ABSTRACT

Introduction: Forensic odontology involves the collection, management, interpretation, evaluation, and presentation of dental evidence for criminal or civil legal proceedings, a combination of various aspects of the dental, scientific and legal professions. Bite mark analysis is an essential aspect of it that can significantly impact the courtroom or when considering the care of children and women deemed at risk. Materials and Methods: Data were obtained and analyzed from previously published literature and electronic database searches of relevant published literature from PubMed and Google Scholar. Forensic odontologists examine, interpret, analyze, and prepare reports on bruises or marks thought to be caused by teeth and, on occasion, are cross-examined in a court of law. Hence, they are considered essential in forensic dentistry in solving crimes to find either suspects or even victims in self-inflicted injuries as they are caused either by humans or animals that reveal biting edges, spacing, restorations, missing teeth, broken teeth, or supernumerary teeth unique to that individual. The main reasons for causation are sexual crimes, homicides, child abuse, and violent fights. There are many methods to analyze, that are classified into manual, semi-automatic, and automatic methods. Objectives: This review highlights the bite marks in detail about their characteristics, methods of collection and different methods of analysis and its applications in forensic odontology. Conclusion: This review shows different methods of analyzing bitemarks and explains the advantages, disadvantages, and limitations, and identifies which method is better advisable and adaptableforanalysis.

ABSTRACT

O presente artigo tem como objetivo analisar a permanência de marcas ditatoriais pós-transição democrática no Brasil a partir de narrativas obtidas em dois estudos psicossociais realizados em Heliópolis - periferia urbana da cidade São Paulo. O primeiro focou a construção e elaboração das memórias de violências do/no Estado de nove jovens moradores, enquanto o segundo reconstruiu as memórias sobre as violações aos direitos humanos de um grupo de mães de adolescentes em medida socioeducativa e de técnicos dos serviços que os acompanham. A partir dos conceitos de violência, trauma psicossocial e memória histórica de Ignacio Martín-Baró, apresenta-se uma análise da persistência das marcas ditatoriais na sociedade brasileira, que atingem, sobretudo, a população jovem negra e periférica sob a forma do genocídio, exclusão, suspeita e violência sistemática dos agentes de Estado.


This article aims to analyze the permanence of dictatorial marks after the democratic transition in Brazil from narratives obtained in two psychosocial studies carried out in Heliópolis - urban outskirts of the city of São Paulo. The first focused on the construction and elaboration of the memories of State violence of nine young residents, while the second reconstructed the memories about the human rights violations of a group of adolescent's mothers in correctional measures and technicians from the services that accompany them. Based on the concepts of violence, psychosocial trauma and historical memory rescued from Ignacio Martín-Baró, an analysis of the persistence of dictatorial marks in Brazilian society is presented, which mainly affects the young black and peripheral population in the form of genocide, exclusion, suspicion and systematic violence by State agents.


El presente artículo tiene como objetivo analizar la permanencia de huellas dictatoriales después de la transición democrática en Brasil desde las narrativas obtenidas en dos estudios psicosociales realizados en Heliópolis - periferia urbana de la ciudad de São Paulo. El primero se centró en la construcción y elaboración de los recuerdos de violencia en / del Estado de nueve jóvenes residentes, mientras que el segundo reconstruyó los recuerdos sobre las violaciones de los derechos humanos de un grupo de madres de adolescentes que cumplen medidas socioeducativas y técnicos de los servicios que los acompañan. Desde los conceptos de violencia, trauma psicosocial y memoria histórica de Ignacio Martín-Baró, se presenta un análisis de la persistencia de huellas dictatoriales en la sociedad brasileña que afecta, sobre todo, a la población joven negra y periférica en la forma de genocidio, exclusión, sospecha y violencia sistemática por parte de agentes estatales.

Introdução: Estrias são alterações no tecido tegumentar, principalmente nas fibras de elastina e colágeno. São frequentemente acompanhadas de problemas psicossociais e de autoestima. Existem diversas técnicas utilizadas, como o microagulhamento, que estimula a produção de colágeno e a dermopigmentação, que camufla as estrias por meio da pigmentação. Objetivo: Verificar a influência do microagulhamento associado a micropigmentação em estrias albas de mulheres. Métodos: Realizou-se um protocolo de Estriaderme em três mulheres de fototipo de pele II a V, de idade média de 27 ± 0,3 anos com estrias albas nos glúteos. O protocolo foi composto por 5 sessões com intervalos quinzenais, em quatro foram realizadas a micropuntura associada ao ácido hialurônico e em uma realizou-se a micropigmentação. As participantes foram avaliadas por fotografias e paquímetro. Resultados: No pós-tratamento, observou-se uma melhora estética nas regiões de aplicação, o tecido tegumentar demonstrou-se mais uniforme, homogêneo e com maior jovialidade. Com redução na espessura das estrias e uma média de redução de 1,8 mm em relação ao pré-tratamento (média ± 3 mm). Conclusão: O protocolo Estriaderme demonstrou ter efeito positivo sobre a pele estriada, no que se refere a sua vascularização, vitalidade, pigmentação, relevo cutâneo, espessura e quantidade. (AU)


