ABSTRACT

High recurrence rate and metastasis rate after primary surgery for hepatocellular carcinoma have been restricting the long-term survival of patients. For the treatment of recurrent hepatocellular carcinoma, comprehensive treatment based on surgery is the main strategy. For patients who are unsuitable for surgery, local ablation,TACE,radiotherapy, systemic chemotherapy, targeted therapy, immunotherapy, traditional Chinese medicine and optimal supportive therapy are effective measures, which can delay disease progression and improve patient survival. According to the patient's characteristics and individual differences, the multidisciplinary treatment model should be adopted to optimize the treatment effect and reduce side effects, which will benefit the patients with better medical experience and longer overall survival rate.

ABSTRACT

OBJECTIVE: To study the protective action on endotheliocytes of fucoidan with different molecular weight from Laminaria japonica, and to explore its mechanism. METHODS: In vivo, endotheliocytes injury model in rats was used to examine the protective action on endotheliocytes of fucoidan with different molecular weight from Laminaria japonica; aortal endothelium section by HE stain under optical microscope was observed, and the the content of von willebrand factor (vWF) in plasma was determined by ELISA. In vitro, endothelial cell culture was used to determine the effect of fucoidan on content of vWF and endothelial microparticles (EMPs). RESULTS: In vivo, aortal endothelium section by HE stain under optical microscope showed low molecular weight (LMW) fucoidan had a remarkable protective effect, and the protection effect of LMWF fucoidans are stronger than middle molecular weight (MMW) fu-coidans. Administration of LMW fucoidan significantly decreased the vWF content. Administration of MMW fucoidan didn't change vWF content. In vitro, Administration of LMW fucoidan significantly decreased the vWF content and EMPs numbers. Administration of MMW fucoidan didn't change the vWF content and EMPs numbers. CONCLUSION: The LMW fucoidan fractions have good protective action on endothelial cells. The protective effect of LMW fucoidans is significantly stronger than MMW fucoidans. The protective action on endotheliocytes of LMW fucoidans relies on decreasing the concentration of EMPs and vWF, and thus inhibiting platelet activation indirectly.

ABSTRACT

OBJECTIVE: To study the antithrombotic activity of fucoidan with different molecular weight from Laminaria japonica, and to explore its mechanism by observing antiplatelet activities and fibrinolysis. METHODS: FeCl3-induced arterial thrombosis in rats were used to examine the antithrombotic activity of four fucoidan fractions with different molecular weight; the TXB2, 6-keto-PGF1α, PLG, t-PA and PAI-1 in plasma were determined by ELISA to evaluate the inhibitory effects of the drugs on platelet aggregation and fibrinolytic system. RESULTS: Four fucoidan fractions with different molecular weight could significantly decreased the weight of the thrombus, and Middle molecular weight (MMW) fucoidan fractions exhibited better antithrombotic activity than low molecular weight fucoidan (LMW). Administration of LMW fucoidan significantly decreased the TXB2 content and promoted 6-keto-PGF1α content. Administration of MMW fucoidan only decreased the TXB2 content and didn't change 6-keto-PGF1α content. In four fucoidan fractions group, the activity of t-PA were increased, the activity of PAI-1 were decreased, the ratio of t-PA/PAI-1 was increased, and the activity of PLG had no change. CONCLUSION: Four fucoidan fractions with different molecular weight have strong anti-thrombosis effect, the action mechanism may have correlation with the increase in the activities of anti-platelet aggregation and fibrinolysis system. ABSTRACT

