RESUMO

Objetivo: Comparar dos pares de fricción (metal/polietileno de ultra alto peso molecular [UHMWPE], metal/polietileno de alto entrecruzamiento) mediante análisis cuantitativo y cualitativo de partículas de polietileno en líquido sinovial a los 3 años postintervención en pacientes portadores de prótesis total de rodilla (PTR). Material y métodos: Se llevó a cabo un estudio de cohortes prospectivo, aleatorizado, con evaluación ciega incluyendo 25 pacientes a quienes se intervino de PTR de manera bilateral, con 6 meses de diferencia. A los 3 años postintervención, se realizaron artrocentesis de rodilla a 12 pacientes y se analizaron las partículas de polietileno. Resultados: No se hallaron diferencias significativas en el número de partículas generadas por los diferentes insertos de polietileno a los 3 años tras la implantación de una artroplastia total de rodilla (3.000×: x¯ entrecruzado=849,7; x¯ UHMWPE=796,9; p=0,63; 20.000×: x¯ entrecruzado=66,3; x¯ UHMWPE=73,1; p=0,76). Tampoco existen diferencias significativas entre los 2 tipos de inserto, entre la posibilidad de encontrar partículas de forma elongada (χ2=0,19; p=0,66) ni redonda (χ2=1,44; p=0,23). Sin embargo, la probabilidad de encontrar partículas de forma fibrilar es 3,08 veces mayor en el UHMWPE. Conclusiones: El polietileno altamente entrecruzado no reduce significativamente la generación de partículas de polietileno en aquellos pacientes intervenidos mediante una artroplastia total de rodilla, con muestras a 3 años postintervención

BACKGROUND: The therapeutic approach to cancer is complex and multidisciplinary. Radiotherapy is among the essential treatments, whether used alone or in conjunction with other therapies. This study reports a clinical audit of the radiotherapy process to assess the process of care, evaluate adherence to agreed protocols and measure the variability to improve therapeutic quality for rectal cancer. METHODS: Multicentre retrospective cohort study in a representative sample of patients diagnosed with rectal cancer in the Institut Català d'Oncologia, a comprehensive cancer centre with three different settings. We developed a set of indicators to assess the key areas of the radiotherapy process. The clinical audit consisted of a review of a random sample of 40 clinical histories for each centre. RESULTS: The demographic profile, histology and staging of patients were similar between centres. The MRI reports did not include the distance from tumour to mesorectal fascia (rCRM) in 38.3% of the cases. 96.7% of patients received the planned dose, and 57.4% received it at the planned time. Surgery followed neoadjuvant treatment in 96.7% of the patients. Among this group, postoperative CRM was recorded in 65.5% of the cases and was negative in 93.4% of these. With regard to the 34.5% (n = 40) of cases where no CRM value was stated, there were differences between the centres. Mean follow-up was 3.4 (SD 0.6) years, and overall survival at four years was 81.7%. CONCLUSIONS: The audit revealed a suboptimal degree of adherence to clinical practice guidelines. Significant variability between centres exists from a clinical perspective but especially with regard to organization and process.

The main purpose of this study was to determine the prevalence of Escherichia coli O157 on bovine carcasses before and after chilling at a large slaughterhouse located in the city of Barcelona, Spain, to assess the effectiveness of dry chilling on reducing E. coli O157 contamination of carcasses. In addition, the study characterized the E. coli O157 strains isolated in terms of virulence factors, antibiotic susceptibility, and their genetic diversity. Individual bovine carcasses were sampled before (n = 300) and after (n = 300) chilling over an 8-month period. Positive samples for E. coli O157 were subjected to virulence screening by PCR (stx1, stx2, and eaeA genes and the fliCH7 gene), antimicrobial susceptibility testing, and molecular typing by pulsed-field gel electrophoresis. A total of 9.7% (29 of 300) of the nonrefrigerated carcasses examined and 2.3% (7 of 300) of the refrigerated carcasses were positive for E. coli O157. All the isolates were serotype O157:H7, 92% (33 of 36) carried the stx1, stx2, and eaeA genes, and 8% (3 of 36) carried the stx2 and eaeA genes. Antimicrobial susceptibility testing showed a high degree of resistance: 29 strains (81%) were resistant to at least 1 antimicrobial of the 12 antimicrobials tested; 69% (25 of 36) were resistant to 4 or more antimicrobials. Molecular typing by pulsed-field gel electrophoresis found a high diversity of genetic types, implying little cross-contamination in the slaughterhouse. This study confirms that E. coli O157:H7 is present on the carcasses slaughtered in Spain, although its prevalence is reduced by the dry chilling process used. The recovered isolates showed potential pathogenesis and a high degree of multidrug resistance, confirming the importance of bovine meat monitoring.