Introduction: Stretch marks are alterations in the integumentary tissue, mainly in the elastin and collagen fibers. They are often accompanied by psychosocial and selfesteem problems. Several techniques are used, such as micro-needling, which stimulates collagen production and dermopigmentation, which camouflages stretch marks by pigmenting the area. Objective: To verify the influence of micropuncture associated with micropigmentation on female striae alba. Methods: A Striaderm protocol was performed on three women of skin phototype II to V according to Fitzpatrick, mean age 27 ± 0.3 years with gluteus alba striae. The protocol consisted of 5 sessions at biweekly intervals, four underwent micropuncture associated with hyaluronic acid and one underwent micropigmentation. The participants were evaluated by photographs and caliper. Results: Post-treatment showed an aesthetic improvement in the application regions, the integumentary tissue was more uniform, homogeneous, and more youthful. With reduction in stretch marks and an average reduction of 1.8 mm compared to pretreatment (mean ± 3 mm). Conclusion: The Striaderme protocol has been shown to have a positive effect on striated skin with respect to its vascularity, vitality, pigmentation, skin relief, thickness, and quantity. (AU)

RESUMEN: El análisis de huellas de mordedura para identificación forense puede ser clave para establecer la inocencia o culpabilidad del acusado. Sin embargo, esta evidencia ha sido cuestionada por su falta de consenso y objetividad, y por sus reportadas identificaciones erróneas. Se presenta una revisión con búsqueda sistemática de casos indicando errores diagnósticos y sus caracterizaciones, y se discuten las recomendaciones realizadas para evitarlos. Se utilizó la estrategia ("bite mark" OR "bitemark") AND ("artefactual" OR "misidentification" OR "misdiagnosis" OR "mistake"), incluyendo casos de huellas de mordedura humana en piel con reportes de errores diagnósticos, excluyendo diseños experimentales, huellas en alimentos u objetos. Fueron seleccionados un total de 13 documentos reportando 21 casos, abarcando los años 1989 a 2014. Los patrones morfológicos con mayor reporte de error diagnóstico fueron los producidos por trauma cortante o contuso por otras causas (10 casos), seguidos de los producidos por condiciones médicas y tratamientos de emergencia (6 casos). Al menos tres de los casos tomaron estado público, dos de ellos con sentencia de muerte y uno con condena de 25 años a prisión perpetua. Se ha enfatizado el dar minuciosidad y estandarización a la evaluación morfológica de este tipo de evidencias por la importante cantidad de condenas erróneas. Esta revisión pone en evidencia la escasa exposición y análisis de estos errores, con la insuficiente información para un aprendizaje significativo crítico y diseño de herramientas para evitarlos. Se coincide en la necesidad de estandarizar y optimizar protocolos para estas evidencias y adoptar una conducta reflexiva para sus análisis con fines de identificación forense.


SUMMARY: Bite mark analysis for forensic identification can be key to establishing the innocence or guilt of the defendant. However, this evidence has been challenged for its lack of consensus and objectivity, and its reported misidentifications. We present a scoping review of cases indicating misidentifications and their characterizations, and recommendations for avoid them are discussed. The strategy ("bite mark" OR "bitemark") AND ("artifactual" OR "misidentification" OR "misdiagnosis" OR "mistake") was used, including cases of human bite marks on skin with reported misdiagnosis, excluding experimental designs, bite marks on food or objects. A total of 13 documents were selected reporting 21 cases, covering the years 1989 to 2014. The morphological patterns with the highest report of misdiagnosis were those produced by cutting or blunt trauma due to other causes (10 cases), followed by those produced by medical conditions and emergency treatments (6 cases). At least three of the cases became public, two of them with a death sentence, and one with a 25-year sentence to life in prison. The thoroughness and standardization of the morphological evaluation of this type of evidence has been emphasized due to the significant number of erroneous convictions. This review highlights the scarce exposure and analysis of these errors, with insufficient information for critical meaningful learning and the design of tools to avoid them. There is agreement on the need to standardize and optimize protocols for these evidences and adopt a reflective behavior for their analysis for forensic identification purposes.

Introduction: The Pacific white-spotted eagle ray Aetobatus laticeps, has recently separated from the Atlantic A. narinari based on both morphological and genetic evidence. This species is characterized by a dark body with numerous white spots all over its dorsal side. Considering the type, shape, number, and distribution of these natural markings as potential identifiers at the individual level, we studied the variation in the spot patterns. Objective: Describe and compare the white spot pattern (type and distribution) of individuals and evaluate their potential use as identifiers at the individual level. Methods: We analyzed 54 videos (105 subsequent extracted photos) and 19 photographic records that were taken at different sites along the Pacific coast of northern Costa Rica. Results: Seventeen distinctive types of white spots were identified across the entire dorsal side of the rays. Significant differences between each major body section (pectoral fins, back, head, and pelvic fins) were found in the type and frequency of white spots. The type 'single spot' was commonly distributed across the entire dorsal side, and the spot pattern on the pelvic fins was informative to identify 72 individuals. Conclusions: The analysis of the type, shape, and distribution of white spots in A. laticeps determined several combinations of white spot patterns that be used for further taxonomic description and provide potential identification of the individual for future population studies along with its distribution.


Introducción: La raya águila de manchas blancas del Pacífico, Aetobatus laticeps, se ha separado recientemente de A. narinari del Atlántico basándose en pruebas tanto morfológicas como genéticas. Esta especie se caracteriza por un cuerpo oscuro con numerosas manchas blancas en toda su parte dorsal. Considerando el tipo, la forma, el número y la distribución de estas marcas naturales como identificadores potenciales a nivel individual, estudiamos la variación en los patrones de manchas. Objetivo: Describir y comparar el patrón de manchas blancas (tipo y distribución) de individuos y evaluar su uso potencial como identificadores a nivel individual en especímenes recapturados. Métodos: Analizamos 54 videos (con 105 fotografías extraídas posteriormente) y 19 registros fotográficos que fueron tomados en diferentes sitios a lo largo de la costa pacífica del norte de Costa Rica. Resultados: Se identificaron diecisiete tipos distintivos de manchas blancas en todo el lado dorsal de los radios. Se encontraron diferencias significativas entre cada sección principal del cuerpo (aletas pectorales, espalda, cabeza y aletas pélvicas) en el tipo y frecuencia de manchas blancas. El tipo 'punto único' se distribuyó comúnmente en todo el lado dorsal, y el patrón de puntos en las aletas pélvicas fue informativo para identificar 72 individuos. Conclusiones: El análisis del tipo, la forma y la distribución de las manchas blancas en A. laticeps determinó varias combinaciones de patrones de manchas blancas que se utilizarán para una descripción taxonómica adicional y brindan una identificación potencial del individuo para futuros estudios de población a lo largo de su distribución.

