RESUMO

One of the most common respiratory chronic diseases is asthma, with 5-16 % of world prevalence. In chronic disease, prevention, diagnosis, management, and palliation are the strategies for a care model; the patient-self management is fundamental with the so-called therapeutic education (TE) to train the patient in the necessary skills. The challenge of TE grows when the patient is a child or a person with special needs, even more in public health care in low- and middle-income countries. This is a problematic human situation, that needs soft system thinking. In this research, the analytic hierarchy process was used to determine the perceived importance of the factors that affect children's asthma health care in a public health institution in Mexico. The soft systems methodology (SSM) was applied to learn about the disease self-management, in order to obtain activities models and an action plan. According to AHP, human resources and education were percived as the most important among the factors that affect children health care within the institution. The perceived importance of two main subfactors (human resources training and TE) was around 34 %, which emphasizes the need to generate strategies for the improvement of the education of patients and health care providers. In MSS, the rich vision, the CATWOE, the root definition, and an activities model were proposed, including the application of an instructional design methodology (ADDIE) for the development of learning objects of asthma for children. Desirable and feasible actions and recommendations include the update of the national clinical practice guides, the use of control measures such as the Asthma Control Test, and a daily asthma diary to register the daily status of the breathing capacity, the drug administration, and the potential trigger events. The proposed model can be used as part of a systemic patient-and-family centered approach for chronic care model (CCM), particularly in diseases unsuitable for prevention interventions.

RESUMO

Sheep pain is an animal welfare issue monitored based on behavioral responses, including appetite. Dominant (alpha) males have priority for accessing limited feed resources, however, the effects of pain on feed interest in members of a group with defined social hierarchy are unknown. Our objective was to investigate effects of acute post-orchiectomy pain on alpha rams' interest in accessing a limited feed resource. Eighteen rams were randomly housed in pens of 3 rams. After acclimation, the first 5-d (consecutive) battery of a behavior test was performed. In this test, 180 g of the regular diet concentrate was placed in a portable trough in the center of the pen; this feed was supplemental to the diet and represented a limited, albeit strongly preferable feed resource. Rams were filmed for 5 min after the feed introduction. Hierarchical levels (alpha, beta, and gamma) were defined based on the social hierarchical index according to higher initiator and lower receptor agonistic behaviors from the social network analyses. After 15 d, a second 5-d behavioral test battery was repeated. On the following day, alpha rams were castrated. Flunixin meglumine was given immediately before surgery and a final behavioral test was performed 8 h post-orchiectomy, concurrent with an expected peak in postoperative pain. For all recordings, the latency, frequency, and duration of time that each ram had its mouth inside the feed trough were recorded, and the Unesp-Botucatu sheep acute pain scale pain scale (USAPS) was applied. The social hierarchical index was highest in alpha rams, followed by beta and gamma. The pain scores were statistically equivalent across the 11 evaluation days for beta and gamma rams, whereas there was an increase in the final evaluation for alpha. There was no difference in latency, frequency, and duration between alpha, beta, and gamma rams across evaluations. We concluded that acute post-orchiectomy pain did not decrease alpha rams' interest in accessing limited feed. Routine feeding offers a valuable chance to detect pain-related behavior using the USAPS in rams. However, dominance may confound appetite-related behaviors in assessing acute pain, as alpha rams' interest in limited feed remained unaffected by the pain.

RESUMO

Rodents establish dominance hierarchy as a social ranking system in which one subject acts as dominant over all the other subordinate individuals. Dominance hierarchy regulates food access and mating opportunities, but little is known about its significance in other social behaviors, for instance during collective navigation for foraging or migration. Here, we implemented a simplified goal-directed spatial task in mice, in which animals navigated individually or collectively with their littermates foraging for food. We compared between conditions and found that the social condition exerts significant influence on individual displacement patterns, even when efficient navigation rules leading to reward had been previously learned. Thus, movement patterns and consequent task performance were strongly dependent on contingent social interactions arising during collective displacement, yet their influence on individual behavior was determined by dominance hierarchy. Dominant animals did not behave as leaders during collective displacement; conversely, they were most sensitive to the social environment adjusting their performance accordingly. Social ranking in turn was associated with specific spontaneous neural activity patterns in the prefrontal cortex and hippocampus, with dominant mice showing higher firing rates, larger ripple oscillations, and stronger neuronal entrainment by ripples than subordinate animals. Moreover, dominant animals selectively increased their cortical spiking activity during collective movement, while subordinate mice did not modify their firing rates, consistent with dominant animals being more sensitive to the social context. These results suggest that dominance hierarchy influences behavioral performance during contingent social interactions, likely supported by the coordinated activity in the hippocampal-prefrontal circuit.

