ABSTRACT

OBJECTIVES@#To observe the effects of electroacupuncture (EA) at "Neiguan" (PC 6) and "Zusanli"(ST 36) on the gastric emptying rate, the level of serotonin (5-HT) and the protein expression of motilin (MTL), ghrelin, substance P (SP) and vasoactive intestinal peptide (VIP) in the antral tissue of the rats with functional dyspepsia (FD) and explore the effect mechanism of EA in treatment of FD.@*METHODS@#A total of 21 SPF male SD rat pups were randomly divided into a normal group, a model group and an EA group, with 7 rats in each group. In the model group and the EA group, FD model was prepared by the gavage with 0.1% sucrose iodoacetamide solution combined with the modified small platform method. After the successful modeling, EA was applied to "Neiguan" (PC 6) and "Zusanli"(ST 36) in the rats of the EA group, with disperse-dense wave, 20 Hz/100 Hz in frequency, stimulated for 30 min, once daily, for 7 days consecutively. Before and after intervention, the general condition of the rats was observed in each group. After the completion of intervention, the gastric emptying rate was measured, the morphological changes of gastric antral tissue were observed using HE staining, the level of 5-HT was detected with ELISA method, and the protein expression of MTL, ghrelin, SP, and VIP was determined with Western blot method in the antral tissue of rats.@*RESULTS@#In the normal group, the rats were in a good mental state, with lustrous fur, flexible movement and the increase of food intake and body mass. In the model group, the rats were poor in mental state, lack of lustre in fur, preference for the body curled up, reduced activity and response; and a part of rats had loose stool, obviously enlarged gastric body and gastric food retention. In the EA group, the general condition of rats, e.g. the mental state, food intake and activity, were improved, the gastric body got smaller obviously and the gastric food retention was reduced when compared with the model group. The antral structure was intact, the glands were rich and no injury of the gastric mucosa was found, e.g. inflammatory reaction and edema in the rats of each group. Compared with the normal group, the gastric emptying rate was decreased (P<0.01), 5-HT level was increased (P<0.01), the protein expression of MTL and ghrelin was reduced (P<0.01) and that of VIP was elevated (P<0.01) in the rats of the model group. The gastric emptying rate was increased (P<0.01), 5-HT level was decreased (P<0.01), and the protein expression of MTL and ghrelin was elevated (P<0.05, P<0.01) in the rats of the EA group when compared with those in the model group.@*CONCLUSIONS@#Electroacupuncture at "Neiguan" (PC 6) and "Zusanli"(ST 36) may effectively relieve gastric dysfunction, strengthen gastric motility and promote gastric emptying so as to alleviate the symptoms of dyspepsia in FD rats, and its mechanism may be related to the regulation of gastrointestinal hormones in the antral tissue.

ABSTRACT

Objective: To observe the effects of acupoint thread-embedding therapy and low-carbohydrate diet therapy on obese patients with food addiction. Methods: Sixty-five eligible patients were randomized into a thread-embedding group of 33 cases and a diet group of 32 cases to respectively receive 12-week treatment. Before treatment, after treatment and at 6-month follow-up, the two groups were observed and compared in terms of body mass (BM), waist circumference (WC), hip circumference (HC), waist-to-hip ratio (WHR), body mass index (BMI), body fat rate (BFR), basal metabolic rate (BMR) and Yale food addiction scale version 2.0 (YFAS 2.0). Results: At the end of treatment, there were no significant differences in the general efficacy, and the improvements in BM, BMI, WC, HC, WHR and BFR between the thread-embedding group and diet group (all P>0.05). At follow-up, the thread-embedding group showed more significant improvements in all the aforementioned indicators compared with the diet group except HC (all P<0.05). At the end of treatment and follow-up, BMR and YFSA 2.0 had more significant improvements in the thread-embedding group than in the diet group (all P<0.05). Conclusion: Acupoint thread-embedding therapy can produce significant efficacy in treating obese patients with food addiction; it can improve the food addiction state and work better in maintaining the efficacy compared with low-carbohydrate diet therapy.

