ABSTRACT Objective: To define a reference chart comparing pressure drop vs. flow generated by a set of arterial cannulae currently utilized in cardiopulmonary bypass conditions in pediatric surgery. Methods: Cannulae from two manufacturers were selected considering their design and outer and inner diameters. Cannula performance was evaluated in terms of pressure drop vs. flow during simulated cardiopulmonary bypass conditions. The experimental circuits consisted of a Jostra HL-20 roller pump, a Quadrox-i pediatric oxygenator (Maquet Cardiopulmonary AG, Rastatt, Germany), and a custom pediatric tubing set. The circuit was primed with lactated Ringer's solution only (first condition) and with human packed red blood cells added (second condition) to achieve a hematocrit of 30%. Cannula sizes 8 to 16 Fr were inserted into the cardiopulmonary bypass circuit with a "Y" connector. The flow was adjusted in 100 ml/min increments within typical flow ranges for each cannula. Pre-cannula and post-cannula pressures were measured to calculate the pressure drop. Results: Utilizing a pressure drop limit of 100 mmHg, our results suggest a recommended flow limit of 500, 900, 1400, 2600, and 3100 mL/min for Braile arterial cannulae sizes 8, 10, 12, 14, and 16 Fr, respectively. For Medtronic DLP arterial cannulae sizes 8, 10, 12, 14, and 16 Fr, the recommended flow limit is 600, 1100, 1700, 2700, and 3300 mL/min, respectively. Conclusion: This study reinforces discrepancies in pressure drop between cannulae of the same diameter supplied by different manufacturers and the importance of independent translational research to evaluate components' performance.

SUMMARY OBJECTIVE: The aim of this study was to evaluate the relationship between the range of motion and lower-limb hemodynamic indices in the tibiotarsal joint of individuals with diabetic neuropathy. METHODS: Twenty volunteers of both sexes, with a mean age of 61.45±7.05 years, were diagnosed with type 2 diabetes mellitus and diabetic peripheral neuropathy. Arterial blood flow was assessed using Doppler ultrasound, and the variables such as average velocity, pulsatility index, and resistivity index were also evaluated. A range of dorsiflexion and plantar flexion joint movements were assessed using digital goniometry before and after exercise. Data distribution was assessed using the Shapiro-Wilk test, followed by Pearson's correlation for normal data and Spearman's correlation for non-normal data, in order to verify the association between variables. RESULTS: A moderate correlation was found between dorsiflexion and pulse rate on two occasions before (rs=0.497) and after initial evaluation (rs=0.511). A low correlation was found between plantar flexion and mean velocity (rs=-0.357), pulsatility index (rs=0.439), and resistivity index (rs=0.328); dorsiflexion and mean velocity (rs=0.374), pulse rate (rs=0.332), and resistance index (rs=0.327) before evaluation, and peak (rs=0.346) was observed after the evaluation of blood circulation. CONCLUSION: There is a correlation between the range of motion of the tibiotarsal joint and the blood circulation of diabetics, ranging from moderate to poor for the different variables evaluated.

ABSTRACT

Abstract Dialysis has been widely used in the treatment of patients with chronic kidney diseases and is considered a global public health issue. This treatment, which has changed the prognosis and quality of life in patients with chronic renal failure, can lead to complications that are often fatal. For this reason, there is a need for validation of alternative tests that favor the monitoring of treated water for dialysis in real-time to promote and prevent injuries to patients submitted to this procedure.

ABSTRACT

Chromosome 5p13 duplication syndrome represents a contiguous gene syndrome involving duplication of several genes on chromosome 5p13. Some clinical phenotypes are related to it, such as: obsessive-compulsive behavior, small palpebral fissures, intellectual disability, global development delay and ocular hypertelorism. The exact mechanism behind these changes has not well known and further studies are needed for this purpose. Since it is a rare and uncommon clinical situation, the case report contributes to the knowledge of the disease and early diagnosis. This condition mainly affects the cognitive neuromuscular system. We describe an 8-year-old Brazilian patient with the duplication of chromosome 5p13.2, karyotype, whose neurodevelopmental evaluation presented cognitive impairment, severe language delay and atypical physical examination, with ocular hypertelorism, right auricular tags, congenital heart defect and long fingers. The patient was diagnosed by comparative genomic hybridization (CGH)-array revealing a 204Kb of DNA duplication. The exact mechanism behind these structural disorders is still unclear and further studies are needed for this purpose. Nevertheless, the diagnostic suspicion of this genetic alteration that, in general, presents late diagnosis, should be considered to enable better clinical support to the patients and family genetic counseling.


A síndrome da duplicação do cromossomo 5p13 representa uma síndrome genética contígua envolvendo a duplicação de vários genes contidos nesta região. Alguns fenótipos clínicos estão relacionados com ela, tais como: comportamento obsessivo compulsivo, fissuras palpebrais pequenas, déficit intelectual, atraso no desenvolvimento global e hipertelorismo ocular. Por ser uma situação clínica rara, o relato do caso contribui para a disseminação do conhecimento acerca da condição, assim como para seu diagnóstico precoce. Descrevemos uma paciente brasileira de oito anos com a duplicação do cromossomo 5p13.2, que na avaliação do neurodesenvolvimento apresentou comprometimento cognitivo, grave atraso da linguagem e dismorfismos como hipertelorismo ocular, apêndice auricular direito, sopro cardíaco, relacionado a defeito do septo ventricular, e dedos alongados. A paciente foi diagnosticada por meio da pesquisa molecular (CGH)-array com ganho de 204Kb de DNA. O mecanismo exato por trás dessas alterações estruturais ainda não está claro e são necessários mais estudos para este fim. Não obstante, a suspeita diagnóstica dessa alteração genética que, em geral, apresenta diagnóstico tardio, deve ser aventada para viabilizar melhor suporte clínico aos pacientes e aconselhamento genético familiar.

ABSTRACT

Abstract This patient was a 73-year-old man who initially came to our service with acute respiratory failure secondary to COVID-19. Soon after hospitalization, he was submitted to orotracheal intubation and placed in the prone position to improve hypoxia, due to severe acute respiratory syndrome (SARS). On the third day of hospitalization, he developed acute oliguric kidney injury and volume overload. The nephrology service was activated to obtain deep venous access for renal replacement therapy (RRT). The patient could not be placed in the supine position due to significant hypoxemia. A 50-cm Permcath (MAHURKARTM, Covidien, Massachusetts, USA) was inserted through the left popliteal vein. This case report describes a possible challenging scenario that the interventional nephrologist may encounter when dealing with patients with COVID-19 with respiratory impairment in the prone position.


