ABSTRACT

Objective:The purpose of this paper is based on the concept of enhanced recovery after surgery, to discuss the timing of taking food and water in knee arthroplasty patients, as well as its feasibility and safety, so as to improve the quality of quality nursing services.Methods:The 226 knee arthroplasty patients in Luoyang Orthopedic-Traumatological Hospital (Henan Provincial Orthopedic Hospital) from September 2020 to November 2021 were selected and divided into a control group and a experimental group using the random number table method, with 113 cases in each group. The control group adopted a conventional postoperative feeding management method; the experimental group adopted the early and timely feeding management method after comprehensive assessment. The data were compared between the two groups, including the incidence and severity of postoperative nausea and vomiting within 6 h and from 6 to 24 h after surgery , the incidence of thirst and hunger at 2 h, 4 h and 6 h postoperatively, and the comfort level.Results:The incidence of postoperative nausea and vomiting was lower in the experimental group [19.5%(22/113), 13.3%(15/113)] than in the control group [32.7%(37/113), 23.9%(27/113)] within 6 h and from 6 to 24 h after surgery, and the severity (21 cases of gradeⅠ, 10 cases of gradeⅡand 6 cases of grade Ⅲ) was also lower than in the control group (17 cases of gradeⅠ, 29 cases of gradeⅡand 18 cases of grade Ⅲ), and the differences were all statistically significant ( χ2 = 5.16,4.21, Z = -2.72, P<0.05). The incidence of thirst were lower in the experimental group [14.2%(16/113), 8.0%(9/113), 2.7%(3/113)] than in the control group [26.5%(30/113), 29.2%(33/113), 40.0%(35/113) at 2 h, 4 h and 6 h postoperatively, and the incidence of hunger at 4 h and 6 h postoperatively [11.5%(13/113), 8.0%(9/113)] were lower than in the control group [32.7%(37/113), 34.5%(39/113)], with statistically significant differences ( χ2 values were 5.35 to 32.39, all P<0.05). The postoperative comfort scale scores of physiological (31.04 ± 1.00) and psychological (33.50 ± 1.45) were higher in the experimental group than in the control group (27.46 ± 1.78) and (31.37 ± 1.29), and the differences were statistically significant ( t values were -102.36 to -66.26, all P<0.05). Conclusions:Early postoperative feeding management at the right time reduces the incidence and severity of postoperative nausea and vomiting inknee arthroplasty patients. In addition, it can also reduce the incidence of thirst and hunger, and improve patients′ postoperative physiological and psychological comfort, which has a facilitating effect on achieving rapid recovery of patients.

ABSTRACT

Objective@#This study aimed to find out the myopia rate and related ocular biological parameters of grade 1-3 students in Peixian, Xuzhou City, and to establish a cohort observation population, so as to provide baseline reference for control strategies for myopia and reduce the myopia rate.@*Methods@#A total of 1 952 students from two nine year schools in Peixian were selected by stratified cluster sampling in June 2020. Visual examination was administered by trained personnel using uniformly equipped instruments, including uncorrected visual acuity, diopter, intraocular pressure, axial length.@*Results@#The overall myopia rate of students from grade one to grade three in two schools in Peixian was 21.77%, and the myopia rate of girls (23.81%) was significantly higher than that of boys (20.16%). With the growth of age and grade, the myopia rate increased continuously, and the difference was statistically significant ( χ 2=134.10, 144.30, P <0.05). The myopia rate of 6-year-old group was 8.87 %. The axial length of the left and right eyes of the myopic group was significantly higher than that of the non myopic group ( t = 411.95, 366.85, P <0.01), the intraocular pressure and mean corneal curvature of the left eye of the myopic group were higher than those of the nonmyopic group ( t =5.91, 10.22 , P <0.05).@*Conclusion@#The baseline survey results show that the myopia rate of grade 1-3 in Peixian of Xuzhou City is low, but myopia has occurred in the 6-year-old group. With the growth of age and grade, the myopia rate continues to rise. So it is urgent to explore effective and feasible myopia prevention and control measures.

