ABSTRACT

Interleukin 17 (IL-17) inhibitors are approved for treating psoriasis, psoriatic arthropathy, and ankylosing spondylitis. IL-17 is involved in the pathogenesis of inflammatory bowel disease (IBD); however, paradoxical events have been reported using selective IL-17 inhibitors such as secukinumab, whose pathophysiological mechanisms have not been fully clarified. Although the incidence of IBD in this group of patients is low, the risk could be reduced by carefully assessing risk factors such as family history, gastrointestinal symptoms, and fecal calprotectin before starting treatment.


Los inhibidores de interleucina 17 (IL-17) se encuentran aprobados para el tratamiento de psoriasis, artropatía psoriásica y espondilitis anquilosante. La IL-17 se encuentra involucrada en la patogenia de la enfermedad inflamatoria intestinal (EII); sin embargo, hasta la fecha se han reportado eventos paradójicos con el uso inhibidores selectivos de IL-17 como el secukinumab, cuyos mecanismos fisiopatológicos no han sido del todo aclarados. Aunque la incidencia de EII en este grupo de pacientes es baja, el riesgo podría disminuirse mediante una evaluación cuidadosa de factores de riesgo tales como historia familiar, síntomas gastrointestinales y la realización de calprotectina fecal previo al inicio del tratamiento.

Abstract Background Kidney disease is a rare manifestation of ankylosing spondylitis (AS) and its pathological alterations remain poorly described. The aim of this study was to investigate the clinical presentation and pathological alterations on kidney biopsy of AS patients and review and discuss the current literature on the issue. Methods: We retrospectively studied the clinical presentation and kidney pathological alterations of 15 Caucasian AS patients submitted to kidney biopsy between October 1985 and March 2021. Results: Patients were predominantly male (66.7%) with median age at the time of kideney biopsy of 47 years [IQR 34 - 62]. Median serum creatinine at presentation was 1.3 mg/dL [IQR 0.9 - 3] and most patients also had either proteinuria (85.7%) and/or hematuria (42.8%). The most common indication for kidney biopsy was nephrotic syndrome (33.3%), followed by acute or rapidly progressive kidney injury (20%) and chronic kidney disease of unknown etiology (20%). Chronic interstitial nephritis (CIN) (n=3) and AA amyloidosis (n=3) were the most common diagnosis. Others included IgA nephropathy (IgAN) (n=2), focal segmental glomerulosclerosis (n=2), membranous nephropathy (n=1), and immune complex-mediated membranoproliferative glomerulonephritis (IC-MPGN)(n=1). Conclusions: We present one of the largest series of biopsy-proven kidney disease in Caucasian AS patients. We found a lower prevalence of IgAN than previously reported in Asian cohorts. We found a higher prevalence of CIN and a lower prevalence of AA amyloidosis than that described in previous series of Caucasian patients. We also present the first case of AS-associated IC-MPGN.


Resumo Antecedentes: A doença renal é uma manifestação rara de espondilite anquilosante (EA) e as suas alterações patológicas permanecem pouco descritas. O objetivo deste estudo foi investigar a apresentação clínica e alterações patológicas na biópsia renal de doentes com EA bem como rever e discutir a literatura atual sobre o assunto. Métodos: Estudamos retrospectivamente a apresentação clínica e alterações patológicas renais de 15 doentes caucasianos com EA submetidos a biópsia renal entre Outubro de 1985 e Março de 2021. Resultados: Os doentes eram predominantemente homens (66,7%) com idade mediana no momento da biópsia de 47 anos [IIQ 34 - 62]. A creatinina sérica mediana na apresentação foi de 1,3 mg/dL [IIQ 0,9 - 3] e a maioria dos pacientes apresentava também proteinúria (85,7%) e/ou hematúria (42,8%). A indicação mais comum para biópsia renal foi a síndrome nefrótica (33,3%), seguida de lesão renal aguda ou rapidamente progressiva (20%) e doença renal crónica de etiologia desconhecida (20%). A Nefrite intersticial crónica (NIC) (n=3) e a amiloidose AA (n=3) foram os diagnósticos mais comuns. Outros incluíram nefropatia por IgA (NIgA) (n=2), glomeruloesclerose segmentar focal (n=2), nefropatia membranosa (n=1) e glomerulonefrite membranoproliferativa mediada por imunocomplexos (GNMP-IC) (n=1). Conclusões: Apresentamos uma das maiores séries de doenças renais comprovadas por biópsia em doentes caucasianos com EA. Encontramos uma prevalência de NIgA menor do que a relatada anteriormente em coortes asiáticas. Encontramos uma maior prevalência de NIC e uma prevalência menor de amiloidose AA do que a descrita em séries anteriores de pacientes caucasianos. Também apresentamos o primeiro caso de GNMP-IC associada à EA.

ABSTRACT

La espondilitis anquilosante es una enfermedad sistémica caracterizada por inflamación del esqueleto axial, las grandes articulaciones periféricas y los dedos, así como dolor, rigidez de la espalda y manifestaciones a otros niveles del organismo como puede ser en el ojo. Se presenta el caso de una paciente de 46 años de edad que acudió al Hospital Gustavo Aldereguía Lima, de Cienfuegos, por dolor lumbar y sintomatología urinaria refractaria a terapéutica habitual, por lo cual fue ingresada para estudio. Fue evaluada según su cuadro clínico y a través del empleo de exámenes complementarios, lo que permitió el diagnóstico de una espondilitis anquilosante. Se le impuso tratamiento con antinflamatorios no esteroideos y fue dada de alta médica con seguimiento por su área de salud. Por lo importante que resulta confirmar el diagnóstico temprano, antes de que ocurran deformidades irreversibles y así evitar secuelas e impotencia en los pacientes que la padecen, con lo cual se les garantiza una mayor calidad de vida, se decidió la presentación de este caso.