Resumo O paciente era um homem de 73 anos de idade que inicialmente veio ao nosso serviço com insuficiência respiratória aguda secundária à COVID-19. Logo após a internação, ele foi submetido à intubação orotraqueal e pronado para melhorar a hipóxia devido à síndrome respiratória aguda grave (SARS - do inglês "severe acute respiratory syndrome"). No terceiro dia de internação, o mesmo desenvolveu lesão renal aguda oligúrica e sobrecarga de volume. O serviço de nefrologia foi acionado para realizar acesso venoso profundo para terapia renal substitutiva (TRS). O paciente não pôde ser colocado na posição de decúbito dorsal devido a uma hipoxemia significativa. Um Permcath de 50 cm (MAHURKARTM, Covidien, Massachusetts, EUA) foi inserido através da veia poplítea esquerda. Este relato de caso descreve um possível cenário desafiador com o qual o nefrologista intervencionista pode se deparar ao lidar com pacientes com COVID-19 com problemas respiratórios e colocados em pronação.

ABSTRACT

The objective of this work was to analyze the genetic diversity using conventional methods and artificial neural networks among 12 colored fiber cotton genotypes, using technological characteristics of the fiber and productivity in terms of cottonseed and cotton fiber yield. The experiment was conducted in an experimental area located at Fazenda Capim Branco, belonging to the Federal University of Uberlândia, in the city of Uberlândia, Minas Gerais. Twelve genotypes of colored fiber cotton were evaluated, 10 from the Cotton Genetic Improvement Program (PROMALG): UFUJP - 01, UFUJP - 02, UFUJP - 05, UFUJP - 08, UFUJP - 09, UFUJP - 10, UFUJP - 11, UFUJP - 13, UFUJP - 16, UFUJP - 17 and two commercial cultivars: BRS Rubi (RC) and BRS Topázio (TC). The experimental design used was complete randomized block (CRB) with three replications. The following evaluations were carried out at full maturation: yield of cottonseed (kg ha-1) and the technological characteristics, which include, fiber length, micronaire, maturation, length uniformity, short fiber index, elongation and strength, using the HVI (High volume instrument) device. Genetic dissimilarity was measured using the generalized Mahalanobis distance and after obtaining the dissimilarity matrix, the genotypes were grouped using a hierarchical clustering method (UPGMA). A discriminant analysis and the Kohonen Self-Organizing Map (SOM) by Artificial Neural Networks (ANN's) were performed through computational intelligence. SOM was able to detect differences and organize the similarities between accesses in a more coherent way, forming a larger number of groups, when compared to the method that uses the Mahalanobis matrix. It was also more accurate than the discriminant analysis, since it made it possible to differentiate groups more coherently when comparing their phenotypic behavior. The methods that use computational intelligence proved to be more efficient in detecting similarity, with Kohonen's Self-Organizing Map being the most adequate to classify and group cotton genotypes.

ABSTRACT Objective: to analyze the association between violence in childhood and increase in the Body Mass Index among adolescents. Method: a cross-sectional study, conducted with 136 adolescents aged from 10 to 19 years old, monitored by a Multiprofessional Residency Program in Adolescents' Health, in health units from the municipality of Divinópolis-MG. The anthropometric assessment was conducted from March to June 2018; and information was collected referring to the socioeconomic level, demographic factors, food consumption, and physical activity of the participants. The variable related to violence in childhood was composed of five groups, obtained from a factorial analysis. Multiple regression models were used to identify the variables associated with the increase in Body Mass Index, with a significance level of 5%. Results: the prevalence of excess weight was 31.8%. The adolescents participating in the study consumed soft drinks (66.2%) and industrialized food products (66.9%) every week and ate in front of the TV every day (54.4%). The most prevalent type of abuse was emotional neglect, which affected 100% of the sample under study. An association was evidenced of physical neglect in childhood and intake of industrialized food products with the increase in the Body Mass Index z-score. Conclusion: violence in childhood and the consumption of industrialized food products were associated to the increase in Body Mass Index among adolescents. Investments in public policies for comprehensive promotion of health and protection of children and adolescents are imperious.


RESUMEN Objetivo: analizar la asociación entre violencia sufrida en la infancia y aumento del Índice de Masa Corporal entre adolescentes. Método: estudio transversal, realizado con 136 adolescentes de 10 a 19 años de edad, monitoreados por un Programa de Residencia Multiprofesional de la Salud del Adolescente, en unidades de salud del municipio de Divinópolis-MG. La evaluación antropométrica se realizó entre los meses de marzo y junio de 2018, y se recolectaron datos referentes al nivel socioeconómico, factores demográficos, consumo de alimentos y actividad física de los participantes. La variable relacionada con la violencia sufrida en la infancia estuvo compuesta por cinco bloques, que se obtuvieron a partir de un análisis factorial. Se emplearon modelos de regresión lineal múltiple para identificar las variables asociadas al aumento del Índice de Masa Corporal, con un nivel de significancia del 5%. Resultados: la prevalencia de exceso de peso fue del 31,8%. Los adolescentes que participaron del estudo ingerían refrescos (66,2%) y alimentos industrializados (66,9%) semanalmente, además de comer frente al televisor a diario (54,4%). El tipo de abuso más prevalente fue la negligencia emocional, que afectó al 100% de la muestra estudiada. Se hizo evidente la asociación de negligencia física sufrida en la infancia y consumo de alimentos industrializados con el aumento de la puntuación z del Índice de Masa Corporal. Conclusão: la violencia sufrida en la infancia y el consumo de alimentos industrializados presentaron una asociación con el aumento del Índice de Masa Corporal entre adolescentes. Es imperioso invertir en políticas públicas para promover la salud integral y proteger a los niños y adolescentes.

