ABSTRACT

Abstract Objective: To assess the predictive value of selected growth phenotypes for neonatal morbidity and mortality in preterm infants < 30 weeks and to compare them with INTERGROWTH-21st (IG21). Methods: Retrospective analysis of data from the Brazilian Neonatal Research Network (BNRN) database for very low birth weight (VLBW) at 20 public tertiary-care university hospitals. Outcome: the composite neonatal morbidity and mortality (CNMM) consisted of in-hospital death, oxygen use at 36 weeks, intraventricular hemorrhage grade 3 or 4, and Bell stage 2 or 3 necrotizing enterocolitis. Selected growth phenotypes: small-for-gestational-age (SGA) defined as being < 3rd (SGA3) or 10th (SGA10) percentiles of BW, and large-for-gestational-age (LGA) as being > 97th percentile of BW. Stunting as being < 3rd percentile of the length and wasting as being < 3rd percentile of BMI. Single and multiple log-binomial regression models were fitted to estimate the relative risks of CNMM, comparing them to IG21. Results: 4,072 infants were included. The adjusted relative risks of CNMM associated with selected growth phenotypes were (BNRN/IG21): 1.45 (0.92-2.31)/1.60 (1.27-2.02) for SGA; 0.90 (0.55-1.47)/1.05 (0.55-1.99) for LGA; 1.65 (1.08-2.51)/1.58 (1.28-1.96) for stunting; and 1.48 (1.02-2.17) for wasting. Agreement between the two references was variable. The growth phenotypes had good specificity (>95%) and positive predictive value (70-90%), with poor sensitivity and low negative predictive value. Conclusion: The BNRN phenotypes at birth differed markedly from the IG21 standard and showed poor accuracy in predicting adverse neonatal outcomes.

ABSTRACT

We aimed to evaluate the efficacy and safety of mepolizumab (MEP) in the management of hypereosinophilic syndrome (HES). A systematic search was performed, and articles published until March 2021 were analyzed. The primary efficacy results evaluated were hospitalization rate related to HES, morbidity (new or worsening), relapses/failure, treatment-related adverse effects, prednisone dosage ≤10 mg/day for ≥8 weeks, and eosinophil count <600/μL for ≥8 weeks. A meta-analysis was conducted, when appropriate. Three randomized controlled trials (RCTs), with a total of 255 patients, were included. The studies contemplated the use of MEP 300 mg/SC or 750 mg/IV. According to the evaluation of the proposed outcomes, when relapse rates/therapeutic failures were assessed, there was a 26% reduction with MEP 300 mg/SC (RD=-0.26; 95% CI: -0.44 to -0.08; p=0.04) and 48% reduction with MEP 750 mg/IV (RD=-0.48; 95% CI: -0.67, -0.30; p<0.00001). For the outcomes, prednisone dosage ≤10 mg/day for ≥8 weeks was 48% (RD=0.48; 95% CI: 0.35 to 0.62; p<0.00001), and the eosinophil count <600/μL for ≥8 weeks was 51% (RD=0.51; 95% CI: 0.38 to 0.63; p<0.00001), both showed a reduction with MEP 300 mg/IV and 750 mg/IV. No statistically significant differences in treatment-related adverse effects outcomes were observed for either dosage (RD=0.09; 95% CI: -0.05 to 0.24; p=0.20; RD=0.09; 95% CI: -0.11 to 0.29; p=0.39). Despite the positive effects observed for the studied outcomes, the exact significance remains unclear.