BACKGROUND:The previous researches indicate that, shock waves can enhance the proliferation of T-cells and the expression of interleukin (IL)-2 through a mechanism that involves p38 mitogen activated protein kinase (MAPK)activation.OBJECTIVE: To investigate if adenosine triphosphate (ATP) release is an underlying mechanism through which low-density shock waves (LDSWs) augment T-cell function.DESIGN: Controlled repetitive measurement by groups, taking cells as subject.SETTING: Department of Orthopedics, the First Hospital of Jilin University.MATERIALS: KDE-2001 Extracorporeal Shockwave Lithotripter (Beijing Zhongke Jian An Meditechs Co., Beijing, China).p38 MAPK inhibitor SB203580 1 mg (BioSource Inc., Camarillo, CA); p38 MAPK kit for detecting phosphorylation (Cell Signaling Technology, Inc. U.S.A.); P2 receptor inhibitor suramin 50 mg (BIOMOL Research Laboratories Inc., PA) was prepared into 0.02 mol/L solution by 1.749 2 mL IMDM. ATP enzyme: apyrase 200 U (Sigma, U.S.A.); P2X7 receptor antagonist KN-62 (BioSource Inc., Camarillo, CA); ATP Bioluminescence Assay Kit CLS Ⅱ (Roche Diagnostics GmbH,Mannheim, Germany).METHODS: The experiment was carried out in the Orthopedic Laboratory of the First Clinical Hospital in Jilin University from January 2005 to December 2006. ①An Extracorporeal Shockwave Lithotripter (at 7 kV generator voltage, 0.3 μF capacitance, 23 MPa positive pressure, 0.18 mJ/mm2 energy flux density) was applied for LDSWs treatment ranging from 50 to 400 impulses. ②ATP release into the culture supernatant from Jurkat T-cells or human peripheral blood mononuclear cells (PBMCs) was determined with a specific ATP Bioluminescence Assay Kit. ③Negative control group excluded antagonist or inhibitor. Human PBMCs were used to determine the effect of LDSWs on activated T-lymphocyte proliferation. Human Jurkat cells were used to study the effects of LDSWs on IL-2 expression. Expression and phosphorylation of p38 MAPK in Jurkat T-cell were measured by Western Immunoblotting with anti-p38 MAPK antibodies and anti-p38 MAPK phospho-specific antibodies that recognized the phosphorylation (on Thr180/Tyr182).MAIN OUTCOME MEASURES: extra-cellular ATP release, IL-2 expression in cell suspension, cellular proliferation and the phosphorylation of p38 MAPK.RESULTS: ①ATP release under the condition without LDSWs was obviously lower than that with LDSWs of 100, 150,200, 250, 300, 360 and 400 impulses (P < 0.01), and ATP release increased with the LDSWs impulse.②Compared with negative control group, the additions of apyrase, KN-62 or suramin attenuated the 3H-TdR incorporation of the phytohemagglutinin-stimulated PBMCs or CD3/CD28-stimulated Jurkat T-cells, which were effected with LDSWs of 100,150, 200, 250, 300, 330 impulses at 0.18 mJ/mm2 (P< 0.01). IL-2 expression in the cellular supernatant was also significant increased (P < 0.01). ATPase, KN-62 or suramin all decreased the effect of LDSWs on p38 MAPK of Jurkat T-cells.CONCLUSION: ①LDSWs deform cellular membranes but have no effect on organelle, which results in ATP release from Jurkat cells. Exogenous ATP release activates P2X7 receptor and p38 MAPK, and increases IL-2 expression. LDSWs enhance T-cell proliferation and IL-2 expression through a mechanism that involves ATP release, P2X7 receptor and phosphorylized p38 MAPK activation. ②The release of ATP plays a key role in the mechanism through which LDSWs regulate the function of T cells.

ABSTRACT

BACKGROUND: The metabolic syndrome is a cluster of related cardiovascular risk factors and it is the cause of morbidity and mortality in cardiovascular diseases. Recently, new diagnostic criteria of glucose metabolism impairment has been recommended. The purpose of this study was to estimate the difference of cardiovascular risk by investigating the prevalence of metabolic syndrome according to the degree of glucose metabolism impairment. METHODS: A population of 757 subjects was selected from a database of individuals who visited a health promotion center. We classified these subjects into 5 groups [Normal, Isolated impaired glucose tolerance (I-IGT), Isolated impaired fasting glucose (I-IFG), combined IGT with IFG (IGT/IFG) and Diabetes]. We compared the general characteristics, Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) and the prevalence of metabolic syndrome in these groups. RESULTS: HOMA-IR and the prevalence of metabolic syndrome in the IGT/IFG and the Diabetes group were significantly greater than the Normal group. HOMA-IR and the prevalence of metabolic syndrome of the I-IGT and the I-IFG group were not significantly different with the Normal group. CONCLUSION: The insulin resistance and the prevalence of metabolic syndrome in the IGT/IFG group was significantly greater than the Normal group, and its presence may increase the risk of cardiovascular diseases. Therefore, it is important to control other combined metabolic disorders to prevent cardiovascular events after effective selection for IGT/ IFG.

ABSTRACT

The cruciate ligaments are important in providing the anterior and posterior stability and usually associated with other ligament injury of the knee joint, and there are many controversies in its treatment. In general, the primary repair for the acute cruciate ligament injury was good, but some had opposite opinions in the treatment of the isolated anterior cruciate ligament injury. In acute injury, early repair of ligament and in delayed case, the reconstrution surgery of ligament was recommended. For good results, it is very important to give prompt management. Authors studies 23 cases in 16 patients with acute cruciate ligament proper injury who were treated by multiple pull out technique at the department of orthopaedic surgery, Chungnam National University Hospital from April, 1984 to July, 1988. The results were as followings: 1. The incidence was high in active male patients. 2. The most common injury was the traffic and automobile accident. 3. the tibial and femoral attachment were the most common detachment site in anterior and posterior cruciate ligament injury, respectively. 4. Isolated cruciate ligament injury was rare. 5. The results were good in 11 cases(47.8%), especially treated within one week.

ABSTRACT

Reconstructjon of old anterior cruciate ligament injury was divided into extraarticular and intraarticular group. The intraarticular ligament reconstruction was a reliable method to correct the rotational axis of knee joint instability. Authors studied 10 cases in 10 patients who were treated by reconstruction of anterior cruciate ligament using patellar tendon under arthroscopy from January, 1988 to June, 1989. The follow up period was over 10 months. 7 cases were negative in Lachman test and 2 cases had mild instability of knee joint. The postoperative result was excellent in 3 cases, good in 4 cases, fair in 1 case, and failure in 1 case according to Clancy's evaluation. We concluded that the reconstruction of anterior cruciate ligament injury using patellar tendon under arthroscopy was an effective method in the treatment of old anterior knee instability.