A variável relativa à violência na infância foi composta por cinco blocos, obtidos a partir de uma análise fatorial. Modelos de regressão linear múltipla foram utilizados para identificação das variáveis associadas ao aumento do Índice de Massa Corporal, com um nível de significância de 5%. Resultados: a prevalência de excesso de peso foi de 31,8%. Os adolescentes do estudo ingeriam refrigerantes (66,2%) e alimentos industrializados (66,9%) semanalmente e se alimentavam em frente à TV diariamente (54,4%). O tipo de abuso mais prevalente foi a negligência emocional, acometendo 100% da amostra estudada. Evidenciou-se associação da negligência física na infância e do consumo de alimentos industrializados com o aumento do índice de escore z de Índice de Massa Corporal. Conclusão: A violência na infância e o consumo de alimentos industrializados associaram-se ao aumento de Índice de Massa Corporal entre adolescentes. Investimentos em políticas públicas para promoção da saúde integral e proteção de crianças e adolescentes são imperativos.

Abstract Aim: This study investigates the impact of social isolation, weight changes, and physical activity (PA) by the electronic tools (eTools) on the quality of life (QoL) of Brazilian medical students during the COVID-19 pandemic. Methods: In this observational longitudinal study, one hundred eighty-nine medical students completed an e-questionnaire, which used the WHOQOL-Bref instrument to report QoL and sociodemographic characteristics, health conditions, and lifestyles related to PA. Cronbach's alpha and Friedman/Dunn and Spearman's correlation tests were performed. Based on descriptive and inferential statistics, a multiple linear regression was used to evaluate the potential predictors of QoL. Results: The COVID-19 pandemic particularly affected the psychological and social domains. Time spent indoors and a non-ideal BMI had a negative impact on QoL. Being female and sexual minority-group membership were associated with lower QoL scores in the psychological and physical domains, respectively. Access to PA eTools was positively associated with QoL. Being monitored by a physical education teacher contributes to satisfactory PA levels during online classes. Conclusion: This study provides novel insights into the access of PA eTools on QoL during the COVID-19 pandemic. During periods of social distancing, people should pay extra attention to self-care strategies to protect mental health and promote positive lifestyle patterns. In addition, using online PA resources monitored by physical education teachers could be a strategy to maintain satisfactory levels of exercise.

ABSTRACT Objective This study aimed to establish the utility values of different health states associated with diabetic retinopathy in a Brazilian sample to provide input to model-based economic evaluations. Subjects and methods This cross-sectional study was performed in a sample of patients with type 2 diabetes mellitus (T2D) who underwent teleophthalmology screening at a primary care service from 2014 to 2016. Five diabetic retinopathy health states were defined: absent, non-sight-threatening, sight-threatening, and bilateral blindness. Utility values were estimated using the Brazilian EuroQol five dimensions (EQ-5D) tariffs. Descriptive statistics were calculated. Analysis of covariance was performed to adjust the utility values for potential confounders. Results The study included 206 patients. The mean (± standard deviation [SD]) utility value was 0.765 ± 0.19 (95% confidence interval [CI], 0.740-0.790). The adjusted mean utility value was 0.748 (95% CI, 0.698-0.798) in patients without diabetic retinopathy, 0.752 (95% CI, 0.679-0.825) in those with non-sight-threatening state, 0.628 (95% CI, 0.521-0.736) in those with sight-threatening state, and 0.355 (95% CI, 0.105-0.606) in those with bilateral blindness. A significant utility decrement was found between patients without diabetic retinopathy and those with a sight-threatening health state (0.748 vs. 0.628, respectively, p = 0.04). Conclusions The findings suggest that a later diabetic retinopathy health state is associated with a decrement in utility value compared with the absence of retinopathy in patients with T2D. The results may be useful as preliminary input to model-based economic evaluations. Further research is needed to investigate the impact of diabetic retinopathy on health-related quality of life in a sample more representative of the Brazilian population.

Justificativa e Objetivos: Apesar dos esforços despendidos pelo Programa Nacional de Imunizações, a coqueluche é um agravo que ainda gera grandes preocupações para a saúde pública. Assim, o objetivo deste estudo foi descrever as evidências na literatura sobre coqueluche na infância que indicassem as práticas de assistência à saúde necessárias para sua prevenção, promoção e controle. Método: Revisão integrativa da literatura nas bases de dados LILACS, MedLine, PubMed, SciELO, Embase, Cumulative Index to Nursing and Allied Health Literature (CINAHL) utilizando os descritores Whooping Cough, Epidemiology, Vaccination. A revisão foi realizada em maio de 2020 e abrangeu a literatura disponível na íntegra, publicada em português, inglês ou espanhol, sem delimitação de ano. Resultados: Dentre os 1.248 artigos selecionados apenas nove foram analisados (a maioria norte-americana), dos quais 55% eram estudos realizados para prevenção de coqueluche. Quanto à autoria dos estudos, em 67% das publicações havia participação de um epidemiologista. Conclusão: Esta investigação mostra as evidências científicas sobre a coqueluche na infância, necessárias para prevenir, controlar, promover a assistência e definir o perfil epidemiológico desse agravo nessa população. Os resultados revelaram nível de evidência cinco, sugerindo que as pesquisas desenvolvidas na área não retratam fortes evidências.(AU)


Background and Objectives: Despite the efforts of the National Immunization Program, pertussis remains a serious health problem. Thus, this study described the evidence in the literature on pertussis during childhood, which elucidates the health-care practices necessary for prevention, promotion, and control of the disease. Methods: This is an integrative literature review conducted in the LILACS, MedLine, PubMed, SciELO, Embase, Cumulative Index to Nursing and Allied Health Literature (CINAHL) databases, using the following descriptors: "Whooping Cough", "Epidemiology", and "Vaccination". The search was restricted to humanities, in English, Portuguese, or Spanish, without year delimitation. The search was completed on May 2020, Results: in total, 1,248 articles were selected, of which only nine articles were analyzed, mostly from North America, and 55% of the analyzed studies were about preventing pertussis. Regarding the authorship of the studies, epidemiologists participated in 67% of the publications. Conclusion: This study presents the necessary scientific evidence on pertussis during childhood is necessary for prevention, control, promotion, and epidemiological profile of this disease in this population. The results revealed a level of evidence of five, suggesting that the research in this area does not show strong evidence.(AU)


