were compiled. Seasonality and predictors of AIE in adult and pediatric populations were analyzed. Results: Of the 564 patients, 145 (25.7%) were confirmed as seropositive, 69 (12.23%) were seronegative according to Graus, and 58% received immunotherapy. The median delay to diagnosis confirmation was 5.97 ± 10.3 months. No seasonality variation was observed after 55 months of enrolment. The following antibodies were found: anti-NMDAR (n=79, 54%), anti-MOG (n=14, 9%), anti-LGI1(n=12, 8%), anti-GAD (n=11, 7%), anti-GlyR (n=7, 4%), anti-Caspr2 (n=6, 4%), anti-AMPAR (n=4, 2%), anti-GABA-BR (n=4, 2%), anti-GABA-AR (n=2, 1%), anti-IgLON5 (n=1, 1%), and others (n=5, 3%). Predictors of seropositive AIE in the pediatric population (n=42) were decreased level of consciousness (p=0.04), and chorea (p=0.002). Among adults (n=103), predictors of seropositive AIE were movement disorders (p=0.0001), seizures (p=0.0001), autonomic instability (p=0.026), and memory impairment (p=0.001). Conclusion: Most common antibodies in Brazilian patients are anti-NMDAR, followed by anti-MOG and anti-LGI1. Only 26% of the possible AIE patients harbor antibodies, and 12% were seronegative AIE. Patients had a 6-month delay in diagnosis and no seasonality was found. Findings highlight the barriers to treating AIE in developing countries and indicate an opportunity for cost-effect analysis. In this scenario, some clinical manifestations help predict seropositive AIE such as decreased level of consciousness, chorea, and dystonia among children, and movement disorders and memory impairment among adults.

As agricultural practices become more sustainable, adopting more sustainable practices will become even more relevant. Searching for alternatives to chemical compounds has been the focus of numerous studies, and bacteriocins are tools with intrinsic biotechnological potential for controlling plant diseases. We continued to explore the biotechnological activity of the bacteriocin Gluconacin from Gluconacetobacter diazotrophicus, PAL5 strain, by investigating this protein's antagonism against important tomato phytopathogens and demonstrating its effectiveness in reducing bacterial spots caused by Xanthomonas perforans. In addition to this pathogen, the bacteriocin Gluconacin demonstrated bactericidal activity in vitro against Ralstonia solanacearum and Pseudomonas syringae pv. tomato, agents that cause bacterial wilt and bacterial spots, respectively. Bacterial spot control tests showed that Gluconacin reduced disease severity by more than 66%, highlighting the biotechnological value of this peptide in ecologically correct formulations.

Introduction: In the scenario of metal toxicity, aluminum (Al) stands out as a ubiquitous type of metal that can be combined with other elements and form different compounds. Al is widely used daily as an adjuvant in vaccines, antacids, food additives (as components of AI-containing food additives), skin care products, cosmetics, and kitchenware, and can be an element or contaminant present in our daily life. Objective: To present a review of the main deleterious effects of Al on human health. Methods: The search was carried out from September 2022 to February 2023 in the Scopus, PubMed, Science Direct, Scielo, and Google Scholar databases, using scientific articles from 2012 to 2023. The quality of the studies was based on the GRADE instrument, and the risk of bias was analyzed according to the Cochrane instrument. Results and Conclusions: A total of 115 files were search returned. Further, 95 articles were evaluated, and 44 were included in this review. Based on the results, measuring Al's relevance to health is essential in medicine. Several studies have demonstrated clinical outcomes and metabolic alterations with Al exposure. The tolerable weekly intake established by the European Food Safety Authority (EFSA) of 1 mg Al/kg body weight can be achieved through dietary exposure alone. Proven neurotoxicity in humans is the critical adverse effect of Al. A carcinogenic effect of Al has not been proven so far. Preventive medicine advocates that exposure to Al should be kept as low as possible. Chelating agents, such as calcium disodium ethylene diamine tetraacetic acid and deferoxamine, are options for acute poisoning, and monomethysilanetriol supplementation may be a long-term strategy with chelation potential. Further studies are needed to assess the impacts of Al on human health.