RESUMO

Context: The increasing use of software and information technology in modern society requires that the deployment of IT solutions should be more efficient and controlled. In this sense, agile methodologies are essential to achieve this goal, but a relevant question arises: Which is the right methodology for reaching that goal? Objective: The purpose of the research was to study the prioritization of agile practices and improvement objectives in the context of software development, using the Analytic Hierarchy Process (AHP) method. For this purpose, 42 agile practices, 16 improvement objectives, 4 organizations dedicated to software development in Colombia and 40 professionals in the area of information and communication technologies in the same country were considered. Method: ology: The methodological approach used was mixed. On the one hand, we applied a quantitative approach for the treatment of data with the AHP method and, on the other hand, a qualitative analysis by consulting experts through a digital survey to validate the prioritization of the improvement objectives. Questionpro was used as a support tool for multicriteria comparison. Results: The main results show that applying AHP allowed us to prioritize 6 improvement objectives and 5 agile practices, where it is highlighted that the prioritization does not discard the other objects of comparison but allows us to put into practice the prioritized elements to favor their progressive implementation. The non-prioritized elements could be part of future iterations of multicriteria comparison that go hand in hand with capacity development and organizational maturity models in the context of global software development. Conclusion: It is highlighted that, although there is evidence of the application of multi-criteria comparison methods in the software development industry, this work applies such methods in the context of four different agile methodologies, which translates into a precedent for the conformation of hybrid methodological approaches.

RESUMO

Desert aquatic species tend to show isolated and disconnected populations due to the fragmented nature of their environment; however, the morphology of the hydrographic basins, added to humid climatic conditions, can allow dispersion between populations in a desert environment. The aim of this study was to examine the influence of drainage morphology on the phylogeographic structure and gene flow (using a fragment of the mitochondrial control region and seven microsatellite markers) of an endemic taxon of the Andean Precordillera in the Atacama Desert, the aquatic frog species Telmatobius pefauri. We detected three genetic clusters, one cluster present in the Lluta basin and two clusters in the Azapa basin. The results suggest that the genetic structure of T. pefauri is influenced by the morphology of the drainage network formed by the Lluta and Azapa basins: localities present in the same drainage, Tignamar River, were less differentiated and showed higher gene flow levels among them than to their conspecifics belonging to the other drainage in the same basin, Seco River, and those belonging to the other basin, Lluta basin. Gene flow patterns and genetic structure to populations Atacama Andean aquatic taxa would be influenced by basin morphology, with dispersion being stimulated in dendritic hydrological systems, and eventually by humid climatic (regional) events.

RESUMO

Biometric technology has transformed human biological characteristics into a new form of privacy, and the misuse of this technology poses challenges to protecting this new privacy. This article initially defines biometric technology and biometric characteristics, further demonstrating why biometric characteristics belong to personal privacy and how biometric technology poses challenges to its protection. Through analysis, this article argues that the essence of these challenges is the conflicts between the ethical principle of privacy protection and the ethical principle of maximizing social benefits. In order to address these challenges, it is necessary first to weigh the fundamental ethical principles. The two basic principles of privacy protection and maximizing social benefits are not mutual antagonism but hierarchy, and this hierarchy should be based on the principle of practical feasibility. That is, applying biometric technology should first meet the principle of practical feasibility and, on this premise, realize the principle of maximizing social benefits based on not infringing on the principle of privacy protection.