Ankylosing spondylitis is a systemic disease characterized by inflammation of the axial skeleton, large peripheral joints, and fingers, as well as pain, stiffness of the back, and manifestations at other levels of the body, such as the eye. We present the case of a 46-year-old patient who attended the Gustavo Aldereguía Lima Hospital, in Cienfuegos, due to lumbar pain and urinary symptoms refractory to usual therapy, for which she was admitted for study. She was evaluated according to her clinical picture and through the use of complementary tests, which allowed the diagnosis of ankylosing spondylitis. Treatment with non-steroidal anti-inflammatory drugs was prescribed and she was discharged with follow-up by her health area. Due to the importance of confirming the early diagnosis, before irreversible deformities occur and thus avoiding sequelae and impotence in patients who suffer from it, to guarantee them a better quality of life, it was decided to present this case.

ABSTRACT

Las espondiloartropatías son un grupo de enfermedades reumáticas que se caracterizan por la presencia de un proceso inflamatorio crónico y mantenido. Entre sus manifestaciones clínicas y complicaciones se incluye la toma de la columna vertebral donde existe una calcificación de los ligamentos vertebrales que limita considerablemente la movilidad de los distintos segmentos de la columna vertebral. Los cambios anatomofuncionales que se generan en estos segmentos traen consigo dificultades a la hora de realizar procedimientos como la punción lumbar. La presente investigación tiene como objetivo describir las precauciones, sugerencias y recomendaciones para realizar la punción lumbar con fines diagnósticos o terapéuticos. Los principales resultados se enfocan en la idenficación y socialización de precauciones y sugerencias a tener en cuenta a la hora de realizar una punción lumbar en pacientes con espondilitis anquilosante(AU)


Spondyloarthropathies are a group of rheumatic diseases characterized by the presence of a chronic and sustained inflammatory process. Among its clinical manifestations and complications, the seizure of the spine is included where there is calcification of the vertebral ligaments that considerably limits the mobility of the different segments of the spine. The anatomical and functional changes that are generated in these segments bring with them difficulties when performing procedures such as lumbar puncture. This research aims to describe the precautions, suggestions and recommendations to perform lumbar puncture for diagnostic and/or therapeutic purposes. The main results focus on the identification and socialization of precautions and suggestions to take into account when performing a lumbar puncture in patients with ankylosing spondylitis(AU)

Introducción: La espondilitis anquilosante es una artropatía que se encuentra dentro del grupo de artropatías seronegativas, siendo la entidad más común y representativa de ellas. Se caracteriza por inflamación crónica del esqueleto axial, con dolor de espalda de tipo inflamatorio y rigidez progresiva. Típicamente se manifiesta en pacientes jóvenes que inician su sintomatología entre los 20 y 30 años. La osificación progresiva en la espondilitis anquilosante envuelve los cartílagos articulares y discos de la columna vertebral, con el desarrollo de puentes óseos entre las vértebras. Esta fusión hace difícil, sino imposible, el abordaje axial para lograr una anestesia epidural o espinal, e incluso una dificultad aumentada del manejo de la vía aérea por la fusión de las vértebras cervicales. Caso clínico: En este reporte de caso se presenta una paciente femenina de 40 años, ya con diagnóstico de espondilitis anquilosante, que acude para una histerectomía total abdominal, que en primera instancia se intenta un abordaje neuro axial fallido y posteriormente se procede al uso de anestesia general con manejo de la vía aérea. Conclusión: La intubación en pacientes despiertos, es un tema ampliamente descrito en la literatura el cual se puede realizar de distintas maneras, en nuestro caso optamos por el uso de bloqueos de la vía aérea superior con posterior uso de fibroscopio para asegurar la intubación endotraqueal. Introduction: Ankylosing spondylitis is an arthropathy that falls within the group of seronegative arthropathies, being the most common and representative entity of them. It is characterized by chronic inflammation of the axial skeleton, with inflammatory back pain and progressive stiffness. It typically manifests in young patients who begin to show symptoms between 20 and 30 years of age. Progressive ossification in ankylosing spondylitis involves the articular cartilage and discs of the spine, with the development of bony bridges between the vertebrae. This fusion makes the axial approach difficult, if not impossible, to achieve epidural or spinal anesthesia, and even increases the difficulty of airway management due to the fusion of the cervical vertebrae. Case report: This case report presents a 40-year-old female patient, already diagnosed with ankylosing spondylitis, who underwent a total abdominal hysterectomy, where a failed neuraxial approach was first attempted and then general anesthesia with airway management was used. Conclusion: Intubation in awake patients is a subject widely described in the literature which can be performed in different ways, in our case we opted for the use of upper airway blocks with subsequent use of fiberscope to ensure endotracheal intubation. ABSTRACT Background: The terms Spondyloarthritis and spondyloarthropathy (Spa) are used to define a group of diseases with related clinical characteristics and genetics. Aim: To report the clinical and demographic characteristics of ankylosing spondylitis (AS) and undifferentiated spondyloarthritis (USpA) and to evaluate the frequency of cyclic citrullinated peptide antibody (anti-CCP) positivity. Material and Methods: Two hundred patients with USpA or AS, 100 control patients with a diagnosis of rheumatoid arthritis (RA) and 100 healthy volunteers were included. For each patient, their detailed medical histories, physical examination, whole blood counts, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), anti-CCP, routine biochemical tests, and HLA-B27 test results were evaluated. ASDAS and BASDAI scores and morning stiffness were used to evaluate the disease activity. Results: The presenting symptom of 73 (73%) patients in the AS group and 58 (58%) patients in the USpA group was pain in axial joints. A family history of Spa was positive in 32 patients from both groups (32%). A positive HLA-B27 was found in 55% of the AS group and 25% of the USpA group (p < 0.01 for the difference between groups). The frequency of positive HLA-B27 was significantly higher in individuals with a family history of SpA (p = 0.022). A positive Anti-CCP was found in 56% of the RA group, a significantly higher frequency compared with other groups (p < 0.001). The frequency of positive Anti-CCP in patients in AS (9%) and USpA (6%) was significantly higher than in healthy controls (p < 0.001). Conclusions: The frequency of anti-CCP positivity was higher in SpA patients than in healthy controls.