Justificación y Objetivos: La tos ferina todavía es un grave problema de salud a pesar de los esfuerzos realizados por el Programa Nacional de Inmunización. El objetivo de este estudio fue identificar las evidencias científicas en la literatura sobre la tos ferina en la infancia que indican las prácticas de asistencia sanitaria necesarias a la prevención, promoción y control de la enfermedad. Métodos: Revisión integrativa de la literatura en las bases de datos LILACS, MedLine, PubMed, SciELO, Embase, Cumulative Index to Nursing and Allied Health Literature (CINAHL) utilizando las palabras clave: Whooping Cough, Epidemiology, Vaccination. Se llevó a cabo en mayo de 2020 la búsqueda de artículos publicados en portugués, inglés y español, disponibles en su totalidad y sin delimitación de año. Resultados: Se seleccionaron un total de 1.248 artículos, de los cuales solo se analizaron nueve, en su mayoría norteamericanos; el 55% de los estudios analizados fueron sobre la prevención de la tos ferina. Respecto de la autoría de los estudios, un epidemiólogo participó en el 67% de las publicaciones. Conclusiones: Esta investigación muestra evidencias científicas sobre la tos ferina en la infancia necesarias para la prevención, el control, la promoción de la asistencia y la definición del perfil epidemiológico de la enfermedad en esa población. Los resultados revelaron que el nivel de evidencia se remite a cinco, lo que sugiere que la investigación en el área no muestra evidencias sólidas.(AU)

ABSTRACT

Resumo O Brasil abriga indivíduos de diversos países em situação de refúgio, tal como é definida pela Lei nº 9.474/1997 ou por autodeclaração. O uso do Sistema Único de Saúde faz parte dos direitos dos refugiados. Apesar desse direito, observa-se a ocorrência de preconceitos, desinformação e dificuldades de acesso a serviços. O objetivo desta pesquisa foi mensurar a qualidade de vida e compreender as necessidades e expectativas de famílias de refugiados africanos em relação à saúde. O estudo foi realizado em dois momentos: quantitativo e qualitativo. Os participantes responderam a escala de qualidade de vida WHOQOL-bref. Foi extraída uma família da amostra para a realização do genograma estrutural e de entrevista semiestruturada com questão norteadora. A análise quantitativa foi feita por estatística descritiva e os discursos obtidos, pela análise de conteúdo proposta por Laurence Bardin. Como resultados, no parâmetro de 0 a 100, a média apresentada foi de 49,57, classificando a qualidade de vida como "nem boa, nem ruim". Esse resultado é condizente com os discursos dos entrevistados e está relacionado à falta de emprego e dinheiro. Em relação à saúde, as avaliações não diferem das dos brasileiros: o serviço apresenta demora no atendimento, faltam médicos e os postos são distantes. Destaca-se também o preconceito vivido por alguns participantes ao serem atendidos pelos serviços de saúde, pelo fato de serem africanos.


Abstract Brazil is home to individuals from different countries in refuge situation, as defined by Law no. 9,474/1997 or by self-declaration. The Brazilian National Health System is available to refugees, but, despite this right, prejudice, misinformation, and difficulties in accessing these services are observed. The objective of this research is to measure their quality of life and to understand the needs and expectations of African refugee families in relation to health. The study was performed in two moments: a quantitative and a qualitative one. Participants responded to the WHOQOL-Bref quality of life scale. A sample family was extracted to compose the structural genogram and to participate in a semi-structured interview with a guiding question. The quantitative analysis was done by descriptive statistics and the discourses obtained were evaluated by the content analysis proposed by Laurence Bardin. Results showed, in the 0 to 100 parameter, a mean of 49.57, classifying the quality of life as "neither good, nor bad". This result is consistent with the discourses of the interviewees, related to lack of employment and money. Regarding health, their evaluations do not differ from those of Brazilians: the service presents delays in care, lack of doctors and the health posts are far away. The prejudice suffered by some participants when being attended by the health services, related to being African, was also highlighted.

Abstract INTRODUCTION: Thalidomide, used to treat erythema nodosum leprosum (ENL), is associated with severe adverse events (AEs) and is highly teratogenic. METHODS: A cross-sectional study was conducted on thalidomide-treated patients with ENL. AEs and selected variables were investigated through interviews and assessment of medical records. Odds ratios with 95% confidence intervals were estimated via logistic regression. RESULTS: Peripheral neuropathy symptoms and deep vein thrombosis (DVT) were the most common AEs reported. Although women of reproductive age used contraceptives, <50% of patients reported using condoms. Polypharmacy was associated with all endpoints, except DVT. CONCLUSIONS: Pharmacovigilance is crucial to prevent harmful thalidomide-associated AEs.

ABSTRACT

O estudo objetivou identificar e descrever as repercussões do diagnóstico de câncer para homens e mulheres, buscando compreender suas similaridades e diferenças. Realizaram-se entrevistas semiestruturadas com oito homens com câncer de próstata e oito mulheres com câncer de mama, residentes em Goiás. As entrevistas foram gravadas, transcritas, lidas exaustivamente e organizadas a posteriori, ancoradas no construcionismo social, nas seguintes temáticas: significados do processo saúde-doença-cuidado, o diagnóstico e o tratamento e suas implicações. Os resultados mostraram que o diagnóstico de câncer despertou sentimentos negativos e questionamentos profissionais e pessoais nos grupos, os quais foram enfrentados por meio da religiosidade/espiritualidade e no relacionamento com pessoas queridas. Além disso, demonstrou que questões de gênero precisam ser consideradas na assistência direcionada aos grupos pesquisados.


The current study aimed to identify and describe the impact of the diagnosis of cancer for men and women, finding out to understand the similarities and differences between the two groups. Semi-structured interviews were carried out with eight men with prostatic cancer and eight women with breast cancer, who have living in Goiás. The interviews were recorded, transcribed, read exhaustively and posteriori organized, anchored in social constructionism, in the themes: the meanings of the health-disease-care process, the diagnosis, and the treatment and its implications. The results showed that the diagnosis of cancer caused negative feelings for both groups. Other factors were questions related to personal professional dimensions that were coping with religiosity/spirituality and in the relationship with loved ones. In addition, it was demonstrated that gender issues need to be considered in health care directed to the two groups researched.