ABSTRACT

ABSTRACT Chronic thromboembolic pulmonary hypertension (CTEPH) is a serious and debilitating disease caused by occlusion of the pulmonary arterial bed by hematic emboli and by the resulting fibrous material. Such occlusion increases vascular resistance and, consequently, the pressure in the region of the pulmonary artery, which is the definition of pulmonary hypertension. The increased load imposed on the right ventricle leads to its progressive dysfunction and, finally, to death. However, CTEPH has a highly significant feature that distinguishes it from other forms of pulmonary hypertension: the fact that it can be cured through treatment with pulmonary thromboendarterectomy. Therefore, the primary objective of the management of CTEPH should be the assessment of patient fitness for surgery at a referral center, given that not all patients are good candidates. For the patients who are not good candidates for pulmonary thromboendarterectomy, the viable therapeutic alternatives include pulmonary artery angioplasty and pharmacological treatment. In these recommendations, the pathophysiological bases for the onset of CTEPH, such as acute pulmonary embolism and the clinical condition of the patient, will be discussed, as will the diagnostic algorithm to be followed and the therapeutic alternatives currently available.


RESUMO A hipertensão pulmonar tromboembólica crônica (HPTEC) é uma doença grave e debilitante, causada pela oclusão do leito arterial pulmonar por êmbolos hemáticos e por material fibroso induzido pela presença desses êmbolos. Essa oclusão eleva a resistência vascular e, por consequência, a pressão do território arterial pulmonar, caracterizando a presença de hipertensão pulmonar. Esse aumento da carga imposta ao ventrículo direito leva a progressiva insuficiência do mesmo e, finalmente, ao óbito. No entanto, ao contrário das outras formas de hipertensão pulmonar, a HPTEC possui uma particularidade muito significativa: a existência de tratamento potencialmente curativo através da tromboendarterectomia pulmonar. Dessa forma, o objetivo primordial do manejo deve ser a avaliação do potencial cirúrgico do paciente em um centro de referência em HPTEC. Entretanto, nem todos os pacientes podem ser submetidos à cirurgia. Para esses pacientes outras alternativas terapêuticas viáveis são a angioplastia de artérias pulmonares e o tratamento farmacológico. Nestas recomendações, discutir-se-ão as bases fisiopatológicas para o surgimento de HPTEC, a partir da embolia pulmonar aguda, bem como o quadro clínico apresentado pelo paciente, o algoritmo diagnóstico a ser seguido e as alternativas terapêuticas disponíveis.

Abstract The finding of pulmonary hypertension (PH) by echocardiography is common and of concern. However, echocardiography is just a suggestive and non-diagnostic assessment of PH. When direct involvement of pulmonary circulation is suspected, invasive hemodynamic monitoring is recommended to establish the diagnosis. This assessent provides, in addition to the diagnostic confirmation, the correct identification of the vascular territory predominantly involved (arterial pulmonary or postcapillary). Treatment with specific medication for PH (phosphodiesterase type 5 inhibitors, endothelin receptor antagonists and prostacyclin analogues) has been proven effective in patients with pulmonary arterial hypertension, but its use in patients with PH due to left heart disease can even be damaging. In this review, we discuss the diagnosis criteria, how etiological investigation should be carried out, the clinical classification and, finally, the therapeutic recommendations for PH.


Resumo O achado de hipertensão pulmonar (HP) em avaliação ecocardiográfica é frequente e preocupante. No entanto, o ecocardiograma é apenas um exame sugestivo e não diagnóstico de HP. Quando se suspeita de acometimento direto da circulação pulmonar, está indicada medida hemodinâmica invasiva para estabelecer o diagnóstico. Essa avaliação permite, além da confirmação diagnóstica, a correta identificação do território vascular predominantemente acometido (arterial pulmonar ou pós-capilar). O tratamento com as medicações específicas de HP (inibidores da fosfodiestarese 5, antagonistas do receptor de endotelina, análogos da prostaciclina e estimulador da guanilil ciclase solúvel) é comprovadamente eficaz para pacientes com hipertensão arterial pulmonar, mas seu uso em pacientes com HP decorrente de doença cardíaca de câmaras esquerdas pode até mesmo ser prejudicial. Discutiremos nesta revisão o critério diagnóstico, a maneira de proceder a investigação etiológica, a classificação clínica e, finalmente, as recomendações terapêuticas na HP.