Introducción: Los términos espondiloartritis y espondiloatropatia (Spa) se usan para definir un grupo de enfermedades con características y genética relacionadas. Objetivo: Informar las características clínicas y demográficas de la espondilitis anquilosante (EA) y espondiloartritis indiferenciada (USpA) - y para evaluar la frecuencia de positividad del anticuerpo péptido citrulinado cíclico (anti-CCP). Material y Métodos: En este estudio observacional se incluyeron doscientos pacientes con USpA y EA, 100 pacientes control con diagnóstico de artritis reumatoide (AR) y 100 voluntarios sanos. Se evaluó la historia clínica, exámen físico, recuentos sanguíneos completos, velocidad de sedimentación globular (ESR), proteína C reactiva (PCR), anti-CCP, pruebas bioquímicas de rutina y resultados de la prueba HLA-B27. Para evaluar la actividad de la enfermedad se utilizaron las puntuaciones ASDAS y BASDAI y la rigidez matutina. Resultados: El síntoma inicial de 73 (73%) pacientes en el grupo de EA y 58 (58%) pacientes en el grupo de USpA fue dolor en las articulaciones axiales. Treinta y dos pacientes de cada grupo (32%) tenían antecedentes familiares de SPA. HLA-B27 fue positivo en el 55% del grupo AS y el 25% del grupo USpA con una diferencia significativa entre los dos grupos (p < 0.001). La frecuencia de positividad HLA-B27 fue mayor en individuos con historia familiar de SpA (p = 0,02). Se encontraron anti-CCP positivos en el 56% del grupo con AR, una frecuencia significativamente mayor en comparación con otros grupos (p < 0,01). La frecuencia de anti-CCP positivo fue mayor los pacientes con AS (9%) y USpA (6%) que en el grupo sano (p < 0,001). Conclusiones: La frecuencia de positividad anti-CCP fue mayor en los grupos de SpA en comparación con los grupos control sanos.

INTRODUCCIÓN. La Espondilitis Anquilosante compromete la calidad de vida del pacien-te por tratarse de una enfermedad de afectación axial, ocular, gastrointestinal y articular discapacitante que limita actividades básicas de la vida diaria que repercute en su entorno social. OBJETIVO. Evaluar la calidad de vida, funcionalidad y actividad de la enfermedad en pacientes con Espondilitis Anquilosante. MATERIALES Y MÉTODOS. Estudio trans-versal analítico, con una poblacion de 166 pacientes, una muestra de 120 con diagnós-tico de Espondilitis Anquilosante de la Unidad Técnica de Reumatología del Hospital de Especialidades Carlos Andrade Marín. Se aplicó un cuestionario estructurado que midió la actividad fisica, presencia del Antígeno Leucocitario Humano B-27, se utilizó escalas va-lidadas a nivel internacional que evaluaron calidad de vida, funcionalidad y actividad de la enfermedad. El análisis univariado, bivariado y multivariado, se calculó con Chi-cuadrado y Odds Ratios en el programa estadistico SPSS 23.0. RESULTADOS. Se observó un 50% (60; 120) de ligera afectación en calidad de vida, en funcionalidad el 74,2% (89; 120) pre-sentó afectación mientras que en actividad de la enfermedad en la encuesta Ankylosing Spondylitis Disease Activity Score se encontró actividad alta con un 43,3% (52; 120) y en Bath Ankylosing Spondylitis Disease Activity Index 55% (66; 120) de enfermedad activa. El tratamiento combinado presentó mayor riesgo para afectación en la funcionalidad, en actividad de la enfermedad baja, alta y muy alta con valor p= 0,022; 0,014; 0,026 de forma respectiva. CONCLUSIÓN. La calidad de vida se vio afectado en mujeres y quienes no realizaron actividad física; se encontró comprometido la funcionalidad en quienes recibie-ron tratamiento combinado.


INTRODUCTION. Ankylosing Spondylitis compromises the quality of life of the patient as it is a disease of axial, ocular, gastrointestinal and articular disabling affectation that limits basic activities of daily life that affects the social environment. OBJECTIVE. To evaluate the quality of life, functionality and activity of the disease in patients with Ankylosing Spond-ylitis. MATERIALS AND METHODS. Analytical transversal study, with a population of 166 patients, a sample of 120 with a diagnosis of Ankylosing Spondylitis from the "Unidad Téc-nica de Reumatología del Hospital de Especialidades Carlos Andrade Marín". A structured questionnaire was applied to measure physical activity, presence of Human Leukocyte Antigen B-27, using internationally validated scales that evaluated quality of life, functio-nality and disease activity. Univariate, bivariate and multivariate analysis was calculated with Chi-square and Odds Ratios in the statistical program SPSS 23.0. RESULTS. A 50% (60; 120) of slight affectation in quality of life was observed, in functionality 74,2% (89; 120) presented affectation while in disease activity the Ankylosing Spondylitis Disease Activity Score found high activity with 43,3% (52; 120) and in Bath Ankylosing Spondylitis Disease Activity Index 55% (66; 120) of active disease. Combined treatment presented a greater risk of affecting functionality, low, high and very high disease activity with p values of 0,022; 0,014 and 0,026, respectively. CONCLUSION. Quality of life was affected in women and those who did not carry out physical activity; functionality was found to be compromised in those who received combined treatment.

Objetivo: Avaliar pacientes com fibromialgia e dor lombar, bus- cando características clínicas de espondiloatrites axiais. Méto- dos: Neste estudo transversal, cem pacientes com fibromialgia e idades entre 18 e 65 anos foram avaliados. A avaliação consis- tiu em um questionário baseado no braço clínico dos critérios Assessment of Spondyloarthritis International Society para diag- nóstico de espondiloatrites axiais e um questionário de impacto da fibromialgia (Revised Fibromyalgia Impact Questionnaire). Re- sultados: Quando o braço clínico da Assessment of Spondyloar- thritis International Society foi aplicado em pacientes com fibro- mialgia e dor lombar, 80% dos pacientes preencheram os critérios para diagnóstico de espondiloatrites axiais. As principais carac- terísticas de espondiloatrite axial foram dor lombar inflamatória, boa resposta a drogas anti-inflmatórias não esteroidais, artrite e entesite. Pacientes com critérios de espondiloatrites axiais posi- tivos apresentaram maiores níveis séricos de proteína C-reativa (p=0,00035). O valor médio do Revised Fibromyalgia Impact Ques- tionnaire para todos os pacientes avaliados foi 63,6, e não houve di- ferença entre os pacientes que preencheram os critérios e aqueles que não preencheram. Conclusão: Pacientes com fibromialgia e dor lombar frequentemente apresentam características clínicas de espondiloatrite axial, e aqueles que preencheram os crité- rios clínicos para espondiloatrites axiais também apresentaram maiores níveis séricos de proteína C-reativa.