El estudio tuvo como objetivo identificar y describir las repercusiones del diagnóstico de cáncer para hombres y mujeres, buscando entender sus similitudes y diferencias. Se realizaron entrevistas semi estructuradas con ocho hombres con cáncer de próstata y ocho mujeres con cáncer de mama, residentes en Goiás. Las entrevistas fueron grabadas, transcritas, leídas exhaustivamente y organizadas a posteriori, ancladas en el construccionismo social, en las temáticas: significado del proceso salud-enfermedad-cuidado, el diagnóstico y el tratamiento y sus implicaciones. Los resultados muestran que el diagnóstico de cáncer despertó en ambos grupos sentimientos negativos y preguntas relacionadas a las dimensiones profesional y personal, las cuales fueron enfrentadas por la religiosidad/espiritualidad y en la relación con sus seres queridos. Además, demostró que cuestiones de género deben ser consideradas en la asistencia en salud dirigida a esa población.

ABSTRACT

A fim de garantir a qualidade final de produtos os laboratórios de análise microbiológica fornecem dados sobre a qualidade dos mesmos em todas as suas etapas de produção. A crescente preocupação com a saúde dos pacientes conduz à busca de métodos que forneçam resultados precisos e rápidos, pois possibilitam que ações corretivas sejam tomadas em tempo real. O presente trabalho teve por objetivo avaliar o potencial de tecnologia alternativa no monitoramento de endotoxina bacteriana na água tratada para diálise e dialisato e avaliar o potencial da citometria de fluxo na análise de água. Para isso utilizou-se Portable Test System (PTS®) como método alternativo para detecção de endotoxina bacteriana no monitoramento da água tratada para diálise e dialisato, o qual foi validado frente ao método convencional farmacopeico. Paralelamente realizou-se revisão narrativa da literatura a fim de avaliar a aplicabilidade da citometria de fluxo em análises de água. A análise dos diferentes parâmetros de validação para endotoxina bacteriana no método alternativo mostrou que, exceto para a menor diluição analisada, houve linearidade e precisão nos resultados. Por outro lado a concentração de 0,25 UE/mL foi a menor que apresentou exatidão e especificidade. Observou-se ainda, que o limite de detecção foi de 0,125UE/mL e o de quantificação de 0,25 UE/mL, portanto o intervalo foi de 0,25-1,0 UE/mL. Adicionalmente pela análise de resistência pode-se perceber que ao variar analistas não houve diferença significativa. Em relação ao tempo de análise em uma condição de rotina laboratorial com muitas amostras, o PTS® mostrou-se demorado. Ressalta ainda, que seria importante que a legislação vigente determinasse a análise mensal de endotoxinas no dialisato. A revisão da literatura evidencia o potencial da tecnologia de citometria de fluxo, pois a mesma mostrou-se satisfatória quando comparada a metodologias convencionais para análise de água. O trabalho desenvolvido permitiu concluir que o PTS®) mostrou-se adequado para analisar amostras in loco, permitindo análises em tempo real, que para as quais haja a expectativa de ausência de endotoxinas ou de concentração respeitando o intervalo de 0,25 UE/mL a 1,0 UE/mL. Quanto a citometria de fluxo, esta mostrou-se uma tecnologia promissora em analisar amostras de água, sendo portanto recomendável proceder a estudos de validação e aplicabilidade


In order to guarantee the final quality of products, the microbiological analysis laboratories provide data about their quality at all production stages. The growing concern for patients' health leads to the search for methods that provide accurate and fast results, as they enable corrective actions to be taken in real time. The present work aimed to evaluate the alternative technology potential in the monitoring of bacterial endotoxin in treated water for dialysis and dialysate and to evaluate the potential of flow cytometry in water analysis. The different validation parameters analysis for bacterial endotoxin in alternative method showed that, except for the lowest dilution analyzed, there was linearity and precision in the results. On the other hand, the concentration of 0.25 EU / mL was the lowest that presented accuracy and specificity. It was further observed that the detection limit was 0.125UE / mL and the quantification limit was 0.25 EU / mL, so the range was 0.25-1.0 EU / mL. Additionally by the ruggedness analysis it was possible to perceived that when varying analysts there was no significant difference. Regarding the analysis time in a laboratory routine condition with many samples, the PTS® was was time consuming. It was also observed that it would be important to determine monthly analysis of endotoxins in dialysate. The literature review evidence the flow cytometry technology potential of the because it was satisfactory when compared to conventional methodologies for water analysis. The research showed that the PTS® was suitable for analyzing samples in loco, allowing real-time analyzes, for which there is expectation of endotoxins absence or concentration respecting the range of 0.25 EU / mL to 1.0 EU / mL. For the flow cytometry, it was shown to be a promising technology for analyzing water samples, and it is therefore advisable to carry out validation and applicability studies

ABSTRACT

The study evaluated the vasorelaxant effect induced by the ethanolic extract of the leaves of Zanthoxylum rhoifolium Lam (EEtOH-Zr/leaves). Wistar rats were treated with the leaf extract containing a single dose of 2,000 mg / kg, v.o. After 14 days, the animals were anesthetized for blood collection and subsequent analysis of the biochemical parameters; they were then euthanized (sodium pentobarbital-100 mg/kg, i.p.) for the removal and morphological analysis of the heart, lung, liver and kidney. The vasorelaxation activity the and vascular reactivity of EEtOH-Zr/leaves were evaluated on artery mesenteric rings isolated from rats. The extract showed no signs of toxicity and no significant difference in the values of the biochemical parameters between the control group and the group of treated animals. In the evaluation of pharmacological activity in the smooth muscle, the EEtOH-Zr/leaves caused vasorelaxant effect on the tonic contraction induced by phenylephrine in mesenteric artery preparations in the presence (pD2=2.17±0.05 µg/mL; Emax=99.8±5.2%) and absence (pD2=2.14±0.05 µg/mL; Emax=95.3±6.4%) of the vascular endothelium. Oral administration of EEtOH-Zr/leaves reduced the contraction induced by the cumulative addition of PHE. It is concluded that the EEtOH-Zr/leaves promote vasorelaxation and reduce vascular reactivity of adrenergic alpha-1 agonist in the mesenteric artery. The results did not show toxic effects of the extract.

