ABSTRACT

Introduction : La présence de matériel prothétique intracardiaque expose au risque thromboembolique, infectieux et/ou hémorragique. L'objectif de ce travail était de décrire les particularités cliniques et étiologiques des complications de prothèses valvulaires mécaniques.Cas cliniques : Il s'agit d'une série de cinq observations de patients présentant une complication de prothèses valvulaires mécaniques colligées au service de cardiologie du Centre Hospitalier Universitaire de Libreville durant la période allant de Janvier 2017 à Décembre 2021. Les complications rapportées étaient deux cas de thrombose obstructive de prothèse, un accident vasculaire cérébral sur microthrombose, une endocardite infectieuse et un accident hémorragique sous antivitamines K. Ce dernier était responsable d'un décès. Conclusion : Ces cas cliniques mettent en exergue les difficultés liées au suivi de l'anticoagulation au long cours chez les patients porteurs de prothèse valvulaire mécanique dans notre contexte. La mise en place d'un programme d'éducation thérapeutique pourrait diminuer ces complications parfois létales


Introduction: The presence of intracardiac prosthetic material exposes to thromboembolic, infectious and/or hemorrhagic risk. The objective of this study was to describe the clinical and etiological particularities of mechanical valve prostheses complications. Clinical cases: five observations of patients presenting with a complication of mechanical valve prostheses collected in the cardiology department of the Libreville University Hospital Center during the period from January 2017 to December 2021, were reported. Complications observed were two cases of obstructive prosthesis thrombosis, a cerebrovascular stroke coming from microthrombosis, an infective endocarditis and a hemorrhagic accident under vitamin K antagonists. This last was responsible for the one death. Conclusion: These clinical cases highlight the difficulties related to the monitoring of long-term anticoagulation in patients with mechanical valve prosthesis in our context. The development of a therapeutic education program could reduce these potentially lethal complications

ABSTRACT

Introduction. Le spectre des atteintes cardiovasculaires au cours de l'infection à VIH a été modifi é par la trithérapie antirétrovirale. L'objectif de ce travail était de décrire le profi l des manifestations cardiovasculaires chez les patients vivants avec le VIH en le comparant à celui de patients séronégatifs. Méthodes. Il s'est agi d'une étude cas-témoins des dossiers de patients respectivement séropositifs et séronégatifs hospitalisés pour une pathologie cardiovasculaire au service de cardiologie du Centre Hospitalier Universitaire de Libreville de janvier 2015 à décembre 2018. L'analyse statistique a été réalisée à l'aide du logiciel Statview 5.0. Lestests de Chi-2 de Pearson ou Exact de Ficher ont été utilisés pour la comparaison des proportions. Résultats. L'étude a porté sur sur l'analyse de 82 et 150 dossiers de patients respectivement séropositifs et séronégatifs. Un âge inférieur à 50 ans était retrouvé chez 70,7% des séropositifs et 43,3% des séronégatifs (p<0,01). Le taux de CD4 moyen des séropositifs était de 189±170/mm3 et 45,1% d'entre eux étaient sous trithérapie antiretrovirale.La cardiomyopathie dilatée était l'atteinte cardiaque la plus fréquente chez les séropositifs (42,7%) et chez les séronégatifs (52,7%) (p=0,14). La maladie thromboembolique veineuse était relevée chez 7(8,5%) séropositifs et 14 (8,8%) séronégatifs (p=0,93). Une péricardite était diagnostiquée chez 25,6% des séropositifs avec une étiologie tuberculeuse dans 85,7% des cas. Les pathologies vasculaires athéromateuses étaient plus fréquentes chez les séronégatifs (23,1%) comparés aux séropositifs (6,1%) (p<0,01). La mortalité des séropositifs était principalement due aux péricardites (71,4%). Conclusion. les manifestations cardiovasculaires liées à l'immunodépression persistent chez les personnes vivant avec le VIH à Libreville. Un dépistage précoce de ces atteintes permettrait de réduire la mortalité.


