RESUMO

There is little evidence on optimizing the effectiveness and implementation of evidence-based early childhood development (ECD) interventions when task-shifted to frontline workers. In this Methods Forum paper, we describe our adaptation of the International Guide for Monitoring Child Development (GMCD) for task-shifting to frontline workers in Guatemala and India. In 2021-2022, implementers, trainers, frontline workers, caregivers, and international GMCD experts collaborated to adapt the GMCD for a task shifted implementation by frontline workers. We used an eight-step co-creating process: assembling a multidisciplinary team, training on the existing package, working groups to begin modifications, revision of draft modifications, tailoring of visual materials and language, train-the-trainers activities, pilot frontline worker trainings, final review and feedback. Preliminary effectiveness of adaptations was evaluated through narrative notes and group-based qualitative feedback following pilot trainings with 16 frontline workers in India and 6 in Guatemala. Final adaptations included: refining training techniques to match skill levels and learning styles of frontline workers; tailoring all visual materials to local languages and contexts; design of job aids for providing developmental support messages; modification of referral and triage processes for children in need of enhanced support and speciality referral; and creation of post-training support procedures. Feedback from pilot trainings included: (1) group consensus that training improved ECD skills and knowledge across multiple domains; and (2) feedback on ongoing needed adjustments to pacing, use of video-based vs. role-playing materials, and time allocated to small group work. We use the Framework for Reporting Adaptations and Modifications to Evidence-based Implementation Strategies (FRAME-IS) framework to document our adaptations. The co-creating approach we use, as well as systematic documentation of adaptation decisions will be of use to other community-based early childhood interventions and implementation strategies.


Main findings: The International Guide for Monitoring Child Development, an early childhood development support and monitoring tool, was successfully adapted for use by frontline workers in rural India and Guatemala.Added knowledge: Our Methods Forum paper uses a detailed framework to document the collaborative, co-creating process used and the adaptive decisions taken.Global health impact for policy and action: Evidence on how best to adapt and optimize early childhood interventions for frontline workers will be useful or scaling up support for children globally.

High-throughput RNA-sequencing can determine the impact of nutrients and their combinations on gene transcription levels in osteocytes, and clarify the biological pathways associated with their impact on bone tissues. Previously, we reported that resveratrol (RES) and peonidin-3-O-glucoside (POG) increased osteoblastogenesis, as well as reduced osteoclastogenesis in transgenic teleost fish models. Here, we perform whole-genome transcriptomic profiling of osteoblasts treated with POG or RES to provide a comprehensive understanding of alterations in gene expression and the molecular mechanisms involved. Cultured human fetal osteoblastic hFOB 1.19 cells were treated with the test compounds, and then RNA was used to prepare RNA-seq libraries, that were sequenced using a NovaSeq 6000. Treatment with POG or RES increased osteoblast proliferation and reduced apoptosis. Transcriptomic profiling showed that of the 29,762 genes investigated, 3177 were differentially expressed (1481 upregulated, 1696 downregulated, FDR ≤ 0.05) in POG-treated osteoblasts. In the RES-treated osteoblasts, 2288 genes were differentially expressed (DGEs, 1068 upregulated, 1220 downregulated, FDR ≤ 0.05). Ingenuity® Pathway Analysis (IPA) of DGEs from RES or POG-treated osteoblasts revealed significant downregulation of the apoptosis, osteoarthritis and HIF1α canonical pathways, and a significant reduction in Rankl mRNA expression. The data suggest that RES and POG have both anabolic and anticlastogenic effects.

Este artigo teve como objetivo analisar o entendimento de 62 Agentes Comunitários de Saúde da cidade de Campo Bom, no Rio Grande do Sul (Brasil), sobre as definições estabelecidas por eles acerca de sua identidade, proporcionando uma reflexão referente às práticas do cotidiano do trabalho. As entrevistas individuais e questionários auto preenchidos foram analisadas pelo seu conteúdo a partir da construção do Discurso do Sujeito Coletivo. As percepções apontam para a autodefinição do agente como um trabalhador que possui escuta qualificada; elo entre a equipe e a comunidade; múltiplas ações, que muitas vezes vão além do estabelecido legalmente; realizações pessoais condicionadas ao "sucesso" profissional; trabalho sob a lógica da educação em saúde. Esta pesquisa demonstrou que, por vezes, é o próprio trabalhador que se coloca na condição de "super-herói", o que nos faz pensar o quanto os processos de trabalho devem ser amplamente e continuamente discutidos.