ABSTRACT The objective of the present study was to assess the presence of cognitive deficits in patients with chronic migraine, and to assess the main factors that trigger cognitive disorders, such as comorbidities or the use of medications. Methods: Chronic migraine and control groups were interviewed in a case-control study. The frequency and intensity of the headache, medication used and associated comorbidities were determined. All patients were submitted to an extended neuropsychological assessment. Results: The chronic migraine group (n = 30) had a worse performance in the Montreal Cognitive Assessment Test (p = 0.00), Verbal Fluency (p = 0.00), Stroop (p = 0.00), Clock Drawing Test (p = 0.00), Digit Span (p = 0.00) and Matrix Reasoning (p = 0.01). After statistical adjustment by linear regression, migraine continued to be the only relevant factor in the poorer performance in the Montreal Cognitive Assessment, Verbal Fluency, Clock Drawing and Stroop tests. Conclusion: Patients with chronic migraine have cognitive deficits in multiple tasks, regardless of the presence of comorbidities or the use of medications.


RESUMO O objetivo do presente estudo foi avaliar a presença de déficits cognitivos em pacientes com migrânea crônica e avaliar os principais fatores que desencadeiam transtornos cognitivos, como comorbidades e uso de medicações. Métodos: Pacientes com migrânea crônica (n = 30) e controles foram entrevistados, em um estudo caso-controle. A frequência e intensidade da cefaleia, medicações utilizadas e comorbidades associadas foram determinadas. Todos os pacientes foram submetidos a uma avaliação neuropsicológica estendida. Resultados: Os pacientes com migrânea crônica apresentaram uma pior performance no Montreal Cognitive Assessment (p = 0.00), Fluência Verbal (p = 0.00), Teste de Stroop (p = 0.00), Teste do Relógio (p = 0.00), Subteste Dígitos (p = 0.00) e Raciocínio matricial da WAIS-III (p = 0.01). Após ajuste estatístico através de Regressão Linear, a migrânea se manteve como único fator relevante para pior desempenho em MoCA, Fluência verbal, Teste do relógio e Teste de Stroop. Conclusão: Pacientes com migrânea crônica apresentam déficits cognitivos incluindo múltiplas tarefas, independentes da presença de comorbidades e uso de medicações.

ABSTRACT Objective To evaluate cutaneous allodynia among patients with chronic and episodic migraine in a tertiary headache clinic. Methods 80 subjects with episodic migraine and 80 with chronic migraine were assessed in a tertiary hospital. The 12-item Allodynia Symptom Checklist/Brazil questionnaire was applied to classify subjects according to the presence and severity of cutaneous allodynia. Results Cutaneous allodynia was identified in 81.3% of the episodic migraine group and 92.5% of the chronic migraine group (p = 0.03). No increased association could be attributed to chronic migraine when adjusted by years with disease (PR = 1.12; 95%CI = 0.99 to 1.27; p = 0.06). The groups also did not differ in the severity of allodynia, and severe presentation was the most frequent. Discussion Both groups seemed to be similarly affected in the cephalic and extracephalic regions, with the same severity. Conclusion Cutaneous allodynia is more frequent in chronic migraine, and its presence and severity seems to be more associated with the duration of the disease.


RESUMO Objetivo Avaliar a característica da alodinia cutânea em indivíduos com migrânea crônica e episódica em um hospital terciário. Métodos 80 sujeitos com migrânea episódica e 80 com migrânea crônica de um hospital terciário foram avaliados. O questionário 12-item Allodynia Symptom Checklist/Brasil foi aplicado e classificou os sujeitos quanto a presença e severidade da alodinia cutânea. Resultados A alodinia cutânea esteve presente em 81,3% dos migranosos episódicos e 92,5% nos crônicos (p = 0.03). Nenhuma associação pode ser atribuída a migrânea ao ser ajustada pela variável anos com doença (PR = 1.12; 95%IC = 0.99 para 1.27; p = 0.06). Os grupos não diferiram em relação à severidade da alodinia e a classificação severa foi a mais frequente. Discussão Ambos os grupos pareceram ser igualmente afetados nas regiões cefálicas e extracefálicas com a mesma severidade. Conclusão A alodinia cutânea é mais frequente na migrânea crônica, mas a presença e severidade parece estar mais associada com a duração da doença.

Abstract Many parks in the city of São Paulo contain remnants of Atlantic Forest. Of the 30 municipal parks in the South of the city, we investigated two in this study (Santo Dias Park and Shangrilá Park) in order to survey their mosquito fauna and investigate the presence of potential bioindicators of environmental conditions and vectors of human pathogens. Mosquitoes were collected monthly between March 2011 and February 2012 using aspirators, Shannon and CDC traps for adult mosquitoes and larval dippers and suction samplers for immature forms. Sampling effort was evaluated by plotting a species accumulation curve, and total richness was estimated using the first-order jackknife. To compare the diversity between the two parks Shannon and Simpson diversity indexes were calculated. Species similarity was compared by the Sorensen similarity index. In all, 8,850 specimens were sampled in both parks. Collections in Santo Dias Park yielded 1,577 adult mosquitoes and 658 immature individuals distributed in seven genera (Aedes, Anopheles, Culex, Limatus, Mansonia, Toxorhynchites and Wyeomyia) and 27 taxonomic units. Among the adult mosquitoes collected, Culex nigripalpus .and Aedes fluviatilis were the most abundant, while the most abundant immature forms were Cx. imitator, Wy. davisi, Wy. galvaoi and Ae. albopictus. Collections in Shangrilá Park yielded 4,952 adult specimens and 1,663 immature forms distributed in eight genera (Aedes, Anopheles, Culex, Limatus, Mansonia, Toxorhynchites, Uranotaenia and Wyeomyia) and 36 taxonomic units. Species accumulation curves in both parks were close to the asymptote, and the total richness estimate was close to the observed richness. Although the observed species richness was higher in the Shangrilá Park, there was no statistically significant difference between the diversity indexes measured. Regarding species composition, the two sites shared 16 species, including those of epidemiological importance such as Culex nigripalpus, Cx. quinquefasciatus, Aedes albopictus and Ae. aegypti. As some of the mosquito taxa found are bioindicators of environmental conditions and have epidemiological potential to carry pathogens, we recommend that urban parks should be included in official mosquito surveillance programs, and regular surveys carried out to detect circulating arboviruses.