Rothmund-Thomson syndrome (RTS) is a rare, heterogeneous autosomal recessive genodermatosis, with poikiloderma as its hallmark. It is classified into two types: type I, with biallelic variants in ANAPC1 and juvenile cataracts, and type II, with biallelic variants in RECQL4, increased cancer risk and no cataracts. We report on six Brazilian probands and two siblings of Swiss/Portuguese ancestry presenting with severe short stature, widespread poikiloderma and congenital ocular anomalies. Genomic and functional analysis revealed compound heterozygosis for a deep intronic splicing variant in trans with loss of function variants in DNA2, with reduction of the protein levels and impaired DNA double-strand break repair. The intronic variant is shared by all patients, as well as the Portuguese father of the European siblings, indicating a probable founder effect. Biallelic variants in DNA2 were previously associated with microcephalic osteodysplastic primordial dwarfism. Although the individuals reported here present a similar growth pattern, the presence of poikiloderma and ocular anomalies is unique. Thus, we have broadened the phenotypical spectrum of DNA2 mutations, incorporating clinical characteristics of RTS. Although a clear genotype-phenotype correlation cannot be definitively established at this moment, we speculate that the residual activity of the splicing variant allele could be responsible for the distinct manifestations of DNA2-related syndromes.

Zika virus (ZIKV), a mosquito-borne pathogen, is an emerging arbovirus associated with sporadic symptomatic cases of great medical concern, particularly among pregnant women and newborns affected with neurological disorders. Serological diagnosis of ZIKV infection is still an unmet challenge due to the co-circulation of the dengue virus, which shares extensive sequence conservation of structural proteins leading to the generation of cross-reactive antibodies. In this study, we aimed to obtain tools for the development of improved serological tests for the detection of ZIKV infection. Polyclonal sera (pAb) and a monoclonal antibody (mAb 2F2) against a recombinant form of the ZIKV nonstructural protein 1 (NS1) allowed the identification of linear peptide epitopes of the NS1 protein. Based on these findings, six chemically synthesized peptides were tested both in dot blot and ELISA assays using convalescent sera collected from ZIKV-infected patients. Two of these peptides specifically detected the presence of ZIKV antibodies and proved to be candidates for the detection of ZIKV-infected subjects. The availability of these tools opens perspectives for the development of NS1-based serological tests with enhanced sensitivity regarding other flaviviruses.

OBJECTIVE: Primary caregivers (e.g., parents, grandparents, other family members) from low-income and ethnically minoritized families tend to face a host of barriers when participating in their children's school activities. Research suggests that demographic match and quality communication between caregivers and teachers could support minoritized families' school-based engagement. This study examined the associations among caregiver-teacher demographic match, caregivers' perceived communication quality with the teacher and caregivers' perceived barriers to school-based engagement. METHOD: Caregivers (n = 565) from 49 Head Start classrooms completed the parent-report versions of the surveys: Barriers to Family Engagement, reporting resource, cultural/relational, and program/context barriers to school-based engagement; and Family-Teacher Communication, reporting communication quality with their children's lead and assistant teachers (n = 102). Caregivers and teachers also completed demographic surveys to provide information about their family background, such as race/ethnicity, primary language, and education level. RESULTS: Multilevel modeling results showed that among the three demographic match variables (i.e., race/ethnicity, primary language, formal education), only language match was associated with caregivers' perception of fewer cultural/relational barriers. Latine and Black non-Latine caregivers reported more cultural/relational and program/context carriers than White, non-Latine caregivers. Finally, caregivers who perceived better communication with their children's teachers reported fewer cultural/relational and program/context barriers. CONCLUSIONS: Primary language match and high-quality communication between families and teachers appear essential in creating a welcoming preschool environment that could alleviate some of the barriers to engagement typically faced by ethnically minoritized and low-income families. Objetivo: compreender a visão do profissional de enfermagem sobre o cuidado compartilhado entre equipe de enfermagem e o familiar da criança com condição crônica. Método: estudo qualitativo, descritivo, exploratório realizado numa enfermaria de pediatria de um Hospital Estadual Universitário do Rio de Janeiro, com 23 profissionais de enfermagem, sendo 5 enfermeiras e 18 técnicas de enfermagem. Os dados foram analisados com base na técnica de análise de conteúdo temática segundo Bardin. Resultados: apreenderam-se três categorias: Desafios na construção do cuidado compartilhado; Maneiras de promover o cuidado compartilhado e Benefícios do cuidado compartilhado. Evidenciam aspectos positivos e que precisam ser melhorados para que o cuidado compartilhado seja efetivo e maneiras de realizá-lo. Conclusão: os resultados deste estudo evidenciaram aspectos positivos e desafios na relação profissional-familiar para efetivo cuidado compartilhado, estes poderão contribuir para assistência da enfermagem de qualidade, evidenciando o aspecto educativo do cuidado, favorecendo a interação entre profissional e familiares.