Objective: To evaluate patients with fibromyalgia and back pain for clinical features of axial spondyloarthritis. Methods: In this cross-sectional study, one hundred fibromyalgia patients between 18 and 65 years old were assessed. The assessment consisted of a questionnaire based on the clinical arm ofthe Assessment of Spondyloarthritis International Society criteria for the diagnosis of axial spondyloarthritis and a questionnaire on the impact of fibromyalgia (Revised Fibromyalgia Impact Questionnaire). Re- sults: When the clinical arm of the Assessment of Spondyloarthri- tis International Society was applied in patients with fibromyal- gia and back pain, 80% of the patients met the clinical criteria for diagnosis of axial spondyloarthritis. The main features of axial spondyloarthritis in fibromyalgia patients were inflammatory back pain, good response to non-steroidal anti-inflammatory drugs (NSAIDs), arthritis, and enthesitis. Patients with positive axial spondyloarthritis criteria presented with higher C- reac- tive protein levels (p=0.00035). The mean value of the Revised Fibromyalgia Impact Questionnaire for all patients assessed was 63.6 and there was no difference between patients that met the criteria for axial spondyloarthritis and those who did not. Con- clusion: Patients with fibromyalgia and back pain often present clinical features of axial spondyloarthritis, and those who met the clinical criteria for axial spondyloarthritis also presented with higher levels of C-reactive protein.

ABSTRACT Objective We aim to identify retrospectively surgically treated patients with an ankylosed spine who sustained a vertebral fracture. Our goal is to evaluate the main outcomes and complications. Methods We selected patients through the database of surgical interventions in the setting of fractures of an ankylosed spine segment between January 1st 2008 and June 30th 2018. We collected data from digital medical records. The parameters analyzed include hospital length of stay, Intensive Care Unit (ICU) admission, perioperative and postoperative complications as well as neurological evolution. Results Fractures occurred in 14 patients with ankylosing spondylitis (82%) and 3 patients with diffuse idiopathic skeletal hyperostosis (18%). All patients were male and the mean age was 69 years. Fourteen fractures occurred after minor trauma (83%), of which 11 were due to falls from standing height or lower (65%). The cervical spine represents the majority of the levels involved (65%). Seven patients were admitted to the ICU (41%) and 11 suffered neurological damage. There was improvement of neurological status in less than 50% and there were high percentages of post-operative complications. Conclusion Patients with ankylosed spine diseases are at higher risk for vertebral fracture, even after minor trauma, and these are located predominantly in the cervical spine. The surgical treatment of these conditions is effective as it allows improvement of the patient's neurological status. However, they still present higher morbidity and mortality, as well as increased post-op complications. Prevention of falls may drastically change patients' outcome, neurological function and independence in activities of daily living. Level of evidence IV; A case series therapeutic study.


RESUMO Objetivo Identificar retrospetivamente casos de tratamento cirúrgico de fraturas vertebrais em pacientes com coluna anquilosada. O nosso propósito consiste em avaliar os principais desfechos e respectivas complicações. Métodos Selecionamos pacientes através do banco de dados de intervenções cirúrgicas no quadro de fraturas de um segmento da coluna anquilosada entre 1 de janeiro de 2008 a 30 de junho de 2018. Coletamos os dados a partir dos prontuários médicos digitais. Os parâmetros analisados incluem período de internação hospitalar, admissão na Unidade de Tratamento Intensivo (UTI), complicações pré- e pós-operatórias, assim como evolução neurológica. Resultados As fraturas ocorreram em 14 pacientes com espondilite snquilosante (82%) e em 3 pacientes com hiperostose esquelética difusa idiopática (18%). Todos os pacientes eram do sexo masculino e a idade média era de 69 anos. Quatorze fraturas ocorreram devido a trauma menor (83%), das quais 11 eram devido a quedas da própria altura ou inferiores (65%). A coluna cervical representa a maioria dos níveis envolvidos (65%). Sete pacientes foram admitidos na UTI (41%) e 11 sofreram lesão neurológica. Houve melhoria do estado neurológico em menos de metade dos pacientes e altas porcentagens de complicações pós-operatórias. Conclusão Os pacientes com doenças da coluna anquilosada têm maior risco de fraturas vertebrais, mesmo após trauma menor, localizando-se predominantemente na coluna cervical. O tratamento cirúrgico dessas condições é eficaz, uma vez que permite melhora do estado neurológico do paciente. Entretanto, ainda apresentam altos índices de morbilidade e mortalidade, assim como maior incidência de complicações pós-operatórias. A prevenção de quedas pode alterar drasticamente o desfecho, função neurológica e independência nas atividades diárias do paciente. Nível de evidência IV; Estudo terapêutico de série de casos.


RESUMEN Objetivo Identificar retrospectivamente casos de tratamiento quirúrgico de fracturas vertebrales en pacientes con columna anquilosada. Nuestro propósito consiste en evaluar sus principales resultados y respectivas complicaciones. Métodos Seleccionamos pacientes mediante banco de datos de intervenciones quirúrgicas en el cuadro de fracturas de un segmento de la columna anquilosada entre el 1 de enero de 2008 al 30 de junio de 2018. Recolectamos los datos a partir de los prontuarios médicos digitales. Los parámetros analizados incluyen período de internación hospitalaria, admisión en la Unidad de Tratamiento Intensivo (UTI), complicaciones pre y postoperatorias, así como evolución neurológica. Resultados Las fracturas ocurrieron en 14 pacientes con espondilitis anquilosante (82%) y en 3 pacientes con hiperostosis esquelética difusa idiopática (18%). Todos los pacientes eran del sexo masculino y la edad promedio era de 69 años. Catorce fracturas ocurrieron debido a trauma menor (83%), de las cuales 11 eran debido a caídas de la propia altura o inferiores (65%). La columna cervical representa la mayoría de los niveles implicados (65%). Siete pacientes fueron admitidos en la UTI (41%) y 11 sufrieron lesión neurológica. Hubo mejora del estado neurológico en menos de la mitad de los pacientes y altos porcentajes de complicaciones postoperatorias. Conclusión Los pacientes con enfermedades de la columna anquilosada tienen mayor riesgo de fracturas vertebrales, incluso después de trauma menor, localizándose predominantemente en la columna cervical. El tratamiento quirúrgico de esas condiciones es eficaz, ya que permite mejora del estado neurológico del paciente. Entretanto, aún presentan altos índices de morbilidad y mortalidad, así como mayor incidencia de complicaciones postoperatorias. La prevención de caídas puede alterar drásticamente los resultados, función neurológica e independencia en las actividades diarias del paciente. Nivel de evidencia IV; Estudio terapéutico de serie de casos.