INTRODUCTION: Stiffness after a total knee arthroplasty (TKA) is one of the most common post-operative complications. The purpose of this study is the evaluation of the effectiveness of TT proximalization osteotomy of improving a lack of flexion and secondary pain in patella baja (infera) post-TKA. MATERIALS AND METHODS: Between April 2007 and July 2012, TT proximalization osteotomy was performed on 21 patients. The average preoperative flexion was 70° (in a range of 60-80). Clinical pre- and post-operative evaluations were performed with Knee Society Score, Western Ontario and McMaster Universities Arthritis Index scales and a satisfaction survey. Modified Blackburn-Peel index and Portner angle were used to evaluate patellar height. RESULTS: After an average follow-up of 35 months (range 18-48), an average flexion of 100° (range 90-100) and an overall satisfaction were obtained. Clinical scores improved significantly. The Blackburn-Peel index and Portner angle improved significantly from 0.3 (range 0.1-0.5) to 0.4 (0.3-0.5) and from 9 (3-15) to 12 (9-18), respectively. Three patients showed no signs of osteotomy consolidation. However, this was not linked to a lack of extension or an increase in local pain. CONCLUSION: TT proximalization osteotomy provides satisfactory results in improving a lack of flexion and pain in patella baja post-TKR.

OBJECTIVE: To describe the clinical features and outcome of anti-NMDA receptor (NMDAR) encephalitis in patients ≥45 years old. METHOD: Observational cohort study. RESULTS: In a cohort of 661 patients with anti-NMDAR encephalitis, we identified 31 patients ≥45 years old. Compared with younger adults (18-44 years), older patients were more often male (45% vs. 12%, p < 0.0001), had lower frequency of tumors (23% vs. 51%, p = 0.002; rarely teratomas), had longer median time to diagnosis (8 vs 4 weeks, p = 0.009) and treatment (7 vs. 4 weeks, p = 0.039), and had less favorable outcome (modified Rankin Scale score 0-2 at 2 years, 60% vs. 80%, p < 0.026). In multivariable analysis, younger age (odds ratio [OR] 0.15, confidence interval [CI] 0.05-0.39, p = 0.0001), early treatment (OR 0.60, CI 0.47-0.78, p < 0.0001), no need for intensive care (OR 0.09, CI 0.04-0.22, p < 0.0001), and longer follow-up (p < 0.0001) were associated with good outcome. Rituximab and cyclophosphamide were effective when first-line immunotherapies failed (OR 2.93, CI 1.10-7.76, p = 0.031). Overall, 60% of patients older than 45 years had full or substantial recovery at 24 months follow-up. CONCLUSIONS: Anti-NMDAR encephalitis is less severe in patients ≥45 years old than in young adults, but the outcome is poorer in older patients. In this age group, delays in diagnosis and treatment are more frequent than in younger patients. The frequency of underlying tumors is low, but if present they are usually carcinomas instead of teratomas in younger patients. Early and aggressive immunotherapy will likely improve the clinical outcome.

RESUMO

Ultralight (<10 milligrams per cubic centimeter) cellular materials are desirable for thermal insulation; battery electrodes; catalyst supports; and acoustic, vibration, or shock energy damping. We present ultralight materials based on periodic hollow-tube microlattices. These materials are fabricated by starting with a template formed by self-propagating photopolymer waveguide prototyping, coating the template by electroless nickel plating, and subsequently etching away the template. The resulting metallic microlattices exhibit densities ρ ≥ 0.9 milligram per cubic centimeter, complete recovery after compression exceeding 50% strain, and energy absorption similar to elastomers. Young's modulus E scales with density as E ~ ρ(2), in contrast to the E ~ ρ(3) scaling observed for ultralight aerogels and carbon nanotube foams with stochastic architecture. We attribute these properties to structural hierarchy at the nanometer, micrometer, and millimeter scales.