Objective: to understand the Nursing profession's view of care shared between nursing teams and relatives/caregivers of children with a chronic condition. Method: this qualitative, descriptive, exploratory study involved 23 nursing personnel (5 nurses and 18 nursing technicians) in a pediatric unit of a state university hospital. Data were analyzed using Bardin thematic content analysis. Results: three categories were identified: Challenges in constructing shared care; Ways to promote shared care; and Benefits of shared care. They highlight both positive aspects to be improved so that shared care is effective and ways to achieve that. Conclusion: this study found positive aspects and challenges in the relationship between families and health personnel with a view to effective shared care. These can contribute to quality nursing care, highlight the educational aspect of care and favor interaction between health personnel and families.


Objetivo: comprender la perspectiva del profesional de enfermería acerca del cuidado compartido entre el equipo de enfermería y el familiar del niño con enfermedad crónica. Método: estudio cualitativo, descriptivo, exploratorio, realizado en una sala de enfermería pediátrica de un Hospital Universitario del Estado en Río de Janeiro, junto a 23 profesionales de enfermería: 5 enfermeros y 18 técnicos de enfermería. Conclusión: los resultados de este estudio mostraron aspectos positivos y desafíos en la relación profesional-familia para un cuidado compartido eficaz, que pueden contribuir a una atención de enfermería de calidad, destacando el aspecto educativo del cuidado, favoreciendo la interacción entre el profesional y los familiares.

Pacientes com câncer de cabeça e pescoço (CCP) apresentam limitações e comprometimentos funcionais. A dor orofacial acomete grande parte desses pacientes e pode ser causada por inúmeros fatores, tanto nos tecidos moles quanto nos duros. Pacientes com CCP em estágio avançado, necessitam receber assistência que possibilite melhor qualidade de vida para ele e seus familiares, como forma de cuidado paliativo, principalmente quando a doença não tem mais chance de ser controlada. A analgesia farmacológica é o principal pilar no tratamento da dor oncológica, mas também pode ser realizada por meio de terapias não farmacológicas. Este estudo teve como objetivo revisar a literatura buscando as diferentes formas de controle da dor orofacial de pacientes com CCP em cuidados paliativos. Foram realizadas buscas nas bases de dados PubMed e SciELO com as palavras "(pain control) AND (palliative care) AND (head and neck cancer)", buscando artigos dos dez últimos anos (2011-2021) e restringindo para ensaios clínicos e ensaios clínicos randomizados. Encontramos dez artigos na base PubMed e nenhum na SciELO. Após a leitura do título e resumo, excluímos cinco por não avaliarem pacientes com CCP ou não terem a abordagem analgésica como objetivo do estudo, sendo incluídosfinalmente 5 artigos em nossa revisão. A maioria dos estudos mostrou que a analgesia realizada nos pacientes em cuidados paliativos acometidos por lesões malignas de cabeça e pescoço acontece com opioides. Nesta revisão observamos poucos estudos clínicos, sendo importante a realização de trabalhos que busquem novas formas de diminuir os sintomas e melhorar a qualidade de vida desses pacientes.