ABSTRACT Renal manifestations of spondyloarthritis are rare. The case is presented of a patient with ankylosing spondylitis of nine years of evolution. He had intense inflammation, as well as articular sequelae, with a significant deterioration in his quality of life. At the initial evaluation, the patient had a serum creatinine of 1.44mg/dL and a 24-h urine protein in the sub-nephrotic range (1.44 g). Renal biopsy showed the presence of Congophilic material, confirming the diagnosis of AA amyloidosis. Treatment with a TNF blocking agent was initiated with clinical improvement, especially regarding articular disease.


RESUMEN Las manifestaciones renales de la espondiloartritis son poco comunes. Se presenta el caso de un paciente con espondilitis anquilosante de 9 anos de evolución, con intensa inflamación y secuelas articulares y con un deterioro significativo en su calidad de vida. En la evaluación inicial, el paciente tenía una creatinina sérica de 1,44mg/dL y una proteína en orina de 24 horas en un rango subnefrótico (1,44 g). La biopsia renal mostró la presencia de material congofílico que confirmaba el diagnóstico de amiloidosis AA. Se inició tratamiento con un anti-TNF, con mejoría clínica, especialmente con respecto al componente articular.

ABSTRACT

Objetivo: O Planserv oferece cobertura à terapia biológica para as patologias de artrite reumatoide (AR), espondilite anquilosante (EA) e artrite psoriática (AP). Em agosto de 2016, 78 pacientes estavam em uso do medicamento Remicade® (infliximabe). Nessa data, o valor do Remicade® foi reduzido para o mesmo valor do Remsima™ (infliximabe biossimilar). Com isso, todos pacientes que estavam usando o Remicade® trocaram por Remsima™. Conduzimos um estudo para medir a descontinuidade da terapia e a economia. Métodos: Estudo de mundo real (coorte prospectiva), não controlado, em pacientes com AR, AP e EA que estavam utilizando Remicade® e trocaram para Remsima™, entre setembro de 2016 e setembro de 2017. O desfecho primário foi o índice de descontinuidade do tratamento (por qualquer causa). O desfecho secundário foi a taxa de "aumento da atividade da doença", medida por meio dos escores SDAI, BASDAI e CASPAR. Foi considerado como "aumento da atividade da doença" qualquer medida superior à medida inicial e que estivesse acima do limite de remissão da doença. Os valores de referência para "aumento da atividade da doença" foram as medidas históricas. O impacto econômico foi medido por uma análise de custo-minimização. Resultados: Em setembro de 2017, 5 (6%) pacientes que realizaram a troca do Remicade® para o Remsima™, descontinuaram a terapia (4 por falhas e 1 perda de acompanhamento). A taxa de descontinuação de referência (Remicade®) foi de 11% (9% de falha e 2% por perda de acompanhamento). As análises de subgrupo (descontinuidade da terapia por tipo de patologia) foram equivalentes. A taxa de "aumento da atividade da doença" ocorreu em 42% dos pacientes para o Remsima™ e em 46% para o Remicade®. As análises de subgrupo (por tipo de patologia) também demonstraram que as taxas de aumento da atividade da doença foram semelhantes entre os grupos. A análise econômica mostrou que a mudança do Remicade® para o Remsima™ trouxe economia de R$ 1,75 milhão de reais (0,5 milhão de dólares), com 1.689 ampolas de infliximabe dispensadas no período. Conclusão: A troca do medicamento Remicade® pelo Remsima™ nos pacientes com AR, EA e AP, no contexto do Planserv, demonstrou ter sido uma medida segura, eficaz e econômica.


Objective: Planserv offers coverage of biological therapy for rheumatoid arthritis (RA), ankylosing spondylitis (AS) and psoriatic arthritis (PA). In August 2016, 78 patients were on Remicade® (infliximabe). It was as "increased disease activity", any measure higher than the initial, and that was above the remission limit of the disease. The reference values for "increased disease activity" were the historical measures. The economic impact measured by a cost minimization analysis. Results: In September 2017, 5 (6%) patients who switched from Remicade® to Remsima™, discontinued therapy (4 due to failure and 1 loss of follow-up). The reference discontinuation rate (Remicade®) was 11% (9% failure and 2% loss of follow-up). Subgroup analyzes (discontinuation of therapy by type of pathology) were equivalent. The rate of "increased disease activity" occurred in 42% of patients for Remsima™, and 46% for Remicade®. Subgroup analyzes (by type of pathology) also showed that rates of increase in disease activity were similar between groups. Resumen: Se han reportado múltiples casos en la bibliografía de asociación de la enfermedad de Takayasu con espondilitis anquilosante; ambas son enfermedades inflamatorias con componentes autoinmunitarios no muy bien establecidos, motivo por el que comunicamos el caso de un paciente de 35 años de edad, quien había sido diagnosticado y tratado por espondilitis anquilosante confirmada con HLA-B27 positivo, padecía también vitíligo, esta última enfermedad con componente de afección autoinmunitaria. El paciente tenía, una semana antes de su fallecimiento, dolor precordial tratado con vasodilatadores; durante la ingesta de alimentos en un restaurante manifestó súbitamente dificultad respiratoria y palidez tegumentaria, llegó a recibir atención hospitalaria con el diagnóstico presuntivo de intoxicación alimentaria que ocasionó la muerte, por lo que se realizó necropsia que descartó esa posibilidad diagnóstica.