ABSTRACT

Worldwide, venous thromboembolism (VTE) is among the leading causes of death from cardiovascular disease, surpassed only by acute myocardial infarction and stroke. The spectrum of VTE presentations ranges, by degree of severity, from deep vein thrombosis to acute pulmonary thromboembolism. Treatment is based on full anticoagulation of the patients. For many decades, it has been known that anticoagulation directly affects the mortality associated with VTE. Until the beginning of this century, anticoagulant therapy was based on the use of unfractionated or low-molecular-weight heparin and vitamin K antagonists, warfarin in particular. Over the past decades, new classes of anticoagulants have been developed, such as factor Xa inhibitors and direct thrombin inhibitors, which significantly changed the therapeutic arsenal against VTE, due to their efficacy and safety when compared with the conventional treatment. The focus of this review was on evaluating the role of these new anticoagulants in this clinical context.


O tromboembolismo venoso (TEV) está entre as principais causas de morte por doenças cardiovasculares no mundo, atrás apenas do infarto agudo do miocárdio e do acidente vascular cerebral. O TEV possui espectro de apresentação que vai desde a trombose venosa profunda até o tromboembolismo pulmonar agudo, de acordo com gravidade crescente de acometimento, sendo seu tratamento baseado na anticoagulação plena dos pacientes. Há muitas décadas, sabe-se que a anticoagulação interfere diretamente na mortalidade associada ao TEV. Até o início deste século a terapia anticoagulante se baseava no uso de heparina, em suas formas não fracionada ou de baixo peso molecular, e de antagonistas da vitamina K, principalmente a varfarina. Ao longo das últimas décadas, foram desenvolvidos novas classes de medicamentos anticoagulantes, inibidores do fator Xa e inibidores diretos da trombina, que mudaram significativamente o arsenal terapêutico do TEV, em função de suas características de eficácia e segurança em relação ao tratamento convencional, sendo o foco principal de esta revisão avaliar seu papel neste contexto clínico.

ABSTRACT

Seedlings of high quality are essential for the deployment of homogeneous plants. Thus, was evaluated the formation of seedlings of Jatropha (Jatropha curcas L.) in different substrates fertilized with fertilizer-based lithothamnium. It was used substrates I) mixed of: never cultivated soil, cattle manure and sand in proportions: 3:1:1 volume/volume (v/v); II) commercial substrate Tropstrato®; III) cattle manure and carbonized rice straw in proportions: 1:1 (v/v); IV) birds manure, carbonized rice straw and never cultivated soil in proportions: 1:1:4, and V) fine vermiculite. Lithothamnium fertilizer levels used were 0%, 5% and 10% (v/v), respectively. The experimental design used was randomize blocks with factorial of 5x3 (five different substrates and three different lithothamnium doses) and four replication. Lithothamnium doses and substrates influenced the seedling growth of Jatropha. Seedlings fertilized with the substrates soil with mineral fertilizers (substrates 1); compost with manure and carbonized rice hulls (Substrate 3); soil with hatchery compost (Substrate 4) fertilized with a dose of 6.8% (v/v) lithothamnium showed the best result.


A qualidade da muda é fundamental para implantação de lavouras homogêneas. Desta forma, avaliou-se o crescimento de mudas de pinhão manso (Jatropha curcas L.,), em diferentes substratos adubados com fertilizante à base de lithothamnium. Foram utilizados os substratos: I) terra de barranco, esterco bovino curtido e areia nas proporções 3:1:1 volume/volume (v/v), respectivamente; II) substrato comercial denominado Tropstrato®; III) esterco de curral e palha de arroz carbonizada na proporção de 1:1 (v/v); IV) compostagem de incubatório de aves, palha de arroz carbonizada e terra de barranco na proporção 1:1:4; e V) vermiculita pura de granulação fina. As doses do fertilizante à base de lithothamnium utilizadas foram: 0%, 5% e 10% (v/v), respectivamente. O desenho experimental foi o delineamento em blocos casualisados, em esquema bifatorial 5x3 (cinco substratos e três doses de fertilizante) e quatro repetições. Cada unidade experimental (parcela) foi composta por doze mudas. As doses de lithothamnium e os substratos influenciaram o crescimento de mudas de pinhão manso. As mudas produzidas com os substratos composto por terra de barranco, esterco bovino curtido e areia; esterco de curral e palha de arroz carbonizada e Compostagem de incubatório de aves, palha de arroz carbonizada e terra de barranco adubadas com a dose média de 6,8% (v/v) de lithothamnium, apresentaram o melhor resultado.