RESUMEN: El aprendizaje de la Anatomía puede resultar un proceso dificultoso y poco motivador para el estudiante, esto debido a su complejo vocabulario. Uno de los desafíos iniciales en el proceso de enseñanza-aprendizaje de la Anatomía Humana es el abordaje de los términos generales del sistema musculoesquelético agrupados con los nombres de reparos, marcas óseas o accidentes de la superficie del hueso. Estos suelen ser mencionados con una escasa o nula definición en los recursos bibliográficos. El objetivo de este trabajo fue revisar los recursos bibliográficos que efectivamente cuentan con definiciones de los términos denominados como reparos, marcas óseas o accidentes de la superficie del hueso, con la finalidad de aportan información aclaratoria sobre el uso de estos términos óseos y así facilitar los procesos de enseñanza-aprendizaje de la Anatomía Humana. Los términos utilizados comúnmente para describir reparos, marcas óseas o accidentes de la superficie del hueso fueron analizados en Terminologia Anatomica (FIPAT, 2019) y en cinco textos de Anatomía Humana. Para facilitar la comprensión, los resultados fueron clasificados y tabulados Se pudo evidenciar una escasez de recursos bibliográficos con definiciones de los reparos, marcas óseas o accidentes de la superficie del hueso, la mayoría de estos términos óseos se encontraron en el sistema musculoesquélico, sin embargo, no son exclusivos de este sistema, además, se observaron errores conceptuales en las definiciones de algunos de los términos óseos. Una manera de comprender mejor estos términos, es revisar de manera frecuente la Terminologia Anatomica.


SUMMARY: Learning Anatomy can be a difficult and unmotivating process for the student, due to its complex vocabulary. One of the initial challenges in the teaching-learning process of Human Anatomy is the approach to the general terms of the musculoskeletal system grouped with the names of repairs, bone marks or accidents on the surface of the bone. These are usually mentioned with little or no definition in bibliographic resources. The objective of this work was to review the bibliographic resources that effectively have definitions of the terms known as repairs, bone marks or accidents on the bone surface, in order to provide clarifying information on the use of these bone terms and thus facilitate the teaching-learning processes of Human Anatomy. The terms commonly used to describe repairs, bone marks or accidents on the bone surface were analyzed in Terminologia Anatomica (FIPAT, 2019) and in five texts on Human Anatomy. To facilitate understanding, the results were classified and tabulated. There was a lack of bibliographic resources with definitions of the repairs, bone marks or accidents on the bone surface. Most of these bone terms were found in the musculoskeletal system. However, they are not exclusive to this system, and conceptual errors were observed in the definitions of some of the bone terms. One way to better understand these terms is to frequently review the Terminologia Anatomica.

La Tafonomía, concepto reconocido en paleontología y antropología, permite estimar tiempo y circunstancias de muerte mediante el estudio de condiciones post-mortem y factores relativos a la supervivencia de restos óseos. Entre las diferentes variables tafonómicas, Binford (1981) propuso una categorización de marcas de mordedura, hoy reconocido estándar para estos estudios. Sin embargo, su diseño original en inglés podría llevar a malinterpretaciones o informes erróneos por no poseer una traducción validada. Se realizó una revisión con búsqueda sistemática de artículos en español, sin límite temporal, que citaran a Binford ("Bones: Ancient Men and Modern Myths") en Google Scholar. Fueron incluidos aquellos que mencionaran esta metodología para categorizar marcas de mordedura como agente tafonómico. Se excluyeron tesis, libros, pósters y resúmenes de congresos. Se identificaron 349 documentos y se seleccionaron 83 según criterios. El 65,12 % refirió sólo a huellas de mordedura animal sin especificar categorías; el 26,51 % mostró traducciones libres o categorizaciones parciales, y el 8,43 % realizó categorizaciones sin traducción. La mayor cantidad de citas fueron realizadas por autores de Argentina y España (54,22 % y 24,10 % respectivamente, como única filiación). Las categorías con más traducciones libres fueron "pitting" y "puncture" (11 y 8 términos diferentes respectivamente). Si bien la categorización de Binford es citada profusamente en países de habla hispana para definir las marcas de mordedura animal en hueso, la ausencia de transculturización conlleva falta de consenso para la comunidad científica. Más allá de una traducción literal, la adaptación y estandarización de estos términos es necesaria para validar esta metodología.