Abstract: In the literature there have been reported multiple cases of persons who have at the same time the diagnosis of Takayasu's arteritis and ankylosing spodylitis, these inflam- matory diseases with inconclusive autoimmune component. This is the reason why in this article we present the case of a man that had been under treatment for ankylosing spondylitis confirmed by HLA-B27, who also had vitiligo. One week before his death, patient suffered precordial pain with treatment at hospital under vasodilator drug. During the intake of food in a restaurant patient had sudden respiratory distress and tegumentary pallor, arriving at hospital care with the presumptive diagnosis of food poisoning that caused death, for which a necropsy was performed and ruled out this diagnostic possibility.

ABSTRACT

A B S T R A C T Objective: The objective of this study was to establish recommendations for the reduction and discontinuation of biological disease-modifying antirheumatic drugs, with the aim of becoming a guide document for health professionals involved in the management of patients with rheumatoid arthritis, ankylosing spondylitis, and psoriatic arthritis. Materials and methods: The recommendations were established through consensus by a panel of experts in rheumatology, and based on the analysis of available scientific evidence obtained from systematic reviews and the clinical experience of the panellists. Results: A total of 33 rheumatoid arthritis related studies were included, with 6 psoriatic arthritis related, and 9 ankylosing Spondylitis related. The recommendations for the reduction of biological therapies were made by establishing a plan to determine when and how to reduce the biological disease-modifying antirheumatic drugs in patients with these 3 diseases, and in some cases lead to the discontinuation of these treatments. Conclusion: The recommendations established in this document will serve as a guide to improve the efficiency of biological therapy in these diseases, reduce the variability in clinical practice, and establish an adequate risk/benefit ratio.


RESUMEN Objetivo: El objetivo de este estudio fue establecer recomendaciones para la disminución y descontinuación de la terapia biológica con el fin de que se convierta en un documento guía para los profesionales de la salud involucrados en el manejo de pacientes con artritis reumatoide, espondilitis anquilosante y artritis psoriásica. Materiales y métodos: Las recomendaciones fueron establecidas mediante consenso desarrollado a través de un panel de expertos en reumatología, basado en el análisis de la evidencia científica disponible obtenida de revisiones sistemáticas y sobre la experiencia clínica de los panelistas. Resultados: Se incluyeron 33 estudios relacionados con artritis reumatoide, 6 de artritis psoriásica y 9 de espondilitis anquilosante. Las recomendaciones para la disminución de las terapias biológicas se realizaron estableciendo un plan para determinar cuándo y cómo reducir los fármacos biológicos modificadores de enfermedades reumáticas en pacientes con estas 3 enfermedades y en algunos casos conducir a la descontinuación de estos tratamientos. Conclusión: Las recomendaciones establecidas en este documento servirán de guía para mejorar la eficiencia de la terapia biológica en estas enfermedades, reducir la variabilidad en la práctica clínica y establecer de manera adecuada una relación riesgo/beneficio.

La espondilitis anquilosante es definida como una espondiloartropatías crónica, inflamatoria e idiopática, que generalmente tiene un comienzo en edades tempranas de la vida. Se plantea que tiene predisposición por el sexo masculino y su principal manifestación clínica es la presencia de un dolor de tipo inflamatorio localizado en la espalda baja y que afecta principalmente la columna lumbar. Sin embargo, puede afectar otras articulaciones como las rodillas, hombros y coxofemorales. La necrosis avascular de la cabeza de fémur es el resultado de la interrupción brusca del aporte sanguíneo al hueso de causas intrínsecas o extrínsecas provocando necrosis y colapso óseo. Se presenta el caso de un paciente joven, de sexo masculino con diagnóstico de espondilitis anquilosante que acude con manifestaciones clínicas y radiológicas que permiten llegar al diagnóstico de una necrosis avascular de la cabeza femoral


Ankylosing spondylitis is defined as chronic, inflammatory and idiopathic spondyloarthropathy, which generally begins at an early age. It is stated that it is predisposed by the male sex and its main clinical manifestation is the presence of a pain of inflammatory type located in the lower back and that mainly affects the lumbar spine. However, it can affect other joints such as the knees, shoulders and coxofemoral joints. The avascular necrosis of the head of the femur is the result of abrupt interruption of the blood supply to the bone of intrinsic or extrinsic causes, causing necrosis and bone collapse. We present the case of a young, male patient diagnosed with ankylosing spondylitis who presents with clinical and radiological manifestations that allow the diagnosis of avascular necrosis of the femoral head.

ABSTRACT

Handling a difficult airway is one of the biggest challenges for an anesthesiologist due to morbidity and mortality that it involves. This is why available devices are permanently reviewed to validate them in the handling of a known difficult airway or previously unknown. Patients with severe ankylosing spondylitis, almost everyone of them, they have a difficult airway due to a stiff neck spine and/or limitation in the mouth opening. In this case report we present a patient with severe ankylosing spondylitis, admitted for a total hip replacement surgery. We followed recommendations for difficult airway management using the devices available in the Anesthesiology Unit of our Hospital. Previous patient consent, we performed an awake intubation using a video laryngoscope and gum elastic bougie, under sedation, succesfully and with no complications.


Manejar una vía aérea difícil es uno de los mayores desafíos para un anestesiólogo debido a la morbilidad y mortalidad que conlleva. Esta es la razón por la cual los dispositivos disponibles se revisan permanentemente para validarlos en el manejo de una vía aérea difícil conocida o desconocida previamente. Los pacientes con espondilitis anquilosante severa, casi todos ellos, tienen una vía aérea difícil debido a una rigidez en la columna cervical y/o limitación en la apertura de la boca. En este caso, presentamos a un paciente con EA grave, admitido para una cirugía de reemplazo total de cadera. Seguimos las recomendaciones para el manejo de la vía aérea difícil usando los dispositivos disponibles en la Unidad de Anestesiología de nuestro Hospital. Con el consentimiento previo del paciente, realizamos una intubación con un laringoscopio de video y goma elástica bougie, bajo sedación, con éxito y sin complicaciones.

