ABSTRACT

OBJECTIVE@#To explore the mechanism of Radix Scrophulariae (RS) extracts in the treatment of hyperthyroidism rats by regulating proliferation, apoptosis, and autophagy of thyroid cell through the mammalian sterile 20-like kinase 1 (MST1)/Hippo pathway.@*METHODS@#Twenty-four rats were randomly divided into 4 groups according to a random number table: control, model group, RS, and RS+Hippo inhibitor (XMU-MP-1) groups (n=6 per group). Rats were gavaged with levothyroxine sodium tablet suspension (LST, 8 μ g/kg) for 21 days except for the control group. Afterwards, rats in the RS group were gavaged with RS extracts at the dose of 1,350 mg/kg, and rats in the RS+XMU-MP-1 group were gavaged with 1,350 mg/kg RS extracts and 1 mg/kg XMU-MP-1. After 15 days of administration, thyroid gland was taken for gross observation, and histopathological changes were observed by hematoxylin-eosin staining. The structure of Golgi secretory vesicles in thyroid tissues was observed by transmission electron microscopy. The expression of thyrotropin receptor (TSH-R) was observed by immunohistochemistry. Terminal-deoxynucleoitidyl transferase mediated nick end labeling assay was used to detect cell apoptosis in thyroid tissues. Real-time quantity primer chain reaction and Western blot were used to detect the expressions of MST1, p-large tumor suppressor gene 1 (LATS1), p-Yes1 associated transcriptional regulator (YAP), proliferating cell nuclear antigen (PCNA), G1/S-specific cyclin-D1 (Cyclin D1), B-cell lymphoma-2 (Bcl-2), Caspase-3, microtubule-associated proeins light chain 3 II/I (LC3-II/I), and recombinant human autophagy related 5 (ATG5). Thyroxine (T4) level was detected by enzyme-linked immunosorbent assay.@*RESULTS@#The thyroid volume of rats in the model group was significantly increased compared to the normal control group (P<0.01), and pathological changes such as uneven size of follicular epithelial cells, disorderly arrangement, and irregular morphology occurred. The secretion of small vesicles by Golgi apparatus was reduced, and the expressions of receptor protein TSH-R and T4 were significantly increased (P<0.01), while the expressions of MST1, p-LATS1, p-YAP, Caspase-3, LC3-II/I, and ATG5 were significantly decreased (P<0.01). The expressions of Bcl-2, PCNA, and cyclin D1 were significantly increased (P<0.01). Compared with the model group, RS extracts reduced the volume of thyroid gland, improved pathological condition of the thyroid gland, promoted secretion of the secretory vesicles with double-layer membrane structure in thyroid Golgi, significantly inhibited the expression of TSH-R and T4 levels (P<0.01), upregulated MST1, p-LATS1, p-YAP, Caspase-3, LC3-II/I, and ATG5 expressions (P<0.01), and downregulated Bcl-2, PCNA, and Cyclin D1 expressions (P<0.01). XMU-MP-1 inhibited the intervention effects of RS extracts (P<0.01).@*CONCLUSION@#RS extracts could inhibit proliferation and promote apoptosis and autophagy in thyroid tissues through MST1/Hippo pathway for treating hyperthyroidism.

