ABSTRACT

OBJECTIVE@#To solve the ESB bus performance and safety problems caused by the explosive growth of the hospital's business, and to ensure the stable interaction of the hospital's business system.@*METHODS@#Taking the construction of our hospital's information system as an example, we used AlwaysOn, load balancing and other technologies to optimize the ESB bus architecture to achieve high availability and scalability of the hospital's ESB bus.@*RESULTS@#The ESB bus high-availability architecture effectively eliminates multiple points of failure. Compared with the traditional dual-machine Cluster solution, the security is significantly improved. The nodes based on load balancing can be scaled horizontally according to the growth of the hospital's business volume.@*CONCLUSIONS@#The construction of the ESB bus high-availability architecture effectively solves the performance and security issues caused by business growth, and provides practical experience for medical information colleagues. It has certain guiding significance for the development of regional medical information.

ABSTRACT

BACKGROUND: Total knee arthroplasty is effective in managing end-stage knee disease. Measured resection and gap balancing are two different techniques. Both of two have advantages and disadvantages. A technique has been developed that combines the benefits of measured resection and gap balancing to optimize the clinical effect of total knee arthroplasty. OBJECTIVE: To evaluate the operation and early clinical effect in total knee arthroplasty about the combination of measured resection and gap balancing technique used in femoral component rotation. METHODS: Totally 30 patients were treated with the combination of measured resection and gap balancing technique in total knee arthroplasty from September 2016 to December 2018, including 4 males and 26 females, at the age of 46-81 years. There were 24 cases of osteoarthritis with varus and 2 cases of osteoarthritis with valgus. There were 4 cases of rheumatoid arthritis with valgus. All patients signed the informed consent. This study was approved by the Hospital Ethics Committee. Effects were evaluated by Visual Analogue Scale, Hospital for Special Surgery score, femorotibial angle and range of motion before surgery and during final follow-up. RESULTS AND CONCLUSION: (1) The incision healed in the first stage after operation. No early complications occurred, such as infection, vascular nerve injury, deep venous thrombosis of the lower extremities and periprosthetic fracture. (2) Totally 30 patients were followed up for 6-30 months. (3) Visual Analogue Scale, Hospital for Special Surgery score, range of motion and femorotibial angle were improved during final follow-up compared with those before surgery (P < 0.05). (4) Surgeons should be familiar with measured resection and gap balancing technique in total knee arthroplasty, and then use the combination of two techniques based on the specific situation of patients to obtain better soft tissue balance and prosthesis position. The combination technique is easy to master and can be operated with traditional osteotomy tools. Thus, good short-term effect can be achieved.

ABSTRACT

OBJECTIVE: To investigate the therapeutic effect of scalp acupuncture and rehabilitation training on balance dysfunction in children with spasmodic hemiplegia so as to provide the reference to the optimization of treatment scheme. METHODS: A total of 60 children with spastic hemiplegia were divided into a routine group and a scalp acupuncture group, 30 cases in each one according to random number table. In the routine group, the rehabilitation training was provided, including exercise training, balance training, spasmotherapy apparatus, electromyography biofeedback apparatus and orthoses. In the scalp acupuncture group, on the base of the treatment as the routine group, scalp acupuncture was supplemented at motor area, foot motor sensory area, equilibrium area and parietal temporal anterior oblique line. Separately, before the treatment, after 3 months treatment and after 6 months treatment, the dimension D and E of the gross motor function measure-88 (GMFM-88) and Berg balance scale (BBS) were adopted to evaluate balance related motor functions and equilibrium function. The differences in the above 3 indicators at different time stages were compared in children between the two groups. RESULTS: Compared with the score before the treatment, BBS score was obviously increased after 3 and 6 months treatment in the patients of the two groups respectively (P<0.05). The score in the dimension D and E after 6-month treatment was increased significantly as compared with the score before treatment and after 3-month treatment in the same group respectively (P<0.05). Compared with the routine group, the score of dimension D and E of GMFM-88 as well as BBS score were all increased obviously in the scalp acupuncture group after 3 and 6 months treatment (P<0.05). CONCLUSION: On the base of routine rehabilitation training, scalp acupuncture can improve balance function of children with spastic hemiplegia better.