ABSTRACT

Abstract The rheumatic diseases, which include systemic lupus erythematosus, rheumatoid arthritis, Behçet's disease, scleroderma, and ankylosing spondylitis, are characterized by involvement of connective tissue, with multiple manifestations. In those diseases, there can be involvement of the peripheral or central nervous system, and that involvement can be primary, presenting as a major feature of the clinical presentation, or secondary, as an effect of the drugs used in order to control a given disease or its complications. Knowledge of the wide variety of imaging findings is crucial to the diagnosis of a rheumatic disease, especially in the early stages, enabling effective treatment and minimizing disability. This pictorial essay, presenting cases from the records of two tertiary teaching hospitals, encompasses cases of patients diagnosed with rheumatic disease and illustrates the neuroradiological findings on magnetic resonance imaging and computed tomography, in order to emphasize the importance of these methods for properly diagnosing rheumatic diseases.


Resumo As doenças reumatológicas, que incluem lúpus eritematoso sistêmico, artrite reumatoide, doença de Behçet, esclerodermia e espondilite anquilosante, são caracterizadas por envolvimento do tecido conjuntivo, com múltiplas manifestações. Nessas doenças, o envolvimento do sistema nervoso central ou periférico pode ser primário, apresentando-se como uma das principais características clínicas, ou secundárias, como efeito das drogas usadas para seu controle. O diagnóstico, especialmente nas fases iniciais, depende do conhecimento de grande variedade de achados em métodos de imagem, permitindo um tratamento eficaz, causando menores deficiências. Este ensaio, com casos de um arquivo didático de dois hospitais terciários, engloba pacientes com diagnóstico de doenças reumatológicas e ilustra os achados neurorradiológicos de ressonância magnética e tomografia computadorizada, a fim de enfatizar a importância desses métodos para o diagnóstico adequado.

ABSTRACT

Contexto: A psoríase é uma doença crônica com taxa de prevalência variável no mundo, e com uma infinidade de opções terapêuticas para o seu tratamento. Para as formas mais graves ou associadas a outras comorbidades, vem se destacando o uso dos imunobiológicos. Dentre estes, os antagonistas do fator de necrose tumoral alfa apresentam grande espectro de uso em todo o território nacional. Descrição do caso: Homem de 36 anos de idade, tabagista, fez uso de infliximabe por dois anos devido a espondilite anquilosante, com insucesso terapêutico, mas sem efeitos adversos. Devido à má resposta clínica, a medicação foi substituída por adalimumabe e, após quatro anos de uso deste imunobiológico, o paciente evoluiu com lesões eritematosas-descamativas encimadas por pústulas estéreis na região palmoplantar. Não tinha história pessoal ou familiar de psoríase, sendo realizado o diagnóstico de efeito paradoxal ao uso da medicação imunobiológica, caso raro. Discussão: Os efeitos adversos dos imunobiológicos incluem infecções, reativação de infecções latentes, insuficiência cardíaca, efeito paradoxal. Este último consiste no aparecimento de psoríase em pacientes que usavam imunobiológicos devido a outras doenças de base; exacerbação de uma psoríase já em tratamento; ou surgimento de novas formas de psoríase. São reações raras, porém causam grande prejuízo na qualidade de vida do paciente, podendo até interferir no tratamento em andamento. Conclusões: Os efeitos paradoxais são divididos entre lesões de novo nos pacientes ou exacerbação de uma patologia previamente existente. Isso dificulta o diagnóstico do efeito adverso e, muitas vezes, retarda o tratamento do paciente. Buscamos, com este relato, alertar os médicos sobre a doença e seu tratamento.

Resumo Objetivo: A uveíte anterior aguda é a principal manifestação extra-articular na espondiloartrite. O objetivo deste estudo foi analisar se a presença da uveíte se associa com diferentes manifestações clínicas, laboratoriais, radiológicas e a terapêutica nos pacientes com espondiloartrite. Métodos: Estudo observacional retrospectivo realizado com 153 pacientes portadores de espondiloartrite atendidos no período de 1997 a 2017 na Grande Florianópolis, Brasil. Foram analisados dados demográficos, laboratoriais, clínicos e do tratamento de pacientes com espondiloartrite em relação a presença ou não de uveíte. Resultados: A uveíte foi encontrada em 26,8% dos pacientes. A presença de complicações foi rara, ocorrendo catarata em somente quatro pacientes e glaucoma em dois deles. Foi observada uma tendência a maior frequência de uveíte anterior aguda no sexo masculino (p=0,06), nos pacientes com história familiar (p=0,19) e HLA-B27 positivos (p=0,14). Pacientes com espondiloartrite e uveíte mais frequentemente usavam anti-TNF (p=0,04) e apresentavam sacroiliite em exames de imagem (p=0,02). Não observou-se associação entre a uveíte e o acometimento cardiovascular (p=0,44), cutâneo (p=0,13) ou gastrointestinal (p=0,10). Conclusão: A uveíte que ocorre em pacientes com espondiloartrite é comum, tem predomínio no sexo masculino e é mais frequente em pacientes com HLA-B27 positivo. O uso de imunobiológicos como o anti-TNF é frequente nos pacientes com uveíte.


Abstract Objective: Acute anterior uveitis (AAU) is the most common extra-articular manifestation of spondyloarthritis. The aim of this study is to analyze if the presence of uveitis is associated with a diferent clinical manifestation, laboratorial, radiological and therapetiuc among spondyloarthritis patients. Methods: This was a observational retrospective study with 153 patients with spondyloarthritis attended in the period from 1997 to 2017 in Florianopolis, Brazil. It was analyzed demografical, laboratorial, clinical and therapeutic data in spondyloarthritis patients with or without uveitis. Results: 26,8% of the patients with spondyloarthritis presented uveitis. The presence of complications was rare, with cataract occurring in only four patients and glaucoma in two of them. A higher frequency of acute anterior uveitis in males (p = 0.06) was observed in patients with a family history (p = 0.19) and HLA-B27 positive (p = 0.14). Patients with spondyloarthritis and uveitis more frequently used anti-TNF (p = 0.04) and presented sacroiliitis on imaging tests (p = 0.02). There was no association between uveitis and cardiovascular (p = 0.44), cutaneous (p = 0.13) or gastrointestinal involvement (p = 0.10). Conclusion: Uveitis in patients with spondylarthritis is common, predominantly in males, and more frequently in HLA-B27 positive patients. The use of immunobiological agents such as anti-TNF is common in patients with uveitis.