ABSTRACT

OBJECTIVE@#To evaluate the efficacy and safety of Tongxiening Granules (, TXNG) in the treatment of irritable bowel syndrome with predominant diarrhea (IBS-D).@*METHODS@#A randomized, double-blind, double-dummy, and positive parallel controlled clinical trial was conducted from October 2014 to March 2016. Totally 342 patients from 13 clinical centers were enrolled and randomly assigned (at the ratio of 1:1) to a treatment group (171 cases) and a control group (171 cases) by a random coding table. The patients in the treatment group were administered orally with TXNG (5 g per time) combined with pinaverium bromide Tablet simulator (50 mg per time), 3 times per day. The patients in the control group were given TXNG simulator (5 g per time) combined with pinaverium bromide Tablets (50 mg per time), 3 times per day. The treatment course lasted for 6 weeks. The improvement of Irritable Bowel Syndrome Symptom Severity Score (IBS-SSS) was used to evaluate the primary outcome. Secondary outcomes included adequate relief (AR) rate, Irritable Bowel Syndrome-Quality of Life Questionnaire (IBS-QOL), Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD), and the recurrence rate at follow-ups. Safety indices including the adverse events (AEs) and related laboratory tests were evaluated.@*RESULTS@#Primary outcome: IBS-SSS at baseline, weeks 2, 4, 6 showed no statistical significance in both full analysis set (FAS) and per protocol set (PPS, P>0.05). After 6 weeks of treatment, the total effective rate of IBS-SSS scores in the treatment group (147/171,86.0%) was higher than the control group (143/171, 83.6%) by FAS (P>0.05). In regard to secondary outcomes, after 6-week treatment, there was no significant difference in AR rate, total score of IBS-QOL, improvement of HAMD and HAMA total scores between the two groups (P>0.05). The recurrence rate at 8-week follow-up was 12.35% (10/18) in treatment group and 15.79% (12/76) in control group, respectively (P>0.05). A total of 21 AEs occurred in 15 cases, of which 11 occurred in 8 cases in the treatment group and 10 AEs in 7 cases in the control group. The incidence of AEs had no statistical significance between the two goups (P>0.05).@*CONCLUSION@#Tongxiening Granules could relieve the symptoms of patients with IBS-D and the treatment effect was comparable to pinaverium bromide. ABSTRACT

BACKGROUND: Femoral head necrosis is a multifactorial disease, and has the youth oriented tendency. It often results in femoral head collapse and leads to total hip arthroplasty. Thus, finding a secure and effective treatment is of clinical benefits to relieve patients' suffering and to reduce social economic burden. Bone marrow mesenchymal stem cells (BMSCs) transplantation has been used in the clinical practice of femoral head necrosis. However, the conclusion remains controversial. OBJECTIVE: To access the safety and efficacy of the core decompression combined with autologous BMSCs transplantation in patients with femoral head necrosis by using meta-analysis approach. METHODS: Randomized clinical controlled trials (RCTs) which compared the therapeutic effects between core decompression combined with autologous BMSCs and core decompression were systematically retrieved from inception to June 20, 2017 in PubMed, The Cochrane Library (Issue 5, 2017), Embase, CNKI, CBM, VIP and WanFang databases. After extraction of the information and evaluation of the study quality, a meta-analysis was performed by RevMan software. RESULTS AND CONCLUSION: Eight RCTs with 323 patients (395 hips), 193 hips in BMSCs group and 202 in conventional therapeutic group, were ultimately included. The revisit time was 12-60 months. The overall quality of the trials was considered moderate-high. The results of meta-analysis show that compared with core decompression alone, autologous BMSCs transplantation combined with core decompression could alleviate the pain [Visual Analogue Scale: mean difference ( MD)=-0.39, 95% confidence interval (CI) (-0.76,-0.01)], enhance the joint function [Harris score: 12 months MD=7.16, 95%CI (3.88, 10.44) and 24 months MD=11.16, 95%CI (8.32, 14.00)], decrease the rate of disease progression in radiography [odds ratio=0.23, 95%CI (0.09, 0.55)]. Although there was no statistical significance between two groups, BMSCs transplantation had trend to reduce the rate of total hip arthroplasty [risk ratio=0.44, 95%CI (0.19, 1.03)]. No obvious complications were found in the course of BMSCs therapy. Given the above, autologous BMSCs transplantation combined with core decompression is a secure and effective therapeutic method for femoral head necrosis.