O estudo teve como objetivo desvelar a percepção dos residentes sobre prevenção de quedas no Programa de Residência Multiprofissional em Saúde para estabelecer melhoria de processos assistenciais em um Hospital Universitário. Realizou-se uma pesquisa qualitativa, descritiva- exploratória, com os residentes do primeiro ano dos programas de cardiologia e terapia intensiva do adulto. Para a coleta de informações, optou-se pela técnica de grupo focal, onde as conversas foram gravadas e transcritas na íntegra, e a análise de conteúdo por BARDIN. Evidenciou-se com os relatos dos participantes, o conhecimento sobre as estratégias de prevenção de quedas, a integração dos profissionais de saúde e a construção de propostas de ações de mudanças na incorporação de atividades multiprofissionais. Concluiu-se com a percepção dos residentes, a necessidade de educação permanente em saúde, o trabalho em equipe no cuidado integral ao usuário e a revisão periódica de protocolos para promover a redução de quedas

Para a efetivação de um cuidado seguro, é fundamental padronizar ações nas instituições de saúde; a necessidade de transporte do paciente durante o seu internamento, demanda da equipe um bom planejamento para minimizar os riscos. Objetivo: identificar como é realizado o transporte intra/extra-hospitalar de crianças/adolescentes de um hospital pediátrico de Curitiba. Método: estudo observacional descritivo, de abordagem quantitativa. Aplicou-se um check-list ara cada transporte pediátrico observado, totalizando 100 transportes. O estudo evidenciou que 18% dos transportes foram realizados de forma segura, 93% foram planejados e classificados pelo(a) enfermeiro(a) do setor, a equipe de transporte foi definida pelo(a) enfermeiro(a) em 94% dos transportes, e 99% estavam com os equipamentos adequados e com boas condições técnicas. Conclui-se que, abordando a segurança do paciente em prol de uma assistência segura e de qualidade, propõe-se a elaboração de um protocolo institucional de transporte de paciente pediátrico.

O objetivo da pesquisa foi desvelar o conhecimento de coordenadores e discentes de graduação sobre a relevância da temática gestão em saúde na formação em Enfermagem e Medicina. Utilizou-se método exploratório-descritivo, analítico, com abordagem mista. Participaram 5 coordenadores e 116 estudantes dos cursos estudados. Realizou-se em três fases: coleta e análise das informações avaliando-se e investigando-se documentalmente os Projetos Pedagógicos de Curso (PPC) e Matrizes Curriculares (MC), buscando-se conteúdos da gestão. Entrevista semiestruturada com coordenadores analisando-se conhecimentos da gestão. Questionário com escala Likert para estudantes, avaliando-se o aprendizado na gestão. Os resultados demonstram que: PPC e MC seguem a legislação, coordenadores possuem pouca percepção e experiência, não tem o conhecimento dos conteúdos em gestão. Estudantes não estão preparados para atuarem na gestão. Conclui-se: PPC e MC devem ser integrados, com aplicação de metodologias ativas nos processos de ensino em gestão da saúde.

Despite the widespread use of nanotechnology in radio-imaging applications, lipoprotein based delivery systems have received only limited attention so far. These studies involve the synthesis of a novel hydrophobic radio-imaging tracer consisting of a hydrazinonicotinic acid (HYNIC)-N-dodecylamide and 99mTc conjugate that can be encapsulated into rHDL nanoparticles (NPs). These rHDL NPs can selectively target the Scavenger Receptor type B1 (SR-B1) that is overexpressed on most cancer cells due to excess demand for cholesterol for membrane biogenesis and thus can target tumors in vivo. We provide details of the tracer synthesis, characterization of the rHDL/tracer complex, in vitro uptake, stability studies and in vivo application of this new radio-imaging approach.

OBJECTIVE: The aim of this prospective study was to compare the three-dimensional effects of the conventional helical uprighting spring (CA) and the mini-implant assisted helical uprighting spring (MIA), using CBCT scans. METHODS: Twenty patients with mesially tipped second mandibular molars were divided into two groups: CA group, in which 10 patients were treated using a conventional helical uprighting spring with conventional anchorage; and MIA group, in which 10 patients were treated using a mini-implant supported uprighting spring. Molar uprighting was observed in both groups for a period of four months. Two standardized 11×5-cm CBCT sections of the mandible were taken, being one prior to uprighting and one at the end of the four month follow-up. Statistical analyses at the beginning of treatment and after a 4 month follow-up were performed, with a significance level of p< 0.05. RESULTS: The mean amount of change in mesiodistal angulation in the MIA group was 8.53 ± 2.13o (p< 0.001) and in the CA group was 9.8 ± 0.5o (p< 0 .001). Statistically significant differences were found between the two groups with regard to buccolingual inclination of canine, first and second premolars (p< 0.05), second molar (p< 0.001) and extrusion of second molar (p< 0.05). CONCLUSIONS: The mean amount of change in the mesial angulation of the second molar in the CA as well as the MIA groups was similar. MIA, which used mini-implant as a source of anchorage, was more effective in preventing movement of the anchorage teeth as well as preventing extrusion of the second molar in the vertical plane, when compared to the CA group, which used dental units as a source of anchorage.