Resumo Parques urbanos do município de São Paulo contêm remanescentes de Mata Atlântica. No sul da cidade há 30 parques municipais, sendo os parques Santo Dias e Shangrila alvos deste estudo. Este estudo teve a proposta de levantamento da fauna de culicídeos desses dois parques no sul da cidade de São Paulo e avaliar a presença de potenciais bioindicadores e espécies vetoras de patógenos aos seres humanos. Os mosquitos foram coletados mensalmente entre março de 2011 e fevereiro de 2012, com aspiradores, armadilhas de Shannon e CDCs para mosquitos adultos e conchas entomológicas e bombas manuais de sucção para os imaturos. O esforço amostral foi avaliado por traçar uma curva de acumulação de espécies, e a riqueza total foi estimada pelo método jackknife de primeira ordem. Para comparar a diversidade entre os dois parques, foram calculados os índices de diversidade de Shannon e de Simpson. A similaridade na composição de espécies foi comparada pelo índice de similaridade de Sorensen. Foram coletados um total de 8.850 espécimes de culicídeos em ambos os parques. Coletas no parque Santo Dias renderam 1.577 mosquitos adultos e 658 imaturos, distribuídos em sete gêneros (Aedes, Anopheles, Culex, Limatus, Mansonia, Toxorhynchites e Wyeomyia) e 27 unidades taxonômicas: Culex nigripalpus, e Aedes fluviatilis foram as mais abundantes unidades taxonômicas coletadas como adultos, enquanto em formas imaturas, as espécies mais abundantes coletadas foram Cx. imitator, Wy. davisi, Wy. galvaoi e Ae. albopictus. Coletas no parque Shangrilá renderam 4.952 espécimes como adultos e 1.663 formas imaturas, distribuídas em oito gêneros (Aedes, Anopheles, Culex, Limatus, Mansonia, Toxorhynchites, Uranotaenia e Wyeomyia) e 36 unidades taxonômicas.. As curvas de acúmulo de espécies em ambos os parques ficaram perto da assíntota, e as estimativas de riqueza total foram próximas às riquezas observadas. Apesar da riqueza observada ter sido maior no parque Shangrilá, não houve diferença estatisticamente significante entre os índices de diversidade mensurados. Em relação à composição de espécies os dois locais compartilharam 16 espécies, incluindo as de maior importância epidemiológica como Culex nigripalpus, Cx. quinquefasciatus, Aedes albopictus e Ae. aegypti. Alguns táxons de culicídeos são bioindicadores de condições ambientais nas áreas ou possuem potencial para veicular patógenos. Atenção deve ser dada a parques urbanos, com inclusão destes locais nos programas oficiais de vigilância entomológica e investigações periódicas na circulação de arbovírus.

ABSTRACT

Introdução: Parques inseridos na malha urbana de grandes metrópoles possuem potencial para manter o ciclo biológico de diversas espécies vetoras de patógenos, como as espécies Culex quinquefasciatus e Culex nigripalpus. Consideradas antropofílicas, essas espécies têm importância epidemiológica e são abundantemente encontradas na cidade de São Paulo. Porém, pouco se sabe sobre as características genéticas dessas espécies em escala microgeográfica. Visando o melhor entendimento sobre a estrutura populacional dessas espécies, foi analisado os padrões da forma alar e a caracterização genética por marcadores de microssatélites, afim de se obter informações que contribuam para o entendimento da situação populacional desses vetores dentro do município. Objetivos: (1) Analisar a variabilidade da forma alar nas populações de Cx. quinquefasciatus e Cx. nigripalpus; (2) Analisar a variabilidade genética e o fluxo gênico nas mesmas populações; (3) Testar a funcionabilidade de primers desenvolvidos para regiões de microssatélites nas populações de Cx. nigripalpus. Material e Métodos: No total, foram estudadas cinco populações de Cx. quinquefasciatus e sete populações de Cx. nigripalpus, coletados em parques urbanos da cidade de São Paulo. Análises discriminantes, como variável canônica, teste de reclassificação cruzada e dendrograma de Neighbor-joining, utilizando os software Morpho J e Past, foram realizadas para a compreensão do formato e tamanho da asa direita nas populações. Em relação ao estudo da estruturação genética, foram testados 12 pares de primers de microssatélites em mosquitos Cx. quinquefasciatus e 33 pares de primers em mosquitos Cx. nigripalpus. Resultados: Análise da morfometria geométrica alar nas populações de Cx. quinquefasciatus demonstrou homogeneidade nos formatos alares, sendo um caracter preservado nessa espécie pela cidade, contudo, há tênues diferenças na população coletada em ambiente mais silvestre. O mesmo foi observado para as populações de Cx. nigripalpus, onde foi possível visualizar uma subestruturação na forma alar dentro da população Shangrilá. Dos primers testados, 12 amplificaram de forma consistente em todas as populações de Cx. quinquefasciatus e seis primers nas populações de Cx. nigripalpus. Os resultados encontrados sugerem que ambas as espécies possuem baixa estruturação genética, com fluxo gênico moderado entre as populações de Cx. quinquefasciatus e baixo entre as de Cx. nigripalpus, apresentando alto índice de heterozigosidade, onde as únicas populações que estão em expansão são as que foram coletadas em ambientes onde a urbanização está avançando. Discussão: Processos de urbanização, somados às mudanças causadas no ambiente, beneficiam e tendem a elevar a abundância dessas espécies em ambientes antropizados. A baixa estruturação genética e morfométrica das asas dessas espécies indicam a adaptação delas na cidade, havendo segregação devido à heterogeneidade do ambiente em que as populações se encontram. Conclusão: Evidências de baixa estruturação entre as populações e indícios de expansão em populações de ambientes mais silvestres indicam que essas espécies estão atreladas ao processo de urbanização da cidade de São Paulo