ABSTRACT

The coordination and unification of Yin and Yang are the basis of normal human life activities. Along with the age growth and aging of the body, women will suffer from menopausal syndrome during menopause. In addition to the significant changes in the genital system, there are also pathological manifestations in estrogen target points including bone, nerve and cardiovascular systems, due to the imbalance of Yin and Yang. Besides the insufficiency of estrogen, the main cause of menopausal syndrome is the changes in the response of target organs to estrogen. In other words, the biological effects mediated by estrogen receptor(ER) alpha and beta subtypes in target cells are often different or even opposite; the changes of expression level and ratio of ERα and ERβ are also important causes for the abnormal estrogenic effects in target organs and the imbalance of Yin and Yang of the body. Therefore, on one hand, the therapeutic mechanism of drugs is ER-mediated estrogenic effect. On the other hand, the drugs have a regulatory effect on ER subtype expression in target cells and Yin-Yang state in target organs and even organisms, so as to cause further changes in the response of target cells to estrogen or estrogenic components, and exert its therapeutic effects. This paper reviews the pharmacological mechanism of gynecological traditional Chinese medicine in harmonizing Yin and Yang in estrogen-positive target cells and the clinical efficacy in the following aspects, including estrogen and its mechanism, the estrogenic effect of ER in traditional Chinese medicine and the mechanism of ER subtype in balancing Yin and Yang and mediating and regulating the main target tissues in menopausal syndrome treatment.

ABSTRACT

Psychiatric disorder as dysfunctional behavioural syndrome is a paradoxical phenomenon that is difficult to explain evolutionarily because moderate prevalence rate, high heritability and relatively low fitness are shown. Several evolutionary genetic models have been proposed to address this paradox. In this paper, I explain each model by dividing it into selective neutrality, mutation-selection balance, and balancing selection hypothesis, and discuss the advantages and disadvantages of them. In addition, the feasibility of niche specialization and frequency dependent selection as the plausible explanation about the central paradox is briefly discussed.

ABSTRACT

Abstract Hominin evolution is characterized by adaptive solutions often rooted in behavioral and cognitive changes. If balancing selection had an important and long-lasting impact on the evolution of these traits, it can be hypothesized that genes associated with them should carry an excess of shared polymorphisms (trans- SNPs) across recent Homo species. In this study, we investigate the role of balancing selection in human evolution using available exomes from modern (Homo sapiens) and archaic humans (H. neanderthalensis and Denisovan) for an excess of trans-SNP in two gene sets: one associated with the immune system (IMMS) and another one with behavioral system (BEHS). We identified a significant excess of trans-SNPs in IMMS (N=547), of which six of these located within genes previously associated with schizophrenia. No excess of trans-SNPs was found in BEHS, but five genes in this system harbor potential signals for balancing selection and are associated with psychiatric or neurodevelopmental disorders. Our approach evidenced recent Homo trans-SNPs that have been previously implicated in psychiatric diseases such as schizophrenia, suggesting that a genetic repertoire common to the immune and behavioral systems could have been maintained by balancing selection starting before the split between archaic and modern humans.

ABSTRACT

The patient who has severely absorbed residual ridges, treatments are challenging to satisfy many factors: support, retention, stability, etc. The neutral zone or monoplane occlusion with non-anatomical tooth would be helpful to get additional retention and stability. The monoplane occlusion has been used long time because it can eliminate horizontal forces and many other advantages. The lingualized occlusion was introduced to improve chewing efficiency and esthetics. But from a stability aspect, it seems controversy between monoplane occlusion and lingualized occlusion. This case report shows the results of the treat two flat residual ridge patients using functional impression; piezography, and made 2 other dentures with monoplane and lingualized occlusion that patient can select denture.