ABSTRACT

BACKGROUND:Stem cell transplantation has been used in the clinical treatment of spinal cord injury. However, the efficacy and safety are still controversial.Although there are many approaches for stem cell transplantation, which one is better is unclear as yet.OBJECTIVE:To systematically evaluate the efficacy and safety of stem cell transplantation for spinal cord injury, and to compare the therapeutic difference in stem cell transplantation via different approaches.METHODS:A computer-based online search was conducted in PubMed,The Cochrane Library (Issue 4,2016),Embase,CNKI,VIP,CBM,and Wan-Fang databases up to May 13,2016 to screen the relevant randomized clinical controlled trials of stem cells in the treatment of spinal cord injury. Two reviewers independently selected the studies, extracted information, and assessed the quality of included trials.Data extracted from eligible studies was pooled and meta-analyzed using Stata13.1 and Gemtc0.14.3 software.RESULTS AND CONCLUSION:A total of 10 randomized controlled trials involving 546 patients (294 in stem cells group and 252 in rehabilitation treatment group) were included. The results of meta-analysis showed that stem cell transplantation had an advantage over rehabilitation treatment in increasing American Spinal Cord Injury Association (ASIA) motor score,ASIA sensory score,Barthel Index, and decreasing the bladder residual urine volume. The incidence of low fever, abdominal distension, headache, lower limb numbness, and meningeal irritation was 14%,7%,7%,8%,and 7%,respectively.Taking the rehabilitation treatment as a common reference, the results of the network meta-analysis showed that there were no significant differences in ASIA motor score,ASIA sensory score,Barthel Index, and incidence of complications among subarachnoid injection, intravenous injection, and intralesional injection.Compared with the rehabilitation treatment, only stem cell transplantation via subarachnoid injection had significant differences in ASIA motor score [MD=9.77,95%CI (0.26,21.46)],and ASIA sensory score [MD=25.79,95%CI (10.07,45.27)].To conclude, the stem cells transplantation via subarachnoid injection is considered the most effective transplantation method.Due to the limitations of the included studies, more high-quality randomized controlled trials are required to verify the above conclusion. In addition, future studies should focus on the long-term efficacy and safety of stem cell transplantation in the treatment of spinal cord injury, and should investigate the clinical effects on spinal cord injury with different ASIA grades, types of stem cells, and transplantation time.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effects and mechanism of electroacupuncture combined with medium frequency therapy on lumbar nerve root compression.</p><p><b>METHODS</b>Seventy-two Sprague-Dawley (SD) rats were randomly divided into a normal group, a sham operation group, a model group, an electroacupuncture group (EA group), a medium frequency group(MF group) and an electroacupuncture combined with medium frequency group (EA + MF group), twelve rats in each group. Models were established by surgery except the normal group and the sham operation group. Rats in the normal group, the sham operation group and the model group were not treated. In the EA group, the rats were treated by electroacupuncture at "Jiaji" (EX-B 2) and "Huantiao" (GB 30) etc., and by medium frequency at the "Jiaji" (EX-B 2) and "Huantiao"(GB 30) in the MF group. Rats in the EA + MF group were treated by both electroacupuncture and medium frequency. All treatments were started on the fifth day of established model, once a day for fourteen days. Rats' lower limb functions were observed before and after treatment, thromboxane B 2 (TXB 2) and prostacyclin F1alpha (PGF1alpha) in blood plasma were tested after treatment, and pathological changes in the local compressed nerve root were observed by light microscope.</p><p><b>RESULTS</b>After treatment, the scores of rats' lower limb neurologic function in three therapy groups were significantly lower than before (all P < 0.01). Compared with the model group, TXB 2 in the EA group and the EA + MF group after treatment were decreased significantly (both P < 0.01), and PGF1alpha in the EA + MF group was increased significantly (P < 0.01), and TXB 2/PGF1alpha level were all regulated favorably in three therapy groups (all P < 0.01), and the pathological scores in the EA group and the EA + MF group were improved significantly (both P < 0.01).</p><p><b>CONCLUSION</b>n Electroacupuncture combined with medium frequency has anti-inflammatory and analgesic effects in model rats with lumbar nerve root compression, and its mechanism may be related with the regulation of homeostasis M between TXB 2 and PGF1alpha so as to improve microcirculation.</p>