Leptospirosis is a waterborne disease and, therefore, stands out for the possibility of environmental contamination, the cross transmission between domestic and wild animals and humans. Opossum species are important reservoirs of this disease making them potential pathogen spreaders. Aiming to verify the presence of Leptospira spp. and the antibodies against Leptospira spp. in the Campus of São Paulo State University, in Jaboticabal, São Paulo, Brazil, freeliving wild life opossum (Didelphis albiventris) were captured for blood and urine sampling. Serological analysis was performed Microscopic Agglutination Test (MAT). Aliquots of urine were seeded in media Ellinghausen-McCullough- Johnson-Harris (EMJH) and Fletcher without antibiotics. The samples in which there was growth of leptospires were forwarded to the Leptospirosis Laboratory of the Institute of Pathobiology in the National Institute of Agricultural Technology, Buenos Aires, Argentina and were genotyped using Multiple Locus Variable number tandem repeat Analysis (MLVA). Of the 15 analyzed animals, nine (60.0%) were reactant to Patoc serovar. The pathogenic specie Leptospira borgpetersenii was isolated and identified in three Didelphis albiventris. The isolation findings of pathogenic specie Leptopsira borgpetersenii in the urine culture of three Didelphis albiventris in a university campus are a major discovery in the area of preventive veterinary medicine and public health and open a discussion about the important role of free-living wild animals as reservoirs of this agent to domestic animals and humans, a condition that serves as a warning for the improvement of health practices...


A leptospirose é uma zoonose de veiculação hídrica e, portanto, se destaca pela possibilidade de contaminação ambiental, o que facilita a transmissão cruzada entre animais domésticos, selvagens e humanos. Espécies de gambás são importantes reservatórios dessa enfermidade, tornando-os potenciais disseminadores do agente. Com o objetivo de verificar a presença de Leptospira spp. e de anticorpos contra Leptospira spp. no Campus da Universidade Estadual Paulista, em Jaboticabal, foram capturados gambás (Didelphis albiventris) de vida livre para a colheita de amostras de sangue e de urina. As análises sorológicas foram efetuadas pela técnica de Soroaglutinação Microscópica (SAM). Alíquotas de urina foram semeadas nos meios Ellinghausen-McCullough-Johnson-Harris (EMJH) e Fletcher sem antibióticos. As amostras que apresentaram crescimento de espiroquetas foram levadas ao Laboratório de Leptospirose do Instituto de Patobiologia, no Instituto Nacional de Tecnologia Agropecuaria, Buenos Aires, Argentina e foram genotipadas com a técnica de Múltiplos Locus de Números Variáveis de Repetição em Tandem (MLVA). Dos 15 animais examinados pela SAM, nove (60,0%) foram reagentes à sorovariedade Patoc. Foi isolada e identificada a espécie patogênica Leptospira bosrpetersenii de três Didelphis albiventris. Os achados de isolamento da espécie patogênica Leptospira borgpetersenii na cultura de urina de três Didelphis albiventris são um grande descobrimento para as áreas da medicina veterinária preventiva e da saúde pública e reforçam a discussão sobre o importante papel dos animais selvagens de vida livre como reservatórios desse agente para animais domésticos e seres humanos, situação que serve de alerta para melhorias nas práticas sanitárias...