The formation of uranyl-peroxide complexes was studied at alkaline media by using UV-Visible spectrophotometry and the STAR code. Two different complexes were found at a H(2)O(2)/U(VI) ratio lower than 2. A graphical method was used in order to obtain the formation constants of such complexes and the STAR program was used to refine the formation constants values because of its capacity to treat multiwavelength absorbance data and refining equilibrium constants. The values obtained for the two complexes identified were: UO(2)(2+) + H(2)O(2) + 4OH(-) <−> UO(2)(O(2))(OH)(2)(2-) + 2H(2)O: log ß°(1,1,4) = 28.1 ± 0.1 (1). UO(2)(2+) + 2H(2)O(2) + 6OH(-) <−> UO(2)(O(2))(2)(OH)(2)(4-) + 4H(2)O: log ß°(1,2,6) = 36.8 ± 0.2 (2). At hydrogen peroxide concentrations higher than 10(-5) mol dm(-3), and in the absence of carbonate, the UO(2)(O(2))(2)(OH)(2)(4-) complex is predominant in solution, indicating the significant peroxide affinity of peroxide ions for uranium and the strong complexes of uranium(VI) with peroxide.

OBJECTIVE: To assess enoxaparin as bridging anticoagulant treatment in cardiac surgery. METHODS: Prospective registry of those patients who underwent cardiac surgery in our centre between December 2003 and June 2004 and required long-term anticoagulation. Subcutaneous enoxaparin was used as bridging anticoagulant treatment according to a pre-established protocol. The global thromboembolic risk was carefully assessed in all patients. All patients were followed up for 3 months. RESULTS: Of 140 patients who were included (mean (SD) age 66 (11); 49% female), 51 were already receiving long-term acenocumarol treatment before the index intervention. 50% of the patients were at high or very high risk for thromboembolic events in the postoperative period. The mean (SD) number of days between surgery and the first dose of anticoagulant was 2.01 (7) for acenocumarol and 1 (1.01) for enoxaparin. The mean (SD) daily dose of enoxaparin was 1.1 (0.27) mg/kg. Six thromboembolic events (4.3%; 95% CI 1.6 to 9.1) occurred, but only four of them were plausibly related to enoxaparin (2.9%; 95% CI 0.8 to 7.1). Six major haemorrhagic events (4.3%; 95% CI 1.6 to 9.1) occurred, but only three were plausibly related to enoxaparin (2.1%; 95% CI 0.4 to 6.1). CONCLUSIONS: These findings show a reasonable rate of adverse events using enoxaparin as bridging anticoagulant treatment in cardiac surgery. Randomised studies are necessary to evaluate the real efficacy and safety of enoxaparin as bridging anticoagulant treatment in cardiac surgery.

Chlorpyrifos is the most widely used insecticide in the Chesapeake Bay region. Recent studies show that this organophospate chemical is consistently present in the air, rain and surface waters of the Chesapeake Bay region, suggesting a long environmental half-life. Hydrolytic degradation of chlorpyrifos is likely a dominant removal process, but existing hydrolysis data do not reflect conditions in the Chesapeake Bay. In this project, hydrolysis rates of chlorpyrifos were measured in sterilized, ambient water from the mouth of four Chesapeake Bay tributaries ranging in salinity from 0 to 17 ppt. The measured hydrolysis half-lives varied from 24 d in the Patuxent River to 126 d in the Susquehanna River. These results indicate that pH alone cannot be used as a single parameter to predict hydrolysis under field conditions. The influence of copper concentration, and other water constituents, need to be further evaluated as they may emerge as independent predictors to assess the fate of pesticides in natural systems.

The relative rate constants for the reaction of ozone were determined for several substituted anilines in aqueous solutions at pH 6.5 and 1.5. At pH 6.5, with the exception of m- and p-nitroaniline, the rate constants obey Hammett's equation: log(k(X)/k(H)) = rho sigma. The departure of m- and p-nitroaniline may be explained by direct conjugation of the reaction center. The commonly used sigma(p)(-) value of 1.27, which extends the range of applicability of the Hammett equation, was insufficient to account for the conjugation effects on ozonation of p-nitroaniline; rho = -1.48 (R = 0.973). Use of amine group atomic charge determinations significantly improved correlations: (k(X)/k(H)) = 48.7 delta - 18.2 (R = 0.996). A linear plot of Hammett constants versus relative rate data at pH 1.5 showed poor correlation: rho = 0.72 (R = 0.572). Poor correlation was similarly observed for amine group atomic charge determinations, suggesting varied reaction mechanisms.