ABSTRACT

Foram conduzidos dois experimentos, com o objetivo de estimar a exigência de valina e isoleucina digestível para codornas de corte em crescimento. No experimento 1, foram utilizadas 2.160 codornas de corte de um dia de idade, não sexadas, distribuídas em um delineamento inteiramente ao acaso, com arranjo fatorial 4x4, sendo quatro níveis de valina digestível (0,90, 1,10, 1,30 e 1,50%) e quatro níveis de isoleucina digestível (0,80, 1,00, 1,20 e 1,40%), totalizando 16 tratamentos, com três repetições e 45 codornas por unidade experimental. O peso corporal, o ganho de peso e a biomassa corporal acumulada aumentaram linearmente em função dos níveis de valina digestível e foram influenciados de forma quadrática, estimando 1,38% de isoleucina digestível. No experimento 2, foram utilizadas 1.440 codornas de corte não sexadas, distribuídas em um delineamento inteiramente ao acaso, com arranjo fatorial 4x4, sendo quatro níveis de valina digestível (0,82, 1,02, 1,22 e 1,42%) e quatro níveis de isoleucina digestível (0,73, 0,93, 1,13 e 1,33%), totalizando 16 tratamentos, com três repetições e 30 codornas por unidade experimental. O desempenho de codornas de corte, no período de 15 a 35 dias de idade, não foi influenciado pelo aumento dos níveis de valina e isoleucina digestível nas rações experimentais. Conclui-se que as exigências de valina e isoleucina digestível para o máximo desempenho de codornas de corte, no período de um a 14 dias de idade, foram de 1,50% e 1,38%, respectivamente, correspondendo às relações valina:lisina de 95% e isoleucina:lisina de 88%. Os menores níveis avaliados, no período de 15 a 35 dias de idade, de 0,82% de valina digestível e de 0,73% de isoleucina digestível, correspondendo às relações valina:lisina de 52% e isoleucina: lisina de 46%, foram suficientes para satisfazer as exigências nutricionais das codornas de corte sem comprometer o desempenho.(AU)


Two experiments were carried out in order to estimate the requirements of digestible valine and isoleucine for growing meat quails. In experiment 1, 2160 meat quails with 1 day of age, not sexed, were used, distributed in a completely randomized design with a 4x4 factorial arrangement, with four levels of digestible valine (0.90, 1.10, 1.30, and 1.50%) and four levels of digestible isoleucine (0.80, 1.00, 1.20, and 1.40 %), totaling 16 treatments with three replications and 45 quails per experimental unit. The body weight, the weight gain, and the accumulated body biomass linearly increased in function of digestible valine levels and were influenced in a quadratic way estimating 1.38% of digestible isoleucine. In experiment 2, 1440 meat quails, not sexed, were used, distributed in a completely randomized design with a 4x4 factorial arrangement, with four levels of digestible valine (0.82, 1.02, 1.22, and 1.42%) and four levels of digestible isoleucine (0.73, 0.93, 1.13, and 1.33%), totaling 16 treatments with three replications and 30 quails per experimental unit. The performance of meat quails from 15 to 35 days of age was not affected by increased levels of digestible valine and digestible isoleucine in the experimental diets. It was concluded that the requirements of digestible valine and digestible isoleucine for maximum performance of meat quail from one to 14 days of age were 1.50% and 1.38%, respectively, corresponding to relations: valine: lysine of 95% and isoleucine: lysine of 88%. The lower levels evaluated, in the period of 15 to 35 days old, 0.82% digestible valine and 0.73% of digestible isoleucine, corresponding to relations valine: lysine of 52% and isoleucine: lysine of 46% were sufficient to meet the nutritional requirements of meat quails without compromising performance.(AU)

ABSTRACT

BACKGROUND: Degenerative osteoarthritis of the knee usually shows arthritic change in the medial tibiofemoral joint with severe varus deformity. In total knee arthroplasty (TKA), the medial release technique is often used for achieving mediolateral balancing. But, in a more severe varus knee, there are more difficult technical problems. Bony resection of the medial proximal tibia (MPT) as an alternative technique for achieving soft tissue balancing was assessed in terms of its effectiveness and possibility of quantification. METHODS: TKAs were performed in 78 knees (60 patients) with vertical bone resection of the MPT for soft tissue balancing from September 2011 to March 2013. During operation, the medial and lateral gaps were measured before and after the bony resection technique. First, the correlation between the measured thickness of the resected bone and the change in medial and lateral gaps was analyzed. Second, the possibility of quantification of each parameter was evaluated by linear regression and the coefficient ratio was obtained. RESULTS: A significant correlation was identified between alteration in the medial gap change in extension and the measured thickness of the vertically resected MPT (r = 0.695, p = 0.000). In the medial gap change in flexion, there was no statistical significance (r = 0.214, p = 0.059). When the MPT was resected at an average thickness of 8.25 +/- 1.92 mm, the medial gap in extension was increased by 2.94 +/- 0.87 mm. In simple linear regression, it was predictable that MPT resection at a thickness of 2.80 mm was required to increase the medial gap by 1.00 mm in knee extension. CONCLUSIONS: The method of bone resection of the MPT can be considered effective with a predictable result for achieving soft tissue balancing in terms of quantification during TKA.