ABSTRACT

Introdução: O Doppler tecidual (DT) evoluiu para a determinação da deformação miocárdica regional pela medida do strain rate (SR) e strain (ε) longitudinal (L) e radial (R) unidimensionais. Objetivos: Determinar padrões de normalidade em neonatos nas primeiras 24 horas de vida e estabelecer valores de referência. Casuística e Método: Foram selecionados 55 neonatos, com idade média de 20,14 ± 14,0 horas. Por meio dos cortes apical e paraesternal, em três ciclos cardíacos consecutivos, digitalmente obtidos com taxas de 300 ± 50 ciclos/s, foram realizadas medidas dos componentes sistólico, diastólico inicial e final das curvas de SR/ε, utilizando programa específico. Comparações múltiplas entre paredes e segmentos foram feitas. Resultados: Verificou-se diferença significativa na deformação longitudinal entre as medidas do SR/ε sistólicos do segmento basal da parede septal, em relação à apical (-1,89 ± 0,60, -25,86 ± 4,83 versus -1,66 ± 0,22, -24,23 ± 3,48), P= 0,04 e P= 0,02. A deformação longitudinal regional do VD também não foi homogênea, com diferenças significantes entre os segmentos basal e apical. Os valores absolutos de SR/ε e todos os seus componentes foram maiores na direção R, quando comparados com a L (SR sistólico 2,98 ± 0,78 s-1 versus (-)1,89 ± 0,60 s-1ε sistólico 53,8% ± 19% versus (-) 24,8% ± 3% P< 0,01). A variabilidade interobservador da medida do SR/ε foi de 0,7% e 1,2%, respectivamente. Conclusão: Os índices regionais de deformação miocárdica baseados no Doppler (SR/ε) constituem técnica clínica reproduzível em neonatos, permitindo a análise da função regional em diferentes segmentos do VE e VD. Representam parâmetros novos, não invasivos e independentes da interpretação visual, podendo ser utilizados no diagnóstico de acometimento cardíaco do neonato.

ABSTRACT

The liver of P. expansa was characterized morphohistologically. To this end, twenty livers from clinicallyhealthy male and female Podocnemis expansa, weighing from 2.0 to 4,5 kg, supplied by the commercial breederFazenda Moenda da Serra, in Araguapaz, state of Goiás, Brazil, were analyzed macro - and microscopically.The coelomatic cavity was opened and the topography of the fresh organs was examined visually. After thehistological preparation, the slides were stained with Hematoxylin and Eosin (HE), Periodic Acid-Schiff(PAS), Gomori Trichrome, Reticulin and Picrosirius. The liver of P. expansa is a voluminous organ with anapproximately rectangular shape and brown coloration, varying from light to dark shades, and is divided into aright lobe, left lobe, and a central portion. The right lobe is the largest of the three portions. The gall bladderis located in a depression in the caudal portion of the right lobe, where the gall duct begins and emptiesinto the duodenum. Histologically, the hepatocytes are arranged in the form of double cords surroundedby winding sinusoidal capillaries. In cross section, they resemble acini containing approximately two to fivehepatocytes surrounding a probable central biliary canaliculus. The hepatocytes are polyhedral or pyramidalin shape, of uniform size, with a few central nuclei and others displaced peripherally, and the cytoplasm islittle eosinophilic when analyzed by the HE staining technique. The parenchyma is supported by delicatereticular fibers surrounding hepatocytes and sinusoids. The parenchyma and perisinusoidal spaces containlarge quantities of melanomacrophages, mainly close to the portal spaces.