Taphonomy, a concept recognized in paleontology and anthropology, allows the estimation of time and circumstances of death by studying post-mortem conditions and factors related to the survival of bone remains. Among the different taphonomic variables, Binford (1981) proposed a categorization of bite marks, now recognized as standard for these studies. However, its original design in English could lead to misinterpretations or erroneous reports for not having a validated translation. A scoping review was conducted for articles in Spanish, without a time limit, that cited Binford ("Bones: Ancient Men and Modern Myths") in Google Scholar. Those who mentioned this methodology to categorize bite marks as taphonomic agent were included. Theses, books, posters and conference proceedings were excluded. We identified 349 documents, and 83 were selected according to criteria. 65.12 % referred only to animal bite marks without specifying categories; 26.51 % showed free translations or partial categorizations, and 8.43 % made categorizations without translation. The largest number of citations were made by authors from Argentina and Spain (54.22 % and 24.10 % respectively, as the only affiliation). The categories with the most free translations were "pitting" and "puncture" (11 and 8 different terms respectively). While Binford's categorization is widely cited in Spanish-speaking countries to define animal bite marks in bone, the absence of transculturation implies a lack of consensus for the scientific community. Beyond a literal translation, the adaptation and standardization of these terms is necessary to validate this methodology.

RESUMEN El Liquen Plano Pigmentado es una dermatosis crónica frecuente de etiología aún desconocida, caracterizada clínicamente por la presencia de pápulas purpúreas, poligonales, pruriginosasdistribuidas en forma simétrica. ElLiquen Plano Pigmentado representa una de todas las variantes clínicas, cuya característica histológica distintiva es la incontinencia de pigmento melánico. Su curso es autolimitado pues se resuelve en un período de meses a años, pero puede persistir indefinidamente. Existen múltiples opciones de tratamiento, que incluyen corticoides tópicos, intralesionales y sistémicos, retinoides, fototerapia, inmunomoduladores tópicos y, en casos graves o resistentes al tratamiento, la ciclosporina, metotrexato. La dermatoscopía es una técnica auxiliar de diagnóstico no invasivaque permite incrementar la certeza diagnóstica en diversasenfermedades cutáneas; en el caso del Liquen Plano Pigmentado se observa la lesión clásica que consiste en una pápula poligonal aplanada, de tonalidad eritemato-violácea que suele tener finas escamas adherentes en la superficie,denominadasestrías de Wickham, queconsisten en estructuras polimórficas blanquecinas y que corresponden histológicamente a zonas de ortoqueratosis.Es una herramienta de gran utilidad para el diagnóstico de estas lesiones.


ABSTRACT The Pigmented Flat Lichen is a frequent chronic dermatosis of etiology still unknown, clinically characterized by the presence of purpuric, polygonal, pruritic papules distributed symmetrically. The Pigmented Flat Lichen represents one of all clinical variants, where the distinctive histological feature is melanin pigment incontinence. Its course is self-limited, it resolves over a period of months to years, but it can persist indefinitely. There are multiple treatment options that include topical, intralesional and systemic corticosteroids, retinoids, phototherapy, topical immunomodulators and in severe or resistant cases to treatment, cyclosporine, methotrexate. Dermatoscopy is a non-invasive diagnostic auxiliary technique that allows accelerating diagnostic certainty in various skin diseases; in the case of the Pigmented Flat Lichen, the classic lesion is observed, consisting of a flattened polygonal papule, with an erythematous-violet hue that usually has fine adherent scales on the surface, called Wickham striae, which consists of whitish polymorphic structures and that corresponds histologically to areas of orthokeratosis. It is a very useful tool for the diagnosis of these lesions.

ABSTRACT

Dog bites are a known public health problem involving physical, mental and emotional traumas. From a forensic point of view, it has been stated that their morphological characters, and the intercanine and interincisive measurements, could allow a taxonomic and specific identification of the implicated animal. The aim of this study was to differentiate and identify the biological profile of a potential aggressor dog by analysing eight morphometric bite patterns belonging to three different dog breeds. The data obtained were analyzed following three categories: a) breeds; b) sexes among breeds; and c) sex within breed. Significant differences were detected among the variables (p≤ 0.05), but only the maximum maxillary intercanines width (MaxCW) allowed a breed differentiation. The other variables allowed a differentiation between two breeds or one breed over the others. The principal components analysis (PCA) allowed visualizing the degree of dispersion and relationship among the scores. It showed three well-defined and separated breed groups, and different degrees of dispersion within and among breeds. The most important variable for such a differentiation was MaxCW. When considering sex among breeds for males, it showed a statistically significant difference, but only the diastema located between the third left mandibular, incisive and the left mandibular canine (C-I-ManL) allowed breed differentiation. For females, only MaxCW allowed a differentiation among breeds. The multivariate analysis permitted with a 95 % confidence interval, a breed and sex differentiation. Besides, the PCA models allowed classifying, identifying, separating and graphically showing the relationship among the variables. This made it possible to differentiate between breeds and sexes. Due to the large range of dog breeds around the world, this multivariate analysis could also help determining the dog's weight and size, narrowing down towards an approximate number of offending dogs, focussing on a certain kind of dog breed, and pinpointing any suspect dog.


Las mordeduras de perros son un conocido problema de salud pública que involucra traumas físicos, mentales y emocionales. En la faz forense, se ha establecido que características morfológicas, así como las medidas intercanina e interincisiva, permitirían una identificación taxonómica y especifica del animal involucrado. El objetivo fue diferenciar e identificar el perfil biológico de un potencial perro agresor analizando ocho patrones morfológicos de mordeduras pertenecientes a tres diferentes razas caninas. Los datos fueron analizados considerando tres categorías: a) razas; b) sexo entre razas; y c) sexo en cada raza. Se observaron diferencias entre las variables (p≤0,05), pero sólo la distancia máxima entre caninos maxilares (MaxCW), permitió una diferenciación entre razas. Las otras variables permitieron una diferenciación entre dos razas o de una raza sobre las otras. El análisis de componentes principales (PCA) permitió visualizar el grado de dispersión y la relación entre las puntuaciones (dentro y entre razas). Se observaron tres grupos (razas) bien definidos y separados con diferentes grados de dispersión dentro y entre razas. La variable más importante para tal diferenciación fue la MaxCW. Al considerar sexo (machos) entre razas, se encontró diferencia estadísticamente significativa, pero sólo la medida del diastema localizado entre el tercer incisivo y canino izquierdo mandibulares (C-I-ManL) permitió la diferenciación entre razas. Para hembras, sólo la MaxCW permitió una diferenciación entre razas. El análisis multivariante permitió, con un intervalo de confianza del 95 %, diferenciar raza y sexo. El modelo PCA permitió además clasificar, identificar, separar y mostrar gráficamente la relación entre las variables. Esto posibilitó diferenciar entre razas y sexos. Debido a la gran variedad de razas de perros en el mundo, este análisis multivariado permitiría estimar peso y tamaño del animal, indicando un número aproximado de perros atacantes, centrándose en ciertos tipos de raza, y localizar a cualquier cánido sospechoso.