Introduction. The spectrum of cardiovascular damage during HIV infection has been modified by triple antiretroviral therapy. The objective of this study was to describe the profile of cardiovascular manifestations in patients living with HIV by comparing it to the one of seronegative patients. Methods. This was a case-control study which focused on the files of patients hospitalized for a cardiovascular pathology in the cardiology department of the Center Hospitalier Universitaire de Libreville from january 2015 to december. 2018. Results. In total, there was on the analysis of the files of 82 seropositive patients and 150 seronegative patients. The age found was less than 50 years old in 70.7% of seropositives and 43.3% of seronegatives (p <0.01). The mean CD4 count in seropositives was 189 ± 170 /mm3 and 45.1% of them were on triple antiretroviral therapy. Dilated cardiomyopathy was the most common cardiac disease in HIVpositive (42.7%) and HIV-negative (52.7%) (p = 0.14). Venous thromboembolic disease was noted in 7 (8.5%) seropositives and 14 (8.8%) seronegatives (p=0.93).Pericarditis was diagnosed in 25.6% of seropositives patients with a tuberculous etiology in 85.7% of cases. Atheromatous vascular pathologies were more frequent in seronegative (23.1%) compared to seropositive (6.1%) (p <0.01). Mortality among seropositive was mainly due to pericarditis (71.4%)

ABSTRACT

Background. Venous thromboembolism (VTE) is regarded as the most preventable cause of inpatient death in hospital settings globally. VTE can be prevented through the provision of non-pharmacological and/or pharmacological thromboprophylaxis following individualised risk screening. The Caprini risk assessment model (RAM) offers a validated and well-established approach for VTE risk assessment in medical inpatients. Literature findings describe a trend towards inappropriate and under-prescribing of thromboprophylaxis in this population. Together with concerns regarding clinicians' perceived importance of VTE risk assessment, the need to clarify these aspects of practice is evident. Objectives. To describe VTE risk assessment and prophylaxis practices of medical practitioners in public sector hospitals in Western Cape Province, South Africa (SA). Methods. A retrospective, cross-sectional study design was employed in the medical wards of two district hospitals and one regional hospital in the Cape Town metropole, Western Cape. Medical folders of adult medical inpatients admitted between January and July 2020 were reviewed to assess VTE risk using the Caprini RAM. Thromboprophylaxis therapy prescribed and contraindications to chemoprophylaxis were also evaluated. Results. Of 380 patients included in the review, 51.6% were female, and the average age was 52.1 years (range 18 - 96); 21.3% had their weight recorded, while none had their height documented. Infectious disease was the predominant diagnosis (49.2%) detected in the sample. Common VTE risk factors identified included bed rest/restricted mobility for <72 hours (76.3%) and serious infection (67.4%). A total of 97.1% of patients (n=369) were found to be at moderate or higher risk of VTE (Caprini score ≥2). Of this at-risk group, 24.1% were eligible to receive chemoprophylaxis, yet no prescription for thromboprophylaxis was identified. Seventy percent of patients (n=266) were prescribed chemoprophylaxis, with enoxaparin accounting for 98.5% of regimens. Contraindications to chemoprophylaxis were recorded in 13.4% of patients. Conclusions. Although rates of VTE prophylaxis in medical inpatients may be improving, thromboprophylaxis still remains critically underutilised in this population. This study highlighted a consequence of this trend, with inappropriate chemoprophylaxis prescribing becoming more evident. Mechanical prophylaxis prescribing in medical inpatients is lacking, despite the associated benefits. RAMs should be adapted for the SA setting, where infectious diseases are prevalent. Future research should assess RAM use by clinicians, as this could provide insight into improving RAM uptake and thromboprophylaxis prescribing.