ABSTRACT

PURPOSE: To evaluate the effects of different concentrations of an anesthetic association in giant amazon turtles (Podocnemis expansa). METHODS: Twenty healthy P. expansa of both sexes weighing between 1.0 and 1.5kg commercially bred in the Araguaia River Valley, Goias, Brazil, were separated into two groups (G1 n=10 and G2 n=10). Each group received a respective protocol: P1= acepromazine (0.5 mg/kg IM) and propofol (5 mg/kg IV) and P2 = acepromazine (0.5 mg/kg IM) and propofol (10 mg/kg IV). The acepromazine was administered in the left thoracic member and the propofol in the cervical vertebral sinus. Assessments were made of the anesthetic parameters of locomotion, muscle relaxation, response to pain stimuli in the right thoracic and pelvic members and heartbeat. RESULTS: The anesthetic induction time was the same for both protocols (P1 and P2); however the P2 effects were of a longer duration. CONCLUSION: The sedation achieved with both protocols (P1 and P2) were satisfactory for the biological sample collection, physical examinations and minor surgeries on this species.


OBJETIVO: Avaliar os efeitos de uma associação anestésica com diferentes concentrações em tartarugas-da-amazônia (Podocnemis expansa). MÉTODOS: Vinte P. expansa, hígidas, de ambos os sexos, com massa corporal entre 1,0 e 1,5 kg, de um criatório comercial localizado no vale do rio Araguaia, Goiás, Brasil, foram distribuídas em dois grupos (G1 n=10 e G2 n=10). Cada grupo recebeu um protocolo sendo: P1 = acepromazina (0,5 mg/kg IM) e propofol (5 mg/kg IV) e P2 = acepromazina (0,5 mg/kg IM) e propofol (10 mg/kg IV), aplicados nos grupos G1 e G2, respectivamente. A acepromazina foi aplicada no membro torácico esquerdo e o propofol no seio vertebral cervical. Foram avaliados os parâmetros anestésicos: locomoção, relaxamento muscular, resposta aos estímulos dolorosos no membro torácico direito e nos membros pelvinos e frequência cardíaca. RESULTADOS: O tempo de indução anestésica foi o mesmo para ambos os protocolos (P1 e P2), porém o P2 apresentou efeitos mais duradouros. CONCLUSÃO: As sedações obtidas por esses protocolos (P1 e P2) foram satisfatórias para a colheita de amostras biológicas, exames físicos e realização de pequenos procedimentos cirúrgicos nesta espécie.

ABSTRACT

PURPOSE: Evaluate the effects of two anesthetic associations in giant Amazon river turtles (P. expansa). METHODS: Twenty P. expansa, healthy, of both sexes, with weights between 1.0 and 1.5 kg of a commercial breeding facility located in the valley of the Araguaia River, Goiás, Brazil, were divided into two groups ( G1 n = 10 and G2 n = 10). Each group received a protocol being: P1 = midazolam (2 mg/kg IM) and ketamine (20 mg/kg IM) and P2 = midazolam (2 mg/kg IM) and ketamine (60 mg/kg IM), applied on G1 and G2, respectively. The drugs were applied in the left forelimb. The clinical parameters evaluated were: locomotion, muscle relaxation, response to pain stimuli in the right thoracic and pelvic members and heart rate. These assessments were made at time 0 (immediately after injection) and times of 5, 10, 20, 30, 45, 60, 90, 120, 150 and 180 minutes after the injections. RESULTS: Group 2 showed a higher heart rate than G1 and more rapid and prolonged immobilization. CONCLUSION: The sedation scores obtained by these protocols (P1 and P2) were satisfactory, with possible pharmacological contention for collecting biological samples and physical examination in P. expansa.