ABSTRACT

With the ever-increasing crime rate in our society, the field of forensic sciences has become highly evolved. Forensic dentists play a pivotal role in various areas of crime scene investigations and thereby help solve innumerable mysteries. Teeth appear to be vital pieces of evidence in several such investigations. Teeth are preserved in the closed cavities of the mouth and are generally resistant to the threatening environmental conditions that may be associated with the death of an individual, making them very useful in postmortem analysis. Teeth thus obtained may be useful in age estimation of the deceased victim or in determining his blood group. Identification of individuals in mass disasters can also be performed based on the unique morphological characteristics of the human dentition and through dental DNA fingerprinting. Again teeth play an all important role in catching a culprit through the positive correlation of the bite marks left behind at the crime scene and the suspect's own teeth marks. Thus, teeth prove to be an important adjunct in forensics. Its scope is ever-increasing with time, and a great amount of research is being carried out to implement the same. A PubMed, MEDLINE, and Scopus search was conducted of the past 70 years using several search terms like “Forensic odontology,” “history of forensic odontology,” “dental DNA fingerprinting,” “forensic age estimation,” “age estimation from teeth” and “bitemarks.” Other articles and textbook references which were considered to be important were also included in this study. The articles gathered were divided into the following groups: history of forensic odontology, teeth and DNA (dental DNA fingerprinting), teeth and blood grouping, teeth and age estimation, and teeth in bite marks.

ABSTRACT

Estrias de distensão são cicatrizes na derme que constrangem os pacientes e oferecem desafios ao tratamento clínico. Resultam do rápido estiramento da pele, frequentemente presente em adolescentes, gestantes, obesos e indivíduos com rápida mudança de peso. Apresentam-se inicialmente como placas eritematosas e edemaciadas (estrias rubras) e, conforme amadurecem, tornam-se esbranquiçadas e atróficas (estrias albas), devido à degradação e reorganização de fibras de elastina e de colágeno. Atualmente, lasers representam modalidade terapêutica não invasiva emergente, que já demonstrou sucesso na redução da vascularização excessiva das estrias rubras, e no estímulo à produção de colágeno e elastina nas albas. Foi realizada revisão da literatura no PubMed referente ao tratamento de estrias atróficas com laser de janeiro de 2000 até dezembro de 2016. Os autores encontraram 28 artigos que se enquadraram nos critérios de inclusão. Existem poucos estudos clínicos randomizados avaliando a eficácia ao longo prazo e a segurança de aparelhos à base de energia. Baseado em casos clínicos e revisões sistemáticas, ambos os lasers - ablativo e não ablativo - fracionados demonstraram melhora modesta do aspecto das estrias distensas. Há tendência a sugestão do laser não ablativo fracionado de 1.540 nanômetro como sendo opção de primeira escolha para tratamento das estrias distensas. Porém, estudos futuros devem focar em desenhos de pesquisa com duração mais longa, medidas objetivas e padronizadas da avaliação dos resultados como biópsias e estudos moleculares, que demonstrem aumento nas fibras elásticas e fibras de colágeno, que correlacionem com a melhora do aspecto clínico das estrias após aplicação de lasers com parâmetros sistematizados.


Stretch marks are scars on the dermis that cause patients to be self-conscious and that pose challenges in clinical treatment. They result from rapid stretching of the skin and often observed in adolescents, pregnant women, obese individuals, and people experiencing rapid change in weight. They initially appear as erythematous and edematous plaques (red striae), and as they mature, they become whitish and atrophic (striae alba) due to the degradation and reorganization of elastin and collagen fibers. Currently, laser treatment is an emerging noninvasive therapeutic modality that is successful in reducing the excessive vascularization of red striae and in stimulating the production of collagen and elastin in the alba. In the present literature review, PubMed was searched for articles on the treatment of atrophic striations with laser that were published from January 2000 to December 2016. The authors have found 28 articles that met the inclusion criteria. Only few randomized clinical trials have evaluated the long-term efficacy and safety of energybased treatments. Based on clinical cases and systematic reviews, both ablative and non-ablative fractional lasers caused moderate improvement in the appearance of distending striae. The 1540-nm non-ablative fractional laser is more likely to be considered the first-line treatment for stretch marks. However, future studies should focus on research with longer duration, studies with objective and standardized measures for the evaluation of results, such as biopsy results, and molecular studies showing an increase in elastic and collagen fibers that correlate to the improvement in the clinical appearance of the striae after using lasers with systematized parameters.