The effect of surfactants on the biodegradation of trifluralin and atrazine (by Streptomyces PS1/5) and coumaphos (by degrading consortia from a contaminated cattle dip) in liquid cultures and soil slurries was tested at different concentrations of a rhamnolipid mixture (Rh-mix) and Triton X-100 (TX-100). The extent of trifluralin biodegradation in liquid culture was improved at high concentrations of both surfactants. The extent of atrazine degradation dropped in the presence of either surfactant. Coumaphos biodegradation improved slightly at Rh-mix dosages >3000 microM; however, it was readily inhibited by TX-100 at amounts above the critical micelle concentration. In soil slurries, the extent of both trifluralin and atrazine biodegradation was higher in Hagerstown A (HTA) soil than in Hagerstown B (HTB) soil and was not significantly affected by the presence of either surfactant. The onset of trifluralin biodegradation was retarded at higher concentrations of surfactants. In the absence of surfactant, up to 98% of coumaphos in both soil slurries was transformed. At increasing dosages of Rh-mix, the onset of coumaphos biodegradation was retarded, but the removal efficiency of the pesticide increased. Rh-mix and TX-100 depletion was observed during Streptomyces PS1/5 growth in liquid cultures. Rh-mix concentration also decreased during coumaphos biodegradation, whereas TX-100 concentration was not affected. These results suggest that surfactants, added for the purpose of increasing the apparent water solubility of hydrophobic organic compounds, may have unintended effects on both the rate and extent of biodegradation of the target compounds if the surfactants can also be degraded by the microorganisms in the system.

Adsorption of metals from aqueous solution onto oxide and other surfaces is known to affect trace metal transport in many natural and engineered systems. It is therefore important to understand whether dissolved metal inputs will be easily bound to particles or will be strongly complexed in solution and transported with the water phase. The effect of poly(acrylic acid) (PAA), representing a model compound for natural organic matter, on the adsorption of Cd(II) onto gamma-Al2O3 was determined using batch adsorption experiments over a pH range from 4 to 10. Initially, interactions among the individual components were evaluated. Cadmium adsorption onto alumina showed a typical S-shaped metal adsorption curve. PAA adsorption onto gamma-Al2O3 decreased with increase in pH. The affinity of PAA for Cd2+ increased strongly with pH. In ternary systems, the presence of PAA resulted in an enhancement of Cd(II) adsorption below pH 6, apparently due to ternary surface complex formation. Above pH 6, a decrease in cadmium adsorption onto gamma-Al2O3 was observed resulting from an increase in the concentration of soluble Cd-PAA complexes. Overall, results indicate that the presence of natural organic matter could have a significant impact on the distribution and mobility of cadmium in the environment. Simple surface complexation modeling was insufficient to describe behavior in the ternary systems due to the complexity of the PAA polymer.

The production of rhamnolipid biosurfactants by P. aeruginosa UG2 was examined under different culture conditions. Rhamnolipid yield was affected by the nature of the carbon sources, the nutrient concentrations, pH, and age of the culture. Hydrophobic substrates like corn oil, lard (rich in unsaturated and saturated fat), and long chain alcohols maximized biosurfactant production (100-165 mg/g substrate). Hydrophilic substrates like glucose, and succinic acid delivered poor yields (12-36 mg/g substrate). Rhamnolipid production was greater when N as (NH4)(2)SO4 and trace metals were added in several periodic doses rather than at the beginning of the process. Increased biosurfactant production was seen in cultures maintained at neutral pH relative to cultures allowed to develop acidic conditions (pH = 6.25). Although the level of rhamnolipid production was affected by culture conditions, the distribution of rhamnolipid subspecies did not vary between cultures. A dirhamnolipid species containing two 10 carbon alpha-hydroxy fatty acids [Rh2C10C10] was the most abundant in the mixtures (60.6 mol%), while the levels of the monorhamnolipid [RhC10C10] (20.7 mol%) and two dirhamnolipids [Rh2C10C12 and its dehydro variant Rh2C10C12-H2] (18.7 mol%) were similar. Biosurfactant mixtures produced with corn oil as sole carbon source solubilized the herbicide trifluralin [2,6-dinitro-N,N-dipropyl-4-(trifluoromethyl)benzamine] to a greater extent. This suggests that the presence of incompletely metabolized hydrophobic by-products acting as co-solvents can increase the solubilization capacity of biosurfactant mixtures.