OBJETIVO: Avaliar os efeitos de duas associações anestésicas em tartarugas da Amazônia em (Podocnemis expansa). MÉTODOS: Vinte P. expansa, hígidas, de ambos os sexos, com massa corporal entre 1,0 e 1,5 kg, de um criatório comercial localizado no vale do rio Araguaia, Goiás, Brasil, foram distribuídas em dois grupos (G1 n=10 e G2 n=10). Cada grupo recebeu um protocolo sendo: P1 = midazolam (2 mg/kg IM) com cetamina (20 mg/kg IM) e P2 = midazolam (2 mg/kg IM) com cetamina (60 mg/kg IM), aplicados nos grupos G1 e G2, respectivamente. Os fármacos foram aplicados no membro torácico esquerdo. Os parâmetros clínicos avaliados foram: locomoção, relaxamento muscular, resposta aos estímulos dolorosos nos membros torácico direito e pelvinos e freqüência cardíaca. Essas avaliações foram feitas no tempo 0 (imediatamente após a injeção) e nos tempos 5, 10, 20, 30, 45, 60, 90, 120, 150 e 180 minutos após as injeções. RESULTADOS: O G2 apresentou maior freqüência cardíaca que o G1 e imobilização mais rápida e prolongada. CONCLUSÃO: As sedações obtidas por esses protocolos (P1 e P2) foram satisfatórias, sendo possível a contenção farmacológica para a coleta de amostras biológicas e exame físico em P. expansa.

ABSTRACT

PURPOSE: To determine whether rocuronium would provide safe, short-term immobilization in Podocnemis expansa. METHODS: Twenty P. expansa, weighing on average 1.59 ± 0.28 kg, were subjected to two protocols: G1 0.25 mg/kg IM of rocuronium and 0.07 mg/kg IM of neostigmine, while G2 received 0.50 mg/kg IM of rocuronium and 0.07 mg/kg IM of neostigmine. The drugs were applied, respectively, in the left and right thoracic members. Assessments were made of the anesthetic parameters of respiratory frequency, heartbeat, righting reflex, cloacal relaxation, palpebral and pupilar reflexes, easy handling, muscle relaxation, locomotion, response to pain stimuli in the right thoracic members, pelvic members and tail, ambient humidity and temperature. RESULTS: They were not found statistical differences between the dosages for the majority of the assessments. G1 was as efficient as G2. A consistent neuromuscular blockade effect was recorded 12 ± 4.21 minutes in G1 and G2. All the animals were recovered in 150 minutes. CONCLUSIONS: Administration of rocuronium at dose of 0.25 to 0.5 mg/kg IM is a safe and effective adjunct to clinical proceedings or pre-anesthetics in P. expansa. Because rocuronium does not provide any analgesic or sedative effects, the duration of neuromuscular blockade without anesthesia should be minimized to avoid undue stress.


OBJETIVO: Determinar se o rocurônio promove imobilização segura e de curta duração em Podocnemis expansa. MÉTODOS: Vinte P. expansa com média de peso 1,59 ± 0,28 kg, foram submetidas a dois protocolos: G1 recebeu rocurônio 0,25 mg/kg IM e neostigmina 0,07 mg/kg IM enquanto G2 rocurônio 0,50 mg/kg IM e neostigmina 0,07 mg/kg IM, aplicados no membro torácico esquerdo e direito, respectivamente. Observaram-se os parâmetros anestésicos: freqüência respiratória e cardíaca, reflexo de endireitamento, relaxamento do esfíncter da cloaca, reflexo palpebral e pupilar, facilidade de manipulação, relaxamento muscular, locomoção, resposta aos estímulos dolorosos no membro torácico direito, nos membros pelvinos e na cauda, temperatura e umidade ambiental. RESULTADOS: Não foram encontradas diferenças estatísticas entre as doses para a maioria dos parâmetros e o G1 foi tão eficiente quanto o G2. Um bloqueio neuromuscular consistente foi observado aos 12 ± 4,21 minutos tanto no G1 como no G2. A recuperação de todos os animais ocorreu em até 150 minutos. CONCLUSÕES: Administração de rocurônio nas doses 0,25 e 0,50 mg/kg IM é segura e efetiva para os procedimentos clínicos ou pré-anestésicos em P. expansa. Como o rocurônio não produz efeitos sedativos ou analgésicos, a duração do bloqueio neuromuscular sem anestesia deverá ser minimizado para evitar estresse.

