ABSTRACT

Objective:To establish a standardized information management system (IMS) for preserving, managing, querying, and performing statistics on biospecimens and their clinical data, which is conducive to improving the utilization of biobank. Methods:Under the premise of ensuring operating environment and data security, a database-based data logic relationship model is created and applied to the IMS to manage and analyze biospecimens and their supporting clinical information of patients enrolled in the biobank of our center. Results:To ensure the establishment of the follow-up cohort, biospecimens and clinical information of inpatients and outpatients were continuously collected in the biobank of our center. Since December 2014, more than 270 000 biospecimens from inpatient, outpatient, and scientific research have been preserved. The IMS optimized by this model efficiently completes the basic work of the biobank. At the same time, the data can be queried jointly and in batches, and then converted into a report format for statistical analysis. Conclusions:The IMS of our center is suitable for application and popularization as a construction and management model for the hospital-level biobank, which meets the daily work of the biobank and diverse research needs, and provides a convenient platform and rich resources for the development of precision medicine.

ABSTRACT

The case report of a 62 years mestiza patient of rural origin is presented, who had been assisted 3 years ago in the ophthalmology service due to a tumor of conjunctive in the left eye. She went again to the service for this reason, but this time it also existed profuse bleeding and serious damage of the ocular structures. Several exams were carried out, among them orbit X rays, ocular echography, orbit and skull tomographies (with contrast), as well as biopsy of the exofitic lesion and immunohistochemical study. The definitive histologic diagnosis was squamous cell carcinoma of conjunctive, reason why it was decided to carry out orbit exeresis, reconstructive cosmetic surgery and, later on, radiotherapy and chemotherapy. The patient had a favorable clinical course and the dissemination of the malignancies was controlled, what could also be avoided if initial medical care might have been opportune.

ABSTRACT

Radiotherapy is one of the therapeutic pillars used in oncology, but ocular damage may occur and this is a well-established phenomenon. The occurrence of complications remains the main concern for the application of radiotherapy, despite the use of new strategies to reduce complications and to improve the therapeutic rate. Here is a 63 years old female patient, who was diagnosed with epidermoid carcinoma of the conjunctiva, moderately differentiated G-II invader (T3N0M0) in the left eye. She was surgically treated and received supplementary superficial radiotherapy As a late complication, the patient presented with radiogenic cataract treated by surgery, in addition to conjunctival aneurysms in the treated area as well(AU)

ABSTRACT

It is necessary predict the effect of aquifer stresses in surface water and wetlands and consider the mutual effects that are produced by the conjunctive use of surface water and groundwater. This was originally made with very simple idealized analytical methods. The next development was the application of finite differences or finite elements numerical models, but poses problems when the model has to be run many times to analyze different management alternatives. When aquifer behavior is linear, as in confined, semiconfined, or unconfined aquifers with not too large changes in its saturated thickness, it is possible to apply the superposition strategy through influence functions. That has simplified significantly modeling and improved the effectiveness of management models. However, for large models, long modeling periods and a large number of alternatives, it is needed to handle and store many influence functions and to consider and store all the previous stresses. In that case, the eigenvalue method can be a more appropriated option. This approach solves the spatially discretized flow equation explicitly and continuously in time, obtaining modal orthogonal components through very simple explicit state equations in function of time. To reduce the computational load, the simulation can be simplified with appropriate truncation using only dominant modes of the components at the expense of a small error. Efficient methods have been developed to get the modal components as well as to perform truncation with limited errors.

ABSTRACT

Objective: Determine the best procedure for the treatment of the surgical primary pterygium according to the recurrence rate. Design: Randomized clinical trials. Material and Methods: 228 patients (228 eyes) carrying primary pterygium level II diagnosed with biomicroscopy, having the surgical treatment from January 2001 to October 2005 in the surgical room of the School Hospital of UFTM. Patients were divided into two groups:Test group (126 eyes): exeresis of the pterygium followed by a transplant of amnionic membrana;Control group (102 eyes): exeresis of the pterygium followed by rotation of the conjunctival flap. Criterion of recidivation considered was the corneal invasion by the fibrovascular tissue in a period of a year after the surgery. Statistics analysis was done using the significance test for equality of two proportions. Results: In the test group we observed 8 tables of recidivation, being 6 in male patients and 2 in female ones. Average age of these patients was 39.6 (ranging from 22 to 53). Recidivation occurred on an average of 3.3 months (ranging from the 2nd to the 7th month). In the control group we observed 17 recidivations of the pterygium being 11 in the male sex and 06 in the female one. The average age of these patients was 35.7 years old (ranging from 20 to 50 years). Recidivation occurred on an average of 3.1 months (ranging from the 1st to the 8th month). The calculated z (2.48) was higher than the table z. Conclusions: Amniotic membrane transplantation is an effective and safe procedure for primary pterygium surgery with a relatively low recurrence rate. However, we couldn't affirm that the amniotic membrane transplant has to be considered the first choice in the primary pterygium surgery treatment.

ABSTRACT

The author develops thoughts about cloning as a myth, in creation, which reveals interesting aspects of some patients. A myth which helps reflections about a specific type of relationship between the subject and the other, about his relationship with reality and about peculiar ways of thinking. The author uses the myth of the clone and some clinical experiences to discuss mental operations which help to understand those patients referred to in the psychoanalytical literature as devitalized, de-objectalized, of difficult access, borderline. Specificities of the transferencial and contratransferencial process with these patients are also discussed.