ABSTRACT

Objective To evaluate the effect of montelukast combined with seretide in treatment of patients with asthma and its influence on balance between Th17 and Treg cells, and expression of IL-4 and IFN-γ.Methods 110 patients with asthma were randomly divided into observation group and control group, observation group of 55 cases, using montelukast combined with seretide, control group of 55 cases, using seretide,compared two groups of patients with pulmonary function changes, peripheral inflammatory factor changes and influence on Th17 and Treg cells balance. Results Before treatment, observation group and control group,FEV1 and FEV1 /FVC value was no significant difference.FEV1 and FEV1 /FVC after treatment were significantly improved compared with before treatment in two groups, differences were statistically significance (P<0.05).Between two groups, FEV1 values were significantly difference (t=8.1156, P=0.0001), FEV1 /FVC also had significant difference (t=4.8933, P=0.0001) .Before treatment, observation group and control group, IL-4 and IFN-γdid not have a statistically significant.After treatment, IL-4 and IFN-γdecreased significantly, with statistical difference compared with before treatment (P<0.05).Between two groups, IL-4 had significant difference (t=6.0273, P=0.0001), IFN-γalso had statistically significant (t=6.6042, P=0.0001).Before treatment, there was no significant difference between Th17 and Treg percentage, after 12 weeks of treatment and 24 weeks, Th17 in observation group were(0.53 ±0.02),(0.32 ±0.01), the control group were (0.78 ±0.03),(0.62 ±0.02), which had differences (P<0.05).After 12 weeks and 24 weeks,observation group Treg respectively(2.21 ± 0.22),(2.47 ±0.10),in control group were(1.50 ±0.11),(1.84 ±0.14), Treg percentage at different time after treatment, the differences had statistically significant (P<0.05).Conclusion Montelukast combined with Seretide in treating with asthma, pulmonary function of patients is obviously improved, and it can improve Th17, Treg cell.

Objective] To summarize clinical experience of Chen Yi’s Yi Zhi Zhu Kao Gao Fang. [Methods] By means of analyzing the principle of prescription,syndrome differentiation and characteristics,combined with typical cases, experiences of Chen Yi’s Yi Zhi Zhu Kao Gao Fang were summarized. [Results] Chen Yi emphasizes the key of Yi Zhi Zhu Kao Gao Fang on balancing the five internal organs, analyzing the heart, liver, spleen and kidney’s function. He uses the way of regulating liver-qi for easy mood, nourishing heart and appeasing spirit to sequence sleeping, strengthening spleen and stomach to regulate middle energizer, replenishing qi to consolidate the superficial resistance, tonifying kidney to consolidate the constitution, respectively. Taking it in low dosage in long time can get the purpose of Yi Zhi Zhu Kao. [Conclusion] Chen Yi ’s Yi Zhi Zhu Kao Gao Fang has clear theory, rigorous formula, curative effect and worth passing on promotion.

ABSTRACT

A goal of total knee arthroplasty is to obtain symmetric and balanced flexion and extension gaps. Controversy exists regarding the best surgical technique to utilize to obtain gap balance. Some favor the use of a measured resection technique in which bone landmarks, such as the transepicondylar, the anterior-posterior, or the posterior condylar axes are used to determine proper femoral component rotation and subsequent gap balance. Others favor a gap balancing technique in which the femoral component is positioned parallel to the resected proximal tibia with each collateral ligament equally tensioned to obtain a rectangular flexion gap. Two scientific studies have been performed comparing the two surgical techniques. The first utilized computer navigation and demonstrated a balanced and rectangular flexion gap was obtained much more frequently with use of a gap balanced technique. The second utilized in vivo video fluoroscopy and demonstrated a much high incidence of femoral condylar lift-off (instability) when a measured resection technique was used. In summary, the authors believe gap balancing techniques provide superior gap balance and function following total knee arthroplasty.