Human paraoxonase (PON1) is a calcium-dependent esterase closely associated with high density lipoprotein (HDL)-containing apolipoprotein AI (apoAI), which has been shown to confer antioxidant properties to HDL. PON1 has been recently implicated in the pathogenesis of atherosclerosis. Low PON1 activities have been found in familial hypercholesterolemia (FH) and diabetes mellitus. We have undertaken a study of the effect of the lipid-lowering drug simvastatin on serum PON1 activity (in relation to paraoxon and arylesterase activity), on apoAI-containing and apolipoprotein B (apoB)-containing lipoproteins, and on lipid peroxide concentrations in 64 (39 women and 25 men) unrelated FH patients. We have also analyzed the influence of the PON1-192 and PON1-55 genetic polymorphisms on the response of PON1 activity to simvastatin therapy. A venous blood sample for a baseline analysis and another after 4 months of simvastatin therapy at a dosage of 20 mg per day were taken. The major effect of simvastatin on lipid traits was to decrease serum cholesterol, low density lipoprotein (LDL) cholesterol, and lipid peroxide concentrations by 19.9%, 26.3%, and 37.3%, respectively. There was also a significant decrease in serum apoB, LDL apoB, and triglyceride concentrations (20.5%, 21.1%, and 15.6%, respectively). Conversely, simvastatin had no significant influence on very low density lipoprotein-lipid content, HDL cholesterol, apoAI concentrations, and lipoprotein AI and AI:AII particles. Remarkably, serum PON1 activity toward paraoxon significantly increased during treatment with simvastatin (168. 7+/-100.3 U/L before therapy versus 189.5+/-116.5 U/L after therapy, P:=0.005). Arylesterase activity displayed only a nonsignificant trend to increase after therapy. Whereas PON1 activity levels were significantly lower in FH patients before simvastatin therapy compared with those of 124 normolipidemic subjects (168.7+/-100.3 versus 207.6+/-125.2 U/L, respectively; P:<0.05), this difference disappeared after simvastatin therapy. After simvastatin therapy, a significantly negative correlation between PON1 activity and lipid peroxide concentration was observed (r=-0.35, P:=0.028). The latter also strongly correlated with LDL cholesterol concentration (r=0.64, P:<0.001). Serum PON1 activity levels were significantly lower in the low-activity PON1-192 QQ and PON1-55 M carriers than in R carriers and in LL carriers, respectively. No significant differences were found in the therapeutic response of PON1 activity between genotype groups (8.5% and 11.1% increase for QQ homozygous and R-carrier FH patients, respectively, and 12.7% and 9.5% increase for LL homozygotes and M carriers, respectively). We conclude that simvastatin may have important antioxidant properties through increasing serum PON1 activity, perhaps as a consequence of reducing oxidative stress, by a mechanism independent of apoAI-containing lipoprotein concentration and without the influence of PON1-192 and PON1-55 genetic polymorphisms. Further studies are clearly warranted to clarify the precise mechanism by which simvastatin therapy is associated with increased PON1 activity.

Metolachlor exists in multiple, different stable conformations in solution. Assignment of the NMR frequencies to chemical structure is a prerequisite to understanding the behavior of individual conformations. (1)H NMR experiments of metolachlor in different chemical environments identified the labile sites of metolachlor and environments that influence conformational/configurational changes. Within very specific chemical environments, metolachlor atropisomers aS,12S (aR,12R) and aR,12S (aS,12R) freely interchange, and consequently, the multiple conformations also interchange. The changes in chemical environments, which most alter the conformations and molecular dynamics of metolachlor, identify the most critical components affecting its environmental fate. These results enable a structural interpretation of conformational changes that can influence the environmental fate of metolachlor.

A HPLC method was developed to quantify rhamnolipid species in a bacterial biosurfactant mixture. The biosurfactant mixtures containing mainly 3-[3'-(L-rhamnopyranosyl-oxy)decanoyloxy]decanoic acid (RhC10C10), 3-[3'-(2'-O-alpha-L-rhamnopyranosyl-oxy)decanoyloxy]decanoic acid (Rh2C10C10), 3-[3'-(2'-O-alpha-L-rhamnopyranosyl-oxy)decanoyloxy]dodecanoic acid (Rh2C10C12), and a dehydrogenated variety of the latter, 3-[3'-(2'-O-alpha-L-rhamnopyranosyl-oxy)decanoyloxy]dodecenoic acid (Rh2C10C12-H2), were isolated from Pseudomonas aeruginosa UG2 cultures grown on corn oil as sole carbon. The rhamnolipid species were identified and quantified after their derivatization to the corresponding phenacyl esters. To confirm the reliability of the HPLC method, the biosurfactant mixtures and the HPLC isolated species were further analyzed. Mass spectroscopy (electrospray ionization and atmospheric pressure chemical ionization techniques) was used to confirm their molecular mass, gas chromatography to verify their fatty acid content, and a colorimetric assay to quantify the rhamnose content.