RESUMO Objetivo: o objetivo deste estudo prospectivo foi comparar, usando imagens de TCFC, os efeitos tridimensionais da mola convencional de verticalização (CA) e da mola de verticalização com ancoragem em mini-implantes (MIA). Métodos: vinte pacientes com segundos molares inferiores inclinados mesialmente foram divididos em dois grupos: grupo CA, no qual 10 pacientes foram tratados usando mola helicoidal de verticalização com ancoragem convencional; e grupo MIA, com 10 pacientes tratados usando mola de verticalização ancorada em mini-implantes. A verticalização dos molares foi observada nos dois grupos por um período de quatro meses. Foram obtidas duas secções tomográficas da mandíbula, com dimensões padronizadas de 11 x 5 cm, uma antes da verticalização e outra ao fim dos quatro meses de acompanhamento. As análises estatísticas ao início do tratamento e após os quatro meses de acompanhamento foram realizadas a um nível de significância de p< 0,05. Resultados: a média das alterações na angulação mesiodistal do grupo MIA foi de 8,53 ± 2,13o (p< 0,001), e do grupo CA foi de 9,8 ± 0,5o (p< 0,001). Diferenças estatisticamente significativas foram encontradas entre os dois grupos em relação à inclinação vestibulolingual do canino, do primeiro e segundo pré-molares (p< 0,05) e do segundo molar (p< 0,001), bem como para a extrusão do segundo molar (p< 0,05). Conclusões: a alteração média na angulação mesial do segundo molar nos grupos CA e MIA foi semelhante. O método MIA, que usou mini-implantes como ancoragem, foi mais efetivo na prevenção da movimentação dos dentes de ancoragem, bem como na prevenção da extrusão do segundo molar no plano vertical, quando comparado com o grupo CA, em que dentes foram usados como fonte de ancoragem.

Objective: To report a case of Multiple myeloma (MM) in mandible with paraesthesia. Material and Methods: The present case report highlights the occurrence of a swelling in the left posterior mandibular region in a 70-year old female patient and draws attention to the first clinical sign of a widespread systemic disease manifesting in the head and neck region. Results: The initial presumptive diagnosis was invalidated by the histopathological examination and other diagnostic tests that revealed the features characteristic for MM. Conclusion: Being a rare disease, MM should be kept on forefront of differential diagnosis for jaw lesions in a geriatric patient with paraesthesia. (AU)


Objetivo: Relatar um caso de mieloma múltiplo (MM) na mandíbula com parestesia. Material e Métodos: O relato de caso descreve a ocorrência de um inchaço na região mandibular posterior esquerda em uma paciente de 70 anos e chama a atenção para o primeiro sinal clínico de uma doença sistêmica disseminada que se manifesta na região da cabeça e pescoço. Resultados: O diagnóstico presuntivo inicial foi invalidado pelo exame histopatológico e outros testes diagnósticos que confirmaram as características do MM. Conclusão: Sendo uma doença rara, o MM deve ser considerado como uma primeira alternativa de diagnóstico diferencial de lesões mandibulares em pacientes geriátricos com parestesia. (AU)

OBJECTIVE: Cardiac surgical operations involving extracorporeal circulation may develop severe inflammatory response. This severe inflammatory response syndrome (SIRS) is usually associated with poor outcome with no predictive marker. Red cell distribution width (RDW) is a routine hematological marker with a role in inflammation. We aim to determine the relationship between RDW and SIRS through our study. METHODS: A total of 1250 patients who underwent cardiac surgery with extracorporeal circulation were retrospectively analyzed out of which 26 fell into the SIRS criteria and 26 consecutive control patients were taken. RDW, preoperative clinical data, operative time and postoperative data were compared between SIRS and control groups. RESULTS: The demographic profile of the patients was similar. RDW was significantly higher in the SIRS versus control group (15.5±2.0 vs. 13.03±1.90), respectively with P value <0.0001. There was significant mortality in the SIRS group, 20 (76.92%) as compared to 2 (7.6%) in control group with a P value of <0.005. Multiple logistic regression analysis revealed that there was significant association with high RDW and development of SIRS after extracorporeal circulation (OR for RDW levels exceeding 13.5%; 95% CI 1.0-1.2; P<0.05). CONCLUSION: Increased RDW was significantly associated with increased risk of SIRS after extracorporeal circulation. Thus, RDW can act as a useful tool to predict SIRS in patients undergoing cardiac surgery with extracorporeal circulation. Hence, more aggressive measures can be taken in patients with high RDW to prevent postoperative morbidity and mortality.