ABSTRACT

INTRODUCTION: Electromagnetic interference caused by electric power lines adversely affects the signals of electronic instruments, especially those with low amplitude levels. This type of interference is known as common-mode interference. There are many methods and architectures used to minimize the influence of this kind of interference on electronic instruments, the most common of which is the use of band-reject filters. This paper presents the analysis, development, prototype and test of a new reconfigurable filter architecture for biomedical instruments, aiming to reduce the common-mode interference and preserve the useful signal components in the same frequency range as that of the noise, using the technique of dynamic impedance balancing. METHODS: The circuit blocks were mathematically modeled and the overall closed-loop transfer function was derived. Then the project was described and simulated in the VHDL_AMS language and also in an electronics simulation software, using discrete component blocks, with and without feedback. After theoretical analysis and simulation results, a prototype circuit was built and tested using as input a signal obtained from ECG electrodes. RESULTS: The results from the experimental circuit matched those from simulation: a 97.6% noise reduction was obtained in simulations using a sinusoidal signal, and an 86.66% reduction was achieved using ECG electrodes in experimental tests. In both cases, the useful signal was preserved. CONCLUSION: The method and its architecture can be applied to attenuate interferences which occur in the same frequency band as that of the useful signal components, while preserving these signals.

ABSTRACT

Background: Austrocedrus chilensis (D. Don) Pic. Ser. et Bizzarri commonly known as Patagonian cypress is a member of the Cupressaceae family, characterized by a high adaptive potential for growing in marginal areas and good timber quality. The species grows over a wide area and under a wide range of rainfall. This study assessed adaptive genetic variation at SNP level in candidate genes involved in response to drought stress. Results: A total of 18 single nucleotide polymorphisms (SNPs) were found among 1,428 bp. Average nucleotide diversity value (π = 0.00312) was similar to those previously reported in other Cupressaceae. The Fst average among genes and populations was 0.163 and the lowest differentiation was observed in continuous and humid populations. A number of neutrality tests were applied to find evidence of positive selection in our candidate gene set, but only AcAQP2 gene in Pedregoso and San Ramón populations revealed significant departures from neutrality with positive values suggesting balancing selection. Conclusions: In this study we report the levels of nucleotide diversity searched in some drought stress candidate genes in Austrocedrus chilensis and the selective factors that may be acting on this species.

BACKGROUND: Despite the importance of soft tissue balancing during total knee arthroplasty (TKA), all estimating techniques are dependent on a surgeon's manual distraction force or subjective feeling based on experience. We developed a new device for dynamic gap balancing, which can offer constant load to the gap between the femur and tibia, using pneumatic pressure during range of motion. METHODS: To determine the amount of distraction force for the new device, 3 experienced surgeons' manual distraction force was measured using a conventional spreader. A new device called the consistent load pneumatic tensor was developed on the basis of the biomechanical tests. Reliability testing for the new device was performed using 5 cadaveric knees by the same surgeons. Intraclass correlation coefficients (ICCs) were calculated. RESULTS: The distraction force applied to the new pneumatic tensioning device was determined to be 150 N. The interobserver reliability was very good for the newly tested spreader device with ICCs between 0.828 and 0.881. CONCLUSIONS: The new pneumatic tensioning device can enable us to properly evaluate the soft tissue balance throughout the range of motion during TKA with acceptable reproducibility.

ABSTRACT

@#Objective To observe the effect of meridian balancing on speech disorder in children with cerebral palsy. Methods 130 childrenaged 1.5~7 years old were divided into the control group (n=62) and treatment group (n=68). The control group received routine rehabilitationwhile the treatment group received meridian balancing in addition. Results The rate of improvement was 89.7% in treatmentgroup and 79% in control group (P<0.05). The rate was more for those with language retardation than with anarthria in the treatment group(P<0.01). Conclusion The meridian balancing is effective on speech disorder in children with cerebral palsy, especially for those with languageretardation.