ABSTRACT

Objective:To investigate the effect of levothyroxine withdrawal before radioiodine therapy on blood lipids and renal function in patients with differentiated thyroid carcinoma (DTC) after operation.Methods:From Mar. 2020 to Apr. 2022, 214 patients with differentiated thyroid cancer were enrolled in the General Surgery Department, Linyi Central Hospital, Shandong Province. All patients stopped taking levothyroxine sodium after total thyroidectomy. The thyroid function index, blood lipid index and renal function index were measured and compared before and after drug withdrawal (before operation) and after drug withdrawal (before radioiodine treatment). The patients were divided into groups according to the duration of drug withdrawal (drug withdrawal group for 3 weeks, drug withdrawal group for 4 weeks), and the differences of thyroid function index, blood lipid index, and renal function index among patients with different drug withdrawal time were compared. The measurement data in accordance with normal distribution were compared between groups, and independent sample t-test was performed. Results:The levels of free thyroxin T4 (FT 4) and free triiodothyronine (FT 3) in DTC patients decreased significantly ( t=57.60, 71.74,all P<0.001), and the levels of thyroid-stimulating hormone (TSH) increased significantly ( t=102.15, P<0.001). After drug withdrawal, the serum lipid index [triglyceride (TG), total cholesterol (TC), high-density lipoprotein (HDL), low density lipoprotein (LDL) ] and renal function index [blood urea nitrogen (BUN), serum creatinine (SCR) ] of DTC patients increased significantly ( t=20.17, 42.50, 12.13, 30.73, 16.09, 43.73, all P<0.001). The levels of FT 3 and FT 4 in the 4-week group were significantly lower than those in the 3-week group ( t=7.75 and 10.07, both P<0.001), and TSH was significantly higher than that in the 3-week group ( t=26.46, P<0.001). The levels of TG, LDL, HDL, TC, BUN and Scr in the 4-week group were significantly higher than those in the 3-week group ( t=10.13, 10.29, 8.53, 11.47, 10.54, 8.55, all P<0.001). Correlation analysis showed that the levels of FT 3 and FT 4 in DCT patients were negatively correlated with the levels of TG, LDL, HDL, TC, BUN and Scr ( r=-0.256, -0.189, -0.249, -0.314, -0.352, -0.231, -0.342, -0.259, -0.304, -0.216, -0.391, -0.271, P=0.011, 0.029, 0.007, 0.004, 0.015, 0.036, 0.002, 0.009, 0.019, 0.017, 0.016, 0.003), and the levels of TSH were correlated with TG, LDL, HDL, TC and BUN Scr level was positively correlated ( r=0.257, 0.308, 0.219, 0.311, 0.251, 0.271, P=0.006, 0.013, 0.032, 0.004, 0.006, 0.014) . Conclusion:Stopping levothyroxine sodium before radioactive iodine treatment after DTC can easily lead to dyslipidemia and decreased renal function in patients, and the longer the withdrawal time is, the more obvious the changes of blood lipids and renal function in patients, and the withdrawal time should be shortened in clinical treatment.

ABSTRACT

Objective:To study the effects of Zuogui Pills on rats with kidney-yin deficiency syndrome of premature ovarian insufficiency.Methods:Totally 40 SD female unmated rats were randomly divided into blank group, model group, Zuogui Pills group and Bujiale group, with 10 rats in each group. Except for blank group, rats in other groups were subcutaneously injected with ZP3 and gavaged with levothyroxine sodium to induce kidney-yin deficiency syndrome model of premature ovarian insufficiency. At the same time of modeling, Zuogui Pills group and Bujiale group received corresponding drugs for gavage, and the other groups received corresponding solvent for gavage, once a day, for consecutive 21 days. On day 0, 7, 14 and 21, ear temperature and body weight of rats were measured, and the ovarian index, uterus index and thyroid index were calculated. Serum levels of adenosine cyclic phosphate (cAMP), cyclic guanosine phosphate (cGMP), Cortisol, follicle-stimulating hormone (FSH), luteinizing hormone (LH) and estradiol (E 2) were detected by ELISA. Pathological changes of ovary was observed with HE staining. Results:On day 14 and 21, compared with model group, the body weight of rats in Zuogui Pills group increased ( P<0.05), and the ear temperature decreased ( P<0.05); compared with model group, the ovarian index, uterine index and thyroid index of rats in Zuogui Pills group decreased ( P<0.05), the levels of serum cAMP/cGMP, cortisol, FSH and LH decreased ( P<0.05), and the level of E 2 increased ( P<0.05). Conclusion:Zuogui Pills have certain improvement effect on rats with kidney-yin deficiency syndrome induced by levothyroxine sodium tablets combined with ZP3.

