RESUMO

INTRODUCTION: This study compared the efficacy of Aryoseven with Novoseven to control bleeding episodes in patients with hemophilia A with inhibitors. METHODS: Sixty-six patients were randomized into 2 groups, with 4 consecutive block randomization. These groups received Aryoseven and Novoseven dosages of 90 to 120 µg/kg intravenously every 2 hours. RESULTS: Median (interquartile range) level of factor VIII (FVIII) inhibitor in groups A and B was 15.0 and 19.0 Bethesda Unit (BU) preadministration. Bleeding onset in group A was 1246 ± 1104 minutes and in group B was 2301 ± 1693 minutes (P = .311). The Kavakli global response scores and treatment success rate was comparable in both the groups. The side effects in groups A (9.7%) and B (2.9%) were comparable. CONCLUSION: Biosimilar recombinant activated FVII is found to be as effective as Novoseven in the treatment of acute joint bleeding in patients with hemophilia with inhibitors. Its usage will decrease the gaps in hemophilia.

In order to define the protein expressional changes related to the process of meglumine antimoniate resistance in anthroponotic cutaneous leishmaniasis (CL), we performed a comparative proteomics analysis on sensitive and resistant strains of Leishmania tropica isolated from Iranian CL patients. Cell proteins were analysed with 2-dimensional electrophoresis and differentially expressed proteins were identified by matrix-assisted laser desorption/ ionization time-of-flight mass spectrometry. Image analysis of the matched maps identified 7 proteins that were either over- or down-expressed: activated protein kinase c receptor(LACK), alpha tubulin (x2), prostaglandin f2-alpha synthase, protein disulfide isomerase, vesicular transport protein and a hypothetical protein. The study shows the usefulness of proteomics in identifying proteins that may express differences between sensitive and resistant L. tropica isolates.

PURPOSE: To report a case of explosive trauma to a patient with a history of radial keratotomy (RK), which resulted in multiple wound dehiscences but not globe rupture. METHODS: A 29-year-old male underwent radial keratotomy 1 year before he sustained facial trauma from a gasoline tank explosion. Corneal abrasions were treated with copious irrigation, topical antibiotics, corticosteroids, and scopolamine. He did not have a ruptured globe in either eye. RESULTS: The incision sites from radial keratotomy had evidence of anterior dehiscence in both eyes. One month following the injury, the patient had corrected visual acuity of 20/20-1 in the right eye and 20/25+1 in the left eye. At that time, the RK wounds were well healed with minimal irregularity over the incision sites. CONCLUSION: This case demonstrates the excellent recovery of visual acuity in a patient with a partial thickness traumatic wound rupture 12 months following radial keratotomy.