Using renewable energies is a global strategy to mitigate the acceleration of global warming generated by industrial processes and is a sustainable way to diversify the energy matrix in all countries. Biomass is a renewable energy source that produces biofuels and generates electricity and heat. The primary purpose of this work is to identify the municipalities in Colombia where agricultural, livestock, and urban residual biomass could be suitable for energy generation in a sustainable and renewable way. To that end, we carried out a Geostatistical Multi-Criteria Decision Methodology using Analytical Hierarchy Processes such as Rank-Sum and Weighted Linear Combination, as well as considering a set of sustainable development indicators applied to official Colombian data. Two scenarios are considered for comparison purposes. The first one is according to expert criteria, and the second one considers The Sustainable Development Goals proposed by the United Nations. Under both proposed scenarios, 127 municipalities were found to be suitable for agricultural-urban residual biomass and 162 for livestock-urban residual biomass for energy generation. One of the main limitations for the use of urban biomass is that municipalities need to have sufficient production potential to fulfill their own energy needs. An additional comparison with previous works to evaluate the performance of the Multi-Criteria Decision Methodologies MCDM is also proposed.

RESUMO

Geoprocessing tools are great allies in the management of water resources, permitting the evaluation of several factors integrated into a multicriteria analysis. This improves the interpretation of the behavior of water and soil in hydrographic basins, allowing the delimitation of priority areas for actions. The objective of this study was to prepare a map of priority areas for water conservation in the Alto Iguaçu basin through a multicriteria analysis, aiming to assist in the management of water resources in the region. The priority map for water conservation was prepared using the weighted linear combination (WLC) method, considering the weight added to the factors that were obtained by the analytic hierarchy process (AHP) method. The method successfully identified priority areas for water conservation, where 40.7% of the Alto Iguaçu watershed was considered High priority and Very High priority, a number consistent with regional characteristics. The multicriteria analysis proved to be a flexible and easy to implement methodology, which can help managers when considering the trade-offs among several factors. The implementation of water conservation actions in areas that have been classified as being of High and Very High priority can benefit the regional ecosystem by controlling erosion, regulating the flow of water, and thus conserving the quantity and quality of water in the basin and its downstream region, benefiting the population and regional economics. RESUMO

BACKGROUND: Confinement of cattle imposes spatial restrictions and predisposes to aversive social encounters that can lead to contusions, wounds, pain, stress, fright, and reduced productivity. Although endogenous testosterone concentrations are linked to agonistic dominance behaviors in males, it is unknown whether decreased blood testosterone concentrations after castration alter social hierarchy rank in Nelore bulls. Therefore, in this study, we investigated the impact of the surgical would inflammation post-orchiectomy on social dynamics in a group of Nelore bulls (Bos indicus). Fourteen Nelore (Bos indicus) bulls were castrated and assessed pre- and post-surgically. Parameters evaluated were agonistic (mounting, headbutting, and fighting) and affiliative (head-play) behavior, plasma testosterone concentrations, average daily weight gain (ADG), and a score for severity of post-surgical infection. Exploratory statistics included social network analysis (SNA), hierarchy rank delta (Δ), and principal component analysis (PCA). Furthermore, statistical inferences included the Wilcoxon test, multiple logistic regression models, and Spearman's correlation. RESULTS: The social dynamic of Nelore bulls was modified after castration based on the findings of the SNA and the PCA. The moderate correlation between the postoperative inflammation level with the Δ, and the significant effect of this level in the logistic model post-castration were partially attributed to effects of pain on social relations. CONCLUSIONS: Our findings suggest the severity of post-surgical inflammation, which has an association with pain intensity, was closely associated with changes in the social hierarchy.