ABSTRACT

Objective:To investigate the effectiveness of early levothyroxine intervention in pregnancy complicated with hypothyroidism.Methods:A retrospective analysis was performed on 132 pregnant patients with hypothyroidism admitted to the Department of Endocrinology, Anyang People’s Hospital from Nov. 2018 to Sep. 2021. Among them, 68 cases with levothyroxine sodium early intervention were included in the intervention group, and 64 cases without intervention were included in the non-intervention group. The differences in vascular endothelial function indexes, thyroid function indexes, and blood lipid indexes before and after treatment in the intervention group were compared with those in the non-intervention group. The incidence of obstetric complications such as preeclampsia, fetal growth restriction, and adverse pregnancy outcomes of abortion and neonatal asphyxia were compared between the two groups. SPSS 21.0 software was used to process data, measurement data were subjected to t test, and enumeration data were subjected to χ 2 test. Results:After treatment, the endothelium-dependent brachial artery blood flow-mediated vasodilation index (FMD) index, nitric oxide (NO) , and endothelin-1 (ET-1) levels in the intervention group were significantly better than those in the non-intervention group [ (10.37%) ±1.54%) vs (7.25% ± 1.09%) , (60.85 ± 7.03) umol/L vs (39.11 ± 4.31) umol/L, (112.96 ± 13.58) umol/L vs (238.85 ± 26.05) umol/L]. After treatment, the serum thyroid stimulating hormone (TSH) in the intervention group was significantly lower than that in the non-intervention group [ (2.25±0.26) mU/L vs (8.79±1.60) mU/L] ( P<0.001) . After treatment, the levels of total cholesterol (TC) , triacylglycerol (TG) and low-density lipoprotein cholesterol (LDL-C) in the intervention group were significantly lower than those in the non-intervention group [ (3.52±0.91) mmol/L vs (6.51±1.31) mmol/L L, (1.30±0.31) mmol/L vs (1.44±0.36) mmol/L, (2.29±0.31) mmol/L vs (3.32±0.44) mmol/L] ( P<0.001, P=0.036, P<0.001) . The incidence of obstetric complications such as preeclampsia, fetal growth restriction, premature rupture of membranes, and abnormal amniotic fluid volume in the intervention group were significantly lower than those in the non-intervention group (5.88% vs 17.19%, 1.47% vs 9.38%, 10.29% vs. 23.44%, 2.94% vs 12.50%) ( P=0.041, 0.043, 0.043, 0.038) ; the incidence of miscarriage, premature birth, neonatal asphyxia, cesarean section and other adverse pregnancy outcomes in the intervention group were significantly lower than those in the non-intervention group (1.47% vs 9.38%, 4.69% vs 15.36%, 2.94% vs 10.94%, 57.35% vs 75.00%) ( P=0.043, 0.031, 0.038, 0.033) . Conclusion:Early intervention with levothyroxine in pregnancy complicated with hypothyroidism is beneficial to improve thyroid function, reduce blood lipid level, protect vascular endothelial function, reduce related obstetric complications, and reduce the incidence of adverse pregnancy outcomes for mothers and infants.

ABSTRACT

Objective To investigate the effect of L-thyroid hormone on pregnancy outcome,fetal and serum human chorionic gonadotropin(hCG)and superoxide dismutase(SOD)in pregnant women with hypothyroidism.Methods From December 2015 to October 2016,86 pregnant women with hypothyroidism in the Women 's and Children's Hospital of Dongyang were divided into observation group(n=41)and control group(n=45)according to the treatment method.The control group was treated routinely.The observation group was treated with L-thyroxine sodium tablets on the basis of the control group.The levels of serum thyrotropin(TSH),thyroxine demand,serum hCG,SOD level,pregnancy outcome,neonatal growth,complications and clinical efficacy were observed and compared.Results The levels of TSH in pregnant women at the second trimester of pregnancy,the end of pregnancy and 1 week after birth were(1.85 ±0.64)mU/L,(1.32 ±0.36)mU/L,(0.41 ±0.33)mU/L,respectively,which was lower than that in the early pregnancy [(2.47 ±0.58)mU/L](t=3.210,8.154,14.479,all P<0.05).At the second trimester of pregnancy and late pregnancy,the thyroid hormone demand was(1.22 ±0.14)mg· kg-1 · d-1,(1.47 ±0.13)mg· kg-1· d-1,respectively,which was significantly higher than that of early pregnancy [(1.02 ± 0.22)mg· kg-1· d-1](t=3.339,8.531,all P<0.05).The thyroxine requirement at 1 week postpartum was(1.18 ± 0.15)mg· kg-1· d-1,which was lower than that at late pregnancy(t=9.202,P<0.05).There were no statistically significant differences in serum hCG and SOD between the two groups before treatment(all P>0.05).After treatment,the levels of serum hCG and SOD in the control group were(34587.81 ±8301.72)mU/L,(151.46 ±24.58)U/mL,respectively,which in the observation group were(43014.37 ±7879.43)mU/L,(174.53 ±31.66)U/mL,respectively,the differences were statistically significant(t=4.816,3.792,all P<0.05).The rate of spontaneous delivery in the observation group(80.49%)was higher than that in the control group(22.22%),and the incidence rates of cesarean section,early spontaneous abortion,preeclampsia and stillbirth were lower than those in the control group,the differences between the two groups were statistically significant(χ2 =29.132,4.051,4.398,5.386,4.836,all P<0.05).The scores of BSID,MDI and PDI of the observation group were(121.15 ±8.55)points,(120.89 ± 11.68)points and(118.57 ±6.52)points,respectively,which were higher than those of the control group [(103.76 ± 7.49)points,(106.15 ±9.73)points,(104.14 ±5.27)points](t=10.052,6.378,11.331,all P<0.05).The incidence rate of complication in the observation group was 4.88%,which was lower than 57.78%in the control group,the effective rate in the observation group was 95.12%,which was significantly higher than that 57.78%in the control group,the differences between the two groups were statistically significant(χ2 =27.341,16.210,all P <0.05).Conclusion L-thyroid hormone can effectively improve the serum hCG and SOD levels in pregnant women,improve the pregnancy outcome of pregnant women with hypothyroidism,reduce the incidence rate of complications and promote the healthy growth of newborns.