ABSTRACT

OBJECTIVE@#To analyze the clinical characteristics and treatment of bilateral and unilateral sudden sensorineural hearing loss (SSHL) and figure out the differences between bi-and unilateral sudden sensorineural hearing loss, and further improve the diagnosis and treatment of SSHL.@*METHOD@#The retrospective study was based on 334 patients with SSHL (315 with uni-SSHL versus 18 with bi-SSHL) and all of the patients were in-patients.@*RESULT@#The incidence of bi-SSHL was 5.4% of patients with SSHL. There was no significant difference on the clinical features such as age, gender, interval from onset of symptoms to seeing doctors and associated symptoms between bi-and unilateral SSHL. 27.8% of the patients with bi-SSHL had preexisting diabetes mellitus, which was significantly higher than the rate of the uni-SSHL (9.8%, P<0.05). Hearing loss was more profound in the uni-SSHL ones, however, the hearing improvements were better than their counterparts. The overall effective rate of the two groups was 58.4% and 13.9% respectively. Earlier diagnosis and treatment with steroids would have a good prognosis.@*CONCLUSION@#Unilateral SSHL is more common than bilateral SSHL. Bilateral SSHL occurs more common with preexisting diabetes mellitus and has worse prognosis compared to unilateral SSHL. The earlier steroid treatment the better prognosis.

<p><b>OBJECTIVE</b>To investigate the polymorphism of DXYS267 locus in China Han population and find the application and characters of its Y-specific single nucleotide substitutions.</p><p><b>METHODS</b>The locus was analyzed by PCR and PAGE in silver-staining. The Y-specific STR was amplified with newly designed primers according to the Y-specific single nucleotide substitutions.</p><p><b>RESULTS</b>Six alleles were detected in Han population in Wuhan. Exact tests demonstrated that genotype frequencies did not deviate from Hardy-Weinberg equilibrium. Heterozygosity of DXYS267 was 0.6706, discrimination power (DP) was 0.8433, and the probability of paternity exclusion (PE) was 0.5957. The Y-specific STR of DXYS267 was successfully amplified with the new primer. The 4 alleles for Y-STR were detected with haplotype diversity (HD) 0.6372.</p><p><b>CONCLUSION</b>The DXYS267 locus is appropriate for individual identification and paternity testing. The new primer is useful for individual and paternity testing involving brothers and mixed stains.</p>

ABSTRACT

Objective To establish the HPLC fingerprint of Sanhuang Xiexin Decoction and provide a method to study the potential basis and the changing of the chemical component for it in different compati- bility. Methods An HPLC method was established with Shimadzu ODS column(150 mm?4.6 mm, 5?m),the mobile phase was methanol-water-0.1% phosphoric acid(0.01 mol/L potassium phosphate monobasic,pH 2.8)as gradient elution, the flow rate was 1.0 mL/min,and the detection wavelength was 260 nm.Through comparing and analyzing the relative retention time of this decoction and of its composi- tion which are positive and negative control fingerprints, the main chromatographic peak origins were con- firmed; The correlated chromatographic peaks were identified by contrasting chromatographic peak reten- tion time and adding reference substances to the sample. Results All tested samples contained the 32 common peaks, the relativity of them were analyzed and 11 peaks were indicated. The similarity of ten batches of samples exceeded 0.92.Conclusion This method shows sensitive and good repeatability, all of the contents are separated well.It is used to determine Sanhuang Xiexin Decoction and its relative prepara- tions.