ABSTRACT

Objective To explore the changes of serum interleukin-13 (IL-13) and squarnous cell carcinoma antigen (SccAg) levels in pediatric asthma and its clinical significance.Methods 45 children with pediatric asthma were selected as observation group,45 healthy children at the same time were selected as control group.IL-13 and SccAg levels in the two groups were detected by ELISA.The correlation between the serum IL-13,SccAg levels and pediatric asthma was analyzed.Results In the observation group,the IL-13,SccAg levels at the attack stage were (148.96 ± 24.56) ng/L,(3.36 ± 0.69) μg/L,respectively,the IL-13,SccAg levels at the remission stage were (90.65 ± 20.14)ng/L,(2.58 ± 0.34) μg/L,respectively,which in the control group were (76.48 ± 17.54) ng/L,(2.30 ± 0.36) μg/L,respectively,the differences were statistically significant (F =5.36,6.39,all P < 0.05).In the observation group,the IL-13,SccAg levels in the patients with moderate or severe disease were (135.48 ± 21.69)ng/L,(2.86 ± 0.36) μg/L,respectively,the IL-13,SccAg levels in the patients with mild disease were (160.38 ± 22.45) ng/L,(3.30 ±0.41) μg/L,respectively,which in the control group were (76.48 ± 17.54) ng/L,(2.30 ± 0.36) μg/L,respectively,the differences were statistically significant (F =5.78,6.78,all P < 0.05).The serum IL-13 and SccAg levels had significantly positive correlation with pediatric asthma during the attack stage and remission stage (r =0.982,0.965,all P < 0.05).Conclusion The serum IL-13 and SccAg levels have certain association with the incidence of asthma,which play an important role in determining the development and reorientation of asthma.

ABSTRACT

Objective To evaluate the relationship between macrophage migration inhibitory factor ( MIF) expression and obese-induced abolition of sevoflurane preconditioning-induced cardioprotection in mice. Methods Forty-eight male C57BL∕6J mice, aged 4 weeks, were divided into 2 groups ( n=24 each) using a random number table method: normal diet group ( Lean group ) and high-fat diet group ( Obese group) . Lean group were fed a normal diet ( 10% kcal) for 12 weeks, while Obese group were fed a high-fat diet ( 60% kcal) for 12 weeks. The weight of mice was measured. Blood samples were collected from the tail vein for determination of blood glucose concentrations, and plasma concentrations of total cho-lesterol, triglyceride, insulin and leptin. After measurement of the parameters mentioned above, Lean group and Obese group were divided into 3 subgroups ( n=8 each) using a random number table method:sham operation groups (L-Sham group, O-Sham group), myocardial ischemia-reperfusion groups (L-IR group, O-IR group) and sevoflurane preconditioning groups (L-IR+Sev group, O-IR+Sev group). The mice were anesthetized and their hearts were immediately removed and retrogradely perfused in a Langendorff apparatus with an oxygenated K-H solution at 37 ℃. Hearts were continuously perfused with K-H solution for 115 min in L-Sham and O-Sham groups. Hearts were subjected to global ischemia for 25 min, followed by 60-min reperfusion after being retrogradely perfused with K-H solution in L-IR and O-IR groups. In L-IR+Sev and O-IR+Sev groups, hearts were subjected to 3 cycles of 5-min perfusion with sevoflurane-contai-ning K-H solution ( final concentration 0. 6 mmol∕L) and 5-min washout, and then hearts were subjected to global ischemia for 25 min, followed by 60-min reperfusion. Left ventricular developed pressure ( LVDP ) , left ventricular end-diastolic pressure ( LVEDP ) , and the maximum rate of increase or decrease in left ventricular pressure ( ±dp∕dtmax) were recorded at the end of reperfusion. Hearts were obtained at the end of reperfusion for determination of myocardial infarct size and expression of MIF ( by Western blot) . Results Compared with Lean group, the weight, blood glucose, levels of plasma total cholesterol, tri-glyceride, insulin and leptin were significantly increased in Obese group (P<0. 05). Compared with L-Sham group, the LVDP and +dp∕dtmax were significantly decreased, LVEDP and -dp∕dtmax were in-creased, myocardial infarct size was increased, and the expression of myocardial MIF was up-regulated in L-IR and L-IR+Sev groups, and the expression of myocardial MIF was up-regulated in O-Sham group ( P<0. 05) . Compared with L-IR group, LVDP and +dp∕dtmax were significantly increased, LVEDP and-dp∕dtmax were decreased, myocardial infarct size was decreased, and the expression of myocardial MIF was up-regulated in group L-IR+Sev, and the expression of myocardial MIF was significantly up-regulated in group O-IR (P<0. 05). Compared with O-Sham group, LVDP and +dp∕dtmax were significantly de-creased, LVEDP and-dp∕dtmax were increased, and myocardial infarct size was increased, and no signif-icant change was found in the expression of MIF in O-IR and O-IR+Sev groups ( P>0. 05) . Conclusion The mechanism by which obese abolishes sevoflurane preconditioning-induced cardioprotection may be relat-ed to inducing MIF over-expression in mice.