Objective:To discuss the effective therapeutic regimen and the thoughts of pharmaceutical care for subclinical hypothy-roidism complicated with anemia and hemorrhoids during pregnancy. Methods:During the drug treatment process, clinical pharmacist provided pharmaceutical care for the patient,and the results were summarized and analyzed. Results: Clinical pharmacist performed pharmaceutical care for the treatment process of hypothyroidism, anemia and hemorrhoids during pregnancy, and when the oral iron treatment of hemorrhoids was with poor efficacy, clinical pharmacist analyzed and discussed the reasons and put forward reasonable sug-gestions. Conclusion:Subclinical hypothyroidism complicated with anemia and hemorrhoids during pregnancy treated with iron agent and wheat cellulose particles must pay attention to the drug interactions. The administration time of levothyroxine sodium and that of i-ron preparations and wheat cellulose particles should be staggered at least 2 hours. Because hypothyroidism can cause iron malabsorp-tion, intravenous iron therapy may be considered when oral iron supplementation shows poor effect.

Objective To observe the changes of serum lipid and liver enzyme after levothyroxine sodium treatment in patients with primary hypothyroidism.Methods Data from 104 patients with primary hypothyroidism together with increased serum lipid and liver enzyme were collected from August 2006 to November 2015.The date were compared of the changes of thyroid three triiodothyronine (T3),thyroxine (T4),free three triiodothyronine (FT3),free thyroxine thyroid (FT4),thyroid stimulating hormone (TSH),total cholesterol (TC),glycerol triester (TG),high density lipoprotein (HDL-C),low density lipoprotein (LDL-C),alanine aminotransferase (ALT),aspartate aminotransferase (AST) before the levothyroxine sodium treatment and after three month of the treatment.Results After three months of levothyroxine sodium treatment,thyroid function (T3,T4,FT3,FT4,TSH) gradually returned to normal levels [(2.02 ± 0.40) vs (0.96 ± 0.24) nmol/L,(95.76 ± 15.23) vs (24.31 ± 9.99) nmol/L,(4.79 ± 0.58) vs (1.96 ±0.57) pmol/L (15.15 ± 1.77) vs (4.70 ± 1.57) pmol/L,(3.05 ± 1.00) vs (82.41 ± 17.18) mU/L,t =-23.57,-44.13,-34.03,-46.48,48.07,P < 0.01],TC,TG,LDL-C,ALT,AST apparently decreased [(4.30 ± 0.67) vs (7.54 ± 1.26) mmol/L,(2.05 ± 0.34) vs (2.14 ± 0.47) mmol/L,(2.35 ± 0.49) vs (4.28 ± 0.88) mmol/L,(23.31 ±3.92) vs (46.27 ± 0.98)U/L,(26.63 ± 4.64) vs (55.33 ± 11.99) U/L,t =33.68,4.24,24.87,29.36,33.83,P <0.01],the decrease was less obvious than before treatment in HDL-C [(1.62 ± 0.36) vs (1.63-± 0.38) mmol/L,t =1.49,P > 0.05].Conclusion Levothyroxine sodium treatment in patients with hypothyroidism can effectively reduce blood lipid and liver enzyme content.

ABSTRACT

Objective To observe the effect ofJiuwei-Sanjie capsuls combined with levothyroxine sodium on IL-17 and IL-23 expression in the patients with Hashimoto’s thyroiditis (HT). Methods A total of 76 HT patients were randomly divided into the control group (n=38) and observation group (n=38). The control group was treated with Levothyroxine Sodium, and the observation group was treated with Levothyroxine Sodium andJiuwei-Sanjie capsuls. The expression of FT3, FT4, TSH, TGAb, and TPOAb were detected by chemiluminescent immunoassay. The expression of IL-17 and IL-23 were detected by ELISA analysis. The clinical effect were compared.ResultsThe clinical effect was 94.7% (36/38) in observation group, which was better than that of 81.6% (31/38) in control group (χ2=2.316,P=0.042) . After treatment, the expression of IL-17 (22.32 ± 4.25 ng/mlvs. 31.55 ± 6.84 ng/ml,t=2.416) and IL-23 (14.72 ± 3.42 ng/mlvs. 23.27 ± 5.26 ng/ml, t=2.512) in observation group was lower than that in control group (P expression in patients with Hashimoto's thyroiditis, and improve thyroid function.