Background. Venous thromboembolism (VTE) is a common complication during and after hospitalisation, and is regarded as the most common cause of preventable death in hospitalised patients worldwide. Despite its importance, there are few data on VTE risk and adherence to prophylaxis prescription guidelines in surgical patients from the South African (SA) public sector, especially from low-resource environments such as Eastern Cape Province.Objectives. To evaluate the risk and prescription of VTE prophylaxis to surgical patients at a tertiary government hospital in the Eastern Cape.Methods. A cross-sectional clinical audit of general surgical inpatients was performed on two dates during July and August 2017. Patients' VTE risk was calculated by using the Caprini risk assessment model (RAM) and thromboprophylaxis prescription evaluated accordingly. Results. A total of 179 patients were included in the study, of whom 56% were male and 44% female. The average age was 45 (range 18 - 83) years. Of the total number of participants, 33% were elective cases and 67% were emergency admissions. With application of RAM, 77% of patients were at risk of VTE (Caprini score ≥2), with 81% of elective and 74% of emergency patients being at risk. The most prevalent risk factors for VTE were major surgery (34%), age 41 - 60 years (30%), age 61 - 74 years (20%) and sepsis during the previous month (27%). A contraindication to chemoprophylaxis was recorded in 30% of patients, with the most prevalent being renal dysfunction (40%), peptic ulcer disease (34%), active bleeding (17%), liver dysfunction (17%), coagulopathy (6%) and recent cerebral haemorrhage (6%). With regard to VTE risk profile and contraindications to chemoprophylaxis, the correct thromboprophylactic treatment was prescribed to 26% of at-risk patients, with 21% of elective and 27% of emergency admission patients receiving the correct therapy.Conclusions. Despite a high proportion of patients being at risk of VTE, the rate of adequate thromboprophylaxis prescription for surgical inpatients at Frere Hospital, East London, SA is very low. Increased availability of mechanical prophylaxis, as well as interventions to improve the rate of adequate prophylaxis prescription, needs to be evaluated for feasibility and effect in this hospital and other SA public hospitals

Introduction : L'association entre la maladie thromboembolique et la tuberculose est peu fréquente mais reste redoutable. Cette complication vasculaire, consécutive à un état d'hypercoagulabilité, peut en constituer une circonstance de découverte. Observation : Nous rapportons le cas d'une patiente âgée de 40 ans, admise dans un tableau d'embolie pulmonaire à risque intermédiaire secondaire à une thrombophlébite au niveau des deux veines iliaques et de la veine cave inférieure, chez qui on a posé le diagnostic d'une spondylodiscite tuberculeuse avec abcès froid en regard de L5 ­ S1. Conclusion : La tuberculose est considérée comme un facteur de risque thromboembolique avéré. La difficulté de la gestion thérapeutique de cette association est surtout liée aux interactions entre les anticoagulants de type antivitamine K ou les anticoagulants oraux directs (AOD), et les antituberculeux, en particulier la rifampicine. Cette maladie infectieuse constitue un véritable problème de santé publique dans certains pays, elle nécessite le renforcement du programme de lutte, des moyens de prévention et de dépistage

Introduction : L'étude épidémiologique transversale ENDORSE montrait une forte prévalence 52% des patients hospitalisés présentant un risque de MTEV dans le monde dont 64% de patients en chirurgie. Ce risque est encore plus élevé pendant la période obstétricale. Objectif : Evaluer les connaissances et attitudes pratiques sur la prévention de la maladie thrombo-embolique du personnel des services de gynécologie-obstétriques de Côte d'Ivoire. Méthodes : Etude multicentrique, mixte, transversale descriptive avec 198 questionnaires anonymes correctement remplis retenus soit 61,9% et un taux de non-réponses élevé de 19% dans les CHU de Côte d'Ivoire destinés au Personnel soignant des structures concernées ayant donné leur consentement libre et éclairé sur une période de 4 mois allant du 1er avril au 31 juillet 2016. La collecte et le traitement des données a été réalisée grâce aux logiciels Epidata et Epi Info 7.Résultats : L'âge moyen était de 39,3 ans avec des extrêmes de 27 et 60 ans et 58,6% du personnel avait moins de 5 années d'activités dans le service. Il s'agissait en majorité des sages-femmes (60,1%) suivi du personnel médical (39,9%). Soixante et onze virgule quatre pour cent (71,4%) jugeaient leur niveau de connaissances suffisant. Les facteurs de risques les plus cités par le personnel médical et paramédical étaient l'immobilisation prolongée (92,4% et 82,4%) suivie de la chirurgie pelvienne (89,9% et 64,7%) et de la contraception orale (86,1% et 51,3%). Trente-huit pour cent des médecins instauraient une héparinothérapie après un accouchement par voie basse contre 95% pour un accouchement par césarienne. Quatre-vingt-six virgule sept pour cent (86,7%) des médecins débutaient l'héparinothérapie préventive entre 12 et 24 heures du post-partum quel que soit le mode d'accouchement, pendant au moins 3 semaines pour 78% d'entre eux.Conclusion : Cette étude montre une connaissance et des attitudes pratiques approximatives d'où la nécessité d'une formation continue du personnel et l'éducation des patients afin d'assurer l'amélioration continue de la qualité des soins