RESUMO

An analytical method for quantification of seventeen pharmaceuticals and one metabolite was validated and applied in the analysis of hospital effluent samples. Two different sampling strategies were used: seasonal sampling, with 7 samples collected bimonthly; and hourly sampling, with 12 samples collected during 12 h. Thus, the variability was both seasonal and within the same day. High variability was observed in the measured concentrations of the pharmaceuticals and the metabolite. The quantification method, performed using weighted linear regression model, demonstrated results of average concentrations in seasonal samples ranged between 0.19 µgL-1 (carbamazepine) and higher than 61.56 µgL-1 (acetaminophen), while the hourly samples showed average concentrations between 0.07 µgL-1 (diazepam) and higher than 54.91 µgL-1 (acetaminophen). It is described as higher because the maximum concentration of the calibration curve took into account the dilution factor provided by DLLME. The diurnal results showed a trend towards higher concentrations in the first and last hours of sampling. The risk quotient (RQ) was calculated using organisms from three different trophic levels, for all the analytes quantified in the samples. Additionally, in order to understand the level of importance of each RQ, an expert panel was established, with contributions from 23 specialists in the area. The results were analyzed using a hybrid decision-making approach based on a Fuzzy Analytic Hierarchy Process (FAHP) and the Technique for Order Preference by Similarity to Ideal Solution (TOPSIS) method, in order to rank the compounds by environmental risk priority. The compounds of greatest concern were losartan, acetaminophen, 4-aminoantipyrine, sulfamethoxazole, and metoclopramide. Comparison of the environmental risk priority ranking with the potential human health risk was performed by applying the same multicriteria approach, with the prediction of endpoints using in silico (Q)SAR models. The results obtained suggested that sulfamethoxazole and acetaminophen were the most important analytes to be considered for monitoring.

RESUMO

The aim of this study was to compare the variation in body weight, scrotal circumference (SC), and seminal quality during a breeding period in dominant and subordinate rams. Data were collected from 12 dyads of rams, each bred with 15 ewes for 7 weeks. Before joining, the dominance relationship between both rams from each dyad was determined. Body weight and SC were recorded weekly in the morning, and semen was collected by electroejaculation, determining the volume, sperm concentration, mass motility, and percentage of sperm with progressive motility. In addition, the total number of sperm and sperm with progressive motility ejaculated were calculated. Dominance had no direct effect or interaction with time on any of the variables analyzed. Body weight, seminal volume, sperm concentration, mass motility, percentage of sperm with progressive motility, and total ejaculated sperm varied with time (p < 0.05), and scrotal circumference and total ejaculated sperm with progressive motility tended to vary with time. In general, all indicators evaluated were affected in the first weeks, when most ewes were cycling, recovering as the breeding progressed. It was concluded that, at least under the conditions of this study, dominance position did not affect the profile of the reproductive variables evaluated, although all of them were affected during the breeding period.

RESUMO

Evaluating the outcomes and tracking the trajectory of biodiversity offsets is essential to demonstrating their effectiveness as a mechanism to conciliate development and conservation. We reviewed the literature to determine the principles that should underpin biodiversity offset planning and the criteria for offset evaluation at the project level. According to the literature, the core principles of equivalence, additionality, and permanence are used as criteria to evaluate conservation outcomes of offsets. We applied the criteria to evaluate offsets of a large iron ore mining project in the Atlantic Forest in Brazil. We examined equivalence in terms of the amount of area per biodiversity value affected and fauna and flora similarity, additionality in terms of landscape connectivity, and permanence in terms of guarantees to ensure protection and restoration offsets lasting outcomes. We found an offset ratio (amount of affected area:offset area) of 1:1.8 for forests and 1:2 for grasslands. Ecological equivalence (i.e., similarity between affected and offset areas) was found for forested areas, but not for ferruginous rupestrian grasslands or for fauna. Landscape metrics showed that connectivity improved relative to the preproject situation as a result of locating restoration offsets in the largest and best-connected forest patch. Permanence of offsets was addressed by establishing covenants and management measures, but financial guarantees to cover maintenance costs after mine closure were lacking. Offsets should be equivalent in type and size, provide conservation outcomes that would not be obtained without them (additionality), and be lasting (permanence). To monitor and evaluate offsets, it is necessary to determine how well these 3 principles are applied in the planning, implementation, and maintenance of offsets. Achieving measurable conservation outcomes from offsets is a long-term endeavor that requires sustained management support, and is information intensive. Thus, offsets require ongoing monitoring and evaluation as well as adaptive management.