ABSTRACT

Objective The present study aims to investigate the effect of the early nasal jejunum nutrition (NJN) on the inflammatory markers of severe acute pancreatitis (SAP) and to research if the NJN improves the recovery of SAP. Method SAP inpatients were observed during 2016. One group who accepted the NJN was experimental group and another group who did not accept the NJN was control group.The difference of serum albumin (ALB), procalcitonin (PCT), C reactive was compared to observe the effect of the early nasal jejunum nutrition (NJN) on the inflammatory markers of severe Acute Pancreatitis protein (CRP), interleukin 6 (IL-6) and serum amylase (AMY) between the highest and lowest levels in two group respectively and the time of diet recovery between two groups. Results (1) The difference of interleukin 6 (IL-6) between the highest and lowest levels in experimental group was significantly smaller than that of the control group;the difference of ALB, CRP and PCT between the highest and lowest levels in experimental group was not different from the control group;2) The time of diet recovery of experimental group was shorter than that of the control group. Conclusion The early NJN can reduce the IL-6 level and the time of diet recovery,and then,improve the SAP recovery.

ABSTRACT

Introducción El tratamiento estándar en estadios tempranos del cáncer de mama es la cirugía conservadora (cc) más radioterapia del volumen total mamario (wbi). Dado que la mayoría de las recidivas locales ocurren cercanas al sitio del tumor (85%) y existiendo la posibilidad de acortar tiempos de irradiación ­debido a que esta insume varias semanas­, podría no ser necesario irradiar toda la mama, sino el área donde estaba situado el tumor más margen de seguridad, lo que insume un menor tiempo. Este el fundamento de la Irradiación Parcial Acelerada Mama (apbi). Consensos internacionales proporcionan guías para administrar apbi fuera de ensayos clínicos. Objetivos El objetivo principal de este trabajo es determinar y analizar las características clinico-anatomopatológicas e inmunohistoquímicas de las pacientes que fueron operadas y luego seleccionadas para ser irradiadas con técnica de apbi con radioterapia externa con intensidad modulada guiada por imágenes (imrt). Material y método Se trata de un trabajo prospectivo, no randomizado. La muestra está constituida por las primeras 20 pacientes ingresadas al protocolo apbi. El reclutamiento se realizó entre marzo de 2012 y julio de 2014. Se analizaron las características clínico-patológicas e inmunohistoquímicas de esas pacientes luego de cc más biopsia del ganglio centinela (bgc). Resultados Media de seguimiento en meses: 37,7 (r: 22-51); edad media: 65,7 años. Tamaño tumoral medio por anatomía patológica: 12,03 mm. Media de número de ganglios centinelas extirpados: 1,7. Todos los tumores fueron unifocales. Los márgenes quirúrgicos resultaron negativos en todos los casos. Tumores Luminal A: 19 pacientes; tumores Luminal B: 1 paciente. Hormonoterapia adyuvante en todas las pacientes. Control locorregional: 100%. Conclusiones A pesar del corto tiempo de seguimiento y del escaso número de pacientes, este análisis sugiere que la apbi en el lecho del tumor marcado intraoperatoriamente con fiduciales de titanio es factible para pacientes que reúnen los criterios estrictos clínico-patológicos e inmunohistoquímicos acordes con las guías internacionales.


Introduction The standard conservative treatment for early stage breast carcinomas is the breast conservative surgery plus whole breast irradiation. Because most local recurrences occur close to the site of the primary tumor (80- 90%) and the possibility of shortening the irradiation times ­because it takes several weeks­, it may not be necessary to irradiate the entire breast, but the area where the tumor was located plus a margin of safety in less time. This is the basis of Accelerated Partial Irradiation of the Breast Objectives Determination and analysis of clinical-pathological and immunohistochemical characteristics in patients selected for Accelerated Partial Breast Irradiation (apbi) performed by imrt + igrt technique after breast conservative surgery plus sentinel node biopsy. Materials and method A prospective, non-randomized study of the first 20 patients performed apbi strictly following the recommendations of international consensus. It was initiated in March 2012 until July 2014. Clinical-pathological and immunohistochemical characteristics of these patients were analyzed to be selected for apbi, after breast conservative surgery plus sentinel node biopsy. Results Median follow up: 37.7 months (r: 22-51); average age: 65.7 years. Mean tumor size by pathological analysis: 12.03 mm. Mean number of sentinel nodes removed: 1.7. All tumors were unifocal. Negative surgical margins in all cases. Luminal A tumors: 19 patients; Luminal B tumors: 1 patient. Adjuvant hormone therapy in all patients. Loco-regional control: 100%. Conclusions Despite the short time of follow up and the small numbers of patients, this analysis suggests that apbi performed by imrt + igrt technique added to the placement of fiduciary marks at the time of surgery is feasible for highly selected patients who meet the clinical-pathological and immunohistochemical selection criteria according to international guidelines