Background: Venous thromboembolic (VTE) is a preventable leading cause of morbidity and mortality worldwide and is a cause of sudden death. Awareness of VTE among doctors will reduce the incidence of unexpected deaths in hospitalised patients.Objective: To assess doctors' awareness of venous thromboembolism in our centre.Design: Cross sectional questionnaire based study.Setting: University of Port Harcourt Teaching Hospital; Nigeria.Subjects: One hundred and twenty four doctors attending a hospital grand-rounds session were recruited. Results: The response rate was 82.7% (124 of 150 questionnaires). Almost half (n=57; 46%) correctly identified VTE; but two (1.6%) did not know what VTE was and 27 (21.8%) stated it to be normal haemostasis. Commonly identified risk factors included: increased body mass index 113 (91.1%); diabetes mellitus 105 (84.7%); pregnancy 105 (84.7%); age 104 (83.9%); and immobility 104 (83.9%). Although clinical features were identified; VTE was acknowledged to be asymptomatic by 62 (50%) responders. About 68 (54.8%) did not know of a VTE risk stratification model and only one (0.8%) had used a model in practice. Responders commonly prescribed anticoagulants for prophylaxis. Although most had come across VTE in practice; 15 (12.1%) had not seen a case of VTE.Conclusion: The knowledge of VTE was average. There was a gap between knowledge and practice for this preventable disorder. Patients in our centre were not routinely risk stratified for VTE. Updating of medical knowledge is encouraged

Background: Acquired resistance to protein C in pregnancy has been established as one of the factors associated with thromboembolic phenomenon, an important cause of maternal mortality and morbidity. Objectives: To establish the mean levels of PCA ratio (measure of protein C resistance) of among our pregnant women since maternal mortality rate of the country is on the increase despite efforts to reduce this trend. Materials and Methods: A prospective study was carried out in a tertiary institution in Enugu State, Southeastern Nigeria over the 7 months period from May 2010 to November 2010. Two hundred pregnant women and 50 non pregnant female controls were recruited and PCA ratio, (coagulometric assay) were determined. Results: There was a non significant difference between the mean and standard deviation PCA ratio of the female non pregnant controls and pregnant women in 2nd trimester 4.32±0.4 and 4.30±0.4 respectively. A significant difference was noted between the controls and pregnant women in 3rd trimester 4.32±0.4 and 3.87±0.5 respectively also between the pregnant women in their 2nd and 3rd trimester 4.30±0.4 and 3.87±0.5 respectively. Conclusion: There is increased protein resistance C in our pregnant women. This may implicate thromboembolic disorders as one of the leading causes of increase maternal mortality despite a downward trend in the prevalence of post partum haemorrhage

Les maladies thromboemboliques veineuses (MTEV) sont graves et mortelles ; elles necessitent une prise en charge d'urgence immediate et multidisciplinaire. Objectifs : Nos objectifs consistent a determiner les aspects epidemio-cliniques; paracliniques et evolutifs de ces maladies; et de rechercher leurs etiologies. Methodes : Il s'agit d'une etude a la fois retrospective et prospective; descriptive et analytique monocentrique realisee a l'USFR de Cardiologie de l'Hopital Universitaire Joseph Raseta Befelatanana; du 1er Janvier 2007 au 31 Decembre 2010. Nous avons recense 43 patients atteints de MTEV. Resultats : 65;12de nos patients ont une thrombose veineuse profonde des membres inferieurs (TVPMI); 32;55une embolie pulmonaire (EP) et 2;33une thrombose veineuse profonde de l'extremite des membres superieurs. Ces pathologies touchent beaucoup plus les femmes de 25 a 55 ans et les hommes a partir de 55 ans. Les principaux signes de decouverte sont la paresthesie; la douleur de la jambe et la douleur de la cuisse pour la TVPMI; et la douleur thoracique; l'hemoptysie et la dyspnee pour l'EP. Les thromboses de la veine surale gauche et de la veine femorale sont les plus frequemment rencontrees au cours de la TVPMI. La thrombose est plus frequemment au niveau de la branche gauche de l'artere pulmonaire pour l'EP; mais peut etre aussi multifocale. Conclusion : Un score de probabilite clinique (score de Wells) moyen ou eleve; avec ou sans positivite du dosage de D Dimeres; est suffisants pour poser le diagnostic. Sous Enoxaparine relaye precocement par Fluindione; 74;41des patients peuvent evoluer de facon favorable