The use of emergency departments (EDs) has increased during the COVID-19 outbreak, thereby evidencing the key role of these units in the overall response of healthcare systems to the current pandemic scenario. Nevertheless, several disruptions have emerged in the practical scenario including low throughput, overcrowding, and extended waiting times. Therefore, there is a need to develop strategies for upgrading the response of these units against the current pandemic. Given the above, this paper presents a hybrid fuzzy multicriteria decision-making model (MCDM) to evaluate the performance of EDs and create focused improvement interventions. First, the intuitionistic fuzzy analytic hierarchy process (IF-AHP) technique is used to estimate the relative priorities of criteria and sub-criteria considering uncertainty. Then, the intuitionistic fuzzy decision making trial and evaluation laboratory (IF-DEMATEL) is employed to calculate the interdependence and feedback between criteria and sub-criteria under uncertainty, Finally, the combined compromise solution (CoCoSo) is implemented to rank the EDs and detect their weaknesses to device suitable improvement plans. The aforementioned methodology was validated in three emergency centers in Turkey. The results revealed that the most important criterion in ED performance was ER facilities (14.4%), while Procedures and protocols evidenced the highest positive D + R value (18.239) among the dispatchers and is therefore deemed as the main generator within the performance network.

RESUMO

Currently, solid waste management strategies in Havana are outdated. This paper aimed to select the most suitable alternative for integrating material recovery facilities (MRF) with waste-to-energy technologies in the city of Havana, Cuba. Seven scenarios were considered: combustion, gasification, and hydrothermal carbonization (HTC) with and without carbon capture, and anaerobic digestion (AD). The selection was based on environmental, techno-economic, and social parameters using an analytic hierarchy process (AHP) as a multi-criteria decision-making tool (MCDM). The MCDM-AHP accounted for qualitative criteria (based on experts' judgments) and quantitative (based on Aspen Plus simulation models). From the MRF, 63% of the input recyclable materials were recovered, representing an energy saving of 256 kW-h/tMSW. The AHP results showed that environmental criteria had the highest priority, resulting in ~63% and ~73% higher than social and techno-economic criteria, respectively. Likewise, from the techno-economic, environmental, and social sub-criteria analysis, investment risk, pollution, and work safety had the major concern compared with the other sub-criteria levels. Overall, MRF+AD was the most suitable scenario (21% preference) for treating Havana's municipal solid waste (MSW), followed by combustion and gasification with carbon capture, respectively. This study confirms that AD is a preference option for emerging economies like Cuba, mainly due to low environmental pollution, high social acceptance, and financial stability in the long term.

RESUMO

Geographic information systems, the integration technique of multi-criteria assessment, together with the analysis hierarchy process, are recognized in decision support to describe and predict locations for implementation of different projects. In Brazil, shrimp farming has been consolidated for more than 30 years in the Northeast and North regions and is emerging in the Southeast. The purposes of this study were to analyze, classify and quantify through spatial planning for the establishment of shrimp farming in the North Fluminense region of the state of Rio de Janeiro, Brazil. The composition of the final model was structured into hierarchical categories with two models, factors, and constraints. The model of factors was subdivided into five sub-models: water abstraction, environmental, transport, public, and economic infrastructure. In turn, restrictions were composed of urbanized areas, environmental reserves. The best areas were identified as those that were flat, close to rivers, related to the environmental sub-model criteria (21%); followed by the sub-models of public, economic, and water abstraction infrastructure (20%), and transport with the lowest weight attributed (17%). The analyses indicated a great potential for the implementation of shrimp farming in the region, estimated in a total area of 628,088 ha, classified as adequate or very adequate (> 0.6).(AU)

Abstract Background The many combinations of chemotherapeutic agents and biologicals available in the Brazilian National Health System for the treatment of metastatic breast cancer require analysis that contribute to decision making. Objective The study's primary aim was to evaluate the first-line treatment of HER2- overexpressing metastatic breast cancer from the Brazilian Unified Health System perspective using multicriteria decision analysis (MCDA). Method The treatment options evaluated were (a) pertuzumab combined with trastuzumab and docetaxel, and (b) trastuzumab in combination with docetaxel. Using the hierarchical analytical method, medical oncologists compared the relevance of five predefined criteria: overall survival, response to treatment, adverse events, cost- effectiveness, and budget impact. Results The therapeutic scheme considered more appropriate by the model was pertuzumab combined with trastuzumab and docetaxel. The most sensitive criteria were adverse events, cost-effectiveness, and budget impact. The results suggest that the classification has a close relationship with the perspective of healthcare professionals participating in the questionnaire. Conclusion Defining the treatment of an incurable disease associated with a short survival time and high-cost treatment options necessitates complex decision-making. MCDA allows the weighting of criteria and considering criteria that would be difficult to measure in other methods, such as cost-effectiveness. These aspects differ from economic models and contribute to a broader evaluation of health decision-making.