ABSTRACT

ResumenO. niloticus se ha introducido en todo el mundo en reservorios de agua dulce y actualmente representa un alimento alternativo para las poblaciones. Dos marcas anuales en las escamas de tilapias se producen en estos ambientes, una de ellas se da por la época de reproducción mientras que la segunda se sugiere que se forma debido a bajas temperaturas, reducción en la oxigenación del agua y altas densidades entre otros estresores. Con el fin de explorar las causas de esta segunda marca se llevó a cabo un estudio experimental que analiza la influencia de cuatro factores en el crecimiento y en consecuencia en la producción de marcas del crecimiento en escamas: reducción de la cantidad de alimento, aumento en la densidad, disminución de la concentración de oxígeno y reducción de la temperatura. El experimento se realizó usando 5 tinas plásticas con 215 L de capacidad, cada una se subdividió en dos secciones para obtener una réplica por tratamiento. Se usaron 20 juveniles por réplica. Las condiciones control fueron las óptimas para la especie: 5 % del peso corporal como alimento, densidad de 3 peces /L, saturación de oxígeno de 4.98 ± 0.5 mg/L y temperatura de 25.0 ± 1.0 oC, mientras que los tratamiento variaron como sigue: Alimento (2.5 % del peso corporal como alimento); densidad (6 peces/L); oxígeno (2.8 ± 1.2 mg/L) y temperatura (19.6 ± 1.0 oC), donde la respuesta se estimó a través de las variaciones en longitud, peso y distancias inter-circulli. Cada tercer día se midió la concentración de fosfatos, amonio y nitratos con un fotómetro Hanna® HI8203. El experimento se dividió en tres etapas de mediciones de talla, peso y toma de escamas. 1) Etapa inicial: Antes de someter los organismos a experimentación. 2) Etapa final: al final del experimento y 3) Etapa compensatoria: donde todos las condiciones experimentales fueron llevadas a las condiciones del control. Todos los tratamientos causaron reducción en longitud, peso y distancias inter-circuli con respecto al control (6.7 ± 0.5 cm; 5.2 ± 1.2 g; 5.4 ± 0.5 mm), pero solo la temperatura y alimento fueron significativos (5.6 ± 0.4 y 5.8 ± 0.5 cm; 3.4 ± 0.7 y 3.3 ± 0.9 g; 3.4 ± 0.3 y 4.1 ± 0.6 mm para los tratamientos de temperatura y alimento, respectivamente) (P < 0.05). La experimentación tuvo una duración de tres semanas, y posteriormente los tratamientos fueron revertidos a condiciones control durante otras tres semanas donde se observó crecimiento compensatorio en los tratamientos de oxígeno y la densidad pero no de la misma manera para los estresores: temperatura y alimento. Las distancias inter-circuli fueron un buen indicador de períodos de estrés.


AbstractOreochromis niloticus has been introduced worldwide into freshwater reservoirs and represents an alternative food for local populations. Two annual marks on fish scales have been reported for fish in these environments, one of them is allocated to reproduction, while the other is thought to be formed following low temperatures, a reduction in oxygen concentration and/or high densities, among others. In order to explore the causes of this second ring, we designed an experimental study to analyze the influence of four factors: a reduction in food amount, an increase in density, a decrease in oxygen concentration and temperature. The experiment was carried out using five 215 L plastic tanks, each one divided into two sections, so as to have treatment replicates (each with 20 juveniles). The control conditions were optimal for the species: 5 % of body weight in food, a density of 3 fish/L, a total oxygen saturation of 4.98 ± 0.5 mg/L, and a temperature of 25.0 ± 1.0 oC; the treatments varied as follows: food of 2.5 % of body weight, density of 6 fish/L, oxygen of 2.8 ± 1.2 mg/L and a temperature of 19.6 ± 1.0 oC. Fish responses to stressors were estimated through variations in length, weight and inter-circuli spaces or circuli spacing in fish scales. Every third day it was measured the concentrations of ammonium, phosphates and nitrates with a photometer. Three stages for length/weight fish measurements and scales sampling were considered: 1) Initial stage: before experiment started, 2) Final stage: at the end of the experiment, and 3) Compensatory stage: when all the experimental treatments were adjusted to the Control conditions; the experiments lasted for three weeks. The results indicated that all the treatments caused a reduction in length, weight and inter-circuli spaces when compared to the controls (6.7 ± 0.5 cm; 5.2 ± 1.2 g; 5.4 ± 0.5 mm, respectively), but only the variables of temperature and food resulted significant (5.6 ± 0.4 and 5.8 ± 0.5 cm; 3.4 ± 0.7 and 3.3 ± 0.9 g; 3.4 ± 0.3 and 4.1 ± 0.6 mm for temperature and food treatments, respectively) (P < 0.05). When treatments were reversed to the control conditions for another three weeks compensatory growth was observed in the oxygen and density treatments, but not in the same way for the temperature and food treatments. We concluded that inter-circuli distances are a good indicator to stress periods in this species.

As estrias são cicatrizes indesejáveis que surgem na pele devido ao rompimento das fibras de colágeno e elastina presentes na derme, causando uma atrofia no tecido provocando o aparecimento de estrias, onde no início têm aspecto avermelhado, e quando se tornam "velhas" apresentam um aspecto esbranquiçado e abrilhantado (estrias nacaradas). O presente estudo contou com a participação de 16 mulheres iniciais, e ao final 3, com idades entre 18 e 35 anos que apresentavam estrias nacaradas na região abdominal pós-gravidez. Elas foram divididas em dois grupos: no Grupo A foi aplicada a eletrogalvanopuntura e, no Grupo B aplicada a acupuntura na técnica de pica-pau (sangria), no intuito de melhorar o aspecto do tecido estriado. Os resultados foram satisfatórios mostrando a eficácia das duas técnicas no tratamento das estrias nacaradas pós-gestacionais.(AU)


Stretch marks are unwanted scars that develop on the skin caused by the breakdown of collagen fibers and elastin found in the dermis, leading to tissue atrophy, forming the stretch marks, which are often red at the beginning, before gradually fading to a silvery white color. This study consisted initially of 16 women, and 3 at the end, aged 18 to 35, suffering of silvery white stretch marks on abdominal area after pregnancy. A sample of women was divided into two comparison groups: Group A used galvanic puncture, and Group B used the aloodletting acupuncture technique, to heal the stretch marks. The results were satisfactory and showed the significant efficacy of both treatments for improving pregnancy stretch marks.(AU)