ABSTRACT

Objective To evaluate the efficacy and safety of levothyroxine sodium combined with Guipi pills in the treatment of hypothyroidism . Methods A total of 114 cases from August 2013 to November 2015 in Lanxi Traditional Chinese Medicine Hospital with hypothyroidism were selected and randomly divided into control group and experiment group, 57 cases in each group.The patients in the both groups were given conventional treatment and symptomatic treatment.Patients in the control group were given levothyroxine sodium tablets 25 μg orally once a day, two weeks after continuous treatment according to the patient’s condition to 50μg.Patients in the experiment group were given levothyroxine sodium tablets 25μg orally once a day combined with Guipi pills 1.2g, three times a day.Two groups of patients a cycle of 28days, a total of 6 cycles of treatment.The clinical efficacy, thyroid hormone, lipid metabolic indexes and adverse drug reactions were compared between two groups.Results After treatment, the total effective rate of the control group was 78.95%, significantly lower than 92.98% of experimental group (P <0.05).The level of serum TSH, TC and TG were significantly decreased in two groups after treatment, and the experimental group was significantly lower than the control group(P<0.05); the levels of FT3 and FT4 were significantly increased, and the experimental group was significantly higher than the control group ( P <0.05 ).The incidence of adverse reactions was 10.53% in the control group and 5.26% in the experimental group, the difference was not statistically significant. Conclusion Levothyroxine sodium combined with Guipi pills in the treatment of hypothyrosis was effective with high safety.

ABSTRACT

Objective To observe clinical effect of self-made xiaozhongsanjie decoction combined with levothyroxine sodium tablets in treatment of thyroid nodule.Methods From July 2013 to July 2014, 52 cases of thyroid nodule patients were randomly divided into two groups: control group( n=25)and study group(n=27).The control group were received levothyroxine sodium tablets, while the study group were given self-made xiaozhongsanjie decoction on the basis of control group.Both groups were treated for 24 weeks.The changes of thyroid stimulating hormone ( TSH ) , serum free triiodothyronine (FT3), serum free tetraiodothyronine (FT4) levels, and the thyroid nodule size of two groups before and after treatment were recorded and compared.ResuIts After 24 weeks’ treatment, the thyroid nodule size of both groups reduced, and the descending range of study group was much larger than that of control group (P<0.05).The TSH levels of both groups descended, and the descending range of study group was much larger than control group (P<0.05).While the FT4 levels all increased in both groups, and the ascending range of study group was much larger than control group (P<0.05).And the total effective rate of study group was 77.78%, which was significantly higher than control group (48.00%, P<0.05). ConcIusion The therapy of self-made xiaozhongsanjie decoction combined with levothyroxine sodium tablets in treatment of thyroid nodule could control or reduce the thyroid nodule size effectively.

ABSTRACT

Objective To observe the clinical effects of modified Shengxian decoction in the treatment of thyroid cancer postoperative hypothyroidism patients.Methods 60 patients of thyroid cancer postoperative hypothyroidism due to deficiency of spleen and kidney were randomly divided into a control group and a treatment group.The control group (28 cases) was by levothyroxine sodium tablets conventional treatment,while the treatment group (32 cases) was treated with Shengxian decoction based on thecontrol group.The changes of the symptom integral and HAMD-17 were observed between two groups before and after treatment.Results After treatment,the total effective rate was 87.5% in the treatment group and 64.3% in the control group,the clinical effect of treatment group was significantly better than the control group (x2=4.499,P< 0.05).After treatment,the total score of HAMD-17 was 6.8±2.3 in the treatment group and 15.8±2.1 in the control group,both significantly improved than that before the treatment (which were 19.4±1.45 and 18.9± 1.32 respectively).Compared with the control group,the treatment group improved significantly (P< 0.01).Conclusion Shengxian decoction plus western medicine has remarkable curative effect in the treatment of thyroid cancer postoperative hypothyroidism,it can improve the depressive state of patients and improve the quality of life.

ABSTRACT

Cronkhite-Canada Syndrome (CCS) is a rare, non-inherited hamartomatous polyposis syndrome. It is characterized by diffuse gastrointestinal polyposis, diarrhea, weight loss and unique ectodermal manifestations such as alopecia, onychodystrophy and cutaneous hyperpigmentation. The etiology of CCS remains unknown, but an autoimmue contribution to the pathogenesis has been postulated. We report here on a 79-year-old male who has been diagnosed with CCS. He had been taking levothyroxine sodium after total thyroidectomy. To the best of our knowledge, this is the first case report of CCS in association with hypothyroidism and supplementation with levothyroxine sodium in the Korean medical literature.