ABSTRACT

Objective To investigate the predictive value of platelet parameters on distant metastasis of different clinical stages of non‐small cell lung cancer (NSCLC) .Methods A retrospective analysis was performed on the inpatients with lung cancer in our hospital from April 2013 to January 2014 .The parameters of platelet count (PLT ) ,mean platelet volume (MPV) ,platelet distribu‐tion width (PDW) ,platelet ‐large cell ratio(P‐LCR) ,platelet hematocrit (PCT ) ,etc .were detected respectively .Results The re‐sults showed that PLT ,MPV ,P‐LCR and PCT in the lung cancer group were higher than those in the benign lung tumor group and the healthy control group ,the differences were statistically significant (P 0 .05) .With the TNM stage increase ,the increased platelet proportion in the stage Ⅲ and Ⅳ was sig‐nificantly higher than that in the stage Ⅰ and Ⅱ (32 .35% ,42 .85% vs .19 .35% ,22 .85% ,P 0 .05) ;PLT ,MPV and PCT in the stage Ⅳ had statisti‐cal difference compared with those in the stage Ⅲ (P< 0 .05) .Conclusion The patients with lung cancer often stimulate the bone marrow megakaryocytes to produce platelets by a particular mechanism .As the staging increase ,the proportion of increased platelet is more obvious .Continuously and dynamically observing platelet parameters is conducive to judge the development and prognosis of the disease .

ABSTRACT

OBJECTIVE:To observe the clinical efficacy and safety of Guizhi fuling capsule combined with Tamoxifen citrate tablet in the treatment of breast hyperplasia. METHODS:216 female patients with breast hyperplasia were randomly divided into control group and observation group. Control group was orally treated with Tamoxifen citrate tablet 10 mg,bid. Observation group was additionally orally treated with Guizhi fuling capsule 3 granules,tid. The treatment course was 3 months. Drug withdrawal was conducted in menstrual period. The clinic data was observed,including clinical efficacy,the maximum mass diameter and breast pain scores before and after treatment,and incidence of adverse reactions. RESULTS:The total effective rate in observation group was significantly higher than control group,the difference was statistically significant(P0.05). CONCLUSIONS:Guizhi fuling capsule combined with Tamoxifen citrate tablet has more significant efficacy than only Tamoxifen citrate tablet in the treatment of breast hyperplasia,with similar safety.

ABSTRACT

Objective To evaluate diagnostic value and clinical significance of procalcitonin (PCT)in infection in intensive care unit (ICU)patients.Methods 96 ICU patients in a hospital between September 2011 and March 2012 were selected for study,levels of patients’PCT,high-sensitivity C-reactive protein (HsCRP)and white blood cell (WBC)count were detected,statistical analysis were conducted.Results Compared with non-bacteria infected patients,serum PCT and HsCRP levels in all bacteria infected patients increased,the difference were significant (Z=-6.102;-3.918,both P 0.05).PCT sensi-tivity,specificity,positive predictive value,and negative predictive value for diagnosing infection was 82.35%, 67.86%,86.15%,and 61 .29% respectively;receiver operating characteristic (ROC)curve of PCT,HsCRP,and WBC was 0.898,0.755,and 0.581 respectively.Conclusion There are higher sensitivity and specificity of PCT to predict infection,which is helpful for early detection of infection in critically ill patients.

ABSTRACT

Objective To investigate the current status of moral distress in clinical nurses,and analyze its influential factors.Methods A total of 240 clinical nurses were recruited by stratified sampling method.They were investigated with general situation questionnaire and moral distress scale.Results The average score of moral distress was (2.66±0.41).Multiple linear regression analysis showed that department of work and qualification were influencing factors on moral distress of clinical nurses.Conclusions This study found that nurses' moral distress was above the middle level.Some targeted measures should be taken by nursing managers,in order to help decrease nurses' moral distress.

ABSTRACT

OBJECTIVE@#To compare the effect of 7 antipsychotic drugs on the life quality of schizophrenia patients including chlorpromazine, sulpiride, clozapine, risperidone, olanzapine, quetiapine, and aripiprazole.@*METHODS@#A total of 1,227 stable schizophrenic patients within 5 years onset who took 1 of the 7 study medications as maintenance treatment were followed up for 1 year at 10 China sites. Patients were evaluated by the short form-36 health survey (SF-36) at the baseline and at the end of 1 year.@*RESULTS@#The life quality was improved obviously at the end of the follow-up. There was significant difference in body pain, vitality, and mental health (P<0.05) among these antipsychotic drugs.@*CONCLUSION@#All 7 antipsychotic drugs can improve the life quality of schizophrenia patients. Atypical antipsychotic drugs, especially olazapine and quetiapine, are superior to typical antipsychotic drugs in improving life quality.