Mating events and aggregations of vulnerable nurse sharks Ginglymostoma cirratum were recorded in the insular protected area of Fernando de Noronha (FEN), Brazil, between April and August 2015. Female sharks were observed clustering in groups of up to 14 individuals in shallow water adjacent to the shore. Several evasive mating behaviours in the presence of males were noticed, including shoreward movement, positioning ventral side up at the sea surface with emerged pectoral and pelvic fins, body rolling and caudal fin thrashing. Fresh bite marks indicative of male courtship and coupling attempts were visible in female's pectoral and caudal fins. Altogether, the observed behaviours match previous reports of non-cooperative female nurse sharks during mating opportunities. An extended mating season coupled with a persistent use of inshore habitats result in nurse sharks being particularly vulnerable to human pressure during a most sensitive stage of their life cycle. The effective conservation of nurse shark populations from the western South Atlantic may thus depend on the protection of critical habitats where this species aggregates to reproduce. Explicitly addressing environmental requirements by vulnerable species in local management strategies is indispensable to ensure that human pressure, including ecotourism development, does not collide with stipulated conservation aims.(AU)


Agregações e eventos reprodutivos do vulnerável tubarão-lixa, Ginglymosytoma cirratum , foram registrados na área insular protegida do Arquipélago de Fernando de Noronha (FEN), Brasil, entre Abril e Agosto de 2015. Grupos constituídos por até 14 fêmeas foram observados em águas rasas adjacentes à linha costeira. Perante a presença de machos, diversos comportamentos evasivos por parte das fêmeas foram identificados, incluíndo movimentação para águas ainda mais rasas, posicionamento na superfície do mar com a face ventral orientada para cima e emersão das nadadeiras peitorais e pélvicas, enrolamento do corpo e batimento da nadadeira caudal na superfície do mar. Foram identificadas marcas recentes de mordida nas nadadeiras peitorais e caudais das fêmeas, sugerindo interações de acasalamento com machos. No geral, os comportamentos observados coincidem com os comportamentos exibidos por fêmeas não aquiescentes à cópula reportados no hemisfério norte. Em FEN, a duração relativamente longa do período de acasalamento, juntamente com a utilização persistente dos habitats mais costeiros, promovem a vulnerabilidade do tubarão lixa às pressões antrópicas durante uma das fases mais sensíveis do seu ciclo de vida. A conservação efetiva das populações de tubarão-lixa do Oceano Atlântico Sul ocidental poderá, assim, depender da proteção dos habitats críticos de agregação onde esta espécie se reproduz. Por conseguinte, a abordagem explícita dos requisitos ambientais associados a espécies vulneráveis no âmbito dos planos de manejo locais deve ser considerada como indispensável para assegurar que as pressões antrópicas, incluíndo o desenvolvimento ecoturístico, não comprometerão os objetivos conservacionistas estipulados.(AU)

Objective To explore the use of CA199,CEA,CA50 and ALP combined with magnetic resonance imaging (MRI) in diagnosis of intrahepatic bile duct stones with early bile duct carcinoma.Methods The clinical diagnosis of 36 cases of patients with hepatolithasis-associated intrahepatic cholangiocarcinoma (HICC) and 118 cases with intrahepatic duct stone (IHDS) were analyzed retrospectively.Serum CA199,CEA,CA50,ALP and magnetic resonance (MRI,MRCP) were performed and the results were analyzed.Results Abdominal pain discomfort in Hepatolithasis-associated intrahepatic cholangiocarcinoma showed multiple symptoms.Three typical Charcot syndromes were rare.On the gender distribution,intrahepatic bile duct stones was frequently found in women,on the contrary,Hepatolithasis-associated intrahepatic cholangiocarcinoma was frequently more found in men (P <0.05) Abdominal pain and fever in patients of Hepatolithasis-associated intrahepatic cholangiocarcinoma was more than that of patients with Intrahepatic bile duct stone (P<0.05) When alkaline phosphatase (ALP) was more than 169 U/L,significant difference was seen between two groups (P< 0.05) According to the diagnosis of HICC,the accuracy of CA199,CEA,CA50 combined with ALP was 88.6%,the accuracy of magnetic resonance examination alone was 90.2%,and the accuracy of multiple serological markers and magnetic resonance was 95.5%.Conclusion MRI combined with serum CA199,CEA,CA50 and ALP can effectively improve the HICC preoperative diagnosis rate.

O presente artigo resulta de investigações sobre questões clínicas de difícil abordagem, cuja expressão situa-se na passagem adolescente. Dentre essas questões encontramos uma grande incidência da busca por marcar o corpo, seja num sentido ornamental, com tatuagens e piercings; ou mesmo na produção de auto-incisões na pele. Serão abordados fundamentos em Freud e Lacan para dar conta dessas questões.


El presente artículo es el resultado de investigaciones sobre cuestiones clínicas difíciles de abordar, cuya expresión se sitúa en el pasaje de la adolescencia. Entre esas cuestiones encontramos una gran incidencia de la búsqueda de marcar el cuerpo, ya sea en un sentido ornamental, con tatuajes y piercings, o en la producción de autoincisiones en la piel. Se abordarán fundamentos en Freud y Lacan para dar cuenta de tales cuestiones.


This article results from research on clinical issues difficult to approach, which expression is located in the adolescent passage. Among these issues it is found a high incidence of searching for body marks, either in an ornamental sense –with tattoos and piercings– or even in the production of auto-incisions on the skin. These issues will be developed from Freud and Lacan's theories.


Cet article est le résultat de recherches faites à propos de sujets cliniques difficiles à aborder, manifestations qui se présentent lors du passage à l’adolescence. Le fait de marquer le corps, soit dans un sens ornemental avec des tatouages et des piercings, soit en se scarifiant, compte parmi ces manifestations. Des principes chez Freud et Lacan seront abordés pour traiter ces sujets.