Do sensory cortices process more than one sensory modality? To answer these questions, scientists have generated a wide variety of studies at distinct space-time scales in different animal models, and often shown contradictory conclusions. Some conclude that this process occurs in early sensory cortices, but others that this occurs in areas central to sensory cortices. Here, we sought to determine whether sensory neurons process and encode physical stimulus properties of different modalities (tactile and acoustic). For this, we designed a bimodal detection task where the senses of touch and hearing compete from trial to trial. Two Rhesus monkeys performed this novel task, while neural activity was recorded in areas 3b and 1 of the primary somatosensory cortex (S1). We analyzed neurons' coding properties and variability, organizing them by their receptive field's position relative to the stimulation zone. Our results indicate that neurons of areas 3b and 1 are unimodal, encoding only the tactile modality in both the firing rate and variability. Moreover, we found that neurons in area 3b carried more information about the periodic stimulus structure than those in area 1, possessed lower response and coding latencies, and had a lower intrinsic time scale. In sum, these differences reveal a hidden processing-based hierarchy. Finally, using a powerful nonlinear dimensionality reduction algorithm, we show that the activity from areas 3b and 1 can be separated, establishing a clear division in the functionality of these two subareas of S1.

RESUMO

Dorper rams (n = 24) were evaluated during the sexual resting season to determine their social rank (SR), either high (HSR) or low (LSR), under intensive management conditions in northern Mexico (25° N). Aggressive behaviors were quantified during male-to-male interactions, and appetitive and consummatory sexual behaviors during male-to-female interactions. Morphometric, live weight (LW), and body condition score (BCS) were recorded. During the early reproductive season, male-to-female behaviors were newly itemized simultaneously by seminal quality and quantity sampling. Finally, the dependent variables of the hemogram components were also quantified. Neither LW (61.25 ± 2.4 kg) nor morphometric variables differed between SR groups. However, BCS (2.25 vs. 2.66 u), sexual behaviors (i.e., approaches: 59.6 vs. 21.73 n, mating with ejaculation: 77.7 vs. 42.86 %, latency to ejaculation: 16.6 vs. 143.07 s), ejaculate volume (0.57 vs. 0.23 mL), and hemogram components favored the HSR rams (p < 0.05). Moreover, in their first male-to-female interaction, >50% of the LSR rams failed to display any sexual activity. HSR rams displayed a greater number of threatening behaviors, managing to displace LSR rams when exposed to estrus ewes during the male sexual resting season; more sexual behaviors; and an increased seminal volume in a non-live weight-dependent fashion.

Objective: To analyze the perception of residents to psychosocial risk factors they face throughout their academic training related to the domains of control and work rewards. Materials and Methods: Qualitative study with phenomenological methodology, which used in-depth interview as a technique to obtain data from the elements contemplated in the Battery of Instruments for the Evaluation of Psychosocial Risk Factors of the Ministry of Social Protection of Colombia. Forty-two residents from 8 medical specialties in Barranquilla were interviewed. The analysis of the information obtained was interpreted based on Schütz's sociological theory. Results: A relationship was observed between workload, time and organization, with little recognition of effort. In addition, there is little attachment to the institutions and there is no stable professional and emotional bond due to the low perception of self-fulfillment in the exercise of their work. Conclusions: The control and reward domains are conjectured as frequent problems in the field of medical residencies. If there is no control of the functions, participation in decision making and in the functioning of the institution, and if there are few opportunities to demonstrate mastery in the acquired responsibilities, the perception of well-being is implicated.