SUMMARY INTRODUCTION: Ankylosing spondylitis (AS) is an idiopathic seronegative spondyloartropathy that involves mainly the axial skeleton and the sacroiliac joints. AS promotes biomechanical changes in the spine that predispose to fractures, spinal deformity and spondylodiscitis. The aim of this article is to report the clinical and laboratorial characteristics of patients with AS who underwent spinal surgery at our Institution. METHODS: Retrospective review of medical charts of patients who had AS and underwent spinal interventions. RESULTS: Nine patients were found and eight were included in the present study. There were three men and six women and the patients' mean age was 57 years old. All patients had pain at the involved spinal level and one patient had tetraparesis due to cervical myelopathy. Acute-phase proteins were positive in six patients (75%), and HLA-B27 was found in two patients (25%). Four patients had the radiological diagnosis of spondylodiscitis (50%) and underwent a spinal disc biopsy. They were all characterized as having aseptic spondylodiscitis. Three patients were free of pain with analgesics in their last follow-up and one patient had only partial solution of his pain. Three additional patients had spinal fractures surgically treated (37.5%) and one patient was operated because of a cervical kyphotic deformity (12.5%). There were no deaths or surgical complications in this series. CONCLUSIONS: the majority of our clinical and laboratories findings were discrepant with the medical literature. These differences may be secondary to regional characteristics or by the fact that our population included only those patients who underwent spinal surgery.


RESUMO INTRODUÇÃO: A espondilite anquilosante (EA) é uma espondiloartropatia soronegativa, caracterizada principalmente pelo envolvimento do esqueleto axial e das articulações sacroilíacas. A EA promove alterações biomecânicas que predispõem a coluna a fraturas, deformidades e à espondilodiscite. O objetivo do presente estudo é reportar as características clínicas e laboratoriais dos pacientes com EA que foram submetidos a procedimentos cirúrgicos na coluna vertebral em nossa instituição. MÉTODOS: Estudo retrospectivo com revisão de dados médicos dos pacientes com EA que foram submetidos a intervenções na coluna vertebral. RESULTADOS: Nove pacientes foram encontrados e oito incluídos no presente estudo. Três pacientes eram homens e seis mulheres, com média de 57 anos de idade. Todos os pacientes apresentavam dor no segmento da coluna acometido pela doença e um paciente tinha tetraparesia por mielopatia cervical. Seis pacientes (75%) apresentaram proteínas de fase aguda com níveis séricos elevados e dois eram HLA-B27 positivos. Em quatro pacientes houve o diagnóstico radiológico presumido de espondilodiscite e estes foram submetidos à biópsia de disco (três por via percutânea e um com biópsia aberta) - em nenhum deles houve identificação de agente infeccioso. Desses, três pacientes tiveram melhora total da dor durante o seguimento, enquanto um deles mantinha dores leves. Houve três casos de fraturas tratadas cirurgicamente (37,5%) e um caso de deformidade cervical cifótica grave (12,5%). Não houve mortes ou complicações relacionadas às cirurgias nessa série. CONCLUSÕES: A maioria dos dados clínicos e laboratoriais de nosso estudo divergiu da literatura. Essas diferenças podem ser atribuídas às características regionais de nossa população ou pelo fato de incluirmos apenas pacientes que foram submetidos à intervenção cirúrgica.

ABSTRACT

La detección por métodos moleculares del alelo HLA-B*27 se ha convertido en los últimos años en Cuba, en una herramienta que apoya el diagnóstico de enfermedades conocidas, de manera general, como espondiloartropatías (SpA). Con el objetivo de determinar el porcentaje de positividad del alelo en individuos con sospecha clínica de SpA y uveitis, se estudiaron muestras de ADN pertenecientes a 236 pacientes caracterizados en género, edad y color de la piel. Para la genotipificación se utilizó el PCR en tiempo real con el estuche comercial LightMix HLA-B*27 SimpleProbe (TIBMOLBIOL). El 42,7% de los casos fueron mayores de 40 años, con un promedio de edad de 39 años y una relación de masculino - femenino de 0,84; sin embargo, los pacientes que debutan con enfermedades relacionadas con el HLA-B*27 deberían ser fundamentalmente varones entre la segunda y tercera década de vida. De las muestras estudiadas solamente 51 (21,6%) resultaron positivas para el alelo en cuestión. Sin embargo, según el tipo de enfermedad, la frecuencia del gen debería encontrarse entre el 36,0 y el 90,0%. Solo se encontraron diferencias estadísticamente significativas en las comparaciones por géneros. El 18,0% de las indicaciones no contaban con impresión diagnóstica y estos casos presentaron la positividad más baja. Los presentes resultados evidencian que las solicitudes de HLA-B*27 que se generan en el sistema de salud cubano no siguen criterios uniformes de sospecha clínica de las SpA y uveítis; y que en las peticiones de exámenes no se incluyen los datos necesarios que permitan una correcta interpretación de esta prueba inmunogenética.


The detection by molecular methods of the HLA-B*27 allele has become in recent years in Cuba in a tool that supports the diagnosis of some diseases generally known as spondyloarthropathies (SpA).In order to determine the percentage of the gene positivity in individuals with clinical suspicion of SpA and uveitis, we studied 236 DNA samples characterized in gender, age and skin color, of patients with HLA-B*27 indication.The genotyping was performed by Real Time - PCR with the LightMix HLA-B*27 SimpleProbe kit (TIBMOLBIOL). The 42.7% of the cases were older than 40 years, with a mean of 39 years old and a male - female ratio of 0.84; however, patients debuting with HLA-B*27-related diseases should be predominantly males between the second and third decade of life. Our results showed that 188 (78.4%) were negative and only 51 (21.6%) were positive however, according to the type of pathology, the frequency of this gene should be between 36.0% and 90.0%.The statistical differences were only associate to gender but not in the groups of different skin color and presumptive diagnosis. The 18.0% of the laboratory test request forms had no diagnostic impression and these cases had the lowest positivity rates. The present results show that the requests for HLA-B*27 generated in the Cuban health system do not follow uniform criteria of clinical suspicion of SpA and uveitis; and that request forms do not include the necessary data to allow a correct interpretation of this immunogenetic test.