Background: The hydrolysis of keratin wastes by microorganisms is considered a biotechnological alternative for recycling and valorization through keratinolytic microorganisms. Despite their resistant structure, keratin wastes can be efficiently degraded by various microorganisms through the secretion of keratinases, which are promising enzymes for several applications, including detergents, fertilizers, and leather and textile industry. In an attempt to isolate keratinolytic microorganisms that can reach commercial exploitation as keratinase producers, the current work assesses the dynamics of keratin biodegradation by several keratinolytic fungal strains isolated from soil. The activity of fungal strains to degrade keratin substrates was evaluated by SEM, FTRIR-ATR spectra and TGA analysis. Results: SEM observations offered relevant information on interactions between microorganism and structural elements of hair strands. FTIR spectra of the bands at 1035­1075 cm-1 assigned to sulfoxide bond appeared because of S­S bond breaking, which demonstrated the initiation of keratin biodegradation. According to TGA, in the second zone of thermal denaturation, where keratin degradation occurs, the highest weight loss of 71.10% was obtained for sample incubated with Fusarium sp. 1A. Conclusions: Among the tested strains, Fusarium sp. 1A was the most active organism in the degradation process with the strongest denaturation of polypeptide chains. Because keratinolytic microorganisms and their enzymes keratinases represent a subject of scientific and economic interest because of their capability to hydrolyze keratin, Fusarium sp. 1A was selected for further studies.

Cardiac hydatid cyst is an uncommon but potentially fatal disease. In cystic Echinococcus humans are an accidental host. Liver and lungs are the most frequently involved organs. Herein a unique case of intramyocardial hydatid cyst of left ventricle along with pulmonary hydatid cyst in a 38-year-old lady is reported. Surgical removal of the cardiac hydatid cyst was done with the aid of cardiopulmonary bypass followed by removal of pulmonary hydatid cyst.

Biofilms formed by Candida albicans, a human pathogen, are known to be resistant to different antifungal agents. Novel strategies to combat the biofilm associated Candida infections like multiple drug therapy are being explored. In this study, potential of chloroquine to be a partner drug in combination with four antifungal agents, namely fluconazole, voriconazole, amphotericin B, and caspofungin, was explored against biofilms of C. albicans. Activity of various concentrations of chloroquine in combination with a particular antifungal drug was analyzed in a checkerboard format. Growth of biofilm in presence of drugs was analyzed by XTT-assay, in terms of relative metabolic activity compared to that of drug free control. Results obtained by XTT-metabolic assay were confirmed by scanning electron microscopy. The interactions between chloroquine and four antifungal drugs were determined by calculating fractional inhibitory concentration indices. Azole resistance in biofilms was reverted significantly (p < 0.05) in presence of 250 µg/mL of chloroquine, which resulted in inhibition of biofilms at very low concentrations of antifungal drugs. No significant alteration in the sensitivity of biofilms to caspofungin and amphotericin B was evident in combination with chloroquine. This study for the first time indicates that chloroquine potentiates anti-biofilm activity of fluconazole and voriconazole.

Diacylglycerol O-acyltransferase 1 (DGAT1) is a microsomal enzyme that catalyzes the final step of triglyceride synthesis. The DGAT1 gene is a strong functional candidate for determining milk fat content in cattle. In this work, we used PCR-SSCP (polymerase chain reaction-single-strand conformation polymorphism) and DNA sequencing to examine polymorphism in the region spanning exon 7 to exon 9 of the DGAT1 gene in Murrah and Pandharpuri buffaloes. Three alleles (A, B and C) and four novel single-nucleotide polymorphisms were identified in the buffalo DGAT1 gene. The frequencies of the alleles differed between the two buffalo breeds, with allele C being present in Murrah but not in Pandharpuri buffalo. The allele variation detected in this work may influence DGAT1 expression and function. The results described here could be useful in examining the association between the DGAT1 gene and milk traits in buffalo.

Este artigo relata o caso de um cão com melanoma oral tratado exclusivamente com eletroquimiioterapia e que, após 17 meses, não apresentou sinais de recidiva.


Cancer is the most common death cause in dogs and its prevalence has been continually increased. Melanoma is the most prevalent malignant oral tumor of dog. This article shows one oral dog melanoma treated exclusively with eletrochemotheray and, after 17 months, did not present reincidence.