Introduction: Urban parks have the potential to harbor and maintain the life cycle of several mosquitoes such as Culex quinquefasciatus and Culex nigripalpus, both are anthropophilic species with epidemiological importance and very abundant in São Paulo city. However, their genetics characteristics are poorly know. Aiming the better understanding of the population structure of these species in microregion, was evaluated the wing shape variation and selected microsatellite loci. In this way, the information obtained can contribute to the understanding of the population situation of these vectors in São Paulo city. Objectives: (1) To evaluate the wing shape variability in populations of Cx. quinquefasciatus and Cx.nigripalpus; (2) To evaluate the genetic variability and gene flow in the Cx. quinquefasciatus and Cx.nigripalpus populations (3) Test microsatellite markers functionality parameters in Cx. nigripalpus, previously used successfully in other Culex species. Material and Methods: Were studied, five populations of Cx. quinquefasciatus and seven population of Cx. nigripalpus collected in urban parks in São Paulo city. Discriminant analysis was made to evaluate the wing shape patterns, such as Canonical variate analysis, cross validated test and Neghbor-joninin dendrogram using Morpho J and Past softwares, were perfomed to understand the size and shape of the right wing in these populations. For the study of genetic structuring, there were tested 12 pairs of microsatellite loci in Cx. quinquefasciatus samples and 33 pairs of microsatellite loci in Cx. nigripalpus samples. Results: The wing shape patterns in Cx. quinquefasciatus population were homogeneous, showing a preserved character in this population, however, has been tenuous differences in the more sylvatic population. The same pattern was observed in Cx. nigripalpus populations and was observed substructuring in the Shangrilá population. From the tested primers, 12 were functional and amplified consistently in the all five Cx. quinquefasciatus population and for Cx. nigripalpus, six primers were amplified. The results suggest both species having low genetic structure, moderate gene flow and the only populations which are expading were collected in areas that the urbanization is increasing. Discussion: Urbanization processes added to the environmental changes benefit and tend to raise the abundance of these species in anthropized locals. The low genetic structure and alar morphometry indicates the adaptation of these species in São Paulo city, there being segregated due to the environment heterogeneity which the population inhabit. Conclusion: Evidence of low structure between populations studied and signs of expansion in populations of more sylvatic environments indicate that these species are linked to urbanization process

RESUMOIntrodução:O samba é uma dança de movimentos intensos e variados que proporcionam alegria e boa resposta cronotrópica, sendo plausível a expectativa de que beneficie o sistema cardiovascular.Objetivo:Desenvolver e avaliar um protocolo de samba brasileiro visando o treinamento físico na reabilitação cardíaca. Métodos: Quinze indivíduos com doença arterial coronariana estável, idade 60,74±5,96 anos, realizaram teste cardiopulmonar máximo com determinação da frequência cardíaca (FC) correspondente ao VO2pico e primeiro e segundo limiares ventilatórios. Durante seis sessões executaram vinte passos de samba, em três andamentos musicais, classificados por metrônomo em lento, médio ou rápido. Posteriormente, realizaram três sessões para verificar o comportamento da FC e a percepção de esforço. A análise descritiva foi utilizada na caracterização da amostra e verificação da FC, o testet ou teste de Wilcoxon foram utilizados para comparar o comporta-mento da FC (nas diferentes sessões), com nível de significância de 5%.Resultados:Houve boa adaptação aos passos de samba, com FC situando-se entre 62% e 72% da FCpico. Durante as sessões de dança os pacientes mantiveram-se em 86 ±4,17% do tempo na zona alvo proposta para treinamento físico na reabilitação (60% a 90% da FCpico) e 77±2,26% do tempo na zona alvo considerada ideal, com a FC situada entre o primeiro e o segundo limiares ventilatórios. Durante todo o tempo os pacientes referiram percepção de esforço de leve a moderada.Conclusão:Os pacientes demonstraram boa adaptação ao protocolo de samba, que se mostrou em condições de ser adotado como proposta de treinamento na reabilitação cardíaca.


ABSTRACTIntroduction:Samba requires intense and varied movements that provide happiness and good chronotropic res-ponse making plausible to expect its benefits on cardiovascular health.Objective:To develop and evaluate a brazilian samba protocol aiming at physical training in cardiac rehabilitation.Methods:Fifteen patients with stable coronary artery disease, aged 60.74±5.96 years, underwent maximal cardiopulmonary test with determination of heart rate (HR) corresponding to the peak VO2 and first and second ventilation thresholds. During six sessions, they underwent twenty samba steps, in three music tempo classified by a metronome (slow, medium and fast). Subsequently, they underwent three sessions to verify the HR behavior and perception of effort. Descriptive analysis has been utilized in order to characterize the sample and check HR; T test or Wilcoxon test have been used to compare HR behavior (in different sessions) with significance level of 5%.Results:There was good adaptation to the samba steps, with HR between 62% and 72% of peak HR. During the dance sessions patients remained 86±4.17% of time in the target zone proposed for physical training in rehabilitation (60% to 90% of HR peak) and 77±2.26% of time in the target zone considered ideal, with HR situated between the first and the second ventilation threshold. Throughout the time the patients reported having perceived mild to moderate effort.Conclusion: Patients showed good adaptation to samba protocol, which proved being able to be adopted as a proposal for training in cardiac rehabilitation.


RESUMENIntroducción:Samba es una danza de movimientos intensos y variados que traen alegría y buena respuesta cronotrópica, y es plausible esperar que beneficia el sistema cardiovascular.Objetivo:Desarrollar y evaluar un proto-colo de samba brasileño destinado a entrenamiento físico en la rehabilitación cardiaca.Métodos:Quince pacientes con enfermedad coronaria estable, edad 60,74 ± 5,96 años, fueron sometidos a prueba de esfuerzo cardiopulmonar máximo para determinar la frecuencia cardíaca (FC) correspondiente a VO2pico y primer y segundo umbrales ventila-torios. Durante seis sesiones realizaron veinte pasos de samba, en tres movimientos musicales clasificados en orden de metrónomo a lento, medio o rápido. Posteriormente se realizaron tres sesiones para comprobar el comportamiento de la FC y el esfuerzo percibido. Se realizó un análisis descriptivo para la caracterización de la muestra y la verificación de la FC; se utilizaron la prueba t o prueba de Wilcoxon para comparar el comportamiento de la FC (en diferentes sesiones), con un nivel de significación del 5%.Resultados:Hubo una buena adaptación a los pasos de samba, com la FC entre el 62% y el 72% de FCpico. Durante las sesiones de baile, los pacientes se mantuvieron en 86 ± 4,17% del tiempo en la zona de destino propuesta para el entrenamiento físico en rehabilitación (60% a 90% de FCpico) y 77 ± 2,26% del tiempo en la zona de destino considerado óptimo, con la FC situada entre el primero y el segundo umbral ventilatorio. Todo el tiempo la percepción percibida pelos pacientes fue de esfuerzos leves a moderados.Conclusión:Los pacientes mostraron una buena adaptación al protocolo de samba, que resultó en condiciones de ser adoptado como una propuesta para el entrenamiento en rehabilitación cardiaca.