ABSTRACT

PURPOSE: Identify a technique to induce brief sedation and hypnosis in Podocnemis expansa. METHODS: Twenty commercially bred P. expansa, weighing on average 1.2 ± 0.24 kg, were subjected to two protocols: G1 was given 1.5 mg/kg IM of xylazine and 5 mg/kg IV of propofol, while G2 received 1.5 mg/kg IM of xylazine and 10 mg/kg IV of propofol. The drugs were applied, respectively, in the left thoracic member and in the cervical vertebral sinus. Assessments were made of the anesthetic parameters of locomotion, muscle relaxation, response to pain stimuli in the right thoracic members, pelvic members and tail, easy handling and heartbeat, as well as ambient temperature and glycemic level. RESULTS: A consistent hypnotic effect was recorded 49.6 ± 22.1 seconds in G2 and after 58.2 ± 55.1 in G1. All the animals of G2 recovered in 198 minutes, and those of G1 in 156 minutes. CONCLUSION: The hypnosis achieved with these associations was satisfactory, and G1 was as efficient as G2, allowing for the pharmacological restraint for the collection of biological samples, physical examinations and minor surgeries on these species.


OBJETIVO: Identificar uma técnica para se induzir sedação e hipnose em Podocnemis expansa. MÉTODOS: Vinte Podocnemis expansa de criatório comercial, com média de peso 1,2 ± 0,24 kg, foram submetidas a dois protocolos: G1 recebeu xilazina 1,5 mg/kg IM e propofol 5 mg/kg IV e o G2 xilazina 1,5 mg/kg IM e propofol 10 mg/kg IV. As drogas foram aplicadas no membro torácico esquerdo e no seio vertebral cervical, respectivamente. Observaram-se os parâmetros anestésicos: locomoção, relaxamento muscular, resposta aos estímulos dolorosos no membro torácico direito, nos membros pelvinos e na cauda, facilidade de manipulação e freqüência cardíaca, além da temperatura ambiental e glicemia. RESULTADOS: Um efeito hipnótico consistente foi observado aos 49,6 ± 22,1 segundos no G2 e 58,2 ± 55,1 segundos no G1. A recuperação de todos os animais do G2 ocorreu em 198 minutos, e em 156 minutos no G1. CONCLUSÃO: A hipnose obtida por essas associações foi satisfatória e o Grupo 1 foi tão eficiente quanto o Grupo 2, o que possibilita a contenção farmacológica para coleta de amostras biológicas, exames físicos e realização de pequenas cirurgias nesta espécie.

Objetivo: estabelecer o perfil epidemiológico dos pacientes submetidos à biópsia renal na fundação.


Objective: to establish the epidemiologic profile of Fundação Hospital de Clínicas Gaspar Vianna (FHCGV) patients submitted to renal biopsy and identify the most common kidney diseases biopsed and related clinical syndromes, formulating clinical and pathologic correlations. Methods: a retrospective study was undertaken of ali renal biopsy registries performed from 2002 to 2005 at Nephrology Service of FHCGV. Data were obtained from Kidney Biopsies Registry Book, which contained personal, clinical and histological data of 32 patients, of both sexes. Results: adults composed most part of the sample (94%) and 56% of ali patients were male. Only three subjects IInderwellt biopsies complicationsj. The most common clinical syndrome was nephritic syndrome (72%), followed by hematuria (25%). Focal segmental glomerulosclerosis (FSGS) (22%) and lupus nephritis (15%) were associated to nephrotic syndrome in most of cases. Conclusion: nephrotic syndrome and FSGS were the most frequent clinical and histological findings at FHCGV, respectively.

Mucocele is an unusual lesion of the appendix characterized by accumulation of mucoid substance inside the appendix, which occurs as a sequel of luminal obstruction, usually by fibrous tissue. Diagnosis is intraoperative in most of the cases. We discuss the clinical, anatomicopathological and therapeutics aspects of the disease.