BACKGROUND: COVID-19 has disproportionately impacted low-income immigrant communities. There is concern that the current uptake of COVID-19 vaccines is suboptimal and that this may be contributing to COVID-19 inequities. However, little is known about the acceptability of COVID-19 vaccines among immigrants in the U.S. Our goal was to gauge COVID-19 vaccine intentions among Brazilian immigrant women living in the U.S. METHODS: We conducted an online survey between July and August 2020 offered in Portuguese and English languages among a convenience sample of Brazilian immigrant women ages 18 years and older. Women were recruited through online advertisements by community-based organizations and social media groups to complete a survey that assessed intention to get a COVID-19 vaccine, attitudes toward vaccines, and perceptions about the pandemic. RESULTS: Of the total sample (N = 353), most (70.8%) indicated they intended to get a COVID-19 vaccine. In bivariate analyses, vaccine intentions were significantly associated with perceptions about the severity of the pandemic, trusted sources of health information, and the number of years lived in the U.S. Multinomial logistic regression models revealed that those who did not intend to be vaccinated had lived a longer time in the U.S. (OR: -0.12 95% CI: -0.19, -0.05), perceived the pandemic to be a minor issue (OR: 1.52, 95% CI: 0.62, 2.42), and trusted information from social networks (OR: -1.94, 95% CI: -3.25, -0.63) or private news sources (OR: -1.71, 95% CI: -2.78, -0.63). CONCLUSIONS: While most women reported they would get a COVID-19 vaccine, efforts to reach those who may be hesitant should target those who have lived in the U.S. for longer periods of time and do not perceive the pandemic to be a major crisis. Healthcare providers may be particularly suited to deliver this information given high levels of trust.

This work aimed to evaluate the in vitro anthelmintic effect of carvone nanoemulsions on Haemonchus contortus. Three R-carvone nanoemulsions were prepared: uncoated R-carvone nanoemulsions homogenized in a sonicator (UNAlg-son) and homogenized in an ultrahomogenizer (UNAlg-ultra) and sodium alginate-coated R-carvone (CNAlg-ultra). The physicochemical characterizations of the nanoemulsions were carried out. The anthelmintic activity was evaluated using egg hatch test (EHT), larval development test (LDT) and adult worm motility test (AWMT). Changes in cuticle induced in adult H. contortus were evaluated by scanning electron microscopy (SEM). The results were subjected to analysis of variance and compared using the Tukey test (P < 0.05). The effective concentration to inhibit 50% (EC50) of egg hatching and larval development was calculated. The particle sizes were 281.1 nm (UNAlg-son), 152.7 nm (UNAlg-ultra) and 557.8 nm (CNAlg-ultra), and the zeta potentials were −15 mV (UNAlg-son), −10.8 mV (UNAlg-ultra) and −24.2 mV (CNAlg-ultra). The encapsulation efficiency was 99.84 ± 0.01%. SEM of the nanoemulsions showed an increase in size. In EHT, the EC50 values of UNAlg-son, UNAlg-ultra and CNAlg-ultra were 0.19, 0.02 and 0.17 mg mL−1, respectively. In LDT, they were 0.29, 0.31 and 0.95 mg mL−1 for UNAlg-son, UNAlg-ultra and CNAlg-ultra, respectively. The adult motility inhibition was 100% after 12 h of exposure to UNAlg-ultra and CNAlg-ultra, while for UNAlg-son, it was 79.16%. SEM showed changes in the buccal capsule and cuticular damage. It was concluded that R-carvone nanoemulsions showed antiparasitic action demonstrating promise for the control of infections caused by gastrointestinal nematodes in small ruminants.

BACKGROUND: There is little information available on stroke epidemiology in the northeast of Brazil. OBJECTIVE: Our objective was to investigate the prevalence of the stroke subtypes, prevalence of cerebrovascular risk factors and patterns of management in a public neurovascular outpatient referral service, in Alagoas. METHODS: Data were prospectively collected from consecutive patients with stroke who were treated in a specialized neurovascular clinic between November 2016 and June 2018. Recurrence was evaluated by telephone 12 months after patients had been included in the study. RESULTS: We evaluated 190 patients (mean age, 60.22 ( 13.29 years; 60.5% males). Ischemic stroke was the most frequent subtype (85.2%). Sedentary lifestyle was the most common risk factor (71.6%), followed by hypertension (62.6%) and stroke family history (41.1%). Only 21.5% of the patients were transported by ambulance to the hospital, and 42.6% received medical support in hospital units or emergency units with no imaging support. The median NIHSS was 2.5 (IQR, 1-5) and mRS was 2 (IQR, 1-3). We found a high rate of undetermined stroke (35.8%), and few patients completed the etiological investigation. One year after inclusion in the study, 12 patients (6.3%) had died and 14 (7.3%) had had another stroke. CONCLUSIONS: The prevalence of cerebrovascular risk factors and clinical presentation were similar to what had been seen in previous series. A notable number of patients received medical support in institutions with no imaging equipment. The high number of cases of undetermined stroke etiology shows the difficulty in accessing healthcare services in Alagoas.