BACKGROUND AND OBJECTIVES: An open-set monosyllabic speech perception test is necessary, not only to devise habilitation programs but to select candidates and estimate performance in pediatric cochlear implant program. The purpose of this study is to develop a validated open-set monosyllabic speech perception test for preschool children. SUBJECTS AND METHOD: Qualitative test development was based on vocabulary familiarity, syllabic and phonemic frequency, and phonetic balancing between the lists. The devised test lists were applied to 138 normal hearing children and 46 children with cochlear implant to examine the validity and reliability. RESULTS: The final two test lists were made up of 100 items with 260 phonemics. The consequential validity of this devised test was proven by the applied results that the speech perception score of children with implanted devices was significantly lower than that of normal children, when controlling for the length of auditory experience. CONCLUSION: The developed monosyllabic speech perception test is the validated instrument for preschool children. It is sensitive to perceptual characteristics of articulatory function in cochlear implanted children. The validity of this test needs to be enhanced by clinical application and further item analysis.

ABSTRACT

PURPOSE: To evaluate the clinical relationship between medial collateral ligament(MCL) complete detachment and mechanical alignment in total knee arthroplasty. MATERIALS AND METHODS: From February 2001 to December 2006, we performed 290 TKAs. 9 TKAs(0.03%) happened MCL complete detachment. 1 TKA was excluded for paraplegia. All cases were women. The mean age was 71.1 years old. The mean follow-up period was 41.1 months. There were 7 degenerative osteoarthritis and 1 rheumatoid arthritis. The clinical evaluation included Knee Society Score(KSS), function score and range of motion(ROM) at preoperative, postoperative 3 months, 6 months, 12 months and final follow-up. We measured the femoro-tibial angle and the mecha- nical axis by anterior-posterior and whole lower extremity radiograph. The medial instability obtained serial valgus stress radiograph. RESULTS: There were 4 neutral and 4 varus alignment. KSS, function score, ROM was significantly improved in both group, and there were no significant differences in both group. On serial valgus stress radiograph, the difference compared with normal side decreased from 5.2 degree(postoperative 3 months) to 3.4 degree(final follow-up) in neutral alignment group, from 2.9 degree to 0.9 degree in varus alignment group. In final follow-up, it revealed that the medial instability of varus alignment group was less than that of neutral alignment group. CONCLUSION: In MCL complete detachment, some stability obtained by repair of medial collateral ligament and bracing. Whole instability was not gone. Therefore, we should make the varus alignment of prosthesis in mechanical axis line position of 34 to 67% on the medial tibial plateau.

ABSTRACT

On the base of element and metabolism balancing,the mathematic model of the human-like collagen expression phase with recombinant Escherichia coli BL21 was developed and the unknown parameters in the model were estimated with the method of nonlinear optimization.The model was in agreement with the growth kinetics and the metabolic kinetics,and the key calculated parameters of ?h,?p and mx were 1.173 mol?C-mol-1,293.814 mol?C-mol-1 and 17.878 mol?C-mol-1?h-1 respectively.This model could preferably predict the macroscopic reaction rates,and in the synthesis phase of human-like collagen,the specific growth rate should be controlled at 0.04 h-1 with controlling glucose feeding rate to gain the highest specific production rate of human-like collagen.

ABSTRACT

Recently, it becomes conspicuous that the network should have enough communication bandwidth and be organized with steadiness to operate and support hospital information system successfully. Seoul National University Hospital completed to reconstruct legacy network which had been used since 1995. We had a consultation to diagnose the current problems and reconstructed the network according to the consultation. The design of network architecture was mainly focused on the circuit route in case of error condition and the suitable bandwidth for the easy communication. We also regulated the use of unsuitable protocols which might broadcast inappropriate data packets over the whole network. After 6 months of operation we evaluated the performance of newly constructed network. The average traffic rate from the input port of main servers was 0.5% and that from the output port was 6% separately. The average traffic rate on the overall ATM backbone showed around 1 %.From the result, we concluded that the newly constructed network had such an enough capability supporting hospital information system without any interruption. Furthermore, we expect that it will be sufficient to support the additional traffic increment by PACS and group ware applications.
